Abstract:A case of torsade de pointes (TdP) with complete atrioventricular block and pacemaker failure that was misdiagnosed as epilepsy is presented herein. An 82-year-old female with recurrent seizure-like attacks showed epileptiform discharge during an electroencephalogram recording. A long QT interval and severe hypokalemia induced runs of TdP, which was related to pacemaker lead fracture, was detected during Holter recording and accompanied with episodes of seizures. After a DDD pacemaker with a new ventricular lead was replaced, there was no recurrence of any seizure-like attacks. Bradycardia-mediated TdP associated with complete atrioventricular block should not be missed in patients with recurrent seizure-like attacks even after pacemaker implantation.

Abstract:Cardiac arrest due to torsade de pointes(TdP) in the acquired form of drug-induced long-QT syndrome(LQTS) is a rare but potentially catastrophic event in hospital settings.Topics reviewed include the ECG characteristics of TdP and signs of impending arrhythmia,cellular mechanisms of acquired LQTS and current opinions about genetic susceptibility,drugs and drug combinations most likely to cause TdP,methods to monitor QT intervals,and immediate management of marked QT prolongation and TdP.

Abstract:Objective:To explore the pathogenesis,diagnosis and therapy effect of torsades de pointes ventricular tachycardia.Methods:The clinic and electrocardiogram(ECG)characteristics of21cases with Tdp were analyzed,according to Jackman classification,Tdp was divided into3types.Type I(interval-dependent long Q-T interval syndrome),15cases,was mostly based on tachycardia or bradycardia due to medicine,electrolyte disturbance(hy-pokalemia,hypomagnesimia,hypocalcemia)and so on.Type II,2cases,was catecholamine-dependent(congenital Q-T interval elongation syndrome).One is catecholamine-dependent,while the other was long interval-dependent.Type III(Tdp caused by short polygeminy interval ventricular prematures),4cases,out of which2cases acute my-ocardial infarct resulted from coronary atherosclerotic heart disease,while the other2cases had no evidence for or-ganic heart disease,for this type of Tdp,verapamil could be used to terminate and prevent its episode quite effec-tively,while the other types could obtain effective outcome with magnesium salt therapy;meanwhile comprehensive treatment should be adopted with aim to different etiological factors or situations.Results:Except1cases and trau-ma who died of ventricular fibrillation due to delayed treatment,the other cases were controlled within30mins to24hours after onset.Conclusions:Most of Tdp is type I,always with syncope,among part of patients ventricular fibril-lation occur.In type III,verapamil is a wonder drug for2cases with non-organic heart disease.The other cases adopte comprehensive treatment including magnesium salt and different etiological factors or situations therapies,which all can offer Tdp an effective control.

Abstract:Funding AcknowledgementsType of funding sources: None.BackgroundFemale gender, degree of QT prolongation and genetic susceptibility are known risk factors for developing torsade de pointe (TdP) during high-degree atrioventricular block (HD-AVB).ObjectiveTo analyze the incidence and clinical characteristics of patients presenting with TdP and AVB (TdP [+]) in comparison with non-TdP patients with AVB (TdP [-]).MethodsAll the ECG from patients prospectively admitted for advanced AVB (2 to 1 and complete) at our university hospital were analyzed. Automated corrected QT (QTc), and manual measurements of QT and JT intervals, and Tpeak-Tend were performed at the time of most severe bradycardia.ResultsFrom Sept 2020 to Nov 2021, 100 patients were admitted for HD-AVB. Among them, 17 patients with TdP were identified (8 men; 81 ± 10 years). No differences could be identified concerning automated QTc, manual QTc (Bazett correction), baseline QRS width and mean left ventricular ejection fraction between the two groups. Potassium serum level on admission and mean number of QT-prolongating drugs per patient were not significantly different between the two groups, respectively: 4.34 ± 0.5 in TdP [+] versus 4.52 ± 0.6 mmol/L (p = 0.33); and 0.6 ± 0.7 in group A versus 0.5 ± 0.7 (p = 0.15). In contrast, manual QTcFR (Fridericia correction), corrected JT, Tpeak-to-end and Tpe/QT ratio were significantly increased in the TdP [+] group: respectively 486 ± 70 versus 456 ± 53 ms (p = 0.04); 433 ± 98 versus 381 ± 80 (p = 0.02); 153 ± 57 versus 110 ± 40 (p < 0.001); 0.27 ± 0.08 versus 0.22 ± 0.06 (p < 0.001).ConclusionThe incidence of TdP complicating acquired AVB was 17 % in a medium-volume center. Longer QTc (Fridericia), corrected JT and Tpeak-to-end were electrocardiographic predictors of TdP, while no clinical characteristics may predict the occurrence of this complication.Comparison between TdP [+] and TdP [-]

Abstract:Torsades de pointes (TdP) is a life-threatening arrhythmia associated with prolongation of the corrected QT (QT c ) interval on the electrocardiogram. More than 100 drugs available in Canada, including widely used antibiotics, antidepressants, cardiovascular drugs and many others, may cause QT c interval prolongation and TdP. Risk factors for TdP include QT c interval >500 ms, increase in QT c interval ≥60 ms from the pretreatment value, advanced age, female sex, acute myocardial infarction, heart failure with reduced ejection fraction, hypokalemia, hypomagnesemia, hypocalcemia, bradycardia, treatment with diuretics and elevated plasma concentrations of QT c interval–prolonging drugs due to drug interactions, inadequate dose adjustment of renally eliminated drugs in patients with kidney disease and rapid intravenous administration. Pharmacokinetic drug interactions associated with the highest risk of TdP include antifungal agents, macrolide antibiotics (except azithromycin) and drugs to treat human immunodeficiency virus interacting with amiodarone, disopyramide, dofetilide or pimozide. Other important pharmacokinetic interactions include antidepressants (bupropion, duloxetine, fluoxetine, paroxetine) interacting with flecainide, quinidine or thioridazine. Pharmacists play an important role in minimizing the risk of drug-induced QT c interval prolongation and TdP through knowledge of drugs that are associated with a known or possible risk of TdP, individualized assessment of risk of drug-induced QT c interval prolongation, awareness of drug interactions most likely to result in TdP and attention to dose reduction of renally eliminated QT c interval-prolonging drugs in patients with kidney disease. Treatment of hemodynamically stable TdP consists of discontinuation of the offending drug(s), correction of electrolyte abnormalities and administration of intravenous magnesium sulfate 1 to 2 g.

Abstract:Torsades de Pointes in the Isolated Rabbit Heart. Introduction : The exact electrophysiologic mechanism of torsades de pointes (TdP) is under intense investigation. No isolated animal heart model of this particular arrhythmia exists. Methods and Results : In isolated rabbit hearts, TdP was induced by means of bradyeardia in the presence of a high concentration of d‐sotalol (10 −4 M) and shortly after lowering the concentration of potassium and magnesium in the perfusate. Multiple simultaneous epicardial and endocardial monophasic action potentials (MAPs) and volume‐conducted 12‐lead ECGs were recorded. d‐Sotalol prolonged repolarization and increased dispersion of ventricular repolarization compared to baseline recordings. With the onset of low potassium and magnesium concentrations, repolarization was further prolonged and dispersion of repolarization was further increased followed by the occurrence of early afterdepolariZations (EADs) in the majority of MAP recordings, i.e., at both endocardial and epicardial locations of both ventricles, upon increase of EAD amplitude, triggered arrhythmias with TdP of up to 42 heats ensued in 10 of 11 hearts studied. MAP duration at 90% repolarization (APD 90 ), dispersion of APD 90 , and the incidence of EADs as well as dispersion of the QT interval and T wave area were significantly higher in heats triggering higemini, couplets, or runs of TdP. Conclusion : TdP observed in this new isolated heart model was associated with markedly increased dispersion of ventricular repolarization and the occurrence of EADs in multiple locations of the heart. TdP is initiated when the amplitude of an EAD reaches threshold for initiation of the first beat of an episode.

Abstract:Background Torsade de pointes (TdP) represents a complex polymorphic ventricular tachycardia. While the triggering mechanisms of early afterdepolarization and increased dispersion of repolarization are well investigated, the sudden self-limiting termination remains poorly understood. Objective Analysis of TdP to investigate factors causing spontaneous termination. Methods We used a large data set of Langendorff experiments in isolated rabbit hearts in which drug-induced QT prolongation, bradycardia and hypokalemia provoke TdP. We included 427 episodes with typical TdP characteristics of polymorphic, self-terminating beats and twisting QRS complexes occurring in the presence of abnormal QT prolongation due to various different QT prolonging agents. The use of eight monophasic action potential catheters allowed the characterization of action potential duration, configuration, and dispersion of repolarization beyond the capabilities of the surface ECG. To identify possible mechanisms of arrhythmia termination, the initial, midpoint, and terminal three ventricular complexes were analyzed for each episode. Results An abrupt decrease in spatial dispersion over the course of a TdP episode was identified as a precursor for termination of TdP. Within the last three beats, a sudden significant decrease in dispersion of repolarization was observed as a predictor of termination. In parallel, there was a decrease in action potential duration (APD 90 ) before termination. Also, a change in action potential configuration (APD 90/50 ratio) in terms of loss of action potential dome with restitution of action potential triangulation was observed. Conclusion In more than 400 TdP episodes homogenization of myocardial repolarization with the recovery of action potential configuration determines the termination of TdP episodes.

Abstract:Torsades de Pointes (TdP) refers to a polymorphic ventricular tachycardia (VT) with undulating QRS axis that occurs in long QT syndrome (LQTS), although the term has been used to describe polymorphic ventricular tachyarrhythmias in which QT intervals are not prolonged, such as short-coupled variant of TdP currently known as short-coupled ventricular fibrillation (VF) and Brugada syndrome. Extensive works on LQTS-related TdP over more than 50 years since it was first recognized by Dessertennes who coined the French term meaning "twisting of the points", have led to current understanding of the electrophysiological mechanism that TdP is initiated by triggered activity due to early afterdepolarization (EAD) and maintained by reentry within a substrate of inhomogeneous repolarization. While a recently emerging notion that steep voltage gradients rather than EADs are crucial to generate premature ventricular contractions provides additions to the initiation mode, the research to elucidate the maintenance mechanism hasn't made much progress. The reentrant activity that produces the specific form of VT is not well characterized. We have conducted optical mapping in a rabbit model of electrical storm by electrical remodeling (QT prolongation) due to chronic complete atrioventricular block and demonstrated that a tissue-island with prolonged refractoriness due to enhanced late Na + current (I Na−L ) contributes to the generation of drifting rotors in a unique manner, which may explain the ECG characteristic of TdP. Moreover, we have proposed that the neural Na + channel Na V 1.8-mediated I Na−L may be a new player to form the substrate for TdP. Here we discuss TdP mechanisms by comparing the findings in electrical storm rabbits with recently published studies by others in simulation models and human and animal models of LQTS.

Development of Therapeutics for Heart Failure

Abstract:One of the most feared complications in medicine is sudden death caused by drug-induced proarrhythmia. Accordingly, concerted efforts have been made to define a drug’s proarrhythmic potential before regulatory approval. Monitoring for QT prolongation is one such method. Patients with heart failure represent a particularly high-risk population for torsade de pointes because of structural and electric remodeling that impair repolarization reserve. However, these patients are frequently excluded from clinical trials. Preclinical models simulate some of the electric remodeling of heart failure and form an important part of safety evaluations during early drug development. Clinical programs usually include a thorough QT study, which attempts to find a small mean change in QTc that serves as a surrogate for the risk of torsade de pointes. The impairment of repolarization reserve in heart failure goes beyond QT prolongation, however, and other clinical methods of measuring this propensity may be necessary to obtain a complete picture of a drug’s arrhythmic potential. Heart failure (HF) is a common and serious affliction. HF is a listed diagnosis in as many as 65% to 70% of hospital admissions each year in the United States.1 The prevalence of this disease continues to increase because of to a rise in coronary artery disease and diabetes incidence, an increase in the age of the population, and a reduction in the probability of sudden cardiac death (SCD) in HF with aggressive use of implantable cardioverter-defibrillators. The net result is a larger population of patients living longer with more comorbidities, and, as a corollary, more polypharmacy. HF has been recognized as one of several clinical risk factors for druginduced proarrythmia.2,3 Many classes of drugs have been shown to improve all-cause mortality in HF and have revolutionized both the treatment and prognosis of this condition. Although there is a need for continued development of drug treatments for HF, we must ensure that these treatments do not increase the risk for life-threatening arrhythmias. Assessing this risk accurately, however, has proven challenging. Although antiarrhythmic drugs are understood to have proarrhythmic potential, it has only recently been understood that noncardiac drugs also carry such risk. The withdrawal of terfenadine (Seldane) from the market in 1998 due to several cases of torsade de pointes (TdP) markedly heightened awareness that such drugs can be cardiotoxic. Since then, numerous noncardiac drugs have been withdrawn from the market because of a TdP risk, including prenylamine, cisapride, and sparfloxacin. In a large sample of patients treated between 1995 and 2003, the use of QT-prolonging drugs in the general population was associated with a significantly increased risk of SCD.4 In patients with coronary artery disease, the use of QTprolonging drugs confers a higher risk of SCD, especially in patients with diabetes mellitus.5 This has led to increased oversight by regulatory bodies. Concern about proarrhythmia has become the most common reason for drug nonapproval since the turn of the century.6 The most important predictor of TdP is QT interval prolongation, and therefore TdP and drug-induced QT prolongation have become virtually synonymous. There appear to be 3 components that are necessary for the genesis of TdP: lengthening of the cardiac action potential duration (APD), early afterdepolarizations (EAD), and transmural dispersion of repolarization allowing for propagation of the EAD and reentry.7 Clinical risk factors for TdP, as described by Roden, include HF, which, in many ways, is an excellent example of how 1 or more of these requisite conditions can be met.2 This article will discuss the cellular basis for repolarization abnormalities in HF, the preclinical assessment of TdP risk including in vitro and in vivo models, clinical assessments including the thorough QT study, ideas for refinement of the process, and finally, examples of emerging HF treatments and how their liabilities are being assessed.

Abstract:This case series describes 2 women on prolonged therapy with class III antiarrhythmics who developed torsades de pointes polymorphic ventricular tachycardia in the setting of catheter ablation for atrial fibrillation as a result of QTc prolonging factors. Abstract:Fluoroquinolone antibacterials levofloxacin,ciprofloxacin,gatifloxacin and moxifloxacin have been reported to cause torsades de pointes(TdP).Clinical manifestations of TdP are dizziness,fainting,or even cardiac arrest.QT interval prolongation and TdP are visible on the electrocardiogram(ECG).The mechanism of TdP is unclear,it may be associated with inhibition of cardiac K+ channels and blocking of K+ outflow.The risk factors for causing TdP are female,advanced age,underlying organic heart disease(especially congestive heart failure,QT interval prolongation and bradycardia),liver and kidney dysfunction,hypokalemia,and hypomagnesemia,as well as combined use with other drugs that can cause QT interval prolongation.Once a patient develops to QT interval prolongation and TdP,the causative agent should be discontinued immediately.Potassium and magnesium supplements are given to inhibit the early after-depolarization.Artificial temporary cardiac pacing or isoprenaline is used to maintain normal heart rate.For the unconscious patient with ventricular fibrillation,external electrical cardioversion could be carried out.
Repolarization Injury and Occurrence of Torsades de Pointes During Acute Takotsubo Syndrome

Abstract:Background: During takotsubo syndrome (TS), QTc prolongation is common, reflecting repolarization injury and providing the substrate for torsades de pointes (TdP). TdP has been reported sporadically in TS, yet QTc prolongation and TdP risk are often overlooked during management. Objectives: In TS patients, we sought to document TdP incidence, characteristics of patients with TdP, and association of QTc with postdischarge survival. Methods: Among consecutive TS patients at a single institution, we documented admission and discharge QTc, TdP incidence, and postdischarge 1-year mortality from 2006 to 2019. For perspective regarding TdP-TS risk, we characterized all published TdP cases from 2003 to 2022. Results: Of 259 patients, median age was 68 (range: 59-77) years; 92% were female. The QTc interval was prolonged (≥460 ms) on admission in 129 (49.8%) patients and at discharge in 140 (54%) patients. QTc was ≥500 ms either on admission or at discharge in 98 (37.8%) patients. In-hospital TdP incidence was 0.8%. Postdischarge mortality was associated with admission but not discharge, QTc: <460 ms (1.6%); 460-499 ms (12.6%); ≥500 ms (8.8%); P = 0.0056. Among 38 published TdP-TS cases, 80% of TdP events were within 48 hours of hospitalization, 90% of events occurred with QTc ≥500 ms, and 47.5% of events occurred with QTc ≥600 ms. Conditions associated with TdP risk were present in fewer than one-third of patients. Conclusions: During TS, QTc ≥500 ms was frequent. TdP incidence was low, with unpredictable occurrence and observed almost entirely with QTc ≥500 ms. A normal admission QTc was associated with >98% survival at 1-year postdischarge.

Abstract:Clinical practice includes contributions from physicians, pharmacists, NPs, and physician assistants. Drug safety considerations are of considerable importance. This article discusses drug-induced proarrhythmia, with a specific focus on torsades de pointes, a polymorphic ventricular tachycardia that typically occurs in self-limiting bursts that can lead to dizziness, palpitations, syncope, and seizures, but on rare occasions can progress to ventricular fibrillation and sudden cardiac death. A dedicated clinical pharmacology study conducted during a drug's clinical development program has assessed its propensity to induce torsades using prolongation of the QT interval as seen on the ECG as a biomarker.Identification of QT-interval prolongation does not necessarily prevent a drug from receiving marketing approval if its overall benefit-risk balance is favorable, but, if approved, a warning is placed in its prescribing information. This article explains why drugs can have a proarrhythmic propensity.
Proton Pump Inhibitors and Serum Magnesium Levels in Patients With Torsades de Pointes

Abstract:Background: Torsades de pointes (TdP) is a life-threatening ventricular tachycardia occurring in long QT-syndrome patients. It usually develops when multiple QT-prolonging factors are concomitantly present, more frequently drugs and electrolyte imbalances. Since proton-pump inhibitors (PPIs)-associated hypomagnesemia is an increasingly recognized adverse event, PPIs were recently included in the list of drugs with conditional risk of TdP, despite only few cases of TdP in PPI users have been reported so far. Objectives: Aim of the present study is to evaluate whether PPI-induced hypomagnesemia actually has a significant clinical impact on the risk of TdP in the general population. Methods: Forty-eight unselected patients who experienced TdP were consecutively enrolled (2008-2017). Shortly after the first TdP episode, in those patients who did not receive magnesium sulfate and/or potassium or calcium replacement therapy, serum electrolytes were measured and their relationship with PPI usage analyzed. Results: Many patients (28/48, 58%) were under current PPI treatment when TdP occurred. Among TdP patients in whom serum electrolyte determinations were obtained before replacement therapy (27/48), those taking PPIs had significantly lower serum magnesium levels than those who did not. Hypomagnesemia occurred in ~40% of patients receiving PPIs (6/14), in all cases after an extended treatment (>2 weeks). In patients taking PPIs the mean QT-prolonging risk factor number was significantly higher than in those who did not, a difference which was mainly driven by lower magnesium levels. Conclusions: In unselected TdP patients, PPI-induced hypomagnesemia was common and significantly contributed to their cumulative arrhythmic risk. By providing clinical support to current recommendations, our data confirm that more awareness is needed when a PPI is prescribed, specifically as regards the risk of life-threatening arrhythmias.

Abstract:Introduction: The rising prevalence of psychiatric disorders has resulted in a significant increase in the use of antipsychotic medications. These agents may prolong the corrected QT interval (QTc), running the risk of precipitating ventricular arrhythmias, notably Torsades de Pointes (TdP). Current recommendations vary regarding the optimal approach to safe prescribing practices and QTc surveillance for antipsychotics. This review summarizes the current literature addressing these clinical concerns. Areas covered: The physiologic basis of the QTc interval, mechanisms underlying its susceptibility to pharmacological influence, specific risks associated with atypical antipsychotic agents, and recommendations for safe prescription practices. We performed a literature review using Pubmed and Embase databases, searching for 'antipsychotics' and 'torsades de pointes.' Expert opinion: Finding a safe and universally accepted protocol for prescribing antipsychotics remains a persistent challenge in medicine. Predictive models that integrate clinical history with demographic and ECG characteristics can help estimate an individual's susceptibility to therapy-associated risks, including QTc prolongation. Agents such as ziprasidone and iloperidone are significantly more likely to prolong the QTc interval compared to others such as brexpiprazole, cariprazine, olanzapine, and clozapine. A personalized approach using low-risk medications when clinically feasible, and at the lowest efficacious dose, offers a promising path toward safer antipsychotic prescribing.

Abstract:Prolonged QT interval is usually seen on routine electrocardiogram (ECG), but in some patients it may only be seen immediately before the evolution of torsades de pointes (Tdp). To unmask the potential risk of Tdp in a patient with normal or borderline prolongation of QTc interval and recurrent syncope, dobutamine was given intravenously at a rate of 20 microg/kg/min. Strikingly, QTc prolongation was induced along with syncope after dobutamine infusion. Torsades de pointes occurred 5 d later when the patient received an implantable cardioverter defibrillator (ICD). Genetic testing revealed a mutation of the KNCQ(1)-gene encoding serine instead of glycine. The patient was treated with 75 mg of metoprolol twice daily, and at 12 mo follow-up he had no syncope or chest tightness. The ICD revealed no ventricular tachyarrhythmias or therapy delivered.
Torsade de pointes caused by citalopram during the pacemaker battery-depletion phase: A case report

Abstract:Drug-induced QT prolongation, primarily antiarrhythmic drugs, is a common cause of torsade de pointes (TdP). Although there have been previous reports of drug-induced TdP in patients, it has not been well documented when caused by citalopram during the pacemaker battery-depletion phase. To improve delirium recognition, we report a case of citalopram-induced TdP during the pacemaker battery-depletion phase. An 84-year-old Chinese female was brought to the hospital presenting recurrent syncope. She lost consciousness and was admitted after her syncope TdP was documented. Her pacemaker was inspected and found to be operating in an extremely ineffective manner. Although she had prolonged QT interval after the pacemaker was replaced, she did not suffer another syncope attack, and ECG monitoring revealed no cardiac arrhythmia or TdP. During her admission, she was treated with citalopram for depression. Citalopram was discontinued when the QT interval shortened progressively. In this study, we described a case of citalopram-induced TdP during the depletion phase of a pacemaker battery. This case should serve as a cautionary lesson to clinicians to avoid using citalopram during the pacemaker battery-depletion phase.

Abstract:The aim of the present study is to report the diagnosis and treatment of a rare case of frequent torsades de pointes (Tdp) in a child with a novel AKAP9 mutation. A 13-year-old girl suffered from repeated syncope and frequent Tdp. An electrocardiogram (ECG) showed frequent multisource premature ventricular contractions with the R-ON-T phenomenon. The QTc ranged from 410 to 468 ms. The genetic test indicated a heterozygous mutation, namely, c.11714T > C (p.M3905T), in the AKAP9 gene, which is a controversial gene in long QT syndrome. After treatment with propranolol, recurrent syncope occurred, and the patient received an implantable cardioverter defibrillator (ICD). Due to frequent electrical storms at home, the child was additionally treated with propafenone to prevent arrhythmia. The antitachycardia pacing (ATP) function in the ICD was turned off, and the threshold of ventricular tachycardia (VT) assessment was adjusted from 180 beats/min to 200 beats/min. The patient was followed up for 12 months without malignant arrhythmia and electric shock.Genetic testing may be a useful tool to determine the origin of channelopathy, but the results should be interpreted in combination with the actual situation. Rational parameter settings for the ICD and application of antiarrhythmic drugs can reduce the mortality rates of children.

Abstract:Objective: This study aimed to identify the most common and top drugs associated with the risk of torsades de pointes (TdP) based on the United States Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) database. Materials and methods: We used OpenVigil 2.1 to query FAERS database and data from the first quarter of 2004 to the third quarter of 2021 were retrieved. The Medical Dictionary for Regulatory Activities (MedDRA) was used to identify TdP cases. We listed the most common drugs associated with the reported TdP cases. Then, the reporting odds ratio (ROR) and the proportional reporting ratio (PRR) for the reporting association between different drugs and TdP risk were calculated. Meanwhile, comparisons were conducted with the QT drug lists of CredibleMeds ® in an attempt to identify drugs with a potential risk of TdP that were not on the list. Results: A total of 9,217,181 adverse event reports were identified, of which 3,807 (0.04%) were related to TdP. TdP was more likely to occur in the elderly and females. Amiodarone (464 cases) was associated with most cases of TdP. According to the disproportionality analysis, the top five drugs with the highest ROR and PRR were tolazoline (ROR 1615.11, 95% confidence interval [CI] 455.59–5725.75, PRR 969.46, χ 2 2960.10), levomethadyl (ROR 1211.01, 95% CI 302.75–4844.04, PRR 807.67, χ 2 1677.03), ibutilide (ROR 1118.74, 95% CI 425.00–2944.91, PRR 765.77, χ 2 3845.27), halofantrine (ROR 660.55, 95% CI 184.21–2368.69, PRR 519.22, χ 2 1076.31), and isoproterenol (ROR 352.20, 95% CI 227.19–546.00, PRR 307.82, χ 2 6692.53). Approximately half of the top 50 drugs (22 for ROR, 30 for PRR) were not outlined on the QT drug lists of CredibleMeds ® . Conclusion: Approximately half of the top risk drugs (22 for ROR, 30 for PRR) were not outlined in the QT drug lists of CredibleMeds ® . Notably, potential risks are of great importance and should be closely monitored in clinical practice. Also, further research is needed to investigate the association between these drugs and TdP.
