Abstract:The meta-analysis conducted by Zhao et al, 1 as published in Nutrition Reviews on January 14, 2024, offers valuable insights into the comparative efficacy of probiotics versus antidepressants in the treatment of major depressive disorder (MDD). However, certain limitations within the study merit critical examination. First, the inclusion of trials where participants concurrently utilized other medications introduces a confounding variable, potentially skewing the observed outcomes. The absence of detailed information regarding the specific medications, dosage regimens, and treatment durations raises concerns regarding the potential influence of these co-administered agents, particularly in cases of treatment-resistant depression, a subgroup purportedly excluded from the analysis. Second, the inclusion of patients with treatment-resistant depression in studies examining augmentation therapies, involving medications such as aripiprazole, brexpiprazole, cariprazine, ketamine, esketamine, rapastinel, and pimavanserin, warrants scrutiny due to the inherent complexities associated with this subset of individuals. 2 Third, in the study conducted by Akkasheh et al (2016) in Zhao et al's article, there was no mention if concomitant antidepressants had been used. Conversely, Kazemi et al (2019) and Ruzdzki et al (2019) reported the use of them. Additionally, Schaub et al (2022) and Tian et al (2022) reported partial use of antidepressants in their research. The use of these medications associated with probiotics is difficult to interpret, because we do not know if the improvement observed in the mood state of patients is related to the antidepressants, to the probiotic, or both. The lack of clarity regarding the interaction between these agents complicates the interpretation of study findings and underscores the need for further elucidation.
