The effectiveness of fire needle therapy in the treatment of recurrent aphthous stomatitis: A systematic review and meta-analysis

Abstract:Recurrent aphthous stomatitis (RAS) is characterized by painful, circular, shallow ulcers on the oral mucosa, surrounded by a distinct erythematous border and a central yellowish-gray pseudomembrane. [ 1 ] Among oral mucosal disorders, RAS exhibits the highest incidence, with a prevalence ranging from 0.5% to 75%. It predominantly affects women between the ages of 10 and 30. [ 2 ] The etiology of RAS remains elusive, but potential factors include local trauma, immune responses, genetic predisposition, dietary deficiencies such as vitamin insufficiency, microbial agents like Helicobacter pylori infection, and psychological stressors. [ 1 ] Common treatments for RAS encompass anti-inflammatory medications, analgesics, and glucocorticoids. However, their effectiveness is often inconsistent, marked by brief periods of relief, recurrent flare-ups, local side effects like mucosal atrophy, and potential systemic adverse reactions. [ 3 ] The chronic nature of RAS can lead to prolonged physical and psychological distress in patients, heightening levels of anxiety and depression. [ 4 ] This amplifies the negative influence on oral health, leading to growing interest among both clinicians and patients in alternative therapeutic approaches. Chinese acupuncture, in particular, has gained traction due to its demonstrated efficacy in treating various ailments. Notably, the World Health Organization endorsed acupuncture for nearly 100 different conditions in 2003. Fire needle therapy stands out within this context due to its simplicity, cost-effectiveness, and minimally invasive nature.
