Abstract:PurposeProphylactic release of the ulnar nerve to reduce the incidence of postoperative nerve symptoms in stiff elbows has been recommended. However, the necessity for routine anterior transposition remains unclear. In this study, we aim to gain an insight into the value of routine transposition in open release of stiff elbows.MethodsWe retrospectively reviewed 94 patients suffering from elbow stiffness with no pre-operative ulnar nerve symptoms. Simple decompression (with in situ decompression or epicondylectomy) and subcutaneous anterior transposition were chronologically performed in 53 and 37 patients, respectively. Another four patients were treated by a single lateral approach with no intervention of the ulnar nerve. Pre- and postoperative range of motion and incidence of ulnar nerve symptoms were recorded. The function of ulnar nerve was measured by Amadio rating scale.ResultsThe incidence of ulnar nerve dysfuction was 18.9 % (ten of 53) and 8.1 % (three of 37) in the simple decompression and transposition groups, respectively. The mean Amadio scores were 7.62 and 8.22, respectively. All these data showed a statistically significant difference (P < 0.05). In the lateral approach group, 50 % (two of four) of patients developed nerve symptoms with a mean Amadio score of 6.50.ConclusionsThe transposition group exhibited a superior nervous outcomes compared with the simple decompression group. No comparison was conducted between the transposition and lateral approach groups because of too few patients in the latter. According to related literature and our experience, we conclude that routine transposition is necessary to prevent postoperative nerve symptoms.

Ulnar Neuritis after Open Elbow Arthrolysis Combined with Ulnar Nerve Subcutaneous Transposition for Post-Traumatic Elbow Stiffness: Outcome and Risk Factors

Abstract:Background: Ulnar neuritis (UN) is a common complication of open elbow arthrolysis for elbow stiffness. The purpose of this study was to evaluate the outcome of subcutaneous anterior transposition of the ulnar nerve during open elbow arthrolysis and to describe the risk factors for UN.Methods: We retrospectively studied 260 patients with post-traumatic elbow stiffness who underwent routine ulnar nerve transposition during open elbow arthrolysis. Patient demographics, clinical characteristics, and incidence and reoperation rate of UN were recorded. UN was defined as new-onset ulnar nerve symptoms and no relief or worsening of pre-existing ulnar nerve symptoms during the period of postoperative rehabilitation. Factors affecting the development of UN were analyzed by univariate and multivariate analyses.Results: A total of 9.2% of the patients had UN, 25% of whom required reoperation for progressive neuropathy. The Dellon grade of patients associated with UN at last follow-up improved significantly compared with that preoperatively. The mean arc of motion in patients with UN decreased during follow-up in a time-dependent manner. Univariate analysis showed that male sex, limited preoperative flexion and arc of motion, preoperative heterotopic ossification (HO), and preoperative ulnar nerve symptoms were significantly associated with the development of UN. Multivariate regression analysis revealed that preoperative HO was the only independent risk factor for the development of UN.Conclusions: UN is still an important complication, although ulnar nerve subcutaneous transposition was performed during open arthrolysis for post-traumatic elbow stiffness. Identified risk factors for UN, especially preoperative HO, should be taken into consideration before surgery. (C) 2016 Journal of Shoulder and Elbow Surgery Board of Trustees.
Evaluation of Arthroscopic Arthrolysis in the Treatment of Posttraumatic Elbow Stiffness

Abstract:Objective:To analyze the clinical efficacy of arthroscopic arthrolysis in the treatment of posttraumatic elbow stiffness.Methods:Thirty-one patients with posttraumatic elbow stiffness treated by arthroscopic arthrolysis in our department from October 2016 to August 2019 were analyzed retrospectively. The range of motion (ROM) and Mayo elbow performance score(MEPS) before and after elbow operation were evaluated.Results:Twenty-six patients were follow-up for more than 6 months, 6 to 62 months, with an average of 27.8 months. Finally, the elbow flexion range of motion increased from (95.4±19.7)° before operation to (132.8±9.2)° after operation, and the extension range of motion increased from (-34.5±6.4)° before operation to (-15.5±12.5)° after operation. The ROM of elbow joint increased from (60.8±28.8)° before operation to (117.2±19.2)° after operation, and MEPS increased from 62.9±16.8 to 89.8±11.1 after operation. There were significant differences in elbow ROM and MEPS before and after operation ( P<0.001). Two patients had complications. One was postoperative elbow hematoma and the other was recurrence of elbow stiffness. Both patients underwent secondary surgery and achieved satisfactory results at the final follow-up. All the patients had no complications of infection and nerve injury. Conclusion:The arthroscopic arthrolysis in the treatment of posttraumatic elbow stiffness can achieve satisfactory clinical efficacy.
Anterior Subcutaneous Transposition of the Ulnar Nerve Improves Neurological Function in Patients with Cubital Tunnel Syndrome

Abstract:Although several surgical procedures exist for treating cubital tunnel syndrome, the best surgical option remains controversial. To evaluate the efficacy of anterior subcutaneous transposition of the ulnar nerve in patients with moderate to severe cubital tunnel syndrome and to analyze prognostic factors, we retrospectively reviewed 62 patients (65 elbows) diagnosed with cubital tunnel syndrome who underwent anterior subcutaneous transposition. Preoperatively, the initial severity of the disease was evaluated using the McGowan scale as modified by Goldberg: 18 patients (28%) had grade IIA neuropathy, 20 (31%) had grade IIB, and 27 (42%) had grade III. Postoperatively, according to the Wilson & Krout criteria, treatment outcomes were excellent in 38 patients (58%), good in 16 (25%), fair in 7 (11%), and poor in 4 (6%), with an excellent and good rate of 83%. A negative correlation was found between the preoperative McGowan grade and the postoperative Wilson & Krout score. The patients having fair and poor treatment outcomes had more advanced age, lower nerve conduction velocity, and lower action potential amplitude compared with those having excellent and good treatment outcomes. These results suggest that anterior subcutaneous transposition of the ulnar nerve is effective and safe for the treatment of moderate to severe cubital tunnel syndrome, and initial severity, advancing age, and electrophysiological parameters can affect treatment outcome.
The necessity of ulnar nerve exploration and translocation in open reduction of medial humeral epicondyle fractures in children

Abstract:Aims: Medial humeral epicondyle fractures (MHEFs) are common elbow fractures in children. Open reduction should be performed in patients with MHEF who have entrapped intra-articular fragments as well as displacement. However, following open reduction, transposition of the ulnar nerve is disputed. The aim of this study is to evaluate the need for ulnar nerve exploration and transposition. Methods: This was a retrospective cohort study. The clinical data of patients who underwent surgical treatment of MHEF in our hospital from January 2015 to January 2022 were collected. The patients were allocated to either transposition or non-transposition groups. Data for sex, age, cause of fracture, duration of follow-up, Papavasiliou and Crawford classification, injury-to-surgery time, preoperative ulnar nerve symptoms, intraoperative exploration of ulnar nerve injury, surgical incision length, intraoperative blood loss, postoperative ulnar nerve symptoms, complications, persistent ulnar neuropathy, and elbow joint function were analyzed. Binary logistic regression analysis was used for statistical analysis. Results: A total of 124 patients were followed up, 50 in the ulnar nerve transposition group and 74 in the non-transposition group. There were significant differences in ulnar nerve injury (p = 0.009), incision length (p < 0.001), and blood loss (p = 0.003) between the two groups. Binary logistic regression analysis revealed that preoperative ulnar nerve symptoms (p = 0.012) were risk factors for postoperative ulnar nerve symptoms. In addition, ulnar nerve transposition did not affect the occurrence of postoperative ulnar nerve symptoms (p = 0.468). Conclusion: Ulnar nerve transposition did not improve clinical outcomes. It is recommended that the ulnar nerve should not be transposed when treating MHEF operatively.
Applied anatomical study of the vascularized ulnar nerve and its blood supply for cubital tunnel syndrome at the elbow region

Abstract:Cubital tunnel syndrome is often accompanied by paresthesia in ulnar nerve sites and hand muscle atrophy. When muscle weakness occurs, or after failure of more conservative treatments, anterior transposition is used. In the present study, the ulnar nerve and its blood vessels were examined in the elbows of 18 adult cadavers, and the external diameter of the nutrient vessels of the ulnar nerve at the point of origin, the distances between the origin of the vessels and the medial epicondyle of the humerus, and the length of the vessels accompanying the ulnar nerve in the superior ulnar collateral artery, the inferior ulnar collateral artery, and the posterior ulnar recurrent artery were measured. Anterior transposition of the vascularized ulnar nerve was performed to treat cubital tunnel syndrome. The most appropriate distance that the vascularized ulnar nerve can be moved to the subcutaneous tissue under tension-free conditions was 1.8 ± 0.6 cm (1.1-2.5 cm), which can be used as a reference value during the treatment of cubital tunnel syndrome with anterior transposition of the vascularized ulnar nerve.
Arthrolysis of posttraumatic elbow stiffness with assistance of external fixator

Abstract:Objective To investigate the value of external fixator in the treatment of posttraumatic elbow stiffness.Methods From January 2007 to December 2009,25 patients with posttraumatic elbow stiffness received surgical arthrolysis with assistance of external fixator.There were 10 patients with grade 1 stiffness,12 patients with grade 2 stiffness and three patients with grade 3 stiffness.The range of motion (ROM) was 0°-75° ( average 33.4°) preoperative,with ＜ 30° in 12 patients.For the patients with grade 1 stiffness,intraoperative distraction with an external fixator rather than open arthrolysis was performed ; for the patients with grade 2 stiffness,the heterotopic ossification was removed through a limited approach before the external fixator was applied; for the patients with grade 3 stiffness,additional osteotomy and internal fixation before arthrolysis were performed.After operation,there was a rehabilitation phase,followed by mobilization of the elbow joint under protection of the external fixator for a mean period of six weeks.Results The intraoperative mean ROM was 105.6°,which was significantly improved than that before operation (P ＜0.05).The maximal flexion degree was above 110°in all patients.No instability occurred.Two patients were excluded from the follow-up:one had ulna fracture two weeks after operation due to an incorrect placement of the pin,and additional internal fixation was then carried out;another patient had numbness in the ulna side when flexed the elbow after arthrolysis,and the ulna nerve transposition was performed through another operation.Other patients were followed up for a mean period of six months.At the latest follow-up,the mean ROM was 97.4°( range,70°-130°),significantly improved than that before operation (P ＜ 0.05 ),and lest 8.2°on average (0°-15 °) compared with the intraoperative ROM (P ＞ O.05).The maximal flexion in all patients was above 100° Conclusion With the advantages of minimal invasive or noninvasive,indirect capsular traction,stability maintenance during the movement,and prevention of the motion loss during the rehabilitation,arthrolysis of the elbow joint with the use of external fixation frame can be a good alternative in the treatment of posttraumatic elbow stiffness.
Clinical Analysis of Subcutaneous Anterior Transposition of the Ulnar Nerve in 135 Patients of Cubital Tunnel Syndrome

Abstract:Objective To analyze the clinical outcomes of subcutaneous anterior transposition of the ulnar nerve in 135 patients of cubital tunnel syndrome.Methods One hundred and thirty-five cases of cubital tunnel syndrome treated with subcutaneous anterior transposition of the ulnar nerve from February 2002 to December 2005 were reviewed.The manifestations and treatment outcomes were analyzed.Among 135 patients,26 were female and 109 male.The male to female ratio is 4.2:1.68.1% of patients were over 41 years old.Fifteen patients had symptoms of clumsiness of hand (11% ).Electrophysiologic examination unexceptionally showed ulnar nerve entrapment at the cubital tunnel.Forty-two cases had history of fractures at the elbow joint.This etiology accounted for 31% of the causes.All the patients were treated with neurolysis and subcutaneous anterior transposition of the ulnar nerve.Results Ninety-two patients were followed-up for 2 to 5 years with an average of 2.5 years.According to the grading system established by Chinese Medical Association,72 eases had excellent results,while 12 patients had good results.Results were rated as poor in 8 eases.The overall good-excellent rate was 91.3%.ConclusionSubcutaneous anterior transposition of the ulnar nerve is a simple and reliable method for treatment of cuibtal tunnel syndrome.It is critical to protect the medial antebrachial nerve and motor branches of the flexor carpi ulnaris and the blood supply of the ulnar nerve,and insure tension free for the rerouted ulnar nerve.Early-diagnosis and treatment of cases with complaint of hand clumsiness and combined elbow fracture should be highlighted.
Aretrospective study of revision cases after ulnar nerve anterior transposition at the elbow

Abstract:Objective To analyze the intraoperative exploration and postoperative follow-up of revision cases of ulnar nerve anterior transposition,to summarize the main reasons for poor initial surgical efficacy of ulnar nerve anterior transposition,as well as the key points and treatment effects of revision surgery.Methods A retrospective analysis was conducted on 37 patients who underwent revision surgery for ulnar nerve anterior transposition in our hospital from January 2008 to August 2019.The general case information,physical examination before and after revision surgery,auxiliary examinations(including neuroelectrophysiological examination and neuroultrasound),and patient self-evaluation scale results were collected,and intraoperative exploration status was recorded.Results All patients were follow-up for 4 to 18 months,with an average of(10.3±3.7)months.The main reasons for the poor initial surgical efficacy of ulnar nerve anterior transposition included the compression of scar tissue around the ulnar nerve,the incomplete removal of local compression factors,and the improper preparation of fascia flap.At the last follow-up after the revision surgery,ulnar numbness in the hand was alleviated in 25 cases(67.6％),ulnar pain in the hand and elbow joint was alleviated in 9 cases(81.8％),and atrophy of the hand muscles was alleviated in 18 cases(81.8％).According to Gu Yudong's criteria for cubital tunnel syndrome,the functional results were evaluated as excellent in 2 cases,good in 23 cases,fair in 10 cases,and poor in 2 cases,with an excellent and good rate of 67.6％.The possible related factors that affect the effectiveness of revision surgery include the time from the initial surgery to the revision surgery,and whether there is severe pain before the revision surgery.Conclusion The initial surgery for ulnar nerve anterior transposition should carefully and cautiously explore and remove all possible factors that may cause the ulnar nerve to become compressed again,and prepare appropriate fascia flaps for anterior transposition.For patients with poor postoperative outcomes,revision surgery can bring relatively good clinical efficacy.
Morphology and Surgical Treatment of Posterior Monteggia Fracture with Associated Ulnohumeral Dislocation

Abstract:Objective To investigate the morphological properties of posterior Monteggia lesion with associated elbow dislocation,to propose its injury mechanism,and to present its surgical methods and its outcomes.Methods The injury mechanism,radiographs and surgical records of patients with posterior Monteggia fractures and associated elbow dislocation were retrospectively reviewed from January 2011 to December 2013.11 patients were included,with 43.3 years old on average.10 were resulted from high-energy injuries.According to the Jupiter classification,Ⅱ A fracture-dislocation 9 cases,Ⅱ B fracture-dislocation 1 case and Ⅱ C fracture-dislocation 1 case.The general medical data,morphological properties,and the surgical methods.Functional outcomes were followed up.Results These cases had several intrinsic morphologic features:fractures of the coronoid tip and its anteromedial aspect;dislocation of the olecranon from the trochlear notch;fracture of the radial head and disruption of the lateral collateral ligament;a normal proximal radioulnar joint.Operation was carried out with emphasis on elbow stability restoration.All patients underwent the primary operations in the supine position and a routine posterior approach was used.An additional anterior approach was used in 1 case (Ⅱ B).The coronoid tip was first fixed,followed by the anteromedial coronoid fragment,the radial head,the olecranon,and then the LCL.An anatomic olecranon plate was used as the fundamental fixation device.Anteromedial coronoid fractures were mostly fixed with a T-shaped metacarpal plate.The coronoid tip fractures were fixed with screw,Kwire or suture,and the injured LCL was repaired with suture anchors.Ten of the 11 patients were followed up with a mean period of 22.7 months.The extension-flexion motion ranged from 45°-140° (104° average),and the pronation-supination motion ranged from 45° 180° (128° average).According to the Broberg and Morrey functional index,the excellent to good ratio was 70％.Conclusion Patients with posterior Monteggia fracture and associated ulnohumeral dislocation have small and comminuted coronoid fractures,and mostly have a normal PRUJ relationship.It might experience a different mechanism to those of the posterior Monteggia fracture without elbow dislocation.Emphases should be address to restore the rotation stability of the elbow when operation.
Operative Treatment of Terrible Triad of the Elbow with a Modified Pugh Standard Protocol

Abstract:Abstract The aim of the present study was to investigate the efficacy of the surgical treatment of terrible triad injuries of the elbow using a modified standard surgical approach, and treatment determined by the mode and mechanism of injury, and intraoperative findings. The study was a retrospective analysis of a prospective cohort of patients presenting with terrible triad injuries of the elbow, who underwent surgical treatment between July 2009 and January 2014. All patients were treated surgically according to a modified Pugh standard protocol. Elbow functional status and range of movements were evaluated at 3 months, 6 months, 1 year, and 2 years or more after surgery. Radiographic signs of post-traumatic arthritis were rated according to the Broberg and Morrey system. Our series included a final cohort of 29 males and 13 females with a mean age of 48.23 ± 10.95 years at the time of injury. The mean follow-up period was 30.47 ± 7.65 months. The mean flexion-extension arc was 107° ± 22°, and the average forearm rotation arc was 145° ± 14°. The mean Mayo Elbow Performance Score was 88 ± 10 points (range 55 to 100 points), with excellent results in 24 elbows, good results in 16, and poor results in 2. Functional results of the elbow improved significantly from 3 months postoperatively (P < .05), but tended to plateau from 1 year after surgery. Thirteen patients had radiographic signs of arthrosis (9 grade 1, 4 grade 2). Postoperative complications were local infection around the incision (n = 1), transient postoperative median nerve paralysis (n = 1), and postoperative posterior interosseous nerve paralysis (n = 1). Four patients required further surgery. Five patients had evidence of heterotopic ossification. Overall, we show that slight modifications to Pugh standard protocol for the surgical treatment of terrible triad injuries can lead to good to excellent results, although there were a good number of complications. The ideal surgical approach and treatment should be based on the mode and mechanism of injury, and intraoperative findings, and modifying the standard surgical treatment approach for terrible triad elbow injuries may be beneficial.
Modified anterior approach for treatment of ulnar coronoid process fracture

Abstract:Objective To investigate the clinical effect of modified anterior approach to manage fracture of the ulnar coronoid process via the space of brachial artery and vein with median nerve.Methods From June 2012 to January 2013,11 patients with ulnar coronoid fracture were fixed via the modified anterior approach.The operation time,intraoperative blood loss and postoperative complications were recorded.Flexion and rotation range of motion about the injured and normal elbow were observed during postoperative follow-up period.Function of elbow joint was evaluated by mayo elbow performance index (MEPI).Results There was approximate 8 cm in length and 5 cm in width between the brachial vessels and median nerve.Operated angle from radial to ulnar side was fifty degrees and from proximal to distal end was sixty degrees.All the patients were available for follow-up.The fracture healed,that is the elbow flexion restored [(130.7 ±5.0) °] was 96.6％ of the unaffected elbow,elbow extension restored [(7.6 ± 8.1) °] was 84.0％ of the unaffected elbow,pronation restored [(86.9 ± 3.8) °] was 98.2％ of the unaffected side,and supination restored [(85.6 ± 6.0) °] was 96.7％ of the unaffected side.MEPI of the elbow joint was over 75 points.Conclusion Modified anterior approach is relatively safe and simple in operation and results in satisfactory function recovery of the elbow joint,providing a new surgical approach for treatment of coronoid process fracture.
Early electrophysiological changes after using PDLLA film in the subcutaneous anterior transposition of ulnar nerve

Abstract:Obective To observe the early electrophysiological changes after using PDLLA film in the subcutaneous anterior transposition in ulnar nerve. Method We collected 34 patient with cubital tunnel syndrome. 10 of them had syndromes which reflected the compression of ulnar nerve after trauma of their elbows , 24 patients had syndromes with no obvious cause. All of the patients underwent the decompression of ulnar nerve at elbow and subcutaneous anterior transposition of the nerve. In the operation, nerves of 19 cases were wrapped by a PDLLA film. The follow-up time ranged from 2 months to 15 months. The follow-up content was to collect elcetromyogram of the patients. We compared the nerve conduction velocities from 5cm above elbow to 5cm below elbow and the wave amplitudes of CMAP which were educed in abductor digiti minimi after stimulating the ulnar nerve at elbow before and after the operation. Results The nerve conduction velocities from 5cm above elbow to 5cm below elbow had improvement in both groups. There was 1 in the control group and 3 in the observed group that CMAP can’t be educed in the abductor digiti minimi, and the nerve conduction velocities were 0 before operation. These patients had certain improvements after operations. The rest patients showed that: in the control group, MNCV improved about 43% averagely; in the observed group, MNCV improved about 58.7%. However, there was no statistically differences between them totally. The amplitudes of CMAP which were educed in abductor digiti minimi improved about 54.6% averagely in the control group, while it improved about 108.3% in the observed group. Totally, there was statistical differences between two groups in this aspect. Conclusion PDLLA film had some electrophysiological effect in the early period of subcutaneous anterior transposition in ulnar nerve.
Subcutaneous Anterior Transposition With Modified Procedures to Prevent Recurrence Associated With the Transposed Ulnar Nerve

Abstract:Subcutaneous anterior transposition of the ulnar nerve is a common surgical treatment for cubital tunnel syndrome. However, there are surgical failures associated with the new compressive sites at the edge of flexor carpi ulnaris (FCU) and resubluxation posterior to the medial epicondyle of the transposed nerve. To reduce the muscle volume at the edge of FCU, we approach the ulnar nerve by dividing the muscle belly of the FCU humeral heads. This procedure can reduce repeated traction forces on the transposed nerve at the edge of the FCU. To keep the transposed ulnar nerve anteriorly, we use a fat flap including the membranous superficial fascia. This flap can softly stabilize the ulnar nerve and act as a pliable cover to prevent perineural scarring or further constriction around the flap. Ninety-three elbows in 90 patients who had undergone this procedure for cubital tunnel syndrome were evaluated. According to Messina's criteria, the numbers of patients showing excellent, good, fair, and poor recovery were 41 (44%), 47 (51%), 5 (5%), and 0 (0%), respectively. Most patients experience resolution of symptoms and good functional outcomes. None of the patients suffered recurrence, infection, or nerve injury.
Clinical Effect Observation of Vascularized Anterior Transposition of Ulnar Nerve for Treatment of Severe Cubital Tunnel Syndrome

Abstract:Objective To evaluate the clinical outcomes of a modified procedure of ulnar nerve anterior transposition in the treatment of severe cubital tunnel syndrome.Methods From September 2015 to May 2016,25 patients with cubital tunnel syndrome underwent vascularized ulnar nerve anterior transposition were followed up after operation.All the patients had severe muscular atrophy and deformity of elbow joint before operation.According to Gu Yudong's classification of cubital tunnel syndrome,they were graded as severe.In this modified procedure,the sufficient neurolysis of ulnar nerve as well as the anterior transposition of ulnar nerve accompanied with superior ulnar collateral artery were conducted.Results All the 25 patients were follow-up from 12 to 21 months with an average of 15.3 months.All the patients achieved sensory recovery in different degrees without temporary worsening of clinical symptoms.Completely sensory recovery was observed in 20 patients (80％).4 patients (16％) remained occasional sensory symptoms while only 1 patient (4％) remained persistently moderate sensory symptoms.The mean two-point discrimination of little finger pulp was improved from preoperative 6.08 mm to postoperative 2.93 mm.Obvious improvement of intrinsic muscular atrophy was observed in 17 patients (68％).According to the Gu Yudong's function evaluation standard of cubital tunnel syndrome,the results were excellent in 16 patients,good in 6 patients and fair in 3 patients with the excellent and good rate being 88％.Conclusion The modified technique of vascularized anterior transposition of unlar nerve appears to be an effective and safe procedure for the treatment of severe cubital tunnel syndrome.It might reduce the incidence of postoperative nerve symptoms caused by devascularization of ulnar nerve.
Distraction Arthrolysis of Posttraumatic Elbow Stiffness with a Hinged External Fixator.

Abstract:The treatment of elbow stiffness remains a challenge for orthopedic surgeons. A hinged external fixator with distraction ability has recently emerged as a new option in the surgical treatment of elbow stiffness. Between January 2007 and December 2009, twenty-five posttraumatic stiff elbows (mean patient age, 29.2 years) received distraction arthrolysis from 1 surgeon group (W.J.W., L.H., P.Z.J.) at the authors' institution. For patients with only periarticular soft tissue contracture (grade 1), close mechanical distraction was performed with the assistance of an external fixator under anesthesia; open arthrolysis was avoided as much as possible. For patients who also had heterotopic ossification (grade 2), it was removed through a limited approach before the external fixator was applied. For patients with osteoarticular surface damage or destruction (grade 3), osteoarticular integrity was restored before arthrolysis. Range of motion increased markedly, from 33.4° (range, 0°-75°) preoperatively to 105.6° (range, 80°-140°) immediately postoperatively (Student's t test, P<.05). Of the 25 patients, 23 were followed for a mean of 16 months. No serious complications occurred. Mean range of motion was 97.4° (range, 70°-130°) at final follow-up, a significant increase from preoperatively (Student's t test, P<.05). No significant loss of range of motion was found at final follow-up compared with intraoperative values (average 8.2° loss; Student's t test, P>.05). This study suggests that a hinged external distraction fixator is a less invasive option for treating posttraumatic elbow stiffness and prevents contracture recurrence after arthrolysis.
Ulnar nerve decompression and transposition with versus without supercharged end-to-side motor nerve transfer for advanced cubital tunnel syndrome: a randomized comparison study

Abstract:OBJECTIVE The objective of this prospective randomized study was to compare ulnar nerve decompression and anterior subfascial transposition with versus without supercharged end-to-side anterior interosseous nerve–to–ulnar motor nerve transfer for advanced cubital tunnel syndrome, to describe performing the nerve transfer through a small incision, and to investigate predictive factors for poor recovery following the procedure. METHODS Between January 2013 and October 2016, 93 patients were randomly allocated to a study group (n = 45) and a control group (n = 48). Patients in the study group were treated with supercharged motor nerve transfer via a 5-cm incision following decompression and anterior subfascial transposition. Patients in the control group were treated with decompression and anterior subfascial transposition alone. Postoperative pinch strength and compound muscle action potential amplitude (CMAPa) were assessed. Function of the limb was assessed based on the Gabel/Amadio scale. Between-group data were compared, and significance was set at p < 0.05. Potential risk factors were collected from demographic data and disease severity indicators. RESULTS At the final follow-up at 2 years, the results of the study group were superior to those of the control group with regard to postoperative pinch strength (75.13% ± 7.65% vs 62.11% ± 6.97%, p < 0.05); CMAPa of the first dorsal interossei (17.17 ± 5.84 mV vs 12.20 ± 4.09 mV, p < 0.01); CMAPa of abductor digiti minimi (11.57 ± 4.04 mV vs 8.43 ± 6.11 mV, p < 0.01); and excellent to good results (0.67 for the study group vs 0.35 for the control group, p < 0.05). Multivariate analysis showed that the advanced age (OR 2.98, 95% CI 2.25–4.10; p = 0.003) in the study group was related to unsatisfactory outcome in the patients. CONCLUSIONS In the treatment of advanced cubital tunnel syndrome, additional supercharged end-to-side anterior interosseous nerve–to–ulnar motor nerve transfer may produce a better function of the hand. The authors also found that cases in the elderly were related to unsatisfactory postoperative results for these patients and that they could be informed of the possibility of worsening surgery results.

Risk Factors for the Occurrence and Progression of Posttraumatic Elbow Stiffness: A Case-Control Study of 688 Cases

Abstract:Background: Elbow stiffness is a severe complication after trauma. Surgical or conservative treatments may be ineffective for restoring functional elbow motion. We aim to evaluate intrinsic and extrinsic factors for the occurrence and severity of elbow stiffness. Methods: This retrospective case–control study included mild/moderate stiffness, severe stiffness, and non-stiffness groups between January 2011 and December 2017 at a single orthopedic center. Multivariable logistic regression analysis and subgroup analysis were used to evaluate age, gender, body mass index, muscle strength, fracture type and site, injury mechanism, immobilization time, elbow dysfunction time, multiple surgeries, nerve symptoms, physical therapy, smoking and alcohol abuse, and dominant hand of stiff elbow as potential risk factors for the occurrence and severity of elbow stiffness. Results: There were 461 patients in the stiffness group and 227 patients in the non-stiffness group. The odds ratios (ORs) of the age, muscle strength, and injury mechanism were 0.960, 0.333, and 0.216 for the occurrence of elbow stiffness. In subgroup evaluation, increased cast immobilization time might be a risk factor for patients receiving conservative therapies (OR = 2.02; p = 0.014). In the evaluation on factors for progression of elbow stiffness, “multiple surgeries” might be a risk factor in surgical treatment by subgroup analysis (OR = 1.943; p = 0.026). Nevertheless, alcohol abuse might increase severity of elbow stiffness in conservatively treated patients (OR = 3.082; p = 0.025). Conclusion: Increased cast immobilization time in the conservative therapy might be a risk factor for stiffness occurrence. Multiple surgeries might be risk factors for stiffness progression. Alcohol abuse potentially increased stiffness severity after conservative treatment.

A Clinical Efficacy Assessment of Simple Decompression of Cubital Tunnel Syndrome

Abstract:Objective To assess the clinical efficacy via a follow-up study of 53 cases of cubital tunnel syndrome with a procedure of simple decompression of ulnar nerve in order to provide a theoretical basis on how to choose an appropriate type of procedure for the patient with cubital tunnel syndrome.Methods The clinical efficacy of the procedure of simple decompression of ulnar nerve was evaluated from the preoperative condition of different severity,preoperative duration of different length via a follow-up study of 53 cases.The difference of the clinical efficacy was statistically analyzed by comparison with 134 cases treated by a procedure of subcutaneous anterior transposition of the ulnar nerve.Results There were no statistically significant differences of clinical efficacy between the procedure of simple decompression and anterior subcutaneous transposition when there was no deformity at the elbow joint.The clinical efficacy was not related with the degree of preoperative severity and preoperative duration,but related with the age of the patients.Conclusion Simple decompression and subcutaneous anterior transposition lead to similar clinical efficacy when there was no deformity at elbow joint.
Novel Technique for Ulnar Nerve Transposition at the Elbow: The Neocubital Tunnel

Abstract:Background: Many standard surgical procedures for cubital tunnel syndrome rely on ulnar nerve transposition at the elbow. Placing the ulnar nerve anterior to the axis of motion decreases compression during flexion. Subcutaneous, subfascial, and submuscular positioning of the ulnar nerve may predispose to nerve irritation, instability, and compression in the two first scenarios, and requires invasive dissection in the third one. With no single procedure demonstrating clear advantages and outcomes, this study reports the results of a novel technique using the epitrochleo-olecranon ligament to create a neo-tunnel, anatomically stabilizing the ulnar nerve. Methods: Nine consecutive patients were enrolled. Patients were evaluated qualitatively for symptomatic improvements using physical examination. The Wilson and Krout, modified McGowan, and PRUNE grading scores were used for quantitative measurement. Results: Postoperatively, all patients reported subjective improvement in symptoms and functional improvement. There were no intraoperative or postoperative complications. Baseline severity of disease was evaluated using the McGowan scale (modified by Goldberg): eight (89%) grade IIA patients and one (11%) grade III patient. Postoperatively, seven (78%) patients were reduced to grade 0, and two (22%) patients to grade 1 (P < 0.001). Using the Wilson and Krout criteria, outcomes were as follows: six (67%) excellent, two (22%) good, and one (11%) fair. The mean postoperative PRUNE survey score was 19.3 (SD ± 24.4). Conclusions: The neocubital tunnel technique is a relatively noninvasive, safe surgical alternative that may be considered when an anterior transposition of the ulnar nerve is indicated for surgical decompression of the cubital tunnel.
