Abstract:Objective The study aimed to evaluate the contributions of levator ani muscle (LAM) injury, vesical neck movement, urethral length and mobility, and urethral sphincter dysfunction observed on magnetic resonance imaging (MRI) towards stress urinary incontinence (SUI) after vaginal delivery. Materials and Methods Fifty primiparous women after 6 months of delivery (15 with SUI and 35 without) and 35 nulliparous as continent controls underwent MRI at rest and Valsalva maneuver. A published levator ani scoring system was used to characterize morphological changes of LAM. The severity of the injury was divided into three categories as none, minor, and major. A series of common parameters including levator plate angle, iliococcygeal angle, and levator hiatus were used to describe the functional conditions of LAM. Urethral mobility was defined based on the rotation of the urethra between Valsalva and rest status. Vesical neck movement was evaluated by its distance to the pubococcygeal line. Urethral sphincter dysfunction was defined as the widening of the proximal urethra and/or funneling at the urethrovesical junction during Valsalva. Results Primiparous incontinent (PI) women had additional major levator ani defects (33.3% vs. 17.1%) while less minor defects (0.7% vs. 31.4%) than primiparous continent (PC) women. Vesical neck downward movement in PI women was more obvious than PC women (28.5 mm vs. 24.2 mm, p = 0.006). Urethral mobility was more active in primiparous women than in nulliparous continent controls (57.4 vs. 52.4), whereas no difference was observed on urethral mobility in the primiparous group (p = 0.25). Urethral sphincter dysfunction and funneling were present in 80% of PI women versus 22.9% in PC women (p < 0.001). Conclusion The MRI findings revealed that de novo SUI was associated with major LAM injury, vesical neck downward movement as well as urethral sphincter dysfunction. Vesical neck funneling on sagittal images can be treated as a valuable predictor for SUI. The intervention for the PI should focus on the elevation of vesical neck, rehabilitation of LAM as well as recovery of the urethral sphincter muscle.
Three-dimensional Pelvic Floor Ultrasound Manifestations of Postnatal Early Stress Urinary Incontinence

Abstract:Purpose To observe the shape of postnatal early female pelvic diaphragmatic hiatus and the mobility of lower urinary tract bladder neck with three-dimensional transperineal ultrasound,and to analyze the related parameters of postnatal early stress urinary incontinence (SUI) to screen SUI early after delivery and provide objective indices for pelvic floor rehabilitation.Materials and Methods 170 primiparae,including 13 cases of SUI group and 157 cases of asymptomatic group,were operated pelvic floor ultrasonic examination,human pelvic floor midsagittal sections and pelvic diaphragmatic hiatus rebuilding images were obtained,and such related biological parameters as bladder neck mobility and pelvic diaphragmatic hiatus indicators were determined.Results The incidence of SUI at 6-8 weeks after delivery was 4.55％,and that after vaginal delivery was 9.61％,the difference was not statistically significant (P＞0.05).The lower migration of the bladder neck (1.83±0.54) cm on Valsalva maneuver,and transverse diameter (421±0.73) cm,vertical diameter (5.89±0.97) cm and area (20.38±6.30) cm2 of pelvic diaphragmatic hiatus in SUI group were greater than those in the asymptomatic group (P＜0.05),and the difference between parameters of the two groups in resting state and anal shrinkage was of no statistical significance (P＞0.05).Conclusion The shape of urethra,lower migration of bladder neck and the parameters of pelvic diaphragmatic hiatus on Valsalva maneuver are sensitive indicators in ultrasonic evaluation of SUI,are conductive to screening of SUI after early delivery in combination with patient symptoms and can guide the high risk puerpera for early pelvic floor rehabilitation.

Abstract:OBJECTIVE:Insensible Urinary Incontinence (IUI) is a situation when you complain of urinary incontinence but are unaware of how it occurred. Therefore, it is necessary to apply highly specific diagnostic methods to promote accuracy in the diagnosis of IUI, including pelvic floor ultrasound (PFU) and urodynamic studies (UDS).METHODS:A total of 41 women with IUI were retrospectively included. Patients were categorized into two groups: the urodynamic urinary incontinence group (UUI group, n=20) and the non-urodynamic urinary incontinence group (NUUI group, n=21), according to the urine leakage during UDS. The baseline clinical characteristics, UDS results, and PFU parameters were collected.RESULTS:Compared with the NUUI group, the UUI group had a smaller maximum cystometric capacity (P=0.008), lower maximum urethral closure pressure (P=0.005), shorter functional urethral length (FUL) (P=0.01), more bladder neck funneling (BNF) (P=0.02), greater BNF depth (P=0.04), and larger BNF area (P=0.01). The area and depth of BNF were negatively correlated with maximum urethral closure pressure (r=-0.42, P=0.01), FUL (r=-0.36, P=0.02 versus r=-0.39, P=0.01), and maximum cystometric capacity (r=-0.35, P=0.03), but positively correlated with maximum urinary flow rate (r=0.33, P=0.04 versus r=0.36, P=0.02). The canonical correlation analysis of the ultrasound parameters and UDS parameters shows that the first pair of canonical variables was statistically significant (r1=0.9, P<0.001).CONCLUSIONS:The PFU is associated with UDS in evaluating IUI. It has the advantages of low cost and high comfort, thus should be used as an auxiliary examination for IUI.
The MR Features of Leavtor Ani Muscle with Stress Urinary Incontinence

Abstract:Objective To study the correlation of morphological and functional changes of the levator animucle muscle and stress urinary incontinence(SUI);to get MRI diagnostic criteria of SUI and to give a possible explanation for SUI mechanisms.Methods Between March 2005 and March 2006,20 female inpatients were referred to our SUI study according to SUI diagnostic criteria.They ranged from 40 to 82 years of age.Twenty three female in-patients without SUI were referred to our study as normal control at the same period.All patients underwent MRI scan.Statiic and straining MRI images consisted of sagittal and axial T2-weighted FSE sequences.Transection scan range from the iliac crest to anus,sagittal scan between bi-acetabula.All images were taken by the same equipment and reported by the same doctor-group.Results Analysis of variance was used to analyze differences in thickness of levator ani muscle,angle between levator ani and plat line, volume of levator ani among sidentong static,straining,pre-and after straining of two groups.Comparisons of two groups' anteroposterior diameter(A-P) and left to right diameter(L-F) of levator ani gap showed no significantly difference.Both in control group and SUI group,there were significant differences between the right and left levator thickness.There was statistical significance between the same levator ani thickness under the pelvic strained and relax station in control group,but no significant difference in SUI group.Conclusion Levator ani's incontinence,that is gap,is on an increasing trend as symptom.There is asymmetry between the left and right levator ani nuscle in normal females.The contractive function of the levator ani miscle is decreased in SUI patients.

Abstract:【Objective】 To study the anatomy and function of normal female floor using static magnetic resonance imaging,to establish normal criteria of position and morphology of pelvic organs and pelvic floor in MRI for the use of proceeding study in pelvic floor dysfunction.【Methods】 Sixty asymptomatic female without pelvic lesion were included and divided into three groups according to their age: 20-40 years old,40-60 years old and exceed 60 years old.All of them have done pelvic static MRI.The distance of bladder neck,uterin cervix and anorectal junction to pubococcygeal line(PCL) in different age in mid-sagittal plane,the levator angle in coronal plane through the middle of the anal canal,and the width,length and area of levator hiatus in the axial view through the lower edge of pubic symphysis were measured.【Results】 The bladder neck and uterin cervix did not descend more than the PCL,and the location of anorectal junction were near the PCL.The distance of the bladder neck to PCL were different in each groups,and the difference was statistically significant(P 0.05).The area of levator hiatus and the levator angle gradually increased in different groups,but the difference was no statistically significant(P 0.05).The right side of the puborectalis is thinner than the left.【Conclusions】 The bladder neck and uterin cervix were gradually descending with age.The anorectal junction should not descend more than the 1.5 cm below PCL.The hibateral puborectal muscles were asymmetric in normal female.MRI can cleally show the female pelvic anatomy and function.

Abstract:Objective: To explore the features of extracellular matrix of pelvic floor muscles in pregnant women with stress urinary incontinence (SUI). Methods: Seventy-one primiparas with regular prenatal examination undergoing vaginal delivery were selected, and were divided into SUI group (n=33) and non-SUI group (n=38). Deep pelvic floor tissues were obtained after delivery, HE staining was conducted, electronic microscopy was performed for ultrastructure observation, and quantitative analysis of collagen and elastic fibers was carried out with Masson's trichrome staining and Verhoeff-van Gieson staining. Besides, another 3 women without pregnancy were served as non-pregnant controls. Results: There were vacuolar degeneration, filament alteration and edema in smooth muscle cells of deep pelvic floor tissues in all pregnant women. There were plentiful of collagen and elastic fibers around striated muscles in non-SUI group. The main component of deep pelvic floor tissues in SUI group was smooth muscle, and there were sparse collagen and elastic fibers around smooth muscles. The relative contents of collagen fibers and elastic fibers of pelvic floor muscle tissues in SUI group were significantly lower than those in non-SUI group (P<0.05). Conclusion: The extracellular matrix structure of pelvic floor muscle tissues in pregnant women with SUI is loose, with less collagen fibers and elastic fibres, which may be associated with the occurence of SUI.
Preliminary investigation on diagnostic value of 2D/3D transperineal ultrasound for pelvic anatomy in 40 patients with stress urinary incontinence

Abstract:Objective To preliminarily investigate the diagnostic value of 2D/3D ultrasound in the patients with stress urinary incontinence (SUI).Methods Totally 40 out-patients diagnosed with SUI in the department of urology in Second Affiliated Hospital of Xi’an Jiaotong University from October 2014 to December 2015 were recruited in study in SUI group, and 40 healthy women were selected into control group during the same period.2D/3D pelvic ultrasound was performed in both groups, and the measurements were recorded.Results Comparison of 2D parameters between two groups showed that the bladder neck shifted slightly backward and downward in SUI group, and the displacements of bladder neck were found lager in SUI group than in the control group after maximum Valsalva ( t=2.31, P<0.05) . Larger posterior urethra-vesical angle was found in SUI group compared to that in the control group both during resting and after maximum Valsalva(t value was 2.57 and 3.09, respectively, both P<0.05).The urethral rotation was significantly greater after maximum Valsalva in SUI group(t=2.67,P<0.05).No statistically significant differences both in urethral length and urethral tilt angle were indicated between two groups during resting (t value was 1.26 and 1.91, respectively, both P >0.05).The pubovisceral muscle and pelvic diaphragm hiatus were clearly identified using 3D ultrasound, and the thickness of pubovisceral muscle after maximum Valsalva in SUI group was smaller than that in the control group(t=2.98,P<0.05).The area of pelvic diaphragm hiatus increased significantly after maximum Valsalva in SUI group compared to the control group(t =5.01,P<0.05).Conclusion The primary pathogenesis of SUI is related to abnormal structural support of the neck of bladder and the urethra.And 2D/3D transperineal ultrasound can be used to effectively evaluate the changes of such support structure in various conditions.
Postpartum stress urinary incontinence: a clinical study of 6,302 cases in Jiangsu Province.

Abstract:OBJECTIVES To investigate the prevalence and influencing factors of stress urinary incontinence (SUI) within 6-8 weeks postpartum in Jiangsu Province. MATERIAL AND METHODS We designed a multi-center cross-sectional study involving seven hospitals in Jiangsu province, and enrolled women who underwent postpartum examination at 6-8 weeks in these hospitals between July 2019 and June 2021. According to the presence or absence of SUI, the enrolled patients were divided into two groups: the SUI group and the non-SUI group, respectively. We assessed the general health status, noted the details of delivery, and checked the pelvic floor electromyographic parameters of the postpartum women in both groups. RESULTS Among 6,302 cases of postpartum women in Jiangsu province, there were 1,579 cases of SUI, with a prevalence of 25.06%. The prevalence of SUI increased significantly with age, BMI, increasing parity, coexisting constipation, organ prolapse, and diastasis recti abdominis. Compared to the non-SUI group, the SUI group had a lower mean value of the pre-baseline rest phase, shorter rise and fall times of fast muscle contractions, and a lower mean value of the endurance contraction phase. Multiple regression analysis revealed associations with weight (especially overweight and obesity), coexisting organ prolapse, constipation, parity, gestational week of delivery, mode of delivery, and mean value of endurance contraction phase. CONCLUSIONS The prevalence of postpartum stress urinary incontinence in Jiangsu Province was 25.06%, and was linked to being overweight, parity > 2, coexisting organ prolapse, constipation, and a decrease in the mean value of the endurance contraction phase of the electromyograph. In this report, we offer a theoretical basis for the effective prevention of postpartum SUI clinically.

Sonographic Study on Structural and Functional Impacts on Female Pelvic Floor Upon Pregnancy, Vaginal Delivery and Elective Cesarean Delivery

Abstract:Objective To evaluate the influence of different modes of delivery on the incidence rate of stress urinary incontinence (SUI) and female pelvic floor anatomy. Methods From May to December 2008, a total of 49 cases from age of 18 to 35 with 28~36 gestational weeks were included into this study (study group). They were further divided into vaginal delivery group(n=24) and elective cesarean section(ECS) group(ECS group, n=22)according to the different modes of delivery. Meanwhile, another 20 healthy childless women of childbearing age were included into control group. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of First Affiliated Hospital of Ji'nan University. Informed consent was obtained from all participants. Each subject received examination during rest and during Valsalva's maneuver for the following parameters: anteroposterior diameter (AP) and lateral diameter (LD) of levator hiatus (LH), posterior urethra-vesical angle (α), flexibility of bladder neck (θ), and distance between the pubic symphysis midpoint and bladder neck (D). The content of serum C-terminal propeptide of type Ⅰ procollagen (PICP) was detected by enzyme linked immunosorbent assay (ELISA) method. The above indicators were compared at 28 to 36 gestational weeks , 6 to 8 and 12 to 14 weeks after delivery between study group and control group. Results The incidence of stress urinary incontinence reduced after delivery. For vaginal delivery group, the flexibility of bladder neck and distance between the pubic symphysis midpoint and bladder neck at 6 to 8 weeks after delivery had larger changes compared to ECS group (P 0.05). Conclusion Pelvic tissue damages under elective cesarean section is less than that of vaginal delivery. Elective cesarean section has protective effects on pelvic structure. Pelvic damages under all methods of delivery are temporary, and it could be recovered by natural course. Serum procollagen peptide structural damage as the pelvic fibrosis serum markers, can indirectly from post-natal pelvic floor injury response to self-repair process of development. However, elective cesarean section has a limited protective effect on pelvic floor function. Abstract:Background: Owing to its tediousness and monotony, traditional pelvic floor muscle training (PFMT) is difficult to ensure the correctness of exercise, and it is difficult for patients to adhere to treatment. We designed this study to evaluate and analyze the efficacy of optimized pelvic floor training of YUN combined with pelvic floor magnetic stimulation on female moderate stress urinary incontinence (SUI) and sexual function.Methods: This is a retrospective cohort study. This study was carried out in 95 female patients with moderate SUI. The inclusion criteria were as follows: premenopausal women aged 25-45; moderate SUI; over 3 months of disease duration; informed consent and cooperation with treatment and follow-up. The participants in group 1 (control group, n=46) were treated with pelvic floor magnetic stimulation, while those in group 2 (trial group, n=49) were treated with pelvic floor magnetic stimulation combined with optimized pelvic floor training of YUN. Evaluations were scheduled before the treatment (0 week), after 6 weeks of treatment (6 weeks), and after 12 weeks of treatment (12 weeks). And compare the differences between the two groups.Results: There was no significant difference in age, body mass index (BMI), duration of disease, and abdominal leak point pressure (ALPP) between the two groups (P>0.05). The total effective rate of the trial group was higher than that of the control group (89.80%, 44/49 <i>vs.</i> 78.26%, 36/46) (P<0.05). The electromyographic values, the International Consultation on Incontinence Questionnaire Short Form (ICI-Q-SF) score, Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire-12 (PISQ-12) score, physiological factors, and emotional factors were all improved significantly in both groups after active treatment, and the improvement of the trial group was more obvious (P<0.05).Conclusions: Optimized pelvic floor training of YUN combined with pelvic floor magnetic are more effective for the treatment of female moderate SUI and sexual function. It has become a safe, effective, and well tolerated new type of pelvic floor functional reconstruction training method with good patient compliance.

Abstract:OBJECTIVE:To identify the morphological characteristics of pelvic diaphragm hiatus in pregnant women with stress urinary incontinence (SUI) by transperineal three-dimensional (3-D) ultrasound. METHODS:From Oct. 2008 to Mar. 2009, 145 pregnant women (third trimester group) at 37-41 weeks of gestation underwent transperineal 3-D ultrasound investigation at Department of Obstetrics and Gynecology, the Sixth People's Hospital, Shanghai Jiaotong University, including 38 pregnant women with stress urinary incontinence (SUI) and the other 107 non SUI pregnant women. In the mean time, 50 normal nulliparous healthy women were chosen as control group. The morphological characteristics of pelvic diaphragm hiatus, the diameter of pelvic diaphragm hiatus, pubovisceral muscle thickness and genitohiatal and levator ani angle were measured at rest, on maximum Valsalva and maximum pelvic floor contraction by 3-D ultrasound, respectively. RESULTS:Loosen connective tissue and pubococcygeus avulsion were observed in some pregnant women at third trimester. The area of pelvic diaphragm hiatus were (15.2+/-1.9), (16.4+/-2.0) and (13.6+/-1.9) cm2, pubovisceral muscle thickness were (0.72+/-0.11), (0.68+/-0.14) and (0.77+/-0.11) cm, levator ani angle were (60+/-8) degrees, (57+/-10) degrees and (64+/-14) degrees at rest, on maximum Valsalva and maximum pelvic floor contraction respectively. These parameters were significantly increased than those in control group [(11.2+/-2.6), (14.5+/-4.5) and (9.2+/-2.6) cm2; (0.66+/-0.10), (0.67+/-0.14) and (0.71+/-0.14) cm; (50+/-4) degrees, (51+/-5) degrees and (46+/-5) degrees] at three maneuvers, respectively (P<0.05). And those parameters of the anteroposterior hiatal diameter, lateral hiatal diameter, perimeter of pelvic diaphragm hiatus and area of pelvic diaphragm hiatus in SUI pregnant women were increased than those in non SUI pregnant women at three maneuvers, respectively (P<0.05). Pubovisceral muscle thickness in SUI pregnant women was significantly lower than that in non SUI pregnant women at maximum pelvic floor contraction (P<0.05), but there were not significant difference between SUI and non SUI pregnant women at rest and on maximum Valsalva in pubovisceral muscle thickness and genitohiatal and levator ani angle (P>0.05). CONCLUSIONS:Pelvic floor anatomic remodeling is identified in late pregnant women. When compared with non pregnant women, the loosen pelvic floor connective tissue and the bigger diameters of pelvic diaphragm are observed in late pregnant women. It is observed that the increased diameters of pelvic diaphragm and decreased thickness of pubovisceral muscle in later pregnant SUI women than those in non SUI pregnant women.

Abstract:This study aimed to explore the application of pelvic floor ultrasound under particle swarm intelligent optimization algorithm in the preoperative and postoperative evaluation of female stress urinary incontinence (SUI) and provide a theoretical basis for clinical diagnosis. In this study, 90 patients with SUI were enrolled, which were randomly and equally assigned into a blank group (healthy physical examination), control group (perineal ultrasound), and experimental group (pelvic floor ultrasound based on particle swarm intelligence optimization algorithm). The ultrasonic image segmentation and processing were carried out by a particle swarm intelligence optimization algorithm. Patients with stress incontinence were classified as group A, and patients without stress incontinence were classified as group B. The results of previous surgical examinations were the standard to judge the accuracy of pelvic floor ultrasound diagnosis based on the swarm intelligence optimization algorithm. The accuracy of diagnosing stress UI in the experimental group was 90.38%, which was significantly higher than that of the control group (54.31%) and the blank group (38.95%) ( P < 0.05). The formation percentage of the urethral internal orifice in the experimental group was 82.5%, which was significantly higher than that of the control group (65.4%) and the blank group (12.5%), and there was a statistical difference among the groups ( P < 0.05). In the resting state, the vertical spacing y between the neck of the bladder and the edge of the pubis of patients in group B was greater than that in group B, the horizontal spacing x between the neck of the bladder and the edge of the pubis was smaller than in the blank group, and there were statistical differences among the groups ( P < 0.05). In the state of Valsalva, the vertical spacing y between the neck of the bladder and the edge of the pubis of patients in group B was smaller than that in group B, the horizontal spacing x between the neck of the bladder and the edge of the pubis was greater than that in group B. The distance of the bladder neck shifting downward was greater than that in group B, and there were statistical differences among the groups ( P < 0.05). In short, the pelvic floor ultrasound based on the particle swarm intelligent optimization algorithm was accurate in the diagnosis of stress UI. The application of pelvic floor ultrasound in the diagnosis of UI provided image data objectively for clinical diagnosis and had a high application value.

Biomechanical Integrity Score of the Female Pelvic Floor for Stress Urinary Incontinence

Abstract:Introduction and hypothesis: This study is aimed at developing and validating a new integral parameter, the Biomechanical Integrity score (BI-score) of the female pelvic floor for stress urinary incontinence conditions. Methods: A total of 130 subjects were included in the observational cohort study; 70 subjects had normal pelvic floor conditions, and 60 subjects had stress urinary incontinence (SUI). A Vaginal Tactile Imager (VTI) was used to acquire and automatically calculate 52 biomechanical parameters for eight VTI test procedures (probe insertion, elevation, rotation, Valsalva maneuver, voluntary muscle contractions in two planes, relaxation, and reflex contraction). Statistical methods were applied (t test, correlation) to identify the VTI parameters sensitive to the pelvic SUI conditions. Results: Twenty-seven parameters were identified as statistically sensitive to SUI development. They were subdivided into five groups to characterize tissue elasticity (group 1), pelvic support (group 2), pelvic muscle contraction (group 3), involuntary muscle relaxation (group 4), and pelvic muscle mobility (group 5). Every parameter was transformed to its standard deviation units using the dataset for normal pelvic conditions, similar to the T-score for bone density. Linear combinations with specified weights led to the composition of five component parameters for groups 1-5 and to the BI-score in standard deviation units. The p value for the BI-score has p = 4.0 × 10-28 for SUI versus normal conditions. Conclusions: Quantitative transformations of the pelvic tissues, support structures, and functions under diseased conditions may be studied with the SUI BI-score in future research and clinical applications.

Abstract:Objective To evaluate the MRI value in changes of mid-urethral ligament injury of female stress urinary incontinence (SUI).Methods Comparison of MRI changes of mid-urethral ligament on 30 healthy female volunteers and 20 female SUI patients.Chi-square test was used to compare the form of SUI patient's mid-urethral support ligaments Results The female nid-urethral support ligaments were composed of 4 groups of ligaments,including the periurethral ligament and pubourethral ligaments (1 pair),and at both sides of the urethra's paraurethral ligaments (1 pair) and suburethral ligament lying dorsal urethra,connecting the urethra and pelvic arcus tendinous fasciae.In normal MRI,ligament was a thin strip and showed low signal on both T1WI and T2WI,T2WI sagittal and cross-section scan was the best combination to show the middle urethral support ligaments changes,with tension;6 patients (20％) in the 30 patients normal control group could be seen tortuously and slack like around the urethra ligaments.Twenty SUI patients mid-urethral support ligaments were performance laxity or rupture,rates were 39％ (47/120) and 42％ (50/120) (x2 =43.191,P ＜ 0.05 ).On T2WI,the ligamentous laxity was floating,and loss tension,also could performance one side extension and thinner than the other side.The ligament rupture was performance of the signal interruption,ligament contracture and one end of liganent attachment points separation.Conclusion MRI can objective effective evaluate the mid-urethral support ligaments' pathological changes in stress urinary incontinence patients.
Comparisons of Electromyography and Digital Palpation Measurement of Pelvic Floor Muscle Strength in Postpartum Women with Stress Urinary Incontinence and Asymptomatic Parturients: A Cross-Sectional Study

Abstract:Aims: Early evaluation of pelvic floor muscle (PFM) in postpartum women is important for the treatment of stress urinary incontinence (SUI). Digital vaginal palpation and electromyography (EMG) evaluation based on Glazer protocol are widely used for the assessment of PFM. However, the correlation among digital palpation, EMG, and morbidity of postpartum SUI is still unclear. This study aims to investigate the relationship between postpartum SUI and PFM examinations. Methods: This hospital-based cross-sectional study included 1,380 parturients during September 2016 to January 2018. We collected the clinical characteristics, PFM strength, and EMG variables of parturients 6–8 weeks after birth. Then the correlation among the results of EMG, digital palpation, and the occurrence of SUI was analyzed. Results: There is no significant difference in digital palpation scores of PFM strength between SUI and non-SUI parturients. The EMG values were closely related to SUI: the multivariate logistic regression revealed that the most reliable evaluation indicators of postpartum SUI were pelvic floor contractile amplitude of endurance contraction (B = 0.021, p = 0.019) and pretest resting baseline (B = 0.056, p = 0.019). Correlation analysis demonstrated that the contraction variables of EMG had a significant correlation with the digital palpation PFM strength in postpartum women (r = 0.467–0.545, p < 0.001). Conclusion: The EMG proved to be reliable in assessing the PFM function in postpartum women. The decreased PFM activity, according to EMG, was correlated with postpartum SUI. Although digital palpation scores were positively correlated with EMG results, no correlation was observed with SUI incidence.

Abstract:To explore the value of perineal ultrasound and urodynamic study in the clinical diagnosis with the morphological and functional changes and evaluation of clinical type in female patients with stress urinary incontinence (SUI).The records of 64 female patients with SUI were reviewed. Fifty females with urinary frequency but with no SUI symptoms served as controls. All of them underwent ultrasound and urodynamic measurement. P<0.05 was considered statistically significant.The maximum flow rate (Q max) was significantly higher in the patients than in the control group, but the maximal urethral pressure (MUP), maximal urethral closing pressure (MUCP) and functional urethral length (FUL) were significantly lower than in the control group. Ultrasound measurements showed that the rotation angle and movement of urethrovesical junction, funneling of bladder neck and posterior urethrovesical angle (PUVA) at rest and during straining were greater in patients than in the control group. Patients with higher abdominal leak point pressure (ALPP) had higher maximal urethral closing pressure (MUCP), greater rotation angle and movement of urethrovesical junction. Patients with higher MUCP had higher ALPP, greater rotation angle and movement of urethrovesical junction.The urodynamic study and perineal ultrasound have a great significance in the diagnosis and typing of SUI.
Correlation of Serum Estrogen and Progesterone Levels and Expression of Estrogen and Progesterone Receptors with Stress Urinary Incontinence in Late Pregnancy

Abstract:Objective To investigate the correlation of serum estrogen and progesterone levels and expression of estrogen and progesterone receptors in pelvic floor tissues with stress urinary incontinence(SUI) in late pregnancy. MethodsSeventy-one pregnant primiparas undergoing vaginal delivery were divided into SUI group(n=33) and non-SUI group(n=38),and serum levels of estradiol(E2),estriol(E3) and progesterone(P) were detected at gestational week 36 to 40.Muscular tissue samples were obtained from the top of lateral incision of perineum from 20 cases in SUI group and 31 cases in non-SUI group after delivery,and pathohistological and ultrastructure observations were performed.Another 3 non-pregnant female patients with carcinoma of vulva or rectum cancer undergoing surgery were served as controls.The expression of estrogen receptor and progesterone receptor in pelvic floor tissues was detected by immunohistochemical staining. Results There was no significant difference in serum E2 and E3 between SUI group and non-SUI group(P0.05).The serum mass concentration of P in SUI group was significantly lower than that in non-SUI group [(184.7±48.2)μg/L vs(223.2±93.5)μg/L](P0.05).Pathohistological and ultrastructure observations revealed that there were vacuolar degeneration,filament alteration and edema in smooth muscle cells of deep pelvic floor.The expression of estrogen receptor and progesterone receptor in smooth muscle cells in non-SUI group was significantly higher than that in SUI group(P0.05).There was no expression of estrogen receptor and progesterone receptor in skeletal muscle cells of pelvic floor.Conclusion Both estrogen and progesterone may protect the structure and function of pelvic floor in pregnant primiparas.
