Abstract:Objective: To investigate the prevalence and clinical characteristics of temporomandibular disorders (TMD) in a population prior to orthodontic treatment and to evaluate the possible risk factors for TMD.Methods: Clinical documentations and radiographs of 9,909 patients with malocclusions were retrospectively reviewed. All the patients were from the Orthodontic Department of Peking University School and Hospital of Stomatology during January 1998 to December 2003. The prevalence of TMD was calculated and the clinical characteristics were investigated. The association between the prevalence and the potential risk factors including aging, gender and malocclusion classifications was analysed using the method of chi-square and multivariate logistic regression analysis.Results: The prevalence of TMD was 8.6% in this population and statistically higher in females (9.5%) than in males (7.0%) (chi 2 = 18.125, P < 0.01). The prevalence had a positive association with age and increased in adolescence and young adulthood (chi 2 = 157.503, P < 0.01). Patients with Angle II malocclusion were more likely to suffer TMD than patients with Angle I malocclusion (odds ratio [OR] = 1.445, P < 0.01).Conclusion: Aging, female gender and Angle II malocclusion were risk factors for the occurrence of TMD in the population.

Abstract:Objective: To investigate the correlation between basic characteristics and clinical features of patients with temporomandibular disorders (TMD). Methods: The R language statistical tool was used to analyze the clinical information of 500 TMD patients, i.e., age, sex, joint noises, mouth opening pattern, and pain symptoms, as well as the results of the mandibular push-back test. A pairwise correlation analysis of each clinical feature was carried out. Results: The highest incidence of TMD was observed in the age group of 20 to 30 years (240/500). Around 2/3 of the patients showed pain symptoms. Abnormal mouth opening patterns, joint noises, and temporomandibular joint synovitis (TMJS) were observed in 48.4, 65.4, and 34% of patients, respectively. Conclusion: Joint click and the corrected deviation of the mouth opening pattern are signs of early-stage TMD, whereas limited mouth opening and TMJS are indicators of progressive stage and complicated TMD.

Abstract:Objective: To describe and analyze the clinical manifestations of patients with orofacial pain of temporomandibular disorders (TMD). Methods: A retrospective study on orofacial pain was conducted for 3 425 patients diagnosed as TMD based on clinical symptoms and signs in the Department of Temporomandibular Disorders and Orofacial Pain, School of Stomatology, The Fourth Military Medical University. The patients included 1 158 males and 2 267 females with a median age of 32 years. The gender, age, course of disorders, pattern and site of pain, CT imaging diagnosis of temporomandibular joint (TMJ) were analyzed. The distribution of gender, age and disorder course interval were described. The differences in frequency of the pattern and site of pain, imaging diagnosis in different gender, age and disease course interval were compared. Chi-square test and non-parametric rank sum test were performed using software SPSS 23.0. Results: Of the 3 425 patients, 29.1% (997/3 245) had signs of joint popping, and 40.1% (1 373/3 425) had restricted opening. The pain frequency was higher in males who had disorder course less than 1 month (P<0.01) and also in males who had open-and-close and/or lateral excursion and/or protrusion pain without tenderness or other pain without tenderness (P<0.05). However, the pain frequency was higher in females who had tenderness (P<0.01). The pain frequencies in those over 56 years old with tenderness combined with open-and-close and/or lateral excursion and/or protrusion pain were higher than in patients of other ages (P<0.01). In patients with unilateral TMJ pain, the frequency in males was higher than females(P<0.01), while the frequency in females was higher in patients with unilateral TMJ pain combined with unilateral or bilateral myalgia and the frequency was higher in patients under 15 years old having bilateral TMJ pain and/or unilateral or bilateral myalgia (P<0.05). In patients with unilateral TMJ pain, the frequency in those with disorder course≤1 month was higher than in those with other disease duration intervals (P<0.01), while in patients with bilateral myalgia or TMJ pain plus unilateral or bilateral myalgia, the frequency in those with disorder course>3 years was higher than in those with other disease duration intervals(P<0.01). In patients with unilateral TMJ pain, the frequency was higher in those having open-and-close and/or lateral excursion and/or protrusion pain (P<0.01). In patients with unilateral myalgia and bilateral myalgia, the frequency was higher in those having tenderness (P<0.01). The frequency of TMJ space changes in male patients was higher than females and the frequency of hyperosteogeny and resorption in females were higher than males (P<0.05). The frequency of TMJ space changes and developmental problems were higher in patients aged 16 to 35 years, while the frequencies of hyperosteogeny, bone resorption and cystis in those over 56 years were higher than other ages (P<0.01). The frequency of TMJ space changes in patients with disorder course≤1 month was higher than in those with other disease duration intervals (P<0.01), while the frequency of hyperosteogeny was higher in patients with disorder course>3 years (P<0.01). Conclusions: The male to female ratio in the present patients with orofacial pain of TMD was about 1 to 2. Most of the patients visited hospital within half a year after the disorders occurred. The pattern and site of the orofacial pain, signs on TMJ CT images showed some distribution regularities in views of gender, age and disorder course.
Analysis of temporomandibular joint structural abnormalities in orthodontic patients

Abstract:PURPOSE: Temporomandibular joint(TMJ) problems are becoming more and more important in orthodontic patients. This study analyzed TMJ problems in orthodontic patients in order to provide reference for treating orthodontic patients with TMJ problems. METHODS: Two hundred and fifty five orthodontic patients were randomly selected in our department during the recent 2 years, including 80 males and 175 females, aged from 10 to 45 years old. X-ray film and MRI on TMJ of both sides at bite and open position were used to analyze TMJ problems. The results were analyzed with SPSS 17.0 software package. RESULTS: Of all 255 patients, 176 had TMJ problems, accounting for 69.02%. Of all 80 male patients, 48 had TMJ problems, accounting for 60.00%. Of all 175 female patients, 128 had TMJ problems, accounting for 73.14%, significantly higher than the male patients. CONCLUSION: The results showed that nearly two third of orthodontic patients have TMJ problems. Most patients have no obvious clinical symptom but at a potential risk for orthodontic treatment. Females are more susceptible to TMJ problems.
Age-related Differences in Diagnostic Categories, Psychological States and Oral Health-Related Quality of Life of Adult Temporomandibular Disorder Patients

Abstract:Objectives The peak prevalence of temporomandibular disorders (TMDs) may occur in middle age. This study determined the proportion of matured adults seeking TMD treatment and compared their diagnostic, psychological and oral health-related quality-of-life (OHRQoL) profiles to younger patients. Methods Adult subjects were recruited from a tertiary TMD centre and assigned to three age groups, namely 18-44 years (young adults [YA]), 45-64 (middle-aged adults [MA]) and >= 65 (old adults [OA]). TMD diagnoses were established with the Diagnostic Criteria for TMDs and categorised as pain-related (PT), intra-articular (IT) and combined (CT) TMDs. Psychological states and OHRQoL were assessed with the Depression, Anxiety, and Stress Scale-21 (DASS-21) and Oral Health Impact Profile-TMDs (OHIP-TMDs). Demographic, DASS-21, and OHIP-TMDs data were analysed using chi-square test, one-way ANOVA and Pearson's correlation (P < .05). Results Middle-aged (19.7%; 136/692) and old (4.0%; 28/692) adults comprised about a quarter of the TMD patients. Although gender distribution was comparable, significant differences in TMD categories were observed (P < .001). Pain-related TMDs were more prevalent in the MA/OA groups while intra-articular TMDs were more frequent in the YA group. No significant difference in DASS-21 and total OHIP scores was noted among three groups. However, the MA and OA groups had significantly lower OHRQoL in the physical pain domain. Correlations between DASS-21 and OHIP-TMDs scores varied with age and ranged from r(s) = 0.47-0.92. Conclusions Matured patients constituted a quarter of TMD cohort and presented higher frequencies of painful TMDs. They have similar psychological profiles to younger patients but experienced lower OHRQoL in physical pain domain.

Abstract:Background Temporomandibular joint disorders (TMDs) are common in young adults, and the link between chronotype profile and TMDs is unclear. Objective This study examined TMD prevalence and chronotype distribution and explored the relationship between chronotype and TMDs in young adults. Materials and Methods A total of 663 students from Sichuan University completed questionnaires. Chronotype profiles were assessed using the Morningness‐Eveningness Questionnaire, and TMDs were screened using the Fonseca Memory Index. To validate the findings, 68 TMD patients and 136 controls were enrolled. Results The prevalence of TMDs was 69.7%, with significant differences among chronotype profiles. The intermediate profile was the most common chronotype. Eveningness profile was associated with higher TMDs prevalence and severity. Muscle pain and side movement difficulty scores were higher in eveningness and intermediate profiles. Female gender (OR 2.345; 95% CI 1.668–3.297) was a TMD risk factor, while morningness profile (OR 0.537; 95% CI 0.297–0.970) was protective. Validation with TMD patients and controls supported these findings, showing higher eveningness profile prevalence in the TMD groups. Conclusions TMDs have a high prevalence in college students, chronotype profiles shown to be associated with TMDs. Morningness is the protection factor in TMDs and PT, eveningness is a risk factor for IT.

A Retrospective Study on the Relationship Between Aging and Tomographic Findings in 174 Patients with TMD

Abstract:The purpose of this study was to examine age-related changes in the tomographic characteristics of the temporomandibular joints (TMJ) of patients with temporomandibular dysfunction (TMD). 174 patietns with TMD from 12–76 years old were divided into six groups by age. Bilateral TMJs of the patients were examined by TMJ tomography, and 91 joints were also performed with arthrography. The frequencies of abnormal condylar shape and osseous changes (including osteosclerosis, flattening, bone erosion and osteophytes) increased with age (P<0.01), less osseous changes were observed in the younger patients (<30 years) with anterior disk displacement with reduction than in the older ones (≥30 years) (P<0.05). No statistically significant difference in the frequency of condylar displacement was found among the groups. It was concluded that the frequencies of abnormal condylar shape and some osseous changes might increase with aging, which might be due to remodeling in TMJ and low adaptive ability of the TMJs in elderly people.

Abstract:Objective. The aim of this study was to characterize occlusal stability in young adults with temporomandibular disorder (TMD).Study Design. Thirty-one patients (aged 19-31 years) with complete natural dentition and Angle class I occlusion who exhibited TMD were compared with 31 age- and sex-matched healthy control subjects. The occlusal registrations were performed using the T-Scan II occlusal imaging and analysis system. Center of occlusal force, asymmetry index of occlusal force, maximal movement of COF, premature contacts, clusion time, and disclusion time were recorded.Results. Compared with control subjects, TMD subjects had a significantly higher frequency of premature contacts (16/32, 50.0%) and greater bilateral asymmetry in the occlusal force. Furthermore, prolonged clusion time and disclusion time also were observed in TMD subjects.Conclusions. Abstract:Objective:o analyze TMJ structure problems in orthodontic patients of different age stage.Methods:301 orthodontic patients were randomly selected in our department during the recent 2 years,including 97 males and 204 females,aged from 10 to 34 years old.Divided by age into 5 teams,10-14 years team,15-19 years team,20-24 years team,25-29 years team,and 30-34 years team.X-ray and MRI on TMJ of both sides were used to analyze TMJ structure problems.The results were analyzed with SPSS 17.0.Results:Of all 301 patients,58 patients were in the 10-14 years team,35 normal and 23 abnormal,abnormal ratio was 39.66％.77 patients were in the 15-19 years team,20 normal and 57 abnormal,abnormal ratio was 74.03％.92 patients were in the 20-24 years team,22 normal and 70 abnormal,abnormal ratio was 76.09％.57 patients were in the 25-29 years team,20 normal and 37 abnormal,abnormal ratio was 64.91％.17 patients were in the 30-34 years team,5 normal and 12 abnormal,abnormal ratio was 70.59％.Conclusion:TMJ structure abnormal ratio of the adult patients was significantly higher than that of the 10-14 years team.The range of 15-19 years was the high-risk stage and after the age of 20,the incidence of the disease is basically stable.
Association between temporomandibular disorders with clinical and sociodemographic features: An observational study

Abstract:Background: Temporomandibular disorders (TMD) are the most common orofacial impairment, but the relevance of certain clinical features in TMD is not clear. Objective: The main objective of this study was to analyse if temporomandibular disorders (TMD) are associated with sociodemographic (eg age, height, weight, body mass index and gender), psychological (eg anxiety and depression) and clinical features (eg use of dental splints, orthodontics, retainers, bruxism, sleep disturbance, familiar prior history of TMD and dental occlusion). Methods: An observational study was conducted to calculate the correlation of TMD, as assessed with DC/TMD criteria, with sociodemographic, psychological and clinical features on 59 voluntary subjects with TMD (n = 45) and without TMD (n = 14). Sex, height, weight, body mass index, age, use of dental splint, orthodontics, retainers, parafunctional disorders, sleep disturbance, familiar history of TMD, bruxism, anxiety, stress and dental occlusion class data were included in a multivariable correlation analysis to determine which variables are associated with TMD and bruxism. Results: TMD was found to be correlated with none of the features assessed (P > .05), but a negative correlation with the use of dental splint (P < .05). Dental occlusion class showed no statistically significant correlation with any assessed feature (P > .05). Conclusion: The etiology of TMD is not clear and considering certain clinical features including dental occlusion are not justified in the evidence-based TMD practice for making irreversible occlusal treatment decisions.
The Current Status And Problems In Clinical And Research Work On Temporomandibular Disorders In China

The current status of clinical work and basic research achievements on TMD in China were surveyed. In addition, the establishment of the Chinese Society of TMD and Occlusion, the problems in TMD treatment and prospects of TMD clinical and research work in China are introduced and discussed.
Investigation of Psychological Status in Female Patients with Different Subtypes of Temporomandibular Disorders

Abstract:Objective To evaluate the levels of psychosocial status of differenu subtypes of temporomandibular disorders(TMD)patients.Methods One hundred and sixty-two female patients who came to the Center for TMD and Orofacial Pain of Peking University School of Stomatology and 48 healthy female as controls were included in this study.The assessment of psychological status of the TMD patients of this study was by using the SCL-90.These patients were classified according to the diagnostic classification proposed by Ma Xu-Chen and Zhang Zhen-Kang in 1998.Multivariate Analysis of Variance and Canonical correlation were performed to determine the association between psychological status level and the diagnostic subgroups of TMD.Results ①Among 162 cases,102 cases(63%)had no any degenerative changes of the joints,60 cases(37%)had degenerative changes of the condyles.The group without any degeneration got higher SCL-90 scores than the other groups,and there was significant difference between the healthy group and the group without any degeneration.②Of the 162 cases,27 cases(16.7%)was the patients with myofacial pain,110 cases(67.9%)with internal derangement,92 cases(56.8%)with inflamatory disorder and 70 cases(37%)with osteoarthrisis.③There was significant difference among the different subgroups(P0.01)in paranoid ideation symptom,and no significant difference in the other factors of SCL-90 among the different subgroups(P0.05).④Significant correlation neither between the psychosocial status levels and radiographs assessment nor between the psychosocial status and the physical assessment were not found.Conclusion Female patients only with myofacial pain have a higher occurrence of paranoid ideation symptoms than the patients with other subtypes of TMD..Furthermore,it has been shown that some TMD patients do have some psychological problems and symptoms of psychopathology in other TMD subtypes,but it is difficult to find out these patients just through the physiological diagnosis.It is very important to establish the two-axis approach to assess the TMD patient both on physical and psychosocial status.

Abstract:Objective. To survey the clinical manifestations and imaging features of temporomandibular disorders (TMD) and analyze the risk factors for pathogenesis/prognosis through a case-control study based on psychogenic factors of patients. Methods. According to the inclusion criteria, 200 adult patients were randomly enrolled from the maxillofacial department of our hospital from January 2020 to May 2021, including 100 patients with TMD as the study group and 100 healthy patients as the control group. The study group can be assigned into four subgroups according to their clinical manifestations: (1) articular area or/and masticatory muscle pain group, (2) mandibular movement abnormality group, (3) joint murmur group, and (4) two or more symptom groups. Based on the study of psychogenic factors of patients, the clinical manifestations and imaging features of TMD were determined, and the risk factors for pathogenesis/prognosis were analyzed. Results. The distribution of psychological status in the TMD group was higher than that in the control group ( P < 0.05 ). The distribution of anxiety, depression, and somatic symptoms in the TMD group was significantly different from that in the control group ( P < 0.05 ). Anxiety, depression, and somatic symptoms were the risk factors for TMD. Compared with the control group, the incidence of abnormal MRI images in patients with temporomandibular disorders was significantly different ( P < 0.05 ). There were significant differences in psychological status (anxiety, depression, and somatic symptoms) among the three groups ( P < 0.05 ). Anxiety, depression, and somatic symptoms were the risk factors for abnormal mandibular movement and joint tremor and murmur ( P < 0.05 ). Somatic symptoms were the risk factors for various clinical symptoms of TMD ( P < 0.05 ). Depression was the risk factor for pain ( P < 0.05 ). Conclusion. In patients with TMD, MRI can early identify disc abnormalities and other related imaging features, which is helpful for more comprehensive clinical evaluation and treatment of TMD patients. There exhibits no significant difference in psychological status (anxiety, depression, and somatic symptoms) of patients with different clinical symptoms, and abnormal psychological status may be one of the risk factors leading to different clinical symptoms and development of different types of TMD patients.

Abstract:Objective To investigate the relationship of temporomandibular disorders( TMD) related pain with age, sex and imaging changes of osteoarthrosis (OA). Methods All the patients were grouped based on age, sex, imaging changes of OA and TMD related pain. Craniomandibular index (CMI) was calculated for all the patients. Results TMD related pain was common in females. Significant differences of CMI were existed in all groups under the age of 45 years old. The occurrence of imagine changes of OA was higher in females than in males (P0.001).Conclusions TMD related pain and imaging changes of OA were more common in females, especially in 16-30 years old females.

Abstract:This study investigates the influence of chronotype on the prevalence of temporomandibular joint disorders (TMD) and the morphology of temporomandibular joint (TMJ). According to the Morningness-Eveningness Questionnaire-Self-Assessment, the participants were divided into morning group (n = 30), intermediate group (n = 83), and evening group (n = 30). Thirty participants were randomly selected from the intermediate group for subsequent examination and measurements. The morphology of TMJs was investigated using questionnaire and clinical examination form in Diagnostic Criteria for Temporomandibular Disorder. Meanwhile, the morphological results of TMJs were measured from cone-beam computed tomography images. The prevalence rate of TMD in the morning group (23%) was significantly lower than that in the intermediate group (56.7%), while there was no difference between the evening (53.4%) and intermediate groups. As to morphological measurements, there was no significant difference among three groups in mediolateral width of condylar process, anteroposterior width of condylar process, radius of condyle, medial joint space, lateral joint space, condylar stress angle, horizontal condylar inclination, width of glenoid fossa, depth of glenoid fossa, and posterior joint space, while there was a significant difference in horizontal condylar angle (p = 0.00490), articular eminence inclination (p < .0001), anterior joint space (p = 0.0163), and superior joint space (p = 0.0004). The morphology of TMJ in the morning group was better than that in the evening and intermediate groups. An association was found between TMD prevalence, temporomandibular morphology, and chronotype.

Abstract:The objective of this study was to investigate the clinical and radiographic features of osteoarthrosis (OA) of the temporomandibular joints (TMJOA) in human adolescents and young adults.Patients (n = 4883) with temporomandibular disorders (age, 11 to 30 years) underwent clinical and radiographic examinations. The radiographic findings were classified as erosive bony changes, proliferative changes mainly, including flattening with uneven sclerosis, and osteophytes of the condyle, and bilaterally short condylar processes. In addition, we interpreted the reassessment radiographs of 156 of the patients.Seven hundred eleven patients had radiographic signs of OA. The frequency of OA was higher in women (563/3360, 16.8%) than in men (148/1523, 9.7%). Most patients (541/711, 76.1%) with signs of OA showed proliferative changes of OA. Moreover, 56.4% of patients with TMJOA (88/156) remained stable.These results suggest that although OA is an age-related disease, aging is not the crucial factor in the pathogenesis of OA.
Assessment of Temporomandibular Joint Disorders: Retrospective Study of Clinical Presentations

Abstract:Objective: This retrospective research aimed to assess the clinical presentations and management strategies for "temporomandibular joint (TMJ)" disorders. Methods: Medical records of patients diagnosed with TMJ disorders between 2017 and 2022 were retrospectively analyzed. Data collected included demographic information, clinical symptoms, radiographic findings, treatment modalities, and treatment outcomes. Statistical analysis was performed to examine associations between variables and treatment outcomes. Results: A total of 150 patients were included in the research, with a mean age of 42.5 years. The most common presenting symptoms were pain (80.0%), limited mouth opening (60.0%), and clicking/popping sounds (40.0%). Muscle tenderness (53.3%) and jaw deviation (20.0%) were also observed. Treatment modalities encompassed pharmacotherapy, physical therapy, occlusal splints, and surgical interventions. Statistical analysis revealed significant associations between certain demographic characteristics and clinical presentations. Conclusion: This research provides valuable insights into the clinical presentations and management strategies for TMJ disorders. Pain, limited mouth opening, clicking/popping sounds, muscle tenderness, and jaw deviation were among the common symptoms observed. A multi-disciplinary approach involving various healthcare professionals is essential to optimize patient outcomes and improve quality of life for individuals affected by TMJ disorders.
Temporomandibular Disorders Symptoms in Asian Adolescents and Their Association with Sleep Quality and Psychological Distress

Abstract:Objectives: The aim of this study was to investigate the prevalence of temporomandibular disorders (TMD) symptoms and their association with sleep quality and psychological distress in Chinese adolescents.Methods: Five hundred seventy-eight adolescents were enrolled in the study and completed a self-reported TMD symptom questionnaire and socio-demographics survey. Sleep quality and psychological distress were assessed. Descriptive statistics and logistic regression analysis were performed with TMD symptoms as the outcome variable.Results: The prevalence of adolescents with at least one TMD symptom was 61.4%. One-third of subjects experienced disturbed sleep, depression, and stress; 65.2% experienced anxiety. Subjects with TMD symptoms had greater psychological distress and disturbed sleep than those who were symptom-free. Logistic regression analysis showed that sleep disturbance and daytime dysfunction and anxiety were significantly related to TMD symptoms.Discussion: The prevalence of TMD symptoms in Asian adolescents is high. Disturbed sleep and psychological distress are correlated with TMD; thus, a further longitudinal research of the causality is warranted.
