Abstract:Assessment, Ahead of Print. Although ecological momentary assessment (EMA) provides an opportunity for the examination of intervention mechanisms in real time, there are few validated tools to assess key treatment mechanisms in EMA studies. Our focus in this study is a potentially critical treatment mechanism, improvement in self-efficacy for managing negative emotions. We examined the psychometric properties of the Patient-Reported Outcomes Measurement Information System (PROMIS) self-efficacy for managing negative emotions scale measured via EMA. Participants (n = 145 college students) in a clinical trial of Dialectical Behavior Therapy skills completed four daily EMAs for 6 weeks (13,153 total responses). Results indicated (a) excellent internal consistency and good factor structure, (b) sufficient variability at both the between- and the within-persons levels, and (c) strong construct and predictive validity. This study supports the validity of an EMA measure of self-efficacy for managing negative emotions that can be used in real time, both in intervention studies and in observational research.

Abstract:Substance use disorder is a growing health problem all over the world. The coexistence of substance misuse, affective temperaments, and emotion dysregulation has not been studied sufficiently. The present study aimed to evaluate the role of affective temperaments and emotion dysregulation on substance use disorder. The Emotion Dysregulation and Affective Temperaments in Opioid Use Disorder study was designed observational. This paper presents only the baseline assessments of the patient and control groups. One-year remission rates of the patients' group will be reported elsewhere after 1-year follow-up. Sixty-seven patients with opioid use disorder and 68 healthy controls enrolled. All participants were administered to The Structured Clinical Interview for DSM-5, Temperament Evaluation of Memphis, Pisa, Paris, San Diego Autoquestionnaire, Difficulties in Emotion Regulation Scale, Beck Anxiety Inventory, and Beck Depression Inventory. Patients with opioid use disorder had higher scores from all temperamental scales and showed higher difficulties on emotion regulation than the control group. Even controlling the confounding effects of anxiety and depression levels, dysthymic and anxious temperament scores were found correlated with the emotion dysregulation score in the patient group. The emotional traits (i.e., affective temperaments) and emotion regulation abilities play a crucial role in substance use disorder. While managing substance use disorder, being aware of affective temperament characteristics and/or interventions to improve emotion regulation skills may be helpful.

Abstract:Background: While substance use disorder is one of the overarching health and social issues that might seriously disrupt individuals' self-control and self-efficacy, most previous studies have been conducted among university students or other groups, and little is known about how the underlying mechanisms between self-control and self-efficacy might impact patients with substance use disorders.Objectives: The purpose of this study is to investigate how resilience and self-esteem mediate the relationships between self-control and self-efficacy among patients with substance use disorders.Methods: We conducted a cross-sectional study of 298 patients with substance use disorder from Shifosi rehab in China. Diagnostic and Statistical Manual of Mental Disorders (5th Edition)-based diagnostic questionnaires were used to collect demographic information and assess addiction severity. The Dual-Modes of Self-Control Scale (DMSC-S) was implemented to measure self-control, while self-esteem was measured using the Self-esteem Scale (SES). The Connor-Davidson Resilience Scale (CD-RISC) was used to measure resilience, and self-efficacy was measured by the regulatory emotional self-efficacy scale (RESE).Results: The correlations between all the dimensions and total scores on the self-control, resilience, self-esteem, and self-efficacy were significantly positive (p < 0.01), indicating that they could predict patients' self-efficacy. Bootstrap testing indicated that resilience and self-esteem fully mediated the relationship between self-control and self-efficacy, relationships between self-control and self-esteem were partially mediated by resilience, and resilience partially mediated the relationship between self-esteem and self-efficacy. Finally, the multiple-group analysis indicated that the relationships among self-control, resilience, self-esteem, and self-efficiency did not differ with respect to gender.Conclusions: The path from self-control through resilience and self-esteem and on to self-efficacy is significant among patients with substance use disorders, suggesting that increasing self-control, resilience, and self-esteem can improve self-efficacy among patients with substance use disorders.

Effects of drug attitudes on treatment compliance and psychiatric symptoms of schizophrenia: an one year follow-up study

Abstract:Objective To explore the effects of integrated treatment model (psychosocial interventions combined with drug therapy) in schizophrenia and analyze the influence of medication attitude on treatment compliance and psychiatric symptoms in different intervention models.Methods A total of 170 patients with schizophrenia were enrolled from 2012 to 2015.Patients were randomly divided into two groups:the integrated treatment group (86 patients) with drug therapy and psychosocial intervention,and the conventional drug treatment group (84 patients) with only drug intervention.Dug attitude inventory (DAI),self-awareness inventor (SAI),positive and negative symptoms scale (PANSS) were used to assess medication attitude,treatment compliance and clinical psychotic symptoms for all patients in the following 3 months,6 months and 12 months.Results Compared with medication-alone group,the integrated treatment group achieved significant improvement in psychiatric symptoms,including positive symptom and general psychopathology as well as significant reduction in scores in DAI and SAI (P＜0.05).In the conventional drug treatment group,the PANSS score was positively correlated with the increased score of DAI (β=0.31,P=0.02),which was mediated by the improvement in treatment adherence (β=0.18,P=0.18).There was no significant correlation between PANSS score and the increase score of DAI in the integrated treatment group(P=0.62).However,the increased score of SAI was positively correlated with the positive symptom of PANSS scale (r=0.31,P=0.01) and with the general psychiatric symptom scale (r=0.36,P＜0.01).Conclusion This study demonstrates that medication attitude can improve the treatment of mental symptoms by increasing compliance in the medication-only group.Symptom improvement is associated with improved compliance but not with medication attitudes in the integrated treatment model.

Abstract:The primary objective of this research is to explore how the level of Mental Health Literacy influences the motivation for treatment among individuals dealing with alcohol or substance addiction. The study involved 128 subjects undergoing inpatient treatment at the Alcohol and Substance Abuse Treatment Center. To gather data, researchers utilized a Personal Information Form, Mental Health Literacy Scale, and Treatment Motivation (TM) survey. The scores obtained from the Treatment Motivation Questionnaire and Mental Health Literacy Scale were 94.21±12.2 and 108.03±11.7, respectively. The analysis revealed a statistically significant, albeit weak, positive correlation between the participants' Mental Health Literacy Scale-Knowledge of How to Seek Mental Health Information subscale mean score and both their total Treatment Motivation Questionnaire score and their Interpersonal Help-Seeking subscale mean scores (r=0.284). Conversely, a statistically significant weak negative correlation emerged between the participants' Mental Health Literacy Scale-Knowledge of Professional Help Available subscale mean score and their Treatment Motivation Questionnaire-Distrust in Treatment subscale mean score (r=−0.230). Conclusions drawn from the study indicate that a lack of knowledge regarding addiction and accessing professional assistance, coupled with concerns surrounding stigma, pose as significant barriers to motivation for seeking treatment.

Abstract:The aim of this study is to evaluate the effect of mindfulness-based psychoeducation on negative automatic thoughts and medication adherence in individuals with cannabis use disorder. The randomized controlled study was conducted between September 2021 and December 2022 in the psychiatry clinic of a university hospital in Turkey. The sample size of the study consisted of 60 individuals (30 experimental groups, 30 control groups). Descriptive Characteristics Form, Negative Automatic Thoughts Questionnaire, and Morisky Medication Adherence Scale were used to collect data. In the study, while eight sessions of mindfulness-based psychoeducation were given to the experimental group, no intervention was applied to the control group. Experimental group posttest negative automatic thoughts and medication adherence scale total mean scores were found to be statistically significant ( p < 0.05). It was determined that the posttest negative automatic thoughts scale total score mean in the experimental group decreased compared to the control group, and the medication adherence scale total score mean increased compared to the control group. It was determined that mindfulness-based psychoeducation given to individuals with cannabis use disorder was effective in negative automatic thoughts and adherence to treatment.

The relationship between male prisoners' self-efficacy and their participation in addiction treatment

Abstract:The purpose of this study was to attempt to verify the existence of a relationship between internal resources (self-esteem and self-efficacy) and motivation (decisional balance) to undergo treatment in prisoners with alcohol addiction participating in voluntary treatment as well as referred to obligatory addiction treatment based on a court decision. The study was carried out in penitentiary units in various parts of Poland in 2018-2019. Participants completed the Decision Balance Scale, Generalized Self-Efficacy Scale, and Multidimensional Self-Esteem Inventory twice - before and after addiction treatment. The study adopted the assumptions of the Transtheoretical Model developed by Prochaska and DiClemente (1984). There were no statistically significant differences between the group of prisoners referred for obligatory treatment and those undergoing voluntary therapy. At the end of the 3-month treatment, there was a significantly smaller number of arguments "for" drinking and less identity integration in both groups studied. The results obtained may indicate that after undergoing therapy, prisoners remain in a contemplation stage due to their inability to adapt therapeutic interactions to individual needs. It seems that changes in identity integration may be indicative of the crisis that is being experienced, which in effect may allow individuals with alcohol addiction to search for and achieve a potentially new, coherent image of themselves.

Abstract:The relationship between abstinence self-efficacy and substance use at 2 years was examined among a sample (<i>N</i> = 470) of persons with substance use disorders and recent incarceration histories. Participants were assigned to residential (therapeutic community/TC or Oxford House) or nonresidential (usual care) conditions. The authors hypothesized abstinence self-efficacy would predict decreased substance use, and residential treatments would moderate this relationship. A conditional effect was observed, with low levels of abstinence self-efficacy predicting significant increases in substance use in the TC and usual care conditions. Supplemental analyses revealed significant decreases in substance use over time among participants in the Oxford House condition, and a significant conditional effect (gender x treatment condition) in relation to substance use. Findings point to the need for researchers to examine factors that mitigate the relationship between abstinence self-efficacy and substance use outcomes, and for treatment providers to consider the Oxford House model for this population.

Abstract:The aim of this study is to examine the effect of regular exercise on depressive anxiety symptoms, treatment motivation and mindfulness levels in individuals who are alcohol or substance dependent. The study had two arms and was randomized controlled: (A) Patients assigned to standard medical treatment, (B) Patients who receive standard treatment are also given access to supervised group exercise. Patients in the exercise group were given exercise for 3 weeks. Hamilton Anxiety (HAM-A), Hamilton Depression Scale (HAM-D), Treatment Motivation Scale (TMQ) and Mindful Attention Awareness Scale (MAAS) were administered to the patients at the beginning and end of the treatment. There was a decrease in HAM-A, HAM-D scores, an increase in TMQ and MAAS scores, and improvement after treatment in both groups. However, there was a significant change in HAM-A, HAM-D and TMQ scores in the group that added exercise to their treatment compared to the group that received only standard therapy. There was no significant difference in MAAS scores. It was discovered as a result that including exercise in addiction therapy could enhance the therapeutic response. Longer-term studies with greater sample sizes must be planned and carried out to produce more trustworthy results.

Abstract:Substance misuse is a pressing public health concern, especially among emerging adults, with implications for psychological health and overall well-being. This cross-sectional study investigates the mediating roles of self-compassion and subjective well-being in the relationship between psychological flexibility and substance misuse among Turkish college students. The study included 445 emerging adults enrolled at a state university in Türkiye. University participants were recruited via convenience sampling using snowball methods. Data were collected using self-report scales. The sample was 69% female ( n = 307) and 31% male ( n = 138) and ranged in age from 18 to 26 years ( M = 20.85, SD = 1.35). In the study, Pearson product-moment correlation analysis was conducted to examine the relationships between the variables and then the serial mediation model was tested. The findings have revealed that psychological flexibility significantly predicts substance misuse, subjective well-being, and self-compassion. Additionally, the study identifies self-compassion as a mediator in the relationship between psychological flexibility and subjective well-being. While self-compassion contributes significantly to well-being, it does not explain the impact of psychological flexibility on substance misuse. Furthermore, subjective well-being also appears to be a mediator in the relationship between psychological flexibility and substance misuse, emphasizing its potential significance in reducing the likelihood of resorting substance misuse. Overall, the study contributes to understanding the pivotal role of self-compassion and psychological flexibility in fostering well-being and preventing substance misuse in the college setting.

Social cognitive predictors of treatment outcome in cannabis dependence

Abstract:Background: Drug-related outcomes expectancies and refusal self-efficacy are core components of Social Cognitive Theory. Both predict treatment outcome in alcohol use disorders. Few studies have reported expectancies and refusal self-efficacy in cannabis dependence. None have examined both, although both constructs are key targets in Cognitive-Behavioural Therapy (CBT). This study tests the predictive role of expectancies and refusal self-efficacy in treatment outcome for cannabis dependence. Design: Outpatients completed a comprehensive assessment when commencing cannabis treatment and predictors of treatment outcome were tested. Setting: A university hospital alcohol and drug outpatient clinic. Participants: 221 cannabis-dependent patients participated in a 6-week CBT program where the goal was abstinence. Measurements: Cannabis Expectancy Questionnaire and Cannabis Refusal Self-Efficacy Questionnaire, cannabis dependence severity [Severity of Dependence Scale], psychological distress [General Health Questionnaire] at baseline; the timeline follow-back procedure at baseline and each session. Findings: Patients reporting lower confidence in their ability to resist cannabis during high negative affect (emotional relief refusal self-efficacy) had a lower likelihood of abstinence (p=0.004), more days of use (p<0.001), and larger amount used (p<0.001). Negative cannabis expectancies predicted greater likelihood of abstinence (p=0.024). Higher positive expectancies were associated with lower emotional relief self-efficacy, mediating its association with outcome (p<0.001). Conclusions: Emotional relief refusal self-efficacy and negative expectancies are predictive of better treatment outcomes for cannabis dependence. Positive expectancies may indirectly predict poorer outcome because of a negative association with self-efficacy, but this conclusion remains tentative as directionality could not be established.

Abstract:Drug addiction is a current global problem, which causes significant damage to the individual and society as a whole. Drug addicts have numerous disorders, among which the emotional sphere occupies an important place. Identification of social and psychological factors affecting the development of emotional intelligence of drug addicts will make it possible to optimize their psychological rehabilitation programmes. The aim of the study is based on establishing the influence of social and psychological factors on the development of the emotional intelligence level in drug addicts. Methods: The research programme uses standardized psychometric diagnostic methods (MSPSS, Self-Monitoring Scale, EQ-test, Self-esteem test). Descriptive statistics, the Kruskal-Wallace H test, and Pearson’s linear correlation coefficient were used. The Results: The study showed that a low level of emotional intelligence prevails among the surveyed drug addicts (M=37.63±13.38). At the same time, people with a low level have pronounced signs of low social self-control (Н=67.64, р≤0.001), social support (Н=67.76, р≤0.001), and self-esteem (Н=89.12, р≤0.001). Correlation analysis revealed a close direct relationship between emotional intelligence and social self-control (r=0.681, p≤0.001), social support (r=0.632, p≤0.001), and self-esteem (r=0.726, p≤0.001). Conclusions: The study found that the development of emotional intelligence of drug addicts is influenced by such social and psychological factors as social self-control, social support, and self-esteem. These factors determine the ability to manage emotions, adequately perceive them and objectively express them. Prospects: The identified results can be used when building a system of psychological rehabilitation for persons with drug addiction. In particular, to develop emotional intelligence, and improving general emotional well-being.
Comparison of psychological reactions, fear of negative evaluation and mood–emotional alexithymia in successful and unsuccessful people in addiction recovery: a survey of Baluchestan in Iran

Abstract:The aim of this study was to examine psychological responses, fear of negative evaluation, and mood-related alexithymia in individuals in addiction recovery who have succeeded versus those who have not. This study employed a causal-comparative design. The research focused on all individuals who sought treatment for addiction at clinics within a specific district in Zahedan city, Iran. Out of the group, 100 individuals were chosen (50 who successfully stopped their addiction and 50 who were unsuccessful in their attempts to quit) through convenience sampling. Data were gathered using the depression, Anxiety, and Stress Scale (DASS-21), Lori's fear of negative evaluation scale, and Toronto's mood–emotional alexithymia scale. Data were examined through both descriptive statistics and multivariate analysis of variance. Based on the results, successful and unsuccessful individuals in addiction recovery showed varying levels of depression, stress, fear of negative evaluation, and having no fear of negative evaluation ( p < .01). In simpler terms, unsuccessful individuals in addiction recovery scored higher in depression, stress, fear of negative evaluation, inability feeling emotions, inability to express feelings compared to successful individuals. But successful individuals in addiction recovery obtained higher mean scores than unsuccessful individuals in addiction recovery on having no fear of negative evaluation. According to the findings psychological reactions, fear of negative evaluation and mood–emotional alexithymia play a significant role in addiction treatment and can be helpful in addiction recovery in unsuccessful people in addiction recovery. It is feasible to enhance the effectiveness of addiction recovery by implementing interventions that target the reduction of depression, stress, fear of negative evaluation, and alexithymia.

Abstract:This study aimed to examine the effect of an emotion recognition and expression program (EREP) on the alexithymia, emotion expression skills and positive and negative symptoms of patients with schizophrenia. The study had a non-randomized, quasi-experimental design including a pretest, post-test, and follow-up test. It was conducted with 36 patients with schizophrenia ( n = 18 intervention group, n = 18 control group) who regularly visited a Community Mental Health Center (CMHC) in Türkiye and participated voluntarily. The EREP was applied to the intervention group for eight weeks. "Personal Information Form", "Emotion Expression Scale (EES)", "Toronto Alexithymia Scale (TAS)", and "Positive Negative Syndrome Scale (PANSS)" were applied to all participants in the pretest, post-test, and follow-up test. The follow-up test was applied one month after the end of the sessions. Number, percentage, chi-square test, and repeated measures analysis of variance were used for data evaluation. In the total alexithymia score, there was a significant difference in the group interaction by time in the intervention group compared to the control group. In terms of total alexithymia score, the post-test and follow-up test mean scores of the intervention group were lower than the control group ( p < 0.05; η2 = 0.122). There was a significant time*group interaction in the positive emotion subscale of the EES ( p < 0.05; η2 = 0.121). The findings of our study indicated that the EREP had a positive effect on the alexithymia scores of patients with schizophrenia. We found that the EREP used in our study contributed to the reduction of alexithymia levels in patients with schizophrenia.

Abstract:BACKGROUND: Emotional conditions governing the family and patients' perceived social support play important roles in the treatment or relapse process of the chronic disease.OBJECTIVES: The current study aimed to investigate the role of family expressed emotion and perceived social support in prediction of addiction relapse.PATIENTS AND METHODS: The descriptive-correlation method was used in the current study. The study population consisted of the individuals referred to the addiction treatment centers in Ardabil from October 2013 to January 2014. The subjects (n = 80) were randomly selected using cluster sampling method. To collect data, expressed emotion test by Cole and Kazaryan, and Multidimensional Scale of Perceived Social Support (MSPSS) were used, and the obtained data was analyzed using the Pearson's correlation coefficient and multiple regression analyses.RESULTS: Results showed a positive relationship between family expressed emotions and the frequency of relapse (r = 0.26, P = 0.011) and a significant negative relationship between perceived social support and the frequency of relapse (r = -0.34, P = 0.001). Multiple regression analysis also showed that perceived social support from family and the family expressed emotions significantly explained 12% of the total variance of relapse frequency.CONCLUSIONS: These results have implications for addicted people, their families and professionals working in addiction centers to use the emotional potential of families especially their expressed emotions and the perceived social support of addicts to increase the success rate of addiction treatment.
The Experience of Therapeutic Community: Emotional and Motivational Dynamics of People with Drug Addiction Following Rehabilitation

Abstract:The present study aims at exploring the emotional and motivational dynamics of people with drug addiction following rehabilitation. Seventy-eight individuals with substance use disorder (89.2% male) aged from 18 to 63 years completed a questionnaire exploring their experience with treatment and the therapeutic community context and Short Form Health Survey assessing health-related quality of life. Cluster analysis and multiple correspondence analysis allowed the detection of different groups of respondents and of the main synthetic factors affecting their experiences. Results identified three groupings of respondents, conceived along two main latent dimensions: expectations of change to recovery (delegation/effort) and perceived usefulness of treatment (denial of dependence/emotional investment). The second dimension was associated with some quality indicators of rehabilitation process (reported motivation at the entry, mental quality of life, and length of time following the treatment), which might potentially affect both treatment retention and success on the long run.
