Abstract:Objectives: The aim of this study was to evaluate prenatal ultrasound features, fetal postnatal outcomes, and the need for postnatal surgery in the suspicion of ovarian torsion. Methods: We included patients with a singleton pregnancy with a suspicion of ovarian fetal cyst referred to our center. Data derived from prenatal ultrasound evaluations, delivery, and postnatal follow-up were then extracted from the hospital registers. Results: The ultrasound features of 32 fetal ovarian cysts and related neonatal outcomes were analyzed. The mean gestational age at diagnosis was 32 weeks (28–36), while the mean diameter of the cyst diagnosis was 34.8 ± 13.2 mm. In 78.1% of cases, the cysts did not change their characteristics during pregnancy, while in 9.4%, they increased their dimensions, and in 12.5%, they reduced their size. In 78.1% of cases, the diagnosis was confirmed postnatally, and in 40% of cases, a spontaneous regression occurred during follow-up. Surgery was performed in 60% of cases, and most of the time (66.7%), an adnexectomy was required. Conclusions: An unfavorable outcome was associated with cystic dimensions and ultrasound feature modifications during pregnancy. However, our study demonstrated that a hemorrhagic content is not always indicative of adnexal torsion, and spontaneous resolution may occur. In addition, only a few of the simple anechoic cysts managed surgically presented with necrosis at histopathological examination; therefore, a conservative approach might be proposed in those cases.

Abstract:Objective To explore the natural development of fetal ovarian cyst and assess the accuracy of ultrasonography in diagnosing fetal ovarian cyst and providing rationales for prenatal evaluation and postnatal tracing of fetal ovarian cyst.Methods From January 2008 to September 2013,pre-and post-natal medical records of ultrasonically suspicious fetuses with ovarian cysts were reviewed retrospectively.And the clinical data included gestation age,mass size,location,mass features and combined malformations,etc.Results Among 99 cases,96 were unilateral and 57 cysts were over 5 crn or complicated.Their average gestational age was 28(19-39) weeks.And 32 cases were lost to follow-ups and 9 opted for termination.For the remaining 58 case,34 cases (58.6％) presented spontaneous regression.Prenatally,6 cases (10.3％) had spontaneous regression.And 28 cases (28.2％) regressed spontaneously after birth and before 1 year.Postnatal operations were performed for 13(22.4％) cysts.And 11 (18.9％) cysts showed no obvious enlargement and were followed up to date.The prenatal accuracy rate of ultrasonic diagnosis was 91％ (53/58).Conclusions Most fetal ovarian cysts found sonographically in the third trimester tend to regress spontaneously during the first year.For cysts over 5 crn or complicated,it is necessary to reevaluate just after birth and establish a correct diagnosis.Thus organ loss due to cyst torsion or misdiagnosing may be avoided.

Abstract:Objectives The finding of an abdominal cyst during pregnancy has an estimated prevalence of 1 in 1000 pregnancies, mostly in second and third trimester. The detection of a fetal abdominal cyst during the first trimester scan is a rare event, whose natural history and prognosis are often unknown and unpredictable as these anomalies can be related to various underlying conditions and originate from different structures. The aim of this study is to evaluate the outcome of fetal abdominal cysts detected in the first trimester in order to understand their possible clinical significance and to offer the proper management according to the available data. Methods We present a case report of a first trimester fetal abdominal cyst detected with subsequent diagnosis of congenital multiple arthrogryposis and we performed a systematic review of the literature to identify the incidence and the outcomes of similar cases. The systematic literature review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA) statement 25 and registered with PROSPERO (CRD42023491729). Results A total of 60 cases of first trimester abdominal cysts were included. Of these, 35% were associated with concurrent or late onset structural anomalies, as in our case report, and 65% were isolated. In pregnancies with isolated fetal abdominal cysts, 56% had a completely normal outcome. Conclusions The finding of an abdominal cyst during the first trimester of pregnancy is in most cases an isolated event with a moderate to good prognosis but it could also be an early sign of other associated abnormalities, including arthrogryposis. Increased ultrasound surveillance and additional genetic testing to rule out possible associated anomalies are pivotal to assess the risk of adverse pregnancy outcomes and to provide appropriate counselling to the patient.
