Abstract:Endometrial cancer (EC) diagnostics is evolving into a system in which molecular aspects are increasingly important. The traditional histological subtype-driven classification has shifted to a molecular-based classification that stratifies EC into DNA polymerase epsilon mutated (POLEmut), mismatch repair deficient (MMRd), and p53 abnormal (p53abn), and the remaining EC as no specific molecular profile (NSMP). The molecular EC classification has been implemented in the World Health Organization 2020 classification and the 2021 European treatment guidelines, as it serves as a better basis for patient management. As a result, the integration of the molecular class with histopathological variables has become a critical focus of recent EC research. Pathologists have observed and described several morphological characteristics in association with specific genomic alterations, but these appear insufficient to accurately classify patients according to molecular subgroups. This requires pathologists to rely on molecular ancillary tests in routine workup. In this new era, it has become increasingly challenging to assign clinically relevant weights to histological and molecular features on an individual patient basis. Deep learning (DL) technology opens new options for the integrative analysis of multi-modal image and molecular datasets with clinical outcomes. Proof-of-concept studies in other cancers showed promising accuracy in predicting molecular alterations from H&E-stained tumor slide images. This suggests that some morphological characteristics that are associated with molecular alterations could be identified in EC, too, expanding the current understanding of the molecular-driven EC classification. Here in this review, we report the morphological characteristics of the molecular EC classification currently identified in the literature. Given the new challenges in EC diagnostics, this review discusses, therefore, the potential supportive role that DL could have, by providing an outlook on all relevant studies using DL on histopathology images in various cancer types with a focus on EC. Finally, we touch upon how DL might shape the management of future EC patients.

Abstract:Pathological scarring resulting from traumas and wounds, such as hypertrophic scars and keloids, pose significant aesthetic, functional and psychological challenges. This study provides a comprehensive transcriptomic analysis of these conditions, aiming to illuminate underlying molecular mechanisms and potential therapeutic targets. We employed a co-expression and module analysis tool to identify significant gene clusters associated with distinct pathophysiological processes and mechanisms, notably lipid metabolism, sebum production, cellular energy metabolism and skin barrier function. This examination yielded critical insights into several skin conditions including folliculitis, skin fibrosis, fibrosarcoma and congenital ichthyosis. Particular attention was paid to Module Cluster (MCluster) 3, encompassing genes like BLK, TRPV1 and GABRD, all displaying high expression and potential implications in immune modulation. Preliminary immunohistochemistry validation supported these findings, showing elevated expression of these genes in non-fibrotic samples rich in immune activity. The complex interplay of different cell types in scar formation, such as fibroblasts, myofibroblasts, keratinocytes and mast cells, was also explored, revealing promising therapeutic strategies. This study underscores the promise of targeted gene therapy for pathological scars, paving the way for more personalised therapeutic approaches. The results necessitate further research to fully ascertain the roles of these identified genes and pathways in skin disease pathogenesis and potential therapeutics. Nonetheless, our work forms a strong foundation for a new era of personalised medicine for patients suffering from pathological scarring.

Abstract:Morphometric approaches facilitate the analysis of quantitative variation in form, typically becoming most useful for the study of organisms that have completed morphogenesis and are at differing stages of growth. Recent conceptual and technical refinements in the characterization and comparison of forms have joined methodological innovations in molecular biology, embryology, and phylogeny reconstruction to advance the study of the evolution of development. Among the phenomena that have recently been examined morphometrically are developmental integration and heterochrony, discoveries that in turn raise deeper questions about the connections among disciplines and among levels of description: the relationship between morphometric variables and characters, between phenomenology and process, and the interplay (and evolutionary relevance) of genes and phenotypes. Morphometrics can continue to play a vital role in evolutionary studies of development as its results generate questions both for its practitioners and for other sorts of biologists to explore.

Recovering Skin Reflectance And Geometry For Diagnosis Of Melanoma

Abstract:In order to achieve an early detection of skin cancers, various state-of-the-art imaging modalities have been investigated from optical, impedance, biomechanical and physiological perspectives to find out the potential biomarkers. However multilayered skin microstructure and a wide spectrum of dynamical chromophores embedded underneath skin make it very challenging to quantify this mostly accessible, but very complex and heterogeneous largest organ of the human body. Rather than concentrating on characterizing those internal features in a microscopic level, both lesion reflectance (colour) and 3D geometry have been suggested to recover through a relative easy and cost effective way towards an improved diagnosis of melanoma. The reflectance recovered can be used as a good replacement for conventional photograph for the measurement of the ABCD criteria, while the geometry of lesion surface provides extra dimension for characterizing the topography disruption of lesion region. As both the reflectance and geometry of skin surface generally reflect the growth of chromophore cells under the skin, any external abnormalities indicating the change of skin conditions must accompany with some irregular evolvement and change with these cells. For example, a blurred and asymmetrical border possibly reveals an abnormal growth of melanocytes in the horizontal phase; whiles 3D surface indentations and protrusions accompanying variegated pigmentation may indicate an aggressive penetration of melanin into the dermal layer. We compared new features derived from reflectance and geometrical information with those traditional ones and demonstrated their significance as additional clues for melanoma diagnosis.

Abstract:The foundations of geometric morphometrics were worked out about 30 years ago and have continually been refined and extended. What has remained as a central thrust and source of debate in the morphometrics community is the shared goal of meaningful biological inference through a tight connection between biological theory, measurement, multivariate biostatistics, and geometry. Here we review the building blocks of modern geometric morphometrics: the representation of organismal geometry by landmarks and semilandmarks, the computation of shape or form variables via superimposition, the visualization of statistical results as actual shapes or forms, the decomposition of shape variation into symmetric and asymmetric components and into different spatial scales, the interpretation of various geometries in shape or form space, and models of the association between shape or form and other variables, such as environmental, genetic, or behavioral data. We focus on recent developments and current methodological challenges, especially those arising from the increasing number of landmarks and semilandmarks, and emphasize the importance of thorough exploratory multivariate analyses rather than single scalar summary statistics. We outline promising directions for further research and for the evaluation of new developments, such as "landmark-free" approaches. To illustrate these methods, we analyze three-dimensional human face shape based on data from the Avon Longitudinal Study of Parents and Children (ALSPAC).

Abstract:▪ Abstract The performance of an organism is the crucial link between its phenotype and its ecological success. When does an organism's morphology affect its performance? Quantitative mechanistic analyses of how function depends on biological form have shown that the relationship between morphology and performance can be nonlinear, context-dependent, and sometimes surprising. In some cases, small changes in morphology or simple changes in size can lead to novel functions, while in other cases changes in form can occur without performance consequences. Furthermore, the effect of a specific change in morphology can depend on the size, shape, stiffness, or habitat of an organism. Likewise, a particular change in posture or behavior can produce opposite effects when performed by bodies with different morphologies. These mechanistic studies not only reveal potential misconceptions that can arise from the descriptive statistical analyses often used in ecological and evolutionary research, but they also show how new functions, and novel consequences of changes in morphology, can arise simply as the result of changes in size or habitat. Such organismal-level mechanistic research can be used in concert with other tools to gain insights about issues in ecology (e.g. foraging, competition, disturbance, keystone species, functional groups) and evolution (e.g. adaptation, interpretation of fossils, and origin of novelty).

Abstract:Big data and associated approaches to analyse it are on the rise, especially in healthcare settings. This growth is also seen with unique applications in the field of dermatology. While big data offer a plethora of opportunity for improving our current understanding of disease and ability to deliver care, as with any technology innovation, the potential pitfalls should be addressed. In this piece, we highlight opportunities and challenges associated with big data in dermatology. Opportunities include large and novel data sources that may offer a wealth of information, automated detection, classification and diagnostics and improved public health monitoring. Challenges include data quality, issues of interpretability and disparities within artificial intelligence (AI) training data sets. Clinicians and researchers in the field should be aware of these developments within the field of big data to understand how best it may be used toward improving patient care and health outcomes, particularly in the field of dermatology.

Abstract:Background: Clinically relevant regenerative medicine is still in its early stages of development. Difficulties in regenerating large-scale and complex structures, the lack of safety data, and the paucity of clinical trials have slowed the process of technological advance. Objective: To familiarize the clinician with techniques available in the laboratory and experimental approaches being tested clinically. In addition, a layout is discussed for how dermatologists can lead the way in bringing regenerative medicine to clinical reality. Methods: This article reviews the relevant literature on regenerative medicine for dermatological applications and discusses findings and techniques in a clinically relevant context. Results: Multiple cell-free and cell-based approaches for regenerating dermatologic tissues have been reported in the basic science and clinical literature. These are reviewed in the order of complexity. Conclusion: Incremental steps are needed to apply the principles of regenerative medicine to simple medical problems first. Such a stepwise approach would commence, for example, with creation of single-function tissues that could fill soft-tissue defects and proceed to the development of fully functional skin grafts. Likewise, cell-free approaches can build the foundation for the more technically demanding cell-based strategies that are likely necessary for achieving the ultimate goal of regenerative dermatology.

Abstract:Summary Dermatopathology has been an integral part of dermatology for more than 100 years and is essential for high quality patient care. In German‐speaking countries, dermatologists can acquire an additional qualification in dermatopathology after appropriate further training. For many years, dermatopathological diagnostics has advanced far beyond morphology. Immunohistochemistry and molecular pathology are nowadays an essential part and a prerequisite for the preservation of our discipline. Due to the increasing implementation of digitalization and artificial intelligence, dermatopathology is forward‐looking and offers an attractive working environment for young colleagues. Dermatopathology is also indispensable for research, and this fact should also be taken into account by creating academic positions and professorships in the future.
Studying morphological integration and modularity at multiple levels: concepts and analysis

Abstract:Although most studies on integration and modularity have focused on variation among individuals within populations or species, this is not the only level of variation for which integration and modularity exist. Multiple levels of biological variation originate from distinct sources: genetic variation, phenotypic plasticity resulting from environmental heterogeneity, fluctuating asymmetry from random developmental variation and, at the interpopulation or interspecific levels, evolutionary change. The processes that produce variation at all these levels can impart integration or modularity on the covariance structure among morphological traits. In turn, studies of the patterns of integration and modularity can inform about the underlying processes. In particular, the methods of geometric morphometrics offer many advantages for such studies because they can characterize the patterns of morphological variation in great detail and maintain the anatomical context of the structures under study. This paper reviews biological concepts and analytical methods for characterizing patterns of variation and for comparing across levels. Because research comparing patterns across level has only just begun, there are relatively few results, generalizations are difficult and many biological and statistical questions remain unanswered. Nevertheless, it is clear that research using this approach can take advantage of an abundance of new possibilities that are so far largely unexplored.

Abstract:The manuscript discusses the historical evolution of observing blood cell morphology under an optical microscope, from the earliest microscopes in the 17th century to the modern digital era, highlighting key advancements and contributions in the field. Blood has historically held symbolic importance in various cultures, with early medical observations dating back to Hippocrates and Galeno. The discovery of cells and subsequent advancements in microscopy by scientists like Hooke and van Leeuwenhoek paved the way for understanding blood cell morphology. Influential figures such as Hewson, Donné, and Ehrlich followed. Diagnostic cytology evolved from manual cell counting to the development of automated hematological systems. Automated complete blood counting came to support microscopic examination in diagnosing hematological disorders. Morphology is crucial in predicting disease outcomes and guiding treatment decisions, particularly hematological neoplasms. The introduction of flow cytometry and its integration with traditional morphological analysis and the new cytogenetic and molecular techniques revolutionized the classification and prognostication of hematologic disorders. Digital microscopy has emerged as a powerful tool in recent years, offering rapid acquisition and sharing of blood cell images. Integrating Artificial Intelligence with digital microscopy has further enhanced morphological analysis, improving diagnostic efficiency. We also discuss the prospects of AI in pre‐classifying blood cells in bone marrow aspirate samples, potentially revolutionizing diagnostic pathways for hematologic diseases. Overall, the manuscript provides a comprehensive overview of the historical development, clinical significance and technological advancements in observing blood cell morphology, underscoring its continued relevance in modern hematology practice.

Abstract:BACKGROUND: Results of molecular studies are redefining the diagnosis and management of a wide range of skin disorders. Dermatology training programs maintain a relative gap in relevant teaching.OBJECTIVE: To develop a curriculum in molecular diagnostics, genomics and personalized medicine for dermatology trainees at our institution. The aim is to provide trainees with a specialty-appropriate, working knowledge in clinical molecular dermatology.METHODS: The Departments of Dermatology and Pathology and Laboratory Medicine collaborated on the design and implementation of educational objectives and teaching modalities for the new curriculum.RESULTS: A multidisciplinary curriculum was developed. It comprises: (i) assigned reading from the medical literature and reference textbook; (ii) review of teaching sets; (iii) two 1 hour lectures; (iv) trainee presentations; (v) 1-week rotation in a clinical molecular pathology and cytogenetics laboratory; and (vi) assessments and feedback. Residents who participated in the curriculum to date have found the experience to be of value.CONCLUSIONS: Our curriculum provides a framework for other dermatology residency programs to develop their own specific approach to molecular diagnostics education. Such training will provide a foundation for lifelong learning as molecular testing evolves and becomes integral to the practice of dermatology.

Abstract:The skin is a physical and immunological barrier to the external environment. Its large surface area is colonized by diverse communities of microorganisms, including bacteria, viruses, fungi, and Demodex species mites. These microorganisms and their genetic material together create the skin microbiome. Physiologic and anatomic properties of skin sites create biogeographical habitats (dry, moist, and sebaceous) where distinct microbiota communities reside. Although, in general, the composition of these habitats is maintained from person to person, the skin microbiome of an individual also has unique microbial features. Dysbiosis occurs when the normal abundance, composition, or location of the microbiota is changed, most notably there is a decrease in flora diversity. Certain skin diseases, including atopic dermatitis, rosacea, and psoriasis are associated with cutaneous dysbiosis, and even disruption of the gut microbiota. Studies have shown that current treatments for these dermatologic conditions can alter/stabilize the skin microbiome, and there is emerging research detailing the impact of prebiotics, probiotics, and postbiotics on these conditions. Although clinical guidelines do not currently exist, clinical studies support the safety and possible benefits of using topical prebiotics and postbiotics and oral probiotics for a variety of skin conditions. Until such guidelines exist, utilizing carefully designed clinical studies to inform clinical practice is recommended.

Abstract:Our increased knowledge of normal skin physiology has ushered in a subtle revolution in cosmetic science. Originally designed as preparations to enhance personal appearance by direct application on to the skin, cosmetics have now taken on a new role in dermatology, through the support of the management of many skin disorders. This evolving role of cosmetics in skin care is primarily due to scientific and technological advancements that have changed our understanding of normal skin physiology and how cosmetics modify its appearance both physically and biologically. The vast array of techniques currently available to investigate skin responsivity to multiple stimuli has brought about a new era in cosmetic and dermocosmetic development based on a robust understanding of skin physiology and its varied responses to commonly encountered environmental insults. Most cosmetic research is undertaken on reconstructed skin models crucial in dermatological research, given the strict ban imposed by the European Union on animal testing. In addition, the design and conduct of trials evaluating cosmetics now follow rules comparable to those used in the development and evaluation of pharmaceutical products. Cosmetic research should now aim to ensure all trials adhere to strictly reproducible and scientifically sound methodologies. The objective of this review is to provide an overview of the multidisciplinary scientific approach used in formulating dermocosmetics, and to examine the major advances in dermocosmetic development and assessment, the safety and regulatory guidelines governing their production and the exciting future outlook for these dermocosmetic processes following good practice rules.

Abstract:Proficiency in dermatopathology is important for comprehending the mechanisms of skin diseases and determining efficacious clinical management. Dermatopathology learning is a core requirement for both dermatology and pathology residencies. Many traditional educational methods have been utilized over decades for teaching our residents. In the recent digitized era, technological advances are impacting the study and practice of medicine. Several innovative tools have become available that complement the conventional learning model. This contribution provides guidelines for establishing a robust dermatopathology rotation for residents and insights into the present and future advances in novel educational tools. Dermatopathology is an ever-growing body of knowledge. It takes years to accumulate practical knowledge of this discipline and requires a continuing effort to maintain and keep it updated. This journey of learning starts during the early years of postgraduate medical education. Attaining competency in dermatopathology is vital for both dermatology and pathology residents. The earlier the exposure they receive, the better the foundation and understanding of cutaneous disease processes they develop. Comprehensive training in the subspecialty of dermatopathology is critical for their medical education and the future practice.

Abstract:Morphological profiling is a valuable tool in phenotypic drug discovery. The advent of high-throughput automated imaging has enabled the capturing of a wide range of morphological features of cells or organisms in response to perturbations at the single-cell resolution. Concurrently, significant advances in machine learning and deep learning, especially in computer vision, have led to substantial improvements in analyzing large-scale high-content images at high-throughput. These efforts have facilitated understanding of compound mechanism-of-action (MOA), drug repurposing, characterization of cell morphodynamics under perturbation, and ultimately contributing to the development of novel therapeutics. In this review, we provide a comprehensive overview of the recent advances in the field of morphological profiling. We summarize the image profiling analysis workflow, survey a broad spectrum of analysis strategies encompassing feature engineering- and deep learning-based approaches, and introduce publicly available benchmark datasets. We place a particular emphasis on the application of deep learning in this pipeline, covering cell segmentation, image representation learning, and multimodal learning. Additionally, we illuminate the application of morphological profiling in phenotypic drug discovery and highlight potential challenges and opportunities in this field.

Dermatopathology education in the era of modern technology

Abstract:Continuing technological advances are inevitably impacting the study and practice of dermatopathology (DP). We are seeing the transition from glass slide microscopy to virtual microscopy, which is serving both as an accessible educational medium for medical students, residents and fellows in the form of online databases and atlases, as well as a research tool to better inform us regarding the development of visual diagnostic expertise. Expansion in mobile technology is simplifying slide image attainment and providing greater opportunities for phone- and tablet-based microscopy, including teledermatopathology instruction and consultation in resource-poor areas with lack of specialists. Easily accessible mobile and computer-based applications ("apps"), including myDermPath and Clearpath, are providing an interactive medium for DP instruction. The Internet and social networking sites are enabling rapid global communication of DP information and image-sharing, promoting collaborative diagnostic research and scholastic endeavors.

Abstract:Much of the striking diversity of life on Earth has arisen from variations in the way that the same molecules and networks operate during development to shape and pattern tissues and organs into different morphologies. However, we still understand very little about the potential for diversification exhibited by different, highly conserved mechanisms during evolution, or, conversely, the constraints that they place on evolution. With the aim of steering the field in new directions, we focus on morphogen-mediated patterning and growth as a case study to demonstrate how conserved developmental mechanisms can adapt during evolution to drive morphological diversification and optimise functionality, and to illustrate how evolution algorithms and computational tools can be used alongside experiments to provide insights into how these conserved mechanisms can evolve. We first introduce key conserved properties of morphogen-driven patterning mechanisms, before summarising comparative studies that exemplify how changes in the spatiotemporal expression and signalling levels of morphogens impact the diversification of organ size, shape and patterning in nature. Finally, we detail how theoretical frameworks can be used in conjunction with experiments to probe the role of morphogen-driven patterning mechanisms in evolution. We conclude that morphogen-mediated patterning is an excellent model system and offers a generally applicable framework to investigate the evolution of developmental mechanisms.

Abstract:Abstract Melanoma plasticity and heterogeneity contribute to therapeutic resistance and mortality, and the ability of melanoma cells to switch from melanocytic to mesenchymal phenotypes results in increased invasion and metastasis. Investigating the role of these phenotypic states has been challenging due to limited marker availability for each cell state. High-parameter molecular methods such as RNAseq have produced more descriptive gene signatures of phenotypic states, but these methods are cell-destructive, labor-intensive, expensive, and can take days to weeks to obtain a readout. Different morphologies have been observed for melanocytic and mesenchymal phenotypic states in culture, thus we hypothesized that morphology could provide an orthogonal method to define and study phenotypes. We utilized the Deepcell platform to develop a classifier to predict the phenotype of melanoma cells as melanocytic or mesenchymal based on morphology alone. This method is free of labels and utilizes viable cells, overcoming the technical and practical limitations of traditional, marker-based methods. We used 20 patient-derived cell lines with known melanocytic or mesenchymal transcription scores to develop the ‘Melanoma Phenotype Classifier’ to phenotype melanoma cells based on morphology alone. A classifier accuracy of >88% was achieved, and morphological analysis of the images revealed distinct morphotypes for each phenotype, highlighting distinct morphological differences. To further link phenotypic state with multi-dimensional morphological profiles, we performed genetic and chemical perturbations known to shift the phenotypic state. The Melanoma Phenotype Classifier successfully predicted shifts in phenotypes driven by these perturbations. These results further demonstrate how phenotype is linked to distinct morphological changes that are detectable by artificial intelligence. Lastly, we applied the Melanoma Phenotype Classifier to dissociated melanoma biopsies, which revealed phenotypic heterogeneity that was confirmed by single cell RNASeq. This work establishes a link between cellular morphology and melanoma phenotype and lays the groundwork for the use of morphology as a label-free method of phenotyping viable melanoma cells. Abstract:For more than a century, microscopic histology has been the cornerstone for cancer diagnosis, and breast carcinoma is no exception. In recent years, clinical biomarkers, gene expression profiles, and other molecular tests have shown increasing utility for identifying the key biological features that guide prognosis and treatment of breast cancer. Indeed, the most common histologic pattern-invasive ductal carcinoma of no special type-provides relatively little guidance to management beyond triggering grading, biomarker testing, and clinical staging. However, many less common histologic patterns can be recognized by trained pathologists, which in many cases can be linked to characteristic biomarker and gene expression patterns, underlying mutations, prognosis, and therapy. Herein we describe more than a dozen such histomorphologic subtypes (including lobular, metaplastic, salivary analog, and several good prognosis special types of breast cancer) in the context of their molecular and clinical features.
