Abstract:OBJECTIVE To investigate the application of intelligent pressure control devices (medical irrigation and suctioning platform with pressure feedback function, and suctioning ureteral access sheath with pressure-measuring function) in flexible ureteroscopy for monitoring and control of renal pelvic pressure. MATERIALS AND METHODS Nine pigs with a total of 18 kidneys were randomly divided into 3 groups according to varied renal perfusion flow rates: group A, 50 mL/min; group B, 100 mL/min; and group C, 150 mL/min. A ureteral catheter connected to an invasive blood pressure monitor was retrogradely placed to the upper renal calyx to measure upper calyceal pressure; the suctioning sheath was connected to a 3-channel tubing and was connected to an invasive blood pressure monitor; and the platform and renal pelvic outlet pressure was measured. Perfusion flow, intraluminal pressure control, and warning values were preset on the platform. A flexible ureteroscope was inserted into the upper, mid, and lower renal calyces, and to the front end of the sheath, respectively, each location was irrigated for 5 minutes, and pressure was measured every second. RESULTS No statistical significance was found between monitor renal pelvic outlet pressure and platform renal pelvic outlet pressure for each group, or between platform renal pelvic outlet pressure and upper renal caliceal pressure for each group at different locations when varied irrigation flow rates were used, or among groups for platform renal pelvic outlet pressure at various irrigation flow rates. CONCLUSION The intelligent devices used in the flexible ureteroscopy can reliably and stably monitor and control the renal pelvic pressure within a safety range. (C) 2016 Elsevier Inc.
[Monitoring of Intrarenal Pressure During Ureteroscopic Lithotripsy].

Abstract:OBJECTIVE:To investigate the changes of intrarenal pressure in response to continuous irrigation at different pressures during ureteroscopic lithotripsy.METHODS:Eleven patients with residual ureteral calculi after percutaneous nephrolithotomy (PCNL), 8 in the proximal section, 2 in the middle section, 1 in the distal section. And one patient with calculi in the distal ureteral section underwent ureteroscopic lithotripsy. Renal fistula tube was connected with a urine dynamic detector and irrigation was performed continuously through the ureteroscope at different pressures. The baseline intra-pelvic pressure (IPP(0)) and maximum intra-pelvic pressure (IPP(max)) were recorded.RESULTS:The average IPP(0) was (16.9 +/- 3.2) cm H(2)O. The IPP levels during the ureteroscopic lithotripsy [(172.6 +/- 32.2) cm H(2)O] were all significantly higher than the IPP(0) (all P < 0.01). In response to the irrigation pump pressure of 100 mmHg, the IPP(max) during the ureteroscopic lithotripsy in the distal, middle, and proximal ureter were 74.6, 93.5, and (110.3 +/- 2.4) cm H(2)O respectively. When the irrigation pump pressure levels were 50, 100, and 200 mm Hg respectively the IPP(max) levels during the ureteroscopic lithotripsy in the proximal ureter were (51.0 +/- 4.5), (110.3 +/- 2.4), and (193.2 +/- 5.9) cm H(2)O respectively (all P < 0.01).CONCLUSION:The renal pelvic pressure is significantly increased during ureteroscopic lithotripsy. High renal pelvic pressure is associated with the irrigation pressure and the position of calculi.

Abstract:To validate the feasibility of a fiber-optic pressure sensor-based pressure measurement device for monitoring intrarenal pressure and to analyze the effects of ureteral acess sheath (UAS) type, surgical location, perfusion flow rate, and measurement location on intrarenal pressure (IRP). The measurement deviations and response times to transient pressure changes were compared between a fiber-optic pressure sensing device and a urodynamic device IRP in an in vitro porcine kidney and in a water tank. Finally, pressure measurements were performed in anesthetized female pigs using fiber-optic pressure sensing device with different UAS, different perfusion flow rates, and different surgical positions at different renal calyces and ureteropelvic junctions (UPJ). According to our operation, the result is fiber optic pressure sensing devices are highly accurate and sensitive. Under the same conditions, IRP varied among different renal calyces and UPJ ( P < 0.05). IRP was lowest at 50 ml/min and highest at 150 ml/min ( P < 0.05). Surgical position had a significant effect on IRP ( P < 0.05). 12/14 Fr UAS had a lower IRP than 11/13 Fr UAS. Therefore fiber optic pressure sensing devices are more advantageous for IRP measurements. In ureteroscopy, the type of ureteral sheath, the surgical position, the perfusion flow rate, and the location of the measurement all affect the intrarenal pressure value.

Abstract:Background: Flexible ureteroscopic lithotripsy (URS) is rapidly becoming a first-line therapy for patients with renal and ureteral calculi. Most current medical infusion devices can only monitor infusion flow and pressure, but not renal pelvic pressure (RPP). Patients and Methods: We designed a patented intelligent system to facilitate flexible URS that included an irrigation and suctioning platform and a ureteral access sheath (UAS) with a pressure-sensitive tip, enabling regulation of the infusion flow precisely and control of the vacuum suctioning by computerized real-time recording and monitoring of RPP. A stable RPP was ensured by pressure feedback technology. Ninety-three patients with renal or ureteral calculi participated in the study and received flexible URS. Gravel particles were sucked out automatically during the flexible URS. Patients were evaluated on postoperative days 1 and 30 by X-ray of kidneys, ureters, and bladder to assess stone-free status. Results: In 81 of the 93 patients, only one surgery was needed to remove the stone. There were nine cases who failed the first surgery due to difficulty in placing the UAS, but flexible URS was performed in these patients after indwelling a Double-J stent to the ureter with the calculus for 2 weeks. Three cases were converted to percutaneous nephrolithotomy due to significant ureteral stenosis. For the 90 patients who underwent flexible URS, the actual RPP was controlled under 20mmHg with clear operative visualization. The stone-free rates at postoperative days 1 and 30 were 90.0% (81/90) and 95.6% (86/90), respectively. Clavien I complications were noted in 13 cases, while Clavien II complications were noted in two cases. No major complications (Clavien III-V) were noted. Conclusions: Our patented system is technically feasible, safe, and efficient for treating upper urinary calculi. The advantages include breaking stones effectively and low complication rates because of its automatic control of RPP.

Abstract:INTRODUCTION: In order to better control renal pelvic pressure (RPP), reduce surgical complications and increase operative efficacy of minimally invasive percutaneous nephrolithotomy (MPCNL), we designed an MPCNL with intelligent monitoring and control of RPP and sought to explore its clinical efficacy and characteristics.SUBJECT AND METHODS: This study included 60 patients with renal staghorn calculi. A 5F ureteral catheter was connected to an invasive blood pressure monitor for monitoring and control of RPP. A 12F renoscope was inserted through a percutaneous tract for lithotripsy using a laser, with the help of an intelligent system to monitor RPP continuously, which includes an irrigation and suctioning platform and an integrated intraluminal pressure sensory PCNL suctioning sheath. Before the lithotripsy, intraluminal pressure control value and perfusion flow were set on the platform.RESULTS: RPP was maintained at a range of -12 to 2 mm Hg. The mean operative time was 120 min. The mean intraoperative bleeding amount was 100 ml. Fifty-two of the 60 patients the stones were cleared by one surgery.CONCLUSIONS: In our suctioning MPCNL with the intelligent control of RPP, the intraoperative RPP could be accurately controlled within the preset safety range. The stone clearance rate was high.

Fluid-electrolyte and Renal Pelvic Pressure Changes During Ureteroscopic Lithotripsy

Abstract:The objective of the study was to evaluate fluid-electrolyte and renal pelvic pressure (RPP) changes during ureteroscopic lithotripsy. Fifteen patients were detected with residual ureteral calculi after minimally invasive percutaneous nephrolithotomy (MPCNL), distal ureter calculi in three, midureter calculi in four, proximal calculi in eight. RPP was measured via the percutaneous nephrostomy tube by urodynamic study at irrigation pressures of 50, 100 and 200 mmHg. Haemoglobin (Hb), haematocrit (Hct), blood urea mitrogen (BUN), creatinine (Cre), serum sodium (Na+), potassium (K+), chlorine (Cl-) were recorded before and after ureteroscopic lithotripsy. There were no significant differences between Hb, Hct, BUN, Cre, Na+, K+, Cl-values. Baseline RPP was (16.37 +/- 3.14) cmH(2)O, RPPs were 46.06 +/- 6.85 cmH(2)O, 99.07 +/- 14.62 cmH2O and 166.27 +/- 33.08 cmH2O at irrigation pressures of 50, 100 and 200 mmHg, they were much higher than baseline RPP (p < 0.0001). RPP in the proximal ureter was much higher than in the distal and middle ureter, 50.98 +/- 4.52 cmH2O versus 40.44 +/- 4.07 cmH2O (p = 0.0004), 110.26 +/- 2.39 cmH2O versus 86.29 +/- 11.60 cmH(2)O (p = 0.0014), 193.21 +/- 5.88 cmH2O versus 135.47 +/- 20.95 cmH2O (p = 0.0002) at irrigation pressures of 50, 100 and 200 mmHg. There were no significant changes in fluid-electrolyte. RPP was significantly increased during ureteroscopic lithotripsy, it was correlated with the irrigation pressure and the position in the ureter.
Effect of renal puncture fistula on prevention of postoperative infections in patients undergoing holmium laser lithotripsy through flexible ureteroscope

Abstract:OBJECTIVE To observe the effect of renal puncture fistula on prevention of postoperative infections in patients undergoing holmium laser lithotripsy through flexible ureteroscope so as to provide guidance for effective prevention of the infections .METHODS A total of 120 renal calculi patients complicated with hydronephrosis infec-tions who were treated in Zhejiang University International Hospital from Jan 2012 to Dec 2014 were recruited as the study objects and randomly divided into the observation group and the control group ,with 60 cases in each . The control group was treated with single holmium laser lithotripsy through flexible ureteroscope ,while the ob-servation group was given the renal puncture fistula at 2 days before the surgery ,followed by the holmium laser lithotripsy through flexible ureteroscope .The operation duration ,hospitalization cost ,and time of recovery of postoperative body temperature ,time of recovery of blood routine ,and time of recovery of urine w hite blood cells were observed and compared between the two groups of patients ;the statistical analysis of data was performed with the use of SPSS 13 .0 software .RESULTS The operation duration of the observation group was (39 .7 ± 2 .6) min ,significantly shorter than (48 .8 ± 2 .3)min of the control group (t=9 .368 ,P<0 .05) .The time of recovery of postoperative body temperature ,time of recovery of blood routine ,and time of recovery of urine w hite blood cells were significantly shorter in the observation group than in the control group (t=4 .168~12 .063 ,P<0 .05) . CONCLUSION The renal puncture fistula drainage is carried out before the holmium laser lithotripsy through flexi-ble ureteroscope ,w hich may facilitate the postoperative recovery of the patients ,reduce the risk of postoperative infections ,and guarantee the therapeutic effect and safety .
The Evaluation of Intrarenal Pressure Using a Novel Single-Use Flexible Ureteroscope with Live Intrarenal Pressure Monitoring—An Experimental Study in Porcine Models

Abstract:(1) Background: This study aims to evaluate how different irrigation settings and the use of ureteral access sheaths (UASs) of varying sizes impact intrarenal pressure (IRP) during flexible ureteroscopy (fURS) procedures in pigs. (2) Methods: This study utilized three anesthetized female pigs. A novel flexible ureteroscope with the ability to continuously record live intrarenal pressure was used to perform ureteroscopy in different settings. Ureteroscopy was performed without UAS and with the use of 11/13 and 12/14 UAS at the ureteropelvic junction. Two different irrigation methods were employed for each parameter: one using gravity flow and the other using manual pumping with a commercial pump. IRP was also recorded with the presence of a laser fiber or lithotripsy basket. (3) Results: The recorded mean IRP during flexible URS without UAS was 28.25 (±11.2) under gravity irrigation; 35.46 (±10.08) under manual pumping; 22.5 (±3.05) and 30.75 (±5.79) with a laser fiber under gravity irrigation and manual pumping, respectively; and 16.45 (±1.27) and 17.27 (±3.69) with a lithotripsy basket under gravity irrigation and manual pumping, respectively. With an 11/13 UAS, the mean IRP was 15.41 (±8.57) and 19.33 (±4.26) under gravity and manual pumping irrigation, respectively; 14.56 (±2.50) and 18.64 (±5.13) with a laser in each irrigation setting, respectively; and 13.10 (±3.39) and 13.86 (±4.63) with a lithotripsy basket, respectively. With a 12/14 UAS, the mean IRP was 7.64 (±3.08) and 9.25 (±1.42) under gravity and manual pumping irrigation, respectively; 9.50 (±6.04) and 10.28 (3.46), respectively, in each setting when the laser fiber was used; and 5.32 (±1.57) and 6.26 (±1.79), respectively, when the lithotripsy basket was inserted. (4) Conclusion: Novel flexible ureteroscopes with integrated pressure sensors are both a feasible and reliable tool during fURS, giving the surgeon the ability to live-track the IRP. The results of the IRP measurements with and without UAS are in accordance with the current literature and exhibit a consistent pattern with previous studies.

An Irrigation System for Noninvasively Estimating Intrarenal Pressure During Flexible Ureteroscopy

Abstract:BACKGROUND:High intrarenal pressure (IRP) during flexible ureteroscopy (FURS) may lead to severe complications. Reported methods for measuring IRP are often inconvenient to use, expensive and involve instruments that occupy the narrow ureter.METHODS:We proposed an irrigation system, which can noninvasively estimate IRP based on the principle of fluid mechanics. To determine the feasibility of our system, we conducted irrigation experiments on a kidney phantom and a porcine kidney. The estimated IRPs were compared with the ground truth IRPs.RESULTS:When no surgical instrument was inserted into the flexible ureteroscope's working channel, our system can estimate IRP with high accuracy. When a surgical instrument was inserted, our system can approximately estimate the level of IRP.CONCLUSIONS:Our proposed irrigation system can noninvasively estimate IRP, presenting a new thought for clinical practice. In future studies, in vivo experiments are needed to further validate and improve the system.

Abstract:The objective of this study was to explore the influence of ureteral stent on renal pelvic pressure by urodynamic study. 41 patients (with unilateral renal and/or ureteral calculi) after minimally invasive percutaneous nephrolithotomy (MPCNL) were placed a 4.7-Fr ureteral stent and 16-Fr nephrostomy tube. Renal pelvic pressure of these patients was measured by urodynamic study at the 5–7 days after MPCNL. Renal pelvic pressure (RPP), intraabdominal pressure (IAP), and vesical pressure (VP) during the filling and voiding phases were detected by urodynamic study with intravesical perfusion. At the baseline, intraabdominal pressure (IAP0) was 27.52 ± 7.03 cmH2O, renal pelvic pressure (RPP0) was 33.07 ± 7.04 cmH2O; at the maximum cystometric bladder capacity (MCBC) during the filling phase, vesical pressure (VPvol) was 41.61 ± 10.34 cmH2O, renal pelvic pressure (RPPvol) was 39.44 ± 7.33 cmH2O; at the maximum vesical pressure during the voiding phase, vesical pressure (VPmax) was 74.95 ± 12.79 cmH2O, renal pelvic pressure (RPPmax) was 65.68 ± 17.03 cmH2O. (1) There was a strong relationship between RPP0 and IAP0 (P = 0.0001); (2) There was statistical significance among RPP0, RPPvol and RPPmax (P = 0.0001); (3) RPP was higher than 40 cmH2O during the voiding phase, and it was obviously relevant to the VP (P = 0.0001) but not to the MCBC (P = 0.2696). RPP increased mildly during the filling phase and dramatically during the voiding phase after stenting. RPP increased higher than the level required for a backflow (40 cmH2O) during the voiding phase. So it was encouraged to remove the stent at earlier stage or decrease using the ureteral stent if possible.

Abstract:Objective To introduce a new method to monitor the intraluminal pressure of the ureter and evaluate its clinical significance.Methods Totally 44 cases of proximal ureteral calculi were enrolled in the study group,21 cases of them had acute obstruction,and the other 23 cases hadchronic obstruction.22 cases without any calculi but needed ureteroscopy examination were enrolled as the control group.Duringthe procedure of lithotripsy,placed the ureterscopy above the obstruction segment immediately after the stone had been cleared to measure the intraluminal pressure of the ureter.Turned off the perfusion,the tail of the ureteroscopywas connected to a signal converter.The intraluminal pressure was measured and displayed in the monitor.We analyzed the correlation between the intraluminal pressure of ureter and the obstruction time,the glomerular filtration rate (GFR),the ureter diameter above stones,the depth of the hydronephrosis in the pelvic,the serum creatinine level,the urinary leukocytes count.The correlation between the intraluminal pressure of ureter and the GFR of the affected side was also analyzed in the chronic obstructive subgroup.Results The mean ureteral pressure in the acute obstructive subgroup,the chronic obstructive subgroup,and the control group is 74.5 mmHg (22-180),32.5 mmHg (9-53),10.2 mmHg (8-13),respectively.The intraluminal pressure in the study group was related to the following factors:the obstruction period (r=0.985),the diameter of the ureter above the calculus(r=0.878),the depth of hydronephronsis in the pelvic(r=0.862);but was not related to the serum creatinine level (r=0.214) or the urinary leukocytes count (r=0.047).The intraluminal pressure was correlated to the GFR of obstructive kidney (r=0.975).Conclusions This new method is simple,non-invasive and quantitative.Measuring the intraluminal pressure of the ureter has practical significance to evaluate the severity of the obstruction caused by the ureteral calculi.

Abstract:Objective: The purpose of the study was to assess the clinical efficacy and safety of a combined perfusion suction platform with pressure feedback control function and an ureteroscopic suction sheath that can measure the ureteropelvic pressure in implementing lithotripsies.Methods: Fifty-two patients who underwent lithotripsy under intelligent monitoring of ureteral intraluminal pressure from June 2016 to January 2018 were retrospectively recruited. The inclusion standard was stone diameter >1.5 cm but <2.5 cm. After the 12/14 Fr suction sheath was placed, manometer interface and suction interface of the sheath were connected to the platform via the pressure sensor and suction tube, respectively. The ureteroscope was connected to the platform perfusion pump, and the crushed stones were aspirated out under negative pressure.Results: According to the location of the stone, 21 (40.4%) cases were classified as upper ureteral stones, 19 (36.5%) were midureteral stones, and 12 (23.1%) were lower ureteral stones. Forty-seven patients underwent successful primary sheath placement and lithotripsy with a mean operative time of 34.5 (standard deviation 18.3) min. Retrograde stone migration did not occur. There were eight patients with hematuria postoperatively. Serious complication was 1.9% with one case of ureteral perforation. Stone clearance was 95.7% at Day 1-2 postoperatively, and 100% at Day 30 postoperatively.Conclusion: Ureteroscopic lithotripsy with intelligent pressure control using our device improved the efficiency of the lithotripsy and rate of stone clearance. The safety of the operation can be ensured. It is worth popularization and application in clinical practice.

Abstract:The present study was designed to take advantage of telemetry data acquisition and develop an easy and reliable system to servocontrol renal perfusion pressure (RPP). Digitized pressure signals from lower abdominal aorta in rats, reflecting RPP, was obtained by a telemetry device and dynamically exported into an Excel worksheet. A computer program (LabVIEW) compared the RPP data with a preselected pressure range and drove a bidirectional syringe pump to control the inflation of a vascular occluder around the aorta above renal arteries. When RPP was higher than the preselected range, the syringe pump inflated the occluder and decreased RPP, and vice versa. If RPP was within range, there was no action. In this way, RPP was servocontrolled within the desired range. In experiments with norepinephrine- or ANG II-induced acute increases in systemic arterial pressure (120-145 mmHg), the system controlled RPP at a constant range of 100-105 mmHg within 30-50 s and differentiated the pressure-dependent and -independent effects on renal functions. In Dahl S rats with high-salt-induced hypertension, this system maintained RPP at 100-120 mmHg over 10 days, while systemic arterial pressures were 150 +/- 5.9 mmHg in uncontrolled animals. This system also has the ability of simultaneity and multiplexing to control multiple animals. Our results suggest that this is an effective and reliable system to servocontrol RPP, which can be easily established with general computer knowledge. This system provides a powerful tool and may greatly facilitate the studies in pressure-dependent/-independent effects of a variety of cardiovascular factors.
The impact of siphoning effect on renal pelvis pressure during ureteroscopy using an in vitro kidney and ureter model

Abstract:Purpose: To experimentally measure renal pelvis pressure (PRP) in an ureteroscopic model when applying a simple hydrodynamic principle, the siphoning effect. Methods: A 9.5Fr disposable ureteroscope was inserted into a silicone kidney-ureter model with its tip positioned at the renal pelvis. Irrigation was delivered through the ureteroscope at 100 cm above the renal pelvis. A Y-shaped adapter was fitted onto the model's renal pelvis port, accommodating a pressure sensor and a 4 Fr ureteral access catheter (UAC) through each limb. The drainage flowrate through the UAC tip was measured for 60 s each run. The distal tip of the UAC was placed at various heights below or above the center of the renal pelvis to create a siphoning effect. All trials were performed in triplicate for two lengths of 4Fr UACs: 100 cm and 70 cm (modified from 100 cm). Results: PRP was linearly dependent on the height difference from the center of the renal pelvis to the UAC tip for both tested UAC lengths. In our experimental setting, PRP can be reduced by 10 cmH20 simply by lowering the distal tip of a 4 Fr 70 cm UAC positioned alongside the ureteroscope by 19.7 cm. When using a 4 Fr 100 cm UAC, PRP can drop 10 cmH20 by lowering the distal tip of the UAC 23.3 cm below the level of the renal pelvis. Conclusion: Implementing the siphoning effect for managing PRP during ureteroscopy could potentially enhance safety and effectiveness.
Comparison of intrarenal pressure between convention and vacuum-assisted ureteral access sheath using an ex vivo porcine kidney model

Abstract:Objective: This study aimed to prove the vacuum-assisted ureteral access sheath (vaUAS) is more effective in maintaining a lower IRP than conventional ureteral access sheath (cUAS). Materials: The model consisted of 12 freshly harvested adult porcine kidneys. Methods: Either a 12/14F cUAS or vaUAS was alternately inserted into the ureter to one cm below the renal pelvis. Upper, middle, and lower calyces were punctured, and 6F pressure monitor catheters were introduced. IRP with cUAS was monitored using various irrigation rates. IRP with vaUAS was monitored with the same irrigation rates; various aspiration pressures; and vent fully closed, 50% closed, and fully open. Results: cUAS with irrigation rate of 50 cc/min resulted in IRP < 30 mmHg. 50 to 100 cc/min should be used with caution. When irrigation rate exceeded 100 cc/min, IRP rose to ≥ 30 mmHg in most instances. With vent closed, vaUAS with vacuum pressure ≥ 150 mmHg and irrigation rate of 50 cc, 100 cc, and 150 cc/min generally resulted in IRPs < 5 mmHg. With vent half closed, vaUAS with vacuum pressure ≥ 300 mmHg and irrigation rate of ≤ 100 cc/min avoided IRP > 30 mmHg. vaUAS with vent open showed limited advantages over cUAS. Conclusion: vaUAS maintains lower IRP than cUAS under same parameters. Both vaUAS and cUAS can be used when irrigation is ≤ 50 cc/min vaUAS showed clear advantages over cUAS in maintaining lower pressure when irrigation rate is ≥ 100 cc/min.
An In Vivo Assessment of a Novel Temperature Control Flexible Ureteroscope System for Monitoring and Controlling Intrarenal Temperature During Flexible Ureteroscopy

Abstract:Objective: To evaluate the efficacy of a novel temperature control flexible ureteroscope system in the precise monitoring and control of intrarenal temperature (IRT) during ureteroscopy. Methods: We developed a novel temperature control flexible ureteroscope system (PT-Scope), including a temperature-monitoring ureteroscope and a irrigation-suction platform for temperature regulation. A porcine thermometry model was established to observe temperature changes under varying holmium laser powers (10, 20, 30 W) and irrigation rates (0, 20, 50 mL/min), utilizing percutaneous nephrostomy thermometry and PT-Scope measurements, with subsequent evaluation of temperature variations at different distances from the laser fiber tip. A porcine kidney stone model was established while porcine was randomly assigned to two groups: In the temperature control group, PT-Scope was connected to the irrigation-suction platform with temperature regulation, while in the nontemperature control group without temperature regulation. Comparative analysis was performed to evaluate differences in IRT between the two groups. Results: Across various laser powers and irrigation rates, the temperature measurement capability of the PT-Scope was precise, demonstrating consistency with percutaneous nephrostomy temperature measurements. The temperature obtained from the PT-Scope reflect the temperature approximately 0.05 cm away from the fiber tip, whereas temperatures close to fiber tip were significantly higher. The peak temperature of the temperature control group vs nontemperature control group were 31.70 ± 2.609°C and 44.37 ± 3.318 °C, respectively (P < 0.01). The mean temperature of the temperature control group vs nontemperature control group was 27.40 ± 2.107 °C vs 35.9 ± 1.921 °C (P < 0.01). Conclusion: PT-Scope has demonstrated the capability to precisely monitor and control IRT within a safe threshold.
The Innovative Use of Ureter Catheter in the Surgery of Obstructive Uropathy

Abstract:Purpose: Relieving obstruction and protecting renal function are the main therapeutic purposes of obstructive uropathy which often involve surgical treatment, and the ureter catheter is one of the surgical instruments commonly used in surgery. We aimed to explore the innovative use of a ureter catheter in the surgery of obstructive uropathy. Methods: We used a ureteral catheter to innovate the surgical procedure of the most common causes of obstructive uropathy: ureteral calculi and stricture, establishing an internal circulation system (ICS), proposing a three-step dilatation method, and reviewing their effects on patients. Furthermore, we introduced a simple real-time intrapelvic pressure measurement device to monitor intrarenal pressure during operation. Results: Postoperative laboratory examination showed that blood CRP, leukocyte neutrophil level, changes in the hemoglobin, urine occult blood, and positive rate of urine culture in the ICS group are significantly lower than those in the control group, corresponding to a lower incidence of bleeding and infection-related complications clinically. A three-month follow-up revealed 1/3 rate of ureteral stricture in the ICS group comparing to the control. We applied the three-step dilatation in patients with severe stenosis in which the balloon could not pass; the overall effective rate was 90.9%. The pressure of the renal pelvis was displayed on the monitor in real time. The surgeon could estimate the degree of filling of the renal pelvis and adjust the intake volume through the data. Conclusion: The innovative application of ureteral catheters in the operation of obstructive uropathy can realize the real-time monitor of intraoperative renal pelvis pressure, reduce the incidence of lithotripsy postoperative complications, and expand the indications of balloon dilatation in ureteral stricture, which has certain clinical significance.
