Abstract:Objectives: Development of resistance in Neisseria gonorrhoeae to ceftriaxone monotherapy or ceftriaxone plus azithromycin dual therapy is a global public health concern. The aim of this study was to analyse the trend in antimicrobial resistance in Hangzhou, China, over the period 2015-17. Methods: In total, 379 clinical isolates were collected from seven hospitals and antimicrobial susceptibility was determined using the agar dilution method. Isolates showing resistance to ceftriaxone, azithromycin or cefixime were analysed for the presence of resistance determinants. STs were determined with the N. gonorrhoeae multi antigen sequence typing (NG-MAST) method and phylogenetic analysis and strain clustering was determined using porB and tbpB sequences. Results: Ceftriaxone resistance, decreased susceptibility to ceftriaxone and azithromycin resistance were observed in 3%, 17% and 21% of the isolates, respectively. This resulted in 5% of the isolates showing both decreased susceptibility to ceftriaxone and azithromycin resistance. Importantly, resistance levels to ceftriaxone and azithromycin increased over the study period, resulting in 5% ceftriaxone resistance, 27% decreased susceptibility to ceftriaxone and 35% azithromycin resistance in 2017 and 11% of the isolates showing both decreased susceptibility to ceftriaxone and azithromycin resistance. Phylogenetic and cluster analysis showed the emergence and expansion in 2017 of a clonally related cluster containing strains with high abundance of decreased susceptibility to ceftriaxone and/or cefixime, which was related to the presence of the mosaic penA allele X. Co-resistance to azithromycin was also observed in this cluster. Conclusions: Our findings have major implications for the future reliability of ceftriaxone monotherapy and ceftriaxone plus azithromycin dual therapy in China.
Epidemic Trends of Tuberculosis in China from 1990 to 2017: Evidence from the Global Burden of Disease Study.

Abstract:Purpose: Tuberculosis remains a major public health problem globally, especially in undeveloped countries. This study aimed to evaluate and review the long-term epidemic trends of tuberculosis in China. Methods: Data were extracted from the Global Health Data Exchange. Metrics (prevalence, incidence and mortality) and Joinpoint regression were used to identify the epidemic trends. Results: From 1990 to 2017, decreasing trends in prevalence (average annual percent change, AAPC: -0.5%, 95% CI: -0.6% to -0.5%), incidence (-3.2%, 95% CI: -3.5% to -2.9%), and mortality (-5.7%, 95% CI: -6.2% to -5.3%) of tuberculosis were observed. The incidence and mortality of multidrug-resistant tuberculosis (MDR-TB) decreased with AAPC of -2.3% (-3.1% to -1.4%) and -4.9% (-5.4% to -4.5%), respectively, while the prevalence increased with an AAPC of 1.2% (0.3% to 2.0%). The burden of extensively drug-resistant tuberculosis (XDR-TB) increased with an AAPC of 12.5% (11.9% to 13.2%) in prevalence, 7.6% (6.5% to 8.7%) in incidence, and 4.5% (3.6% to 5.4%) in mortality. The disease burden of tuberculosis increased with age and peaked among those aged over 70. Conclusion: The epidemic of tuberculosis decreased in China, while the disease burden was still challenging to control. MDR-TB and XDR-TB should be emphasized along with the epidemic. It will certainly be a difficult task to achieve the post-2015 global targets by 2025 and 2035.
Spatial–temporal Analysis of Pulmonary Tuberculosis among Students in the Zhejiang Province of China from 2007–2020

Abstract:BackgroundPulmonary tuberculosis (PTB) is a serious chronic communicable disease that causes a significant disease burden in China; however, few studies have described its spatial epidemiological features in students.MethodsData of all notified PTB cases from 2007 to 2020 in the student population were collected in the Zhejiang Province, China using the available TB Management Information System. Analyses including time trend, spatial autocorrelation, and spatial–temporal analysis were performed to identify temporal trends, hotspots, and clustering, respectively.ResultsA total of 17,500 PTB cases were identified among students in the Zhejiang Province during the study period, accounting for 3.75% of all notified PTB cases. The health-seeking delay rate was 45.32%. There was a decreasing trend in PTB notifications throughout the period; clustering of cases was seen in the western area of Zhejiang Province. Additionally, one most likely cluster along with three secondary clusters were identified by spatial–temporal analysis.ConclusionAlthough was a downward trend in PTB notifications among students during the time period, an upward trend was seen in bacteriologically confirmed cases since 2017. The risk of PTB was higher among senior high school and above than of junior high school. The western area of Zhejiang Province was the highest PTB risk settings for students, and more comprehensive interventions should be strengthened such as admission screening and routine health monitoring to improve early identification of PTB.
Mohnarin Report of 2011 : Monitoring of Bacterial Resistance in Northwest China

Abstract:OBJECTIVE To investigate the resistance of clinical isolates from hospitals in China so as to provide clinical reference in the use of antibacterial agents.METHODS All the bacterial susceptibility results from 49 tertiary hospitals around China from 1st Jan,to 31st Dec,2011 were collected by unified protocol and the data was processed with software WHONET 5.6.The sensitive results from both disc diffusion and automatic microbiological system methods were acceptable.RESULTS A total of 118 868 bacterial isolates and their antibacterial susceptibility data were collected during the survey period,which included 34 208(28.8%) strains of gram-positive bacteria and 84 660(71.2%) strains of gram-negative bacteria.Staphylococcus(20 894 strains),Enterococcus(7977strains) and Streptococcus(4058 strains) were the top three gram-positive isolates,while Escherichia coli(21 780 strains),Klebsiella spp(15 097 strains),Pseudomonas aeruginosa(13928 strains) and Acinetobacter spp(13 592 strains) were the most common gram-negative isolates.The methicillin-resistant S.aureus,S.epidermidis and S.haemolyticus were 50.5%,82.6%,and 87.3%,respectively.The isolation rate of E.faecium was higher than that of E.faecalis,the drug resistance to the glycopeptide class of drugs was similar to the last monitoring;85.4% of S.pneumoniae strains were sensitive to penicillin.The detection rates of ESBLs-producing E.coli and K.pneumoniae were 71.2% and 50.3%,respectively.Enterobacteriaceae maintained the higher susceptibility to carbapenems,the drug susceptibility rate declined as compared with 2010.The non-fermenting bacteria were highly resistant to carbapenems,and the drug resistance rate of A.baumannii was increased.CONCLUSION The bacterial resistance of the clinical isolates is severe in China,especially the ESBLs-producing Enterobacteriaceae,and multidrug-resistant non-fermenting bacteria are prevalent.

Abstract:A comprehensive surveillance system for bacterial resistance in tertiary hospitals has been established in China that involves tertiary hospitals in distinct regions nationwide, enabling the collection of a large amount of antimicrobial surveillance data. Antimicrobial resistance in China has become a serious healthcare problem, with high resistance rates of most common bacteria to clinically important antimicrobial agents. Methicillin-resistant S. aureus, ESBL-producing Enterobacteriaceae and carbapenem-resistant Acinetobacter baumannii represent more than 50% of microbial isolates. Additionally, bacterial resistance to fluoroquinolones, macrolides and third-generation cephalosporins is of serious concern. The molecular epidemiology and resistance mechanisms of the antimicrobial strains in China exhibited regional specificity, as well as the influence of dissemination of international clonal complexes. The molecular characteristics of MRSA, ESBL- and carbapenemase-producing Enterobacteriaceae, and macrolide-resistant gram-positive Streptococci in China were significantly different from those in other countries and regions, while S. pneumoniae serotypes appear to have been affected by the global spread of prevalent clones in other parts of the world. Moreover, important antimicrobial resistant bacteria such as community-acquired-MRSA, multidrug-resistant P. aeruginosa and extensive-resistant A. baumannii, and the antimicrobial resistance in primary healthcare and outpatient setting should be intensely monitored and investigated in the future.

Abstract:OBJECTIVE To investigate the antibiotics resistance of clinical isolates from central-southern China in 2011 so as to provide evidence for the clinical medication.METHODS The antimicrobial susceptibility testing of the clinical isolates from 6 hospitals in the central-southern China was tested by Kirby-Bauer method or automated system,the results were determined according to CLSI,2011,and the statistical analysis was performed by WHONET5.6.RESULTS A total of 23 985 strains were isolated from 6 hospitals in central-southern China in 2011,with 69.3% of the gram-negative bacilli and 30.7% of the gram-positive cocci.The top five species of pathogens were Escherichia coli,Pseudomonas aeruginosa,Staphylococcus aureus,Acinetobacter baumannii and Klebsiella pneumonia.The detection rates of the MRSA and MRSCN were 54.3% and 75.6%,respectively.Less than 2.0% of the Enterococcus spp were resistant to vancomycin or teicoplanin.The isolation rates of ESBLs-producing E.coli,Klebsiella spp and P.mirabilis were 74.8%,54.6%,and 14.5%,respectively.The drug susceptibility rates of Enterobacteriaceae to carbapenems ranged between 90.3% and 100%.The drug resistance rates of A.baumannii to imipenem and meropenem were about 50.0%,and less than 30.0% of P.aeruginosa.The S.maltaphilia isolates were most susceptible to levofloxacin and minocycline,with the drug susceptibility rates higher than 80.0%,and less than 60.0% to ceftazidime and ticarcillin-clavulanic acid.The drug susceptibility rates of Burkholderia to meropenem and minocycline were higher than 70.0% and was the lowest to ticarcillinclavulanic acid(17.1%).The isolation rates of β-lactamase-producing H.influenzae was 44.8%,the drug susceptibility rates to all the antibiotics were higher than 70.0% except ampcillin and sulfamethoxazole / trimethoprim.CONCLUSION The gram-negative bacilli remain prevalent in the clinical isolates,and bacterial resistance rate is on the rise.The prevalence of ESBLs-producing Enterobacteriaceae isolates increase significantly.Great attention should be paid to the prevalence of vancomycin-resistant Enterococci,carbapenems-resistant Enterobacteriaceae and multi-drug resistant gram-negative bacilli.

Abstract:OBJECTIVE To approach the distribution and drug resistance of the bacteria causing bloodstream infections in China so as to guide the clinical use of antibiotics.METHODS Disc diffusion test,MIC test and E-test were used to detect the antimicrobial sensitivity of bacterial isolates from blood and bone marrow cultures.WHONET 5.6 was applied for analyzing the bacterial sensitive data from 49 tertiary Mohnarin member hospitals in China from Jan 1 to Dec 31 2011.RESULTS A total of 10 783 strains were isolated,including 5389 strains of gram-positive bacteria(50.0%),5374 strains of gram-negative bacteria(49.8%) and 20 strains of anaerobes of(0.2%).Coagulase-negative Staphylococci(3098 strains,28.7%),Escherichia coli(2066 strains,19.2%),Klebsiella spp(1018 strains,9.4%),Staphylococcus aureus(828 strains,7.7%) and Enterococcus species(730 strains,6.8%) were the most common species of isolates.The detection rates of the methicillin-resistant S.aureus and coagulase-negative Staphylococci were 50.8% and 67.4%,respectively.Vancomycin-and linezolid-resistant S.aureus and CNS strains were not detected.The resistance rates of Enterococcus faecalium and E.faecium to vancomycin were 0.5% and 3.7%,while to teicoplanin were 0.6% and 3.1%,respectively.One strain of E.faecalium and 7 strains of E.faecium resistant to linezolid were detected.The positive rates of ESBLs-producing E.coli and K.pneumoniae were 69.0% and 48.0%,respectively.Carbapenems-resistant Enterobacteriaceae kept an upward trend.The resistance rates of Staphylococcus spp isolated from the adults to quinolones were significantly higher than those from children.CONCLUSION Staphylococcus,E.coli and Klebsiella are the predominant pathogens in bloodstream infection in China.The positive rates of ESBLs-producing E.coli and K.pneumoniae cultured from blood and bone marrow are high,and the drug resistance of Enterobacteriaceae to carbapenems antibiotics keeps an upward trend.
Tuberculosis in China: A longitudinal predictive model of the general population and recommendations for achieving WHO goals.

Abstract:Background and objective: Tuberculosis (TB) remains a major public health challenge. China accounts for more than 10% of the global TB burden, and effective modelling of TB trends remains limited. Methods: We used data drawn primarily from two Chinese nation-wide cross-sectional epidemiological surveys combined with data from China's National Disease Reporting Network to construct an eight-state Markov model that simulates TB prevalence. By adjusting the relevant parameters, we evaluated which characteristics have the greatest bearing upon prevalence and efficacy of the response measures. Results: If current trends continue, the prevalence of TB in China will enter an 8-year period of decline from approximately 390 to 200 cases per 100 000 population and stabilize at 163 cases per 100 000 population, which is a figure well above the World Health Organization (WHO) goal of eliminating TB by 2050. We find that the proportion of notified cases in the population, the rate of progression from latent to active and the overall treatment success rate are the chief factors affecting disease progression. Conclusion: We suggest a 90-90-90 strategy, wherein the proportion of notified cases in the population reaches 90%, the risk of progression from latent to active is decreased by 90% compared with the current level and the overall treatment success rate is increased to 90%. This strategy could reduce TB prevalence to less than 10 cases per 100 000 population within 5 years and to 1.77 cases per 100 000 population within 50 years.

Increasing trends of non-tuberculous mycobacteria clinical isolates in Guangzhou, China

Abstract:Non-tuberculous mycobacteria (NTM) are one of major public health concern. The current study aimed to find the prevalence trends of NTM in Guangzhou, China from January 2018 to December 2023. A total of 26,716 positive mycobacterial cultures were collected. Thirty-six specimens with incomplete personal information were excluded. The remaining 26,680 specimens were identified using a gene chip method. 16,709 isolates were Mycobacterium tuberculosis (MTB) (62.63 %), and 9,971 were NTM (37.37 %). 43.43 % (4,330/9,971) of NTM isolates were male, and 56.57 % (5,641/9,971) were female (χ2 = 24.36, P < 0.05), a male to female ratio of approximately 1:1.30. Infections in individuals with aged 40 years and above was higher (77.63 %) than below 40 years (22.37 %) (χ2 = 4.94, P = 0.026). The annual NTM isolation rates from 2018 to 2023 were 32.03 %, 34.00 %, 36.27 %, 38.58 %, 38.99 %, and 43.24 %, respectively, showing an increasing trend (χ2 for trend = 0.097, P < 0.05) (R = 0.097, P < 0.05). Out of 9,971 NTM isolates, 8,881 cases include only five common NTM species (MAC, M. abscessus/M. chelonae, M. kansasii, M. fortuitum, and M. gordonae). The overall NTM isolation rate was 37.37 %. The NTM isolation rate was significantly higher than the national average, showing an increasing trend over the last six years.

Abstract:What is already known about this topic? Bacterial resistance surveillance is crucial for monitoring and understanding the trends and spread of drug-resistant bacteria. What is added by this report? The number of strains collected in 2022 increased compared to 2021. The top five bacteria, including Escherichia coli, Klebsiella pneumoniae, Staphylococcus aureus, Pseudomonas aeruginosa, and Acinetobacter baumannii, remained largely unchanged. The detection rate of methicillin-resistant strains continued to decrease. Among clinical Enterobacterales isolates, the resistance rate to carbapenems was generally below 13%, except for Klebsiella spp., which had a resistance range of 20.4% to 21.9%. Most clinical Enterobacterales isolates were highly susceptible to tigecycline, colistin, and polymyxin B, with resistance rates ranging from 0.1% to 12.6%. The detection rate of meropenem-resistant P. aeruginosa and meropenem-resistant Acinetobacter baumannii showed a decreasing trend for the fourth consecutive year. What are the implications for public health practice? Multidrug-resistant bacteria remain a significant public health challenge in clinical antimicrobial treatment. To effectively address bacterial resistance, it is essential to enhance both bacterial resistance surveillance and the prudent use of antimicrobial agents.
Genomic Epidemiology of Neisseria Gonorrhoeae in Shenzhen, China, During 2019–2020: Increased Spread of Ceftriaxone-Resistant Isolates Brings Insights for Strengthening Public Health Responses

Abstract:ABSTRACT The antimicrobial resistance (AMR) in gonorrhea poses global threat of increasing public health concern. In response to this concern, molecular surveillance has been widely utilized to detail the changes in the evolution and distribution of Neisseria gonorrhoeae during AMR transmission. In this study, we performed a comprehensive molecular surveillance of 664 N. gonorrhoeae isolates collected in Shenzhen, one of the cities with the largest mobile population in China, 2019–2020. In 2020, ceftriaxone showed an unprecedented high resistance rate of 24.87%, and 67.83% of the ceftriaxone-resistant (Cro-R) isolates harbored a nonmosaic penA allele. The Cro-R isolates with nonmosaic penA alleles showed a tremendous increasing trend from 0.00% in 2014 to 20.45% in 2020, which proves the need for monitoring nonmosaic penA-related resistance. Importantly, genotyping indicated that multilocus sequence typing ST11231 (35.71%) had a notable rate of ceftriaxone resistance, which might become the focus of future surveillance. Whole-genome sequencing analysis showed that the internationally spreading FC428 clones have circulated in Shenzhen region with typical ceftriaxone resistance (MIC ≥ 0.5 mg/L) maintained. Our surveillance combined with genomic analysis provides current information to update gonorrhea management guidelines and emphasizes that continuous AMR surveillance for N. gonorrhoeae is essential. IMPORTANCE We conducted a comprehensive molecular epidemiology analysis for antimicrobial-resistant Neisseria gonorrhoeae in Shenzhen during 2019–2020, which provided important data for personalized treatment and adjustment of monitoring strategy. Briefly, the proportion of ceftriaxone-resistant (Cro-R) isolates reached a stunning prevalence rate of 24.87% in 2020. A typical increment of Cro-R isolates with nonmosaic penA alleles proves the necessity of monitoring nonmosaic AMR mechanism and involving it into developing molecular detection methods. Whole-genome sequencing analysis showed that the international spreading FC428 clone has been circulating in Shenzhen with typical ceftriaxone resistance (MIC ≥ 0.5 mg/L) maintained. In summary, we conducted a comprehensive epidemiology study, providing significant data for therapy management. Our results not only improve the understanding of the distribution and transmission of AMR in N. gonorrhoeae but also provide effective AMR data for improving surveillance strategies in China.

Abstract:Objectives This study summarizes the epidemiological characteristics, species distribution, and drug sensitivity of clinical nontuberculous mycobacteria (NTM) isolates at the Public Health Clinical Center of Chengdu, China, from January 2017 to December 2022. Methods We retrospectively analyzed data from patients with clinically isolated NTM strains. Chi-square analysis assessed the rate of Mycobacterium strain isolation over 6 years. Results The number of samples tested for Mycobacterium tuberculosis (MTB) and/or NTM increased each year, while MTB detection decreased and NTM detection rose significantly each year (P=0.03). The average age of NTM patients was 51 ± 17.53 years, with a 14.1% HIV infection rate. The predominant isolates were Mycobacterium avium-intracellulare (MAC) and M. chelonae / M. abscessus , with 96.4% of cases being of Han ethnicity. Amikacin, moxifloxacin, and clarithromycin were effective against M. avium and M. intracellulare ; linezolid, amikacin, and cefoxitin were effective against M. chelonae / M. abscessus . Over 90% of NTM cases originated from the respiratory tract. Conclusion The NTM isolation rate in Southwest China has risen in recent years, primarily among elderly patients with a high HIV co-infection rate. The main NTM isolates were MAC and M. chelonae / M. abscessus . Amikacin, moxifloxacin, clarithromycin, and linezolid exhibited strong antibacterial activity against SGM, while amikacin and linezolid displayed relatively better antibacterial activity against RGM. The prevalence of NTM infection may be positively associated with regional economic development and health conditions.

Prevalence and Patterns of Drug-Resistant Mycobacterium tuberculosis in Newly Diagnosed Patients in China: A Systematic Review and Meta-Analysis

Abstract:Background Tuberculosis (TB), one of the deadliest infectious diseases globally, is increasingly exacerbated in China by the emergence of resistant Mycobacterium tuberculosis (MTB) strains. Drug-resistant TB, including mono-drug resistant TB, multidrug-resistant TB (MDR-TB), and extensively drug-resistant TB (XDR-TB), presents significant public health challenges. Methods We conducted a systematic literature review from January 2010 to February 2024 using databases such as PubMed, Embase, Web of Science, and Google Scholar. Our focus was on empirical data related to drug resistance patterns in newly diagnosed TB cases. Non-empirical studies were excluded through meticulous filtering. For meta-analysis, we used Review Manager (RevMan) 5.2 and assessed evidence quality using the Newcastle-Ottawa Scale (NOS). Results Our search strategy identified 40 studies that met the inclusion criteria, encompassing a total sample size of 87,667 participants. Among new TB cases, the estimated prevalence of MDR-TB in China was 6.9% (95% CI: 5.6–8.1%). Prevalence rates for mono-drug resistance to first-line anti-TB medications were as follows: isoniazid at 18.2% (95% CI: 16.4–20.6%), rifampicin at 10.5% (95% CI: 8.6–12.8%), and ethambutol at 5.7% (95% CI: 4.1–7.3%). The prevalence of streptomycin resistance, a former first-line anti-TB drug, was 17.1% (95% CI: 14.6–19.1%). The prevalence of other types of mono-drug resistance was 15.2% (95% CI: 13.9–17.3%), and for XDR-TB, it was 0.9% (95% CI: 0.6–1.4%). Conclusions The high prevalence of drug-resistant TB in China poses a significant public health challenge. There is an urgent need for targeted interventions and continued surveillance to combat the spread of drug-resistant TB.

Abstract:In 2023, a range of pulmonary infectious diseases remain a significant public health concern. Meanwhile, the growing problem of antibiotic resistance, rising tuberculosis cases with lagging control measures, and increased susceptibility to fungal pneumonia after viral infections, have complicated the diagnosis and treatment of respiratory infectious diseases. Despite these challenges, the year saw several significant research achievements in this field. Key advances include a deeper understanding of disease pathogenesis, improvements in pathogen detection technologies, the development of innovative antiviral and antibacterial drugs, progress in vaccine research, and advances in pulmonary rehabilitation methods. This review provided a concise summary of these key findings in 2023, with the aim of providing insights for future research and healthcare initiatives.

Abstract:Background: Information on prevalence and resistance spectrum of nontuberculous mycobacteria (NTM) in China is mainly based on regional or local data. A national survey of NTM pulmonary disease was carried out based on acid-fast positive sputum samples collected in 2013.Methods: Sputum samples collected from enrolled presumptive cases were cultured using L-J medium in 72 nationwide tuberculosis surveillance sites from the 31 Chinese mainland provinces and isolates were sent to national tuberculosis reference laboratory (NTRL). The species of re-cultured strains were identified by MALDI-TOF MS and minimal inhibitory concentrations (MICs) were determined to evaluate the drug susceptibility of NTM isolates.Results: Of 4917 mycobacterial isolates cultured, 317 (6.4%, 95% CI, 5.8% to 7.2%) were confirmed as NTM, among which 287 (7.7%, 95% CI, 6.9% to 8.6%) were from the southern region. In inland and coastal China, 87.7% (95% CI, 78.7% to 93.2%) and 50.0% (95% CI, 43.7% to 56.3%) of isolates, respectively, were slow growing mycobacteria (SGM), with the remaining rapid growing mycobacteria (RGM). A total of 29 species were detected, M. abscessus had higher clarithromycin-inducible resistance rates than M. massiliense (65.67% vs 2.22%). M. kansasii was present lower resistance rates in linezolid and moxifloxacin than Mycobacterium avium-intracellulare complex and other SGM.Conclusions: More NTM pulmonary disease was observed in southern and coastal China (p<0.01). SGM was widely distributed, and more RGM are present in southern and coastal China (p<0.01). The antimicrobial resistance spectrum of different NTM species was significant different, accurate species identification would be facilitated to NTM pulmonary disease treatment.

Abstract:China, with the third largest share of global tuberculosis cases, faces a substantial challenge in its healthcare system as a result of the high burden of multidrug-resistant and rifampicin-resistant tuberculosis (MDR/RR-TB). This study employs a genomic epidemiological approach to assess recent tuberculosis transmissions between individuals, identifying potential risk factors and discerning the role of transmitted resistant isolates in the emergence of drug-resistant tuberculosis in China. We conducted a population-based retrospective study on 5052 Mycobacterium tuberculosis (MTB) isolates from 70 surveillance sites using whole genome sequencing (WGS). Minimum spanning tree analysis identified resistance mutations, while epidemiological data analysis pinpointed transmission risk factors. Of the 5052 isolates, 23% (1160) formed 452 genomic clusters, with 85.6% (387) of the transmissions occurring within the same counties. Individuals with younger age, larger family size, new cases, smear positive, and MDR/RR were at higher odds for recent transmission, while higher education (university and above) and occupation as a non-physical workers emerged as protective factors. At least 61.4% (251/409) of MDR/RR-TB were likely a result of recent transmission of MDR/RR isolates, with previous treatment (crude OR = 2.77), smear-positive (cOR = 2.07) and larger family population (cOR = 1.13) established as risk factors. Our findings highlight that local transmission remains the predominant form of TB transmission in China. Correspondingly, drug-resistant tuberculosis is primarily driven by the transmission of resistant tuberculosis isolates. Targeted interventions for high-risk populations to interrupt transmission within the country will likely provide an opportunity to reduce the prevalence of both tuberculosis and drug-resistant tuberculosis.
