Abstract:Elastography is a method of imaging that produces a type of image, called an elastogram. An area of research for almost 30 years, this diagnostic, noninvasive technique is used to evaluate the stiffness of soft tissues. It is done in correlation with a conventional sonogram. Elastography transmits information about internal tissue structures that has not been previously attainable. Discussion includes three types of elastography imaging: strain imaging, color elasticity, and shear-wave imaging. This imaging is based on the fundamentals of strain imaging and the basis that malignant tumors are harder than benign tumors. Stiffer tissues such as masses distort less than softer tissues, and the variation can be visualized on the elastic properties seen on the elastogram. Currently, elastography is being used in many applications, yet there is still reluctance by some in the medical community to fully accept elasticity imaging. Quantification and reproducibility of images will ensure the fate of this addition to the field of sonography.
Anisotropic elasticity measurements of the retina using optical coherence elastography

Abstract:Anisotropic elasticity measurements of the retina are essential for retinal disease diagnosis and function assessment. Optical coherence elastography (OCE) is a high-resolution imaging technique for mapping the elasticity distribution of tissues. However, previous OCE measurements quantified the tissue elasticity in a single direction, resulting in a biased estimation of the elastic properties. In this study, we propose an OCE method with acoustic radiation force (ARF) excitation to map the retinal anisotropic elasticity in the depth and lateral directions. The axial elasticity was analyzed using the natural frequency of free vibration, and the lateral elasticity was quantified using the elastic wave velocity. After evaluating the feasibility of the OCE method on the phantoms, the anisotropic elasticity of ex vivo porcine retinas was mapped. The results show that the OCE method with ARF excitation can assess the elasticity in orthogonal directions and provide a comprehensive understanding of the elasticity of the anisotropic tissues.

Abstract:In dynamic optical coherence elastography (OCE), surface acoustic waves are the predominant perturbations. They constrain the quantification of elastic modulus to the direction of wave propagation only along the surface of tissues, and disregard elasticity gradients along depth. Longitudinal shear waves (LSW), on the other hand, can be generated at the surface of the tissue and propagate through depth with desirable properties for OCE: (1) LSW travel at the shear wave speed and can discriminate elasticity gradients along depth, and (2) the displacement of LSW is longitudinally polarized along the direction of propagation; therefore, it can be measured by a phase-sensitive optical coherence tomography system. In this study, we explore the capabilities of LSW generated by a circular glass plate in contact with a sample using numerical simulations and tissue-mimicking phantom experiments. Results demonstrate the potential of LSW in detecting an elasticity gradient along axial and lateral directions simultaneously. Finally, LSW are used for the elastography of ex vivo mouse brain and demonstrate important implications in in vivo and in situ measurements of local elasticity changes in brain and how they might correlate with the onset and progression of degenerative brain diseases.
Simultaneous reconstruction and displacement estimation for spectral-domain optical coherence elastography

Abstract:Optical coherence elastography allows the characterization of the mechanical properties of tissues, and can be performed through estimating local displacement maps from subsequent acquisitions of a sample under different loads. This displacement estimation is limited by noise in the images, which can be high in dynamic systems due to the inability to perform long exposures or B-scan averaging. In this work, we propose a framework for simultaneously enhancing both the image quality and displacement map for elastography, by motion compensated denoising with the block-matching and 4D filtering (BM4D) method, followed by a re-estimation of displacement. We adopt the interferometric synthetic aperture microscopy (ISAM) method to enhance the lateral resolution away from the focal plane, and use sub-pixel cross correlation block matching for non-uniform deformation estimation. We validate this approach on data from a commercial spectral domain optical coherence tomography system, whereby we observe an enhancement of both image and displacement accuracy of up to 33% over a standard approach.

Abstract:Dynamic optical coherence elastography (OCE) tracks mechanical wave propagation in the subsurface region of tissue to image its shear modulus. For bulk shear waves, the lateral resolution of the reconstructed modulus map (i.e., elastographic resolution) can approach optical coherence tomography (OCT) capabilities, typically a few tens of microns. Here we perform comprehensive numerical simulations and acoustic micro-tapping OCE experiments to show that for the typical situation of guided wave propagation in bounded media, such as cornea, the elastographic resolution cannot reach the OCT resolution and is mainly defined by the thickness of the bounded tissue layer. We considered the excitation of both broadband and quasi-harmonic guided waves in a bounded, isotropic medium. Leveraging the properties of broadband pulses, a robust method for modulus reconstruction with minimum artifacts at interfaces is demonstrated. In contrast, tissue bounding creates large instabilities in the phase of harmonic waves, leading to serious artifacts in modulus reconstructions.

Abstract:Alteration in the elastic properties of biological tissues may indicate changes in the structure and components. Acoustic radiation force optical coherence elastography (ARF-OCE) can assess the elastic properties of the ocular tissues non-invasively. However, coupling the ultrasound beam and the optical beam remains challenging. In this Letter, we proposed an OCE method incorporating homolateral parallel ARF excitation for measuring the elasticity of the ocular tissues. An acoustic-optic coupling unit was established to reflect the ultrasound beam while transmitting the light beam. The ARF excited the ocular tissue in the direction parallel to the light beam from the same side of the light beam. We demonstrated the method on the agar phantoms, the porcine cornea, and the porcine retina. The results show that the ARF-OCE method can measure the elasticity of the cornea and the retina, resulting in higher detection sensitivity and a more extensive scanning range.

Abstract:Abstract Visualizing viscoelastic waves in materials and tissues through noninvasive imaging is valuable for analyzing their mechanical properties and detecting internal anomalies. However, traditional elastography techniques have been limited by a maximum wave frequency below 1-10 kHz, which hampers temporal and spatial resolution. Here, we introduce an optical coherence elastography technique that overcomes the limitation by extending the frequency range to MHz. Our system can measure the stiffness of hard materials including bones and extract viscoelastic shear moduli for polymers and hydrogels in conventionally inaccessible ranges between 100 Hz and 1 MHz. The dispersion of Rayleigh surface waves across the ultrawide band allowed us to profile depth-dependent shear modulus in cartilages ex vivo and human skin in vivo with sub-mm anatomical resolution. This technique holds immense potential as a noninvasive measurement tool for material sciences, tissue engineering, and medical diagnostics.

A Transient Elastography System for Soft-Tissue Stiffness Measurement and Wavelet De-Noising of Displacement Estimation in Elastogram

Abstract:Elastography is an emerging ultrasonic imaging technique. A system based on transient elastography is built to measure biological tissues' stiffness in the paper. Then, wavelet de-noising of the displacement estimatation to reduce artifacts due to high overlap in transient elastography is presented. High overlap between data segments is essential to improve axial resolution in transient elastography. However, overlap more than 90% or more causes the increase of correlated errors, which in turn generate "worm" artifact in strain. Wavelet de-noising can reduce errors in the displacement estimates strikingly by thresholding the wavelet coefficients, therefore reducing the worm artifact. The strain results from tissue-mimicking phantoms, illustrates the improvement in the elastogram by wavelet de-noising algorithm.

Abstract:We describe a new method for quantitative imaging of strain and elastic modulus distributions in soft tissues. The method is based on external tissue compression, with subsequent computation of the strain profile along the transducer axis, which is derived from cross-correlation analysis of pre- and post-compression A-line pairs. The strain profile can then be converted to an elastic modulus profile by measuring the stresses applied by the compressing device and applying certain corrections for the nonuniform stress field. We report initial results of several phantom and excised animal tissue experiments which demonstrate the ability of this technique to quantitatively image strain and elastic modulus distributions with good resolution, sensitivity and with diminished speckle. We discuss several potential clinical uses of this technique.

Abstract:Tissue elasticity is closely related to the velocity of shear waves within biologic tissue. Shear waves can be generated by an acoustic radiation force and tracked by, e.g., ultrasound or magnetic resonance imaging (MRI) measurements. This has been shown to be able to noninvasively map tissue elasticity in depth and has great potential in a wide range of clinical applications including cancer and cardiovascular diseases. In this study, a highly sensitive optical measurement technique is proposed as an alternative way to track shear waves generated by the acoustic radiation force. A charge coupled device (CCD) camera was used to capture diffuse photons from tissue mimicking phantoms illuminated by a laser source at 532 nm. CCD images were recorded at different delays after the transmission of an ultrasound burst and were processed to obtain the time of flight for the shear wave. A differential measurement scheme involving generation of shear waves at two different positions was used to improve the accuracy and spatial resolution of the system. The results from measurements on both homogeneous and heterogeneous phantoms were compared with measurements from other instruments and demonstrate the feasibility and accuracy of the technique for imaging and quantifying elasticity. The relative error in estimation of shear wave velocity can be as low as 3.3% with a spatial resolution of 2 mm, and increases to 8.8% with a spatial resolution of 1 mm for the medium stiffness phantom. The system is shown to be highly sensitive and is able to track shear waves propagating over several centimetres given the ultrasound excitation amplitude and the phantom material used in this study. It was also found that the reflection of shear waves from boundaries between regions with different elastic properties can cause significant bias in the estimation of elasticity, which also applies to other shear wave tracking techniques. This bias can be reduced at the expense of reduced spatial resolution.
Simultaneous tensile and shear measurement of the human cornea in vivo using S0- and A0-wave optical coherence elastography

Abstract:Understanding corneal stiffness is valuable for improving refractive surgery, detecting corneal abnormalities, and assessing intraocular pressure. However, accurately measuring the elastic properties, particularly the tensile and shear moduli that govern mechanical deformation, has been challenging. To tackle this issue, we have developed guided-wave optical coherence elastography that can simultaneously excite and analyze symmetric (S0) and anti-symmetric (A0) elastic waves in the cornea at frequencies around 10 kHz and allows us to extract tensile and shear properties from measured wave dispersion curves. By applying acoustoelastic theory that incorporates corneal tension and a nonlinear constitutive tissue model, we verified the technique using elastomer phantoms and ex vivo porcine corneas and investigated the dependence on intraocular pressure. For two healthy human subjects, we measured a mean tensile modulus of 3.6 MPa and a mean shear modulus of 76 kPa in vivo with estimated errors of < 4%. This technique shows promise for the quantitative biomechanical assessment of the cornea in a clinical setting.

Abstract:We demonstrate spectroscopic photonic force optical coherence elastography (PF-OCE). Oscillations of microparticles embedded in viscoelastic hydrogels were induced by harmonically modulated optical radiation pressure and measured by phase-sensitive spectral-domain optical coherence tomography. PF-OCE can detect microparticle displacements with pico- to nano-meter sensitivity and millimeter-scale volumetric coverage. With spectroscopic PF-OCE, we quantified viscoelasticity over a broad frequency range from 1 Hz to 7 kHz, revealing rich microstructural dynamics of polymer networks across multiple microrheological regimes. Reconstructed frequency-dependent loss moduli of polyacrylamide hydrogels were observed to follow a general power scaling law G''∼ω0.75, consistent with that of semiflexible polymer networks. Spectroscopic PF-OCE provides an all-optical approach to microrheological studies with high sensitivity and high spatiotemporal resolution, and could be especially beneficial for time-lapse and volumetric mechanical characterization of viscoelastic materials.
In vivo Optical Coherence Elastography Reveals Spatial Variation and Anisotropy of Corneal Stiffness

Abstract:Objective: The mechanical properties of corneal tissues play a crucial role in determining corneal shape and have significant implications in vision care. This study aimed to address the challenge of obtaining accurate in vivo data for the human cornea. Methods: We have developed a high-frequency optical coherence elastography (OCE) technique using shear-like antisymmetric (A0)-mode Lamb waves at frequencies above 10 kHz. Results: By incorporating an anisotropic, nonlinear constitutive model and utilizing the acoustoelastic theory, we gained quantitative insights into the influence of corneal tension on wave speeds and elastic moduli. Our study revealed significant spatial variations in the shear modulus of the corneal stroma on healthy subjects for the first time. The central cornea exhibited a shear modulus of 74 kPa, while the corneal periphery showed a decrease to 41 kPa. The limbus demonstrated an increased shear modulus exceeding 100 kPa. We obtained wave displacement profiles that are consistent with highly anisotropic corneal tissues. Conclusion: Our approach enabled precise measurement of corneal tissue elastic moduli in situ with high precision (< 7%) and high spatial resolution (< 1 mm). Significance: The high-frequency OCE technique holds promise for biomechanical evaluation in clinical settings, providing valuable information for refractive surgeries, degenerative disorder diagnoses, and intraocular pressure assessments.

Abstract:Estimating the elasticity of hydrogel phantoms in a cell culture plane is important for understanding the cell behavior in response to various types of mechanical stimuli. Hence, a noncontact tool for measuring the elastic properties of hydrogel phantoms in such three-dimensional cell cultures is required. A well-known method to determine the mechanical properties of hydrogels is the transient wave method. However, due to the multiple reflections of waves from the boundaries, a bigger cell culture plane or multiple directional filters may be required. In this study, we utilized reverberant shear wave elastography, which is based on the autocorrelation principle, to evaluate the shear wave speed in hydrogel samples within a culture dish. Numerical simulations were performed first to confirm the validity of the reverberant elastography method. Subsequently, we used this method to measure the wave speeds in hydrogel phantoms with different concentrations. Shear rheology tests were also performed, and their results were found to be in good agreement with the measured shear wave speeds. The proposed method could be useful for measuring the elasticity of tissues in tissue engineering applications in an inexpensive and noncontact manner.

A Rod-Like Acoustic Radiation Force in Ultrasound-Based Elastography: A Simulation Study

Abstract:Many groups have dedicated to develop elasticity-imaging methods that induce tissue displacements by an acoustic radiation force (ARF). In this paper, we employ a latest model, 2-D CMUT array transducer, to generate a rod-like radiation force. Compared to spot-like force excitation, the rod-like focal force excitation has the following advantages in future practice: First, it can extend the displacement field; further, a rod-like excitation induces a shear wave field in a preferential propagation direction, which will help to measure the elastic properties in the anisotropic tissues, such as biceps and cardiac muscles. The 2-D array transducer is lateral focused at several depths with no elevational focalization by setting the delay time between the elements. The shape of rod-like focus has a length of about 16mm along the elevational direction. The uniformity and the width of the rod-like focal region at each focal depth is explored in our simulation study. Simulated results show that both the nonuniformity and the width of the focal region will increase with the focal depth. The standard deviation (SD) of the normalized pressure in the focal region will also increase with the focal depth. When the focal depth is relative near (<25mm), both the uniformity and the width of the focal region reach a satisfactory level (SD < 0.12, width< 3.2mm).
Real-Time Nondestructive Viscosity Measurement of Soft Tissue Based on Viscoelastic Response Optical Coherence Elastography

Abstract:Viscoelasticity of the soft tissue is an important mechanical factor for disease diagnosis, biomaterials testing and fabrication. Here, we present a real-time and high-resolution viscoelastic response-optical coherence elastography (VisR-OCE) method based on acoustic radiation force (ARF) excitation and optical coherence tomography (OCT) imaging. The relationship between displacements induced by two sequential ARF loading—unloading and the relaxation time constant of the soft tissue—is established for the Kelvin-Voigt material. Through numerical simulation, the optimal experimental parameters are determined, and the influences of material parameters are evaluated. Virtual experimental results show that there is less than 4% fluctuation in the relaxation time constant values obtained when various Young's modulus and Poisson's ratios were given for simulation. The accuracy of the VisR-OCE method was validated by comparing with the tensile test. The relaxation time constant of phantoms measured by VisR-OCE differs from the tensile test result by about 3%. The proposed VisR-OCE method may provide an effective tool for quick and nondestructive viscosity testing of biological tissues.

Photoacoustic Multispectral Elastography Based on the Photoacoustic Oscillation Effect for Microelastomers in Deep Tissue

Abstract:The detection of microelastomers in deep tissue has significant biomedical value. However, elastography of microelastomers is still limited to superficial tissues. Here, we propose photoacoustic multiacoustic spectral elastography based on the photoacoustic oscillation effect. After illumination with a pulsed laser, a light-absorbing elastomer vibrates and emits damped ultrasound waves, i.e., eigenvibro signals, in a short time. The eigenvibro signals contain elastic information. Our proposed method utilizes a causal damped wavelet to enhance and decompose the eigenvibro signals. Each wavelet basis corresponds to a distinct frequency, enabling the wavelet transformation of the photoacoustic signals and their synthesis to generate frequency-specific images. These individual images collectively form multispectral images, effectively displaying elastic features at their respective frequencies. Then, the elastogram of microelastomers in the region of interest is reconstructed and includes the intensity image, peak-frequency image, quality-factor image, and elasticity-to-viscosity ratio image. Both numerical simulations and phantom experiments validate the suitability of our proposed method, even with interference from inelastic optical absorbers. Our study has potential biomedical value for the assessment of diseases resulting from the development of microelastomers.
Shear Wave Speed Estimation Using Reverberant Shear Wave Fields: Implementation and Feasibility Studies

Abstract:Elastography is a modality that estimates tissue stiffness and, thus, provides useful information for clinical diagnosis. Attention has focused on the measurement of shear wave propagation; however, many methods assume shear wave propagation is unidirectional and aligned with the lateral imaging direction. Any deviations from the assumed propagation result in biased estimates of shear wave speed. To address these challenges, directional filters have been applied to isolate shear waves with different propagation directions. Recently, a new method was proposed for tissue stiffness estimation involving creation of a reverberant shear wave field propagating in all directions within the medium. These reverberant conditions lead to simple solutions, facile implementation and rapid viscoelasticity estimation of local tissue. In this work, this new approach based on reverberant shear waves was evaluated and compared with another well-known elastography technique using two calibrated elastic and viscoelastic phantoms. Additionally, the clinical feasibility of this technique was analyzed by assessing shear wave speed in human liver and breast tissues, in vivo. The results indicate that it is possible to estimate the viscoelastic properties in each scanned medium. Moreover, a better approach to estimation of shear wave speed was obtained when only the phase information was taken from the reverberant waves, which is equivalent to setting all magnitudes within the bandpass equal to unity: an idealization of a perfectly isotropic reverberant shear wave field.

Abstract:Elastography contrast imaging has great potential for the detection and characterization of abnormalities in soft biological tissues to help physicians in diagnosis. Transient shear-waves elastography has notably shown promising results for a range of clinical applications. In biological soft tissues such as muscle, high mechanical anisotropy implies different stiffness estimations depending on the direction of the measurement. In this study, we propose the evolution of a noise-correlation elastography approach for in-plane anisotropy mapping. This method is shown to retrieve anisotropy from simulation images before being validated on agarose anisotropic tissue-mimicking phantoms, and the first results on in-vivo biological fibrous tissues are presented.

In this study, we implement surface wave elastography using high-speed optical interferometry to characterize the elastic properties of tissue-mimicking phantoms and ex-vivo native caprine liver tissue by imaging the surface wave induced by an electromechanical actuator. The sinusoidal mechanical excitations ranging from 120 Hz to 1.2 kHz on the surface of tissues are captured using a high-speed camera with a frame rate of 4 kHz at micrometer resolutions. The surface wavefront reconstruction is performed using a phase-shifting algorithm and linear regression is used to calculate the surface wave velocity. The mechanical stiffness estimated from the optical system is compared with the results of mechanical compression testing measurements. The results from this multimodal platform combining optical interferometry and vibrational spectroscopy using SWE are highly promising towards a non-invasive or minimally invasive imaging for in-vivo and ex-vivo mechanical characterization of tissues with future clinical applications.
