Abstract:Female pelvic floor dysfunction is a common disease which has become one of the five common chronic diseases threatening women's health. As the supporting structures, pelvic floor muscle is critical to support the pelvic organs, maintain continence, and prevent prolapse. The quantitative evaluation of pelvic floor muscle biomechanics is of great value for the diagnosis and treatment of female pelvic floor dysfunction. Our study aims to propose a noninvasive, quantitative, and objective method for motion analysis of the pelvic floor muscles. Ultrasound data, force data during continuous contractive activities from 37 subjects are synchronously collected using an ultrasound scanner and a self-designed intravaginal pressure acquisition device. A two-dimensional motion tracking algorithm is used to monitor the displacement of the pelvic floor muscles. The muscle displacement field is then computed. The parameter, defined as M-pu, which is the tangential displacements of one interested point near pubis shows good correlation with clinical measurement parameter (L-BP) (r = -0.93, P < 0.05). The continuous control ability of the pelvic muscles is further evaluated by measuring the average maintaining time of the maximal contraction force and the muscle thickness during this maintaining period. Both of them are well correlated with the clinical grading results of prolapse. It is concluded that the ultrasound measurements of tissue motions and biomechanics are of great value for the clinical pelvic floor prolapse diagnosis.

Abstract:Objectives: The aim of the present study is the assessment of thickness and percentage change in thickness of the musculus obliquus externus abdominis (OE), musculus obliquus internus abdominis (OI) and musculus transversus abdominis (TrA) among women suffering from stress urinary incontinence (SUI) in comparison with healthy controls. Material and methods: The thickness and its percentage changes of the OE, OI and TrA among women with and without SUI were assessed. We observed the reactions of the abdominal muscles among 84 women by means of ultrasound imaging. Results: The thickness of the OE was significantly greater in the SUI group during rest and tension of the lower part of the abdomen. Significant changes were found in the OI for the controls during isometric tension of the abdominal muscles, tension of the lower part of the abdomen, and ASLR (active straight leg raise) test of right leg. The thickness changes in the TrA were significant for the controls during isometric tension of the abdominal muscles. Conclusions: Our results suggest interactions of the pelvic floor and the abdominal muscles during pelvic floor activity, differing in the women with SUI and controls.

Abstract:Introduction Urinary incontinence is a common symptom during pregnancy, the most common type of incontinence in pregnant women is SUI. Stress urinary incontinence affects 54% of all pregnant women. These symptoms are usually experienced during pregnancy and postpartum and can negatively affect quality of life, interfering with social, professional and family activities. Pelvic floor muscle training supervised by a therapist is an effective treatment for UI in women and has been shown to be effective; However, there is still clinical uncertainty about the application of these exercises in pregnant women. Objective The objective of this study is to conduct a review of the literature and present the effect of pelvic floor exercise in pregnant women with urinary incontinence, in order to validate whether there are benefits or not. Methods An integrative review of the literature was carried out, surveys were carried out in the electronic databases PubMed and PEDro, using the descriptors "Exercise", "Physical Activities", "Urinary Incontinence" and "Pregnant Women". Only randomized clinical trials were selected and the search was carried out between April and May 2022. Results 71 studies were found, only 10 met the established criteria and agreed with the objective of the present review. Conclusion An integrative review of the literature was carried out and it was observed that pelvic muscle training helps reduce urinary incontinence in pregnant women and promotes a better quality of life, without associated risks. Abstract:Objective: This study aimed to evaluate effectiveness of pelvic floor muscles training on females’ sexual function throughout pregnancy and postpartum.Methods: A quasi experimental research design was applied at the antenatal clinic of Mansoura University hospital, Egypt. A purposive sample of 72 nulliparous singleton pregnant client free from any connotation affect sexual practice was studied. The subjects were allocated into two groups; control group received conventional antenatal and postnatal care, while the intervention group received the same care besides performing pelvic floor muscle exercise at 20 weeks pregnancy until 10-12 weeks postpartum. Female sexual function, sexual quality of life, and strength of the pelvic floor muscle contraction were evaluated at baseline, at 28-30 weeks gestation, and at 10-12 weeks postpartum, by using the female sexual function index scale, sexual quality of life-female, and the Oxford grading scale.Results: The total female sexual quality of life index scores in the intervention group were higher than those of the control group at the pregnancy and postpartum follow ups (22.3 ± 6.9 vs. 15.9 ± 6.8 & 26.0 ± 6.7 vs. 13.5 ± 6.3 respectively; p < .001). The sexual quality of life-female scores were significantly higher in the intervention group than in the control group at the pregnancy follow up evaluation (54.2 ± 15.9 vs. 36.9 ± 9.7 respectively, p < .001) and at the postpartum follow up evaluation (59.8 ± 13.5 vs. 30.3 ± 7.0 respectively, p < .001). The pelvic floor muscles strength was significantly better in the intervention group than in the control group at the pregnancy follow up evaluation and at the postpartum follow up evaluation.Conclusions: Pelvic floor muscle training during early gestation weeks was an effective tool for improvement of the female sexual function, sexual quality of life-female, and pelvic floor muscles strength during pregnancy and postpartum. Thus, it should be encouraged for pregnant mothers at early weeks and continued till postpartum.

Abstract:Objective To investigate the measurement standards and clinical application of observing anatomical morphologies and contractile function of puborectalis muscle on pelvic floor parasagittal plane by endoluminal two-dimensional ultrasonography.Methods The thickness and thickening rate of puborectalis muscle of 78 young nulliparous women were measured on pelvic floor parasagittal plane at three levelsurethral level (anterior),vaginal level (central) and rectal level (posterior),both at rest and during contraction.And the thickening rates of these parts during contraction were compared with each other.Interclass correlation coefficients were calculated to evaluate the consistency of the datas.Results At rest,thickness of anterior part of the left was (9.23 ± 0.20)mm,and that of the right was (8.99 ± 0.20)mm.During contraction,thickness of anterior parts of bilateral puborectalis muscle were significantly thicker than that of central or posterior parts (P ＜0.05).The interclass coefficients were more than 0.93 and 0.83.Conclusions The endoluminal two-dimensional ultrasonography can be used to observe morphologies and contractile function dynamically of puborectalis on pelvic floor parasagittal plane.It is simple,reproducible and worthy of clinical promotion.

Abstract:A standardized methodology for the ultrasound evaluation of the pelvic sidewall has not been proposed to date. Herein, a collaborative group of gynecologists and gynecological oncologists with extensive ultrasound experience presents a systematic methodology for the ultrasonographic evaluation of structures within the pelvic sidewall. Five categories of anatomical structures are described (muscles, vessels, lymph nodes, nerves and ureters). A step‐by‐step transvaginal ultrasound (or, when this is not feasible, transrectal ultrasound) approach is outlined for the evaluation of each anatomical landmark within these categories. Accurate assessment of the pelvic sidewall using a standardized approach improves the detection and diagnosis of non‐gynecological pathologies that may mimic gynecological tumors, reducing the risk of unnecessary and even harmful intervention. Furthermore, it plays an important role in completing the staging of malignant gynecological conditions. Transvaginal or transrectal ultrasound therefore represents a viable alternative to magnetic resonance imaging in the preoperative evaluation of lesions affecting the pelvic sidewall, if performed by an expert sonographer. A series of videoclips showing normal and abnormal findings within each respective category illustrates how establishing a universally applicable approach for evaluating this crucial region will be helpful for assessing both benign and malignant conditions affecting the pelvic sidewall. © 2024 The Author(s).
