Abstract:Background and Objective: Obstructive sleep apnoea syndrome (OSAS) is an increasingly common disorder, characterized by repeated upper airway collapse during sleep, resulting in oxygen desaturation and disrupted sleep. Untreated OSAS leads to a range of downstream diseases and patients with OSAS bring great challenges to anesthesia management as well. Considering that the recognized gold standard for the diagnosis of OSAS, the full-night, in-laboratory polysomnography (PSG) is expensive, and accessibility to a sleep facility is not always easy, some quick, convenient, and intelligent screening tools are necessary. Artificial intelligence (AI) has grown considerably in the prediction of OSAS over the past few years. This review summarizes current AI techniques used for screening strategies applied to OSAS.

Abstract:Objectives: Snoring is a common symptom of obstructive sleep apnea (OSA) which is considered to be potential predictors of the obstruction site. Successful treatment of OSA depend on the determination the types of obstruction site. This study aimed to develop a machine learning-based model to detect obstruction site using snoring sound.Methods: Patients with OSA underwent drug-induced sleep endoscopy (DISE) and the snoring sounds were recorded simultaneously. We extracted acoustic features based on Mel-frequency cepstral coefficients (MFCC). A k-nearest neighbors (KNN) was used for snore classification.Results: Total 42 patients with OSA were enrolled. The accuracy of model was 85.55 %, F1 score was 85.04. With combined age, gender and Body Mass Index (BMI), the accuracy of model was 87.98 %, and F1 score was 87.96. The model exhibited accuracies of 83 %, 93 % and 92 %; an AUC of 85.88, 89.22 and 88.17 in detecting ret-ropalatal, retrolingual and multilevel obstructions.Conclusion: Our results suggest that combing snoring sound with age, gender and BMI, the machine learning based model can help automatically assess obstruction site. The model may have potential utility as a clinical tool to help for clinical decision-making.

Abstract:Purpose:This study aimed to propose a novel deep-learning method for automatic sleep apneic event detection and thus to estimate the apnea hypopnea index (AHI) and identify obstructive sleep apnea (OSA) in an event-by-event manner solely based on sleep sounds obtained by a noncontact audio recorder.Methods:We conducted a cross-sectional study of participants with habitual snoring or heavy breathing sounds during sleep to train and test a deep convolutional neural network named OSAnet for the detection of OSA based on sleep sounds. Polysomnography (PSG) was conducted, and sleep sounds were recorded simultaneously in a regular room without noise attenuation. The study was conducted in two phases. In phase one, eligible participants were enrolled and randomly allocated into training and validation groups for deep learning algorithm development. In phase two, eligible patients were enrolled in a test group for algorithm assessment. Sensitivity, specificity, accuracy, unweighted Cohen kappa coefficient (κ) and the area under the curve (AUC) were calculated using PSG as the reference standard.Results:A total of 135 participants were randomly divided into a training group (n, 116) and a validation group (n, 19). An independent test group of 59 participants was subsequently enrolled. Our algorithm achieved a precision of 0.81 and sensitivity of 0.78 in the test group for overall sleep event detection. The algorithm exhibited robust diagnostic performance to identify severe cases with a sensitivity of 95.6% and specificity of 91.6%.Conclusion:Our results showed that a deep learning algorithm based on sleep sounds recorded by a noncontact voice recorder served as a feasible tool for apneic event detection and OSA identification. This technique may hold promise for OSA assessment in the community in a relatively comfortable and low-cost manner. Further studies to develop a tool based on a home-based setting are warranted.

Abstract:Obstructive sleep apnea hypopnea syndrome (OSAHS) is a high incidence disease with serious hazard and potential danger. The polysomnography(PSG) has become the gold standard to diagnose OSAHS. However, the PSG is limited for household use because of its operational complexity, technical nature and high consumption.Currently, Microphone, as a non-contacting tensor, is an alternative method. And researchers devoted to analyze respiratory sound for detecting and evaluating OSAHS patients. In this paper, a classifier based on Long Short-Term Memory (LSTM) is proposed to identify the respiratory event-related snoring from simple snoring. Firstly, we collected the sleep sound of 33 patients and 10 normal people from the hospital. And 4780 abnormal snoring segments and 10,740 normal snoring segments were recorded by Mics. Then Mel-frequency cepstrum coefficients(MFCC), Mel Filter Banks (Fbanks), Short-time Energy and Linear Prediction Coefficient(LPC), representing the different characteristics of snoring, are extracted as characteristic features of snoring.At last, a multi-input model based on LSTM is designed, which can receive various audio features to synthesize information to identify snoring. Compared with single feature network processing, the use of multiple feature coefficients can identify the features of snoring at a fine-grained level. In the experiment, our method could classify respiratory event related snoring and normal snoring at accuracy 95.3%, and the accuracy of the three-category snore related to the severity of OSAHS can reach 81.6%. The recognition results can be used for the auxiliary diagnosis of OSAHS.

Abstract:In this study, the development of an automatic algorithm is presented to classify the nocturnal audio recording of an obstructive sleep apnoea (OSA) patient as OSA related snore, simple snore and other sounds. Recent studies has been shown that knowledge regarding the OSA related snore could assist in identifying the site of airway collapse. Audio signal was recorded simultaneously with full-night polysomnography during sleep with a ceiling microphone. Time and frequency features of the nocturnal audio signal were extracted to classify the audio signal into OSA related snore, simple snore and other sounds. Two algorithms were developed to extract OSA related snore using an linear discriminant analysis (LDA) classifier based on the hypothesis that OSA related snoring can assist in identifying the site-of-upper airway collapse. An unbiased nested leave-one patient-out cross-validation process was used to select a high performing feature set from the full set of features. Results indicated that the algorithm achieved an accuracy of 87% for identifying snore events from the audio recordings and an accuracy of 72% for identifying OSA related snore events from the snore events. The direct method to extract OSA-related snore events using a multi-class LDA classifier achieved an accuracy of 64% using the feature selection algorithm. Our results gives a clear indication that OSA-related snore events can be extracted from nocturnal sound recordings, and therefore could potentially be used as a new tool for identifying the site of airway collapse from the nocturnal audio recordings.

Abstract:Obstructive sleep apnea (OSA) is a chronic breathing disorder during sleep that affects 10-30% of adults in North America. The gold standard for diagnosing OSA is polysomnography (PSG). However, PSG has several drawbacks, for example, it is a cumbersome and expensive procedure, which can be quite inconvenient for patients. Additionally, patients often have to endure long waitlists before they can undergo PSG. As a result, other alternatives for screening OSA have gained attention. Speech, as an accessible modality, is generated by variations in the pharyngeal airway, vocal tract, and soft tissues in the pharynx, which shares similar anatomical structures that contribute to OSA. Consequently, in this study, we aim to provide a comprehensive review of the existing research on the use of speech for estimating the severity of OSA. In this regard, a total of 851 papers were initially identified from the PubMed database using a specified set of keywords defined by population, intervention, comparison and outcome (PICO) criteria, along with a concatenated graph of the 5 most cited papers in the field extracted from ConnectedPapers platform. Following a rigorous filtering process that considered the preferred reporting items for systematic reviews and meta-analyses (PRISMA) approach, 32 papers were ultimately included in this review. Among these, 28 papers primarily focused on developing methodology, while the remaining 4 papers delved into the clinical perspective of the association between OSA and speech. In the next step, we investigate the physiological similarities between OSA and speech. Subsequently, we highlight the features extracted from speech, the employed feature selection techniques, and the details of the developed models to predict OSA severity. By thoroughly discussing the current findings and limitations of studies in the field, we provide valuable insights into the gaps that need to be addressed in future research directions.

Abstract:Obstructive sleep apnea (OSA) is a condition commonly affecting middle-aged men that can disturb sleep, cause day-time tiredness, and increase the risk of heart disease. Speech can serve as a valuable biomarker for identifying and predicting the severity of OSA due to its connection with changes in throat structure. This study proposes a new deep-learning-based method for detecting OSA by analyzing speech recordings of participants in sitting and lying positions. The method utilizes a Siamese structure that employs a pre-trained XLSR model to encode ten utterances for each position, reducing the amount of necessary training data and enabling comparison of throat structure changes between the two positions through voice analysis. The study also explores the use of patient characteristic features. Results show this approach achieves an F1 value of 0.725 on our in-house dataset, proving the feasibility of end-to-end speech OSA detection with foundation models.
Severity Evaluation of Obstructive Sleep Apnea Based on Speech Features.

Abstract:PURPOSE:There are upper airway abnormalities in patients with obstructive sleep apnea (OSA), and their speech signal characteristics are different from those of unaffected people. In this study, the severity of OSA was evaluated automatically by machine learning technology based on the speech signals of Chinese people.METHODS:In total, 151 adult male Mandarin native speakers who had suspected OSA completed polysomnography to assess the severity of the disease. Chinese vowels and nasal sounds were recorded in sitting and supine positions, and the accuracy of predicting the apnea-hypopnea index (AHI) of the participants using a machine learning method was analyzed based on features extracted from the speech signals.RESULTS:Among the 151 participants, 75 had AHI > 30 events/h, and 76 had AHI ≤ 30 events/h. Various features including linear prediction cepstral coefficients (LPCC) were extracted from the data collected from participants recorded in the sitting and supine positions and by using a linear support vector machine (SVM); we classified the participants with thresholds of AHI = 30 and AHI = 10 events/h. The accuracies of the classifications were both 78.8%, the sensitivities were 77.3% and 79.1%, and the specificities were 80.3% and 78.0%, respectively.CONCLUSION:This study constructed a severity evaluation model of OSA based on speech signal processing and machine learning, which can be used as an effective method to screen patients with OSA. In addition, it was found that Chinese pronunciation can be used as an effective feature to predict OSA.

Abstract:Obstructive sleep apnea (OSA) is a common sleep disorder. The diagnosis of OSA based on snoring is low-cost, convenient and non-invasive. In this study, we place a microphone under the patient’s bed and combined with full-night polysomnography to record audio signals. Five machine learning models and two OSA diagnostic schemes are used to classify night audio as non-snoring, snoring, or OSA-related snoring. Our experiment has achieved good results, and the highest diagnosis rate of OSA can reach 97%. Based on the trained classification model, we design a system that can diagnose OSA in real-time. Tests on the system show that it can diagnose apnea by detecting OSA-related snoring. We hope that this approach can develop into a new tool to help a large number of potential OSA patients understand their sleep health.

Abstract:Snoring is one of the most prominent symptoms of Obstructive Sleep Apnea-Hypopnea Syndrome (OSAH), a highly prevalent disease that causes repetitive collapse and cessation of the upper airway. Thus, accurate snore sound monitoring and analysis is crucial. However, the traditional monitoring method polysomnography (PSG) requires the patients to stay at a sleep clinic for the whole night and be connected to many pieces of equipment. An alternative and less invasive way is passive monitoring using a smartphone at home or in the clinical settings. But, there is a challenge: the environment may be shared by people such that the raw audio may contain the snore activities of the bed partner or other person. False capturing of the snoring activity could lead to critical false alarms and misdiagnosis of the patients. To address this limitation, we propose a hypothesis that snore sound contains unique identity information which can be used for user recognition. We analyzed various machine learning models: Gaussian Mixture Model (GMM), GMM-UBM (Universial Background Model), and a Deep Neural Network (DNN) on MPSSC - an open source snoring dataset to evaluate the validity of our hypothesis. Our results are promising as we achieved around 90% accuracy in identification and verification tasks. This work marks the first step towards understanding the practicality of snore based user monitoring to enable multiple healthcare applicaitons.

Abstract:Introduction: Obstructive Sleep Apnea-Hypopnea Syndrome (OSAHS) is a common sleep-related breathing disorder that significantly impacts the daily lives of patients. Currently, the diagnosis of OSAHS relies on various physiological signal monitoring devices, requiring a comprehensive Polysomnography (PSG). However, this invasive diagnostic method faces challenges such as data fluctuation and high costs. To address these challenges, we propose a novel data-driven Audio-Semantic Multi-Modal model for OSAHS severity classification (i.e., ASMM-OSA) based on patient snoring sound characteristics. Methods: In light of the correlation between the acoustic attributes of a patient's snoring patterns and their episodes of breathing disorders, we utilize the patient's sleep audio recordings as an initial screening modality. We analyze the audio features of snoring sounds during the night for subjects suspected of having OSAHS. Audio features were augmented via PubMedBERT to enrich their diversity and detail and subsequently classified for OSAHS severity using XGBoost based on the number of sleep apnea events. Results: Experimental results using the OSAHS dataset from a collaborative university hospital demonstrate that our ASMM-OSA audio-semantic multimodal model achieves a diagnostic level in automatically identifying sleep apnea events and classifying the four-class severity (normal, mild, moderate, and severe) of OSAHS. Discussion: Our proposed model promises new perspectives for non-invasive OSAHS diagnosis, potentially reducing costs and enhancing patient quality of life.

Abstract:Abstract Sleep apnea hypopnea syndrome (OSAHS) is a high-incidence disease with serious harm and potential dangers. Currently, the traditional scheme for monitoring sleep quality mainly focuses on monitoring two physiological signals: electroencephalogram (EEG) and heartbeat. However, in the sleep state, respiration is also an important physiological signal. This paper proposes a sleep apnea detection method based on snoring sound analysis using deep learning. Firstly, snoring sound signals are preprocessed and feature extraction is performed using Mel-frequency cepstral coefficients (MFCC). The extracted features are then used to train a DS-MS neural network model, and the optimal detection model is obtained through iterations. The experimental results show that the accuracy of the proposed detection model can reach 94.17%.

Abstract:Abstract According to the Obstructive Sleep Apnea Syndrome (OSAS), a wearable sleep monitoring system is designed based on machine learning using snoring sound signal. The system uses the measured data of snoring to analyze the symptoms of OSAS, so as to realize the preliminary judgment based on the snoring data. After the snoring signal is collected by the sensor, a signal conditioning circuit is used to convert it into a voltage signal to improve the amplitude and signal-to-noise ratio of the signal. Finally, in order to combined with the features to establish a neural network classifier model, the apnea-hypopnea index (AHI) is analyzed and calculated. And the result shows that the average accuracy in identifying the severity of the four OSAS categories is 59%. The system proposed in this paper has a good application development prospect in intelligent monitoring and medical instruments.

Abstract:Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a common and high-risk sleep-related breathing disorder. Snoring detection is a simple and non-invasive method. In many studies, the feature maps are obtained by applying a short-time Fourier transform (STFT) and feeding the model with single-channel input tensors. However, this approach may limit the potential of convolutional networks to learn diverse representations of snore signals. This paper proposes a snoring sound detection algorithm using a multi-channel spectrogram and convolutional neural network (CNN). The sleep recordings from 30 subjects at the hospital were collected, and four different feature maps were extracted from them as model input, including spectrogram, Mel-spectrogram, continuous wavelet transform (CWT), and multi-channel spectrogram composed of the three single-channel maps. Three methods of data set partitioning are used to evaluate the performance of feature maps. The proposed feature maps were compared through the training set and test set of independent subjects by using a CNN model. The results show that the accuracy of the multi-channel spectrogram reaches 94.18%, surpassing that of the Mel-spectrogram that exhibits the best performance among the single-channel spectrograms. This study optimizes the system in the feature extraction stage to adapt to the superior feature learning capability of the deep learning model, providing a more effective feature map for snoring detection.

Abstract:Obstructive Sleep Apnea (OSA) is characterized by repetitive episodes of airflow reduction (hypopnea) or cessation (apnea), which, as a prevalent sleep disorder, can cause people to stop breathing for 10 to 30 seconds at a time and lead to serious problems such as daytime fatigue, impaired memory, and depression. This work intends to explore automatic detection of OSA events with 1-second annotation based on blood oxygen saturation, oronasal airflow, and ribcage and abdomen movements. Deep Learning (DL) technology, specifically, Convolutional Neural Network (CNN), is employed as a feature detector to learn the characteristics of the highorder correlation among visible data and corresponding labels. A fully-connected layer in the last stage of the CNN is connected to the output layer and constructs the desired number of outputs for sleep apnea events classification. A leave-one-out cross-validation has been conducted on the PhysioNet Sleep Database provided by St. Vincents University Hospital and University College Dublin, and an average accuracy of $79 .61$% across normal, hypopnea, and apnea, classes is achieved.

Abstract:Obstructive sleep apnea (OSA) is primarily characterized by intermittent nocturnal hypoxia and sleep fragmentation. Arousals interrupt sleep continuity and lead to sleep fragmentation, which can lead to cognitive dysfunction, excessive daytime sleepiness, and adverse cardiovascular outcome events, making arousals important for diagnosing OSA and reducing the risk of complications, including heart disease and cognitive impairment. Traditional arousal interpretation requires sleep specialists to manually score PSG recordings throughout the night, which is time consuming and has low inter-specialist agreement, so the search for simple, efficient, and reliable arousal detection methods can be a powerful tool to clinicians. In this paper, we systematically reviewed different methods for recognizing arousal in OSA patients, including autonomic markers (pulse conduction time, pulse wave amplitude, peripheral arterial tone, heart rate, etc.) and machine learning-based automated arousal detection systems, and found that autonomic markers may be more beneficial in certain subgroups, and that deep artificial networks will remain the main research method for automated arousal detection in the future.
A Snoring Sound Dataset for Body Position Recognition: Collection, Annotation, and Analysis

Abstract:Obstructive Sleep Apnea-Hypopnea Syndrome (OSAHS) is a chronic breathing disorder caused by a blockage in the upper airways. Snoring is a prominent symptom of OSAHS, and previous studies have attempted to identify the obstruction site of the upper airways by snoring sounds. Despite some progress, the classification of the obstruction site remains challenging in real-world clinical settings due to the influence of sleep body position on upper airways. To address this challenge, this paper proposes a snore-based sleep body position recognition dataset (SSBPR) consisting of 7570 snoring recordings, which comprises six distinct labels for sleep body position: supine, supine but left lateral head, supine but right lateral head, left-side lying, right-side lying and prone. Experimental results show that snoring sounds exhibit certain acoustic features that enable their effective utilization for identifying body posture during sleep in real-world scenarios.

In this study, sleep sound signals are used to detect OSA in patients.Approach: A deep feature transfer-based OSA detection approach is proposed. First, a deep convolutional neural network is trained on large-scale labeled audio data sets to distinguish respiration sounds from environmental noise. Second, the trained model is transferred to recognize respiration sounds in sleep sound signals. Third, the deep features of the detected respiration sounds are used to train a logistic regression classifier to identify OSA patients from potential patients. Polysomnography-based diagnosis is used as a reference.Main results: A self-collected data set of 132 potential OSA patients is applied in OSA detection experiments. The OSA detection performances are tested on four models for different apnea-hypopnea index thresholds and sexes resulting in accuracies of 80.17%, 80.21%, 81.63% and 77.22%. The corresponding areas under the receiver operating characteristic curves are 0.82, 0.80, 0.81 and 0.79. In addition, the proposed method presented a significant performance improvement compared with the state-of-the-art methods.Significance: Big data, deep learning and transfer learning can be successfully applied to improve diagnostic accuracy in OSA detection. The performance of the proposed approach is superior to that of traditional audio analysis technology. The proposed method significantly reduces difficulties in OSA detection and diagnosis, such that potential OSA patients can perform initial inspections by themselves at home.

Abstract:Obstructive Sleep Apnea Syndrome (OSAS) is the most common sleep-related breathing disorder. It is caused by an increased upper airway resistance during sleep, which determines episodes of partial or complete interruption of airflow. The detection and treatment of OSAS is particularly important in stroke patients, because the presence of severe OSAS is associated with higher mortality, worse neurological deficits, worse functional outcome after rehabilitation, and a higher likelihood of uncontrolled hypertension. The gold standard test for diagnosing OSAS is polysomnography (PSG). Unfortunately, performing a PSG in an electrically hostile environment, like a stroke unit, on neurologically impaired patients is a difficult task; also, the number of strokes per day outnumbers the availability of polysomnographs and dedicated healthcare professionals. Thus, a simple and automated recognition system to identify OSAS among acute stroke patients, relying on routinely recorded vital signs, is desirable. The majority of the work done so far focuses on data recorded in ideal conditions and highly selected patients, and thus it is hardly exploitable in real-life settings, where it would be of actual use. In this paper, we propose a convolutional deep learning architecture able to reduce the temporal resolution of raw waveform data, like physiological signals, extracting key features that can be used for further processing. We exploit models based on such an architecture to detect OSAS events in stroke unit recordings obtained from the monitoring of unselected patients. Unlike existing approaches, annotations are performed at one-second granularity, allowing physicians to better interpret the model outcome. Results are considered to be satisfactory by the domain experts. Moreover, based on a widely-used benchmark, we show that the proposed approach outperforms current state-of-the-art solutions.
