Abstract:Objective: To investigate kinesiophobia, physical activity levels and barriers to physical activity in women with breast cancer and breast cancer survivors. Methods: The case-control study was conducted at the Breast Clinic of Acıbadem Maslak Hospital, and the Department of Physiotherapy and Rehabilitation at Acibadem Mehmet Ali Aydinlar University, Turkey, from October 2021 to July 2022, and comprised patients with breast cancer in group A, breast cancer survivors in group B and healthy controls in group C. The primary outcome measures were physical activity levels, barriers to physical activity and kinesiophobia levels, while the secondary outcome measures were levels of anxiety, depression, fatigue and quality of life. Data was collected using standard tools. Data was analysed using SPSS 22. Results: Of the 212 women, 70(33%) were in group A with mean age 50.71±11.30 years, 70(33%) in group B with mean age 47.64±9.85 years, and 72(34%) in group C with mean age 47.03±7.48 years. Group C had better physical activity levels, fatigue and quality of life scores than the other groups, but it had worse perceptions of physical activity and more individual, psychosocial and environmental barriers to physical activity compared to the other groups (p<0.05). Group A had more barriers to exercise related to fear of overall body pain, poor balance, fear of falling and fear of feeling worse post-exercise compared to the other groups (p<0.05). Group B subjects were more afraid that lymphoedema might be exacerbated if they exercised (p<0.05). Conclusions: Women with breast cancer and breast cancer survivors had worse scores for physical activity levels, fatigue and quality of life compared to the healthy controls. All three groups had a variety of barriers to physical activity.

Abstract:Perceptual and Motor Skills, Ahead of Print. Kinesiophobia is a well-known barrier to positive clinical outcomes among patients with various diseases, but there remain few comprehensive studies of kinesiophobia among patients with myocardial infarction (MI). In this cross-sectional study, we investigated the presence of kinesiophobia and its relationship with clinical outcomes among 42 patients with MI who completed the Tampa Scale of Kinesiophobia Swedish Version for Heart (TSK-SV Heart) the International Physical Activity Questionnaire-Short Form (IPAQ-SF), the Six-Minute Walk Test (6MWT), the Hospital Anxiety and Depression Scale (HADS), the modified Medical Research Council (mMRC) Dyspnea Scale, the Charlson Comorbidity Index (CCI), and the MacNew Heart Disease Health-Related Quality of Life Questionnaire. The participants' mean TSK-SV Heart score was 39.24 (SD = 6.65), and 71.4% of these patients reported a high level of kinesiophobia. The TSK-SV Heart score demonstrated a strong correlation with the IPAQ-SF, 6MWT walking distance, and mMRC score (p < .001), and a moderate correlation with the HADS, CCI, and MacNew Heart Disease HRQoL (p < .05). Patients with a high level of kinesiophobia had lower IPAQ-SF, 6MWT walking distance, and HRQoL and higher mMRC, CCI, and HADS scores than patients with low levels of kinesiophobia (p < .05). Kinesiophobia was common and represented a considerable risk factor for physical-psychosocial dysfunctions in these patients with MI. To maintain functional independence and to increase physical activity level, clinicians should consider kinesiophobia from early to late-stage disease and should add a treatment focus that seeks to eliminate kinesiophobia in cardiac rehabilitation programs.

Abstract:Physical activity has been shown to reduce the risk for first-ever stroke as well as recurrent stroke with positive effects on almost all known modifiable risk factors. However, the perceived barriers and facilitators for engaging in physical activity have been insufficiently studied and may differ between cultures. Therefore, the aim of this study was to explore the perceptions of community-dwelling physically inactive individuals with stroke about barriers and facilitators to their participation in physical activity. This qualitative study included two focus groups of ten individuals with stroke classified as physically inactive based on accelerometer recordings. A semi-structured set of questions was posed in each focus group session and the transcripts were analysed using thematic analysis. The results revealed three main themes and subthemes: individual factors (physical impairments, psychological factors, spirituality), interaction with the family (attitudes of family members/close community) and social and environmental factors (hobbies, lack of or presence of facilities, the use of orthosis). We conclude that physical activity participation is affected by a multitude of factors as well as cultural differences. Thus, interventions aimed at increasing participation in physical activity after stroke should not only be planned according to individual clinical characteristics but also take into account a range of personal-to-social factors, including cultural differences.

Abstract:Stroke generally leads to physical problems, most commonly paralysis, cognitive, sensory, perceptual, and communication problem. During the recovery process, physical activity plays an important role in improving patients. However, physical inactivity among stroke patients is high, including those in the acute and chronic stage. The adherence of them towards exercise and rehabilitation therapy declined after discharge, thus lead to poor outcomes of recovery. To review the literature on the factors that influence stroke patients' adherence to physical activity and to identify the intrapersonal, interpersonal, and environmental factors that affect the adherence of stroke patients' towards physical activity. Four electronic databases (EBSCO: CINAHL Plus with Full text, Willey Online Library, and ScienceDirect) were searched from 2008 to 2019, reporting any factors that affecting stroke people participate in the physical activity. Articles were included if they examined the factors that influence stroke patients' adherence to physical activity and were written in English. The methodological quality of the studies was appraised using McMaster critical appraisal tools, and PRISMA guidelines were used for online searching strategy. A narrative summary of findings was undertaken reporting on individual study characteristics, country of origin, participants, and potential factors. 10 articles met the inclusion criteria. The intrapersonal factors include physical impairment, balance, and mobility, fear of falls, aging, forgetful, reduce in self-efficacy, and exercise benefits. Interpersonal factors were social support, lack of attendant and support from healthcare, physiotherapist, or gym trainer. Economic factors, transportation, and exercise facility were the environmental factors that influence stroke patient adherence to physical activity. These three factors were interrelated to change the individual perception towards exercise among stroke patients. This review enables the development of an intervention to improve participation in physical activity among stroke patients.

Kinesiophobia in Elderly Polish Patients After Ischemic Stroke, Including Frailty Syndrome

Abstract:Introduction: In patients after stroke, the relationship between the occurrence of kinesiophobia and the accompanying frailty syndrome, as well as the acceptance of the disease and the level of mood, has not been recognized so far. The aim of this study was to determine the prevalence of kinesiophobia in elderly Polish people after ischemic stroke, including the frailty syndrome and the associations between the prevalence of kinesiophobia and feelings of anxiety and degree of the illness acceptance. Methods: A cross-sectional study was used to achieve the study objectives. The study involved 152 hospitalized patients aged of minimum 60 (mean age 63), qualified for post-stroke rehabilitation, including 76 women and 76 men. The patients were divided into two groups, with kinesiophobia (119 persons) and without kinesiophobia (33 persons). The Tampa Scale of Kinesiophobia (TSK), the Tilburg Frailty Indicator (TFI), the Hospital Anxiety Depression Scale (HADS) and the Acceptance of Illness Scale (AIS) were used. Study results were calculated using MedCalc Software. Results: Kinesiophobia has been demonstrated in 78% of people after ischemic stroke. The values of TFI and HADS were higher in the patients with kinesiophobia (p<0.001). In patients with ischemic stroke, it was shown that the level of kinesiophobia increased with higher anxiety (p<0.001), higher total TFI score, (p<0.05), and a lower level of illness acceptance (p<0.001). Conclusion: The occurrence of kinesiophobia in elderly Polish people after ischemic stroke is common and the determinants of its development are the coexistence of the frailty syndrome, anxiety and a low level of illness acceptance. In post-stroke patients, the presence of kinesiophobia should be considered, especially in the situation of comorbid frailty syndrome. The issue of kinesiophobia in patients after stroke requires further in-depth research, especially in the field of cognitive-behavioral prevention aimed at ways to reduce this phenomenon.

Abstract:The global prevalence of strokes is on the rise, with the disease having a high morbidity and mortality rate. [ 1 ] At present, the recurrence rate of stroke is 11% to 28.3%, [ 2 ] which seriously affects patients' quality of life. There is evidence to suggest, however, that over 90% of strokes are contributed to physical activity (PA) inactivity, which belongs to modifiable risk factors. [ 3 ] PA refers to any physical activity that results in skeletal muscle contractions, which lead to energy expenditure. [ 4 ] In recent years, guidelines on PA of stroke have also emerged, but mainly in developed countries, such as the United States, [ 5–7 ] the United Kingdom, [ 8 ] Spain, [ 9 ] Canada, [ 10 ] and Australia. [ 11 ] We can see that the medical and social support for stroke patients varies among different countries, such as emotion, information, self-management, and tool support. Increasing PA can decrease the risk of cardiovascular disease, delay the decline of cognitive function, and reduce relapses. [ 12 ] Additionally, an individual's mental health can also be improved. [ 13–15 ] Despite the known benefits, PA implementation among stroke patients remains insufficient in many countries. A study by American scholar Rand [ 16 ] concluded that 58% of stroke patients spent <30 minutes a day participating in PA in their recovery from the disease. West [ 17 ] pointed out that stroke patients spent only 12.8% and 22.7% of their day performing moderate and high-intensity PA, respectively. Belgian scholar Vanroy [ 18 ] and American scholar Tudor-Locke [ 19 ] investigated the daily steps of stroke patients, Vanroy found that 54% of stroke patients took <5500 steps per day, Tudor-Locke agreed with his results and expressed in the research that their sedentary time exceeded 8 to 9 hours per day. It can be seen that the PA of stroke patients is seriously insufficient.

Abstract:OBJECTIVE. Physical activity contributes to the primary, secondary, and tertiary prevention of multiple diseases. However, in some patients, an excessive, irrational, and debilitating fear of movement (i.e., kinesiophobia) is thought to induce avoidance behaviors, contributing to decreased engagement in physical activity. The aim of this study was to examine whether kinesiophobia is negatively associated with physical activity in several health conditions and what factors may influence this relationship. METHODS. Five databases were searched for studies including both a measure of kinesiophobia and physical activity. Two reviewers screened articles for inclusion, assessed risk of bias, and extracted data from each study. Pearson product-moment correlations were pooled from eligible studies using the generic inverse pooling and random effects method to examine the relationship between kinesiophobia and physical activity. RESULTS. Seventy-four studies were included in the systematic review and 63 studies (83 estimates, 12,278 participants) in the main meta-analysis. Results showed a small-to-moderate negative correlation between kinesiophobia and physical activity (r = -0.19; 95% confidence interval: -0.26 to -0.13; I2 = 85.5%; p < 0.0001). Funnel plot analysis showed evidence of publication bias, but p-curve analysis suggested that our results could not be caused by selective reporting. A subgroup meta-analysis showed that the correlation was statistically significant in patients with cardiac, rheumatologic, neurologic, or pulmonary conditions, but not in patients with chronic or acute pain. CONCLUSION Our results suggest that higher levels of kinesiophobia are associated with lower levels of physical activity in several health conditions that are not necessarily painful. IMPACT. Kinesiophobia should be dissociated from pain and considered in relation to specific health conditions when implementing interventions to promote physical activity. Kinesiophobia may have prognostic implications in patients for whom physical activity contributes to prevent recurrence or worsening of their condition.

Abstract:Objective: Physical activity and exercise are central components in rehabilitation after a myocardial infarction. Kinesiophobia (fear of movement) is a well-known barrier for a good rehabilitation outcome in these patients; however, there is a lack of studies focusing on the patient perspective. The aim of this study was to explore patients' perceptions of kinesiophobia in relation to physical activity and exercise 2 to 3 months after an acute myocardial infarction. Methods: This qualitative study design used individual semi-structured interviews. Face-to-face interviews were conducted with 21 patients post-myocardial infarction who were screened for kinesiophobia (≥32 on the Tampa Scale for Kinesiophobia Heart). The interviews were transcribed and analyzed according to an inductive content analysis. Results: An overarching theme was defined as "coping with fear of movement after a myocardial infarction-a dynamic process over time" comprising 2 subthemes and explaining how coping with kinesiophobia runs in parallel processes integrating the patient's internal process and a contextual external process. The 2 processes are described in a total of 8 categories. The internal process was an iterative process governed by a combination of factors: ambivalence, hypervigilance, insecurity about progression, and avoidance behavior. The external process contains the categories of relatives' anxiety, prerequisites for feeling safe, information, and the exercise-based cardiac rehabilitation program. Conclusion: Coping with fear of movement after a myocardial infarction is a dynamic process that requires internal and external support. To further improve cardiac rehabilitation programs, person-centered strategies that support the process of each person-as well as new treatment strategies to reduce kinesiophobia-need to be elaborated. Impact: Patients with a myocardial infarction were found to be ambivalent about how they expressed their fear of movement; therefore, it is crucial for physical therapists to acknowledge signs of fear by listening carefully to the patient's full story in addition to using adequate self-reports and tests of physical fitness. These results will inform the design, development, and evaluation of new treatment strategies, with the overall aim of reducing kinesiophobia and increasing physical activity and participation in exercise-based cardiac rehabilitation.
Sarcopenia, frailty prevalence, and related factors in patients with stroke: A cross-sectional analytical study from Turkey

Abstract:Background: Sarcopenia and frailty may develop at varying rates depending on the metabolic, structural, and functional changes in the muscle structure after stroke. Objective: To evaluate the prevalence of sarcopenia and frailty in patients with stroke and affecting clinical parameters. Methods: This was a cross-sectional and analytical study. Twenty-six (49.1%) women and 27 men (50.9%) with stroke with an average age of 60.2 ± 10.3 (median = 62) years were included in the study. To evaluate physical performance, balance, and mobility, we used the 4-meter walk test (4-MWT), timed up and go test, chair sit and stand test, and the Berg Balance Scale (BBS). Sarcopenia was diagnosed according to the Asian Working Group for Sarcopenia 2019 criteria. The SARC-F scale was used for sarcopenia. Results: The average stroke duration was 21.6 ± 18.2 months. Prefrailty was present in 14 (26.4%) patients, frailty was present in 34 (64.2%) patients, and two (3.8%) patients were normal. Sarcopenia was detected in 28 (52.8%) patients. According to handgrip strength, the prevalence of low muscle strength was 17% (n= 9). In the sarcopenic group, female sex (p= 0.005) was more frequent, 4-MWT was longer (p= 0.003), chair stand test results were lower (p< 0.001), the clinical frailty scale (CFS) was higher (p< 0.001), fall risk was higher compared with the BBS (p< 0.001), there was lower HG strength (p= 0.009), lower malnutrition scores (PNI, prognostic nutritional index, p< 0.001), and lower lymphocyte counts (p= 0.037). EAT-10 scores and dysphagia presence (p= 0.026), the presence of malnutrition (p< 0.021) and Nottingham Health Profile score (p< 0.001) were higher in the sarcopenic group. Conclusion: Sarcopenia and frailty frequently develop in patients with chronic stroke, with low lymphocyte counts, physical performance, muscle strength, and mobility and balance scores, and it is associated with poor prognostic factors including high disease severity, dysphagia, malnutrition, risk of falling, and poor quality of life.

Abstract:The 830-nm AlGaAs/InGaAs laser diodes (LDs) adopting multistep-graded index double-barrier separate confinement heterostructures (GRIN-DBSCHs) with small divergence beams and improved temperature characteristics under a high-output-power operation are reported. The double-barrier separate confinement heterostructure (DBSCH) design provides good carrier confinement and prevents current leakage by adding a multistep grading layer between cladding and waveguide layers. Simultaneously, the DBSCH design can facilitate reducing the divergence angle at high-power operation and widening the transverse mode distribution to decrease the power density around emission facets. In addition, the p-side doping depth is optimized to effectively raise the barrier height for reducing the electron overflow. Gaussian-like narrow far-field patterns are measured with the full-width at half-maximum vertical divergence angle to be between 11° and 13°. A threshold current of 16.5 mA and a slop efficiency of 0.98 W/A are obtained in the continuous-wave operation condition at room temperature. The maxima optical power densities of 21.5 MW/cm2 per laser facet and good characteristic temperature values of threshold current (T0) and slope efficiency (T1) are achieved.

Abstract:Objective: Task-oriented therapy (TOT) is used to increase the effectiveness of upper extremity (UE) in activity daily living (ADL). This study aimed to investigate the effect of TOT on the participation and ADLs of stroke patients. Methods: Between October 2018 and February 2019, 28 chronic stroke patients were included in the study treated in Kocaeli University Hospital, Department of Physical Medicine and Rehabilitation, Turkey. The performance areas and participation status of the patients in which they experienced limitations were evaluated with the Canadian Occupational Performance Scale (COPM), and their level of independence in ADLs was evaluated with the Modified Frenchay Activity Index (mFAI) and Barthel Index (BI). All patients were included in the occupational therapy (OT) program five days a week for three weeks at Kocaeli University Hospital. Three ADLs in which they had difficulties were studied with 28 patients. Each of the activities was designed specifically for the patient. All assessments were repeated after three weeks of treatment. Results: A total of 28 patients, 12 females and 16 males, diagnosed with stroke, were included in the study. A statistically significant increase was found in the COPM performance and satisfaction value compared to the pre-treatment value (p<0.001). A statistically significant difference was found between pre- and post-treatment mFAI and BI values (p<0.001).
