Abstract:Objective: To develop a novel endoscopic system that reduces trauma to the patient and declutters the surgical field for the surgeon in minimally invasive heart valve surgery. Methods: We designed and developed a retractor-camera combination for minimally invasive heart surgery; the cable and camera were connected to the underbelly of the left atrial retractor blade to provide an illuminated, wide-angle view of the mitral valve. We conducted ex vivo, in vivo, and, ultimately, a first-in-man randomized, nonanonymized clinical trial on 20 patients who required minimally invasive mitral valve surgery. Data from the preoperative period and the immediate postoperative period were gathered, and patients were followed for 3 months. Results: Our camera-retractor combo demonstrated safe and efficient exposure and vision in all detailed studies. The total operation time for the intervention group (316.5 ± 65.1 min) was slightly shorter than for the control group (317.5 ± 50.8 min). There was no significant difference in the ability to provide exposure of the mitral valve and surrounding left atrial tissue for both control and test articles. The novel camera-retractor reduced clutter in the operation field substantially and eliminated chest penetration for the camera. We also observed that the camera did not fog, did not stain with blood, and did not require frequent corrections to its position. Conclusions: We launch a novel atrial retraction-imaging platform that is less invasive for the patient and eliminates clutter and redundant movements for the surgeon.

Abstract:Endoscope provides visualization and access to internal organs, regarded as a crucial instrument usually held by surgical assistants in minimally invasive laparoscopic surgery. However, handheld endoscopy poses challenges like operator fatigue, instability, and limited dexterity, potentially compromising procedure accuracy and efficiency. Meanwhile, rigid straight endoscopes lack flexibility, limiting their effectiveness in navigating complex anatomical structures and tight spaces during minimally invasive surgeries (MIS). To provide surgeons a stable and flexible endoscope view and enhance the efficiency of the endoscopy procedures, we present a linkage couples based endoscope-holder robot. A linkage mechanism based on linkage couples was designed as the bendable structure to steer the endoscope. The bending range was enlarged by accumulating linkage couple units. An actuation package mounted on a robotic arm, providing two degrees of freedom (DoFs) for the movement of the endoscopic device, enabling both bending and rotation of the endoscope. The bending range and the resistance to deformation ability were validated on the 3D-print robot prototype, reporting large bending range exceeding -180 deg to 160 deg and 100 g workload. Model experiments demonstrated the ability of the endoscope-holder robot to bypass obstacles and search targets in a constrained space. The advent of bendable endoscope-holder robots is expected to provide a more stable and flexible surgical view for minimally invasive procedures, thereby facilitating efficient and effective surgeries.

Abstract:Minimal invasion is an important trend in surgery. However, the endoscope, as one of the key devices for monitoring the process of minimally invasive surgery, is limited by its size and working space it operates in, which result in a considerably narrow field of view. In particular, when a surgical instrument enters through the tool channel, the instrument occupies most of the area in an endoscopic image. This hampers the surgeon’s field of view and has a negative impact on the surgery. This study proposes a novel method for removing the occlusion caused by surgical instruments in endoscopic images by making foreground occlusions on endoscopic images transparent using image restoration and interframe information filling. Compared with unprocessed images, this method can provide a clearer field of view that is necessary for minimally invasive endoscopic surgeries and improve the quality of surgeries. Clinical endoscopic images are used to verify the feasibility of the proposed method, and the results show that the proposed method improves the visual effect of endoscopic images by removing surgical-instrument occlusions. This demonstrates the considerable potential of the proposed method for use in clinical applications.

Abstract:Nowadays, gastrointestinal cancer has widely impacted people's health worldwide due to its high mortality rate. Early treatment of gastrointestinal cancer by endoscopic procedure can greatly increase survival rates of patients. Nevertheless, current flexible endoscopic instruments lack of degree of freedom and surgical triangulation, thereby presenting challenges in the execution of endoscopic procedures. To solve these problems, this letter proposes a novel miniature dual-bending flexible instrument with an outer diameter of 2.8 mm and a length of 1100 mm, which is compatible with commercial endoscope and various endoscopic platforms. Discrete frame stacking structure with unfolding and decoupling design empowers the instrument to achieve high flexibility, accurate bending motions and surgical triangulation. The flexible instrument can achieve a large hemi-spherical workspace with a radius of 24 mm without considering linear movement. The measured tip positioning accuracy is better than 0.59 mm for wrist bending motion. Combined with the various end effectors, the instrument can perform complex endoscopic procedures facilitated by the leader-follower control method. Through in-vivo animal experiments, the clinical potential and practicality of the instrument have been demonstrated.

Abstract:We developed a nonmagnetic rigid and flexible outer sheath with pneumatic interlocking mechanism using flexible toothed links and a wire-driven bending distal end. The outer sheath can be switched between rigid and flexible modes easily depending on surgical scenes, and the angle of its distal end can be controlled by three nylon wires. All components of flexible parts are made of MRI-compatible nonmagnetic plastics. We manufactured the device with 300-mm long, 16-mm outer diameter, 7-mm inner diameter and 90-mm bending distal end. Holding power of the device in rigid mode was maximum 3.6 N, which was sufficient for surgical tasks in body cavity. In vivo experiment using a swine, our device performed smooth insertion of a flexible endoscope and a biopsy forceps into reverse side of the liver, intestines and spleen with a curved path. In conclusion, our device shows availability of secure approach of surgical instruments into deep cavity.
s-CAM: An Untethered Insertable Laparoscopic Surgical Camera Robot with Non-Contact Actuation

Abstract:Fully insertable robotic imaging devices represent a promising future of minimally invasive laparoscopic vision. Emerging research efforts in this field have resulted in several proof-of-concept prototypes. One common drawback of these designs derives from their clumsy tethering wires which not only cause operational interference but also reduce camera mobility. In this paper, a tetherless insertable surgical camera (s-CAM) robot with non-contact transabdominal actuation is presented for single-incision laparoscopic vision. Wireless video transmission and control communication using onboard power help eliminate cumbersome tethering wires. Furthermore, magnetic based camera actuation gets rid of intrinsic physical constraints of mechanical driving mechanisms, thereby improving camera mobility and reducing operational interference. In addition, a custom Bluetooth low energy (BLE) application profile and a real-time operating system (RTOS) based multitask programming framework are also proposed to facilitate embedded software design for insertable medical devices. Initial ex vivo test results of the s-CAM design have demonstrated technical feasibility of a tetherless insertable laparoscopic camera. Effective imaging is confirmed at as low as 500 lx illumination. Wireless laparoscopic vision is accessible within a distance of more than 10 m. Transabdominal BLE communication is stable at over -52 dBm and shows its potential for wireless control of insertable medical devices. RTOS based sfotware event response is bounded within 1 ms while the CPU usage is at 3∼5%. The device is able to work for 50 min with its onboard power. For the mobility, the robot can translate against the interior abdominal wall to reach full abdomen quadrants, tilt between -180∘ and +180∘, and pan in the range of 0∘∼360∘. The s-CAM has brought robotic laparoscopic imaging one step further toward less invasiveness and more dexterity.

Abstract:Minimally Invasive Surgery (MIS) can reduce iatrogenic injury and decrease the possibility of surgical complications. This paper presents a novel flexible and bendable endoscopic device, “Roboscope”, which delivers two instruments, two miniature scanning fiber endoscopes, and a suction/irrigation port to the operation site through a single portal. Compared with existing bendable and steerable robotic surgical systems, Roboscope provides two bending degrees of freedom for its outer sheath and two insertion degrees of freedom, while simultaneously delivering two instruments and two endoscopes to the surgical site. Each bending axis and insertion freedom of Roboscope is independently controllable via an external actuation pack. Surgical tools can be changed without retracting the robot arm. This paper presents the design of the Roboscope mechanical system, electrical system, and control and software systems, design requirements and prototyping validation as well as analysis of Roboscope workspece.

Abstract:Developing advanced surgical tools for minimally invasive procedures represents an activity of central importance to improving human health. A key challenge is in establishing biocompatible interfaces between the classes of semiconductor device and sensor technologies that might be most useful in this context and the soft, curvilinear surfaces of the body. This paper describes a solution based on materials that integrate directly with the thin elastic membranes of otherwise conventional balloon catheters, to provide diverse, multimodal functionality suitable for clinical use. As examples, we present sensors for measuring temperature, flow, tactile, optical and electrophysiological data, together with radiofrequency electrodes for controlled, local ablation of tissue. Use of such 'instrumented' balloon catheters in live animal models illustrates their operation, as well as their specific utility in cardiac ablation therapy. The same concepts can be applied to other substrates of interest, such as surgical gloves.

Abstract:There has been a longstanding expectation that the optical resolution embodiment of photoacoustic tomography could have a substantial impact on gastrointestinal endoscopy by enabling microscopic visualization of the vasculature based on the endogenous contrast mechanism. Although multiple studies have demonstrated the in vivo imaging capability of a developed imaging device over the last decade, the implementation of such an endoscopic system that can be applied immediately when necessary via the instrument channel of a video endoscope has been a challenge. In this study, we developed a 3.38 mm diameter catheter based, integrated optical resolution photoacoustic and ultrasonic miniprobe system and successfully demonstrated its intrainstrument channel workability for the standard 3.7 mm diameter instrument channel of a clinical video endoscope based on a swine model. Through the instrument channel, we acquired the first in vivo photoacoustic and ultrasonic dual mode endoscopic images from the esophagogastric junction, which is one of the most frequently cited anatomical sites in the gastrointestinal tract in relation to Barretts esophagus. Moreover, from a rat colorectum in vivo imaging experiment, we visualized hierarchically developed mesh like capillary networks with a hole size as small as 50 microns, which suggests the potential level of image details that could be photoacoustically provided in clinical settings in the future.
Surgical robot for single-incision laparoscopic surgery

Abstract:This paper introduces a novel surgical robot for single-incision laparoscopic surgeries. The robot system includes the cone-type remote center-of-motion (RCM) mechanism and two articulated instruments having a flexible linkage-driven elbow. The RCM mechanism, which has two revolute joints and one prismatic joint, is designed to maintain a stationary point at the apex of the cone shape. By placing the stationary point on the incision area, the mechanism allows a surgical instrument to explore the abdominal area through a small incision point. The instruments have six articulated joints, including an elbow pitch joint, which make the triangulation position for the surgery possible inside of the abdominal area. The presented elbow pitch structure is similar to the slider-crank mechanism but the connecting rod is composed of a flexible leaf spring for high payload and small looseness error. We verified the payload of the robot is more than 10 N and described preliminary experiments on peg transfer and suture motion by using the proposed surgical robot.

Abstract:Minimally invasive surgeries (MISs) or natural orifice transluminal endoscopic surgeries (NOTESs), such as the transurethral resection of bladder tumor (TURBT), require the surgical robot to be miniaturized to perform surgical procedures in narrow spaces. However, miniaturization of surgical robots poses problems in its fabrication and sensor integration. In this article, a miniature continuum surgical robot is proposed with force-sensing surgical forceps, which can be fabricated through a 2-D lamination process and converted into 3-D through folding. This multimaterial laminated structure facilitates the integration of photoelectric sensors into the surgical forceps. Then, the gripping force of the surgical forceps is sensed from the change of the output voltage of the integrated photoelectric sensor. The sensitivity of gripping force to the change of output voltage is 3.6 N/V, and the maximum error during calibration is 0.03 N. The proposed robot and the gripping force sensing method are validated through simulations and experiments, such as gripping and releasing objects of various hardness and heights. It is also validated that the gripping force information could also improve the gripping performance in the object handling experiment and ex vivo porcine intestine gripping experiment.

Abstract:In minimal invasive surgery, it is important to rebuild and visualize the latest deformed shape of soft-tissue surfaces to mitigate tissue damages. This paper proposes an innovative Simultaneous Localization and Mapping (SLAM) algorithm for deformable dense reconstruction of surfaces using a sequence of images from a stereoscope. We introduce a warping field based on the Embedded Deformation (ED) nodes with 3D shapes recovered from consecutive pairs of stereo images. The warping field is estimated by deforming the last updated model to the current live model. Our SLAM system can: (1) Incrementally build a live model by progressively fusing new observations with vivid accurate texture. (2) Estimate the deformed shape of unobserved region with the principle As-Rigid-As-Possible. (3) Show the consecutive shape of models. (4) Estimate the current relative pose between the soft-tissue and the scope. In-vivo experiments with publicly available datasets demonstrate that the 3D models can be incrementally built for different soft-tissues with different deformations from sequences of stereo images obtained by laparoscopes. Results show the potential clinical application of our SLAM system for providing surgeon useful shape and texture information in minimal invasive surgery.

Abstract:Objective: The development of surgical microscope-associated cameras has given rise to a new operating style embodied by hybrid microsurgical and exoscopic operative systems. These platforms utilize specialized camera systems to visualize cranial neuroanatomy at various depths. Our study aims to understand how different camera settings in a novel hybrid exoscope system influence image quality in the context of neurosurgical procedures. Methods: We built an image database using captured cadaveric dissection images obtained with a prototype version of a hybrid (microsurgical/exoscopic) operative platform. We performed comprehensive 4K-resolution image capture using 76 camera settings across three magnification levels and two working distances. Computer algorithms such as structural similarity (SSIM) and mean squared error (MSE) were used to measure image distortion across different camera settings. We utilized a Laplacian filter to compute the overall sharpness of the acquired images. Additionally, a monocular depth estimation deep learning model was used to examine the image's capability to visualize the depth of deeper structures accurately. Results: A total of 1,368 high-resolution pictures were captured. The SSIM index ranged from 0.63 to 0.85. The MSE was nearly zero for all image batches. It was determined that the exoscope could accurately detect both the sharpness and depth based on the Laplacian filter and depth maps, respectively. Our findings demonstrate that users can utilize the full range of camera settings available on the exoscope, including adjustments to aperture, color saturation, contrast, sharpness, and brilliance, without introducing significant image distortions relative to the standard mode. Conclusion: The evolution of the camera incorporated into a surgical microscope enables exoscopic visualization during cranial base surgery. Our result should encourage surgeons to take full advantage of the exoscope's extensive range of camera settings to match their personal preferences or specific clinical requirements of the surgical scenario. This places the exoscope as an invaluable asset in contemporary surgical practice, merging high-definition imaging with ergonomic design and adaptable operability.
Stereoscopic head-mounted display incorporated into microsurgical procedures: technical note

Abstract:Objective: We have previously evaluated video technologies that have allowed for the use of electronic imaging during microneurosurgical and endoscopic procedures. This stereoscopic camera set and monitor/recording system allows for the use of stereoscopic images during surgery by the primary surgical assistant and ancillary personnel. It also allows for stereoscopic recording and playback using a wide video home system format. We describe a novel prototype of a head-mounted display (HMD) that allows the surgical team to simultaneously visualize the surgical field stereoscopically and includes picture-in-picture, voice control, and stereoscopic recording capabilities. Instrumentation: A stereoscopic HMD with 640 x 480 (video graphics array) pixel resolution and the ability to display 24-bit images has been designed. This device weighs 900 g. It is interfaceable with common video display formats. Results: This stereoscopic HMD is being evaluated in a prospective multicenter trial of open microsurgical and endoscopic minimally invasive procedures. In our experience to date, there have been no equipment failures or complications attributable to the use of the display system. The equipment was well accepted by users who reported significant benefits in visualization. Conclusion: The use of a stereoscopic HMD may result in improved efficiency and safety in both endoscopic and open microsurgical procedures. We have verified that the HMD is comfortable during the course of a surgical procedure, is reliable, and allows for accessibility to the operative field with an excellent field of view and three-dimensional perception. Positioning and dexterity within the operative field are also enhanced. Additional uses relate to surgical training, multimodal information display, and operative rehearsals.
Remote Monitoring System for Condition Management of Patients in Free Flap Surgery

Abstract:Introduction: Free flap surgery is inconvenient because an attending physician must observe a patient's condition every day to ensure that normal tissue is restored within 72 h after the surgery. To address this problem, this paper proposes a remote monitoring technology to observe a patient's condition in real time. Methods: To design a monitoring system, the camera consists of MCU board, DC-DC converter, alarm performance, Wi-fil module, and server, and the camera and MCU part is connected to the server through the wi-fi network. A camera obtains the images of the surgical site once every 2 s, and the images are transmitted to the attending physician or nurse via Wi-Fi communication. The working distance between camera and surgical site is 56 cm, and the viewing angle of a camera is 60° (radius). Results: A video shooting test is also performed, in which the images are obtained once per hour between 17:00 and 08:00 the next day; the results show that high-quality images are obtained in the video shooting test. The imaging error is zero (0 GB) in the video shooting test results. Discussion and conclusion: The imaging of the surgical site can be obtained by camera system, and the proposed method is that there no storage error occurs during the shooting process. In addition, the shooting performance has high velocity. It is possible to control the WD according to a patient's body via a holding manipulator used for the camera. The new method is expected to be used for remote patient management, for a wide range of procedures, in the medical field.

Abstract:In the ever advancing field of minimally invasive surgery, flexible instruments with local degrees of freedom are needed to navigate through the intricate topologies of the human body. Although cable or concentric tube driven solutions have proven their merits in this field, they are inadequate for realizing small bending radii and suffer from friction, which is detrimental when automation is envisioned. This paper reports on the characterization of a chip-on-tip endoscope, consisting out of a soft robotic pneumatic bending microactuator equipped with a 1.1 × 1.1 mm2 CMOS camera. As such, the total diameter of the endoscope measures 1.66 mm. To show the feasibility of using this system in a surgical environment, a preliminary test on an eye mock-up is conducted.

Abstract:Spinal endoscopy has evolved significantly since its inception, offering minimally invasive solutions for various spinal pathologies. This study introduces a promising innovation in spinal endoscopy-a single-use digital endoscope designed to overcome the drawbacks of traditional optic endoscopes. Traditional endoscopes, despite their utility, present challenges such as fragility, complex disinfection processes, weight issues, and susceptibility to mechanical malfunctions. The digital endoscope, with its disposable nature, lighter weight, and improved image quality, aims to enhance surgical procedures and patient safety. The digital endoscope system comprises a 30-degree 1000 × 1000 pixel resolution camera sensor with a 4.3 mm working channel, and LED light sources replacing optical fibers. The all-in-one touch screen tablet serves as the host computer, providing portability and simplified operation. Image comparisons between the digital and optic endoscopes revealed advantages in the form of increased field of view, lesser distortion, greater close-range resolution, and enhanced luminance. The single-use digital endoscope demonstrates great potential for revolutionizing spine endoscopic surgeries, offering convenience, safety, and superior imaging capabilities compared to traditional optic endoscopes.
Micro laser ablation system integrated with image sensor for minimally invasive surgery

Abstract:A novel micro surgical system with a micro laser ablation module integrated with an imaging sensor is designed and developed for minimally invasive surgery. The system consists of a Nd:YAG laser source with the wavelength of 1064nm to remove lesion, a CCD camera to guide laser, a parallel four-bar mechanism, an endoscope bending section, a rigid steel tube, an aluminum-alloy supporting table and a personal computer for sending control commands. The advantage of the system is the small size of its distal module, i.e., the diameter and the length of the module are 3.5 millimeters and 15.0 millimeters, respectively. A methodological frame for ablating lesion using the system proposed is developed based on the system configuration. Experiments on ex-vivo swine tissue are implemented to evaluate laser ablation performance of the prototyped system.
