Abstract:Human error constitutes a significant cause of accidents across diverse industries, leading to adverse consequences and heightened disruptions in maintenance operations. Organizations can enhance their decision‐making process by quantifying human errors and identifying the underlying influencing factors, thereby mitigating their repercussions. Consequently, it becomes crucial to examine the value of human error probability (HEP) during these activities. The objective of this paper is to determine and simulate HEP in maintenance tasks at a cement factory, utilizing performance shaping factors (PSFs). The research employs the cross‐impact matrix multiplication applied to classification (MICMAC) analysis method to evaluate the dependencies, impacts, and relationships among the factors influencing human error. This approach classifies and assesses the dependencies and impacts of different factors on HEP, occupational accidents, and related costs. The study also underscores that PSFs can dynamically change under the influence of other variables, emphasizing the necessity to forecast the behavior of human error over time. Therefore, this paper utilizes the MICMAC method to analyze the interdependencies, relationships, and impact levels among different variables. These relationships are then utilized to optimize the implementation of the system dynamics (SD) method. An SD model is employed to forecast the system's behavior, and multiple scenarios are presented. By considering the HEP value, managers can adjust organizational conditions and personnel to ensure acceptability. The paper also presents various scenarios related to HEP to assist managers in making informed decisions.

Abstract:An analytical multiobjective maintenance planning model that maximizes reliability while minimizing cost and human error is proposed. In order to incorporate human error, the model minimizes the maximum human error over the planning horizon. Human Error Assessment and Reduction Technique (HEART) is used to quantify the human error. Maintenance activities include adjustment and replacement activities, in which each of them consumes a certain amount of human resource, spare parts, and budget and brings about a specified level of reliability and human error. Economic dependence is also considered, in which grouping maintenance activities reduces total cost. However, this may increase human error probability due to operator fatigue or time pressure. The main purpose is to investigate the relationship between human factors and maintenance activities to find the preferred maintenance plan. A multiple production line is considered as a case study. A sensitivity analysis is performed, and the effects of grouping and human factors on the preferred maintenance plan are discussed. It is shown how human proficiency may affect reliability and cost.

Abstract:Failures in mining machinery can abruptly halt mineral production and operations, emphasizing the indispensable role of humans in maintenance and repair operations. Addressing human errors is crucial for ensuring a safe and reliable system, particularly during maintenance activities where accidents frequently occur. This paper focuses on evaluating Human Reliability (HR) to enhance activity implementation effectiveness. Given the challenge of limited and uncertain data on human errors, this study aims to estimate the probability of human errors using Bayesian networks (BN) under uncertain parameters. Applying this approach to assess HR in the maintenance and repair operations of mining trucks at Golgohar Iron Ore Mine in Iran, the study identifies critical factors influencing error occurrence in a fuzzy environment. The results highlight key factors impacting human error and offer insights into estimating HR with minimal human intervention.

Abstract:Many occupational environments require labor intensive activities, which could result in fatigue and injuries and cause decreased work performance. Recently, the breakthroughs of Industry 4.0 have allowed for tracking worker conditions. Based on these, various analytical models have been performed with the aim of guiding work design in occupational environments, which contributes to ultimately achieving a safe and productive Operator 4.0. However, there is a lack of systematic analysis and implementation guidelines. In this paper, we review the literature on upper-limb fatigue modeling in occupational settings, since it is reported shoulder injuries are a pressing issue. From the reviewed papers, our main observations are that the majority of studies focus on identifying human fatigue/reliability indices, and the corresponding factors contributing to these indices, through statistical and biomechanical models. However, existing models lack the consideration of several important aspects of upper-limb fatigue analysis, such as the full use of wearable sensor data in a real-time manner, consideration of the heterogeneity of workers and measurement devices, and the difference between simulated and actual work environment. Moreover, the issue of fatigue development for the workers' upper extremities, which could significantly have an impact on workers' performance, has not yet been studied from a system reliability point of view to the best of the authors’ knowledge. Reliability engineering models that are widely used to model machines can play an important role in modeling humans in occupational settings. We discuss the limitations of the reviewed articles and provide insights for future research.

Abstract:Accurate detection of possible machine failure allows manufacturers to identify potential fault situations in processes to avoid downtimes caused by unexpected tool wear or unacceptable workpiece quality. This paper aims to report the study of more than 20 fault detection models using machine learning (ML), deep learning (DL), and deep hybrid learning (DHL). Predicting how the system could fail based on certain features or system settings (input variables) can help avoid future breakdowns and minimize downtime. The effectiveness of the proposed algorithms was experimented with a synthetic predictive maintenance dataset published by the School of Engineering of the University of Applied Sciences in Berlin, Germany. The fidelity of these algorithms was evaluated using performance measurement values such as accuracy, precision, recall, and the F-score. Final results demonstrated that deep forest and gradient boosting algorithms had shown very high levels of average accuracy (exceeded 90%). Additionally, the multinomial logistic regression and long short-term memory-based algorithms have shown satisfactory average accuracy (above 80%). Further analysis of models suggests that some models outperformed others. The research concluded that, through various ML, DL, and DHL algorithms, operational data analytics, and health monitoring system, engineers could optimize maintenance and reduce reliability risks.

Machine Learning-based Predictive Maintenance for Fault Detection in Rotating Machinery: A Case Study

Abstract:In the realm of industrial production, condition monitoring plays a pivotal role in ensuring the reliability and longevity of rotating machinery. Since most of the production facilities rely heavily on vibration analysis, it has become the cornerstone of condition monitoring practices. However, manual analysis of vibration signals is a time-consuming and expertise-intensive task, often requiring specialized domain knowledge. The current research addresses the aforementioned challenges by proposing a novel semi-automated diagnostics system. The approach leverages historical vibration data in the form of Fast Fourier Transform (FFT) spectrums. The system extracts energy features from the frequency domain by dividing the frequency range into a predefined number of bins and summing the energy values within each bin. Subsequently, each datapoint is labeled based on the corresponding machine condition, enabling the system to learn diagnostic patterns by employing machine learning models. This approach facilitates efficient and accurate diagnostics with minimal manual intervention. The resulting dataset effectively represents and provides an interpretable result. Support Vector Machines (SVM), and ensemble algorithms are utilized to diagnose the faults instantaneously and with minimal error rates. The proposed system is capable of providing early warnings and thus prevents further deterioration and unplanned downtimes. Experimental validation using real-world data demonstrates the system's efficacy, achieving an accuracy of over 90%.

Abstract:Abstract Reliability plays a significant role in the overall performance of manufacturing systems, and accurate reliability assessments can guarantee their functionalities and efficiencies. This paper develops a reliability model of a manufacturing system considering machine degradation, human errors, and their mutual facilitation effects (MFE): machine degradation may increase fatigue rate and weaken learning process, thereby facilitating the occurrence of human errors, while human errors may, in turn, facilitate the machine degradation process. Although MFE may reduce system reliability, current reliability models ignore this effect. In this paper, to describe the MFE, a stochastic human error model under the effects of learning–forgetting and fatigue–recovery is proposed, where the human error rate depends on the current machine degradation state; next, based on their facilitation effects on machine degradation, human errors are divided into safety, damage, and fatal zones; finally, we incorporate the zoned human error model into the machine degradation model and develop a Semi‐Markov reliability model. A Monte Carlo method is implemented to calculate system reliability. The turret of a lathe operated by a worker is utilized to illustrate the effectiveness of the reliability model.

Abstract:Preventive maintenance of electrical equipment is of great significance to maintain power system stable, prolong the service life of equipment and reduce the system power loss. The effect of maintenance is heavily affected by human factors, and human errors might have a negative influence on maintenance results. In view to this situation, this paper analyzes human factors in maintenance firstly, and then establishes a periodic maintenance model considering the impact of human errors. Numerical example analysis illustrates that the optimal policy which maximizes the availability is affected by human errors, percentage of maintenance failure and repair time. At last, this paper introduces a practical method to quantify human error probability, and several measures are suggested to manage human errors in maintenance.

Abstract:If machine failures can be detected preemptively, then maintenance and repairs can be performed more efficiently, reducing production costs. Many machine learning techniques for performing early failure detection using vibration data have been proposed; however, these methods are often power and data-hungry, susceptible to noise, and require large amounts of data preprocessing. Also, training is usually only performed once before inference, so they do not learn and adapt as the machine ages. In this article, we propose a method of performing online, real-time anomaly detection for predictive maintenance using hierarchical temporal memory (HTM). Inspired by the human neocortex, HTMs learn and adapt continuously and are robust to noise. Using the Numenta Anomaly Benchmark, we empirically demonstrate that our approach outperforms state-of-the-art algorithms at preemptively detecting real-world cases of bearing failures and simulated 3-D printer failures. Our approach achieves an average score of 64.71, surpassing state-of-the-art deep-learning (49.38) and statistical (61.06) methods.

Abstract:Machine workshops generate high scrap rates, causing non-compliance with timely delivery and high production costs. Due to their natural characteristics of a low volume, high-mix production batches, and serial and parallel configurations, generally the causes of their failure are not well documented. Thus, to reduce the scrap rate, and evaluate and improve their reliability, their system characteristics must be considered. Based on them, our proposed methodology allows us to evaluate the system, subsystem, and component–subsystem relationship by using either the Weibull and/or the exponential distribution. The strategy to improve the system performance includes reliability tools, expert interviews, cluster analysis, and root-cause analysis. In the application case, the failure sources were found to be mechanical and human errors. The component maintenance/setup, institutional conditions/attitude, and subsystem process/operation were the machine factors that presented the lowest reliability indices. The improved activities were monitored based on the Weibull β and η parameters that affect the system reliability. Finally, by using a life–effort analysis, and the method of comparative analysis of two sequential periods, we identified the causes that generated a change in the Weibull parameters. The contribution of this methodology lies in the grouping of the tools in the proposed application context.

Abstract:This study examines reliability of equipment (RoE) through three approaches: equipment design reliability, human reliability (HR), and maintenance‐based reliability. HR plays a key role in minimizing human error (HE) and subsequently enhancing RoE. Given that equipment reliability is influenced by numerous factors, these technologies come with various constraints, multiple outputs, and inputs. Compared to mathematical programming and analytical models, simulation methods in the field of reliability are relatively limited. However, system dynamics (SD) modeling is well‐suited to capture dynamics and complexity of systems, making it a valuable tool for long‐term strategic decision‐making. In this study, a combination of SD and regression approaches has been employed to explore the relationship between reliability and key variables such as HE and profit. Initially, the variables influencing reliability are identified, and then SD is utilized to understand the processes and interactions among these variables. Furthermore, linear regression is employed to establish the relationship between affective variables, reliability, and HE. To validate the results obtained from the proposed method, a sensitivity analysis is conducted. The results demonstrate the effectiveness of the proposed model. Simulation results indicate that implementing policies such as employee training and preventive maintenance significantly enhances RoE, leading to increased sales and profits for the organization. Therefore, managers should prioritize these variables and allocate adequate attention and resources to them.

Abstract:Human errors significantly contribute to aviation accidents during aircraft maintenance. Therefore, managing human errors becomes a critical aspect of aviation maintenance. The existing literature has extensively analysed human errors. However, there is a gap in accurately identifying and prioritising critical human errors and effectively managing them to reduce incidents and accidents. This research work proposes a novel fuzzy approach for human error analysis using a hybrid FMEA with Fuzzy AHP-TOPSIS. We identified inadequate maintenance processes, attention/memory, inadequate documentation, inadequate supervision, judgement/decision-making, and knowledge/rule base as some of the critical human errors in aircraft maintenance. Based on these findings, we recommended practically implementable solutions at the organisational, workspace, and individual levels to mitigate human errors in aircraft maintenance.

Abstract:Human factors have important effects on equipment reliability evaluation and maintenance quality. However, they are usually neglected in conventional researches. Hence, this paper proposes an improved approach to evaluate equipment reliability based on proportional hazard model (PHM), considering the running age, health condition and human factors. State transition is supposed to happen at any time instead of just at inspection point, which is more realistic. A generalized Pareto distribution (GPD) is adapted to describe the influence of human errors on equipment reliability evaluation. And on this base, a modified periodic maintenance model is established considering the maintenance might be imperfect resulting from human errors. HCR, a widely used human reliability analysis method, is introduced to express the relation between human reliability and maintenance time. Results of numerical case study demonstrate the influence of human reliability, maintenance time, failure repair time and percentages of failure after maintenance, which is characterized by the variation of maintenance availability.

Abstract:This paper discusses integrated model of batch production scheduling and machine maintenance scheduling. Batch production scheduling uses minimize total actual flow time criteria and machine maintenance scheduling uses the probability of machine failure based on Weibull distribution. The model assumed no nonconforming parts in a planning horizon. The model shows an increase in the number of the batch (length of production run) up to a certain limit will minimize the total actual flow time. Meanwhile, an increase in the length of production run will implicate an increase in the number of PM. An example was given to show how the model and algorithm work.

Abstract:Human factors play a significant role in increasing the operational safety of maritime transport and offshore facilities. A significant number of human errors occur during the maintenance phase. However, the qualification of human error interpretation before undertaking any quantification in the maintenance procedure should be given more attention. It is necessary to find a reasonable qualitative non-linear based method with good interpretation of WHY and HOW accidents occur. This provides good insight, sources of risk and the possibilities for minimizing the potential risk. As maritime operations move into Arctic and Antarctic environments, this will become even more crucial. Decision makers must therefore be able to recognize how cold weather affects human performance and work out how availability, survivability or maintenance of a system goes wrong. This will help assessors to review the details of the process and ask relevant questions rather than blindly finding answers. This paper presents a new reciprocal interaction of qualitative risk-based methodology for human error estimation by applying “Functional Resonance Analysis Method” (FRAM). This methodology has the potential to be considered the first step of any future quantitative assessment for human error estimations. The present study is an imperative milestone for coupling between nonlinear qualitative and quantitative based methods in risk assessment to systematically identify human errors. The developed methodology has been applied to a case study for the maintenance of a component in a ship sailing in a harsh environment.

Optimized Task Assignment and Predictive Maintenance for Industrial Machines using Markov Decision Process

Abstract:This paper considers a distributed decision-making approach for manufacturing task assignment and condition-based machine health maintenance. Our approach considers information sharing between the task assignment and health management decision-making agents. We propose the design of the decision-making agents based on Markov decision processes. The key advantage of using a Markov decision process-based approach is the incorporation of uncertainty involved in the decision-making process. The paper provides detailed mathematical models along with the associated practical execution strategy. In order to demonstrate the effectiveness and practical applicability of our proposed approach, we have included a detailed numerical case study that is based on open source milling machine tool degradation data. Our case study indicates that the proposed approach offers flexibility in terms of the selection of cost parameters and it allows for offline computation and analysis of the decision-making policy. These features create and opportunity for the future work on learning of the cost parameters associated with our proposed model using artificial intelligence.

Maintenance Strategies for Industrial Multi-Stage Machines: The Study of a Thermoforming Machine

Abstract:The study of reliability, availability and control of industrial manufacturing machines is a constant challenge in the industrial environment. This paper compares the results offered by several maintenance strategies for multi-stage industrial manufacturing machines by analysing a real case of a multi-stage thermoforming machine. Specifically, two strategies based on preventive maintenance, Preventive Programming Maintenance (PPM) and Improve Preventive Programming Maintenance (IPPM) are compared with two new strategies based on predictive maintenance, namely Algorithm Life Optimisation Programming (ALOP) and Digital Behaviour Twin (DBT). The condition of machine components can be assessed with the latter two proposals (ALOP and DBT) using sensors and algorithms, thus providing a warning value for early decision-making before unexpected faults occur. The study shows that the ALOP and DBT models detect unexpected failures early enough, while the PPM and IPPM strategies warn of scheduled component replacement at the end of their life cycle. The ALOP and DBT strategies algorithms can also be valid for managing the maintenance of other multi-stage industrial manufacturing machines. The authors consider that the combination of preventive and predictive maintenance strategies may be an ideal approach because operating conditions affect the mechanical, electrical, electronic and pneumatic components of multi-stage industrial manufacturing machines differently.

Abstract:Systems and machines undergo various failure modes that result in machine health degradation, so maintenance actions are required to restore them back to a state where they can perform their expected functions. Since maintenance tasks are inevitable, maintenance planning is essential to ensure the smooth operations of the production system and other industries at large. Maintenance planning is a decision-making problem that aims at developing optimum maintenance policies and plans that help reduces maintenance costs, extend asset life, maximize their availability, and ultimately ensure workplace safety. Reinforcement learning is a data-driven decision-making algorithm that has been increasingly applied to develop dynamic maintenance plans while leveraging the continuous information from condition monitoring of the system and machine states. By leveraging the condition monitoring data of systems and machines with reinforcement learning, smart maintenance planners can be developed, which is a precursor to achieving a smart factory. This paper presents a literature review on the applications of reinforcement and deep reinforcement learning for maintenance planning and optimization problems. To capture the common ideas without losing touch with the uniqueness of each publication, taxonomies used to categorize the systems were developed, and reviewed publications were highlighted, classified, and summarized based on these taxonomies. Adopted methodologies, findings, and well-defined interpretations of the reviewed studies were summarized in graphical and tabular representations to maximize the utility of the work for both researchers and practitioners. This work also highlights the research gaps, key insights from the literature, and areas for future work.

Abstract:Introduction: In many workplaces today, the incidence of human error can lead to catastrophic accidents in which human error is the main cause of accidents. Due to the vital role of the control room in guiding and controlling various sites of the pipe industry, especially the outer coating sector, the incidence of any error can lead to human accidents, damage to machinery, interruption in production. This study aimed to identify and evaluate human error by Human Error Calculator (HEC) method in the epoxy control room of a pipe mill company. Materials and Methods: In the present descriptive cross-sectional study, the HEC method was used to identify and evaluate human errors. The HEC technique is provided by Risk Map Company, in which the probability of human error is based on five factors affecting the occurrence of human error, including a degree of urgency, complexity, importance, degree of individual skill, and task repetition, using a disk-shaped tool called Risk Disk is determined through direct observation, available instructions and interview with the head of the mentioned unit. Results: According to the results of this study, out of 11 identified tasks, five job tasks with a risk number of 70% have a high probability of human error, four job tasks with a risk number of 50%, and one job task with a number There is a 40% risk of moderate human error, And a job task with a 20% risk number has an increased chance of human error. Conclusion: The results of the present study showed that the HEC method is easy to use and is a simple and useful tool for professionals to calculate the probability of human error. In addition, HEC is a practical, effective and beneficial method for managers to reduce human error.

Abstract:Theoretically, prevention through design (PtD) is the most effective approach for eliminating human error in the field of construction. However, there is a lack of rigorous empirical analysis regarding the focal point of PtD. Hence, this research aims to develop a system dynamics model that depicts a mechanism for identifying the focal point of PtD and relevant design factors with respect to human error. Elevator installation serves as the scope of the model, and three sub-models of “cognition-behavior,” “environment-cognition,” and “design-environment” are developed. Their quantitative relationships are analyzed based on empirical data prior to the development of the system dynamics model. Subsequently, safety performance is simulated based on an eight-hour work day, and sensitivity analyses are conducted to determine the impact of each design factor on human error. This study provides insights into PtD using various design factors that can be generalized and used to develop safer designs under complex working environments.
