Abstract:Cognitive Behavioral Therapy (CBT) is a well-established intervention for mitigating psychological issues by modifying maladaptive cognitive and behavioral patterns. However, delivery of CBT is often constrained by resource limitations and barriers to access. Advancements in artificial intelligence (AI) have provided technical support for the digital transformation of CBT. Particularly, the emergence of pre-training models (PTMs) and large language models (LLMs) holds immense potential to support, augment, optimize and automate CBT delivery. This paper reviews the literature on integrating AI into CBT interventions. We begin with an overview of CBT. Then, we introduce the integration of AI into CBT across various stages: pre-treatment, therapeutic process, and post-treatment. Next, we summarized the datasets relevant to some CBT-related tasks. Finally, we discuss the benefits and current limitations of applying AI to CBT. We suggest key areas for future research, highlighting the need for further exploration and validation of the long-term efficacy and clinical utility of AI-enhanced CBT. The transformative potential of AI in reshaping the practice of CBT heralds a new era of more accessible, efficient, and personalized mental health interventions.

Using Conversational AI to Facilitate Mental Health Assessments and Improve Clinical Efficiency Within Psychotherapy Services: Real-World Observational Study

Abstract:Background Most mental health care providers face the challenge of increased demand for psychotherapy in the absence of increased funding or staffing. To overcome this supply-demand imbalance, care providers must increase the efficiency of service delivery. Objective In this study, we examined whether artificial intelligence (AI)–enabled digital solutions can help mental health care practitioners to use their time more efficiently, and thus reduce strain on services and improve patient outcomes. Methods In this study, we focused on the use of an AI solution (Limbic Access) to support initial patient referral and clinical assessment within the UK’s National Health Service. Data were collected from 9 Talking Therapies services across England, comprising 64,862 patients. Results We showed that the use of this AI solution improves clinical efficiency by reducing the time clinicians spend on mental health assessments. Furthermore, we found improved outcomes for patients using the AI solution in several key metrics, such as reduced wait times, reduced dropout rates, improved allocation to appropriate treatment pathways, and, most importantly, improved recovery rates. When investigating the mechanism by which the AI solution achieved these improvements, we found that the provision of clinically relevant information ahead of clinical assessment was critical for these observed effects. Conclusions Our results emphasize the utility of using AI solutions to support the mental health workforce, further highlighting the potential of AI solutions to increase the efficiency of care delivery and improve clinical outcomes for patients.

AI-enabled conversational agent increases engagement with cognitive-behavioral therapy: A randomized controlled trial

Abstract:Timely support after referral to mental healthcare is crucial, yet patients often face prolonged wait times without intervention. Digital mental health interventions offer scalable solutions, but many struggle to achieve acceptable patient engagement. Tailoring and personalizing materials to individual needs is paramount for driving engagement, a task that generative artificial intelligence AI (genAI) is potentially able to achieve. To examine this promise, we conducted a randomized controlled trial using a genAI-enabled therapy app, Limbic Care, which delivers personalized cognitive behavioral therapy (CBT) materials, against PDF workbooks delivering static CBT content, as commonly used in standard care. Adults with elevated symptoms of anxiety or depression (N = 540) were randomly assigned to the app or control group for six weeks. The app group exhibited a threefold increase in engagement (2.4 times higher usage frequency, 3.8 times longer usage durations). While both groups showed similar overall symptom improvement, participants who engaged with the app's clinical personalization capabilities experienced significantly greater reductions in anxiety symptoms and enhanced well-being than those who engaged with the standard CBT materials. Importantly, the app was safe, with no increase in adverse events compared to standard care. Our findings suggest that genAI-enabled therapy apps can safely enhance patient engagement and improve clinical outcomes through clinically personalized interventions.

Abstract:Recent advances in AI and machine learning (ML) promise significant transformations in the future delivery of healthcare. Despite a surge in research and development, few works have moved beyond demonstrations of technical feasibility and algorithmic performance. However, to realize many of the ambitious visions for how AI can contribute to clinical impact requires the closer design and study of AI tools or interventions within specific health and care contexts. This article outlines our collaborative, human-centered approach to developing an AI application that predicts treatment outcomes for patients who are receiving human-supported, internet-delivered Cognitive Behavioral Therapy (iCBT) for symptoms of depression and anxiety. Intersecting the fields of HCI, AI, and healthcare, we describe how we addressed the specific challenges of (1) identifying clinically relevant AI applications ; and (2) designing AI applications for sensitive use contexts like mental health. Aiming to better assist the work practices of iCBT supporters, we share how learnings from an interview study with 15 iCBT supporters surfaced their practices and information needs and revealed new opportunities for the use of AI. Combined with insights from the clinical literature and technical feasibility constraints, this led to the development of two clinical outcome prediction models. To clarify their potential utility for use in practice, we conducted 13 design sessions with iCBT supporters that utilized interface mock-ups to concretize the AI output and derive additional design requirements. Our findings demonstrate how design choices can impact interpretations of the AI predictions as well as supporter motivation and sense of agency. We detail how this analysis and the design principles derived from it enabled the integration of the prediction models into a production interface. Reporting on identified risks of over-reliance on AI outputs and needs for balanced information assessment and preservation of a focus on individualized care, we discuss and reflect on what constitutes a responsible, human-centered approach to AI design in this healthcare context.

Abstract:Goal of the Evaluation Technique founded on artificial intelligence has the ability to drastically change the way mental health care is provided. This paragraph provides a brief history of artificial intelligence (AI) and its newer technologies in insurance, an analysis of notable actual studies on AI focused on personal wellbeing, and a consultation about how AI can bolster patient care while taking into account its present constraints, areas requiring further study, and wider dilemmas due to the advancements.remarkable discoveries. We looked at 28 studies on emotional stability which used medical records, mood self - report measures, neuroscience data, novel tracking systems (like smartphones and videos), and online communities to indicate, define, or clade mental health issues like ptsd, psychoses or other psychosocial problems, as well as attempts at suicide. [1] The majority of those investigations should then be viewed as early real evidence works illustrating the prospects that use machine learning algorithms to recognize psychiatric concerns and which types of algorithms yield the outstanding outcome, even though they collectively revealed high accuracies and deliver great case studies of possibilities in psychological therapies.

Abstract:Evidence is growing that several computer-aided psychotherapy (CP) systems can effectively improve a range of common mental health problems. Most clients find CP acceptable because of its convenience, confidentiality, and reduction of stigma. CP can be accessed in a clinic, but recently clients have used CP especially on the Internet at home, with brief support on a telephone helpline and/or by email. Brief and efficient screening and support greatly reduce attrition. CP's efficacy, and encouragement of its dissemination and implementation by some national funding bodies and governmental agencies, has led to its spread as a regular care option and is increasing access to psychological therapies in some countries. Transfer of this new approach from use in tight research studies to use as an integrated part of everyday care under widely varying conditions generates teething problems that are being managed in diverse ways across different centers. Anonymized Internet audit of CP outcomes facilitates effective care and clinical governance. This review examines the current state of the art as well as the science and broad applications of CP.

Administration and Policy in Mental Health and Mental Health Services Research

Abstract:Leonard Bickman's article on the future of artificial intelligence (AI) in psychotherapy research paints an encouraging picture of the progress to be made in this field. We support his perspective, but we also offer some cautionary notes about the boost AI can provide. We suggest that AI is not likely to transform psychotherapy research or practice to the degree seen in pharmacology and medicine because the factors that contribute to treatment response in these realms differ so markedly from one another, and in ways that do not favor advances in psychotherapy. Despite this limitation, it seems likely that AI will have a beneficial impact, improving empirical analysis through data-driven model development, tools for addressing the limitations of traditional regression methods, and novel means of personalizing treatment. In addition, AI has the potential to augment the reach of the researcher and therapist by expanding our ability to gather data and deliver interventions beyond the confines of the lab or clinical office.

Abstract:Background Students in need of mental health care face many barriers including cost, location, availability, and stigma. Studies show that computer-assisted therapy and 1 conversational chatbot delivering cognitive behavioral therapy (CBT) offer a less-intensive and more cost-effective alternative for treating depression and anxiety. Although CBT is one of the most effective treatment methods, applying an integrative approach has been linked to equally effective posttreatment improvement. Integrative psychological artificial intelligence (AI) offers a scalable solution as the demand for affordable, convenient, lasting, and secure support grows. Objective This study aimed to assess the feasibility and efficacy of using an integrative psychological AI, Tess, to reduce self-identified symptoms of depression and anxiety in college students. Methods In this randomized controlled trial, 75 participants were recruited from 15 universities across the United States. All participants completed Web-based surveys, including the Patient Health Questionnaire (PHQ-9), Generalized Anxiety Disorder Scale (GAD-7), and Positive and Negative Affect Scale (PANAS) at baseline and 2 to 4 weeks later (T2). The 2 test groups consisted of 50 participants in total and were randomized to receive unlimited access to Tess for either 2 weeks (n=24) or 4 weeks (n=26). The information-only control group participants (n=24) received an electronic link to the National Institute of Mental Health’s (NIMH) eBook on depression among college students and were only granted access to Tess after completion of the study. Results A sample of 74 participants completed this study with 0% attrition from the test group and less than 1% attrition from the control group (1/24). The average age of participants was 22.9 years, with 70% of participants being female (52/74), mostly Asian (37/74, 51%), and white (32/74, 41%). Group 1 received unlimited access to Tess, with daily check-ins for 2 weeks. Group 2 received unlimited access to Tess with biweekly check-ins for 4 weeks. The information-only control group was provided with an electronic link to the NIMH’s eBook. Multivariate analysis of covariance was conducted. We used an alpha level of .05 for all statistical tests. Results revealed a statistically significant difference between the control group and group 1, such that group 1 reported a significant reduction in symptoms of depression as measured by the PHQ-9 (P=.03), whereas those in the control group did not. A statistically significant difference was found between the control group and both test groups 1 and 2 for symptoms of anxiety as measured by the GAD-7. Group 1 (P=.045) and group 2 (P=.02) reported a significant reduction in symptoms of anxiety, whereas the control group did not. A statistically significant difference was found on the PANAS between the control group and group 1 (P=.03) and suggests that Tess did impact scores. Conclusions This study offers evidence that AI can serve as a cost-effective and accessible therapeutic agent. Although not designed to appropriate the role of a trained therapist, integrative psychological AI emerges as a feasible option for delivering support. Abstract:In this paper the application of computer technology and the use of the Internet in mental health care are critically reviewed. A number of on-line screening devices have been developed for anxiety disorders, mood disorders, and substance abuse disorders, with great potential for clinical practice. On line assessment is generally equivalent to clinical assessment. A number of studies have shown that self-help treatment programmes on stand-alone computers are as effective as routine clinical care. The Internet enhances the therapeutic possibilities of computers by offering feedback of therapists and more tailor-made treatment. A number of randomized clinical trials (RCTs) have shown that Internet-based treatment is more effective than no-treatment and as effective as face-to-face treatment. Research so far has been limited to anxiety disorders, burn-out, depression, headache, insomnia, tinnitus and obesity. Further, exposure through virtual reality has been found effective in a number of RCTs in specific phobias, but results with respect to the effects of the use of virtual reality techniques to other disorders are inconclusive. It is concluded that computer-driven assessment and treatment has many advantages and few disadvantages. A number of reasons are discussed which will preclude large-scale implementation of computer-driven assessment and therapy in the near future.

Abstract:The COVID-19 pandemic has intensified the need for mental health support across the whole spectrum of the population. Where global demand outweighs the supply of mental health services, established interventions such as cognitive behavioural therapy (CBT) have been adapted from traditional face-to-face interaction to technology-assisted formats. One such notable development is the emergence of Artificially Intelligent (AI) conversational agents for psychotherapy. Pre-pandemic, these adaptations had demonstrated some positive results; but they also generated debate due to a number of ethical and societal challenges. This article commences with a critical overview of both positive and negative aspects concerning the role of AI-CBT in its present form. Thereafter, an ethical framework is applied with reference to the themes of (1) beneficence, (2) non-maleficence, (3) autonomy, (4) justice, and (5) explicability. These themes are then discussed in terms of practical recommendations for future developments. Although automated versions of therapeutic support may be of appeal during times of global crises, ethical thinking should be at the core of AI-CBT design, in addition to guiding research, policy, and real-world implementation as the world considers post-COVID-19 society.

Abstract:Background Post‐traumatic stress disorder (PTSD) poses a global public health challenge. Evidence‐based psychotherapies (EBPs) for PTSD reduce symptoms and improve functioning (Forbes et al., Guilford Press, 2020, 3). However, a number of barriers to access and engagement with these interventions prevail. As a result, the use of EBPs in community settings remains disappointingly low (Charney et al., Psychological Trauma: Theory, Research, Practice, and Policy, 11, 2019, 793; Richards et al., Community Mental Health Journal, 53, 2017, 215), and not all patients who receive an EBP for PTSD benefit optimally (Asmundson et al., Cognitive Behaviour Therapy, 48, 2019, 1). Advancements in artificial intelligence (AI) have introduced new possibilities for increasinfg access to and quality of mental health interventions. Aims The present paper reviews key barriers to accessing and engaging in EBPs for PTSD, discusses current applications of AI in PTSD treatment and provides recommendations for future AI integrations aimed at reducing barriers to access and engagement. Discussion We propose that AI may be utilized to (1) assess treatment fidelity; (2) elucidate novel predictors of treatment dropout and outcomes; and (3) facilitate patient engagement with the tasks of therapy, including therapy practice. Potential avenues for technological advancements are also considered.

Abstract:Introduction: There is growing evidence that computer-delivered or computer-assisted forms of cognitive behavior therapy (CCBT) are helpful, but cost-effectiveness versus standard therapies is not well established. Objective: To evaluate the cost-effectiveness of a therapist-supported method for CCBT in comparison to standard cognitive behavior therapy (CBT). Methods: A total of 154 drug-free major depressive disorder outpatients were randomly assigned to either 16 weeks of standard CBT (up to twenty 50-min sessions) or CCBT using the Good Days Ahead program (including up to 5.5 h of therapist contact). Outcomes were assessed at baseline, weeks 8 and 16, and at 3 and 6 months post-treatment. Economic analyses took into account the costs of services received and work/social role impairment. Results: In the context of almost identical efficacy, a form of CCBT that used only about one third the amount of therapist contact as conventional CBT was highly cost-effective compared to conventional therapy and reduced the adjusted cost of treatment by USD 945 per patient. Conclusions: A method of CCBT that blended internet-delivered modules and abbreviated therapeutic contact reduced the cost of treatment substantially without adversely affecting outcomes. Results suggest that use of this approach can more than double the access to CBT. Because clinician support in CCBT can be provided by telephone, videoconference, and/or email, this highly efficient form of treatment could be a major advance in remote treatment delivery. Abstract:The digital revolution has made integrating artificial intelligence (AI) into the medical sector easier. AI is currently being used to speed up the detection of diseases in their early stages, facilitate the delineation and understanding of disease variations, and improve optimised treatment protocols. Psychology is no exception, as new role players have emerged, most notably the incorporation of AI into both psychological research and clinical practice. However, there is an apparent unease with AI as a technological breakthrough advancing science in mental health, such as data privacy, national guidelines on the use of AI, successful integration of users into the clinical setting and empathy, compared to human psychologists. Regardless, AI functions in psychology are gradually snowballing. We present an opinion on the impact of AI on improving mental health based on the exploration of available published evidence. We highlight the potential of AI in improving mental health care through mental health disorder detection, diagnosis, treatment, and public health education. The article also discusses the potential challenges and future directions, highlighting the importance of interdisciplinary collaboration and developing novel AI-based approaches to improve mental health outcomes.

Abstract:Introduction The advent of artificial intelligence (AI) and machine learning has sparked interest in its applicability in the mental health domain, offering potential improvements in the efficiency and personalization of psychiatric services. Objectives To characterize the methodological and technical approaches in studies utilizing machine learning and natural language processing (NLP) within mental health, to evaluate their potential and impact in psychiatric clinical practice, and to address the associated ethical concerns. Methods A systematic review, adhering to the PRISMA guidelines, was conducted across four primary medical databases. Emphasis was placed on studies that applied machine learning and NLP techniques to psychiatric contexts, extracting data from sources such as medical records and social media. Results From 327 identified articles, 58 were considered relevant. Major themes included symptom extraction, illness severity classification, therapy effectiveness comparison, and psychopathological insight derivation. Notably, most studies focused on specific populations like social media users, emergency room attendees, or those within medical databases. Methodological findings showcased a preference for efficient classifiers and Python as the primary platform. Conclusions Machine learning and NLP offer a promising new avenue for psychiatric research and clinical practice, enabling the extraction of previously inaccessible patient information and supporting the decision-making process. However, the field must address inherent limitations, ethical considerations, and ensure that the tools augment, rather than replace, clinical judgment. Disclosure of Interest None Declared

With nearly one in five adults facing mental health issues and suicide ranking as a leading cause of death among the young, the strained mental health system seeks innovative solutions. The text discusses the rapid evolution of AI, particularly in image analysis for early physical health diagnoses, and its promising applications in mental health, including predictive analytics for various disorders. AI's ability to analyze written language, speech characteristics, and physiological signals from wearables offers avenues for remote monitoring and early prognosis. Despite the need to address ethical considerations, particularly biases in data sets and concerns about potential patient detachment, the article advocates for AI as a complementary tool rather than a replacement for human involvement in mental health services. Overall, the article emphasizes the transformative potential of AI in enhancing diagnostics, monitoring, and treatment strategies for mental health disorders.

Abstract:The utilization of artificial intelligence (AI) in psychiatry has risen over the past several years to meet the growing need for improved access to mental health solutions. Additionally, shortages of mental health providers during the COVID-19 pandemic have continued to exacerbate the burden of mental illness worldwide. AI applications already in existence include those enabled to assist with psychiatric diagnoses, symptom tracking, disease course prediction, and psychoeducation. Modalities of AI mental health care delivery include availability through the internet, smartphone applications, and digital gaming. Here we review emerging AI-based interventions in the form of chat and therapy bots, specifically conversational applications that teach the user emotional coping mechanisms and provide support for people with communication difficulties, computer generated images of faces that form the basis of avatar therapy, and intelligent animal-like robots with new advances in digital psychiatry. We discuss the implications of incorporating AI chatbots into clinical practice and offer perspectives on how these AI-based interventions will further impact the field of psychiatry.

The therapeutic effectiveness of artificial intelligence-based chatbots in alleviation of depressive and anxiety symptoms in short-course treatments: A systematic review and meta-analysis

Abstract:Background The emergence of artificial intelligence-based chatbot has revolutionized the field of clinical psychology and psychotherapy, granting individuals unprecedented access to professional assistance, overcoming time constraints and geographical limitations with cost-effective convenience. However, despite its potential, there has been a noticeable gap in the literature regarding their effectiveness in addressing common mental health issues like depression and anxiety. This meta-analysis aims to evaluate the efficacy of AI-based chatbots in treating these conditions. Methods A systematic search was executed across multiple databases, including PubMed, Cochrane Library, Web of Science, PsycINFO, and Embase on April 4th, 2024. The effect size of treatment efficacy was calculated using the standardized mean difference (Hedge's g ). Quality assessment measures were implemented to ensure trial's quality. Results In our analysis of 18 randomized controlled trials involving 3477 participants, we observed noteworthy improvements in depression (g = −0.26, 95 % CI = −0.34, −0.17) and anxiety (g = −0.19, 95 % CI = −0.29, −0.09) symptoms. The most significant benefits were evident after 8 weeks of treatment. However, at the three-month follow-up, no substantial effects were detected for either condition. Limitations Several limitations should be considered. These include the lack of diversity in the study populations, variations in chatbot design, and the use of different psychotherapeutic approaches. These factors may limit the generalizability of our findings. Conclusion This meta-analysis highlights the promising role of AI-based chatbot interventions in alleviating depressive and anxiety symptoms among adults. Our results indicate that these interventions can yield substantial improvements over a relatively brief treatment period.

Personalized Cognitive Health in Psychiatry: Current State and the Promise of Computational Methods

Abstract:Abstract Background Decades of research have firmly established that cognitive health and cognitive treatment services are a key need for people living with psychosis. However, many current clinical programs do not address this need, despite the essential role that an individual’s cognitive and social cognitive capacities play in determining their real-world functioning. Preliminary practice-based research in the Early Psychosis Intervention Network early psychosis intervention network shows that it is possible to develop and implement tools that delineate an individuals’ cognitive health profile and that help engage the client and the clinician in shared decision-making and treatment planning that includes cognitive treatments. These findings signify a promising shift toward personalized cognitive health. Study Design Extending upon this early progress, we review the concept of interindividual variability in cognitive domains/processes in psychosis as the basis for offering personalized treatment plans. We present evidence from studies that have used traditional neuropsychological measures as well as findings from emerging computational studies that leverage trial-by-trial behavior data to illuminate the different latent strategies that individuals employ. Study Result We posit that these computational techniques, when combined with traditional cognitive assessments, can enrich our understanding of individual differences in treatment needs, which in turn can guide evermore personalized interventions. Conclusion As we find clinically relevant ways to decompose maladaptive behaviors into separate latent cognitive elements captured by model parameters, the ultimate goal is to develop and implement approaches that empower clients and their clinical providers to leverage individual’s existing learning capacities to improve their cognitive health and well-being.
