Abstract:Objectives: To evaluate the feasibility, effectiveness, and safety of computed tomography-assisted auricular cartilage grafting for treating alar base depression secondary to unilateral cleft lip. Design and Setting: For patients with obvious depression of the alar base, the difference in heights of the alar base and the piriform margin between the cleft side and the noncleft side were measured with computed tomography. If both were >3.0 mm, the cartilage was harvested postauricularly and subdivided into 2 to 4 pieces. A multiple layer graft was inserted under the depressed alar base. The procedure was performed from 2006 to 2013, and the follow-up period was 3 to 15 months. Participants: Chinese patients with alar base depression secondary to unilateral cleft lip were selected. Intervention: Suture and cartilage graft techniques. Main Outcomes Measures: Differences in bilateral alar base heights and piriform apertures. Results: There was no wound dehiscence, exposure of bone, or donor site morbidity. The difference in heights in the bilateral alar bases and piriform apertures decreased. There were no obvious scars in any of the cases. Conclusions: This technique has several advantages including ease of operation, minimal trauma, satisfactory outcomes, and useful references for operation provided by computed tomography. It is a superior alternative for reconstruction of secondary alar depression.
Donor site morbidities of concha cartilage harvesting using a retroauricular approach for cleft rhinoplasty: retrospective study

Abstract:Objectives: Concha cartilage is recommended for correction of cleft nasal deformities. Morbidities at the donor site have been reported in esthetic rhinoplasty cases. Reports on cleft patients are limited, so we investigated the complications of concha cartilage harvesting using the retroauricular approach in cleft rhinoplasty and their management.Materials and Methods: This was a retrospective review of the charts of 63 patients with cleft deformities who underwent septorhinoplasty with concha cartilage. All cases were harvested using a retroauricular approach. Data on patient demographics, surgery type, amount of cartilage harvested, and complications were gathered.Results: Sixty-three patients were enrolled (21 males and 42 females). The mean age of patients was 20.2±5.9 years. Complications were observed in 6 cases (9.5%) and included delayed wound healing (4.8%), prolonged postoperative pain (1.6%), postoperative paresthesia (1.6%), and prominauris (1.6%).Conclusion: The rate of complications associated with concha cartilage harvesting using a retroauricular approach is low. The use of meticulous surgical techniques, especially hemostasis control and adequate wound dressing, is key to minimizing postoperative complications.
Auricular Deformity Correction with Simultaneous Reconstruction of The Conchal bowl Using Autologous Ear Cartilage For "Flat Ear": A Novel Surgical Technique

Abstract:Backgrounds "Flat ear" is a subtype of Grade II conchal-type microtia characterized by severely underdeveloped conchal bowl. Traditional surgical techniques often rely on autologous costal cartilage, which poses several risks and complications. This study aimed to introduce a novel surgical technique using autologous ear cartilage in auricular deformity correction surgery with simultaneous conchal bowl reconstruction for "flat ear". Methods A total of 19 patients were involved in this single-center, retrospective cohort study. All patients underwent the described surgical technique. The data before the surgery, immediately after the surgery, and at the last follow-up were collected, including length, width, perimeter of auricle and depth of conchal bowl. Patient satisfaction was assessed via the Visual Analog Scale (VAS). Results The study included 12 males and 7 females, with an average age of 10.67 ± 3.86 years. Postoperative results showed significant improvements in auricular measurements, with the affected ear achieving near symmetry with the normal ear. The mean conchal bowl depth was restored from a preoperative 4.33±1.78 mm to 17.32±1.28 mm postoperatively. VAS scores for patient satisfaction increased significantly from 1.44±0.92 preoperatively to 7.72±1.49 postoperatively, with stability observed at the last follow-up. Conclusion The novel technique offers a promising alternative to traditional costal cartilage-based auricular deformity correction surgery, providing excellent aesthetic outcomes and high patient satisfaction. This approach may expand treatment options for patients with "flat ear" and related auricular deformities, with reduced invasiveness and potential for future auditory rehabilitation.

Iatrogenic Conchal Defect Secondary to Auricular Cartilage Graft

Abstract:Background: Cleft lip and palate are the most common craniofacial congenital malformations. Timing of the nasal repair remains somewhat controversial. Some authors perform a combined nasal and lip repair in infancy while others advocate for a staged repair with the nasal component occurring later in childhood. Frequently, secondary repair is needed to address residual nasal deformities in early adulthood. Conchal cartilage has become increasingly popular as a source of cartilage for secondary reconstruction. Donor site morbidities include hematoma formation, scar formation, and wound healing complications. To the best of the authors' knowledge, there is 1 previous report of a full through-and-through conchal defect as a complication of auricular cartilage graft harvesting. The authors report an infrequently described iatrogenic conchal defect due to cartilage harvesting managed with a single-stage reconstruction using bookend flaps. Methods: A 19-year-old female with a history of unilateral cleft lip and palate repair underwent secondary rhinoplasty using conchal cartilage. A bolster dressing was applied to the conchal harvest site. The patient was unable to attend her postoperative clinic visit and was eventually seen 2 months postoperatively with the bolster still in place. A full-thickness conchal defect was present when the bolster was removed. Results: A pressure dressing such as a bolster is commonly used to prevent hematoma formation after conchal cartilage graft harvesting. This is routinely removed during the initial postoperative visit. Prolonged placement can result in donor site complications such as a full-thickness conchal defect. Conclusion: A functional and aesthetic reconstruction of a full-thickness iatrogenic conchal defect was achieved with excellent results using a local posterior flap, and 2 anteriorly based bookend flaps for closure. To the best of the authors' knowledge, this report is the first to describe a single-stage reconstruction of an iatrogenic defect in the concha as a complication of conchal cartilage harvesting.

Abstract:Introduction The autologous conchal cartilage of good elasticity is easy to harvest, thus is often used in nasal tip plasty of East Asians. However, the operation techniques vary a lot among different surgeons. This article aims to introduce 3 techniques commonly used in clinical practice. Methods One hundred three patients were included in this study and divided into 3 groups according to the shape of the nasal tip during 2017 to 2019. The patients were followed up for at least 6 months. All patients were measured with Standardized Cosmesis and Health Nasal Outcomes Survey (SCHNOS). ImageJ software was used to measure and calculate the projection ratio for lateral position standard image, and columella-labial angle (CLA) was measured. Results In group 1, SCHNOS for nasal obstruction (SCHNOS-O) score were 13.23 ± 7.61 and 14.49 ± 10.55 (P > 0.05); SCHNOS for nasal cosmesis (SCHNOS-C) score were 66.55 ± 31.23 and 21.73 ± 18.91 (P < 0.001); projection ratio were 0.51 ± 0.04 and 0.57 ± 0.05 (P < 0.001); CLA were 91.02° ± 5.67° and 93.55° ± 4.64° (P > 0.05), preoperative and postoperative, respectively. In group 2, SCHNOS-O score were 16.76 ± 13.44 and 15.44 ± 9.26 (P > 0.05); SCHNOS-C score were 61.87 ± 44.14 and 19.16 ± 29.37 (P < 0.001); projection ratio were 0.50 ± 0.05 and 0.58 ± 0.03 (P < 0.001); CLA were 92.32° ± 6.39° and 94.86° ± 5.96°(P > 0.05), preoperative and postoperative, respectively. In group 3, SCHNOS-O score were 15.25 ± 7.83 and 17.73 ± 11.66 (P > 0.05); SCHNOS-C score were 52.03 ± 33.38 and 17.73 ± 11.66 (P < 0.001); projection ratio were 0.53 ± 0.05 and 0.57 ± 0.02 (P < 0.001); CLA were 91.65° ± 5.75° and 93.21° ± 5.38° (P > 0.05), preoperational and postoperational, respectively. Conclusions None of these 3 techniques cause or aggravate nasal obstruction, and all of them can achieve high cosmetic satisfaction. Technique 1 and technique 2 are suitable for the patients with moderate and moderate to severe short nose that is common in East Asia, which can better increase the protrusion of the tip and length of the nose. Technique 3 is suitable for the patients with better shape of the nose, who need to slightly increase the protrusion of the nasal tip and increase the upward rotation.
A Novel Rhinoplasty Sculpture Technique Using Ancient Chinese Architectural Technology

Abstract:Rhinoplasty is one of the most delicate procedures in plastic and reconstructive surgery. Precision in every millimeter is vital in ensuring a better quality of surgical outcome. Many methods have been developed to fulfill this requirement. One such method is an implant, whether autogenous or artificial. Artificial implants involve several surgical complications, such as recipient rejection of implant material, infections, and rigid feel of the nasal tip. Hence, autogenous implants were eventually more widely applied, with material being obtained from the septum, concha, or rib cartilages. Therefore, the authors developed a new technique of rhinoplasty, using the osteochondral rib as an autogenous implant based on the ancient Chinese architectural technology called the DouGong method. The authors hereby present the results of 288 patients treated in our faculty wherein the data from the patients and the preoperative and postoperative 3-dimensional computed tomography scans were processed using Mimics software. The uniqueness of these implants is that the joint between the nasal dorsum and the columella strut is fixated without any screw, stitches, or K-wire. This procedure proved to be very useful as this technique not only minimized the application of fixation techniques, but also helped achieve a better nasofrontal angle, nasolabial angle, and columella length. After monitoring follow-ups of our patients, the authors hereby propose the use of this DouGong-based novel technique to improve the overall quality and outcome of corrective rhinoplasty.

Abstract:A successful total auricular reconstruction with autogenous costal cartilage depends primarily on the sculpture of the cartilaginous auricular framework. Various refinements and modifications have allowed exceptional results in experienced hands, whereas a high-defining tragus and a deep conchal cavity remain the weak links in most cases. Therefore, we advocate a modification on the fabrication of the tragus. By fastening a cartilaginous cube under the tragus, the reconstructed tragus can be projected adequately and the conchal cavity is deepened simultaneously. A total of 166 consecutive patients underwent auricular reconstruction adopting this modification between 2007 and 2009. Three cases have been selected to illustrate the favourable result of this technique. Consequently, a harmonious contour of the reconstructed auricle with detailed anatomical landmarks is achieved by precise sculpting. The reconstructed tragus is proved to be stable and rigid in the follow-up.

Modifications of Three-Dimensional Costal Cartilage Framework Grafting in Auricular Reconstruction for Microtia

Abstract:BACKGROUND:Total auricular reconstruction of microtia with autogenous costal cartilage poses one of the most demanding challenges in plastic and reconstructive surgery because of the unique and complex three-dimensional shape of the auricle. Attaining an ideal result of the constructed ear depends primarily on the fabricated cartilaginous ear framework. In this article, the authors discuss the essential modifications of ear framework grafting. The authors elaborate the relevant anatomical details for a harmonious ear framework and present a favorable result of the contour of the reconstructed auricle.METHODS:Fabricating the auricular framework corresponds roughly with Brent's method. As the cartilage framework used for reconstruction is always imperfect or insufficient for the fabrication of anatomical structures, especially the tragus and the conchal bowl, individualized framework grafting based on different patient age groups and different degrees of strength and thickness of the rib cartilages have been devised accordingly.RESULTS:A total of 126 patients underwent reconstruction using autogenous costal cartilage between 2007 and 2008. Three cases performed with the new modification of the framework fabrication are shown. They reveal that a harmonious ear framework was achieved and presented a reconstructed auricle with a favorable result. Surgery-related complications occurred in four cases.CONCLUSIONS:Consequently, individualized framework fabrications are useful in every case and the cartilaginous cube fastened under the constructed tragus is proving to keep the tragus in a stable and erect position. Therefore, the modifications allow one to attain a harmonious ear framework and satisfactory three-dimensional outline of the auricle.
[Clinical Study of the Autologous Concha Cartilage Transplantation to Repair the Auricle Defect].

Abstract:Objective:To investigate the curative effect of the auricular defect repaired by the autologous concha cartilage. Methods:Seven cases with the auricular defect were repaired using the autologeous concha cartilage which combined with the retroauricular flap or retroauricular fascia with skin graftin. Results:Total 7 patients were followed up for 6 months after the surgery. The shape and size of the repaired ears were similar to the healthy ears. The teansplanted cartilages were not absorbed or deformed. Conclusion:This method is harmless and simple,which has reliable and satisfactory effects on auricle defect. We can operate under local anethesia and avoid cutting the rib cartilage.
A Simple Combination Approach for Costal Cartilage Augmentation Rhinoplasty

Abstract:Autologous costal cartilage graft is optimal for augmentation rhinoplasty but with complications including markedly visible graft margins and dorsal contour irregularities. Therefore, the authors introduced the diced costal cartilage graft to smoothen the margins of bilateral spreader graft for nasal dorsum augmentation. A total of 6 patients were recruited for this augmentation rhinoplasty. The pre- and postoperative photographs were taken for improvement evaluation. During a mean follow-up of 15 months, none of the patients developed major complications that may require removal or revision of the costal cartilage grafts. Five patients (80%) were very satisfied with the aesthetic outcome. This simple algorithm facilitates the substantial augmentation and removes the visibility of dorsal graft in the costal cartilage rhinoplasty for the East Asian population.
A Novel Technique for Short Nose Correction in Asians: M-Shaped Conchal Cartilage Combining With Septal Extension Graft.

Abstract:Background: Multiple techniques are available for short nose deformity correction. The septal extension graft represents the commonest method employed in Asians. However, a large number of surgeons hardly obtain esthetically satisfactory results since the majority of designs do not reflect the normally encountered surface anatomy of the nasal tip cartilage. Objective: The authors designed a novel technique, which combined the M-shaped conchal cartilage with the septal extension graft for overcoming the above shortcoming in Asians. Methods: Between February 2013 and March 2016, 33 patients presenting short nose deformity were surgically treated with the M-shaped conchal cartilage combined with the septal extension graft. The graft was an altered septal extension graft using the septal cartilage alongside the conchal cartilage. The harvested septal cartilage was located to the caudal septum and fixed with sutures. The conchal cartilage was trimmed into 2 strips, which were sutured together in an M-shape and firmly fixed in a bilateral manner to the caudal septal extension graft. Then, the alar cartilage was fixed with the M-shape graft. In all patients, nasal lengths, nasal tip projections, and nasolabial angles were assessed before and after surgery, respectively. Results: Nasal tip projections and nasal lengths showed remarkable increases, while columellar-labial angles were overtly decreased, in individuals surgically treated by the novel technique. Conclusions: This study presents a new method for correcting short nose deformity in Asians. The M-shaped conchal cartilage combined with the septal extension graft may effectively lengthen the nose while closely representing the actual surface of the esthetic nasal tip.
Fabrication of a Three-Dimensional Autogenous Costal Cartilage Framework in Auricular Reconstruction: Experience with 250 Cases of Asian Lobule-Type Microtia.

Abstract:BACKGROUND:Ear reconstruction is the necessary choice for patients with congenital lobule microtia. The best results are still obtained by autogenous cartilage ear reconstruction. We developed a three-dimensional autogenous costal cartilage framework for auricular reconstruction with the purpose of achieving a new auricle of high fidelity.METHODS:The patient's sixth to eighth rib cartilages were harvested as auricular framework materials, which were divided into three laminates for carving fine anatomic structures. By connecting and fixing, a three-dimensional auricular framework with a concha, helix, antihelix, triangular fossa, and navicular fossa could be used for the auricular reconstruction.RESULTS:The results obtained were considered satisfactory with vivid individual appearances and neat stable structures. Six frameworks were exposed or modified due to hematoma, infection, or cutaneous necrosis.CONCLUSIONS:With a three-dimensional auricular framework, the minute anatomic structures cited above make the external ear silhouette more vivid.
Comprehensive Application of Autologous Costal Cartilage Grafts in Rhino- and Mentoplasty.

Abstract:Background: Underdevelopment of nose and chin in East Asians is quite common. Rhinoplasty and mentoplasty are effective procedures to solve the above-depicted defects and can achieve remarkable cosmetic effects. An autologous costal cartilage graft has become an ideal material for rhinoplasty, especially for revision surgery. However, many problems in the clinical application of costal cartilage remain unresolved. This study is to investigate application strategies of autologous costal cartilage grafts in rhino- and mentoplasty. Methods: The methods involved are as follows: application of an integrated cartilage scaffold; comprehensive application of diced cartilage; and chin augmentation of an autologous costal cartilage graft. Results: In this study, satisfactory facial contour appearance was immediately achieved in 28 patients after surgery; 21 patients had satisfactory appearance of the nose and chin during the 6- to 18-month follow-up. Cartilage resorption was not observed. Two patients had nasal tip skin redness and were cured after treatment. Conclusion: This procedure can be used to effectively solve: curvature of the costal cartilage segment itself; warping of the carved costal cartilage; and effective use of the costal cartilage segment. The procedure has achieved satisfactory outcomes, and its application is worth extending to clinical practice.
Nasal deformity corrected by auricular cartilage in conjuction with silicone prosthesis

Abstract:Objective To explore the application of auricular cartilage in conjuction with silicone prosthesis to correct the nasal deformity. Metheds Augmentation rhinoplaslty was perfommnd on patients with a short nose with a low flat, blunt break point; at the same time the upper lateral part of the nasal ala cartilage was cut to reduce the width and uhgthen the nasal columella ,and the auricular cantilage in cinjuction with silicone prosthesis was used to raise and extend the break point. Results 32 cases were involved, the effects were satisfactory and there is no complication by follow-up from 3 to 18 months. In addition, the appearance of the pinna in the donor site had no change. Conclusion Auricular cartilage in conjuction with silicone prosthesis can accomplish the supportine function of nasal cartilage struction and a simple augmentation rhinoplasly with this methed is effective to correce the nasal deformity cause by cleft lip.

Abstract:Traditional unilateral costal cartilage harvesting for auricle reconstruction in children tends to cause thoracic deformities, our study develop a novel bilateral costal cartilage harvesting (BCCH) method to prevent and reduce thoracic deformities without increasing postoperative pain and cosmetic concerns for patients. Hypothesis Unilateral costal cartilage harvesting (UCCH) for auricle reconstruction in children tends to cause thoracic deformities. Therefore, our study aimed to develop a novel bilateral costal cartilage harvesting (BCCH) method to prevent and reduce thoracic deformities. Methods Patients with unilateral microtia who underwent either UCCH (n = 50) or BCCH (n = 46) were enrolled in this study. The grafts for the BCCH group were harvested from the 6th costal cartilage of the ipsilateral hemithorax and the 7th and 8th cartilage from the other hemithorax. Computed tomography and physical examination were performed to identify any physical deformities in the chest contours post‐surgery. The cosmetic appearance of the thoracic scars post‐surgery was evaluated using the Scar Cosmesis Assessment and Rating Scale (SCAR) and Visual Analogue Scales (VAS cosmetic). The numerical rating scale (NRS) was used to quantify the pain in donor sites. The reconstructed ears were assessed during the follow‐up period. Abstract:Objective To discuss and summarize the surgical management and issue in Autogenetic Costal Cartilage Harvest for Total Auricular Reconstruction.Methods From 2003 to 2006,126 cases with microtia underwent two-stage operation for total reconstruction of the auricle,the costal cartilages from the contra lateral chest are harvested with the perichondrium left.The remaining costal cartilage of some of the patients is cut into small blocks and filled the dead space formed by the perichondral pocket.Results The costal cartilages can meet the desire of the operation in all of the 126 patients.there is no aeropleura happened.And in one year's flow-up,there were no postoperative chest wall deformities and developmental disorder identified.Conclusion The incidence of aeropleura can be reduced by the methods.It is related between thoracic deformity and perichondrium left and refilled of the costal cartilages.

Abstract:Secondary rhinoplasty is a surgical procedure aimed at reshaping the nose after the unsatisfactory results of primary rhinoplasty, which usually requires a reliable source of cartilage for grafting. Septal cartilage is usually the first option for rhinoplasty, but in cases where it is insufficient, rib cartilage might be used, which is the focus of the current study. The article details the surgical techniques for rib cartilage harvesting, highlighting the preference for the seventh rib done under general anaesthesia. The surgery requires a precise incision, harvesting of the cartilage, and careful closure while the cartilage is stored in a saline solution until use. It also introduces innovative approaches to minimize complications and improve patient satisfaction, including trans-umbilical endoscopic harvesting, the use of the 10th rib to reduce morbidity, and employing fresh frozen homologous rib cartilage as a grafting alternative. Based on various studies, most cases following rhinoplasty with rib cartilage showed a high satisfaction rate despite the complexity of the procedure. Patient consideration includes good communication with realistic expectations through imaging techniques. We also discuss complications, which can heavily impact the patient's quality of life. We hope that by providing this information, our paper will provide surgeons and researchers with the latest information on this topic.

Abstract:BACKGROUND:The nasal tip plays a crucial role both esthetically and functionally. The application of nasal tip grafts is an effective method for improving nasal tip form. Ear cartilage is a common choice for nasal tip grafts, but it still presents several challenges in clinical application that need to be addressed. This study aims to address the issues associated with the use of ear cartilage in clinical rhinoplasty applications through the development of a novel septal extension graft using ear cartilage for nasal tip reconstruction. METHODS:From May 2018 to April 2022, a total of 132 cases of nasal tip reconstruction surgeries were performed using a seagull-shaped nasal septum extension graft, constructed with bilateral cavum concha cartilage. Among these cases, 25 patients had previously undergone rhinoplasty using silicone implant, 7 patients had undergone augmentation rhinoplasty using expanded polytetrafluoroethylene, whereas the rest were primary rhinoplasty cases. All patients were followed up for a period ranging from 3 months to 4 years postoperatively, with photographs taken to assess the nasal tip morphology. RESULTS:In this study, all patients exhibited good healing of the incisions made at the posterior aspect of the auricular concha, with no occurrences of hematoma and inconspicuous scarring. In 116 cases, significant improvement in nasal appearance and a realistic nasal tip form were achieved postoperatively, yielding satisfactory outcomes. Only 16 patients experienced minor issues with nasal tip morphology, which were subsequently improved through further surgical procedures. CONCLUSION:This study reports a surgical technique for nasal tip refinement using bilaterally harvested cavum concha cartilage to construct a seagull-shaped nasal septal extension graft. The procedure has achieved satisfactory outcomes, and its application is worth extending to clinical practice.
