Abstract:Sun exposure can alter microbiome composition, potentially leading to dysbiosis, compromised skin barrier function, and immune system activation. Following UVR exposure occurs an increase in Cyanobacteria spp., Fusobacteria spp., and a decrease in Lactobacillaceae spp. and Pseudomonadaceae spp. Lactobacillus spp. and Clostridium sensu were the most discriminately higher genera in the healthy skin microbiome. Staphylococcus epidermidis was shown to be able to generate electrons which deactivate ROS which otherwise cause cellular damage. Current sun protection methods generally overlook microbiome considerations. Tailored sunscreens that prioritize both skin and microbiome health may offer enhanced defense against SR‐induced skin conditions. The skin microbiome undergoes constant exposure to solar radiation (SR), with its effects on health well‐documented. However, understanding SR's influence on host‐associated skin commensals remains nascent. This review surveys existing knowledge on SR's impact on the skin microbiome and proposes innovative sun protection methods that safeguard both skin integrity and microbiome balance. A team of skin photodamage specialists conducted a comprehensive review of 122 articles sourced from PubMed and Research Gateway. Key terms included skin microbiome, photoprotection, photodamage, skin cancer, ultraviolet radiation, solar radiation, skin commensals, skin protection, and pre/probiotics. Experts offered insights into novel sun protection products designed not only to shield the skin but also to mitigate SR's effects on the skin microbiome. Existing literature on SR's influence on the skin microbiome is limited. SR exposure can alter microbiome composition, potentially leading to dysbiosis, compromised skin barrier function, and immune system activation. Current sun protection methods generally overlook microbiome considerations. Tailored sun protection products that prioritize both skin and microbiome health may offer enhanced defense against SR‐induced skin conditions. By safeguarding both skin and microbiota, these specialized products could mitigate dysbiosis risks associated with SR exposure, bolstering skin defense mechanisms and reducing the likelihood of SR‐mediated skin issues.

Abstract:Human skin apart from functioning as a physical barricade to stop the entry of pathogens, also hosts innumerable commensal organisms. The skin cells and the immune system constantly interact with microbes, to maintain cutaneous homeostasis, despite the challenges offered by various environmental factors. A major environmental factor affecting the skin is ultraviolet radiation (UV-R) from sunlight. UV-R is well known to modulate the immune system, which can be both beneficial and deleterious. By targeting the cells and molecules within skin, UV-R can trigger the production and release of antimicrobial peptides, affect the innate immune system and ultimately suppress the adaptive cellular immune response. This can contribute to skin carcinogenesis and the promotion of infectious agents such as herpes simplex virus and possibly others. On the other hand, a UV-established immunosuppressive environment may protect against the induction of immunologically mediated skin diseases including some of photodermatoses such as polymorphic light eruption. In this article, we share our perspective about the possibility that UV-induced immune suppression may alter the landscape of the skin's microbiome and its components. Alternatively, or in concert with this, direct UV-induced DNA and membrane damage to the microbiome may result in pathogen associated molecular patterns (PAMPs) that interfere with UV-induced immune suppression.

Abstract:Sunscreens are topical formulations incorporating filters that protect our skin against ultraviolet radiation (UVR) emitted by the sun. Sunscreen use has been increasingly encouraged to protect against sunburn, skin cancer and photoaging that can occur because of prolonged and cumulative sun exposure. However, sunscreens and their constituent UVR filters have been purported to be problematic themselves. In this narrative review, we will describe the history of sunscreens, types of UVR filters and how sunscreens are classified and rated. We will also explore some of the controversies regarding sunscreens, including concerns about their safety and environmental impact. Awareness of these potential consequences is paramount to the process of informed decision-making.

Abstract:Objective Ultraviolet radiation (UVR) is a known environmental key factor for premature skin aging. Only few scientific evidence is available to support the effects of UVR on the skin microbiome. This in vivo pilot study aimed to evaluate the impact on the skin microbiome upon erythemal UV exposure and the protection of UV‐exposed skin microbiome by UV filters. Methods Ten female volunteers were treated with an SPF 20 sunscreen and placebo formulation (without UV filters) on their upper middle backs and irradiated with an erythemal dose (2 MED) by a solar simulator. Skin swabbing samples from four zones (i.e., unexposed, exposed, sunscreen‐ and placebo‐treated on exposed skin) were collected for the microbiome analysis before and 2 hours after UV exposure, respectively, and processed via shallow 16S rRNA Amplicon and Shotgun Metagenomic sequencing. An in vitro UV method was developed to confirm the protection of isolated bacterial strains by single UV filters and combinations. Results Alpha diversity was impacted by significant inter‐individual differences and by treatment rather than by irradiation. Cutibacterium acnes was found to be most abundant and a confounding factor for diversity. On a species level Lactobacillus crispatus was negatively associated with UVR and placebo treatment, whereas there was a positive association with the sunscreen treatment. The sunscreen treatment also favored an interaction network with central Micrococcus genus. The in vitro results showed that both single UV filters and combinations had specific effects on the survival rates of L. crispatus, C. acnes, and S. epidermidis. Conclusion We identified potential microorganisms and bacterial interactions that were associated with an SPF 20 sunscreen treatment. The specific protection of L. crispatus as a key player in the UV‐exposed skin microbiome and reduction of C. acnes population by UV filters might lead to new cosmetic concepts for photoprotection.

Abstract:Introduction and objective: The sun emits ultraviolet radiation in the UV-A, UV-B and UV-C ranges. Photoprotection is used to prevent the impact of this radiation on the skin. It effectively counteracts photoaging, photodermatoses and photocarcinogenesis. The aim of the study is to discuss the impact of UV radiation on the skin and the role of sun protection, also in special groups of patients - children and non-immunocompetent people. The article also presents a summary of data on the effect of sunscreens on vitamin D synthesis. Materials and methods: The literature was reviewed in the Pubmed, Google Scholar database and in the recommendations of the Polish Dermatological Society with the use of keywords. State of knowledge: Erythema is the most visible effect of sunburn, but exposure to UV radiation is primarily photocarcinogenesis or photoimmunosuppression. The SPF factor is the main determinant of the effectiveness of sunscreens, but effective protection against both UVB and UVA radiation is essential. Daily use of sunscreen reduces skin damage associated with exposure to light and does not negatively affect vitamin D synthesis. Conclusions: The basis of skin protection is using sunscreens, avoiding excessive exposure to solar radiation and using protective clothing, hats and sunglasses. Thanks to these activities, we can significantly reduce the number of cases of certain skin cancers. It is also important to remember that sun protection applies to everyone, especially children and non-immunocompetent people.

Abstract:With increasing awareness regarding the risks of sunburn, photoaging, and skin cancer, the use of sunscreens has increased. Organic and inorganic filters are used in sunscreen products worldwide. Concerns have been raised regarding the environmental effects of commonly used organic ultraviolet (UV) filters, including oxybenzone (benzophenone-3), 4-methylbenzylidene camphor, octocrylene, and octinoxate (ethylhexyl methoxycinnamate). Studies have identified UV filters such as oxybenzone, octocrylene, octinoxate, and ethylhexyl salicylate in almost all water sources around the world and have commented that these filters are not easily removed by common wastewater treatment plant techniques. Additionally, in laboratory settings, oxybenzone has been implicated specifically as a possible contributor to coral reef bleaching. Furthermore, UV filters such as 4-methylbenzylidene camphor, oxybenzone, octocrylene, and octinoxate have been identified in various species of fish worldwide, which has possible consequences for the food chain. As dermatologists, it is important for us to continue to emphasize the public health impact of excessive sun exposure and advise our patients about proper photoprotection practice, which consists of seeking shade, wearing photoprotective clothing (including hats and sunglasses), and applying appropriate sunscreens.

Abstract:Sunscreens have been on the market for many decades as a means of protection against ultraviolet-induced erythema. Over the years, evidence has also shown their efficacy in the prevention of photoaging, dyspigmentation, DNA damage, and photocarcinogenesis. In the USA, most broad-spectrum sunscreens provide protection against ultraviolet B (UVB) radiation and short-wavelength ultraviolet A (UVA) radiation. Evidence suggests that visible light and infrared light may play a role in photoaging and should be considered when choosing a sunscreen. Currently, there is a paucity of US FDA-approved filters that provide protection against long UVA (> 370 nm) and none against visible light. Additionally, various sunscreen additives such as antioxidants and photolyases have also been reported to protect against and possibly reverse signs of photoaging. This literature review evaluates the utility of sunscreen in protecting against photoaging and further explores the requirements for an ideal sunscreen.

Impact of ultraviolet radiation and exposome on rosacea: Key role of photoprotection in optimizing treatment

Abstract:Background: Pathophysiology of rosacea is not completely understood and involves a complex interaction among genetics, ultraviolet (UV) light, microorganisms, impaired skin barrier, neuronal and vascular dysfunction, and immune system disruption. Aims: To describe the etiology of rosacea with an emphasis on the role of UV radiation and exposome, and to review the importance of non-pharmacologic strategies focusing on photoprotection. Methods: We conducted a narrative review of the literature. We performed literature searches with PubMed from January 1990 to November 2020 using the keywords "rosacea", "pathogenesis", "ultraviolet radiation", "exposome", "photoprotection", "sunscreens" and "non-pharmacologic agents". The search was limited to English, Spanish, and French language articles. Results: Several environmental factors such as UV light, diverse microorganisms, air pollution, tobacco smoking, nutrition, and psychological stress showed to trigger or worsen rosacea. UV radiation was reported to induce pro-inflammatory, pro-angiogenic, and pro-fibrotic responses. We found 6 original articles about the impact of sunscreens on rosacea. The use of sunscreens containing ingredients with emollient, anti-inflammatory, and/or vasoregulatory properties was shown to significantly improve symptomatology. Conclusion: UV radiation and the exposome play a key role in the development of rosacea. UV light is implicated in all significant aspects of rosacea: skin inflammation, neoangiogenesis, telangiectasia, and fibrosis, and may even initiate rosacea. While the use of sunscreens is widely recommended, the literature on the impact of photoprotection in rosacea is scarce. Adequately formulated sunscreens could not only provide the required level of photoprotection, but may also help to mitigate the barrier dysfunction, neutralize facial redness (tinted sunscreens), and decrease inflammation and vascular dysfunction.

Abstract:Ultraviolet (UV) radiation is ubiquitous in the environment, which has been classified as an established human carcinogen. As the largest and outermost organ of the body, direct exposure of skin to sunlight or UV radiation can result in sunburn, inflammation, photo-immunosuppression, photoaging and even skin cancers. To date, there are tactics to protect the skin by preventing UV radiation and reducing the amount of UV radiation to the skin. Nevertheless, deciphering the essential regulatory mechanisms may pave the way for therapeutic interventions against UV-induced skin disorders. Additionally, UV light is considered beneficial for specific skin-related conditions in medical UV therapy. Recent evidence indicates that the biological effects of UV exposure extend beyond the skin and include the treatment of inflammatory diseases, solid tumors and certain abnormal behaviors. This review mainly focuses on the effects of UV on the skin. Moreover, novel findings of the biological effects of UV in other organs and systems are also summarized. Nevertheless, the mechanisms through which UV affects the human organism remain to be fully elucidated to achieve a more comprehensive understanding of its biological effects.

Abstract:Sunlight radiation is a fundamental component of our daily lives. Specifically, blue light as well as UV light appear to play a role in the development of oxidative stress, DNA damage, photoaging, and pigmentation through the chromophores in skin tissues. However, several skin problems like psoriasis, eczema, and atopic dermatitis can be avoided with short-duration exposures to low-energy blue light radiation or UV radiation. In addition, exploring the effects of blue light as well as UV radiation on skin is quite essential for the development of minimally invasive antiaging strategies and for the design of innovative cosmetic formulations in modern aesthetics and cosmetology. Thus, in this review, we present the advantages as well as the disadvantages of light radiation, with a special focus on blue light and UV radiation activity on the human skin. We also discuss the molecular action of blue light and UV radiation on human skin. Other types of light radiation are included to holistically approach the effect of light on human skin.

Abstract:In recent years, the mechanisms of repeated ultraviolet light (UV) radiation-induced spatial and temporal changes in the immune microenvironment and the imbalance are the hot research topic in the fields of skin, immunity, tumor and aging. Chronic UV radiation (UVR) can induce cellular phenotypic and functional changes in the skin microenvironment, and directly and indirectly alter the skin immune microenvironment. UVR induces immunosuppression, including activation of regulatory T cells and alterations in the number and function of antigen-presenting cells. The interactions among B cells, T cells, Langerhans cells and mast cells play an important role in UVR-induced immunosuppression. In addition, the effects of UVR on T cell co-inhibitory and co-stimulatory signals have been increasingly appreciated. In this paper, we review the recent progress in the imbalance of the immune microenvironment in the chronically photodamaged skin.
Photoprotection: Current developments and controversies

Abstract:This review aimed at summarizing some of the key points that were discussed during the photoprotection session at the International Forum of Dermatology in 2022. This international conference was designed to address prominent topics of clinical dermatology in a holistic way, allowing to articulate multiple viewpoints. Therefore, this review does not claim to be exhaustive, but is instead intended to give an overview of recent developments and ongoing controversies in the field of photoprotection. Cumulative ultraviolet radiation (UVR) exposure is the major aetiological factor in the development of photoageing, photoimunosuppression and photocarcinogenesis. UVA (320-400 nm) penetrates into the dermis and damages DNA and other intracellular and acellular targets primarily by generating reactive oxygen species (ROS). It is the major contributor to photoageing, characterized by fine and coarse wrinkles, dyspigmentation and loss of elasticity. UVB (290-320 nm) is responsible for sunburns through direct damage to DNA by the formation of 6-4 cyclobutane pyrimidine dimers (CPDs) and pyrimidine 6-4 pyrimidone photoproducts. Both UVA and UVB exposure increase the risk of basal cell carcinoma, squamous cell carcinoma and melanoma. In recent years, visible light (VL; 400-700 nm) has also been implicated in the exacerbation of conditions aggravated by sun exposure such as hyperpigmentation and melasma. Photoprotection is a critical health strategy to reduce the deleterious effects of UVR and VL. Comprehensive photoprotection strategies include staying in the shade when outdoors, wearing photoprotective clothing including a wide-brimmed hat, and sunglasses, and the use of sunscreen. Due to the absorption of UV filters, the safety of sunscreens has been questioned. Newer sunscreens are becoming available with filters with absorption even beyond the UV spectrum, offering enhanced protection compared with older products. Prevention of photocarcinogenesis, sun-induced or sunlight-exacerbated hyperpigmentary conditions and drug-induced photosensitivity is an important reason for adopting comprehensive photoprotection strategies.
A Review of Sunscreen Safety and Efficacy zyxwvutsrqponmlkjihgfedcbaZYXWVUTSRQPONMLKJIHGFEDCBA

Abstract:The use of sunscreen products has been advocated by many health care practitioners as a means to reduce skin damage produced by ultraviolet radiation (UVR) from sunlight. There is a need zyxwvutsrqponmlkjihgfedcbaZYXWVUTSRQPONMLKJIHGFEDCBA to better understand the efficacy and safety of sunscreen products given this ongoing campaign encouraging their use. The approach used to establish sunscreen efficacy, sun protection factor zyxwvutsrqponmlkjihgfedcbaZYXWVUTSRQPONMLKJIHGFEDCBA (SPF), is a useful assessment of primarily UVB (290-320 nm) filters. The SPF test, however, does not adequately assess the complete photoprotective profile of sunscreens specifically against long wavelength UVAI (340-400 nm). Moreover, to date, there is no singular, agreed upon method for evaluating UVA efficacy despite the immediate and seemingly urgent consumer need to develop sunscreen products that provide broad-spectrum UVB and UVA photoprotection. With regard to the safety of UVB and UVA filters, the current list of commonly used organic and inorganic sunscreens has favorable toxicological profiles based on acute, subchronic and chronic animal or human studies. Further, in most studies, sunscreens have been shown to prevent the damaging effects of UVR exposure. Thus, based on this review of currently available data, it is concluded that sunscreen ingredients or products do not pose a human health concern. Further, the regular use of appropriate broad-spectrum sunscreen products could have a significant and favorable impact on public health as part of an overall strategy to reduce UVR exposure. As such, the medical community and other health care providers have advocated a photoavoidance strategy consisting of limiting sunlight exposure between midday hours of 1100 and 1500, wearing protective clothing and using sunscreens. Because sunscreens prevent sunburn and their use is encouraged, it has been suggested that sun exposure may actually be prolonged because users believe they are protected and therefore will spend more time in the sun. This potential consequence raises several ancillary concerns. For example, because most sunscreens are primarily UVB (290-320 nm) and, in some cases, short wavelength UVAII (320-340 nm) filters, then use of such products changes the UVR spectrum to which the skin is exposed. Consequently, if behavior is modified by sunscreen use resulting in longer periods of sun exposure, then the dose of long-wavelength UVR, 340 nm and above, would be increased. Further, even though sunscreens prevent sunburn, little is known regarding the threshold or dose-response for UVR-induced effects on other endpoints such as immunosuppression or DNA damage. Finally, because sunscreens are becoming widespread and available, questions have been raised regarding their long-term safety, particularly in the presence of UVR. The intent of this review is to address these concerns, when possible, with direct evidence and discuss ways that sunscreen products might be improved. To this end, it seems necessary to examine some basic concepts regarding the complexities of UVR and its effects on skin. Abstract:Background: Acne pathophysiology includes a complex interaction among inflammatory mediators, hyperseborrhea, alteration of keratinization and follicular colonization by Propionibacterium acnes. Aims: To describe the impact of the exposome on acne and how photoprotection can improve outcomes. Methods: A narrative review of the literature was carried out; searches with Google Scholar and Pubmed from January 1992 to November 2022 were performed. The keywords used were "acne," "sunscreens," "photoprotection," "cosmetics," "cosmeceuticals," "pathogenesis," "etiology," "exposome," "sunlight," "stress," "lack of sleep," "diet," "postinflammatory hyperpigmentation," "pollution," "exposome," "ultraviolet radiation," and "visible light." Results: Environmental factors such as solar radiation, air pollution, tobacco consumption, psychological stress, diverse microorganisms, nutrition, among others, can trigger or worsen acne. Solar radiation can temporarily improve lesions. However, it can induce proinflammatory and profibrotic responses, and produce post-inflammatory hyperpigmentation and/or post-inflammatory erythema. While photoprotection is widely recommended to acne patients, only four relevant studies were found. Sunscreens can significantly improve symptomatology or enhance treatment and can prevent post-inflammatory hyperpigmentation. Furthermore, they can provide camouflage and improve quality of life. Based on acne pathogenesis, optimal sunscreens should have emollient, antioxidant and sebum controlling properties. Conclusions: The exposome and solar radiation can trigger or worsen acne. UV light can induce post-inflammatory hyperpigmentation/erythema, and can initiate flares. The use of specifically formulated sunscreens could enhance adherence to topical or systemic therapy, camouflage lesions (tinted sunscreens), decrease inflammation, and reduce the incidence of post-inflammatory hyperpigmentation/erythema.

Abstract:The link between chronic sun exposure of human skin and harmful clinical consequences such as photo-aging and skin cancers is now indisputable. These effects are mostly due to ultraviolet (UV) rays (UVA, 320–400 nm and UVB, 280–320 nm). The UVA/UVB ratio can vary with latitude, season, hour, meteorology and ozone layer, leading to different exposure conditions. Zenithal sun exposure (for example on a beach around noon under a clear sky) can rapidly induce visible and well-characterized clinical consequences such as sunburn, predominantly induced by UVB. However, a limited part of the global population is exposed daily to such intense irradiance and until recently little attention has been paid to solar exposure that does not induce any short term clinical impact. This paper will review different studies on non-extreme daily UV exposures with: (1) the characterization and the definition of the standard UV daylight and its simulation in the laboratory; (2) description of the biological and clinical effects of such UV exposure in an in vitro reconstructed human skin model and in human skin in vivo, emphasizing the contribution of UVA rays and (3) analysis of photoprotection approaches dedicated to prevent the harmful impact of such UV exposure.

Abstract:Introduction Prolonged exposure to ultraviolet (UV) radiation from the sun poses numerous risks to the skin, ranging from premature aging to serious health conditions such as skin cancer. UV radiation can penetrate the skin's surface, causing damage to its cells and DNA, which can lead to the formation of wrinkles, sunspots, and other signs of aging. Therefore, consistent and effective sun protection is essential for maintaining the health and vitality of the skin, as well as reducing the risk of sun-related skin damage and diseases. Aim of the study The objective of the study is to conduct a thorough investigation into different aspects of photoprotection and its effects on the skin. This entails assessing the effectiveness and safety of sunscreens, as well as evaluating the impact of sun-protection clothing on skin health. Materials and methods The purpose of this review is to assess the current literature of the effectiveness of the various sun protection measures. The literature was reviewed in the Pubmed, Google Scholar data base. Results Prolonged UV exposure incites photoaging, carcinogenesis, and immunosuppression, amplifying the risk of skin malignancies. Sun-protective clothing, with adequate UPF ratings, emerges as a pivotal element in mitigating UV-induced skin damage, notably reducing the development of pigmented moles and melanoma. Furthermore, effective sunscreen usage, coupled with broad-spectrum protection, is essential in averting UV-induced skin damage and curbing the incidence of skin cancer. These findings emphasize the imperative of comprehensive photoprotection strategies, integrating sunscreen application, sun-protective clothing, and individual risk assessment, to safeguard against solar-induced skin damage and mitigate the prevalence of skin cancer.

Abstract:Skin areas exposed to sunlight's ultraviolet radiation display increased vulnerability to photoaging. This phenomenon manifests through noticeable signs such as dryness, uneven pigmentation, lentigines, hyperpigmentation, wrinkle formation, and decreased skin suppleness. Within the skin care domain, there is an escalating interest in harnessing the potential of plant-derived natural compounds for combatting photoaging. The present article seeks to comprehensively review scientific investigations elucidating the intricate cellular and molecular mechanisms contributing to the onset of UV-induced skin photoaging. Furthermore, the purview of this review article extends to the brief consolidation of the photoprotective attributes inherent within a curated selection of ten polyphenolic compounds. The overarching focus pertains to the mechanisms underpinning the photoprotective potency of these polyphenols. Notably, empirical investigations conducted within controlled laboratory settings and animal models proffer compelling evidence that these polyphenols can mitigate the adversities brought forth by UV radiation. In the evolving consensus, there emerges a proposition advocating for integrating polyphenols into sunscreens as a measure capable of conferring a favourable enhancement of the protective potency against UV-mediated deleterious effects. Moreover, the prospective utility of polyphenols is tacitly evoked within the milieu of dermatopathologies implicated in cutaneous responses to solar UV radiation, encompassing inflammatory cascades, oxidative perturbations, and DNA damage.

Abstract:Skin cancer is a global and increasingly prevalent issue, causing significant individual and economic damage. UV filters in sunscreens play a major role in mitigating the risks that solar ultraviolet radiation poses to the human organism. While empirically effective, multiple adverse effects of these compounds are discussed in the media and in scientific research. UV filters are blamed for the disruption of endocrine processes and vitamin D synthesis, damaging effects on the environment, inducement of acne and neurotoxic and carcinogenic effects. Some of these allegations are based on scientific facts while others are simply arbitrary. This is especially dangerous considering the risks of exposing unprotected skin to the sun. In summary, UV filters approved by the respective governing bodies are safe for human use and their proven skin cancer-preventing properties make them indispensable for sensible sun protection habits. Nonetheless, compounds like octocrylene and benzophenone-3 that are linked to the harming of marine ecosystems could be omitted from skincare regimens in favor of the myriad of non-toxic UV filters.

Abstract:Ultraviolet (UV) radiation has been demonstrated to cause skin disorders, including sunburn and relative symptoms of prolonged exposure. It has been reported that sunscreens have beneficial effects in reducing the incidence of skin disorders (sunburn, skin aging, and immunosuppression) through their ability to absorb, reflect, and scatter UV. Many commercial products have recently been manufactured from not only usual organic and inorganic UV filters, but also hybrid and botanical ingredients using typical formulations (emulsion, gel, aerosol, and stick). Particularly, these products have been supplemented with several preeminent properties to protect against the negative effects of not only UVB, but also UVA. However, the use of sunscreen has faced many challenges, including inducing photoallergic dermatitis, environment pollution, and deficiency of vitamin D production. Therefore, consumers should efficiently apply suitable products to improve sun protection. as well as to avoid the side effects of sunscreen.
