Abstract:The uniportal access video-assisted thoracic surgery has now become an increasingly popular approach to manage thoracic surgical diseases. Among all lobes, left upper lobectomy is considered a difficult procedure via a single incision. Although there are many reports on this technique, standard procedure has not come to an agreement yet. A 50-year-old woman was admitted to our hospital for finding of a left upper lobe lesion for 2 months. No abnormal signs were observed in physical and laboratorial tests. A ground-glass opacity nodule was detected on the left upper lobe on computed tomography. No enlarged lymph nodes or distant metastasis was detected. Early-stage lung cancer was initially diagnosed in this patient. Contrast with traditional uniportal left upper lobectomy procedures, our procedure has a reversed order of dissection. The arterial branches were divided first; then, the upper bronchus, the apicoposterior artery, and upper vein was stapled together at last. On our observation and experience, this procedure can simplify the uniportal left upper lobectomy procedure and reduce operation time potentially. Additionally, we would like to introduce our improved closure technique for uniportal video-assisted thoracic surgery: double-embedding stitching method.
Transaxillary Uniportal Video Assisted Thoracoscopic Surgery for Right Upper Lobectomy

Abstract:Uniportal video-assisted thoracic surgery (VATS) was growing popular since its first introduction. Based on the conventional uniportal VATS, we modified the technique and introduced transaxillary uniportal VATS lobectomy in this case report. In March 2017, transaxillary uniportal VATS was firstly attempted on a patient suffering from right upper lobe lesion at the Department of Thoracic Surgery, Zhongshan Hospital, Fudan University. A 4-cm single incision was made at the fossa axillaris paralleled to the skin folds, to which a soft wound protector was applied to reach the third intercostal space along the anterior axillary line. The right upper lobectomy was performed through the transaxillary incision. The surgery was accomplished without conversion to thoracotomy or application of extra incision. The truncus anterior artery, the upper lobe bronchus and upper pulmonary vein was mobilized and dissected in order. The target lobe was removed through the fossa axillaris incision. The operation duration was 110 minutes with limited blood loss. The patient was discharged 3 days post-operatively. Transaxillary uniportal VATS lobectomy is safe and feasible, and the procedure showed cosmetic advantages. Further studies based on larger population are required to determine these findings.

Abstract:<p>The study aimed to describe a novel port design and simple surgical strategy for uniportal thoracoscopic right middle lobectomy (RML). In this approach, a 3-4 cm incision was created at sixth intercostal space posterior to the scapular line. The surgery was performed by serial division of the anterior oblique fissure, vein, bronchus, artery, and horizontal fissure. Based on our preliminary experience, this approach could provide an appropriate direction and angle for dissection and stapling, solving the challenge of conventional uniportal RML lobectomy.</p>

Abstract:Background: Uniportal video-assisted thoracoscopic surgery (VATS) is an emerging technique in the field of thoracic surgery. In this article, we aimed to describe uniportal VATS lobectomy that was conventionally performed in an Eastern center, which might help facilitate a good beginning in practice this technique. Methods: The indications of uniportal VATS lobectomy are merely equal, if not the same, to multiportal VATS lobectomy. All patients would receive a combination of epidural and general anesthesia, and were provided with patient-controlled analgesia (PCA) postoperatively. Generally, the patient was kept in a folding knife gesture (with the cranial side slightly raised up and caudal side pushed down) in lateral decubitus position. Results: In uniportal VATS lobectomy, the geographical view differed from multiportal VATS, while the thoracic cavity required carefully exploration to identify any unexpected invasion or metastasis. Since the incision was located at the anterior axillary line close to the anterior part of the hilar, the posterior part of the hilar, and diaphragm were far from the surgical incision. Conclusions: Uniportal VATS, is not the happy ending in thoracic surgery. Beside its rapid progression, there remained a lot of questions to answer. Its uncovered benefits and drawbacks over traditional VATS, its possible improvements, and less invasive peri-operative managements could be the future topics. A comparison between uniportal versus multiportal VATS using high-level methodology is therefore required. With the development of surgical minds and instruments, uniportal VATS would have done more to the patients.
Uniportal Video-Assisted Thoracoscopic Right Upper Lobectomy with Systematic Lymphadenectomy

Abstract:Over the past decade, uniportal video-assisted thoracic surgery (VATS) has been reported to be a promising, less invasive alternative with potentially better cosmesis and less postoperative pain and paraesthesia. Although uniportal VATS has now evolved into a sophisticated technique capable of performing some of the most complex thoracic procedures, this approach to lobectomy is not standardized, and the surgical procedure still varies between surgeons. Here, we describe our uniportal VATS procedure during right upper lobectomy in a patient with a nodule in the right upper lobe. Subsequent mediastinal lymphadenectomy was performed to remove lymph nodes from the 2nd, 3rd, 4th, 7th, 8th, and 9th groups. Although there are some details that are different compared to the conventional VATS approach, as experience with uniportal VATS has grown, this approach is a viable alternative approach for lobectomy in selected patients.

Abstract:BACKGROUNDUniportal video-assisted thoracoscopic surgery (VATS) lobectomy is an emerging technique for the surgical resection of non-small cell lung cancer (NSCLC). Besides its wide debates on safety and efficacy throughout the world, there were few report on uniportal VATS from the Eastern countries. In this article, we summarized our primary experience on uniportal VATS lobectomy in an Eastern center.METHODSFrom October 2013 till February 2014, 54 consecutive uniportal VATS lobectomy were performed in the Department of Thoracic Surgery, Zhongshan Hospital of Fudan University. Patients' clinical features and operative details were recorded. Post-operatively, the morbidity and mortality were recorded to analyze the safety and efficacy of uniportal VATS lobectomy for NSCLCs.RESULTSAmong the 54 planned uniportal VATS lobectomy, there was one conversion to mini-thoracotomy due to lymph node sticking. Extra ports were required in two patients. The uniportal VATS lobectomy was achieved in 51 out of 54 patients (94.4%). The average operation duration was 122.2±37.5 min (90-160 min). The average volume of estimated blood loss during the operation was 88.8±47.1 mL (50-200 mL). The mean chest tube duration and hospital stay were 3.2±1.9 days and 4.6±2.0 days, respectively. There was no postoperative mortality in this study. Two patients suffered from prolonged air leakage (5 and 7 days), and one atrial fibrillation was observed in this cohort.CONCLUSIONSBased on our primary experience, uniportal VATS lobectomy is a safe and effective procedure for the surgical resection of NSCLCs. The surgical refinements and instrumental improvements would facilitate the technique. Further studies based on larger population are required to determine its benefits towards patients with NSCLCs.

Abstract:In recent years improvements in thoracoscopic instrumentation and technology including high definition cameras and miniaturized energy devices have led to uniportal video-assisted thoracoscopic (VATS) lobectomy becoming more common (1). However, uniportal VATS sleeve lobectomy (SL) is more difficult, and the competence threshold for performing such surgery is high, so only a small number of expert VATS surgeons have mastered it (2). Furthermore, the left upper SL is usually more complicated due to the presence of the aortic arch, the bigger size of the pulmonary artery and absence of the intermediate bronchus (3). In this article, we describe improvements to bronchial exposure and simplification of the anastomosis technique with existing equipment which reduces the complexity of uniportal VATS left upper SL.

Abstract:In the past 20 years, video-assisted thoracic surgery has made a great progress, from 4-ports to 2-ports, and eventually to this revolutionary approach-uniportal video-assisted thoracic surgery (VATS). It can share the same instruments, the same surgical principles, the same strategies of trouble-shooting and the same postoperative short-term outcomes with conventional VATS via the improvement of instruments and surgical skills. And it has already been safe to adopt uniportal VATS in complicated pulmonary resections. In this study, we shared five video clips about uniportal VATS for complicated pulmonary resections: sleeve resection, segmentectomy, pneumonectomy and angioplasty. We hope these video clips can be instrumental for young surgeons.

Abstract:A 62-year-old female was admitted to our hospital after computed tomography (CT) revealed a 2.5 cm × 2.1 cm mass in the left upper lobe. PET/CT scan diagnosed as malignant lesion with no signs of metastasis. Under general anesthesia, the patient was placed in right lateral decubitus position. A 4cm incision was made in the 4th intercostal space with plastic protector. The camera was placed in the upper part of the incision and the instruments were inserted below the camera. Left upper lobectomy along with systematic lymphadenectomy was performed. Total surgical time was 135 min and estimated blood loss was 70 mL. The chest tube was removed on the 2nd postoperative day and the patient was discharged on the 3rd postoperative day with no complication. This uniportal VATS single-direction lobectomy for the left upper lobe is feasible and amplifies the concept of thoracotomy-like minimally invasive surgery.
Clinical Analysis of Single Utility Port Video-Assisted Thoracic Surgery in Surgical Treatment of Non-Small Cell Lung Cancer

Abstract:Objective To explore the feasibility and clinical application value of single utility port video-assisted thoracic surgery(VATS) pulmonary lobectomy and systematic mediastinal lymphadenectomy in surgical treatment of non-small cell lung cancer(NSCLC). Methods We reviewed the clinical data of 42patients with NSCLC received single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy from March 2011 to March 2013. The utility port for observation was about 1.5 cm long and located at the 7th and 8th rib in the middle axillary line. The port for operation was about 4.0-5.0 cm long and located at the 3rd and 4th rib in the outer edge of pectoralis major. Results All of the 42 patients were successfully subjected to single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy, none of them needed converse to transit-assisted incision or thoracotomy. Operative duration was 90-200 min; intraoperrative blood loss was 50-400 ml;lymph node dissection number was 9-16. After the surgery, the patients received chest drainage for 5-8 days. They were discharged from hospital in 6-10 days after the surgery without severe complications. Conclusions Based on the conventional three utility ports VATS, the single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy caused less injury in surgical treatment of non-small cell lung cancer(NSCLC). It has certain clinical advantages and was a minimally invasive surgical treatment for selected patients with NSCLC.

Abstract:Single-port video-assisted thoracoscopic surgery (VATS) has recently been proposed as an innovative minimally invasive alternative to the standard three-port VATS for lobectomies, most of which are performed using a conventional rigid thoracoscope. Here, we report a single-port VATS approach for right upper lobectomy and systematic lymph node dissection using a flexible endoscope. A 61-year-old male smoker presented with a pulmonary nodule. A single-port VATS procedure was performed through a 4-cm intercostal incision using a flexible laparo-thoraco videoscope. Right upper lobectomy and systematic lymph node dissection were performed. The total operating time was 106 in. The procedure was successful and the recovery uneventful. The patient's chest tube was removed on the third day, and he was discharged home on the fourth. The use of a flexible videoscope facilitated the single-port VATS procedure by avoiding interference between the videoscope and other operating instruments and providing ample space for the surgeon.
The Application of Uniportal Video-Assisted Thoracoscopic Anatomical Segmentectomy for Lung Resection: A Retrospective Clinical Study

Abstract:Background: Video-assisted thoracoscopic surgery (VATS) is a new area of exploration and evolution in thoracic minimal invasive surgery. The uniportal VATS approach has become popular during lung resection for small nodules and ground glass lesions. Our objective is to investigate the efficacy, availability and safety of uniportal VATS anatomical segmentectomy compared with conventional VATS in patients for lung resection. Methods: Surgical patients of perioperative period who admitted and underwent uniportal, two-port and conventional three-port VATS segmentectomy were analyzed and compared retrospectively during the year 2017 to 2018. Results: During the research period, of 111 patients who had VATS anatomical segmentectomy, 38 underwent uniportal, 43 underwent two-port, and 30 underwent three-port VATS. Four patients underwent conversion to thoracotomy. There were no postoperative mortalities, and there were no significant differences among the three groups in surgical outcomes, including operative time, blood loss, conversions to thoracotomy, drainage time and volume, lymph node dissection, postoperative complications and hospital stay. The pain scores of visual analog scale (VAS) significantly decreased in uniportal group when operation is finished (P < 0.05). Conclusion: This study demonstrates that uniportal VATS anatomical segmentectomy is a quite safe surgical technology, as well as feasible, which can cause reduced postoperative pain and less surgical trauma compared to conventional VATS. More experiences and observations of large samples are on the way.
Non-grasping en bloc mediastinal lymph nodes dissection in uniportal video-assisted thoracic surgery for lung cancer

Abstract:Objective To analyze the safety, feasibility and operative technique details of non-grasping en bloc mediastinal lymph nodes dissection technique in uniportal video-assisted thoracic surgery(VATS) for lung cancer.Methods From April, 2014 to March, 2015,46 patients with lung cancer received non-grasping en bloc mediastinal lymph nodes dissection after uniportal VATS lobectomy.Clinical data of the cases were analyzed retrospectively.There were 19 males and 27 females.The age was(57.2 ± 9.0) (38-73) years.The first 6 cases were performed in the lateral decubitus position while the later 40 cases were all performed in the semiprone position.Results All cases accepted uniportal VATS non-grasping en bloc mediastinal lymph nodes dissection successfully.Arm fatigue of surgeon and assistant was obviously relieved when the patient was placed in the semiprone position.The thoracic drainage time was(3.2 ± 2.1) (1-12)days and the postoperative length of hospital-stay was(6.0 ± 4.5) (2-27) days.The number of dissected mediastinal lymph nodes stations was (4.3 ± 0.8) (3-6)and the number of dissected mediastinal lymph nodes was (11.8 ± 4.9) (4-30).There were 42 cases with stage No , lease wit stage N1, and 3 cases with stage N2 in pathological examination.Five patients developed minor postoperative complications.No perioperative death occurred.Conclusion Uniportal VATS non-grasping en bloc mediastinal lymph nodes dissection for lung cancer was safe and feasible, which could decrease the interference of the instruments and help to keep the surgical field clear.Non-grasping en bloc mediastinal lymph nodes dissection would be performed more smoothly in the semiprone position with less damage to lung and better ergonomics.

Abstract:Background: Conventional video-assisted thoracoscopic surgery (VATS) lobectomy for locally advanced non-small cell lung cancer (NSCLC) is a feasible and safe surgery in high-volume centers with significant VATS experience. Uniportal VATS lobectomy has been recently been reported to be a promising, less invasive approach. The purpose of this study is to explore the safety and feasibility of uniportal video-assisted thoracoscopic surgery (U-VATS) for the treatment of patients with locally advanced NSCLC. Methods: From January 2013 to September 2015, a total of 132 patients with locally advanced NSCLC underwent U-VATS or open thoracotomy major pulmonary resections and standard mediastinal lymph node dissection. Patients were divided into two groups: (I) locally advanced NSCLC underwent U-VATS (U-VATS); (II) locally advanced NSCLC underwent open thoracotomy (open). A descriptive and retrospective study was performed, including the operative time, operative blood loss, postoperative chest tube duration, postoperative hospital stay, lymph node dissection, postoperative complications and postoperative recovery. Results: A total of 132 patients with locally advanced NSCLC were included in this study: 64 (U-VATS) vs. 68 (open) patients. The patient demographic data was similar in both groups. Median operative time (157.0 vs. 160.6) and median number of lymph nodes (35.5 vs. 32.5) were similar in both groups. Chest tube duration and hospital of stay were statistically shorter in U-VATS group while rate of complications were higher in open thoracotomy group. One patient died on the 55th postoperative day because of tumor metastasis and bronchopleural fistula. A higher percentage of patients who underwent UVATS resections were able to receive adjuvant therapy timely compared to the open group. Conclusions: Uniportal VATS major pulmonary resections and mediastinal lymph node dissection is a safe and feasible procedure for the treatment of locally advanced NSCLC. Particularly it is suitable for the frail patients with locally advanced NSCLC who require comprehensive treatment.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>To investigate the uniportal video‐assisted thoracoscopic surgery (VATS) technique and safety of non‐small cell lung cancer (NSCLC) patients treated with uniportal and three‐port VATS.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We retrospectively evaluated 146 consecutive patients with NSCLC who underwent VATS lobectomy between January 2018 and May 2018. The general clinical date, perioperative data and life quality were individually compared and analyzed between the two groups.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Intraoperative blood loss was significantly lower in the uniportal than in the three‐port group (<i>p</i> = 0.035), and significantly shorter chest tube drainage and postoperative hospital stay durations were found in the uniportal than in the three‐port group (<i>p</i> = 0.022 and <i>p</i> = 0.008). The postoperative 24 and 72 h numerical rating scale (NRS) scores were significantly lower in the uniportal group than in the three‐port group (<i>p</i> < 0.001 and <i>p</i> < 0.001). There were no significant differences between the two groups in the number or stations of total lymph node dissected (<i>p</i> = 0.222 and <i>p</i> = 0.159). There were no significant differences between the two groups in the postoperative total or respiratory complications (<i>p</i> = 0.917 and <i>p</i> = 0.930). </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Uniportal VATS is a safe and effective alternative for patients with NSCLC. It is a preferable option for appropriate cases as it is conducive to patients' postoperative recovery and quality of life.</p></section>

Uniportal video-assisted thoracoscopic surgery for pulmonary nodular lesions

Abstract:Objective To assess the safety and efficacy of uniportal video- assisted thoracoscopic surgery (UVATS) for pulmonary nodular lesions. Methods Seventy six patients with pulmonary nodular lesions received UVATS and dissection of systematic lymph nodes based on the results of frozen pathology From July 2010 to May 2015. Results Among 76 patients, 58 underwent lung segmentectomy, 18 underwent lung lobeectomy, 26 received dissection of systematic lymph nodes. The operation time was (58.7±21.3) minutes, the intraoperative blood loss was (35.2±14.5) ml, the volume of drainage was 260± 150)ml, the VAS score was (2.3±0.9), and the postoperative hospital stay was (6.8±2.3) days. The VAS scores were significantly lower than those in the control group (three ports VATS) (P<0.01), while there were no differences in the volume of drainage, the operation time and the blood loss between two groups (both P>0.05). Al the patients achieved postoperative recovery and no severe complication occurred. Al the patients in this study were followed up regularly for an average of 33.1 months and there was no recurrence. Conclusion The UVATS is a safe and feasible method in the treatment of pulmonary nodular lesions and dissection of systematic lymph nodes, and with the advantages of lower morbidity and rapid postoperative recovery.

Abstract:Uniportal video-assisted thoracoscopic surgery (VATS) has now evolved into a sophisticated technique that can be used in some of the most complex thoracic procedures; however, this approach to segmentectomy is not standardized, and the surgical procedure varies between surgeons. Methods: Medical records of 128 patients with NSCLC who underwent surgical treatment in the First School of Clinical Medicine, Southern Medical University from August 2020 to February 2022 were retrospectively analyzed. There were 60 patients who underwent uniportal VATS (UVATS group) and 68 patients underwent multiportal VATS (MVATS group). The relevant indexes, complications, postoperative pain levels and quality of life, recurrence, metastases and survival between the two groups were compared. Results: UVATS was associated with longer operation time and higher intraoperative blood loss compared to MVATS (P<0.05). The postoperative drainage volume, and the visual analogue scale (VAS) scores at 24 and 72 hours were lower in the UVATS group compared to the MVATS group, while the chest tube retention time and hospitalization time were shorter than those in the MVATS group (P<0.05). The quality of life at six months after surgery in the UVATS group was significantly higher than that in the MVATS group (P<0.05). Conclusions: UVATS and MVATS have similar outcomes in patients with NSCLC. Although UVATS surgery takes longer and is associated with more interoperative bleeding, it can reduce postoperative pain, shorten postoperative recovery time, and help further improve the quality of life of patients after surgery. doi: https://doi.org/10.12669/pjms.40.6.9313 How to cite this: Zheng X, Wang W, Li X, He P, Wu X. Abstract:Minimally invasive techniques, typified by video-assisted thoracoscopic surgery, are widely practiced in the treatment of thoracic diseases all around the world, and video-assisted thoracoscopic surgery has been recognized as a standard treatment method for early staged lung cancer. Among them, robotic-assisted thoracoscopic surgery, which has the advantages of providing a three-dimensional view and better maneuverability, has emerged as a next-generation technique in the field of minimally invasive surgery and is also gaining its popularity with the idea of Enhanced Recovery After Surgery deeply rooted in patients' minds. Up to now, robotic-assisted thoracoscopic surgery usually requires 3 or 4 ports with 1 or 2 additional access incisions. Meanwhile, traditional video-assisted thoracoscopic surgery can now be completed with uniportal method, with less postoperative pain and higher patient satisfaction with respect to the number of incisions in comparison with the multi-port technique. To inform the integration of these new minimally invasive techniques, here, we present a case in which uniportal right upper lobectomy was performed using the 4<sup>th</sup> generation <i>da Vinci</i> Robotic Surgical System (Xi). With continuous innovation in robotic minimally invasive techniques and improvements in surgical skills, we believe more patients will benefit from robotic-assisted thoracoscopic surgery with single port in the near future.

Abstract:In recent years, the field of minimally invasive thoracic surgery has experienced significant advancements driven by improvements in video-assisted thoracoscopic surgery (VATS) techniques and surgical instruments. These advances have given rise to uniportal VATS as a new area of exploration in minimally invasive thoracic surgery. This technique presents several potential advantages, including reduced access trauma, less postoperative pain, improved cosmesis, fewer complications, shorter hospital stays, and faster rehabilitation, ultimately leading to an improvement in patient quality of life.
