Abstract:NEW! The Institute's new research initiatives includes a panel study of secondary traumatic stress and related factors and a survey of social workers and other human service professionals. This is an effort to do two things simultaneously: First to provide a resource to professionals and volunteers working with the traumatized people or others who may be in crisis or are experiencing acute stress. Second, the Panel helps generate longitudinal data to be able to understand and help these professionals. For example, we hope to test the utility of a new measure of secondary traumatic stress reactions developed by Brian Bride others. Volunteers seeking information about their own functioning will be treated as Panel Members will take the tests online. Their answers will help compute baseline and psychometric data for particular professionals (e.g., social workers, teachers, nurses) working with particular clients (e.g., acute care patients, rape survivors, inner city children).

Understanding how social worker compassion fatigue and years of experience shape custodial decisions

Abstract:Background: Compassion fatigue (i.e., a worker's diminished ability to empathize with clients) is common among "helping workers" and can result in psychological detachment from clients as a coping mechanism. Objective: In the present research, we explored the relationship between social workers' compassion fatigue and years of job experience on hypothetical child custody case judgments. Participants and setting: In two separate studies, individuals with experience working with children in child dependency court (predominantly social workers, Study 1: N = 173, Study 2: N = 119) were recruited on Amazon's Mechanical Turk and read a vignette depicting a mother attempting to regain custody. Results: Supporting hypotheses, compassion fatigue significantly mediated the relationship between increased years of social worker job experience on recommendations that a neglectful mother receive custody, Indirect Effect = .06, CIs [.026, .127] (Study 1). We also found preliminary support for our hypothesized theoretically derived serial path model, in which (a) social worker compassion fatigue predicts anticipated secondary traumatic stress associated with the child neglect case, B = .54, p = .0001; (b) secondary traumatic stress predicts detachment from the neglected child, B = .27, p = .0003; (c) detachment from the child predicts job efficacy cynicism B = .65, p < .0001; and (d) job efficacy cynicism predicts decisions to allocate custody to the neglectful mother, B = .46, p = .005 (Study 2). Conclusion: Our research shows that compassion fatigue among social workers may change the lens through which they perceive cases of child abuse.
Exploring the Experiences of Compassion Fatigue Amongst Peer Support Workers in Homelessness Services

Abstract:Abstract Peer support workers have lived experiences of the challenges their clients face. While research has shown peer work can benefit recovery, the negative consequences have not been clearly addressed. This study aimed to explore the experiences of compassion fatigue amongst peer support workers in homelessness services, and the coping strategies used. An explorative qualitative design was adopted. Six peer support workers, in homelessness services, were recruited via snowball sampling. Data was collected using semi-structured interviews and analysed using thematic analysis. Five themes were identified: relentless nature of working in homelessness services, change, making meaning of past experiences, organisational support, and personal coping strategies. There were two novel findings: (1) multiple clients recounting traumatic experiences, and (2) being unfairly blamed for lack of progress, exacerbating compassion fatigue. The findings of this study furthers limited research on compassion fatigue and can be used to develop protocols and practices for organisations that utilise peer support.

Abstract:Compassion fatigue (CF), or loss of ability to empathize or feel compassion for others for whom one cares, is a growing concern for emergency physicians (EP). EPs, by the nature of their jobs, work under unpredictable conditions at odd hours with high levels of exposure to traumatic events. They are placed under substantial psychological, physical, and cognitive pressure, with little opportunity to recover or reflect. CF occurs when this workplace stress leads to feelings of being overwhelmed, helpless, unsupported, and unable to cope. Additionally, primary traumatic stress from threats of workplace violence and secondary traumatic stress (STS) from witnessing the suffering of others increase the likelihood of developing CF. Unchecked, this progression to CF causes reduction in quality of care to patients, reduction in patient satisfaction, increased levels of EP depression and anxiety, increased levels of EP substance use, and increased attrition from the specialty. To truly improve CF, individuals and organizations should be aware of the contributors to CF: namely, emotional exhaustion, depersonalization, primary and STS, and personal achievement. EPs should maximize their resilience to CF by using cognitive behavioral techniques and mindfulness, taking care of their physical health, seeking meaning and development within their work, developing hobbies outside of work, and creating boundaries between work and home. Organizations should actively address the known drivers of physician burnout: workload and job demands, efficiency and resources, meaning in work, culture and values, control and flexibility, work community, and work-life integration. Organizations should also provide adequate safety within facilities to reduce the threat of primary trauma and should supply adequate support and destigmatization for post-traumatic symptoms for EPs suffering from STS.
Secondary traumatic stress and vicarious post-traumatic growth among social workers who have worked with abused children

Abstract:Summary Working with abused children can have positive and negative implications for social workers. Some of these implications have been conceptualized as secondary traumatic stress and vicarious post-traumatic growth. The current study examined the question of whether these two phenomena share similar contributing factors. Based on the social-ecological theory and previous studies on secondary traumatic stress, we examined the contribution of personal factors and level of exposure to trauma (i.e. years of work experience, professional exposure to child abuse, mastery) and environmental factors (i.e. social support, supervision, and role stress) to the explained variance of secondary traumatic stress and post-traumatic growth, with the goal of understanding these two potential outcomes. This cross-sectional study comprised 255 social workers who worked with abused children. Findings The results showed that despite the correlation between the two outcomes—post-traumatic growth and secondary traumatic stress—they shared only one common predictive factor: the extent of social workers’ exposure to abused children. Role stress was correlated positively, and mastery was correlated negatively, with secondary traumatic stress only. Years of work experience was correlated positively with post-traumatic growth only. Applications The current study shows the importance of continuing efforts to identify the factors that contribute to post-traumatic growth. At the same time, efforts should be made to foster certain personal and environmental factors to reduce secondary traumatic stress levels.

The Emotional Impact on Mental Health Workers in the Care of Patients with Mental Disorders in the Pandemic and Post COVID-19 Pandemic: a Measure of "Burnout" and "Compassion Fatigue"

Abstract:Background: Emotional pressure, fear, and uncertainties affected healthcare workers (HCWs) who played a significant role during the COVID-19 pandemic. After the pandemic crisis, the consequences on the health of mental HealhCare Workers are still significant. Our work aimed to evaluate burnout and compassion fatigue in HCWs. Method: In our observational study, 102 (65 females, 37 males) mental HCWs were evaluated during and post-COVID-19 pandemic. We used the Compassion Fatigue-Short Scale, Balanced Emotional Empathy Scale, and Beck Depression Inventory. Results: Subscale Secondary Trauma Stress of CF-SS demonstrated an increase in the rate in the post-pandemic phase (24.51% in all HCWs). The percentage reached in males was high (37.84%). Instead, the levels of Job Burnout remained constant in the two periods analyzed (during and post-COVID-19). Depressive symptoms remained constant with a prevalence in females of the post-COVID period. Conclusion: The results confirm increased stress secondary to the traumatic event, while the levels of job burnout are high. Closely associated with compassion fatigue are levels of empathy that were found to be unchanged.

Abstract:Supporting students' emotional health has become indispensable considering unprecedented collective trauma and present-day stressors including the COVID-19 pandemic, gun violence, longstanding systemic inequities, and a polarized political climate impacting the educational system. In addition to navigating personal and occupational stressors, teachers are frequently exposed to secondhand accounts and effects of students' traumatic experiences, increasing their risk of compassion fatigue (i.e., the mental, physical, and emotional exhaustion that results from working with and caring for individuals who are impacted by distress and trauma). Teachers' emotional resilience is essential in the fight against compassion fatigue, burnout, and withdrawal from the profession. The first of a two-part series, this article lays groundwork that encourages pre-service, early career, and veteran physical and health educators to reflect on risk factors and protective factors that safeguard their socioemotional health as they are asked to continue to give. It begins by describing the role of teachers as emotional first responders in the classroom, followed by risk factors and symptoms of compassion fatigue. The importance, challenges, and limitations of practicing "self-care" as a commonly suggested way to prevent and manage compassion fatigue are addressed. Finally, a call is made for a paradigmatic shift aimed at protecting and uplifting teachers and students. The second part of the series continues this discussion by providing guided activities that encourage teachers to explore, practice, and reflect on personalized and practical coping strategies for emotion regulation that promote well-being and healthful longevity in the profession.
Agency via Life Satisfaction as a Protective Factor From Cumulative Trauma and Emotional Distress Among Bedouin Children in Palestine

Abstract:Adopting an ecological perspective on children's functioning and psychological well-being, we investigated the association between agency and life satisfaction, and its bearing on trauma symptoms and negative emotions in a group of Bedouin children living in the occupied Palestinian territories. Specifically, we hypothesized that the more children were agentic, the more they would be satisfied with their lives; and that greater life satisfaction would be associated with better affect balance, and reduced trauma symptoms. A sample of 286 Bedouin children attending primary schools in four different villages in the Jordan Valley completed the <i>multidimensional students' life satisfaction scale (MSLSS)</i>, <i>positive affect and negative affect scale for children (PANAS-C), Children's Impact of Event Scale (CRIES-13), and the children's hope scale (CHS).</i> Structural equation modeling was performed to evaluate the cumulative network of direct and indirect effects between children's agency, life satisfaction, and trauma symptoms. The findings confirmed the key role of life satisfaction in mitigating traumatic reactions. Higher levels of life satisfaction were associated with reduced negative emotions and trauma symptoms, suggesting that agency may be viewed as a pre-determining factor with the potential to protect children from trauma symptoms. We discuss the implications for research and clinical practice.

Abstract:Journal of Career Assessment, Ahead of Print. Trauma work involves intervening with others enduring acute pain and suffering, often with heavy psychological and physical health impacts. An important question is whether dispositional empathy helps or hurts trauma workers in their occupational functioning. The current study addresses this gap in the research literature by using a person-centered approach to examine the empathy profiles and professional outcomes of a broad sample of trauma workers (n = 315). We measured their trait empathy and organizational outcomes (occupational burnout, person-job fit, turnover intentions, job performance), and found three distinct empathy profiles which differed significantly in their occupational functioning. A 'self-focused' empathy profile (dominated by high personal distress responding) reported the worst functioning; an 'other-oriented' profile (high on perspective taking and empathic concern) had more positive functioning, and an unexpected 'low reactivity' profile (a full SD below the general population on empathy facets) showed the lowest exhaustion. Exploratory analyses revealed that first responders (e.g., police, firefighters, EMTs, paramedics) were overrepresented in the 'low reactivity' profile, while psychology-related professions (e.g., psychologists, counsellors, social workers) were underrepresented in that profile. The significance of these results, as well as their implications for empathy research and vocational counselling in the field of trauma work, are discussed.

In the trauma-informed care trenches: Teacher compassion satisfaction, secondary traumatic stress, burnout, and intent to leave education within underserved elementary schools

Abstract:Background: School-based trauma-informed care (TIC) represents a novel way of supporting children with trauma histories. However, minimal research has focused on how TIC impacts teacher wellbeing, which is concerning given the relationship between occupational stress and turnover from education. Objective: This study investigated teacher characteristics associated with perceived effectiveness of TIC and intent to leave education. Participants and setting: The study included 163 teachers in underserved elementary schools serving a high percentage of low-income, English-language learner Latinx students. Methods: Teachers completed a survey regarding an ongoing TIC program and quality of life for helping professionals. Results: Higher compassion satisfaction and secondary traumatic stress (STS), as well as lower burnout, were associated with perceived effectiveness of TIC. Older teachers, and those with lower compassion satisfaction and higher burnout, were more likely to report intentions to leave education. STS and perceived effectiveness of TIC were not associated with turnover intent. Conclusions: Results suggest that teacher occupational wellbeing, particularly compassion satisfaction, plays a role in buy-in to TIC and retention among teachers.
The Courage to Care: Teacher Compassion Predicts More Positive Attitudes Toward Trauma-Informed Practice

Abstract:Purpose: With greater awareness of the prevalence and impact of childhood trauma and adversity, teachers are now assuming a more active role in creating emotionally healthy environments and responding to student distress. However, supporting trauma-affected students can be a source of amplified teacher stress. Compassion has been identified as a promising construct for frontline professionals in terms of promoting psychological wellbeing, and increasing the sensitivity to detect, tolerate and respond to distress in others. It has also been identified as an important aspect of trauma-informed practice. Nevertheless, the role of compassion in teachers' attitudes towards, and readiness for implementing trauma-informed practices has not yet been explored. This study aimed to address this gap. Methods: A sample of 377 primary and post-primary teachers in Ireland completed the Attitudes Related to Trauma-Informed Care Scale, the Professional Quality of Life Scale, the Self-Compassion Scale, and a socio-demographic survey. Results: Teachers were found to hold generally positive attitudes toward trauma-informed care. They exhibited low to moderate levels of secondary traumatic stress and burnout, and notably high levels of compassion satisfaction, indicating that they tend to embrace their caring role and find meaning and purpose in their work. Regression analyses showed that compassion satisfaction was the strongest predictor of positive attitudes toward trauma-informed care, followed by self-compassion. Older teachers were more likely to display positive attitudes toward trauma-informed care, whilst teachers in single-sex boys' schools held attitudes that were less favorable. Conclusion: This study suggests potential benefits for both teachers and students of positioning compassion at the center of educational policy and practice. The results are contextualized within the Irish and international educational landscape.
Coping Behaviors Mediate Associations between Occupational Factors and Compassion Satisfaction among the Intimate Partner Violence and Sexual Assault Workforce

Abstract:The intimate partner violence (IPV) and sexual assault (SA) workforce faces significant occupational stressors yet little is known about positive occupational outcomes associated with this work. Therefore, this study analyzed factors associated with compassion satisfaction among the IPV/SA workforce in one southwestern U.S. state (n = 623). Drawing from the Resilience Portfolio Model (Grych et al. 2015), researchers examined the possible role of coping behaviors in mediating associations between compassion satisfaction and workplace resources / assets, perceived job security, and resilience. Analyses revealed partial mediation in the models that included workload, values, and resilience as independent variables, suggesting that these factors both influence workers' coping behaviors and have an independent association with compassion satisfaction. Models investigating control, rewards, community, fairness, and perceived job security indicated significant total effects of the independent variables on compassion satisfaction. Overall, IPV/SA workers who engaged more frequently in a range of coping behaviors reported higher levels of compassion satisfaction. The findings point to implications for organizational and employee practice, including building in worktime for key individual coping behaviors, balancing workloads among staff members, and enhancing organizational level coping strategies, such as team supervision and team care planning.
Care When It Counts: Establishing Trauma-Sensitive Care as a Preventative Approach for 0–3-Year-Old Children Suffering from Trauma and Chronic Stress

Abstract:Adverse childhood experiences are an important societal concern. Children aged 0–3 are particularly vulnerable to unpredictable chronic stress due to the critical period for brain development and attachment. Trauma-sensitive care is a preventative approach to reduce the burden of stressful experiences by committing to positive relationships. Professional caregivers are ideally placed to offer trauma-sensitive care; however, earlier research reveals that the tools they need to consciously apply trauma-sensitive care principles are missing. The current study organized living labs (co-creative research method) to present trauma-sensitive care as a preventative approach aimed at children aged 0–3. Two living labs were organized in Belgium and Hungary, where professional caregivers collaborated to create a protocol that offers guidelines on how to implement trauma-sensitive care. The resulting protocol included a theoretical foundation on trauma as well as a translation of these guidelines into practical recommendations. The protocol was evaluated by incorporating it into a training intervention delivered to 100 professional caregivers from childcare organizations across four European countries. The protocol received positive feedback from participants, with results indicating a self-reported increase in knowledge, attitude and practice of trauma-sensitive care principles. We conclude that this trauma-sensitive care protocol is a promising answer to the needs of professional caregivers working with children aged 0–3.

Abstract:When teachers care for children with trauma histories, they are at risk of developing compassion fatigue (CF), or a reduced empathic capacity (Hupe and Stevenson in J Child Custody Res Issues Pract 16(4):364–386, 2019. https://doi.org/10.1080/15379418.2019.1663334). They may also develop secondary traumatic stress (STS), a secondary condition resulting from a person learning about details of a traumatic event experienced by someone in their care (Essary et al. in Kappa Delta Pi Record 56(3):116–121, 2020). While CF and STS have been studied widely in healthcare and mental health professionals (Baird and Kracen in Couns Psychol Q 19(2):181–188, 2006; Caringi et al. in Adv Sch Ment Health Promot 8(4):244–256, 2015. https://doi.org/10.1080/1754730X.2015.1080123; Cieslak et al. in Psychol Serv 11(1):75–86, 2014), STS and CF have been understudied in the teaching profession (Caringi et al., 2015; Christian-Brandt et al. in Child Abuse Neglect 110(3):104437, 2020; Hupe & Stevenson, 2019). As such, we sought to complete a systematic review of the literature to answer two questions: (1) To what extent are CF and STS being studied in teachers?; and (2) How have CF and STS been studied in teachers? Qualitative data analysis led to the emergence of four themes across all included studies: (1) conceptualization of CF and STS; (2) teachers are at risk of developing CF and STS; (3) varying approaches can mitigate the risk of CF and STS in teachers; and (4) there is limited research on CF and STS in teachers. Limitations and directions for future research and practice are described.

Quantifying Compassion Fatigue in Ancillary and Clinical Staff in an Adult Emergency Department

Abstract:Introduction: Emergency department (ED) staff are at a high risk for compassion fatigue (CF) due to a work environment that combines high patient acuity, violence, and other workplace stressors. This multifaceted syndrome has wide-ranging impacts which, if left untreated, can lead to adverse mental health conditions including depression, anxiety, and substance use disorders. However, the majority of studies examining CF look solely at clinicians; as a result, there is little information on the impact of CF across other roles involved in supporting patient care. We conducted this study to establish the prevalence of CF across both clinical and non-clinical roles in the adult ED setting. Methods: For this single institution, cross-sectional study, all full- and part-time ED staff members who worked at least 50% of their shifts in the ED or within the adult trauma service line were eligible to participate. Using the Professional Quality of Life Scale, which measures CF via compassion satisfaction (CS), burnout (BO), and secondary traumatic stress (STS), we assessed for group differences between roles using non-parametric one-way ANOVA. Results: A total of 152 participants (response rate = 38.0%) completed the survey. This included attending physicians (n = 15, 9.7%), resident/fellow physicians (n = 23, 15.1%), staff nurses (n = 54, 35.5%), emergency technicians (n = 21, 13.8%), supportive clinical staff (n = 28, 18.4%), and supportive ancillary staff (n = 11, 7.2%). Across all roles, the majority of respondents had average levels of BO (median = 25.0, interquartile range [IQR] 20.0-29.0) and STS (median = 23.0, IQR 18.0-27.0) coupled with high levels of CS (median = 38.0, IQR 33.0-43.0). There was a difference in CS by role (P = .01), with nurses reporting lower CS than attending physicians. Secondary traumatic stress also differed by role (P = .01), with attending physicians reporting lower STS than both emergency technicians and nurses. Group differences were not seen in BO. Conclusions: Rates of compassion fatigue subcomponents were similar across all ED team members, including non-clinical staff. Programs to identify and mitigate CF should be implemented and extended to all roles within the ED.

Abstract:Compassion fatigue is a condition unique to the human service occupations, characterized by a state of tension and preoccupation with the traumatized clients by re-experiencing traumatic events. With increased incidences of traumatic events both nationally and globally, the greater burden of care and after-care is usually borne by health care professionals. The objective of this study was to determine the predictive role of work setting in the development of compassion fatigue among health care professionals in Moi Teaching and Referral Hospital (MTRH) Eldoret, Kenya. The study was guided by Figley Model of Compassion Fatigue and adopted the Ex Post Facto research design. The target population was 76 doctors, 212 nurses and 33 counselors working in twelve units (grouped into more traumatizing and less traumatizing) offering specialized patient care services. A stratified simple random sampling technique was used to select a sample of 82 participants comprising of 19 Doctors, 54 Nurses and 9 Counselors. The independent variable was Years of Experience while Compassion Fatigue was the dependent variable. Data was collected using a demographic questionnaire, the “Professional Quality of Life Scale (ProQoL) version V”. Data was analyzed using analysis of variance (ANOVA) and t-tests. All the inferential statistics were tested at 0.05 level of significance and data presented in form of percentages, frequencies and means while graphic presentation was in form of graphs. The result showed a statistically significant association between work setting and compassion fatigue with a t of 6.266 and a p of <0.05 on the basis of which the hypothesis was therefore rejected. Based on the findings of this study, it is recommended that professional licensing bodies such as Kenya Medical Association, Kenya Counseling and Psychologist Association, Nursing Council of Kenya which license doctors, counselors and nurses respectively should include wellness and impairment of care providers in their respective curricular.

Abstract:Teachers play a critical role in preparing our children and adolescents for a successful future. However, despite the large number of students impacted by trauma and adversity, teachers are often not well prepared to provide trauma-sensitive support. Furthermore, while working to support students exposed to trauma and adversity, teachers may experience empathy-based stress exacerbating already high levels of stress among them. This narrative review explores the issue of empathy-based stress within the context of the prosocial classroom model which proposes that teachers' social and emotional competence and well-being are key to their ability to create and maintain supportive learning environments critical to student academic and behavioral outcomes.

Compassion fatigue following the September 11 terrorist attacks: a study of secondary trauma among New York City social workers

Abstract:Experience suggests that individuals working in the caring and psychotherapeutic professions are among those to provide mental health services to disaster victims suffering from psychological trauma following catastrophic events. Yet, few studies have focused on the emotional exhaustion from working with such clients, referred to as compassion fatigue (CF) in this study, and how CF differs from other occupational hazards, such as secondary trauma (ST) and job burnout. In the present study, we used recently validated scales to predict ST and job burnout related to providing services to those affected by the World Trade Center (WTC) attacks. Our study data were based on a random survey of 236 social workers living in New York City (NYC), over 80% of which reported being involved in post-WTC disaster counseling efforts. Our analyses indicated that controlling for demographic factors, years of counseling, and personal trauma history, ST was positively associated with WTC recovery involvement (p <. 001) and negatively associated with having a supportive work environment (p < . 01). In contrast, job burnout was negatively associated with having a supportive work environment (p < .01), but not associated with WTC involvement or WTC counseling efforts. We discuss these results in light of future conceptual and empirical research needs.

Abstract:Abstract Background Professional caregivers working in child and youth welfare institutions are frequently faced with the complex mental health issues, emotional needs and challenging coping strategies of clients with cumulated traumatic experiences, leaving them prone to developing high levels of stress, burn-out and compassion fatigue. Trauma-informed care (TIC) is a milieu-therapeutic approach that aims to promote the self-efficacy and self-care of youth welfare staff by guiding them to a better understanding of their own and their clients’ stress symptoms and countertransference. Despite increasing efforts to implement TIC practices, and more widespread recognition of their value in youth welfare systems, there is a lack of studies evaluating the effectiveness of this approach. The aim of this study was to assess the effects of TIC practices in youth welfare institutions on both the physiological stress of staff members and clients’ physical aggression towards their caregivers. . Methods Data was obtained from a longitudinal study investigating the effectiveness of TIC in 14 residential youth welfare institutions. Our sample consisted of 47 youth welfare employees (66.0% female) aged from 23 to 60 years ( M = 37.4, SD = 10.4 years). Hair cortisol concentration (HCC) and occurrences of client physical aggression were assessed at four annual measurement time points (T1 to T4). Results Participants in five institutions employing TIC practices (intervention group) showed significantly lower HCC at T4 than staff members from institutions who did not receive training in TIC (control group), indicating reduced physiological stress levels. At T4, the intervention group reported significantly less physical aggression than the control group. Conclusions TIC might be a promising approach for reducing the emotional burden of employees and institutions should invest in training their staff in TIC practices. More research is necessary, to investigate the benefits and efficacy of TIC, both to youths and staff members, and to foster a better understanding of which specific factors may contribute to stress reduction.
