Eosinophilic Granulomatosis With Polyangiitis Presenting As Unexplained Weight Loss

Abstract:Introduction Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare disorder characterized by the presence of eosinophilia (blood eosinophil count ≥1000 cells/microL or eosinophil % > 10) and organ dysfunction. We present a patient with suspected EGPA as the cause of a constellation of symptoms, most predominantly weight loss. Case Description A 59-year-old female presented to primary care clinic reporting a 40-lb weight loss over the past year. History was significant for intermittent episodes of shortness of breath and productive cough with prior diagnosis of eosinophilic esophagitis treated with an elimination diet. Work-up with otolaryngology and pulmonology showed chronic sinusitis and normal pulmonary function with elevated fractional exhaled nitric oxide (FeNO). Bloodwork showed eosinophilia to 800 (19%) but was otherwise insignificant. The patient returned to clinic one year later after extensive worldwide travel and on new homeopathic medications, complaining of rash, edema and nerve pain of the foot with worsening weight loss. Labs showed absolute eosinophil count of 20,550 (68.1%) and she was sent to the hospital. Workup for infection, malignancy, and myeloproliferative disorders was negative. pANCA testing was negative. Patient was discharged with allergy follow-up. She was treated with prednisone and mepolizumab outpatient. Discussion EGPA commonly begins with allergic symptoms and asthma. Left untreated, hypereosinophilia can present with symptoms such as weight loss, fatigue, and progressive organ dysfunction. We report this case to encourage physicians to consider EGPA on the differential in patients with significant eosinophilia and nonspecific constitutional symptoms. Treatment includes corticosteroids and anti-IL-5 therapies such as mepolizumab.
