Exploring the role of dreams: insights from recent studies

Abstract:Purpose of review: Dreaming has only recently become a topic of scientific research. This review updates current findings on dream studies, emphasizing recent research on the neural mechanisms of dreaming. Additionally, it summarizes new evidence on the functional role of dreams, including insights from studies on dreams and nightmares during the coronavirus disease 2019 (COVID-19) pandemic. Recent findings: Recent advances on the neural basis of mental activity during sleep have shifted towards dream-related phenomena, such as dream experiences in relation to parasomnias and hypnagogic hallucinations. Although some findings are consistent with the main models explaining dream recall (i.e., continuity hypothesis; activation hypothesis), some results contrast with the role of parieto-occipital region in dream experience. Moreover, recent findings - related to COVID-19 pandemic - underlined that dream experiences could express emotion regulation processes as well as provide a simulation of reality to prepare for future dangerous or social interactions. Summary: Overall, we highlighted the intricate interplay between brain regions in dreaming and suggest that dreams serve multiple functions, from reflecting waking-life experiences to simulating adaptive responses to threats. Understanding the neural bases and functions of dreaming can provide valuable insights into human mental health, nevertheless, further research is needed.

Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.

Epidemic Dreams: Dreaming about health during the COVID-19 pandemic

Abstract:The continuity hypothesis of dreams suggests that the content of dreams is continuous with the dreamer's waking experiences. Given the unprecedented nature of the experiences during COVID-19, we studied the continuity hypothesis in the context of the pandemic. We implemented a deep-learning algorithm that can extract mentions of medical conditions from text and applied it to two datasets collected during the pandemic: 2,888 dream reports (dreaming life experiences), and 57M tweets mentioning the pandemic (waking life experiences). The health expressions common to both sets were typical COVID-19 symptoms (e.g., cough, fever, and anxiety), suggesting that dreams reflected people's real-world experiences. The health expressions that distinguished the two sets reflected differences in thought processes: expressions in waking life reflected a linear and logical thought process and, as such, described realistic symptoms or related disorders (e.g., nasal pain, SARS, H1N1); those in dreaming life reflected a thought process closer to the visual and emotional spheres and, as such, described either conditions unrelated to the virus (e.g., maggots, deformities, snakebites), or conditions of surreal nature (e.g., teeth falling out, body crumbling into sand). Our results confirm that dream reports represent an understudied yet valuable source of people's health experiences in the real world.

Abstract:Purpose: Preliminarily evaluate the feasibility and efficacy of using meditative virtual reality (VR) to improve the hospital experience of intensive care unit (ICU) patients. Methods: Effects of VR were examined in a non-randomized, single-center cohort. Fifty-nine patients admitted to the surgical or trauma ICU of the University of Florida Health Shands Hospital participated. A Google Daydream headset was used to expose ICU patients to commercially available VR applications focused on calmness and relaxation (Google Spotlight Stories and RelaxVR). Sessions were conducted once daily for up to seven days. Outcome measures included pain level, anxiety, depression, medication administration, sleep quality, heart rate, respiratory rate, blood pressure, delirium status, and patient ratings of the VR system. Comparisons were made using paired t-tests and mixed models where appropriate. Results: The VR meditative intervention was found to improve patients' ICU experience with reduced levels of anxiety and depression; however, there was no evidence suggesting that VR had any significant effects on physiological measures, pain, or sleep. Conclusion: The use of VR technology in the ICU was shown to be easily implemented and well-received by patients.

Abstract:Rehabilitation in orthopedics is not only one of the main contents of rehabilitation medicine, but also an important part of clinical orthopedics. The relationship of mutual interdependence and facilitation exists between orthopedic rehabilitation and clinical orthopedics. It emphasizes the importance of rehabilitation for improving the clinical effects. The concept of holistic integrative medicine is introduced. The meaning of rehabilitation in the orthopedics is explored focusing on the function. A multidisciplinary approach is taken to promote the functional recovery of patients and their early returning to society. Finally, from the dream of Chinese orthopedic rehabilitation, the joint development of regenerative medicine and rehabilitation training, integration of clinical orthopedics and rehabilitation, progress of medical model and a new tread in modern rehabilitation equipment are foreseen.

Abstract:Imagine being given the immense responsibility and gift as a researcher to deliver a person's final nature experience. People living with severe chronic obstructive pulmonary disease (COPD) are often sheltered from nature experiences due to their disease status; however, they have rich memories of past nature experiences. The goals of this overall journey of co-creation were two-fold: (1) co-create personalized nature-based utilizing immersive 360-degree virtual reality (VR) experiences based on participants' experiences in outdoor spaces; (2) utilize a narrative approach to explore the lived realities of people living with severe COPD. Throughout this collaborative research process, I made home visits, phone calls, and many trips to four people's personalized outdoor places in the Upstate of South Carolina. In doing so, I lived the experience of person-centered research. This essay focuses specifically on one participant, David, his wife and caregiver, Anne, and the co-creation of David's last virtual trip to his most cherished places in nature. Re-gifting David's last nature experience in his favorite outdoor places profoundly changed my research approach. This essay considers how I reconciled the disconnect between the "scientific" language used in health research and the lived experience of being a researcher during end-of-life processes, specifically attending to the terms: attrition , participant , and researcher .

Abstract:The function of dreams is a longstanding scientific research question. Simulation theories of dream function, which are based on the premise that dreams represent evolutionary past selective pressures and fitness improvement through modified states of consciousness, have yet to be tested in cross-cultural populations that include small-scale forager societies. Here, we analyze dream content with cross-cultural comparisons between the BaYaka (Rep. of Congo) and Hadza (Tanzania) foraging groups and Global North populations, to test the hypothesis that dreams in forager groups serve a more effective emotion regulation function due to their strong social norms and high interpersonal support. Using a linear mixed effects model we analyzed 896 dreams from 234 individuals across these populations, recorded using dream diaries. Dream texts were processed into four psychosocial constructs using the Linguistic Inquiry and Word Count (LIWC-22) dictionary. The BaYaka displayed greater community-oriented dream content. Both the BaYaka and Hadza exhibited heightened threat dream content, while, at the same time, the Hadza demonstrated low negative emotions in their dreams. The Global North Nightmare Disorder group had increased negative emotion content, and the Canadian student sample during the COVID-19 pandemic displayed the highest anxiety dream content. In conclusion, this study supports the notion that dreams in non-clinical populations can effectively regulate emotions by linking potential threats with non-fearful contexts, reducing anxiety and negative emotions through emotional release or catharsis. Overall, this work contributes to our understanding of the evolutionary significance of this altered state of consciousness.

Fifty years ago, my older brother and I watched the Apollo 11 mission from India, sharing a worldwide sense of pride. The legacy of Apollo continues, with technological and medical benefits ([ 1 ][3]). It has inspired a new generation to think big and take great risks. Fifty years later, I watched Chandrayaan-2, India's lunar exploration mission, from the United States, connected to my brother by the internet though thousands of miles apart. The latest Moon mission provides a cause for celebration.The public's exposure to advances in science, medicine, and engineering continues on many fronts. Fifty years ago, it was a luxury for a country to perform space exploration; now a private Moon lander is the buzz ([ 2 ][4]). A society may survive on its earthly activities, but it thrives on the human imagination that has given us rockets, satellites, and a continued search for knowledge. The Chandrayaan-2 mission drives a change in India that was initiated by the Apollo mission so many years ago.Space exploration spurs global creativity, including China's successful Chang'E-4 landing on the far side of Moon ([ 3 ][5]) and the bold attempt of the Israeli spacecraft Beresheet ([ 4 ][6]). Science can transcend time and distance to unite people and nurture their imaginations. If agencies like the Indian Space Research Organisation and the National Aeronautics and Space Administration worked together, along with China and private companies, another Moon journey would be possible. Connecting Dreams with Visual Brainstorming Instruction

Abstract:Recent breakthroughs in understanding the human brain have revealed its impressive ability to efficiently process and interpret human thoughts, opening up possibilities for intervening in brain signals. In this paper, we aim to develop a straightforward framework that uses other modalities, such as natural language, to translate the original dreamland. We present DreamConnect, employing a dual-stream diffusion framework to manipulate visually stimulated brain signals. By integrating an asynchronous diffusion strategy, our framework establishes an effective interface with human dreams, progressively refining their final imagery synthesis. Through extensive experiments, we demonstrate the method ability to accurately instruct human brain signals with high fidelity. Abstract:Impacted by the COVID-19 epidemic, the human sub-health in national high-tech zones (hereinafter referred to as high-tech zones) has become more prominent. It is critical for the mental sub-health group in the high-tech zone to relieve the anxiety and tension caused by the pressure of life and work. This paper uses SketchUp virtual engine (Unity 2019) software, and 3D roaming technology to carry out the ecological landscape transformation design of the Baotzixi ecological corridor in the East Lake High-tech Zone, to construct a 3D roaming landscape scene and measure its therapeutic effect by inviting subjects to participate in an interactive experience experiment on the ErgoLAB platform. The results illustrate that: (1) the thermogram trend shows that the more attractive the 3D roaming landscape scene is, the stronger the subjects’ interest is; (2) the participants have a positive emotional arousal state in the immersive experience of the 3D roaming landscape scene after the modification design; and (3) the mean skin conductance (SC) fluctuation variance of the subjects is 5.819%, indicating that the healing effect is significant in the state of positive emotional arousal. The research results show that there is a connection between the subjects and the 3D roaming landscape scene after the transformation design of “high interest, emotional arousal and significant healing”.

Medicine in Mind, Healing at Heart: Big Data and Its Limits in Compassionate Care

Abstract:In 2020, as a 24-year-old MD PhD student studying cardiovascular population health, I suffered a non-ST elevated myocardial infarction while playing basketball. It was confirmed the cause was familial hypercholesterolemia leading to 95%+ occlusions of both the left anterior descending and left circumflex arteries. The following reflective essay describes my experience as a patient grappling with his own mortality and how those experiences have shaped how I now view my purpose as a future physician. The purpose of this essay is to encourage all physicians and physicians in training to practice reflection and to allow themselves to fully experience the pain and suffering of the patients for whom they care, even amidst times of personal uncertainty.
