Abstract:Given the lack of valid and reliable instruments for evaluating the quality of communication between physicians and cancer patients and their family caregivers in China, this study translated and culturally adapted the Quality of Communication questionnaires for cancer patients (QOC-P) and their family caregivers (QOC-F) for use in the Chinese context and evaluated their psychometric properties.
What problem does this paper attempt to address?
The problem that this paper attempts to solve is that China lacks effective assessment tools to measure the communication quality between doctors and cancer patients and their family caregivers. Specifically, the study aims to translate and culturally adapt the Quality of Communication questionnaires (QOC) into a Chinese version and evaluate its psychometric properties in the Chinese context. These questionnaires include QOC - P for patients and QOC - F for family caregivers.
### Research Background
- **High Incidence of Cancer and Complex Treatment Options**: According to data from the International Agency for Research on Cancer, in 2020, there were approximately 19.3 million new cancer cases globally, and nearly 10 million deaths. It is expected that by 2040, the global cancer burden will increase by 47% to 28.4 million cases.
- **Importance of Doctor - Patient Communication**: High - quality doctor - patient communication helps in better decision - making, improves medical compliance and patients' emotional well - being, strengthens the doctor - patient relationship, and thus increases patients' satisfaction with care.
- **Current Situation of Doctor - Patient Communication in China**: The quality of doctor - patient communication in China is not satisfactory. Medical violence incidents occur frequently, and there is a shortage of doctors, resulting in limited doctor - patient communication time. In addition, hospice care has a low popularity in China, and there is insufficient communication between doctors and patients regarding hospice care.
### Research Objectives
- **Translation and Cultural Adaptation**: Translate QOC - P and QOC - F into Chinese and carry out cultural adaptation to make them suitable for the Chinese context.
- **Psychometric Evaluation**: Evaluate the content validity, construct validity, convergent validity and reliability of the Chinese - version QOC - P and QOC - F.
### Methods
- **Translation and Cultural Adaptation Process**: Adopt Brislin's translation model and ensure the accuracy of translation and cultural adaptability through steps such as double - blind translation, synthesis, back - translation and expert review.
- **Sample Selection**: Use the convenience sampling method to recruit 228 pairs of cancer patients and their family caregivers from six tertiary hospitals in Hunan.
- **Data Collection**: Collect general demographic information and clinical data of participants and conduct questionnaire surveys using the Chinese - version QOC - P and QOC - F.
- **Data Analysis**: Use SPSS 26.0 and AMOS 24.0 for data processing, including normality tests, descriptive statistics, exploratory factor analysis (EFA), confirmatory factor analysis (CFA), Pearson correlation analysis and reliability analysis, etc.
### Results
- **Factor Analysis**: Through exploratory factor analysis, QOC - P and QOC - F are divided into two dimensions: general communication and end - of - life communication.
- **Reliability**: The Cronbach's α coefficients range from 0.905 to 0.953, indicating good internal consistency. The intra - class correlation coefficients of the two - week test - retest reliability are 0.993 and 0.991 respectively, indicating good stability.
- **Validity**: The content validity index (QOC - P: 0.857, QOC - F: 1.0) and split - half reliability (QOC - P: 0.833, QOC - F: 0.935) are both satisfactory. QOC - P and QOC - F are negatively correlated with anxiety and depression, negatively correlated with decision - making regret, and positively correlated with shared decision - making.
### Conclusions
- **Main Conclusions**: The Chinese - version QOC - P and QOC - F have good psychometric properties in the Chinese context and are suitable for evaluating the communication quality between doctors and cancer patients and their family caregivers.
- **Suggestions for Future Research**: Future research should use more diverse and inclusive samples and further test the structure of the Chinese - version QOC - P and QOC - F through confirmatory factor analysis.
