Abstract:Background: Volume replacement is one of the vital techniques of oncoplastic surgery (OPS) when applying breast-conserving surgery. The clinical application of peri-mammary artery perforator flaps for this indication is uneven in China. Here, we describe the results of our clinical experience with peri-mammary artery flaps for partial breast reconstruction. Methods: In this study, 30 patients underwent partial breast resection for quadrant breast cancer followed by partial breast reconstruction with peri-mammary artery perforator flaps, which included the thoracodorsal artery perforator flap (TDAP), anterior intercostal artery perforator flap (AICAP), lateral intercostal artery perforator flap (LICAP), and lateral thoracic artery perforator flap (LTAP). All the patients' operation plans were discussed comprehensively and were performed by sticking to every step. The satisfaction outcome was assessed with the extracted version of the BREAST-Q version 2.0, Breast Conserving Therapy Module Preoperative and Postoperative Scales both preoperatively and postoperatively. Results: According to the study outcomes, the mean flap size was 5.3*4.2*2.8 cm (3.0-7.0*3.0-5.0*1.0-3.5 cm). The mean surgical time was 142 min (100-250 min). No partial flap failure was detected, and no severe complications were observed. Most patients were satisfied with the outcomes regarding the dressing, sexual life, and breast shape postoperation. Furthermore, the sensation of the surgical area, scar satisfaction, and recovery state gradually improved. Overall, LICAP and AICAP had higher scores when different flaps were compared. Conclusions: Based on this study, we found that peri-mammary artery flaps had significant value in breast-conserving surgery, especially in patients with small or medium-sized breasts. Perforators could be detected by vascular ultrasound before the operation. More than one perforator could be found most of the time. No severe complications occurred when performing a suitable plan, including discussing and recording the operation procedure; the focus of care, the choice for precise and proper perforators, and the mechanism for hiding the scars were all considered and recorded in a specific chart. Patients were satisfied with the reconstruction technique of peri-mammary artery perforator flaps after breast-conserving, and the satisfaction of AICAP and LICAP was higher. In general, this technique is suitable for partial breast reconstruction and has no negative impact on patient satisfaction.

Abstract:Objective:To explore the feasibility and cosmetic effect of repairing partial breast defects with pedicled lateral thoracic artery perforator(LTAP) flap.Methods:Pedicled LTAP flap was applied to repair partial breast defects in 12 patients from the Department of Breast Surgery of Shenzhen Maternal and Child Health Hospital of Southern Medical University from May 2019 to October 2020. Before the operation, the Doppler ultrasound was used to explore the perforator branch of the lateral thoracic artery in the 3rd to 4th intercostal area of the lateral thoracic wall, and a transverse or nearly transverse pedicled LTAP flap containing the perforator branch was designed. During the operation, the flap was elevated from the deep surface of the deep fascia, through which the lateral thoracic vessels were faintly visible. Then the vascular direction was determined with an audible Doppler blood flow detector. From the deep fat layer and the deep surface of the deep fascia, the pedicle with a width of about 1.2 cm and a thickness of about 0.5 cm was formed by careful dissection along the main vessel to the axillary direction. The flap was transferred to the breast defect after the epidermis of the flap was removed. The survival of the flaps was observed, and the reconstructed breast shape was evaluated.Results:A total of 12 patients were included. All patients were female, aged from 32 to 58 years old, with an average of 41.7 years. Among them, there were 8 cases of breast cancer and 4 cases of non-lactation mastitis. The lesions were all located in the lateral breast area. In this study, the tissue weight of tumor or inflammatory lesions was (78.3±5.1) g(56-92 g), and the LTAP flap with pedicle was (9.3±0.6) cm (8.2-12.5 cm)in length and (5.6±0.4) cm(3.8-7.2 cm) in width. In this group, 1 patient developed an infection of the donor site drainage entrance. After wound dressing and taking oral antibiotics, the wound healed quickly and healed well. Postoperative follow-up was conducted for 2-17 months, with an average of 8.2 months. There were 10 cases with good cosmetic effect and 2 cases with average cosmetic effect, and there were no patients with poor cosmetic effect. All patients were satisfied with the shapes of the reconstructed breasts. Among them, 5 patients completed radiotherapy, and the appearance change of the breast was not obvious. All patients were followed up and underwent color ultrasound examination of the breasts, which showed that the flaps survived well. There was no recurrence of tumor or inflammation.Conclusions:Pedicled LTAP flap is an alternative operation scheme for partial breast reconstruction after breast-conserving surgery for breast cancer or lesion resection for non-lactation mastitis, especially for reconstructing of the lateral region. It has the advantages of simple operation, easy survival of the flaps, low complications, and relatively hidden incisions.

Abstract:Background: The intercostal artery perforator flap has traditionally been used to reconstruct small or moderate-sized single defects in the lateral or lower medial breast during breast-conserving surgery. We report a modification of the intercostal artery perforator flap that allows for reconstruction of larger breast tumors than previously described flap designs. Methods: A retrospective study of breast cancer patients undergoing breast-conserving surgery and immediate partial breast reconstruction with an extended chest wall perforator flap. Primary outcomes were successful tumor excision, adequate radial margins, postoperative complications, and delays to adjuvant radiotherapy. Results: Thirty patients were included. Mean radiological tumor size was 27 mm (11-56 mm) and excision volume, 123 cm3 (18-255 cm3). All tumors had satisfactory excision margins, and no patient required further surgery for re-excision. In the early postoperative period, one patient required radiological drainage of seroma, and one returned to theater for debridement of fat necrosis affecting the flap. Ten other patients were managed on an outpatient basis for minor wound complications. All patients were followed up annually for 5 years. No patients had a delay to adjuvant treatment or required revisional procedures for cosmesis. Conclusions: The modified chest wall perforator flap allows for breast conservation for larger tumors from all quadrants of the breast, including centrally located tumors and reconstruction of the axillary defect following lymph node clearance. The length of the flap allows for the use of multiple perforators in the pedicle area and freedom of the flap to reach the defects. This can be performed with low morbidity and no delay to adjuvant radiotherapy.
Anatomical Research of Pedicled Lateral Thoracic Artery Perforator Flap in Breast Reconstruction after Breast-Conserving Surgery

Abstract:Objective:To explore the anatomical basis of pedicled lateral thoracic artery perforator flap in breast reconstruction after breast-conserving surgery.Methods:(1)To observe and record the resources and distribution of lateral thoracic artery in 8 adult specimens. Eight fresh adult cadavers(16 sides), 6 males, 2 females, aged from 28 to 71 years, 39.5 years on average. The specimens were perfused with red gel joining lead oxide via femoral artery. The occurrence rate, location, course, branch and vascular anastomosis of bilateral lateral thoracic arteries with peripheral blood vessels were dissected from the upper to the clavicle, the lower to the fifth rib, the outer to the axillary front, and the inner to the parasternum. The sources of the lateral thoracic perforator vessels were systematically classified. The outer diameter and pedicle length of the lateral thoracic artery were measured. (2) From August 2015 to February 2019, 12 cases of early breast cancer patients received breast-conserving surgery and pedicled lateral thoracic artery perforator flap transfer breast reconstruction in Hunan Province Cancer Hospital. All patients were female. Age ranged from 14 to 52 years old (43.2 years old on average). Eight cases were invasive ductal carcinoma and 4 cases were invasive lobular carcinoma. The locations of the masses were the upper lateral quadrant ( n=9), the lower lateral quadrant ( n=3). Survival of the flap and the shape of the reconstructed breast were observed after operation. Results:Lateral thoracic artery was observed in 15/16 sides of 8 specimens, pierced between the pectoralis major muscle and latissimus dorsi muscle, outer diameter average of 0.80 mm(0.7-0.9 mm); pedicle length average of 8.15 cm (6.8-10.4cm), had rich vascular anastomosis with the thoracoacromial artery, internal mammary artery, thoracodorsal artery and intercostal artery. Lateral thoracic artery was classified in 6 types, originated from thoracoacromial artery (8/15, type Ⅰ), axillary artery (4/15, type Ⅱ), thoracodorsal artery (2/15, type Ⅲ), subscapular artery (1/15, type Ⅳ), multi-resources (1/15, type Ⅴ). In one specimen, lateral thoracic artery was absent (1/15, type Ⅵ). The average weight of the mass of tumor and breast tissue was 70-95(80.0±5.7)g. Size of the flaps were 10.5-13.5 (11.2±0.4) cm in length, 4.5-7.5 (6.3±0.5) cm in width, and 3.0-4.5(3.4±0.4) cm in thickness. Complications developed in 2 of the cases, fat necrosis in two and wound dehiscence in one, all healed smoothly with dressing treatment. All flaps survived. The reconstructed breasts' shape, texture and elasticity were good. There was no flap contracture deformation in all cases. Only linear scar left in the lateral thoracic donor sites. Function of shoulders were not affected. All 12 patients were followed up for 6 to 20 months (11.5 months on average) with satisfactory results. No recurrence of breast cancer happened.Conclusions:The lateral thoracic artery perforator has variable resources, instant existence and extensive performance. The pedicled lateral thoracic artery perforator flap can be a useful, alternative and reliable technique in breast reconstruction after breast conserving surgery, especially in laterally located breast cancer.

Abstract:This is a case report of a patient with a borderline phyllodes tumor in the left breast. Seventeen months after the resection of the phyllodes tumor from the patient's left breast, the tumor occurred again 5 months ago in the surgical region. A large defect was generated after the extended resection of the left breast mass, and it was repaired with a contralateral internal mammary artery perforator flap. After the operation, bilateral breast symmetry was good, and the patient was satisfied with the shape of the breast. Postoperative follow-up was performed for 15 months, and no local recurrence was observed.

Abstract:Objective To explore the clinical application of the profunda artery perforator flap in breast reconstruction of patients with early-staged breast cancer.Methods Six cases of early breast cancer patients received radical or modified radicaI surgery.The PAP flap was used to reconstruct breast at the same time or in the second phase.Four cases of breast cancer were in stage Ⅰ and 2 cases was in stage Ⅱ.Results The length of flap was (25.2 ± 0.2) cm,the width of flap was (6.4 ± 0.8) cm,the thickness of flap was (2.6 ± 0.2) cm.The length of pedicle was (7.2 ± 0.6) cm,the outer diameter of pedicle was (1.6 ± 0.3) mm.The average weight of flap was 345 g(ranged from 225 g to 575 g).All profunda artery perforator flaps were successful.In one flap fat necrosis occurred and healed with dressing treatment.The reconstructed breasts' shape,texture and elasticity was good and no flap contracture deformation happened.Only linear scar left in the donor sites,the function of thighs was not affected.All 6 patients were followed up for 2-3 months (2.5 months on average) with satisfied result.No local recurrence happened.Conclusions PAP flap is suitable to reconstruct breast for breast cancer patients.
Pedicled thoracodorsal artery perforator flap in immediate partial breast reconstruction during breast conserving surgery for breast carcinoma

Abstract:Objective To discuss the design of pedicled thoracodorsal artery perforator flap (TDAP flap),and to evaluate the aesthetic results and donor-site complications for immediate partial breast reconstruction (IPBR) after breast conserving surgery (BCS) for breast cancer patients.Method Clinical data of 13 breast cancer cases treated with BCS + IPBR using TDAP flap from November 2004 to November 2010 were retrospectively analyzed.Perforators were identified with Doppler preoperatively in all patients.Results All perforators originated within a median distance of 8.0 cm ( range,7.5 to 9.5 cm) from axillary plica at the posterior line of axilla.Median area of the flaps was 6.0 × 8.0 cm ( range,5.0 × 7.0 cm to 8.0 × 10.0 cm).One flap was muscle-sparing,while a small muscle strip was left embedding the perforators in other twelve flaps to increase the reliability of the vascular pedicle.Postoperatively patients were followedup from 4 to 71 months.Median follow-up time was 41 months.Flap necrosis and seroma in the donor-site were not found in all patients.Aesthetic results were graded as excellent or good in 9 patients,fair in 3,and poor in one.Conclusions TDAP flap is a good choice for IPBR after BCS for breast cancer patients whether lesions in outer quadrants or inner quadrants,especially for those patients with excisional biopsy.Preoperative mini-Doppler is helpful for determining the precise location of the main perforators,and decreasing the risk of vessels injury.

Abstract:Resurfacing shallow defects of extremities with an appropriately thin ﬂap is a common but difﬁcult task. This can be accomplished by harvesting the thoracodorsal artery perforator ﬂap from the lateral aspect of the upper back. This ﬂap can be based on the musculocutaneous perforator, which usually emerges in an area (cid:1) 8 to 10 cm below the posterior axillary fold and 2 to 3 cm posterior to the lateral border of the latissimus dorsi muscle, or the septocutaneous perforator of the thoracodorsal artery, which emerges anterior to the lateral border of the latissimus dorsi muscle. Between February 2001 and May 2005, we used the thoracodorsal artery perforator ﬂap for extremity reconstruction in 18 clinical cases. The existing soft tissue defects among all patients resulted from trauma, scar release, chronic ulcer, and tumor ablation. The main advantages of the thoracodorsal artery perforator ﬂap are that it only requires cutaneous tissue to achieve better accuracy in reconstructive site and it minimizes the donor-site morbidity by preserving the function of the latissimus dorsi muscle and hiding the prominent scar of the donor region. However, searching for a reliable perforator and the tedious process of intramuscular retrograde dissection of the perforator are time-consuming and skill-demanding.
Free Thoracodorsal Artery Perforator Flap in Extremity Reconstruction: 12 Cases.

Abstract:The need for thin flap coverage has increased, especially for contouring or covering shallow defects of extremities. The free thoracodorsal artery perforator flap harvested from the upper lateral back can be useful for this purpose. The thoracodorsal artery supplies the latissimus dorsi muscle and supplies perforating branches to the overlying skin. The flap is based upon the proximal perforator of the thoracodorsal artery, which usually emerges in an area approximately 8 - 10 cm below the posterior axillary fold and 2 - 3 cm posterior to the lateral border of the latissimus dorsi muscle. Between February of 2001 and April of 2003, we used the free thoracodorsal artery perforator flap for distal limbs reconstruction in 12 clinical cases, including three hands, two forearms and seven feet. The soft tissue defects resulted from trauma, scar release, chronic ulcer, or tumour ablation. The main advantages of the thoracodorsal artery perforator flap are that it contains no muscle, allowing more reconstructive precision, and morbidity is minimised by preserving the function of the latissimus dorsi muscle and hiding the donor scar. However, meticulous intra-muscular retrograde dissection of the perforator, to the thoracodorsal artery, is necessary in order to obtain suitable pedicle length and vessel diameter.The authors conclude that the free thoracodorsal artery perforator flap has greater potential for resurfacing large defects of distal limbs, because of its suitable thickness and hidden donor site. LICAP Versus TDAP for the Reconstruction of Partial Breast Defects

Abstract:Perforator flaps are the latest development in reconstructive surgery. Pedicled chest wall perforator flaps can be utilized in many cases of partial breast reconstruction. This research compares the outcome and technique of thoracodorsal artery perforator flap (TDAP) and the lateral intercostal artery perforator flap (LICAP) in the reconstruction of partial breast defects. Patient records were reviewed for the time period between 2011 and 2019 at the Breast Unit of the National Cancer Institute of Cairo University. Eighty three patients were accessible for the study. (46 cases of TDAP flap and 37 cases of LICAP flap). Relevant clinical data were extracted from patients' records. A special visit was organized for all 83 patients, where a digital photograph was taken in an antroposterior view. The photographs were later processed via BCCT.core software to obtain an objective cosmetic outcome assessment. Complication rates and cosmetic outcome were comparable for both techniques. TDAP flap proved to require more tedious dissection and preoperative Doppler mapping to localize perforator vessels. On the other hand, LICAP was technically easier with more consistent perforators. Pedicled chest wall perforator flaps constitute an excellent reconstructive option in partial breast defects. TDAP flap and LICAP are two reliable perforator flaps which can reconstruct outer breast defects with acceptable outcome.
[Breast-Conserving Surgery with Immediate Partial Breast Reconstruction Using Pedicled Thoracodorsal Artery Perforator Flap: a Clinical Analysis of 33 Patients].

Abstract:Objective: To explore the application value of pedicled thoracodorsal artery perforator flap in immediate partial breast reconstruction for breast cancer. Methods: This study is a prospective case series studies. Totally 128 cases of primary breast cancer patients who prepared to receive the breast-conserving surgery combine with immediate partial breast reconstruction of pedicled thoracodorsalartery perforator flap were enrolled in Breast Cancer Prevention and Treatment Center of Peking University Cancer Hospital from June 2013 to March 2016. Finally, the operations had been completed successfully in 33 eligible cases. All patients were female with a median age of 40 years (ranging from 22 to 52 years). The perforator vessel location, the donor area design, the post-operative complications, the influence of radiation and chemotherapy had been evaluated. Results: The average diameter of thoracic dorsal artery perforators measured by Doppler ultrasound before the operation was (1.5±0.4) mm (ranging from 0.6 to 2.7 mm). The average size of flaps was 15 cm×6 cm. The average time of operations was (271±72) minutes (ranging from 120 to 245 minutes). Drainage tube removed on (4.7±2.1) days after operation (ranging from 3 to 12 days). All patients received follow-up, and there was no local recurrence and distant metastasis during a median follow-up of 17(12) months (M(Q(R))) (ranging from 5 to 38 months). All TDAP flaps were survival, the wound complication rates was 6% (2/33). Conclusions: The breast reconstruction of pedicled thoracodorsal artery perforator flap is a good choice of repairing local breast defect of breast conserving surgery.Its advantages are no-influence of latissimus dorsi function and little complications in donor area.
Application and option of lateral femoral circumflex artery system perforator flaps in reconstruction of oral and maxillofacial defect

Abstract:The lateral femoral circumflex artery system perforator flaps include anterolateral thigh perforator flap supplied by the lateral descending branch and oblique branch of lateral femoral circumflex artery,anteromedial thigh perforator flap supplied by the medial descending branch of lateral femoral circumflex artery,and tensor fascia lata perforator flap by the ascending branch of lateral femoral circumflex artery.The advantages of these flaps include the low donor site morbidity,the convenient donor site,the good vascular quality,the large soft tissue volume,the ability to be harvested as a chimeric flap with multiple various tissue components,and being far from the head and neck region which is fit for two-team approach.The lateral femoral circumflex artery system perforator flaps can be flexibly chosen according to the different defects,so as to obtain the best effect in oral and maxillofacial reconstruction.
Anatomy study of thoracodorsal artery perforator flap and its clinical significance in reconstruction of head and neck defects

Abstract:Objective To study the anatomy of the thoracodorsal artery perforator flap (TDAPF) and explore its clinical application in reconstruction of head and neck defects.Methods Five fresh cadavers were prepared,and morphosis and blood supply of TDAP flaps were examined by microsurgical anatomy.During dissections,the following parameters were recorded: number and type of perforators vessels,diameter of perforators,pedicle length,diameter of the original vessels,route(infra fascia and supra fascia),and their position were located by anatomical landmarks.Results There were 44 TDAP flap perforators in all specimen with 54.5% from the lateral branch and 36.8% from the medial branch of the thoracodorsal artery.The lateral branch perforator was mostly located within 2 cm of the anterior border of the latissimus dorsi muscle.Mean pedicle length of the largest perforator was 12.41±2.84 cm.Average diameter of the infrascapular artery and the infrascapular vein was 3.59±0.62 mm and 3.75±0.41 mm,and that of the thoracodorsal artery and the thoracodorsal vein was 2.71±0.43 mm and 2.98±0.18 mm.Conclusion TDAP flaps are useful in reconstruction of head and neck defects because of their constant position,large caliber,and long pedicle.
Bilateral Free Profunda Artery Perforator Flaps(pap) for Immediate Unilateral Breast Reconstruction：a Case Report and Review of Literature

Abstract:[Abstact] Objective To investigate the possibility of using the profunda artery perforator flap (PAP) for breast reconstruction cases who are not the good candidates for back and abdominal donor sites. Methods This case was a breast cancer patient who required an i mmediate breast reconstruction with autologous tissue,skinny, bi-lateral severe breast atrophy and mastoptosis after lactation, twice cesarean delivery and umbilical hernia in August, 2016. Portable Doppler was used to detect the profunda artery perforator. The donor sites were designed according to the tissue defect after the mastectomy. The incisions were designed near to the inferior gluteal folds and medial gluteal areas. The left PAP flap was 20 cm × 5 cm, and the right PAP flap was 18 cm × 5 cm. The pedicles length of the both flaps were about 6.0 cm, and the perforator artery diameter of the both flaps were 2.2 mm. The right PAP vessels were connected to the left thoracodorsal vessels with end-to-end anastomosis. The left PAP vessels were connected to the proximal part of left internal ma mmary vessels with end-to-end anastomosis. The indocyanine green angiogra-phy under microscope showed the signs of patency in all the vessel anastomosis. Results The intraoperative patho-logic diagnosis was extensive intraductal carcinoma, and the sentinel lymph nodes biopsy showed negative result. The re-constructive breast has a nice appearance, natural and proper IMF. Both of the PAP flaps survive 100%, and the donor sites healed well and remained inconspicuous. The patient can walk freely on the 5th day postoperative, and back to home on the 7th day. Within the follow-up for one month, the patient was very satisfied with the new breast and a better gluteal appearance. Conclusion The free PAP flaps provide an excellent alternative to an ab-dominal and back donor sites for achieving the breast reconstruction due to the relaible supplying pedicle vessels and invisible donor scars.
Clinical Application of Latissimus Dorsi Perforator Flap

Abstract:Objective To study the clinical application of latissimus dorsi perforator flap in tissue defect repair. Methods Thoracodorsal perforator flaps were used in 21 cases with reconstructive surgery,including 8 facial and cervical repair,3 protothorax wound repair and axillary soft tissue defect repair,and 10 limb wound repair.The flap size ranged from 6 cm×8 cm to 15 cm×21 cm,and 3 were pedicled flap grafting and 18 free grafting.Thoracodorsal nerve and latissimus dorsi muscle were free from hurt during the free flap procedure,and simple perforator flap was formed. Results All the perforator flaps survived with satisfactory results.The function of latissimus dorsi was well preserved postoperatively,and both direct suture and free grafting yielded better appearance than the traditional latissimus dorsi flap. Conclusion Compared with the traditional latissimus dorsi flap,the latissimus dorsi perforator flap is characterized by the thinness of tissue and minimal sacrifice of donor structure with the preservation of thoracodorsal nerve and latissimus dorsi muscle,which has an expanded application range.
A Case of Using Profunda Artery Perforator Flap As an Intraoperative Salvage for Failed Deep Inferior Epigastric Artery Perforator Flap Harvesting in Autologous Breast Reconstruction

Abstract:On April 11, 2019, a 36-year-old female patient was admitted to Hunan Cancer Hospital. Five years after the modified radical mastectomy for right breast cancer, she planned to undergo scar releasing and reconstruction of right chest wall and free deep inferior epigastric artery perforator (DIEP) flap transfer, right breast reconstruction, and left breast mastopexy. The defect of right chest wall after scar resection was 18 cm×10 cm. During the operation, the vascular pedicle of DIEP flap was accidentally injured, then the profunda artery perforator flap in left inner thigh was designed for salvage. The size of the flap was 20 cm×11 cm, the thickness was 4.5 cm, the length of perforator vessel pedicle was 7.6 cm, and the weight of the flap was 360 g. The right breast defect was repaired with the transferred flap and the deep cavity was filled. The vascular pedicle of profunda artery was anastomosed with the proximal end of the right internal mammary artery. The blood supply of the flap was good during surgery. The left breast mastopexy was completed at the same time, and the donor site of thigh and abdomen was closed directly. The flap survived well and the donor site healed well after surgery. During the follow-up of 7 months post surgery, the appearance and function of thigh donor site were good, no obvious complications were found, and the reconstructed breast was smaller than the contralateral side. This case suggests that the profunda artery perforator flap could be a valuable option as an alternative for DIEP harvesting failure for autologous breast reconstruction.

Abstract:To study the anatomical characteristics of thoracoacromial artery perforator flap（TAAP）, and to explore the advantages and disadvantages of TAAP in head and neck reconstruction. Four fresh cadavers （8 hemichests） were collected for anatomical observation, the blood supply of chest skin was observed through autopsy, the presence, number, location, caliber, and landmark on the chest surface of all thoracoacromial artery perforators per hemichest were recorded, including the distance of each from the midpoint of the clavicle. The diameters of the thoracoacromial artery and perforating vessels were measured with vernier calipers （accuracy 0.05 mm）, the pedicle length and thickness were measured with a cm scale （accuracy: 1 mm） after the flap was obtained, and the retained photos were recorded. No perforating branch （12.5%） was found on one side of the 8 hemichests, two perforating branches （12.5%） were found on one hemichest, and one perforating branches （75.0%） were found on the rest of the hemichests. The perforating point was about between the clavicular head of pectoralis major （clavicular part） and the sternocostal head （sternocostal part）. The vessels at the beginning of perforation were generally bulky, with an average diameter of 2.25 mm, however, the vessel diameter was significantly reduced after the perforation of the flap. The pedicle length of thoracoacromial artery perforator flap ranged from 5.43 cm to 9.03 cm, with an average length of 7.14 cm. The pedicle length from the exit point of perforator muscle gap to the flap was 2.32-4.63 cm, with an average length of 3.28 cm. The distance between the exit point of perforator muscle space and the lower edge of the midpoint of the clavicle was 3.31-4.52 cm, with an average distance of 3.77 cm. The thoracoacromial artery perforator flap has some advantages such as similar color as head, neck and maxillofacial region, stable blood supply, relatively consistent vascular pedicle length and caliber size, relatively larger flap, less damage to pectoralis major muscle, and protection of chest shape, thoracic movement and shoulder joint movement function. Although the clinical application of this flap is limited by the uncertainty of perforating vessels, postoperative asymmetry of the nipple and residual chest scar, it still has a broad application prospect in head and neck reconstruction.

Abstract:Objective To explore the clinical effects of sequential flap of thigh perforator for closure of the anterolateral thigh free flap donor site.Methods From March,2014 to May,2015,8 consecutive patients underwent elective anterolateral thigh free flap reconstruction for limb soft tissue defect.To design a sequential flap of thigh perforator to achieve primary closure of the anterolateral thigh free flap donor site.The area of anterolateral thigh free flap ranged from 9.0 cm×6.0 cm to 20.0 cm×1 1.0 cm,respectively.The area of thigh perforator flap ranged from 7.0 cm×3.0 cm to 12.0 cm×7.0 cm.Results All flaps healed uneventfully.Distal skin flap necrosis appeared in 1 flap which was healed after local transposition flap.All the patients were followed up for 1 to 6 months,with mean time of 4 months.The color,texture and appearance of flaps were satisfactory.Conclusion Sequential flap of thigh perforator,with reliable blood supply and constant position,represents an ideal model to close the anterolateral thigh free flap donor site of small defect.
The Harvestion and Application Experience of Free Multi-Lobed Anterior Thigh Flap

Abstract:Objective:To introduce the technique and clinical application of free lobed anteromedial thigh perforator pedicle flap.Methods:From September 2015 to September 2021, 72 patients with perforating buccal and oral cancer defects were treated at the Oncology Plastic Surgery Department of Hunan Cancer Hospital. There were 61 males and 11 females, with an average age of 36.7 years (31-56 years). According to Union for International Cancer Control (UICC) TNM staging, there were 20 cases of T3N1M0, 13 cases of T3N2M0, 24 cases of T4N0M0, and 15 cases of T4N1M0. All defects were planned to be repaired with free lobed anteromedial perforator flaps. When there was only one set of vascular pedicle, the perforating vascular pedicle artery was anastomosed with the superior thyroid artery, and the accompanying vein was anastomosed with the superior thyroid vein by end-to-end.Results:The areas of soft tissue defects after radical resection of oral and buccal cancers in 72 patients were between 5.0 cm × 4.0 cm and 11.0 cm×7.0 cm; the areas of the first anterior femoral skin islands were between 5.0 cm × 4.0 cm and 13.0 cm×7.0 cm; the areas of the second anterior femoral skin islands were between 5.0 cm × 3.0 cm and 10.0 cm × 7.0 cm; and all flap donor sites were directly closed. In 35 cases, the vascular pedicle was accompanied by a lateral femoral muscle flap for filling the gap defect at the base of the mouth. The average length of the vascular pedicles of the flaps was 7.2 cm (range: 6.8-8.2 cm). The average diameter of the vascular pedicle arteries was 1.6 mm (range: 1.4-2.2 mm). The blood flow reconstruction of flap was completed by anastomosing one accompanying vein. The average diameter of the accompanying veins was 2.1 mm (range: 1.6-2.8 mm). Postoperative hematoma occurred in 3 patients, with one having vascular crisis. After emergency exploration, 2 of them were successfully saved, and the other one had complete necrosis of skin flap, which was repaired by pedicled pectoralis major skin flap transplantation. With following up of 12-38 months, the appearances of the flaps were satisfactory without significant swelling. The mouth opening and language function were satisfactory in all cases, and only linear scars were left in the donor sites, with no significant impact on thigh functions. Five patients with local recurrence of tumor were treated with second radical resection and repair with pedicled pectoralis major myocutaneous flap. Six patients developed cervical lymph node metastasis (4 on the same side and 2 on the opposite side) and underwent neck dissection again.Conclusion:The anatomical basis of the branches of the anteromedial femoral perforating branches in the anterolateral region of the thigh can be helpful to prepare the anterolateral femoral lobed flap, which is suitable for repairing the perforating defects after the radical operation of oral and buccal cancers.

Abstract:Objective To explore the clinical application of the deep inferior epigastric artery perforator flap in bilateral breast reconstruction of patients with breast cancer. Methods Deep inferior epigastric artery perforator flap was applied for breast reconstruction in four cases of breast cancer patients received radical or modified radical surgery in Hunan Provincial Cancer Hospital. All patients are female, age ranged from 31 to 53 years old (36. 2±5. 9). Results The length of flap was (15. 1±0. 4) cm, the width of flap was (12. 6±0. 3) cm, the thickness of flap was (4. 3±0. 5) cm. The length of pedicle was (12. 6±0. 3) cm, the outer diameter of artery was (1. 8±0. 2) mm, the outer diameter of vein was (2. 1± 0. 4) mm.The average weight of flaps was 235 g(ranged from 195 g to 335 g). In one case flap fat necrosis occurred and in other one donor site fat necrosis was noted. The two flaps both healed with dressing treatment and no other complications were found. The reconstructed breasts'shape, texture and elasticity were good and no flap contracture deformation happened. Only linear scar left in the donor sites, the function of abdomen did not affected. All 4 patients were followed up for 14 to 33 months ( 21. 7 months on average) with satisfied result. No local recurrence happened. Conclusions Free deep inferior epigastric artery perforator flap is suitable to reconstruct bilateral breast for breast cancer patients.
