Abstract:Objective: To determine the association of isolated non-visualization of fetal gallbladder(NVFGB) in the second-trimester ultrasound examination with perinatal outcomes. Methods: Clinical data of fetuses with isolated NVFGB diagnosed in our hospital were retrospectively analyzed from January 2013 to July 2016. Postnatal follow-up was also conducted,and outcomes of interest were emphatically observed. Results: During the study period, totally 37 fetuses were successfully followed up, nine of which had additional anomalies and(or) abnormal karyotypes. In three of the 28 isolated cases, the pregnancy was terminated. The remaining 25 fetuses were delivered, 24 were normal, while 1 had adverse outcomes: thyroid aplasia, no cases of biliary atresia were found. In 17 of the 25 isolated fetuses delivered, the gallbladder was eventually seen in follow-up sonograms later in gestation or on neonatal ultrasound. Conclusion: Ultrasonography is an effective method for prenatal diagnosis and follow-up observation of NVFGB. When it is isolated, the outcome is good. Evaluation of NVFGB should include repeated ultrasound scans, amniocentesis for karyotype and biochemical test, as well as parental genetic screening.

Abstract:A term male infant was born by spontaneous vaginal delivery to a 23 year old gravida 2 para 2 mother. His weight, length, and head circumference were 2720g (8%), 47cm (6%) and 31.5cm (1%) respectively; the remainder of his physical examination was unremarkable. The pregnancy was complicated by intra-uterine growth retardation, gestational diabetes controlled with glyburide, and gestational thrombocytopenia. The mother had a past medical history of obesity, BMI of 32 (>95%), HPV infection, and atypical squamous cells of the cervix. A fetal ultrasound at 38 weeks gestational age showed echogenic foci consistent with cholelithiasis (Figure 1); a previous US at 37 weeks gestational age was normal. On routine laboratory testing the mother’s blood type was O positive and the infant’s was A+ (positive). His transcutaneous bilirubin was 4.3 mg/dL at 36 hours of life. Both mother and son were discharged after two days without problem.

Abstract:Echogenic intracardiac focus (EIF) constitutes a finding in the ultrasound study that indicates an area which is echogenically bright in the fetal heart and is as bright as the bone that moves synchronically to the atrioventricular valves. Microcalcifications of the papillary muscles or chordae tendinae are being represented by this echogenicity and are mostly present in the left ventricle (90% of cases). EIF appears usually at the ultrasound that is realized in the mid-trimester in a percentage that reaches 3.5% in euploid fetuses and 15% to 30% in fetuses with trisomy 21. In the current paper, the rare and curious case of a 21-year-old primigravida woman is described, who presented for ultrasound scan at the 12th week of gestation. The scan revealed the presence of EIF, which is very rare, as it is well-known that it usually appears in the second trimester of pregnancy. Counseling and debriefing for dismissing parents' anxiety is necessary as well as further examinations, because EIF has low sensitivity. This specific case report could constitute a beginning in the research of whether investigating EIF in the first trimester of pregnancy is possible and which are the benefits of its detection for the mother, the fetus and the whole family, in clinical practice.
Outcomes of pregnancies diagnosed with absent or abnormal fetal gallbladder in a tertiary center

Abstract:Objective The objective of this study was to investigate the ultrasonographic characteristics and outcomes of fetuses with atypical and non‐visualized fetal gallbladder in our tertiary care hospital. Methods A retrospective analysis was conducted on cases in which the fetal gallbladder was not visualized or exhibited atypical characteristics at our institution over a four‐year period. The patients were divided into two groups: absent gallbladder and atypical gallbladder. The groups with isolated and additional anomalies were analyzed according to their ante‐ and postnatal characteristics. Results The study comprised 78 patients (37 absent, 41 atypical gallbladder). In the isolated fetal absence of gallbladder group, the gallbladder was visualized in three of 13 patients during antenatal ultrasonographic follow‐up and in half of the remaining 10 patients during postnatal follow‐up. In the postnatal period, five newborns with absent isolated gallbladder are being followed up with suspicion of biliary atresia and isolated gallbladder agenesis. In the absence of a gallbladder with an additional anomaly group, 58% of fetuses died during the intrauterine and neonatal period. Fetuses in the isolated atypical gallbladder group are being followed as healthy after birth. Pregnancies with atypical gallbladder appearance and additional anomalies resulted in 33% neonatal death, 12% intrauterine demise, and 25% termination of pregnancy. Conclusion In instances where the fetal gallbladder is not consistently discernible, it is imperative to exercise caution with regard to the possibility of biliary atresia. In the event that the fetal gallbladder exhibits unusual characteristics, a meticulous examination for the presence of additional anomalies is recommended.
