Abstract:Objective To assess the value of scoring by contrast-enhanced ultrasound (CEUS) combined with ultrasonic elastography (UE) in the differential diagnosis of Breast Imaging Reporting and Date System for Ultrasound (BI-RADS-US) 4 lesions. Methods From April 2012 to February 2016, a total of 172 BI-RADS 4 lesions in 158 patients who underwent CEUS and UE at the Second Affiliated Hospital of Medical College of Zhejiang University were included. The scores of each lesion were evaluated. The diagnostic results were compared with biopsy or operative pathology. ROC curve was used to analyze the value of scoring by CEUS combined with UE in the differential diagnosis of BI-RADS 4 lesions. Results Among the 172 lesions, pathological diagnosis showed benign lesions in 80 cases, and malignant lesions in 92. The area under the ROC curve (AUC) of CEUS scoring alone was 0.894, and the sensitivity, specificity, and accuracy were 84.78%, 85.00%, and 84.88%, respectively. The area under the ROC curve (AUC) of UE scoring alone was 0.844, and the sensitivity, specificity, and accuracy were 82.61%, 82.50%, and 82.56%, respectively. When the cut-off value of the score of CEUS combined with UE was set at 7, the AUC was 0.951, and the sensitivity, specificity, and accuracy were 94.57%, 93.75%, and 94.19%, respectively, which were all higher than those of the two modalities alone (P<0.05). Conclusion Scoring by CEUS combined with UE can significantly improve the accuracy in the diagnosis of BI-RADS 4 lesions, and it can be used as a supplement to the BI-RADS classification to reduce unnecessary preoperative biopsy. Abstract:Objective:To evaluate the relationships among contrast-enhanced ultrasound (CEUS) features, molecular type, and biomarker expression of breast cancer.Methods:A retrospectively analysis of breast cancer patients confirmed by pathology were performed using Breast Imaging Report And Data System (BI-RADS) ultrasound category lesions in the Second Affiliated Hospital Zhejiang University School of Medicine from May 2020 to April 2021. All patients underwent conventional ultrasound and CEUS before biopsy and/or surgery. The relationships among BI-RADS category, quantitative and qualitative CEUS features and biomarker expression of breast cancer were evaluated.Results:All 149 patients with 149 breast lesions were included. The numbers of BI-RADS category 4A, 4B, 4C, and 5 were 8, 60, 49, and 32, respectively. Among them, the numbers of Luminal A like, Luminal B like (human epidermal growth factor receptor-2 (HER-2) positive), Luminal B like (HER-2 negative), HER-2 overexpression and triple negative type were 81, 29, 17, 15, and 7. No significant correlations were found among BI-RADS category, molecular types, and biomarker estrogen receptor (ER), progesterone receptor (PR), HER-2, and antigen Ki-67 (Ki-67) expression (all P>0.05). There were no correlations between quantitative or qualitative CEUS features and molecular types of breast cancer (all P>0.05). There were no correlations between qualitative CEUS variables and ER, PR, HER-2, and Ki-67 expression (all P>0.05). Ascending slope (AS) were negatively correlated with ER and PR expression( r=-0.40, P=0.01; r=-0.35, P=0.03). Descending slope (DS) were positively correlated with ER and PR expression( r=0.42, P=0.01; r=0.36, P=0.03). Arrive time (AT) were positively correlated with HER-2 expression( r=0.37, P=0.02). Conclusions:AS and DS are correlated with ER and PR expression.Arrive time (AT) is correlated with HER-2 expression. The quantitative variables of CEUS are helpful for evaluation of biomarker expression in breast cancer.
Enhancement Patterns Of Gastric Carcinoma On Contrast-Enhanced Ultrasonography: Relationship With Clinicopathological Features

Abstract:The aim of this study was to assess the relationship between the enhancement patterns and clinicopathological features of gastric cancer using intravenous contrast-enhanced ultrasonography (CEUS). In this Ethics Committee-approved prospective study, five hundred fifty two patients with gastric cancer who gave informed consent were examined preoperatively with CEUS. The enhancement pattern of each tumor was analyzed visually. Gross and histopathological findings on the postoperative specimens were compared with the preoperative CEUS findings. The most common CEUS pattern in differentiated gastric cancer was homogeneous enhancement, whereas heterogeneous enhancement was the most common pattern in undifferentiated gastric cancer. The proportion of heterogeneous enhancement was significantly different between the two histological subtypes (Chi- square = 146.735, P<0.001). The sensitivity and specificity of early heterogeneous enhancement on CEUS in diagnosing undifferentiated gastric cancer were 78.84% and 72.59% respectively. Gastric cancers with heterogeneous enhancement were more often Borrmann III and IV macroscopic types than those with homogeneous enhancement (66.56% vs. 30.80%, P<0.001), more commonly T3 and T4 depth of invasion than those with homogeneous enhancement (71.52% vs. 59.60%, P<0.05), more often showed lymphatic invasion than those with homogeneous enhancement (84.44% vs. 76.40%, P<0.05), and were less likely to receive curative gastrectomy than those with homogeneous enhancement (74.83% vs. 86.40%, P<0.005). The intra- and inter-observer reproducibility were both almost perfect for assessing enhancement patterns, with Kappa values of 0.916 (P<0.001) for intra-observer and 0.842 (P<0.001) for inter-observer reproducibility. CEUS provided detailed information about tumor vascularity and contrast enhancement patterns in gastric cancer. CEUS is promising as a new and useful method to predict the histological type of gastric cancer.
Contrast enhanced ultrasonography vs.contrast enhanced computed tomography for the diagnosis of focal lesions of the pancreas

Abstract:Objective To assess the characterization and usefulness of contrast enhanced ultrasound (CEUS) to diagnose focal lesions of pancreas in comparison to contrast enhanced computed tomography (CECT).Methods 177 cases of focal solid lesions of the pancreas confirmed by pathology were collected.The enhanced patterns and diagnostic capability of CEUS were analyzed,and these results were compared with those of CECT.Results The diagnostic accuracies of ultrasound,CEUS and CECT were 61.6％ (109/177),89.3％ (158/177),and 92.1％ (163/177),respectively.There was no significant difference between the diagnostic accuracies of CEUS and CECT (P =0.071).The sensitivity,specificity,positive predictive value (PPV),negative predictive value (NPV),and accurate rate for CEUS were 90.9％,63.6％,73.8％,86.2％,82.5％ (the areas under the curve,AUC =0.820),and 89.8％,69.3％,76.7％,85.9％,84.7％ for CECT (AUC =0.847),respectively,when the hypo-enhanced was regarded as the cut value.No significant difference was found between these results from CEUS and CECT (P =0.071).Conclusions The diagnostic ability of CEUS in focal lesions of the pancreas is similar to that of CECT.

Abstract:To observe ultrasonographic features of urothelial carcinoma in renal pelvis and evaluate contrast-enhanced ultrasound (CEUS) in diagnosis.Fifty-two patients with urothelial carcinoma underwent preoperative conventional US, colour Doppler flow imaging (CDFI) and CEUS.Of 52 total lesions, 41 (78.8%) could be clearly identified by US, and 49 (94.2%) were enhanced by CEUS. Among US-imaged lesions, 39 (95.1%) were solid tumours, and two (4.9%) were mixed solid-cystic; 25 (61.0%) were isoechoic, 11 (26.8%) hypoechoic, and five (12.2%) hyperechoic. Analysis of tumour blood flow by CDFI characterised 17 avascular lesions (41.5% of total), 16 hypovascular (39.0%), and 8 hypervascular (19.5%). The resistance index ranged from 0.65 to 0.88 (mean of 0.71). Enhancement was seen in 49 lesions after injection of SonoVue. A slow enhancement pattern was observed in 36 lesions (73.5%) relative to renal cortex, and 13/49 (26.5%) showed simultaneous enhancement. At peak enhancement, 38 lesions (77.6%) were hypo-enhanced, six (12.2%) iso-enhanced, and five (10.2%) hyper-enhanced. There were 12 lesions with intertumoural necrosis or haemorrhage (24.5%) that were heterogeneously enhanced, and 37 (75.5%) were homogeneously enhanced. A fast washout pattern was observed in 46 lesions (93.9%), synchronous washout in two (4.08%), and slow washout in one (2.04%).Slow-in, fast-out, and hypo-enhancement properties are associated with renal urothelial carcinoma and may thus have diagnostic value. We found that CEUS is able to identify tumours that are ambiguous by conventional US, and it thus significantly improves the confidence of diagnosis.

Abstract:OBJECTIVE:To study the clinical diagnostic value of contrast-enhanced ultrasound (CEUS) in bladder occupied lesions.METHODS:38 cases of conventional-ultrasound-found bladder occupied lesions did color Doppler flow imaging (CDFI) and CEUS checks. By comparing the difference between two types of blood flow imaging technologies in displaying the flow of bladder occupied lesions and observing the perfusion modes of contrast agents to enter lesions, the perfusion characteristics of CEUS were analyzed. Finally, they were contrasted with the surgical pathology results.RESULTS:Of all the 38 cases, there were 51 bladder occupied lesions, including 43 bladder malignant tumors, 2 bladder inverted papillomas, and 6 glandular cystitis lesions. The blood flow display rate of bladder occupied lesions was 100% using CEUS. Apparently, it was higher than that of CDFI (62.7%), and the result of these showed a statistically significant difference (P < 0.05). Using CEUS, 46 malignant lesions and 5 glandular cystitis lesions were indicated, and the diagnostic accuracy rate was 86.3%.CONCLUSION:CEUS can improve the blood flow display rate of bladder occupied lesions, and it can also observe the real-time blood flow of these lesions. It can help judge their nature and has a higher clinical value in differentiating the benign from the malignant.

Abstract:Contrast-enhanced ultrasound (CEUS) is an advanced imaging technique that integrates conventional US with the intravenous injection of specific US contrast agents (UCAs), combining the non-invasiveness of US with the higher accuracy of contrast-enhanced imaging. In contrast with magnetic resonance imaging (MRI), computed tomography (CT) and cystoscopy, CEUS has few contraindications, and UCAs are non-nephrotoxic agents that can be safely used in patients with kidney failure. CEUS is a well-established method for the detection of liver lesions and for echocardiography, and its indications are expanding. The updated 2018 WFUMB-EFSUMB guidelines have added the urinary bladder under non-hepatic applications of CEUS. The technique is able to distinguish between benign tissue, such as clots or hematoma, and malignant lesions by perfusing the mass with contrast agent. Thanks to the different perfusion rates of the various layers of the bladder wall, CEUS is also able to predict tumor invasion depth and stage. Despite that, current urological guidelines do not include CEUS as a plausible imaging technique for bladder urothelial carcinoma. The main reason for this omission might be the presence of scarce randomized evidence and the absence of large validated series. In this review, we describe the rationale behind the use of CEUS in bladder cancer and the added value of this imaging technique in the detection and staging of bladder lesions. In addition, we researched the available literature on the topic and then described the results of randomized clinical trials and a meta-analysis investigating the accuracy of CEUS in bladder cancer diagnosis and staging. The reported studies show that CEUS is a highly accurate diagnostic and staging tool for BC, reaching levels of specificity and sensitivity in differentiating between Ta-T1, or low-grade BC, and T2, or high-grade BC, that are comparable to those shown by the reference standard methods. Nonetheless, several limitations were found and are highlighted in this review. The aim of this study is to further validate and promote the use of CEUS as a quick, economic and effective diagnostic tool for this high-impact disease.

Abstract:Contrast-enhanced ultrasound (CEUS) is an advanced ultrasound (US) technique utilizing ultrasound contrast agents (UCAs) to provide detailed visualization of anatomic and vascular architecture, including the depiction of microcirculation. CEUS has been well-established in echocardiography and imaging of focal hepatic lesions and recent studies have also shown the utility of CEUS in non-hepatic applications like the urinary system. The updated guidelines by the European Federation of Societies for Ultrasound in Medicine and Biology (EFSUMB) from 2018 describe the use of CEUS for non-hepatic applications. CEUS' excellent safety profile and spatial resolution make it a superior modality to conventional US and is often comparable and even superior to CECT in some instances. In comparison to other cross-sectional imaging modalities such as CECT or MRI, CEUS offers a safe (by virtue of non-nephrotoxic US contrast agents), accurate, cost-efficient, readily available, and a quick means of evaluation of multiple pathologies of the urinary system. CEUS also has the potential to reduce the overall economic burden on patients requiring long-term follow-up due to its low cost as compared to CT or MRI techniques. This comprehensive review focuses on the applications of CEUS in evaluating the urinary system from the kidneys to the urinary bladder. CEUS can be utilized in the kidney to evaluate complex cystic lesions, indeterminate lesions, pseudotumors (vs solid renal tumors), renal infections, and renal ischemic disorders. Additionally, CEUS has also been utilized in evaluating renal transplants. In the urinary bladder, CEUS is extremely useful in differentiating a bladder hematoma and bladder cancer when conventional US techniques show equivocal results. Quantitative parameters of time-intensity curves (TICs) of CEUS examinations have also been studied to stage and grade bladder cancers. Although promising, further research is needed to definitively stage bladder cancers and classify them as muscle-invasive or non-muscle invasive using quantitative CEUS to guide appropriate intervention. CEUS has been very effective in the classification of cystic renal lesions, however, further research is needed in differentiating benign from malignant renal masses.

Abstract:The conventional ultrasound and color doppler ultrasound have been a focus in the field of emergency and critical care medicine in recent years,but cannot evaluate the microcirculation perfusion quantitatively.Thus,their diagnostic accuracy in predicting clinical outcomes for critically ill patients is suboptimal.Contrast-enhanced ultrasound (CEUS) is based on conventional ultrasound in which ultrasound contrast agent is used to enhance echo and signal intensity,and helps to characterize tissue perfusion more clearly and dynamically.It has been widely used in the field of emergency and critical care medicine,such as the location and dynamic evaluation of traumatic lesions,renal microcirculation monitoring for the acute kidney injury (AKI) and accuracy evaluation of central venous catheterization.CEUS provides a simple,non-invasive and effective way for clinical diagnosis and treatment of critically ill patients.

Abstract:OBJECTIVE:To explore the value of contrast-enhanced ultrasound (CEUS) in the diagnosis of renal cystic lesion.METHODS:Totally 88 patients with 97 atypical cystic lesions of kidneys were examined by conventional ultrasound (color Doppler flow imaging and power Doppler flow imaging), CEUS, and contrast-enhanced computed tomography (CECT), respectively. The results from the three imaging approaches were classified by Bosniak classification system and compared with each other. In patients who underwent surgeries, the pathological results were used to validate the imaging results.RESULTS:The results of CEUS and CECT on atypical cystic lesions were not significantly different (P > 0.05). The results gained by conventional ultrasound were significantly different from those of CEUS and CECT (P < 0.05). Compared with the pathological results of 32 patients who underwent surgery, the sensitivities of CEUS in the diagnosis of benign, doubtfully malignant, and malignant cystic tumors of kidney were 63.2%, 95.0%, and 94.7%, respectively, the specificities were 92.9%, 76.9%, and 85.7%, respectively, and the accuracies were 75.8%, 87.9%, and 90.0%, respectively.CONCLUSIONS:The classification of atypical cystic lesions using CEUS is highly consistent with CECT. CEUS is valuable in the diagnosis of benign and malignant renal cystic lesions.
Clinical Application Value of Contrast-Enhanced Ultrasound in the Diagnosis of Renal Space-Occupying Lesions

Abstract:Objective: To study the clinical application value of contrast-enhanced ultrasound (CEUS) in diagnosing renal space-occupying lesions. Methods: Sixty-seven patients with renal space-occupying lesions detected by routine ultrasound examination received the contrast-enhanced ultrasound examination. When observing the perfusion mode of the mass, we analyzed the perfusion characteristics of contrast-enhanced ultrasound and compared them with the surgical pathological results. Results: Sixty-seven lesions, which were identified in 67 patients with renal space-occupying lesions, included 55 renal malignant tumors and 12 benign ones. The sensitivity of qualitative diagnosis by CEUS imaging was 96.4%, the specificity was 66.7%, and the accuracy was 91.0%. Conclusion: The real-time blood supply of renal space-occupying lesions helps judge their nature according to the enhancement mode. It has high clinical application value in diagnosing benign and malignant lesions.
Contrast-enhanced Ultrasonography in Ureteral Lesions: a Preliminary Study

Abstract:Objective To explore the diagnostic value of contrast-enhanced ultrasonography (CEUS) in ureteral lesions.Methods The ultrasonogram of conventional ultrasound and CEUS were retrospectively analyzed in 19 confirmed ureteral lesions cases by operation and pathology.The size,inner echoes,boundary and color blood signal of ureteral lesions were observed by conventional ultrasound.The modality and phases of enhancement,including arrival time,peak time,washout time and appearance of internal structure,were observed by CEUS.Results The 19 ureteral tumors were of maximum widths between 2.1 to 7.7 cm.Conventional ultrasound showed 7 hypoechoic masses,10 isoechoic masses,1 hyperechoic mass and 1 unclear mass.Color Doppler flow imaging showed fairly rich blood signal in 8 tumors,a small amount of blood signal around tumor in 6 tumors and no blood signal in the rest.In early phase,CEUS showed enhancement in all 19 ureteral lesions,including synchronously enhancement in 8 tumors and delayed enhancement in 11 tumors.In peak time,hypoechogenicity compared to the normal renal cortex was shown in 3 tumors,hyperechogenicity in 11 tumors and isoechogenicity in 5 tumors.In late phase,fast wash-out was displayed in 16 tumors,isochronously wash-out in 1 tumor,delayed wash-out in 1 tumor and unclear in 1 tumor.The detection rates of blood supply and clear boundary in ureteral lesions and the diagnostic sensitivity for ureteral cancer were 74% (14/19),16% (3/19),50% (8/16) by conventional ultrasound respectively.The detection rates of blood supply and clear boundary in ureteral lesions and the diagnostic sensitivity for ureteral cancer were 100% (19/19) ,58% (11/19) ,94% (15/16) by CEUS respectively.The change of these performances was statistically significant between conventional ultrasound and CEUS.Conclusions CEUS can improve blood supply,boundary and the diagnostic sensitivity of ureteral lesions.
Contrast-enhanced Ultrasonographic Diagnosis of Renal Pelvic Carcinoma

Abstract:Objective To explore the potential value of contrast-enhanced ultrasonography(CEUS) in diagnosis of renal pelvic carcinoma.Methods The ultrasonogram of conventional ultrasound and CEUS were analyzed retrospectively in 22 patients of renal pelvic carcinoma proved pathologically.The size,echo,boundary and color flow signal of renal pelvic lesions were observed with conventional ultrasound.The enhancement modality and phase of tumors were also observed with CEUS,including wash-in and wash-out time,as well as the perfusion appearances.Results The maximum diameters ranged from 1.5 cm to 8.5 cm in 22 renal pelvic tumors.Conventional ultrasound detected flat mass in 7 tumors,irregular mass in 15 tumors;the same side hydronephrosis in 11 patients.Color Doppler flow imaging(CDFI) disclosed fairly rich flow signal of 5 tumors,a small amount of flow signal around tumor in 9 tumors and no-flow signal in the rest tumors.CEUS detected the cortical phase enhancement in all renal pelvic tumors,including synchronously enhancement in 8 tumors and delayed enhancement in 14 tumors.In peak time,hypoechogenicity compared to the normal renal cortex was showed in 18 tumors,hyperechogenicity in 3 tumors and isoechogenicity in 1 tumor.Fast wash-out in medulla phase was displayed in 20 tumors,isochronously wash-out in 1 tumor and delayed wash-out in 1 tumor.The diagnostic accuracy of the conventional ultrasound and CEUS was 63.64%(14/22) and 81.82%(18/22),respectively.Conclusion CEUS can depict blood flow supply and improve the diagnostic rate of renal pelvic carcinoma.

Abstract:Objective To investigate the value of contrast-enhanced ultrasound(CEUS)in differential diagnosis of complex renal cysts and clear renal cell carcinoma with cystic change(CRCCC).Methods The ultrasonographic datas of 82 lesions in 82 patients with complicated renal cysts or CRCCC confirmed by pathology were analyzed.The characteristics of conventional ultrasound and CEUS were observed and evaluated.The lesions were graded according to Bosniak classification criteria.Results Pathological examination showed that 36 cases were complicated renal cysts and 46 cases were CRCCC. Routine ultrasound showed there were 9 cases (25.0％) with cystic masses and 27 cases (75.0％) with solid and cystic masses in complex renal cysts,of which 14 cases (38.9％) could detect color flow signals.In CRCCC,2 cases(4.3％)were with cystic masses and 44 cases(95.7％)were with solid and cystic masses, of which 33 cases(75.0％)could detect color flow signals.CEUS showed that only 18 cases(50.0％)of the complex renal cysts showed enhancement of cystic wall or septum,with equal or low enhancement at the peak,9 cases(50.0％) accompanied by decrease of renal cortex,35 cases (97.2％) had thin and regular cystic wall,no enhancement of cystic wall in all lesions,and 33 cases(91.7％)had septal thickness less than 1 mm.Forty-five cases (97.8％) of CRCCC showed enhancement of cystic wall or septum,40 cases(88.9％)showed equal or high enhancement at peak,30 cases(66.7％)were faster than the decrease of renal cortex,37 cases (80.4％) showed uneven thickness of cystic wall,24 cases (52.2％) showed enhancement of cystic wall nodules,and 28 cases (60.9％) showed uneven thickness of septum.After CEUS,33 cases(91.7％)of complex renal cysts were classified as grade Ⅰ and Ⅱ,while 42 cases(91.3％) of CRCCC were classified as grade Ⅲ and Ⅳ.Conclusions The CEUS manifestations of complex renal cysts are different from those of CRCCC.The application of Bosniak criteria in CEUS is helpful for the differential diagnosis of complex renal cysts and CRCCC.

Abstract:Objective To study images of contrast-enhanced ultrasound(CEUS) in cystic renal cell carcinoma(CRCC) and its value was investigated.Methods The images of conventional Ultrasound(US) and CEUS examination were reviewed in the 21 patients.All cases were diagnosed with CRCC by operation and pathological examination.The size,inner echoes,boundary and color flow of tumors were observed by conventional ultrasound.Enhancement patterns and phases of cystic renal lesions on ultrasound were evaluated retrospectively,including enhancement start time,peak time,wash-out time and enhancement appearances for internal structures of cystic renal masses.Results The diameter of tumor ranged from 1.0-7.3 cm in the 21 tumors.The images showed 18 solid-cystic lesions,3 cystic lesions,8 with clear boundaries and 13 with ill-defined margins.The color flow was shown in the solid component or on septa in 16 cases and no color flow in the rest of these cases.All lesions showed whole-lesion enhancement in the arterial phase.The start time for mean enhancement was 14.6±3.6 seconds with a peak time of 21.2±3.7 seconds and a wash-out time of 31.3±5.0 seconds respectively.The common enhancement pattern(14/21) of the lesions were simultaneous wash-in and wash-out.All lesions include septal enhancement(16) and irregular wall or solid component enhancement(5).Conclusions The CEUS may effectively show the blood supply and sophisticated internal structures of cystic renal masses,which could be useful for the diagnosis of CRCC.

Abstract:OBJECTIVE:To evaluate the usefulness of contrast-enhanced ultrasonography (CEUS) in the diagnosis of biliary diseases. METHODS:CEUS was performed in 57 patients with biliary diseases. The contrast enhancement characteristics and the morphologic features were observed. The ultrasonographic results were compared with those obtained through conventional 2-D ultrasound (2D-US), color Doppler flow ultrasound (CDFI), and clinical, surgical, and laboratory findings. In some cases, the ultrasonographic results were also compared with those obtained from contrast-enhanced computed tomography (CECT). RESULTS:The diagnostic accuracy of 2D-US combined with CEUS was significantly higher than that of 2D-US combined with CDFI 87.7% vs 71.9%; chi(2) = 4.41, P < 0.05). CEUS clearly showed the presence/absence of blood supply in biliary lesions and offered real-time imaging of the microcirculation perfusion in the lesions. It also offered useful information to differentiate biliary tumors from stones, bile mud, and/or blood clots. It distinctly displayed the size and contour of the lesions as well as the infiltrated range, depth, and the involved area. However, CEUS is most useful in reflecting blood perfusion patterns; it had limited value in differentiating the malignancies of polypoid lesions. The diagnostic accuracy (87.0% vs 91.3%;chi(2) = 0. 45, P > 0.05) and the size and range of the lesions displayed (0.4-6.2 cm vs 0.4-6.0 cm, P = 0.721) were not significantly different between CEUS and CECT. CONCLUSION:CEUS is a useful tool in the routine ultrasonography of biliary diseases.

Abstract:Objective To investigate the value of contrast-enhanced ultrasonography (CEUS) in differential diagnosis of gallbladder lesions.Methods Ninety-four cases of gallbladder lesions,including 84 benign lesions(62 cholesterol polyps,8 adenomas,14 adenomyomatosis)and 10 carcinomas of the gallbladder,were examined by CEUS before surgery to observe the characteristics of vascular structures and gallbladder wall.Outcomes were compared with pathological findings.Results The vascular structures of cholesterol were showed as"homogeneous dotted"(32/62)or"branched vessel"(14/62)or"central vessel"type(16/62);adenomatosis as"branched vessel"(4/8)or"central vessel"type(4/8);adenomyomas as"homogeneous dotted"type(14/14);and carcinomas of the gallbladder as tortuous-type(10/10).The gallbladder wall was intact in 90.5%(76/84)benign lesions and was destroyed in 80.0%(8/10)carcinomas of the gallbladder.If the destruction gallbladder wall was considered as malignancy,the sensitivity,specificity,and accuracy were 80.0%(8/10),90.5%(76/84),and 89.4%(84/94),respectively,which were significantly better than conventional ultrasound(P0.01).Conclusion CEUS could show the vascular structures and gallbladder wall intactness,which were useful for the differential diagnosis of gallbladder lesions.

Abstract:Objective:This study was to investigate the application value of contrast-enhanced ultrasonograph(CEUS) in diagnosing abdominal disease.Methods: Thirty seven patients with focal liver lesions(FLLs),5 patients with renal cell carcinomas and 12 patients with vascular diseases were examined by CEUS.Appearances of targets in conventional ultrasound and CEUS were reviewed and refered to the European guidelines update in 2008.Results: Color Doppler flow imaging showed that 17 lesions were detected blood flow,however,CEUS showed 36 lesions.Different types of FLLs had different enhancement appearances.The sensitivity,specificity and accuracy of CEUS in diagnosing malignant FLLs were 83%,93% and 87%,respectively.Renal cell carcinomas showed multiform appearances in CEUS.CEUS was more helpful to diagnose vascular disease exactly.Conclusions: CEUS has a high clinical application value in diagnosis of FLLs,and it is prospective in diagnosing other abdominal diseases.
Contrast-enhanced Ultrasonography for Evaluation of Cystic Renal Mass: in Comparison to Contrast-Enhanced CT and Conventional Ultrasound

Abstract:Purpose To assess the value of contrast-enhanced ultrasonography (CEUS) in evaluating cystic renal lesions compared with conventional ultrasound (US) and contrast-enhanced computed tomography (CECT). Methods One hundred and three patients with complex cystic renal masses underwent preoperative US and CEUS, among which 70 conducted CECT at our institution. The images were analyzed with the number of septa, septa and wall thickness and the presence of solid component, and final diagnosis was made. Results In malignancies, CEUS demonstrated more septa, thicker wall or septa, and more solid components than US and CECT. CEUS permitted categorization of 51.7% (30/58) and 28.6% (10/35) of malignant tumors in higher grade than by US and CECT, respectively. In benign lesions, CEUS detected more septa than CECT and correctly diagnosed benign cysts which appeared as solid lesions in US. CEUS permitted downgrading of 71.1% (32/45) and 17.1% (6/35) of benign lesions compared to US and CECT. The diagnostic performance of CEUS was better than US for benign cystic lesions. The phenomenon that solid-like component by US did not enhance by CEUS was a strong predictor of benign disease, with a positive predictive value (PPV) of 100%. Enhancement of solid, soft tissue by CEUS was highly predictive of malignancy, with a PPV of 100%. Conclusions CEUS was superior to US and CECT in visualizing the number of septa septa and wall thickness, and the presence of solid component of cystic renal lesions. CEUS may play a similar role to CECT in the diagnosis of renal cystic lesions, and better than US.

Abstract:The aim of this study is to evaluate if contrast enhanced ultrasound (CEUS) can improve the differential diagnostic performance of gallbladder (GB) lesions.Materials and methods: Forty-nine patients (18 men, 31 women; mean age, 54.8±14.4 years, range age, 22-78 years) with GB lesions (mass-forming and wall-thickened types) were enrolled in this study. All patients underwent conventional ultrasonography (US) and CEUS examination. The imaging characteristics of GB lesions were analyzed to compare the diagnostic performance of US and CEUS. The final diagnosis was obtained by histopathology.Results: There were significant differences between benign and malignant GB lesions with regards to size, shape, vascularity, the integrity and margin of GB wall and time to iso-enhancement on CEUS (p<0.05). However, no significant difference was found concerning the enhancement patterns between the two groups (p>0.05). Logistic regression analysis showed that the boundary between liver and GB wall (p=0.017) and vascularity on color Doppler flow imaging (p=0.013) were two independent predictors of malignancy. The diagnostic accuracy of US could be improved in combination with CEUS (65.3% vs 83.7%). The diagnostic accuracy of the GB wall thickening type was higher than the mass forming type.Conclusion: CEUS could improve the diagnostic performance of GB lesions, especially for wall-thickened type lesions
