Abstract:MicroRNAs are a class of endogenous noncoding single-stranded RNA molecules with approximately 20-24 nucleotides and are associated with a broad range of biological processes. Researchers found that microRNAs are abundant in tissues, and more importantly, there are also trace circulating microRNAs that exist in biological fluids. In recent years, circulating microRNAs had emerged as promising diagnostic and prognostic biomarkers for the noninvasive detection of diseases with high specificity and sensitivity. More importantly, specific microRNA expression signatures reflect not only the existence of early-stage diseases but also the dynamic development of advanced-stage diseases, disease prognosis prediction, and drug resistance. To date, an increasing number of potential miRNA biomarkers have been reported, but their practical application prospects are still unclear. Therefore, microRNAs, as potential diagnostic and prognostic biomarkers in a variety of diseases, need to be updated, as they are of great importance in the diagnosis, prognosis and prediction of therapeutic responses. In this review, we summary our current understanding of microRNAs as potential biomarkers in the major diseases (e.g., cancers and cardio-cerebrovascular diseases), which provide the basis for the design of diagnosis and treatment plan and the improvement of the cure rate.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is to explore the potential of circulating microRNAs as disease biomarkers, especially their application in non - invasive detection. Specifically, the paper focuses on the following aspects: 1. **Early diagnosis**: The biomarkers currently used in clinical practice (such as alpha - fetoprotein (AFP), carcinoembryonic antigen (CEA) and prostate - specific antigen (PSA)) have deficiencies in sensitivity and specificity. Therefore, new and more effective biomarkers need to be found to achieve early diagnosis of diseases. 2. **Disease progression and prognosis prediction**: Specific microRNA expression profiles can not only reflect the presence of early - stage diseases, but also reveal the dynamic development of advanced - stage diseases, prognosis prediction and drug resistance. 3. **Treatment response prediction**: The association between microRNAs and chemotherapy resistance makes them ideal biomarkers for predicting patients' response to treatment, which has important clinical value. By reviewing the current understanding of microRNAs as potential biomarkers, especially their application in major diseases (such as cancer and cardiovascular and cerebrovascular diseases), this paper provides a basis for disease diagnosis, treatment plan design and improvement of the cure rate. In addition, the paper also discusses the limitations and development trends of circulating microRNAs as biomarkers, and points out the technical and biological challenges that need to be addressed in future research.
