Abstract:In modern healthcare, the integration of artificial intelligence (AI) has revolutionized clinical practices, particularly in data management and patient visit summary creation. Manual creation of patient summary is repetitive, time-consuming, prone to errors, and increases clinicians' workload. AI, through voice recognition and Natural Language Processing (NLP), can automate this task more accurately and efficiently. Erectile dysfunction (ED) clinics, which deal with specific pattern of conditions together with an involvement of broader systemic issues, can greatly benefit from AI-driven patient summary. This scoping review examined the evidence on AI-generated patient summary and evaluated their implementation in ED clinics.

Abstract:Introduction Since November 2022, Artificial Intelligence (AI) chatbots have grown in popularity including in urologic conditions. However, their accuracy and quality has not been evaluated systematically. In this study, we sought to assess the accuracy and quality of AI chatbots in the management of Erectile Dysfunction (ED). Objective We aim to evaluate the accuracy and quality of open-source language models in fielding common clinical questions pertaining to ED compared to board certified urologists. Methods Two AI open-source language models, ChatGPT and Google Bard, were fielded 15 standard questions related to ED some of which included causes, risk factors and treatment options of ED. Two board certified urologists were given the same questions on a standard survey. A third blinded board-certified urologist served as a grader using the AUA guidelines for ED on a Likert scale to assess the accuracy, robustness, and bias of each response. Urologist and AI responses were graded and aggregated using Likert scales. Results Overall AI responses were significantly more accurate (p<0.01), robust (p<0.01), and unbiased (p<0.01). Additionally, Google Bard had the highest scores all around followed by ChatGPT. The urologists' responses were approximately 38% lower compared to the AI responses. Conclusions This study suggests that AI responses were superior compared to urologists in the areas of accuracy, robustness, and bias pertaining to the management of ED. Albeit, the chatbots have promising role in urologic conditions including ED, its widespread clinical use and adoption warrants further evaluation in the context of clinical decision making and enhancing patient care. Frequently asked questions on erectile dysfunction: evaluating artificial intelligence answers with expert mentorship

Abstract:The present study assessed the accuracy of artificiaI intelligence-generated responses to frequently asked questions on erectile dysfunction. A cross-sectional analysis involved 56 erectile dysfunction-related questions searched on Google, categorized into nine sections: causes, diagnosis, treatment options, treatment complications, protective measures, relationship with other illnesses, treatment costs, treatment with herbal agents, and appointments. Responses from ChatGPT 3.5, ChatGPT 4, and BARD were evaluated by two experienced urology experts using the F1 and global quality scores (GQS) for accuracy, relevance, and comprehensibility. ChatGPT 3.5 and ChatGPT 4 achieved higher GQS than BARD in categories such as causes (4.5 ± 0.54, 4.5 ± 0.51, 3.15 ± 1.01, respectively, p < 0.001), treatment options (4.35 ± 0.6, 4.5 ± 0.43, 2.71 ± 1.38, respectively, p < 0.001), protective measures (5.0 ± 0, 5.0 ± 0, 4 ± 0.5, respectively, p = 0.013), relationships with other illnesses (4.58 ± 0.58, 4.83 ± 0.25, 3.58 ± 0.8, respectively, p = 0.006), and treatment with herbal agents (3 ± 0.61, 3.33 ± 0.83, 1.8 ± 1.09, respectively, p = 0.043). F1 scores in categories: causes (1), diagnosis (0.857), treatment options (0.726), and protective measures (1), indicated their alignment with the guidelines. There was no significant difference between ChatGPT 3.5 and ChatGPT 4 regarding answer quality, but both outperformed BARD in the GQS. These results emphasize the need to continually enhance and validate AI-generated medical information, underscoring the importance of artificiaI intelligence systems in delivering reliable information on erectile dysfunction.

Abstract:Background Clinical decision-making in gastrointestinal surgery is complex due to the unpredictability of tumoral behavior and postoperative complications. Artificial intelligence (AI) could aid in clinical decision-making by predicting these surgical outcomes. The current status of AI-based clinical decision-making within gastrointestinal surgery is unknown in recent literature. This review aims to provide an overview of AI models used for clinical decision-making within gastrointestinal surgery. Methods A systematic literature search was performed in databases PubMed, EMBASE, Cochrane, and Web of Science. To be eligible for inclusion, studies needed to use AI models for clinical decision-making involving patients undergoing gastrointestinal surgery. Studies reporting on reviews, children, and study abstracts were excluded. The Probast risk of bias tool was used to evaluate the methodological quality of AI methods. Results Out of 1073 studies, 10 articles were eligible for inclusion. AI models have been used to make clinical decisions between surgical procedures, selection of chemotherapy, selection of postoperative follow up programs, and implementation of a temporary ileostomy. Most studies have used a Random Forest or Gradient Boosting model with AUCs up to 0.97. All studies involved a retrospective study design, in which external validation was performed in one study. Conclusions This review shows that AI models have the potentiality to select the most optimal treatments for patients undergoing gastrointestinal surgery. Clinical benefits could be gained if AI models were used for clinical decision-making. However, prospective studies and randomized controlled trials will reveal the definitive role of AI models in clinical decision-making.

Abstract:Context: Artificial intelligence refers to a set of systems that are capable of performing functions similar to human intelligent functions. Today, artificial intelligence has been successfully incorporated into clinical decision support systems (CDSS). Evidence Acquisition: The current study aimed to briefly present a narrative mini-review on clinical reasoning and artificial intelligence. Data were gathered from Google Scholar, ScienceDirect, and PubMed databases using the "clinical decision support system, artificial intelligence, and clinical reasoning" keywords. Results: Clinical decision support systems are divided into two categories: knowledge-based and data-driven. The first category is called the rule-based expert system, and the second category is also named the machine-learning system. The usefulness of the mentioned systems and artificial intelligence in interpreting algorithmic and statistical information, where the human element can easily make a mistake, is that they are much more efficient and work with fewer errors. However, when it comes to dealing with a patient and his complaints and symptoms, because of the requirement for clinical judgment, the human element works much better in obtaining a mental image of the patient’s condition. Artificial intelligence is specifically used in scenarios such as the diagnosis of electrolyte disorders, interpreting ECG findings, and recognizing the causes of myocardial hypertrophy. Nonetheless, artificial intelligence has challenges, such as a lack of responsibility for medical decisions and treatment errors. Conclusions: Referring to the above-mentioned benefits and challenges of artificial intelligence, artificial and human intelligence cannot be superior to each other, and both have an irreplaceable role in clinical decision-making. The new view is that the goal of CDSS is to help the physician make better decisions by processing vast pieces of information as a whole entity rather than individually.

This narrative review explores the role of AI in diagnosing, treating, and managing dry eye disease. AI-driven tools such as machine learning algorithms, imaging technologies, and diagnostic platforms are examined for their ability to enhance diagnostic accuracy, personalize treatment approaches, and optimize patient outcomes. Furthermore, the review addresses the limitations of AI technologies in ophthalmology, including the need for robust clinical validation, data privacy concerns, and the ethical considerations of integrating AI into clinical practice. The role of artificial intelligence for the application of integrating electronic health records and patient-generated data in clinical decision support

Abstract:This narrative review aims to identify and understand the role of artificial intelligence in the application of integrated electronic health records (EHRs) and patient-generated health data (PGHD) in clinical decision support. We focused on integrated data that combined PGHD and EHR data, and we investigated the role of artificial intelligence (AI) in the application. We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines to search articles in six databases: PubMed, Embase, Web of Science, Scopus, ACM Digital Library, and IEEE Computer Society Digital Library. In addition, we also synthesized seminal sources, including other systematic reviews, reports, and white papers, to inform the context, history, and development of this field. Twenty-six publications met the review criteria after screening. The EHR-integrated PGHD introduces benefits to health care, including empowering patients and families to engage via shared decision-making, improving the patient-provider relationship, and reducing the time and cost of clinical visits. AI's roles include cleaning and management of heterogeneous datasets, assisting in identifying dynamic patterns to improve clinical care processes, and providing more sophisticated algorithms to better predict outcomes and propose precise recommendations based on the integrated data. Challenges mainly stem from the large volume of integrated data, data standards, data exchange and interoperability, security and privacy, interpretation, and meaningful use. The use of PGHD in health care is at a promising stage but needs further work for widespread adoption and seamless integration into health care systems. AI-driven, EHR-integrated PGHD systems can greatly improve clinicians' abilities to diagnose patients' health issues, classify risks at the patient level by drawing on the power of integrated data, and provide much-needed support to clinics and hospitals. With EHR-integrated PGHD, AI can help transform health care by improving diagnosis, treatment, and the delivery of clinical care, thus improving clinical decision support.

Abstract:The potential of artificial intelligence (AI) in diagnosing and managing sexually transmitted infections (STIs), nonvenereal genital diseases, and overall sexual health is immense. AI shows promise in STI screening and diagnosis through image recognition and patient data analysis, potentially increasing diagnostic accuracy while ensuring inclusivity. AI can fuel the transformation of e-health and direct-to-consumer services, enhancing targeted screening and personalized interventions while improving the user-friendliness of services. There is a significant role for AI in sexual education, particularly its use in interactive, empathetic chatbots. AI's integration into health care as a decision support tool for primary health-care providers can boost real-time diagnostic accuracy. Furthermore, AI's use in big data can enhance real-time epidemiology, predictive analysis, and directed interventions at population levels. However, challenges such as real-world diagnostic accuracy, liability, privacy concerns, and ethical dilemmas persist. Future directions include an emphasis on inclusivity, language accommodation, and swift research-to-practice transitions. Collaboration among policymakers, researchers, and health-care providers is needed to leverage AI's transformative potential in sexual health.

Abstract:Purpose of review: Benign prostatic hyperplasia (BPH) is prevalent in nearly 70% of men over the age of 60, leading to significant clinical challenges due to varying symptom presentations and treatment responses. The decision to undergo surgical intervention is not straightforward; the American Urological Association recommends consideration of surgical treatment after inadequate or failed response to medical therapy. This review explores the role of artificial intelligence (AI), including machine learning and deep learning models, in enhancing the decision-making processes for BPH management. Recent findings: AI applications in this space include analysis of non-invasive imaging modalities, such as multiparametric Magnetic Resonance Imaging (MRI) and Ultrasound, which enhance diagnostic precision. AI models also concatenate serum biomarkers and histopathological analysis to distinguish BPH from prostate cancer (PC), offering high accuracy rates. Furthermore, AI aids in predicting patient outcomes post-treatment, supporting personalized medicine, and optimizing therapeutic strategies. AI has demonstrated potential in differentiating BPH from PC through advanced imaging and predictive models, improving diagnostic accuracy, and reducing the need for invasive procedures. Despite promising advancements, challenges remain in integrating AI into clinical workflows, establishing standard evaluation metrics, and achieving cost-effectiveness. Here, we underscore the potential of AI to improve patient outcomes, streamline BPH management, and reduce healthcare costs, especially with continued research and development in this transformative field.

Abstract:Purpose of review: The aim of the present manuscript is to provide an overview on the current state of artificial intelligence (AI) tools in either decision making, diagnosis, treatment options, or outcome prediction in functional urology. Recent findings: Several recent studies have shed light on the promising potential of AI in functional urology to investigate lower urinary tract dysfunction pathophysiology but also as a diagnostic tool by enhancing the existing evaluations such as dynamic magnetic resonance imaging or urodynamics. AI may also improve surgical education and training because of its automated performance metrics recording. By bringing prediction models, AI may also have strong therapeutic implications in the field of functional urology in the near future. AI may also be implemented in innovative devices such as e-bladder diary and electromechanical artificial urinary sphincter and could facilitate the development of remote medicine. Summary: Over the past decade, the enthusiasm for AI has been rising exponentially. Machine learning was well known, but the increasing power of processors and the amount of data available has provided the platform for deep learning tools to expand. Although the literature on the applications of AI technology in the field of functional urology is relatively sparse, its possible uses are countless especially in surgical training, imaging, urodynamics, and innovative devices.

Abstract:This review aims to assess the effectiveness of AI-driven CDSSs on patient outcomes and clinical practices. A comprehensive search was conducted across PubMed, MEDLINE, and Scopus. Studies published from January 2018 to November 2023 were eligible for inclusion. Following title and abstract screening, full-text articles were assessed for methodological quality and adherence to inclusion criteria. Data extraction focused on study design, AI technologies employed, reported outcomes, and evidence of AI-CDSS impact on patient and clinical outcomes. Thematic analysis was conducted to synthesise findings and identify key themes regarding the effectiveness of AI-CDSS. The screening of the articles resulted in the selection of 26 articles that satisfied the inclusion criteria. The content analysis revealed four themes: early detection and disease diagnosis, enhanced decision-making, medication errors, and clinicians' perspectives. AI-based CDSSs were found to improve clinical decision-making by providing patient-specific information and evidence-based recommendations. Using AI in CDSSs can potentially improve patient outcomes by enhancing diagnostic accuracy, optimising treatment selection, and reducing medical errors.

Abstract:Introduction The use of artificial intelligence (AI), which can emulate human intelligence and enhance clinical results, has grown in healthcare decision-making due to the digitalization effects and the COVID-19 pandemic. The purpose of this study was to determine the scope of applications of AI tools in the decision-making process in healthcare service delivery networks. Materials and methods This study used a qualitative method to conduct a systematic review of the existing reviews. Review articles published between 2000 and 2024 in English-language were searched in PubMed, Scopus, ProQuest, and Cochrane databases. The CASP (Critical Appraisal Skills Programme) Checklist for Systematic Reviews was used to evaluate the quality of the articles. Based on the eligibility criteria, the final articles were selected and the data extraction was done independently by 2 authors. Finally, the thematic analysis approach was used to analyze the data extracted from the selected articles. Results Of the 14 219 identified records, 18 review articles were eligible and included in the analysis, which covered the findings of 669 other articles. The quality assessment score of all reviewed articles was high. And, the thematic analysis of the data identified 3 main themes including clinical decision-making, organizational decision-making, and shared decision-making; which originated from 8 subthemes. Conclusions This study revealed that AI tools have been applied in various aspects of healthcare decision-making. The use of AI can improve the quality, efficiency, and effectiveness of healthcare services by providing accurate, timely, and personalized information to support decision-making. Further research is needed to explore the best practices and standards for implementing AI in healthcare decision-making.

The purpose of this systematic was to evaluate how AI can enhance prostate cancer diagnosis and treatment. We search five different databases for available studies using a pre-specified search strategy. The study was conducted using PRISMA guidelines. A total of 1058 studies were found on five different databases (Scopus N=287, Web of Science N=204, PubMed/EMBASE N=306, Cochrane Library N=94, Google Scholar N=167). The articles were stored in the ENDNOTE library and all the duplicates were removed. We found 19 relevant studies that were included after a stringent assessment criterion based on the inclusion and exclusion criteria. Included studies were assessed for risk bias assessment using the Newcastle Ottawa Scale (NOS) in which one study was found to have high-risk bias assessment, seven were identified to have low-risk bias assessment while the rest of the studies were moderate. Clinicians can recognize complex correlations and handle massive data sets with the help of artificial intelligence. These tasks are extremely laborious and challenging for humans to complete. It is feasible to employ fewer resources while increasing overall effectiveness and precision in prostate cancer detection and treatment by utilizing AI algorithms and lowering the degree of subjectivity. This review provides a comprehensive comparison of AI algorithms in detection of prostate cancer.
