Abstract:The early detection of mild cognitive impairment, a condition of increasing impact in our aging society, is a challenging task with no established answer. One promising solution is the deployment of robotic systems and ambient assisted living technology in the houses of older adults for monitoring and assistance. In this work, we address and discuss a qualitative analysis on the feasibility and acceptability of a socially assistive robot (SAR) deployed in prospective users' houses to monitor their cognitive capabilities through a set of digitalised neuropsychological tests and spot questions conveniently integrated within the robotic assistant's daily tasks. We do this by describing an experimental campaign where a robotic system, integrated with a larger framework, was installed in the house of 10 users for a duration of at least 10 weeks, during which their cognitive capabilities were monitored by the robot. Concretely, the robots supervised the users during the completion of the tests and transparently monitored them by asking questions interleaved in their everyday activities. Results show a general acceptance of such technology, being able to carry out the intended tasks without being too invasive, paving the way for an impactful at-home use of SARs.

Abstract:Social robots are coming to our homes and have already been used to help humans in a number of ways in geriatric care. This article aims to develop a framework that enables social robots to conduct regular clinical screening interviews in geriatric care, such as cognitive evaluation, falls' risk evaluation, and pain rating. We develop a social robot with essential features to enable clinical screening interviews, including a conversational interface, face tracking, an interaction handler, attention management, robot skills, and cloud service management. Besides, a general clinical screening interview management (GCSIM) model is proposed and implemented. The GCSIM enables social robots to handle various types of clinical questions and answers, evaluate and score responses, engage interviewees during conversations, and generate reports on their well-being. These reports can be used to evaluate the progression of cognitive impairment, risk of falls, pain level, and so on by caregivers or physicians. Such a clinical screening capability allows for early detection and treatment planning in geriatric care. The framework was developed and implemented on our 3-D-printed social robot. It was tested on 30 older adults with different ages, achieved satisfying results, and received their high confidence and trust in the use of this robot for human well-being assessment. Note to Practitioners—This article is motivated by the goal of using a social robot to perform geriatric well-being assessment through clinical screening interviews. In order to conduct clinical screening interviews, the social robot needs the following essential features: having a verbal conversational interface, adapting to different types of clinical screening interviews, scoring and evaluating answers, having nondirective listening responses, and enabling directive listening responses. The proposed general clinical screening interview management (GCSIM)-model demonstrates these capabilities on the social robot. The robot can give structured clinical screening interviews with different question–answer sheets. This will help advance assistive technologies for use by geriatric physicians, nurses, and social service professionals to keep older adults healthy, safe, and independent at home. Robots will become more and more essential in working alongside geriatric practitioners to help monitor older adults at home and to provide early detection and warning of cognitive/mental health problems, falls' risk, and so on. This early detection property can improve quality-of-care and help older adults remain living at home.

Abstract:Abstract Social Assistive Robotics is increasingly being used in care settings to provide psychosocial support and interventions for the elderly with cognitive impairments. Most of these social robots have provided timely stimuli to the elderly at home and in care centres, including keeping them active and boosting their mood. However, previous investigations have registered shortcomings in these robots, particularly in their ability to satisfy an essential human need: the need for companionship. Reports show that the elderly tend to lose interests in these social robots after the initial excitement as the novelty wears out and the monotonous familiarity becomes all too familiar. This paper presents our research facilitating conversations between a social humanoid robot, Nadine, and cognitively impaired elderly at a nursing home. We analysed the effectiveness of human–humanoid interactions between our robot and 14 elderly over 29 sessions. We used both objective tools (based on computer vision methods) and subjective tools (based on observational scales) to evaluate the recorded videos. Our findings showed that our subjects engaged positively with Nadine, suggesting that their interaction with the robot could improve their well-being by compensating for some of their emotional, cognitive, and psychosocial deficiencies. We detected emotions associated with cognitively impaired elderly during these interactions. This study could help understand the expectations of the elderly and the current limitations of Social Assistive Robots. Our research is aligned with all the ethical recommendations by the NTU Institutional Review Board.

Abstract:Worldwide, there are approximately 10 million new cases of dementia reported each year. Due to impairments in cognitive functioning such as loss of memory, linguistic abilities, and problem-solving skills, persons living with dementia (PLWDs) may face challenges in accomplishing activities of daily living (ADLs). Socially assistive robotics (SAR) holds promise as an effective tool to assist PLWDs with their ADLs; however, developing a powerful, reliable SAR requires a robust dataset of PLWDs performing ADLs to ensure the efficacy and reliability of the SAR. In this paper, we present the development of a teleoperated robotic platform using a humanoid social robot. This platform provided instructions to perform ten Performance Assessment of Self-care Skills (PASS) tasks, while multimodal sensors were employed to collect the individual's physiological signals, behaviors, and interactions with the robot. We conducted a preliminary study to investigate the acceptability, user experience, and feasibility of the platform in a simulated home environment with five young cognitively normal individuals. Participants' experiences and satisfaction with the platform were evaluated through a questionnaire. The results demonstrate the capability of the robot administering PASS tasks. However, one limitation of the current platform is the low efficiency of controlling the robot to move in the Smart Home. Our next step is to implement navigational abilities in the robot and then conduct the experiments with a large cohort of PLWDs to generate our multimodal dataset. This work aims to contribute significantly towards the development of a powerful, reliable, and trustworthy SAR to help PLWDs with their ADLs.

Abstract:Sustained attention is essential for older adults to maintain an active lifestyle, and the deficiency of this function is often associated with health-related risks such as falling and frailty. The present study examined whether the well-established age-effect on reducing mind-wandering, the drift to internal thoughts that are seen to be detrimental to attentional control, could be replicated by using a robotic experimenter for older adults who are not as familiar with online technologies. A total of 28 younger and 22 older adults performed a Sustained Attention to Response Task (SART) by answering thought probes regarding their attention states and providing confidence ratings for their own task performances. The indices from the modified SART suggested a well-documented conservative response strategy endorsed by older adults, which were represented by slower responses and increased omission errors. Moreover, the slower responses and increased omissions were found to be associated with less self-reported mind-wandering, thus showing consistency with their higher subjective ratings of attentional control. Overall, this study demonstrates the potential of constructing age-related cognitive profiles with attention evaluation instruction based on a social companion robot for older adults at home.

Abstract:The ageing population phenomenon is pushing the design of innovative solutions to provide assistance to the elderly. In this context a socially–assistive robot can act as a proactive interface in a smart-home environment, providing multimodal communication channels and generating positive feelings in users. The present paper reports results of a short term and a long term evaluation of a small socially assistive humanoid robot in a smart home environment. Eight elderly people tested an integrated smart–home robot system in five real–world scenarios. Six of the participants experienced the system in two sessions over a two week period; the other two participants had a prolonged experience of eight sessions over a three month period. Results showed that the small humanoid robot was trusted by the participants. A cross–cultural comparison showed that results were not due to the cultural background of the participants. The long term evaluation showed that the participants might engage in an emotional relationship with the robot, but that perceived enjoyment might decrease over time.

Evaluation of a companion robot based on field tests with single older adults in their homes

Abstract:The growing number of older adults places insurmountable load on family members and professional caregivers. Assistive technology with the aid of robots can present a possible solution. The goal of this article was to test a companion robot supporting older adults in their home environments. Eight senior volunteers were involved in the field test. They all lived alone and were self-supporting. The robot was capable of providing cognitive assistance to manage the user's daily routine. Each participant used the robot for 94.9 ± 19.6 days. The primary communication modality was voice communication in their natural language, while the touch screen display was also available for interaction with the robot. The assistance given in daily living was evaluated by subjective (the user's opinion) and objective (logged data) criteria. The most useful and the least reliable robot functions according to the users were the navigation and the verbal communication. Entertainment, locomotion, and weather forecast were the most frequently used functions, while the shopping list was the least popular. The companion robot used in the test was accepted enthusiastically by the senior subjects. Specific robot functions (mainly navigation in the apartment and the speech recognition) require improvement to better accommodate real circumstances.
Identifying Mild Cognitive Impairment by Using Human-Robot Interactions

Abstract:Background: Mild cognitive impairment (MCI), which is common in older adults, is a risk factor for dementia. Rapidly growing health care demand associated with global population aging has spurred the development of new digital tools for the assessment of cognitive performance in older adults. Objective: To overcome methodological drawbacks of previous studies (e.g., use of potentially imprecise screening tools that fail to include patients with MCI), this study investigated the feasibility of assessing multiple cognitive functions in older adults with and without MCI by using a social robot. Methods: This study included 33 older adults with or without MCI and 33 healthy young adults. We examined the utility of five robotic cognitive tests focused on language, episodic memory, prospective memory, and aspects of executive function to classify age-associated cognitive changes versus MCI. Standardized neuropsychological tests were collected to validate robotic test performance. Results: The assessment was well received by all participants. Robotic tests assessing delayed episodic memory, prospective memory, and aspects of executive function were optimal for differentiating between older adults with and without MCI, whereas the global cognitive test (i.e., Mini-Mental State Examination) failed to capture such subtle cognitive differences among older adults. Furthermore, robot-administered tests demonstrated sound ability to predict the results of standardized cognitive tests, even after adjustment for demographic variables and global cognitive status. Conclusion: Overall, our results suggest the human-robot interaction approach is feasible for MCI identification. Incorporating additional cognitive test measures might improve the stability and reliability of such robot-assisted MCI diagnoses.

Abstract:Social robots represent a valid opportunity to manage the diagnosis, treatment, care, and support of older people with dementia. The aim of this study is to validate the Mini-Mental State Examination (MMSE) test administered by the Pepper robot equipped with systems to detect psychophysical and emotional states in older patients. Our main result is that the Pepper robot is capable of administering the MMSE and that cognitive status is not a determinant in the effective use of a social robot. People with mild cognitive impairment appreciate the robot, as it interacts with them. Acceptability does not relate strictly to the user experience, but the willingness to interact with the robot is an important variable for engagement. We demonstrate the feasibility of a novel approach that, in the future, could lead to more natural human–machine interaction when delivering cognitive tests with the aid of a social robot and a Computational Psychophysiology Module (CPM).

Abstract:The use of robots in elderly care represents a dynamic field of study aimed at meeting the growing demand for home-based health care services. This article examines the application of robots in elderly home care and contributes to the literature by introducing a comprehensive and functional architecture within the realm of theInternet of Robotic Things (IoRT). This architecture amalgamates robots, sensors, and Artificial Intelligence (AI) to monitor the health status of the elderly. This study presented a four-actor system comprising a stationary humanoid robot, elderly individuals, medical personnel, and caregivers. This system enables continuous monitoring of the physical and emotional well-being of the elderly through specific sensors that measure vital signs, with real-time updates relayed to physicians and assistants, thereby ensuring timely and appropriate care. Our research endeavors to develop a fully integrated architecture that seamlessly integrates robots, sensors, and AI, enabling comprehensive care for elderly individuals in the comfort of their homes, thus reducing their reliance on institutional hospitalization. In particular, the methodology used was based on a user-centered approach involving geriatricians from the outset. This has been of fundamental importance in assessing their receptivity to the adoption of an intelligent information system, and above all, in understanding the issues most relevant to the elderly. The humanoid robot is specifically designed for close interaction with the elderly, capturing vital signs, emotional states, and cognitive conditions while providing assistance in daily routines and alerting family members and physicians to anomalies. Furthermore, communication was facilitated through an external Telegram bot. To predict the health status of the elderly, a machine learning model based on the Modified Early Warning Score (MEWS), a medical scoring scale, was developed. Five key lessons emerged from the study, showing how the system presented can provide valuable support to physicians, caregivers, and older people.

A Care Robot with Ethical Sensing System for Older Adults at Home

Abstract:Many studies have explored emotional and mental services that robots can provide for older adults, such as offering them daily conversation, news, music, or health information. However, the ethical issues raised by using sensors for frail older adults to monitor their daily movements or their medication intake, for instance, are still being discussed. In this study, we develop an older adult-guided, caregiver-monitored robot, Dori, which can detect and recognize movement by sensing human poses in accordance with two factors from the human-centered artificial intelligence (HCAI) framework. To design the care robot's services based on sensing movement during daily activities, we conducted focus group interviews with two groups-caregivers and medical staff-on the topic of care robot services not for patients but for prefrail and frail elderly individuals living at home. Based on their responses, we derived the focal service areas of cognitive support, emotional support, physical activity support, medication management, and caregiver management. We also found the two groups differed in their ethical judgments in the areas of dignity, autonomy, controllability, and privacy for services utilizing sensing by care robots. Therefore, the pose recognition technology adopted in the present work uses only joint coordinate information extracted from camera images and thus is advantageous for protecting human dignity and personal information.
Towards Cognitive Robots That People Accept in Their Home

Abstract:It is intractable for assistive robots to have all functionalities pre-programmed prior to deployment. Rather, it is more realistic for robots to perform supplemental, on-site learning about user's needs and preferences, and particularities of the environment. This additional learning is especially helpful for care robots that assist with individualized caregiver activities in residential or assisted living facilities. As each care receiver has unique needs and those needs may change over time, robots require the ability to adapt and learn on-site. In this work, we propose the study design to investigate the impacts on end-users of observing robot learning. We will assess user attitudes towards robots that conduct some learning in the home as compared to a baseline condition where the robot is delivered fully capable. We will additionally compare different modes of learning to determine whether some are more likely to instill trust.

Abstract:Social robots are becoming an integrated part of our daily life due to their ability to provide companionship and entertainment. A subfield of robotics, Socially Assistive Robotics (SAR), is particularly suitable for expanding these benefits into the healthcare setting because of its unique ability to provide cognitive, social, and emotional support. This paper presents our recent research on developing SAR by evaluating the ability of a life-like conversational social robot, called Ryan, to administer internet-delivered cognitive behavioral therapy (iCBT) to older adults with depression. For Ryan to administer the therapy, we developed a dialogue-management system, called Program-R. Using an accredited CBT manual for the treatment of depression, we created seven hour-long iCBT dialogues and integrated them into Program-R using Artificial Intelligence Markup Language (AIML). To assess the effectiveness of Robot-based iCBT and users' likability of our approach, we conducted an HRI study with a cohort of elderly people with mild-to-moderate depression over a period of four weeks. Quantitative analyses of participant's spoken responses (e.g. word count and sentiment analysis), face-scale mood scores, and exit surveys, strongly support the notion robot-based iCBT is a viable alternative to traditional human-delivered therapy.

Abstract:Eldercare is one of the most important healthcare concerns, particularly in countries whose populations are rapidly aging. This paper proposes the use of a robot system to improve the quality of life of elderly people in two ways, first by monitoring their home care services using image and sound sensors in the robots, and the second by assisting elders by understanding social situations using face authentication technology. This paper presents a concrete set of security policies to protect the privacy of the care-receiver of such home care services. An IT system architecture is also presented to monitor the data in a secure manner. Experimental results show the effectiveness and the practicality of the proposed robot system for typical home care services.

Abstract:Socially assistive robots are increasingly being used to support the social, cognitive, and physical well-being of those who provide care (healthcare professionals) and those in need of care (older adults). However, the effectiveness of persuasive socially assistive robot behaviors and their impact on the sustained motivation of older adults is still not well understood. This extended abstract describes our prior human-robot interaction study on investigating the effectiveness of persuasive social robot behaviors with care providers, followed by our current research assessing the impact of these persuasive robot behaviors on the well-being of older adults in long-term care. The findings provide insights into engagement and sustained motivation of older adults when providing assistance.

Abstract:Cognitive impairment has become epidemic in older adult population. The recent advent of tiny wearable and ambient devices, a.k.a Internet of Things (IoT) provides ample platforms for continuous functional and cognitive health assessment of older adults. In this paper, we design, implement and evaluate AutoCogniSys, a context-aware automated cognitive health assessment system, combining the sensing powers of wearable physiological (Electrodermal Activity, Photoplethysmography) and physical (Accelerometer, Object) sensors in conjunction with ambient sensors. We design appropriate signal processing and machine learning techniques, and develop an automatic cognitive health assessment system in a natural older adults living environment. We validate our approaches using two datasets: (i) a naturalistic sensor data streams related to Activities of Daily Living and mental arousal of 22 older adults recruited in a retirement community center, individually living in their own apartments using a customized inexpensive IoT system (IRB #HP-00064387) and (ii) a publicly available dataset for emotion detection. The performance of AutoCogniSys attests max. 93\% of accuracy in assessing cognitive health of older adults.

We provide an overview of various actual potential applications at home in the literature concerning social robots and virtual humans. In addition, we report our latest research progress in our institute, i.e., Social communication, falling-down detection, affect computing, attention capture. All the research and applications demonstrate that the social robots and virtual humans can provide a better daily life in home environment.
EFFECTS OF COMPANION ROBOTS ON DEPRESSION AND SOCIAL ISOLATION AMONG KOREAN AMERICAN OLDER ADULTS

Abstract:Abstract Objectives During the COVID-19, older immigrants in low resource areas who are already excluded from much of social life, had to distance themselves even further, deepening their social isolation. Lack of stimulation can be particularly detrimental, as it influences mood and increases loneliness. Selected for cost-effectiveness and interactive features, we introduced the Companion Robot (CR) system. Preliminary studies conducted in South Korea found statistically significant improvement in quality of sleep and cognition among low-income older patients living alone with multiple chronic conditions and cognitive impairment. Method. Thirty CRs were deployed to solo-living Korean American older adults. We followed up with them after 3 months. In this Stage 1A clinical trial, we tested feasibility, usability and acceptability of CRs, investigating which features of CRs will help improve health behaviors such as medication adherence and daily exercises. Robotic Social Attitude (RoSAS) are assessed with a generic 18-items questionnaire (Krägeloh et al., 2019). Results The primary clinical outcomes measured at pre-and post-intervention were medication adherence and reduction of depression, social isolation and disabilities. The user-friendly doll-shaped design of CRs helped participants feel comfortable interacting with the CR on their own. Exposure to individualized CR-delivered content, including quizzes for brain training and inspirational messages, combined with consistent interaction with CR appeared to be beneficial for cognitive function. Discussion Since very few participants had reported diagnosis of dementia, we need to screen for undetected dementia using the Modified Mini-Mental Status Exam. The next stage is to implement further cultural adaptation for the U.S. participants.

Twenty-three institutionalized persons (60% women; average age 89; average MMSE score of 20.8) with Alzheimer Disease (AD) were recruited and invited to participate in prospective memory exercise sessions, conducted either by a caregiver or by a robot (assisted by a caregiver). They were divided into two groups equivalent in age, level of education and MMSE score. In addition, the sessions were recorded in order to compare the interaction behaviors of the 2 groups, using a validated observation grid. The results showed that: 1) prospective memory tasks are better performed when offered by the caregiver; 2) when strong help linked to the recovery index is provided to perform the tasks, the robot or caregiver no longer show significant differences; 3) participants interact more with the caregiver than with the robot. Our results confirm that the use of companion robots is a promising way to help caregivers manage the daily activities of people with Alzheimer's. However, to optimize this assistance, further investigations should be conducted to improve the fluidity of interactions between the patient and the robot.
