Abstract:Foreign body ingestion is a common problem encountered at gastroenterology clinics and emergency rooms which can cause serious complications. Usually, foreign bodies are directly visible with flexible endoscopes and can be readily removed. However, when foreign bodies migrate into the deeper tissue of the esophagus, surgery is typically required. There is currently no consensus regarding the best treatment. In this report, we present two cases in which fish bones embedded in the submucosal and muscularis propria of the esophagus were successfully removed via endoscopic submucosal dissection (ESD). Both patients were discharged without any complications.
The Role Of Endovascular Stent Graft In The Successful Management Of Esophageal Foreign Body With Dangerous Vascular Complications

Abstract:The esophageal foreign body with dangerous vascular complications is a rare but usually fatal disease, including aortoesophageal fistula (AEF), arterio-esophageal fistula, penetrating aortoesophageal injury, artery dissection etc. Although surgical intervention is mandatory to these entities, there is little consensus about the optimal management of these conditions due to rare cases reported. With advent of the endovascular surgical strategy, the notion of treatment has been changed, and the successful cases reported had increased. Herein, we report three cases with dangerous vascular complications. First case is a 31-year-old man accidentally swallowing a pill with an oblong plastic packaging one day before admission and clinical manifestation of retrosternal pain and worsening dysphagia, with the diagnose of potential AEF. The second case is a 58-year-old woman swallowing a duck bone accidentally five days before admission and clinical manifestation of mild odynophagia, with the diagnose of aortic intramural hematoma. The third case is a 46-year-old-man swallowing a fish bone accidentally one day before admission and clinical manifestation of mild odynophagia and abnormal foreign body sensation, with the diagnose of one penetrating aortoesophageal injury. They were all successfully managed by combination of an emergent endovascular stent graft and endoscopic technique, and uneventful with follow-up of 14 months, 19 months and 23 months respectively.

Abstract:Esophageal food bolus impaction is a medical emergency. While most foreign objects and food boluses pass spontaneously, 10% to 20% may require endoscopic removal. Delaying esophagogastroduodenoscopy (EGD) is discouraged, as prompt removal within 24 hours is recommended by the American Society for Gastrointestinal Endoscopy (ASGE). Current gastrointestinal treatment guidelines advise against obtaining radiographs before endoscopic evaluation in suspected uncomplicated, nonbony esophageal food bolus cases.

Coca-Cola and Creon Therapy for Esophageal Food Bolus Impaction and Bezoar

Abstract:Introduction: Endoscopy is the main tool for the management of esophageal food impaction and bezoars. We describe a case where endoscopy was unsuccessful, and success was achieved with a cocktail of Creon and Coca-Cola. Case Description/Methods: 82-year-old man with no comorbidities presented with shortness of breath and dizziness. In the emergency room, he was found to have stridor and progressive respiratory distress requiring endotracheal intubation for airway protection. Direct laryngoscopic examination was normal. A CT PE study ruled out pulmonary embolism, findings suggestive of lower lobes pneumonia as well as a distended esophagus compressing the trachea, and possible esophageal food impaction were seen. Endoscopy revealed a severely dilated esophagus with a large amount of food in the entire esophagus and a phytobezoar in the lower esophagus. An esophageal overtube was deployed and repeated attempts to remove the food and bezoar with a wide range of instruments such as raptor forceps, Roth net, and talon grasping forceps, failed. Attempts to push it into the stomach were unsuccessful. Due to the size of the bezoar and prolonged duration, the procedure was aborted. Surgical removal was deferred given the patient’s advanced age and its associated mortality and morbidity. A bronchoscopy examination showed left lower lobe secretions and possible food aspiration. A nasogastric tube was placed into the esophagus and a cocktail of 3 capsules of creon dissolved in 165 mL of Coca-Cola was administered thrice a day over period of 4 days. A repeat endoscopy on day 5 showed a diffusely dilated esophagus with distal esophagitis. There was no evidence of food or bezoar in the esophagus, stomach, and proximal duodenum. Patient was liberated from mechanical ventilation on day 6 and a high-resolution esophageal manometry on day 8 revealed type 1 achalasia. Discussion: Endoscopy remains the mainstay for the diagnosis and treatment of esophageal food impaction and bezoars. Other treatment options like surgical intervention are associated with high morbidity and mortality. Case reports have described the use of Coca-Cola and the addition of Creon in the treatment of esophageal food impactions and bezoars. In our experience, this method appears to be a safe, effective and inexpensive treatment, and should be considered an alternative therapy when endoscopy is unsuccessful prior to more aggressive interventions (see Figure 1).Figure 1.: A: Coronal section of chest CT showing distended esophagus with impacted food bolus (yellow arrow). A Case Report of Food Impaction Relieved by Warm Water Drinking Therapy

Abstract:Background: Emergency department (ED) management of esophageal food impaction without high-grade obstruction is highly variable, without definitive and validated interventions being supported in medical literature. Case report: We discuss a 34-year-old male patient with diagnosis of eosinophilic esophagitis and history of multiple food impactions presenting to the ED with a food impaction. Based on a known esophageal history with repeated failure of pharmacologic interventions, the patient was submitted to a new conservative treatment of warm water drinking. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: This case report suggests warm water ingestion as a novel, safe, and successful treatment method in the management of esophageal food bolus impaction. As a conservative treatment not deviating greatly from current ED treatment options, it can reduce patient length of stay and decrease exposure to potential morbidity via invasive endoscopic or surgical intervention. It should be further investigated and validated with a large cohort study.

Abstract:The prevalence of esophageal food impaction (EFI) is rising, particularly in the context of the growing incidence of eosinophilic esophagitis (EoE). While the majority of EFI resolve spontaneously, 10-20% need endoscopic intervention, and less than 1% require surgery. Common tools used to remove EFI include alligator forceps, polypectomy snares, and Roth nets. We present 2 cases of EFI, in which food bolus extraction was performed efficiently using a biliary stone extraction balloon catheter.

Abstract:Foreign body ingestion is a common GI complaint seen in emergency rooms, with approximately 10-20% requiring endoscopic intervention. Adults with psychiatric illnesses, the incarcerated, edentulous and/or intoxicated are most at risk. Plastic and thin metal objects are not readily visualized with radiography, and computed tomography scans without contrast may be necessary for identification. Sharp objects within the esophagus increase risk of perforation and require emergent endoscopic intervention. Urgent endoscopic interventions are required for objects >6 cm above the proximal duodenum utilizing varied retrieval devices.

Comprehensive swallowing exercises to treat complicated dysphagia caused by esophageal replacement with colon: A case report

Abstract:Introduction: Surgical procedures for colonic replacement of the esophagus are most commonly associated with anastomotic stricture which cause dysphagia. In this report, we describe a rare case of a patient who demonstrated dysphagia resulting from an anastomotic stricture following esophageal replacement with the colon. All the treatments to dilate the anastomotic stricture were ineffective. To investigate the new treatment strategy for a case with complicated dysphagia, clinical dysphagia evaluations, functional oral intake scale (FOIS), videofluoroscopic swallowing study as well as high-resolution manometry were used to evaluate the swallowing function of the patient before and after treatments. Interventions: Comprehensive swallowing exercises included the protective airway maneuver, tongue pressure resistance feedback exercise, Masako Maneuver swallowing exercise, and the effortful swallowing exercise. Outcomes: Comprehensive swallowing exercises showed good effect in the patient. The FOIS score increased from level 1 to level 7. The videofluoroscopy digital analysis showed that the pharynx constriction rate was 23% and 50%, before and after treatment, respectively. The data from the high-resolution manometry displayed that: the value of the velopharyngeal pressure peak was 82.8 mmHg before treatment and 156.9 mmHg after treatment; the velopharyngeal contraction duration time was 310 milliseconds before treatment and 525 milliseconds after treatment; the value of the hypopharynx pressure peak was 53.7 mmHg before treatment and 103.2 mmHg after treatment; and the hypopharynx contraction duration time was 390 milliseconds before treatment and 1030 milliseconds after treatment. The swallowing visualization illustrated that a bolus could normally pass through the anastomotic stoma, and the bolus leakage was no longer present. The patient was able to eat various consistencies of food independently, and we were able to remove the jejunum nutrient catheter before discharging the patient. Conclusion: For patients with dysphagia caused by anastomotic stricture following esophageal replacement with colon, the swallowing function can be improved by enhancing the pharyngeal impetus when treatment using esophageal dilation is ineffective.
AGA Clinical Practice Update on the Optimal Management of the Malignant Alimentary Tract Obstruction: Expert Review

Abstract:Background & aims: The purpose of this expert review is to describe the current methodologies available to manage malignant alimentary tract obstructions as well the evidence behind the various methods (including their efficacy and safety), indications, and appropriate timing of interventions. Methods: This is not a formal systematic review but is based on a review of the literature to provide best practice advice statements. No formal rating of the quality of evidence or strength of recommendation is carried out. BEST PRACTICE ADVICE 1: For all patients with alimentary tract obstruction, the decision about specific interventions should be made in a multidisciplinary setting including oncologists, surgeons, and endoscopists and take into account the characteristics of the obstruction, patient's expectations, prognosis, expected subsequent therapies, and functional status. BEST PRACTICE ADVICE 2: For patients who present with esophageal obstruction from esophageal cancer and who are potential candidates for resection or chemoradiation, clinicians should not routinely insert a self-expanding metal stent (SEMS) without multidisciplinary review because of high rates of stent migration, higher morbidity and mortality, and potentially lower R0 (microscopically negative margins) resection rates. BEST PRACTICE ADVICE 3: For patients who present with esophageal obstruction from esophageal cancer who are potential candidates for resection and who have concerns of malnutrition, clinicians may consider the use of enteral feeding tubes (via nasogastric or percutaneous route). Clinicians should be aware of the potential risk of abdominal wall tumor seeding as well as making subsequent gastric conduit formation difficult with percutaneous endoscopic gastrostomy placement. BEST PRACTICE ADVICE 4: For patients who present with esophageal obstruction from esophageal cancer who are not candidates for resection, clinicians should consider either SEMS insertion or brachytherapy as sole therapy or in combination. Clinicians should not consider the use of laser therapy or photodynamic therapy because of the lack of evidence of better outcomes and superior alternatives. BEST PRACTICE ADVICE 5: For patients with malignant esophageal obstruction who are undergoing SEMS placement, clinicians should use a fully covered or partially covered SEMS and not an uncovered SEMS, with consideration of a stent-anchoring/fixation method. BEST PRACTICE ADVICE 6: For patients with gastric outlet obstruction who have a life expectancy greater than 2 months, have good functional status, and who are surgically fit, surgical gastrojejunostomy should be considered. BEST PRACTICE ADVICE 7: For patients with gastric outlet obstruction who are undergoing surgical gastrojejunostomy, a laparoscopic approach is favored over an open approach because of lower blood loss and shorter hospital stay. BEST PRACTICE ADVICE 8: For patients with gastric outlet obstruction who are not candidates for gastrojejunostomy (surgical or endoscopic ultrasound-guided), clinicians should consider the insertion of an enteral stent. BEST PRACTICE ADVICE 9: Enteral stents should not be used in patients with multiple luminal obstructions or severely impaired gastric motility because of the limited benefit in these scenarios. Clinicians can consider placement of a venting gastrostomy in these patients. BEST PRACTICE ADVICE 10: Depending on the experience of the endoscopist, endoscopic ultrasound-guided gastrojejunostomy is an acceptable alternative to surgical gastrojejunostomy and enteral stent placement. Clinicians should be aware that there are currently no dedicated Food and Drug Administration-approved devices for endoscopic ultrasound-guided gastrojejunostomy. BEST PRACTICE ADVICE 11: For patients with malignant colonic obstruction who are candidates for resection, insertion of SEMS is a reasonable choice as a "bridge to surgery" to allow for one-stage, elective resection. BEST PRACTICE ADVICE 12: For patients with malignant colonic obstruction who are not candidates for resection, either SEMS placement or a diverting colostomy are reasonable choices depending on the patient's goals and functional status. BEST PRACTICE ADVICE 13: SEMS is a reasonable option for patients with proximal (or right-sided) malignant obstructions, both as a "bridge to surgery" and in the palliative setting. BEST PRACTICE ADVICE 14: SEMS placement is a reasonable alternative for patients with extracolonic malignancy who are not candidates for surgery, although their placement is more technically challenging, clinical success rates are more variable, and complications (including stent migration) are more frequent.

Abstract:Foreign body ingestion is a common diagnosis that presents in emergency departments throughout the world. Distinct foreign bodies predispose to particular locations of impaction in the gastrointestinal tract, commonly meat boluses in the esophagus above a preexisting esophageal stricture or ring in adults and coins in children. Several other groups are at high risk of foreign body impaction, mentally handicapped individuals or those with psychiatric illness, abusers of drugs or alcohol, and the geriatric population. Patients with foreign body ingestion typically present with odynophagia, dysphagia, sensation of having an object stuck, chest pain, and nausea/vomiting. The majority of foreign bodies pass through the digestive system spontaneously without causing any harm, symptoms, or necessitating any further intervention. A well-documented clinical history and thorough physical exam is critical in making the diagnosis, if additional modalities are needed, a CT scan and diagnostic endoscopy are generally the preferred modalities. Various tools can be used to remove foreign bodies, and endoscopic treatment is safe and effective if performed by a skilled endoscopist.

Abstract:Head and neck cancers are a relatively common malignancy with radiotherapy as an important component of its management. Esophageal strictures or complete obstruction are major adverse effects of radiotherapy, but can also be caused by acid reflux disease, caustic injury, or Plummer-Vinson syndrome. We describe a case of complete esophageal obstruction, that was successfully managed with combined anterograde and retrograde endoscopic procedure in the absence of fluoroscopic guidance.

Abstract:This Guideline is an official statement of the European Society of Gastrointestinal Endoscopy (ESGE). It addresses the removal of foreign bodies in the upper gastrointestinal tract in adults. Recommendations Nonendoscopic measures 1 ESGE recommends diagnostic evaluation based on the patient's history and symptoms. ESGE recommends a physical examination focused on the patient's general condition and to assess signs of any complications (strong recommendation, low quality evidence). 2 ESGE does not recommend radiological evaluation for patients with nonbony food bolus impaction without complications. We recommend plain radiography to assess the presence, location, size, configuration, and number of ingested foreign bodies if ingestion of radiopaque objects is suspected or type of object is unknown (strong recommendation, low quality evidence). 3 ESGE recommends computed tomography (CT) scan in all patients with suspected perforation or other complication that may require surgery (strong recommendation, low quality evidence). 4 ESGE does not recommend barium swallow, because of the risk of aspiration and worsening of the endoscopic visualization (strong recommendation, low quality evidence). 5 ESGE recommends clinical observation without the need for endoscopic removal for management of asymptomatic patients with ingestion of blunt and small objects (except batteries and magnets). If feasible, outpatient management is appropriate (strong recommendation, low quality evidence). 6 ESGE recommends close observation in asymptomatic individuals who have concealed packets of drugs by swallowing ("body packing"). We recommend against endoscopic retrieval. We recommend surgical referral in cases of suspected packet rupture, failure of packets to progress, or intestinal obstruction (strong recommendation, low quality evidence). Endoscopic measures 7 ESGE recommends emergent (preferably within 2 hours, but at the latest within 6 hours) therapeutic esophagogastroduodenoscopy for foreign bodies inducing complete esophageal obstruction, and for sharp-pointed objects or batteries in the esophagus. We recommend urgent (within 24 hours) therapeutic esophagogastroduodenoscopy for other esophageal foreign bodies without complete obstruction (strong recommendation, low quality evidence). 8 ESGE suggests treatment of food bolus impaction in the esophagus by gently pushing the bolus into the stomach. If this procedure is not successful, retrieval should be considered (weak recommendation, low quality evidence). The effectiveness of medical treatment of esophageal food bolus impaction is debated. It is therefore recommended, that medical treatment should not delay endoscopy (strong recommendation, low quality evidence). 9 In cases of food bolus impaction, ESGE recommends a diagnostic work-up for potential underlying disease, including histological evaluation, in addition to therapeutic endoscopy (strong recommendation, low quality evidence). 10 ESGE recommends urgent (within 24 hours) therapeutic esophagogastroduodenoscopy for foreign bodies in the stomach such as sharp-pointed objects, magnets, batteries and large/long objects. We suggest nonurgent (within 72 hours) therapeutic esophagogastroduodenoscopy for medium-sized blunt foreign bodies in the stomach (strong recommendation, low quality evidence). 11 ESGE recommends the use of a protective device in order to avoid esophagogastric/pharyngeal damage and aspiration during endoscopic extraction of sharp-pointed foreign bodies. Endotracheal intubation should be considered in the case of high risk of aspiration (strong recommendation, low quality evidence). 12 ESGE suggests the use of suitable extraction devices according to the type and location of the ingested foreign body (weak recommendation, low quality evidence). 13 After successful and uncomplicated endoscopic removal of ingested foreign bodies, ESGE suggests that the patient may be discharged. If foreign bodies are not or cannot be removed, a case-by-case approach depending on the size and type of the foreign body is suggested (weak recommendation, low quality evidence).

Abstract:Introduction: The occurrence of acute respiratory failure as a result of esophageal bezoars is a rare phenomenon. We present a patient who failed initial endoscopic intervention. Successful resolution was achieved through a novel approach involving a combination of Creon and Coca-Cola. Subsequently, the patient was diagnosed with achalasia, a condition that potentially contributed to the formation of the esophageal bezoar. Case Description. An 82-year-old man presented with respiratory distress, necessitating endotracheal intubation for airway protection. A chest computed tomogram (CT) showed pneumonia and a distended esophagus compressing the trachea, raising the possibility of an esophageal food bolus. Endoscopy revealed a severely dilated esophagus containing a significant amount of food and a phytobezoar in the lower esophagus. He failed various endoscopic techniques to remove the obstruction. Given the patient's poor surgical candidacy, he was started in a thrice-daily regimen of Creon dissolved in 165 mL of Coca-Cola, over a 4-day period. A subsequent endoscopy revealed no discernible evidence of food or bezoar. The patient was weaned from mechanical ventilation. A high-resolution esophageal manometry identified type 1 achalasia. Conclusion: Esophageal food impaction leading to respiratory failure is rare. Endoscopy remains the mainstay approach. Surgical interventions carry significant risks. This case emphasizes the potential for noninvasive management in patients with esophageal bezoars and also underscores the significance of contemplating esophageal pathologies when addressing cases of respiratory failure. The use of Coca-Cola and Creon emerges as a safe, effective, and cost-efficient treatment, providing a feasible option when endoscopy proves unsuccessful before considering more aggressive interventions.

Abstract:Airway stenting has become integral to the therapeutic endoscopic management of benign and malignant obstructive airway diseases. Despite the increased use of stents, the absence of clear guidelines for surveillance and maintenance poses the potential for unique stent-associated complications. Our case reports a rare incident of tracheal stent dislodgement, leading to its ingestion and unexpected discovery within the stomach. This case serves the purpose of shedding light on a rare yet potentially life-threatening complication and discussing types of stent and characteristics to enhance gastroenterologists' understanding of stent-related challenges and equips them to anticipate and strategize the appropriate course of action.
Esophageal foreign bodies in adults: systematic review of the literature

Abstract:OBJECTIVE: Esophageal Foreign Body (FB) impaction represents a major challenge for healthcare providers. Aim of this systematic review was to analyse the current literature evidence on prevalence, presentation, treatment, and outcomes of impacted esophageal FB.MATERIALS AND METHODS: Literature search was conducted between 2000 and 30th June 2018 according to the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) statement.RESULTS: Sixty-one studies matched the inclusion criteria. The total number of included patients was 13,092. The cervical esophagus was the most frequent impaction site (67%) and sharp-pointed objects were the most common FB (38.1%). Retrosternal pain (78%), dysphagia (48%), and odynophagia (43.4%) were the most common symptoms. A flexible or rigid endoscopic approach was adopted in 65.1% and 16.8% of patients, respectively. Overall, 17.8% of patients had a complication related to the impacted esophageal FB or to the endoscopic manoeuvers. A surgical approach was required in 3.4% of patients. The overall mortality was 0.85%.CONCLUSIONS: Surgery is the upfront treatment in patients with esophageal perforation or endoscopically irretrievable esophageal FB. A minimally invasive approach can be adopted in selected patients. Effective prevention of FB impaction should focus on accurate diagnostic work-up to search for an underlying motility disorder after a first episode of esophageal impaction, even if transient and self-resolving.

Abstract:Achalasia is a rare esophageal motility disorder characterized by impaired peristalsis and lower esophageal sphincter (LES) dysfunction, presenting with dysphagia, regurgitation, chest pain, and weight loss. Barium swallow studies are commonly used in diagnosing achalasia, showing a dilated esophagus with a "bird beak" appearance at the gastroesophageal junction (GEJ). Here we report a unique case of esophageal barolith impaction following a barium swallow study in a patient with achalasia.

Abstract:Complete obliterated esophagus (COE) is a consequence of advanced esophageal strictures that is challenging to treat due to technique complexity and high rate of complications. Esophageal stenting has been used in COE; however, there is currently no standard protocol or guideline for COE treatment given the technique's difficulty and considerable risk of bleeding and perforation. In this report, we describe a unique and successful case of COE recanalization using only anterograde endoscopic ultrasound (EUS)-guided puncture technique with stent placement.
