Comparison of CT guided Transthoracic core needle Biopsy(CNB)and Fine Needle Aspiration(FNA)Biopsy in terms of Diagnostic value, complication and Samples Adequacy for Molecular testing in Lung Cancer.

Abstract:Introduction: CT guided CNB and FNA Biopsy are used to diagnose lung cancer. Nowadays, it is necessary to do molecular testing for proper treatment. We aimed to determine the difference between methods in terms of sensitivity, complication and samples adequacy for molecular testing. Methods: CNB and FNA cases were reviewed between 2021 and 2023. 20 gauge needle were used for all FNA cases and 18 gauge for CNB. Complication was reviewed. Lung adenocarcinoma, squamous cell carcinoma(SCC)or non-small cell lung cancer(NSCLC)samples were checked for adequacy for molecular testing. Lesion's size and distance of parenchyma crossed were measured. Results: 304 cases included. Diagnosis was reached in 94.4% of FNA and CNB cases. Lung adenocarcinomas,SCC or NSCLC samples remained to perform molecular studies in 98% of FNA and 96% of CNB cases. No statistical differences were found among procedures for reaching diagnosis(p=0.986, Fisher exact test)or adecuacy for molecular studies(p=0.166). Most complication were pneumothorax and parenchymal hemorrhage. A statistically difference was found between pneumothorax and lesion size(p=0.023)and distance of parenchyma crossed(p=0.004)for both procedure. Conclusion: Both methods have high sensitivity, adequacy for molecular studies ,reduce the need for invasive surgical procedures and less costs.
