Abstract:Objective: To analyze the risk factors of perioperative complications within 30 days of carotid endarterectomy(CEA) in the treatment of carotid atherosclerosis stenosis(CAS) during 2011-2017, and to discuss the techniques for reducing the perioperative complication rates. Methods: From August 2011 to August 2017, 486 patients with CAS were retrospective included, and 61 of them underwent bilateral CEA, with a total of 547 cases of CEA included. Perioperative complications were collected within 30 days after operation, and the risk factors related to perioperative complications were analyzed by statistical analysis. Results: In total 547 cases, 12 cases had a postoperative stroke, while 1 case died. A total of 7 cases underwent cranial nerve injury, and 5 cases had an incision related complications. In chi-square test analysis, data suggested that there was a significant difference in the incidence of complications in patients with heart disease, preoperative neurological score difference, contralateral carotid serious stenosis or occlusion and intraoperative shunt in CCA/ICA technique application (P<0.05). In the multivariate Logistic regression, it suggested that poor preoperative neurological score and contralateral carotid serious stenosis or occlusion were independent risk factors for perioperative stroke and death. Conclusion: Our results showed that CEA is effective to prevent stroke and treat patients with CAS. Patients with poor preoperative neurological score and contralateral carotid serious stenosis or occlusion may increase the risk of postoperative stroke rates.
Risk factors of intracranial infection after operation in department of neurosurgery

Abstract:OBJECTIVE To explore the risk factors of postoperative neurosurgical patients with intracranial infec‐tion ,so as to provide the basis for clinical minimizing and avoiding the risk of intracranial infection .METHODS Totally 344 cases of hospitalized patients with craniotomy in our hospital from Mar .2011 to Jul .2015 were chosen as study objects .According to the results of postoperative infection ,the independent risk factors of intracranial in‐fection were analyzed by logistic regression analysis .SPSS 19 .0 was explored for statistical analysis .RESULTS Totally 344 patients were checked and 32 of them were found to be infected with the infection rate of 9 .30% .Ex‐ternal ventricular drainage ,ASA score ,cerebrospinal fluid leakage and operation time were proved to be related risk factors causing infections (P<0 .05) .Multi‐factor non‐condition logistic regression analysis showed that ex‐ternal ventricular drainage ,ASA score ,cerebrospinal fluid leakage and operation time were independent risk fac‐tors causing intracranial infections after operation in the department of neurosurgery (P<0 .05) .CONCLUSION The factors that lead to postoperative intracranial infection in the department of neurosurgery are various .Special attention must be paid on external drainage ,ASA score ,cerebrospinal fluid leakage and operation time to improve the prognosis of patients ,develop targeted preventive measures ,improve the treatment effect ,and reduce postop‐erative infection rate .
Risk Factors Associated With Outcomes of Non-cardioembolic Ischemic Stroke In Patients Stratified By Essen Stroke Risk Score

Abstract:Abstract Background: Identifying risk factors of cardiovascular events is crucial for stroke prevention and they can be used as predictive factors of stroke outcomes.In this study, it is to evaluate the risk factors that predict outcomes of acute non-cardioembolic ischemic stroke in patients stratified by Essen Stroke Risk Score (ESRS). Methods: A retrospective study was carried out in acute non-cardioembolic ischemic stroke patients in a Chinese tertiary-care teaching hospital. ESRS stratification and factors that might influence the outcomes of stroke, as indicated by fatal or non-fatal combined vascular events of recurrent stroke, myocardial infarction, or primary intracranial hemorrhage, were documented. Univariate analysis and multivariable regression analysis was used to identify independent predictors of stroke outcomes. Results: A total of 878 patients with acute non-cardioembolic ischemic stroke who completed a mean follow-up of 5.2 years were enrolled, and 163 patients experienced at least one component of the combined vascular event. In patients with an ESRS ≤ 3, age ≥ 65 years (OR , 2.935; 95% CI 1.625-5.301, P < 0.001) and clopidogrel treatment (OR , 1.685 ; 95% CI , 1.026-2.768; P = 0.041) were significantly associated with stroke outcomes. In patients with an ESRS > 3, age ≥ 65 years (OR , 2.107, 95% CI , 1.208-3.673 ; P = 0.008) and history of diabetes (OR , 1.465 ; 95% CI , 1.041–2.062 ; P = 0.027) were risk factors for stroke outcomes , whereas clopidogrel treatment (OR , 0.542; 95% CI , 0.356–0.824; P = 0.003) was a protective factor for stroke outcomes. Conclusions: According to this study, clopidogrel treatment, blood pressure control, and glycemic control are protective factors for stroke outcomes in high-risk patients (ESRS>3).
Risk Factors and Stroke Characteristic in Patients with Postoperative Strokes

Abstract:BACKGROUND:Intravenous thrombolysis and intra-arterial thrombectomy are now the standard therapies for patients with acute ischemic stroke. In-house strokes have often been overlooked even at stroke centers and there is no consensus on how they should be managed. Perioperative stroke happens rather frequently but treatment protocol is lacking, In China, the issue of in-house strokes has not been explored. The aim of this study is to explore the current management of in-house stroke and identify the common risk factors associated with perioperative strokes.METHOD:Altogether, 51,841 patients were admitted to a tertiary hospital in Shanghai and the records of those who had a neurological consult for stroke were reviewed. Their demographics, clinical characteristics, in-hospital complications and operations, and management plans were prospectively studied. Routine laboratory test results and risk factors of these patients were analyzed by multiple logistic regression model.RESULT:From January 1, 2015, to December 31, 2015, over 1800 patients had neurological consultations. Among these patients, 37 had an in-house stroke and 20 had more severe stroke during the postoperative period. Compared to in-house stroke patients without a procedure or operation, leukocytosis and elevated fasting glucose levels were more common in perioperative strokes. In multiple logistic regression model, perioperative strokes were more likely related to large vessel occlusion.CONCLUSION:Patients with perioperative strokes had different risk factors and severity from other in-house strokes. For these patients, obtaining a neurological consultation prior to surgery may be appropriate in order to evaluate the risk of perioperative stroke.
Stroke prevention strategies for cardiac surgery: a systematic review and meta‐analysis of randomized controlled trials

Abstract:This review presents a comprehensive synthesis of existing interventions for stroke prevention in cardiac surgery, and identifies gaps in research which may benefit from future large‐scale trials. Potentially effective stroke prevention strategies identified were left atrial appendage closure (LAAC), which significantly reduced stroke incidence in patients with preoperative atrial fibrillation; and preoperative amiodarone administration. Background Stroke is a much‐feared complication of cardiac surgery, but existing literature on preventive strategies is fragmented. Hence, a systematic review and meta‐analysis of stroke prevention strategies for cardiac surgery was conducted. Methods An electronic literature search was conducted to retrieve randomized controlled trials (RCTs) investigating perioperative interventions for cardiac surgery, with stroke as an outcome. Random‐effects meta‐analyses were conducted to generate risk ratios (RRs), 95% confidence intervals (95% CI), and forest plots. Descriptive analysis and synthesis of literature was conducted for interventions not amenable to meta‐analysis, focusing on risks of stroke, myocardial infarction and study‐defined major adverse cardiovascular events (MACE). Results Fifty‐six RCTs (61 894 patients) were retrieved. Many included trials were underpowered to detect differences in stroke risk. Among pharmacological therapies, only preoperative amiodarone was shown to reduce stroke risk in one trial. Concomitant left atrial appendage closure (LAAC) significantly reduced stroke risk (RR = 0.55, 95% CI = 0.36–0.84, P = 0.006) in patients with preoperative atrial fibrillation, and there was no difference in on‐pump versus off‐pump coronary artery bypass grafting (CABG) (RR = 0.94, 95% CI = 0.64–1.37, P = 0.735). Much controversy exists in literature on the timing of carotid endarterectomy relative to CABG in patients with severe carotid stenosis. The use of preoperative remote ischemic preconditioning was not found to reduce rates of stroke or MACE. Conclusion This review presents a comprehensive synthesis of existing interventions for stroke prevention in cardiac surgery, and identifies gaps in research which may benefit from future, large‐scale RCTs. LAAC should be considered to reduce stroke incidence in patients with preoperative atrial fibrillation.

Abstract:Abstract Background Postoperative delirium is a frequent event after cardiac surgery. This meta-analysis aimed to identify relevant risk factors. Method In this meta-analysis, all original researches regarding patients undergoing mixed types of cardiac surgery (excluding transcatheter procedures) and postoperative delirium were evaluated for inclusion. On July 28th 2020, we searched PubMed, Embase, Web of Science and Scopus. Data about name of first author, year of publication, inclusion and exclusion criteria, research design, setting, method of delirium assessment, incidence of delirium, odds ratio (OR) and corresponding 95% confidence interval (CI) of risk factors, and other information relevant was collected. OR and 95% CI were used as metrics for summarized results. Random effects model was applied. Results Fourteen reports were included with a total sample size of 13,286. The incidence of delirium ranged from 4.1 to 54.9%. Eight risk factors were identified including aging, diabetes, preoperative depression, mild cognitive impairment, carotid artery stenosis, NYHA functional class III or IV, time of mechanical ventilation and length of intensive care unit stay. Conclusion In this study several risk factors associated with postoperative delirium after cardiac surgery were identified. Utilizing the information may allow us to identifying patients at high risk of developing postoperative delirium prior to delirium onset.

[Prognostic Factors of Postoperative Stroke in General and Orthopedic Surgeries: a Nested Case-Control Study].

Abstract:OBJECTIVE:To evaluate the prognostic factors of postoperative stroke in general and orthopedic surgeries using a nested case-control design.METHODS:The retrieval of information was performed from the medical records database in the Peking University Third Hospital. A total of 596 records with stroke diagnosis from January 2009 to December 2011 were recruited as the study cases, among which 29 cases were diagnosed with stroke occurred postoperatively. 174 cases with similar surgical types, date of operation and anesthesia technique were explored as control group according to the principle of the nested case-control design. The data were analyzed using Logistic regression model.RESULTS:Univariate analysis showed that age, hypertension, diabetes, stroke or TIA history, non-atrial arrhythmias during surgery were prognostic factors for postoperative stroke. Logistic regression analysis showed that patients with stroke or TIA history, atrial fibrillation and age were independent prognostic factors for postoperative stroke, stroke or TIA history were the most significant factor associated with postoperative stroke (OR=13.01), followed by atrial fibrillation (OR=7.77) and age (OR=6.40).CONCLUSION:Stroke or TIA history, atrial fibrillation, and age are independent prognostic factors for postoperative stroke. Hypertension, diabetes, and non-atrial arrhythmias during surgery are prognostic factors for postoperative stroke.
Incidence and risk factors associated with postoperative stroke in the elderly patients undergoing hip fracture surgery

Abstract:Abstract Objectives Stroke is one of the rare but devastating complications after hip fracture in the elderly. By far, there is still scarce data on postoperative stroke in elderly patients with hip fractures. Methods This was a retrospective study of prospectively collected data. Between October 2014 to December 2018, patients aged above 65 years who underwent operative treatment for hip fractures were included. Inpatient medical surveillance and scheduled telephone follow-up at 1, 3, 6, and 12 months after operation was conducted to identify who developed an incident stroke. Variables of interests were extracted from patients’ inpatient medical records. Univariate analysis and multivariate logistic regression analysis were used to identify the independent risk factors associated with stroke. Results During the study period, a total of 3743 patients were included, among whom 56 were found to have a stroke after operation, representing an incidence of 1.5% (95% CI, 1.1 to 1.9%). The multivariate analyses showed that advanced age (1-year increment; OR, 1.32; 95% CI, 1.08 to 1.48), history of previous stroke (OR, 4.79; 95% CI, 1.86 to 6.56), ASA III and above (OR, 2.62; 95% CI, 1.27 to 3.68), long-term use of aspirin (OR, 3.63; 95% CI, 1.41 to 4.78), and elevated RDW level (each increment of 1%, OR, 1.21; 95% CI, 1.02 to 1.36) were independently associated with postoperative stroke. Conclusions Although most are not modifiable, these risk factors help in counseling patients regarding the risk of postoperative stroke, individual risk stratification, and targeted optimization of medical conditions and should be firmly kept in treating surgeon’s mind.

Analysis of preoperative risk factors of postoperative complications in patients undergoing endovascular repair surgery

Abstract:Objective To summarize the perioperative outcomes of patients undergoing endovascular repair,and to screen out related preoperative risk factors in order to provide reference for the promotion of postoperative rehabilitation of patients.Methods The clinical data of 2 896 patients undergoing elective endovascular repair in the Department of Vascular Surgery,Zhongshan Hospital,Fudan University from Jan.2013 to Dec.2014 were analyzed to screen out related preoperative risk factors of postoperative complications retrospectively.Results A total of 148 patients had postoperative complications,including 2 cases of death during hospitalization.The incidence of complications in the patients older than 75 years old,with hypertension,diabetic mellitus,renal dysfunction,chronic obstructive pulmonary disease (COPD) or ASA physical status more than grade Ⅱ is 9.7％,8.1 ％,14.3 ％,8.0 ％,29.7 ％,6.3 ％,respectively.The multivariate Logistic regression analysis identified that patients older than 75 years old (P =0.000,OR =43.29,95％CI:22.51-83.28),with hypertension (P=0.000,OR=3.822,95 ％CI:2.37-6.16),diabetic mellitus (P=0.025,OR=1.714,95％CI:1.07 -2.75),renal dysfunction (P =0.017,OR =2.692,95 ％ CI:1.19-6.07) or COPD (P =0.000,OR =7.158,95％CI:3.83-13.37) and ASA physical status more than grade Ⅱ (P =0.000,OR =27.77,95％CI:13.79-55.93) were the independent risk factors with postoperative complications of endovascular repair surgery.Conclusions The patients older than 75 years old,with hypertension,diabetic mellitus,renal dysfunction or COPD and ASA physical status more than grade Ⅱ were the independent risk factors for endovascular repair surgery.

Abstract:Cerebral infarction, a common central nervous system complication after adult cardiac surgery, is one of the main factors leading to the poor prognosis of cardiac surgery patients besides cardiac insufficiency. However, there is currently no effective treatment for cerebral infarction. Therefore, early prevention and diagnosis of postoperative cerebral infarction are particularly important. There are many factors and mechanisms during and after cardiac surgery that play an important role in the occurrence of postoperative cerebral infarction, such as intraoperative embolism, systemic inflammatory response syndrome, atrial fibrillation, temperature regulation, blood pressure control, use of postoperative blood products, and so forth. The mechanism by which most risk factors act on the human body, leading to postoperative cerebral infarction, is not well understood, and further research is needed. Therefore, this paper aims to summarize and explain the relevant risk factors, mechanisms, clinical signs, imaging characteristics, and early diagnosis methods of cerebral infarction complications after cardiac surgery, and provides useful data for the establishment of related diagnosis and treatment standards.
Analysis of Perioperative Cardiovascular Risk Factors among Non-Cardiac Surgical Patients

Abstract:Objective To evaluate the independent risk factors of perioperative major cardiovascular events among non-cardiac surgical patients. Methods A retrospective study in a surgical ICU in teaching hospital. Consecutive adult (above 18 years old) non-cardiac surgical patients from December 1, 2012 to November 30, 2013 were studied. The endpoint was the main postoperative cardiovascular events, and accordingly, the patients were divided into event group and non-event group. Clinical data and operative data were recorded and compared between the two groups. The independent risk factors of cardiovascular adverse events were summarized by regression analysis. Results The 1 286 patients, including 63 cases (4. 9% ) of event group were studied. Through the single factor comparative analysis and multi-factors regression analysis, we found that age ( OR = 1. 030), the history of coronary heart disease ( OR = 2. 392), heart failure (OR = 7. 333), diabetes (OR = 1. 995), cerebral infarction (OR = 2. 230), renal failure (OR =4. 468), high risk operation ( OR = 10. 716 ), blood loss during operation ( OR = 1. 022 ) as well as emergency operation (OR = 2. 583) were the independent risk factors of major cardiovascular events (all P﹤ 0. 05). The RCRI score was statistically different between two groups (P = 0. 000). Conclusions The independent risk factors of perioperative major cardiovascular events include age, the history of coronary heart disease, heart failure, diabetes, cerebral infarction, renal failure, high risk operation, blood loss during operation as well as emergency operation.

Abstract:Background: Acute type A aortic dissection (AAAD) is a disease with high mortality, for which total aortic arch replacement (TAAR) combined with stent elephant implantation (SETI) is a reliable surgical treatment; however, it is associated with a high incidence of postoperative stroke. This retrospective study aimed to find preoperative and postoperative risk factors for postoperative stroke in patients with TAAR combined with SETI, and to provide predictive models and single-factor threshold suggestions. Methods: From October 2019 to March 2021, 229 AAAD patients who underwent TAAR and SETI were selected. Patients were divided into stroke group (n = 23) and non-stroke group (n = 206), and preoperative/intraoperative factors were evaluated by independent-samples T-test/ Mann-Whitney U test/Chi-Square test and odds ratio (OR) analysis. The Logistic regression equation and decision tree were used to construct the prediction model of the probability of postoperative stroke. Bayesian-learning model and 2-order derivation were used to calculate the inflection points of the continuous variables. Findings: Platelet count (PLT), International normalised ratio (INR) value, presence of diabetic history, and cardiopulmonary bypass (CPB) time were independent predictors of postoperative stroke (P-value < 0.05), and the above four factors were used to construct the Logistic regression equation. As for the decision-tree model, a radical model with higher accuracy in stroke predicting was chosen. Three inflection points for the effect of continuous variables (PLT count = 60 × 10^9/L; INR value = 1.82; CPB time = 300 min) on postoperative stroke were found by 2-order derivation. Interpretation: PLT count, INR value, presence of diabetic history, and CPB time were significant preoperative and intraoperative risk factors for postoperative stroke, and the identification and modeling of these factors can help us to take more active brain protection measures in high-risk patients. Risk factor analysis of postoperative complications in colorectal cancer patients

Abstract:Objective: To investigate the risk factors of complication rate after colorectal cancer operation. Methods: This study recruited a total of 254 non-emergency colorectal cancer patients admitted to our hospital between December 2005 and December 2006, and then evaluated the effects of life style, preoperative factors and intraoperative factors on postoperative complications. Results: Single factor analysis showed that the postoperative complication rate was not significantly affected by gender, age, obesity, palliative/curative resection, anesthesia style as well as preoperative drinking or smoking history. Preoperative complications (P = 0.001), tumor stage and operation time (P = 0.025) affected the postoperative complicatin rate. Multi-factor logistic regression analysis showed that preoperative complications were the only risk factor of postoperative complications [P = 0.001, OR = 5.871, (95% CI 2.958-11.651)]. Conclusion: Old age as such does not represent a contraindication for surgical treatment. Preoperative complications, operation time and tumor stage significantly affect the postoperative complication rate. Preoperative complications are the strongest risk factor of all. Therefore, reasonable perioperative managements and shortening operation time are the key to reducing postoperative complications.
Risk factors and operative risk of large vessel occlusion and stroke during cardiac surgery

Abstract:Objective: Perioperative Large Vessel Occlusions (LVOs) occurring during and following surgery are of immense clinical importance. As such, we aim to present risk factors and test if the Society of Thoracic Surgery (STS) mortality and stroke risk scores can be used to assess operative risk. Methods: Using data containing 7 index cardiac operations at a single tertiary referral center from 2010 to 2022, logistic and multivariate regression analysis was performed to identify factors that correlate to higher operative LVO and stroke rate. Odds ratios and confidence intervals were also obtained to test if the STS-Predicted Risk of Mortality (PROM) and -Predicted Risk of Stroke (PROS) scores were positively correlated to operative LVO and stroke rate. Results: Multivariate modeling showed primary risk factors for an operative LVO were diabetes (OR: 1.727 [95 % CI: 1.060-2.815]), intracranial or extracranial carotid stenosis (OR: 3.661 [95 % CI: 2.126-6.305]), and heart failure as defined by NYHA class (Class 4, OR: 3.951 [95 % CI: 2.092-7.461]; compared to Class 1). As the STS-PROM increased, the relative rate of LVO occurrence increased (very high risk, OR: 6.576 [95 % CI: 2.92-14.812], high risk, OR: 2.667 [1.125-6.322], medium risk, OR: 2.858 [1.594-5.125]; all compared to low risk). STS-PROS quartiles showed a similar relation with LVO risk (quartile 4, OR: 7.768 [95 % CI: 2.740-22.027], quartile 3, OR: 5.249 [1.800-15.306], quartile 2, OR:2.980 [0.960-9.248]; all compared to quartile 1). Conclusions: Patients with diabetes, carotid disease and heart failure are at high risk for operative LVO. Both STS-PROM and -PROS can be useful metrics for preoperative measuring of LVO risks.
A Risk Factor Analysis of 580,117 Patients from UK National Adult Cardiac Surgical Audit Cohort

Abstract:Cerebrovascular accident is the most ominous complication observed after cardiac surgery, carrying an increased risk of morbidity and mortality. Analysis of the problem shows its multidimensional nature. In this study, we aimed to identify major determinants among classic variables, either demographic, clinical or type of surgical procedure, based on the analysis of a large dataset of 580,117 patients from the UK National Adult Cardiac Surgical Audit (NACSA). For this purpose, univariate and multivariate logistic regression models were utilized to determine associations between predictors and dependent variable (Stroke after cardiac surgery). Odds ratios (ORs) and 95% confidence intervals (CIs) were constructed for each independent variable. Statistical analysis allows us to confirm with greater certainty the predictive value of some variables such as age, gender, diabetes mellitus (diabetes treated with insulin OR = 1.37, 95%CI = 1.23–1.53), and systemic arterial hypertension (OR = 1.11, 95%CI = 1.05–1.16);, to emphasize the role of preoperative atrial fibrillation (OR = 1.10, 95%CI = 1.03–1.16) extracardiac arteriopathy (OR = 1.70, 95%CI = 1.58–1.82), and previous cerebral vascular accident (OR 1.71, 95%CI = 1.6–1.9), and to reappraise others like smoking status (crude OR = 1.00, 95%CI = 0.93–1.07 for current smokers) or BMI (OR = 0.98, 95%CI = 0.97–0.98). This could allow for better preoperative risk stratification. In addition, identifying those surgical procedures (for example thoracic aortic surgery associated with a crude OR of 3.72 and 95%CI = 3.53–3.93) burdened by a high risk of neurological complications may help broaden the field of preventive and protective techniques.

Risk Factors for Stroke Development After Thoracic Aortic Surgery

Abstract:Objectives: Stroke after thoracic aortic surgery is a complication that is associated with poor outcomes. The aim is to characterize the intraoperative risk factors for stroke development. Design: A retrospective analysis. Setting: Tertiary, high-volume cardiac surgery center. Participants: Patients who had surgical repair of thoracic aortic diseases from January 1, 2017, through December 31, 2021. Interventions: None. Measurements and main results: A total of 704 patients were included, of whom 533 had ascending aortic aneurysms, and 171 had type A aortic dissection. The incidence of postoperative stroke was 4.5% (95% CI 2.9%-6.6%) for ascending aortic aneurysms compared with 12.3% (95% CI 7.8%-18.16%) in type-A aortic dissections. Patients who developed postoperative strokes had significantly lower intraoperative hemoglobin median (7.5 gm/dL [IQR 6.8-8.6] v 8.55 gm/dL [IQR 7.3-10.0]; p < 0.001). The median cardiopulmonary bypass time was 185 minutes (IQR 136-328) in the stroke group versus 156 minutes (IQR 113-206) in the nonstroke group (p = 0.014). Circulatory arrest was used in 57.8% versus 38.5% of the nonstroke patients (p = 0.017). The initial temperature after leaving the operating room was lower, with a median of 35.0°C (IQR 34-35.92) in the stroke group versus 35.5°C (IQR 35-36) in the nonstroke cohort (p = 0.021). Conclusions: This single-center study highlighted the potential importance of intra-operative factors in preventing stroke. Lower hemoglobin, longer duration of cardiopulmonary bypass, deep hypothermic circulatory arrest, and postoperative hypothermia are potential risk factors for postoperative stroke. Further studies are needed to prevent this significant complication in patients with thoracic aortic diseases.

Abstract:Objective:To improve the level of prevention,diagnosis and treatment for patiens with psychonosema after cardiac surgery in a surgical intensive care unit. Methods:Analyse the clinical situation,risk factors,therapies, therapeutic effect and prognosis of patiens with psychonosema after cardiac surgery in a surgical intensive care unit. Results:The incidence of postoperative psychonosema was 4.8%.After treatment,26 patiens fully cured while 3 patiens died. Conclusion: Eld,low degree of heart function,affiliated another disease, ect,are preoperative risk factors.Type of operations,extracorporeal circulation,duration of extracorporeal circulation,technical of operation and extracorporeal circulation,ect,are intraoperative risk factors.And the environment of ICU,length of stay in ICU, duration of tracheal cannula,insufficiency of cardiorespiratory function,infection,drug abuse,ect,are postoperative risk factors.
[Analysis of Factors Related to Complications of Vascular Surgeries: a Single-Center and Prospective Cohort Study].

Abstract:OBJECTIVE:To analysis the factors related to the complications of vascular surgeries in a general hospital.METHODS:A single-center and prospective study was organized from January, 2012 to December, 2012 in Peking Union Medical College Hospital. Patients who underwent selective primary vascular procedures were included in the study.RESULTS:Totally 826 consecutive cases who met the inclusive criteria were recruited, with 566 males (68.5%) and an average age of (59 ± 14) (13-89) years old. There were 38 patients (4.6%) suffering complications after the procedures, including 2 deaths (0.2%) during hospitalization. The complication rates were significantly higher in the patients with hypertension, cardiac disease, history of cerebral strokes, renal function insufficiency or diabetic mellitus (P < 0.05). The complication rates were significantly higher in group with high grade of ASA (ASA class 3 and 4) or in group underwent high grade of vascular surgeries (Class 3 and 4) , comparing with group with low grade of ASA (ASA class 1 and 2) (relative risk ratio, RR = 2.579; 95%CI: 1.333-4.993; P < 0.01) , or with group underwent low grade of vascular surgeries (Class 1 and 2) (RR = 4.268; 95%CI: 2.197-8.293; P < 0.01).CONCLUSIONS:Hypertension, cardiac disease, history of cerebral stroke, renal function insufficiency or diabetic mellitus are risk factors related to complications of vascular surgeries. The patients with high grade of ASA or underwent high grade of vascular surgeries may have significant higher risks of complications after the procedures.
Risk Factors of Postoperative Atrial Fibrillation After Isolated Coronary Artery Bypass Grafting Surgery in the Recent 10 Years: Clinical Analysis of 6229 Patients

Abstract:Background: Postoperative atrial fibrillation (POAF) is a common complication after coronary artery bypass grafting (CABG) that prolongs hospitalization and increases expenses. Hypothesis: Perioperative risk factors may predict POAF. Methods: From March 2015 to January 2023, 6229 patients who underwent isolated CABG and were in sinus rhythm before CABG were included in this retrospective study. The preoperative and postoperative variants of patients were collected and analyzed by univariate analyses between the patients with and without POAF. Multivariate logistic regression analysis was then used to study the independent risk factors for POAF. Results: The incidence of POAF in this group of patients was 30.94%. Univariate analyses demonstrated that age (p < 0.001), hypertension (p < 0.001), smoking (p < 0.05), cardiopulmonary bypass (CPB) time (p < 0.01), and ejection fraction (EF, p < 0.01) were the risk factors for POAF. Multivariate logistic regression analysis determined the independent risk factors associated with POAF were old age (odds ratio [OR] = 1.062, p = 0.000) and low EF (OR = 0.980; p = 0.008). Conclusions: In the current era, after isolated CABG surgery, there is still a quite high incidence of POAF (30.94% in this group of CABG patients). The main risk factors correlating to POAF include age, hypertension, smoking, CPB time, and EF. Among these factors, multivariate analysis identified old age and low EF as the independent risk factors associated with POAF. Particular care should be taken in the perioperative period for these patients in the prevention of POAF.
Usefulness of Statins Pretreatment for the Prevention of Postoperative Atrial Fibrillation in Patients Undergoing Cardiac Surgery

Abstract:Abstract Background. Postoperative atrial fibrillation (AF) remains the most common arrhythmic complication following cardiac surgery. We performed a meta-analysis based on all currently available randomized controlled trials (RCTs) to confirm the hypothesis that statins pretreatment may lower the risk of postoperative AF in patients undergoing cardiac surgery. Methods and results. The published literature was scanned by formal searches of electronic databases up through August 2010. RCTs were eligible for inclusion if they compared preoperative statins treatment versus control in patients scheduled for cardiac surgery and had the data of postoperative AF reported. Pre-specified criteria were met by eight RCTs involving 841 patients. During the follow-up period, 80 of 422 patients (19.0%) in the statins pretreatment group developed postoperative AF, significantly less than 149 of 419 (35.6%) patients assigned to the control group (P < 0.001). Postoperative hospital stay was significantly shortened in patients pretreated with statins compared with the control (P < 0.01). Conclusions. This meta-analysis supports the effectiveness of statins pretreatment on reducing the incidence of postoperative AF in patients undergoing cardiac surgery.
