Abstract:Endogenous klebsiella endophthalmitis (EKE) is one of the Asia's most common endogenous bacterial endophthalmitis,the incidence of which increased significantly in recent years.The prognosis of EKE is extremely poor,and its risk factors,prognosis and standard treatments are controversial.Because lack of well-understanding of the disease,only scattered cases are reported from the mainland of China.For a better recognition of EKE,a review of its repidemiology,risk factors,treatments and prognostic factors are made,in order to improve the understanding and to guide the clinical treatments.

Abstract:With the flourishing development of precision medicine, theranostics, generally recognized as the integration of diagnosis and treatment, has emerged as a prominent trend in clinical research. However, theranostics primarily emphasizes the end result of integration, without providing sufficient details on how precise diagnosis and synergetic individualized treatment could be achieved and what clinical challenges could be effectively addressed in clinical practice. Molecular probe technology provides a robust method to bridge the gap between theory and practice. Through meticulous design of the chemical structure, imaging labels or drugs were conjugated to tumor-targeting peptides, antibodies, or inducers to form molecular probes, which allow a seamless switch between targeted intervention and targeted imaging with consistency in time, space, and biodistribution. Thus, this review proposes a concept called "molecular eye", which refers to a comprehensive system for precise diagnosis and treatment of major clinical diseases based on molecular probe technology. This medical system emphasizes the chemical basis of probe development and optimization, which can provide precise actionable information for clinical decision making, allow molecular-targeted therapy, expand the indications of old therapy, and accelerate the regulatory approval of molecular drugs. "Molecular eye" resembles the piercing eye of the Monkey King, which can detect previously "invisible" diseases and facilitate disease diagnosis, treatment, real-time evaluation, and pathology research, guiding drug development. The emergence of the "molecular eyes" will provide opportunities and challenges in the fields of clinical practice and medical research and propel the progression of contemporary medicine toward precision medicine.

Abstract:Abstract Eye diseases are remarkably common and encompass a large and diverse range of morbidities that affect different components of the visual system and visual function. With advances in omics technology of eye disorders, genome-scale datasets have been rapidly accumulated in genetics and epigenetics field. However, the efficient collection and comprehensive analysis of different kinds of omics data are lacking. Herein, we developed EyeDiseases (https://eyediseases.bio-data.cn/), the first database for multi-omics data integration and interpretation of human eyes diseases. It contains 1344 disease-associated genes with genetic variation, 1774 transcription files of bulk cell expression and single-cell RNA-seq, 105 epigenomics data across 185 kinds of human eye diseases. Using EyeDiseases, we investigated SARS-CoV-2 potential tropism in eye infection and found that the SARS-CoV-2 entry factors, ACE2 and TMPRSS2 are highly correlated with cornea and keratoconus, suggest that ocular surface cells are susceptible to infection by SARS-CoV-2. Additionally, integrating analysis of Age-related macular degeneration (AMD) GWAS loci and co-expression data revealed 9 associated genes involved in HIF-1 signaling pathway and voltage-gate potassium channel complex. The EyeDiseases provides a valuable resource for accelerating the discovery and validation of candidate loci and genes contributed to the molecular diagnosis and therapeutic vulnerabilities with various eyes diseases.

Abstract:Age-related cataract is one of the most important eye diseases leading to blindness and visual impairment worldwide. Epidemiologic studies had shown that the prevalence and incidence of age-re-lated cataract were relately high and varied among groups with different age, gender, region and race. Vari-ous types of cataract were associated with different factors including age, sex, smoking, diabetes mellitus and estrogen. Age-related cataract was still a severe public health problem for elderly, women and illiterate groups. Although great progress had been made in the diagnosis and treatment of cataract, the cataract sur-gery rate in China remained low, and the project of vision restoration from cataract still had a long way to go.

Centralized Clinical Eye Research Platform：innovation and Practice

Abstract:Clinical research is a multidisciplinarye collaborative activity that involves a particular person or group of patients, is based in a health delivery organization, and intends to investigate the diagnosis, therapy, prognosis, and etiology of human diseases. China is rich in the disease resources, however, the corresponding researches are far behind that in developed countries. On the one hand, we paid insufficient attention on clinical research. On the other hand, we do not have enough well-established clinical research platforms to support the doctors/investigators . The centralized clinical research platform, sharing the facilities, spaces, manpower, databases, experiences, and regulations, is a new model for clinical research. Eye Hospital of Wenzhou Medical University set up a clinical and epidemiological research center in 2013, by now, 64 projects are underway or completed. Sixty percent of the projects are prospectively designed, and the majority are longitudinal studies and with a large sample sizes. With the limited experience, we deem that the centralized clinical research center is an effective and practical model for clinical research in China.

Abstract:The ocular surface and orbit constitute unique microenvironments in the human body. Current advances in molecular research have deepened our understanding of tumor development in these regions. Tumors exhibit greater heterogeneity compared to normal tissues, as revealed by pathological and histological examinations. The tumor microenvironment (TME) plays a crucial role in the proliferation and progression of cancer cells. Factors from the external environment or the body's own inflammation and microcirculation interact within the TME, maintaining a delicate balance. Disruption of this balance, through uncontrolled signal pathway activation, can transform normal or benign tissues into malignant ones. In recent years, various systemic immunotherapies have been developed for cancer treatment. This study reviews the epidemiology of ocular surface and orbital tumors include squamous cell carcinoma, basal cell carcinoma, sebaceous carcinoma and lymphoma in conjunction with their occurrence, growth, and underlying mechanisms. We propose that by examining clinical histopathological images, we can identify specific and shared microscopic features of tumors. By collecting, classifying, and analyzing data from these clinical histopathological images, we can pinpoint independent diagnostic factors characteristic of tumors. We hope this study provides a basis for future exploration of the mechanisms underlying different ocular diseases.

Abstract:The European Eye Epidemiology (E3) consortium is a recently formed consortium of 29 groups from 12 European countries. It already comprises 21 population-based studies and 20 other studies (case–control, cases only, randomized trials), providing ophthalmological data on approximately 170,000 European participants. The aim of the consortium is to promote and sustain collaboration and sharing of data and knowledge in the field of ophthalmic epidemiology in Europe, with particular focus on the harmonization of methods for future research, estimation and projection of frequency and impact of visual outcomes in European populations (including temporal trends and European subregions), identification of risk factors and pathways for eye diseases (lifestyle, vascular and metabolic factors, genetics, epigenetics and biomarkers) and development and validation of prediction models for eye diseases. Coordinating these existing data will allow a detailed study of the risk factors and consequences of eye diseases and visual impairment, including study of international geographical variation which is not possible in individual studies. It is expected that collaborative work on these existing data will provide additional knowledge, despite the fact that the risk factors and the methods for collecting them differ somewhat among the participating studies. Most studies also include biobanks of various biological samples, which will enable identification of biomarkers to detect and predict occurrence and progression of eye diseases. This article outlines the rationale of the consortium, its design and presents a summary of the methodology.

Abstract:In the last 5 years, a great progress in the clinical treatment and basic research of ocular fundus diseases in China has been obtained. An abundance of clinical experience and a great deal of research data have been accumulated. In the field of clinical work, the photodynamic therapy (PDT) and transpupillary thermotherapy (TTT) of choroidal neovascular membrane have been established gradually in China. A rapid progress has been achieved in the vitreous surgery, intraocular injection of triamcinolone acetonide for treating macular edema, radial optic neurotomy (RON) for treating central retinal vein occlusion, the multi-access prevention and management of retinopathy of prematurity and intraocular tumor, and the update of the techniques and equipments for vitreous and retinal surgeries, etc. In the field of laboratory work, Chinese scientists achieved lasting and great progress in many fields: diabetic retinopathy, retinoblastoma, proliferative vitreoretinopathy, retinal pigment epithelial cells, retinal transplantation, gene therapy of ocular fundus diseases, etc. All of these achievements implied that both the clinical work and basic research of ocular fundus diseases in China have approached international advanced technology, while some fields have achieved the international advanced level.

Abstract:Translational medicine emphasizes the application of the results of basic research to clinical practice and the solution of clinical issues through basic research. It is very important for the development direction of the medical field. Over the past decade, the translational medicine research in ocular surface disease has drawn international attention and developed rapidly. This field in China has made great progress, but overall there are still some gaps with the international advanced level. We need to establish the concept and the platform of translational research, explore scientific and effective methods, and create a whole chain to enhance our ocular research. It also requires research institutions and government departments to give full support. Finally, we need to explore a suitable way for the development of Chinese translational medical research.

Abstract:Glaucoma is a multifactorial optic degenerative neuropathy characterized by the loss of retinal ganglion cells. It is a combination of vascular, genetic, anatomical, and immune factors. Glaucoma poses a significant public health concern as it is the second leading cause of blindness after cataracts, and this blindness is usually irreversible. It is estimated that 57.5 million people worldwide are affected by primary open-angle glaucoma (POAG). People over 60 years of age, family members of those already diagnosed with glaucoma, steroid users, diabetics, as well as those with high myopia, hypertension, central cornea thickness of <5 mm, and eye injury are at an increased risk of glaucoma. By 2020, it is expected that approximately 76 million people will suffer from glaucoma with that number estimated to reach 111.8 million by 2040. In this article, we perform an extensive literature review focusing on the epidemiology of glaucoma and try to determine the number of people affected; we categorize them by sex, location, and level of income. Furthermore, we strive to estimate the future projection of the disease in the next 20 years (2040) while determining the disease burden, including the cost involved in treating and preventing the disease and the disease and disability projection of glaucoma.

Abstract:The prevalence of visual impairment (VI) and age-related eye diseases has increased dramatically with the growing aging population in mainland China. However, there is limited comprehensive evidence on the progress of ophthalmic epidemiological research in mainland China to enhance our awareness of the prevention of eye diseases to inform public health policy. Here, we conducted a literature review of the population-based epidemiology of VI and age-related eye diseases in mainland China from the 1st of January 1946 to the 20th of October 2021. No language restrictions were applied. There was significant demographic and geographic variation in the epidemic of VI and age-related eye diseases. There are several factors known to be correlated to VI and age-related eye diseases, including age, gender, family history, lifestyle, biological factors, and environmental exposures; however, evidence relating to genetic predisposition remains unclear. In addition, posterior segment eye diseases, including age-related macular degeneration and diabetic retinopathy, are amongst the major causes of irreversible visual impairments in the senile Chinese population. There remains a significant prevention gap, with only a few individuals showing awareness and achieving optimal medical care with regards to age-related eye diseases. Multiple challenges and obstacles need to be overcome, including the accelerated aging of the Chinese population, the lack of structured care delivery in many underdeveloped regions, and unequal access to care. Despite the progress to date, there are few well-conducted multi-center population-based studies following a single protocol in mainland China, which findings can hopefully provide valuable cues for governmental decision-making and assist in addressing and halting the incidence of VI and age-related eye diseases in China.

Epigenetics research in eye diseases: a bibliometric analysis from 2000 to 2023

Abstract:Clinical relevance: A bibliometric analysis is a quantitative study that utilises methods such as citation analysis to evaluate research performance. A bibliometric analysis could provide a valuable reference for ophthalmic researchers to understand the trends in epigenetics research. Background: The number of studies on epigenetics in eye diseases has exceeded 5,000, but the progress and scope of epigenetic research on eye diseases remain unclear. The study aimed to bibliometrically analyse epigenetic research conducted in eye diseases. Methods: Studies concerning epigenetic research on eye diseases from 2000-2023 were searched using the Web of Science Core Collection. Following this, the included studies were analysed for citations, journals, keywords, authors, and countries, using the Bibliometrix package in R Studio. Results: In total, 3758 studies were included in the analysis, including 3099 original articles, 599 reviews, 11 editorials, and 49 early access articles. Investigative Ophthalmology & Visual Science was the most published journal with 185 articles, and Proceedings of the National Academy of Sciences of the United States of America was the most cited journal, with 8727 citations. The journal with the highest h-index was Oncogene (h-index = 38).Renu A Kowluru from the Kresge Eye Institute, Wayne State University, Detroit, USA, had the most citations with 1,690 and the highest h-index (h-index = 23). China and the USA were the countries with the highest number of publications (1739) and total citations (40533), respectively. Furthermore, from 2000-2023, the top five frequent research topics were diabetic retinopathy, 522; microribonucleic acid, 469; retinoblastoma, 370; apoptosis, 268; and epigenetics, 206. Conclusions: The results of this bibliometric study provide the current status and trends of epigenetic research in eye diseases and will help researchers identify areas of current interest in the field, which should help highlight new research directions.

Abstract:Cytokines, known as "communication language between cells", are involved in the physiological and pathological processes of tissues and body organs. Investigating cytokines and eye diseases can not only reveal the molecular mechanism of their pathogenesis, but also monitor the indicators of cytokines, which can also be used to make the diagnosis, predict the outcome of the disease, clarify the timing of disease intervention, and help to formulate new targeted treatment strategies. The concept and research origin of cytokines about eye diseases such as diabetes ophthalmopathy, retinal vein occlusion, age-related macular degeneration, autoimmune uveitis, glaucomatous optic neuropathy, proliferative vitreoretino-pathy and retinopathy of prematurity were reviewed in this study.

Abstract:Introduction The prevalence of eye diseases has been increasing worldwide. In China, in addition to conventional medicine, Traditional Chinese Medicine (TCM) plays an important role in maintaining people's vision health. Although less flexible and targeted than TCM decoction, Chinese Patent Medicines (CPMs) are stable and well used. In recent years, CPMs have been increasingly used in ophthalmology clinics by TCM practitioners and by Western doctors in general hospitals. However, comprehensive evidence for using CPMs in ophthalmology is lacking. Aim We will apply the methodology of scoping review to systematically search and sort out the available evidence on oral CPMs for the treatment of eye diseases, identify the distribution of evidence in this field and provide a basis for clinical practice and medical decisions. Methods The scoping review will be implemented in the following seven steps: (1) defining the research question; (2) searching National Essential Medicines List (2018 edition ), National Basic Medical Insurance, Work Injury Insurance and Maternity Insurance Drug Catalog (2020 edition ) and Chinese Pharmacopoeia (2020 edition ) for oral CPMs for the treatment of eye diseases; (3) searching Embase, Web of Science, PubMed, Cochrane Library, Chongqing VIP Chinese Scientific Journals Database, Chinese National Knowledge Infrastructure, Chinese Biomedical Literature Database and Wanfang Database for relevant literature published from inception to December 2021; (4) developing eligibility criteria; (5) screening the studies based on inclusion criteria; (6) extracting relevant data and lastly, (7) collating, summarising and reporting the results. Ethics and dissemination Since the scoping review aims at collecting data from publicly available publications, this study does not require ethical approval. The results will be published in a peer-reviewed journal and presented at scientific conferences.

Abstract:Purpose The purpose of this study was to investigate the hotspots and research trends of ophthalmology research. Method Ophthalmology research literature published between 2017 and 2021 was obtained in the Web of Science Core Collection database. The bibliometric analysis and network visualization were performed with the VOSviewer and CiteSpace. Publication-related information, including publication volume, citation counts, countries, journals, keywords, subject categories, and publication time, was analyzed. Results A total of 10,469 included ophthalmology publications had been cited a total of 7,995 times during the past 5 years. The top countries and journals for the number of publications were the United States and the Ophthalmology. The top 25 global high-impact documents had been identified using the citation ranking. Keyword co-occurrence analysis showed that the hotspots in ophthalmology research were epidemiological characteristics and treatment modalities of ocular diseases, artificial intelligence and fundus imaging technology, COVID-19-related telemedicine, and screening and prevention of ocular diseases. Keyword burst analysis revealed that “neural network,” “pharmacokinetics,” “geographic atrophy,” “implementation,” “variability,” “adverse events,” “automated detection,” and “retinal images” were the research trends of research in the field of ophthalmology through 2021. The analysis of the subject categories demonstrated the close cooperation relationships that existed between different subject categories, and collaborations with non-ophthalmology-related subject categories were increasing over time in the field of ophthalmology research. Conclusions The hotspots in ophthalmology research were epidemiology, prevention, screening, and treatment of ocular diseases, as well as artificial intelligence and fundus imaging technology and telemedicine. Research trends in ophthalmology research were artificial intelligence, drug development, and fundus diseases. Knowledge from non-ophthalmology fields is likely to be more involved in ophthalmology research.

Abstract:The treatment of ocular fundus diseases is of significant issue in the clinic, but there exists large controversy on how to standardize the clinical treatment and how to evaluate the effectiveness of treatment on ocular fundus diseases. Emerging application of evidence-based medicine (EBM) provides us a rigorous and feasible pathway for the clinical treatment of ocular fundus diseases. We can improve the quality of clinical treatment research by exploring high quality randomized control trial (RCT) on the basis of patients with ocular fundus diseases in China; and by making full use of the best clinical evidences at home and abroad according to the EBM methods, which may further improve diagnosis and treatment of common ocular fundus diseases in China.

Abstract:Abundant progress has been made in the study of molecular biology in the last decade, which included the practical application of RNA interference, proteomic, molecular image and nano technology. The communication between bioscience with other branches (including ophthalmology) enhanced the further development of bioscience.
