Abstract:Recently, a Letter to the Editor critiquing the recommendations of the Japanese Clinical Practice Guidelines for Rehabilitation in Critically Ill Patients, 2023, was published. The comment centered on the recommendation, "Weak recommendation against the use of endoscopy-based management (GRADE 2D: certainty of evidence = 'very low')" for the clinical question, "Should critically ill patients be managed based on video endoscopic assessment of swallowing?" In response, we outline the rationale behind our recommendations and their clinical implications.

Abstract:We express our deep appreciation to our three commentators, William Coburn, Jill Gardner, and Judy Teicholz, recognizing the value we gained by each of them reading our paper closely and respectfully, and responding accordingly. We begin with William Coburn, who saw our efforts as the latest move in the evolution of the complex system we call Self Psychology. We feel he appreciated our account of how Self Psychology has developed and changed both within Kohut’s lifetime and since his death, moving from a one-person to a two-person psychology, in part in response to interaction with infant research, Intersubjectivity theory, and other relational theories. We then move to Jill Gardner, who raises questions about just how differently we view the nature of the self and transference and asks if there are predetermined aspects of each that are not relationally co-created in the analysis. Finally, we turn to Judy Teicholz, for whom Kohut has always been a relational Self Psychologist. We feel this is an ahistorical view that fails to enter into the classical one-person psychology milieu in which Kohut was trained and initially worked, and obscures the significant changes in Self Psychology that emerged both in his lifetime and through the efforts of subsequent theorists. All in all, we are grateful for the time and effort all three of our commentators devoted to what we wrote.

In China, this has been largely because of historical, political, and resource-limitation reasons, most notably for singling out psychiatry as a nonpriority in the delivery of a shorter, less intensive rural physician training program. The attendant significant negative impact over the years on the mental health of the country's rural population is well documented in our article. In Dr. Mahendran's report, this absence of competence is related to medical modernization that lost sight of the centrality of psychiatry as part of general medicine. We advocate that as psychiatrists, we cannot lose sight of this and should increase our championing of psychiatric training at all levels of medical education, lest psychiatry become irrelevant at the most basic level of medical care.TorontoChinaThe authors' disclosures accompany the original article.Accepted for publication in January 2012. Abstract:Immuno-oncology has revolutionized cancer care, drug development, the design of clinical trials, standard treatment paradigms, and the evaluation of response to therapy. These are all areas, however, that have not fully incorporated principles of tumor immunology. Insufficient emphasis is put on the effect drugs have on the immune system, and specifically, the impact that multiple lines of therapy can have on the functioning of the immune system, hindering a robust anti-tumor immune response. A paradigm shift in how we approach the development of novel immunotherapeutic agents is necessary to facilitate the effective improvements in patient outcomes.

Is It Time to Change the Type 2 Diabetes Treatment Paradigm? No! Metformin Should Remain the Foundation Therapy for Type 2 Diabetes

Abstract:Most treatment guidelines, including those from the American Diabetes Association/European Association for the Study of Diabetes and the International Diabetes Federation, suggest metformin be used as the first-line therapy after diet and exercise. This recommendation is based on the considerable body of evidence that has accumulated over the last 30 years, but it is also supported on clinical grounds based on metformin’s affordability and tolerability. As such, metformin is the most commonly used oral antihyperglycemic agent in the U.S. However, based on the release of newer agents over the recent past, some have suggested that the modern approach to disease management should be based upon identification of its etiology and correcting the underlying biological disturbances. That is, we should use interventions that normalize or at least ameliorate the recognized derangements in physiology that drive the clinical manifestation of disease, in this circumstance, hyperglycemia. Thus, it is argued that therapeutic interventions that target glycemia but do not correct the underlying pathogenic disturbances are unlikely to result in a sustained benefit on the disease process. In our field, there is an evolving debate regarding the suggested first step in diabetes management and a call for a new paradigm. Given the current controversy, we provide a Point-Counterpoint debate on this issue. In the point narrative that precedes the counterpoint narrative below, Drs. Abdul-Ghani and DeFronzo provide their argument that a treatment approach for type 2 diabetes based upon correcting the underlying pathophysiological abnormalities responsible for the development of hyperglycemia provides the best therapeutic strategy. Such an approach requires a change in the recommendation for first-line therapy from metformin to a GLP-1 receptor agonist. In the counterpoint narrative below, Dr. Inzucchi argues that based on the medical community’s extensive experience and the drug’s demonstrated efficacy, safety, low cost, and cardiovascular benefits, metformin should remain the “foundation therapy” for all patients with type 2 diabetes, barring contraindications. —William T. Abstract:Thank you for the opportunity to respond to the letter to the editor from Kobayashi et al1 regarding our recent publications investigating droplet splatter patterns during endoscopic endonasal and otologic procedures during the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic using a cadaver model. We found their perspective on the use of cadavers for surgical education and research (CSER) in Japan before and during the SARS-CoV-2 pandemic both interesting and important. We thank them for sharing this publication with the international otolaryngology community.

Abstract:Dear Editors, a response to reviewers' comments is required when submitting a revised manuscript: Many journals simply state that response should be made in a point-by-point manner and indicate all revisions. How to format a response is not stated in the Recommendation of the International Committee of Medical Journal Editors [1]. The first author (SM) wrote 531 PubMed-indexed papers and reviewed 2491 manuscripts (Web of Science) [2], and based on our experience we propose a reviewer-friendly response format ([Table 1]). Where? What should be written? * No need to write a long gratitude. Refrain from stating gratitude for each response. ** A reviewer can focus on whether his/her comments are incorporated. If there are more than one reviewer, "different colors" are much more effective. # State agreement/disagreement first and not gratitude. ## Do not write "See lines 25–26 of the revised manuscript" without describing the revised parts, which would oblige reviewers to read a response and then confirm it in the revised manuscript. Revised parts should be identical to those in the text. This enables the reviewers to determine whether the revisions are appropriate. Thus, this Response Sheet should "streamline" the review process in an expedited manner. 1. At the beginning of the response: State a short gratitude here only once.* State the meaning of colors.** (Example): The changed parts are indicated in red (Reviewer 1) and blue (Reviewer 2). The newly added parts are indicated in green. 2. For each answer: Copy and paste the reviewer's comments in the order presented. Response: Begin with "I agree" or "I disagree". # (Example): I agree. I revised as indicated below (red font, lines 25–26 of the revised manuscript). Write (copy and paste) lines 25–26 of the revised version in red font (all sentences that were revised should be described here). # # First, revisions should be indicated in different font colors in a reviewer-by-reviewer manner: red for Reviewer 1 and blue for Reviewer 2. The reviewer is most interested in whether his/her comments (not the other's) have been incorporated. With different colors, each reviewer can easily check it. Additional revisions are often required after incorporating the comments of the reviewers. Thus, revisions that were not requested by reviewers should be indicated in green. The color code should be stated at the beginning of the response. Red, blue, and green are examples. Colors that are easy to read (not pink or sky blue) should be used. Second, copy and paste each of the reviewers' comments in the order presented, followed by the response. Reviewers usually do not remember their comments' details and thus glancing at copy-and-paste comments makes them recall the situation they addressed. They can focus on whether the revision is appropriate. Describing the summary of the reviewer's comments should be avoided. Reviewers are obliged to confirm whether the summary is right. Third, for each response, refrain from stating gratitude, instead, state "I agree" or "I disagree". The reviewers first wish to know your decision. They next check how their comments are incorporated. Gratitude should be stated once at the beginning of the Response Sheet. Disagreement causes no problem if the author's reasoning is right. There is no need to incorporate all the comments of reviewers. These response guidelines will make the re-review process easier. The Response Sheet tells all: Reviewers need not check the reviewer's original comments, the author's response, the original text, and the revised manuscript. A review is usually performed while looking at a small screen of a personal computer or laptop, and not on printed text. Thus, this Response Sheet makes re-review a "streamlined" process. This is reviewer-friendly. Reviewers are goodwill volunteers but are usually busy clinicians or researchers. This is especially true for doctors of obstetrics and gynecology. They deal with life-threatening emergent conditions. Reviewers wish to thoroughly review a revised manuscript in an expedited manner. Naturally, reviewers' better understanding of your response will increase the possibility of acceptance. At least, this streamlined response sheet will prevent rejection based on the reviewers' misunderstanding of your revision. We do not consider that the present response format is the best. However, we, after trial and error, grasped this style and employed it for two decades. Thus, this format is time-tested. If the journal has some additional or specific requirements for response, please incorporate them. We hope that this format may be of some help for the authors to revise the manuscript.
