Abstract:Continuous monitoring of blood pressure (BP), one of the most critical vital signs, can provide direct and dynamic insight into cardiovascular status. Ultrasound has emerged as a widely-used approach for BP measurement due to its non-invasiveness and high penetration capability. This paper demonstrates a continuous, real-time, non-invasive, and wearable BP monitoring system based on an aluminum nitride (AlN) piezoelectric micromachined ultrasonic transducers (PMUTs) array. A $23\times 26$ PMUTs array, in size of $3\times 3$ mm2, is fabricated and packaged as a wearable device. The PMUTs array exhibits excellent acoustic directivity and penetration depth, allowing for the positioning of the radial artery and capture of the diameter waveform. Highly accurate monitoring of BP waveform is demonstrated, with precision and accuracy within 4.5 mmHg and 5 mmHg, respectively, meeting requirements of clinical use. This wearable device can maintain good and long-term contact with the skin, and provide stable monitoring of BP waveforms without any discomfort, even during the motion of hand. [2023-0085]

Abstract:Pulse signals contain abundant information which is useful in healthcare. Various approaches have been suggested with an aim of acquiring high-quality pulse signals, but are usually limited by either low accuracy or complicated apparatus. In this work, we proposed a "quasi applanation tonometry" method for pulse detection, where an adjustable static pressure was applied on wrist to compress artery and pulse signals were simultaneously recorded at the same position. To realize the method, a wearable compound sensing system, consisting of a piezoresistor and a piezoelectret sensor, was developed to measure the static pressure and pulse signals, respectively. The results showed that clear pulse signals with detailed information were successfully acquired with the sensing system, and by increasing the static pressure the pulse amplitude increased accordingly until a maximum was achieved. Further increase of the static pressure would lower the pulse amplitude and destroy the waveform gradually. A linear relation between the mean arterial pressure and the static pressure for maximum pulse amplitude was fitted, which would support the proposed method for the application of blood pressure estimation. In addition, heartbeat and respiration monitoring were also realized based on the recorded pulse signals. In general, this work has demonstrated a simplified pulse detection method by developing a piezoresistor-piezoelectret compound sensing system and realized some applications of pulse detection in health monitoring.

Abstract:Continuous monitoring of blood pressure (BP) and multiparametric analysis of cardiac functions are crucial for the early diagnosis and therapy of cardiovascular diseases. However, existing monitoring approaches often suffer from bulky and intrusive apparatus, cumbersome testing procedures, and challenging data processing, hampering their applications in continuous monitoring. Here, w e introduce a heterogeneously hierarchical piezoelectric composite for wearable continuous BP and cardiac function monitoring, overcoming the rigidity of ceramic and the insensitivity of polymer. By optimizing the hierarchical structure and components of the composite, the developed piezoelectric sensor delivers impressive performances, ensuring continuous and accurate monitoring of BP at Grade A level. Furthermore, the haemodynamic parameters are extracted from the detected signals, such as local pulse wave velocity, cardiac output and stroke volume, all of which are in alignment with clinical results. Finally, the all‐day tracking of cardiac function parameters validates the reliability and stability of the developed sensor, highlighting its potential for personalized healthcare systems, particularly in early diagnosis and timely intervention of cardiovascular disease. Abstract:Arterial pulse waves contain a wealth of parameters indicative of cardiovascular disease. As such, monitoring them continuously and unobtrusively can provide health professionals with a steady stream of cardiovascular health indices, allowing for the development of efficient, individualized treatments and early cardiovascular disease diagnosis solutions. Blood pulsations in superficial arteries cause skin surface deformations, typically undetectable to the human eye; therefore, Microelectromechanical systems (MEMS) can be used to measure these deformations and thus create unobtrusive pulse wave monitoring devices. Miniaturized ultrathin and flexible Aluminium Nitride (AlN) piezoelectric MEMS are highly sensitive to minute mechanical deformations, making them suitable for detecting the skin deformations caused by cardiac events and consequently providing multiple biomarkers useful for monitoring cardiovascular health and assessing cardiovascular disease risk. Conventional wearable continuous pulse wave monitoring solutions are typically large and based on technologies limiting their versatility. Therefore, we propose the adoption of -thick biocompatible, skin-conforming devices on piezoelectric AlN to create versatile, multipurpose arterial pulse wave monitoring devices. In our initial trials, the devices are placed over arteries along the wrist (radial artery), neck (carotid artery), and suprasternal notch (on the chest wall and close to the ascending aorta). We also leverage the mechano-acoustic properties of the device to detect heart muscle vibrations corresponding to heart sounds S1 and S2 from the suprasternal notch measurement site. Finally, we characterize the piezoelectric device outputs observed with the cardiac cycle events using synchronized electrocardiogram (ECG) reference signals and provide information on heart rate, breathing rate, and heart sounds. The extracted parameters strongly agree with reference values as illustrated by minimum Pearson correlation coefficients (r) of 0.81 for pulse rate and 0.95 for breathing rate.

Abstract:Wearable pulse detection devices can be used for daily human healthcare monitoring; however, the relatively poor flexibility and low sensitivity of the pulse detection devices are hindering the scrutiny of pulse information during pulse diagnosis of different pulse positions. This paper developed a novel and wearable pulse detection device based on three flexible pressure sensors using synthetic graphene and silver composites as the pressure sensing material. The structural design of the pulse detection device is firstly presented; the core component of pressure sensors is using the sawtooth protrusions to convert pressure induced by radial pulse vibrations into localized deformation of graphene composites. The fabricated pulse detection device is characterized by high pressure sensing performance, including relatively high sensitivity (8.65% kPa−1), broad sensing range (12 kPa), and good dynamic response with a response time of about 100 ms. Then, the pulse detection device is worn on a human wrist to detect the pulses from three pulse positions, namely, ‘Cun’, ‘Guan’, and ‘Chi’, and the results demonstrated the capability of using our device to detect pulse signals. The physical conditions of the subject, such as arterial stiffness index, can be further analyzed through the characteristics of the acquired pulse signals, demonstrating the potential application of using wearable pulse detection devices for human health monitoring.

Abstract:Monitoring of arterial blood pressure via cuffless pulse waveform measurement at the wrist has an important clinical value for the early diagnosis and prevention of cardiovascular disease. However, accurate measurement of the radial pulse waveform is challenging owing to its subtle, wideband, and preload-dependent variation characteristics. Evidence shows that uncertainties or variations of wearing pressure and skin temperature can cause artifact signals in wrist pulse measurements, thus degrading blood pressure estimate accuracy and hindering precise clinical diagnosis. Herein, we report a flexible multisensory pulse sensor utilizing natural piezo-thermic transduction of human skin in conjunction with thin-film thermistors for the accurately measuring radial artery pulse waves with high fidelity and good anti-artifact performance. The flexible pulse sensor achieved a wide pressure measuring range (228.2 kPa), low detection limit (4 Pa), good linearity ( R 2 = 0.999), low hysteresis (2.45%), fast response (88 ms), and good durability and stability, thereby enabling accurate pulse measurement with high fidelity. The pulse sensor also monolithically integrated the simultaneous detections of skin temperature and wearing pressure for resisting artifact effects in pulse measurements. Through the fusion of multiple features extracted from the pulse waveform, wearing pressure, skin temperature and user's personal physical characteristics using an efficient multilayer perceptron, blood pressure is accurately estimated and good generalizability is achieved.

Abstract:Continuous monitoring of an arterial pulse using a pressure sensor attached on the epidermis is an important technology for detecting the early onset of cardiovascular disease and assessing personal health status. Conventional pulse sensors have the capability of detecting human biosignals, but have significant drawbacks of power consumption issues that limit sustainable operation of wearable medical devices. Here, a self-powered piezoelectric pulse sensor is demonstrated to enable in vivo measurement of radial/carotid pulse signals in near-surface arteries. The inorganic piezoelectric sensor on an ultrathin plastic achieves conformal contact with the complex texture of the rugged skin, which allows to respond to the tiny pulse changes arising on the surface of epidermis. Experimental studies provide characteristics of the sensor with a sensitivity (≈0.018 kPa<sup>-1</sup> ), response time (≈60 ms), and good mechanical stability. Wireless transmission of detected arterial pressure signals to a smart phone demonstrates the possibility of self-powered and real-time pulse monitoring system.

Abstract:Cardiovascular diseases remain the leading cause of death worldwide. The rapid development of flexible sensing technologies and wearable pressure sensors have attracted keen research interest and have been widely used for long-term and real-time cardiovascular status monitoring. Owing to compelling characteristics, including light weight, wearing comfort, and high sensitivity to pulse pressures, physiological pulse waveforms can be precisely and continuously monitored by flexible pressure sensors for wearable health monitoring. Herein, an overview of wearable pressure sensors for human pulse wave monitoring is presented, with a focus on the transduction mechanism, microengineering structures, and related applications in pulse wave monitoring and cardiovascular condition assessment. The conceptualizations and methods for the acquisition of physiological and pathological information related to the cardiovascular system are outlined. The biomechanics of arterial pulse waves and the working mechanism of various wearable pressure sensors, including triboelectric, piezoelectric, magnetoelastic, piezoresistive, capacitive, and optical sensors, are also subject to systematic debate. Exemple applications of pulse wave measurement based on microengineering structured devices are then summarized. Finally, a discussion of the opportunities and challenges that wearable pressure sensors face, as well as their potential as a wearable intelligent system for personalized healthcare is given in conclusion.

Abstract:A unconstrained and implantable piezoelectric vascular electronic system for real‐time, long‐term, and wireless hemodynamic monitoring is developed. Bioinspired by nature leaf sheath, a growable sheath with elasticity, and conformal shape adaptive to the dynamically growing arterial vessels is integrated with the piezoelectric biosensor. The patient‐centered healthcare requires timely disease diagnosis and prognostic assessment, calling for individualized physiological monitoring. To assess the postoperative hemodynamic status of patients, implantable blood flow monitoring devices are highly expected to deliver real time, long‐term, sensitive, and reliable hemodynamic signals, which can accurately reflect multiple physiological conditions. Herein, an implantable and unconstrained vascular electronic system based on a piezoelectric sensor immobilized is presented by a "growable" sheath around continuously growing arterial vessels for real‐timely and wirelessly monitoring of hemodynamics. The piezoelectric sensor made of circumferentially aligned polyvinylidene fluoride nanofibers around pulsating artery can sensitively perceive mechanical signals, and the growable sheath bioinspired by the structure and function of leaf sheath has elasticity and conformal shape adaptive to the dynamically growing arterial vessels to avoid growth constriction. With this integrated and smart design, long‐term, wireless, and sensitive monitoring of hemodynamics are achieved and demonstrated in rats and rabbits. It provides a simple and versatile strategy for designing implantable sensors in a less invasive way.

Abstract:The Fabric‐based Array Ultrasonic Sensors (FAUS) are produced by combining an "S"‐shaped stretchable island bridge electrode with a fabric substrate, followed by an embedded piezoelectric material. The FAUS are soft, breathable, comfortable to wear, and have the advantages of large penetration depth and non‐destructive. It can locate arterial vessels deep in the tissue and continuously and accurately monitor blood pressure waveforms. Blood pressure is an important indicator that reflects human health status. Since long‐term monitoring of blood pressure waveforms can provide important information for the diagnosis and prevention of cardiovascular disease. Wearable, non‐invasive, and continuous monitoring of blood pressure waveforms have become a current research hotspot. In this paper, a fabric‐based ultrasound sensors are produced for continuous non‐invasive monitoring of blood pressure waveforms by combining an "S"‐shaped stretchable island bridge electrode with a fabric substrate utilizing hot press transfer. The results show that the 4 × 5 array ultrasound sensors can locate arterial vessels deep in the tissue, allowing accurate, and continuous monitoring of blood pressure. Its axial resolution can reach 330 μm, and the elongation at the break of the "S"‐shaped electrode can reach 35.6%. The prepared sensor has good air permeability and stability, and does not cause skin irritation during long‐term wear.

Abstract:Pulse signal contains important human physiological information for the auxiliary diagnosis of cardiovascular disease. Accurate measurement on pulse waveform is challenging due to its subtle, multi-peaks and preloading characteristics. This paper reports a novel wearable multisensory pulse sensor based on piezo-thermic transduction, which employs human natural skin in conjunction with a silver-particle reinforced porous polydimethylsiloxane (PDMS) membrane to transduce pulse pressure into thermo-attribute for highly-sensitive detection of radial artery pulse waves. The sensor demonstrate superior sensitivity and excellent linearity in a preload diagnostic measuring range, enabling continuous, and highly sensitive monitoring of radial pulse pressure wave, skin temperature, and wearing pressure for personal healthcare and medical diagnosis.
Accurate blood pressure detection enabled by graphdiyne piezoionic sensors with ultrafast out-plane ion transfer

Abstract:Flexible piezoionic sensors with softness haptic perception are critically important to artificial intelligence. However, their sensitivity is limited to relatively low level far from practical application because ion transfer is always restricted at surface and in-plane spaces of electrode materials. Here, we discover ultrafast out-plane ion transfer enabled by triangle pores existing in graphdiyne architecture, and then prepare flexible sensors with excellent performances exceeding other reported piezoionic materials. It delivers high piezoionic voltage with excellent cycling stability. The high performances are further verified by ion-pore structure model with pore utilization factor of 99.3%. Moreover, accurate patch detection of blood pressure is realized easily and stably based on graphdiyne piezoionic sensors, which completely slips the leash of cuff problem on commercial sphygmomanometer. Our work sheds light on in-depth discovery of new properties of graphdiyne for electrochemistry.

Here, we report a thin, soft, miniaturized system (TSMS) that combines a conformal piezoelectric sensor array, an active pressure adaptation unit, a signal processing module, and an advanced machine learning method, to allow real wearable, continuous wireless monitoring of ambulatory artery BP. By optimizing the materials selection, control/sampling strategy, and system integration, the TSMS exhibits improved interfacial performance while maintaining Grade A level measurement accuracy. Initial trials on 87 volunteers and clinical tracking of two hypertension individuals prove the capability of the TSMS as a reliable BP measurement product, and its feasibility and practical usability in precise BP control and personalized diagnosis schemes development.

Abstract:Human pulse signals contain important and useful physiological information for the auxiliary diagnosis of cardiovascular disease. Here, a wearable pulse sensor based on piezo-thermic transduction is reported using a structured silver-particle reinforced polydimethylsiloxane (PDMS) membrane, for monitoring radial arterial pulse waves. The structured silver-particle reinforced PDMS membrane is optimally designed to meet the specific requirements on sensitivity, linearity, and effective preload measuring range for pulse detection by adjusting the air gap volume fraction and silver particle volume fraction of the structured material. The sensor is endowed with high sensitivity, good linearity in preload measuring range, allowing to detect the subtle pulse waveforms of subjects at different ages under different contact pressures, such as superficial (Fu), medium (Zhong) and deep (Chen). The developed pulse device provides a promising approach for homecare pulse monitoring.
