Abstract:The purpose of this study was to investigate the effect of blue light phototherapy on the expression of circadian genes in peripheral blood mononuclear cells (PBMC) and plasma melatonin levels in neonates. Real-time reverse-transcriptase polymerase chain reaction (RT-PCR) was used to determine the expression of Bmal1 and Cry1 in PBMC, and an enzyme-linked immunosorbent assay was used to determine plasma melatonin levels in 32 breast-milk jaundiced neonates before and after phototherapy, compared with 29 control neonates. The results showed that the expression of Bmal1 was decreased and Cry1 increased significantly after phototherapy. Plasma melatonin levels were decreased after phototherapy. There was no statistical difference in Bmal1 and Cry1 gene expression and plasma melatonin levels in the control group. In conclusion, phototherapy does affect the expression of the circadian genes Bmal1 and Cry1 in PBMC and plasma melatonin concentration in jaundiced neonates. Our results suggest that phototherapy should be timed according to circadian rhythms when treating jaundiced neonates.

Abstract:OBJECTIVE: To explore the efficacy of blue light-emitting diodes(LEDs) as a novel light source for phototherapy. METHODS: Equimolar bilirubin standard albumin solutions were irradiated with two light sources; LEDs and single side conventional blue light( sCBL). Light intensity was measured with a light meter. The emission intensity of LEDs was the same as sCBL. To estimate the effects of the two light sources, the change in bilirubin isomer before and during phtotherapy was determined using high-performance liquid chromatography. RESULTS: The effects of different light sources on equimolar bilirubin standard albumin solutions, the lumirubin (LR) and 4Z, 15E-Bilirubin IX alpha(ZE) formation of LEDs was the same as sCBL(P>0.05). CONCLUSION: LEDs is equally effective as single sCBL for phototherapy in vitro.
Clinical efficacy analysis of intermittent and continuous blue light irradiation treatments on neonatal hyperbilirubinemia

Abstract:Objective: To observe the efficacy of intermittent and continuous phototherapy treatments on neonatal unconjugated hyperbilirubinemia and their adverse effects on children.Methods: 180 cases of children were randomly selected and divided into two groups,namely,intermittent phototherapy group and continuous light therapy group.Analysis of the situation was made of the intermittent and continuous blue light treatments.Results: The difference between blu-ray irradiation of serum bilirubin before and after treatment in two groups was statistically significant(P<0.05).The adverse events of blu-ray irradiation after treatment in two groups were statistically significant difference(P<0.01).Conclusion: Intermittent blue light irradiation can reach the same effect of continuous blue light,but the intermittent blue light incidence of adverse reactions is significantly lower than that of the continuous blue light,worthy to be popularized.

Abstract:Objective:To retrieve, evaluate and integrate relevant evidence on the management of the developmentally supportive environment in the neonatal intensive care unit(NICU), and to provide clinical references.Methods:Evidence on NICU environmental management was retrieved from Guidelines International Network, Joanna Briggs Institute, National Institute for Health and Care Excellence, Scottish Intercollegiate Guide Network, National Guideline Clearinghouse, Registered Nurses′ Association of Ontario, Yimaitong and other Websites, BMJ Best Practice, UpToDate, Cochrane Library, PubMed, Embase, Wanfang Database, CNKI and other database.The evidence included guidelines, evidence summaries, best clinical practice manuals, expert consensus and systematic reviews.The date limit was from the establishment of the databases to March 31, 2021.Results:Totally 16 articles were involved, including 4 guidelines, 9 systematic reviews, and 3 expert consensus.Finally, 20 pieces of best evidence on four aspects were su-mmarized: sound, light, touch, and smell.There were 11 A-level recommendations and 9 B-level recommendations.The evidence suggested that health care workers should reduce noise and protect premature infants from being exposed to bright light, noxious gases, and negative touch stimuli.Besides, benign auditory and olfactory stimuli, circadian light, and mother-infant skin-to-skin contact should be used to promote the development of premature infants.Conclusions:This study is a summary of the recommendations on NICU environmental management.It is well-designed and has achieved fruitful results, showing great significance for reducing environmental stress of premature infants in the NICU.However, the current recommended methods for providing benign stimulation require validation of more high-quality, well-designed research.It is recommended that medical staff should selectively apply the evidence to clinical practice according to the actual situation.
Blue-green Light Phototherapy for Preterm Jaundice

Abstract:Objective To evaluate the efficacy of blue-green light phototherapy for jaundice in preterm newborns.Methods Total 124 preterm newborns with birth-weight below 2000 g were included in this study.Infants in group A (gestation age ≤32 weeks or/and instability,n =72) were treated with bluegreen light (group A1 ) or with single side blue light (group A2) randomly.Infants with gestation age ＞32weeks or/and stability (n =52) were also treated with blue-green light (group B1 ) or with single side blue light ( group B2) randomly.Peak levels of serum bilirubin ( PSB),the withdrawal time of jaundice and the duration of phototherapy were compared between groups.Results Group A:PSB of groupAl was lower than that of groupA2 [ (219 ± 31 ) μmol/L vs.( 257 ± 39 ) μ mol/L,P ＜ 0.01 ].The withdrawal time of jaundice group Al was earlier than that of group A2 [ ( 10.3 ±3.0) d vs.( 16.2 ±4.2) d,P ＜0.01 ].The duration of phototherapy of groupA1 was shorter than that of group A2 [ (87 ±34) h vs.(23 ±49) h,P ＜0.01 ].Group B:There were no differences in PSB and withdrawal time of jaundice between two subgroups ( P ＞0.05).The duration of phototherapy of groupB1 was shorter than that of group B2 [ (59 ±23) h vs.(73 ±74) h,P ＜ 0.05 ].Conclusion Blue-green light phototherapy is more effective than single side of blue light in treatment of jaundice in preterm newborns.

Abstract:Since its discovery in 50 ́s of the last century, 1 phototherapy of neonatal jaundice has become the golden standard in the treatment of severe neonatal jaundice. 2 Exposure of the newborn to the light of a specific wavelength (blue or blue-green light) leads to photisomerization and photooxidation of unconjugated bilirubin (UCB), the products of which are more polar and excretable through the kidneys into the urine and the liver into the bile. In the last seven decades, phototherapy has become a modern, evidence-based therapeutic tool, with the development of treatment protocols to maximize its efficacy. Among the factors most importantly affecting the efficacy of phototherapy are irradiance intensity (depending on the skin area exposed), distance of the light source (various sources have been developed, from fluorescence tubes, halogen spotlights, fiberoptic pads/blankets, to light-emitting diodes), the exposed skin, intermittence/continuance of phototherapy, and the spectrum of light. 2 Blue light, such as that emitted from blue light-emitting diodes (LED ́s) within the narrow range of 450–470 nm, has long been believed to be the most efficient, 3 based on the fact that the absorption maximum of UCB in aqueous media is around 440 nm. 4 However, recent data suggest that wavelengths longer than those reflecting the absorption maximum of UCB are even more effective in lowering serum UCB concentrations in newborns suffering from severe neonatal jaundice. The first experimental study on efficient bilirubin oxidation with blue-green/turquoise green light was reported as early as 1981 5 followed by several clinical studies proving the efficacy of green light. 6,7,8,9,10,11,12,13,14 In the recent study, Ebbesen and colleagues confirmed a significantly higher efficacy in reducing UCB of blue-green LED light phototherapy with a peak emission of 478 nm compared to standard blue LED light with a peak emission of 459 nm, 15 which was supported by quantifying the production of the main individual bilirubin photooxidation products.

Effect of High Quality Nursing Service During Blue Light Irradiation for the Treatment of Neonatal Jaundice

Abstract:Objective To explore the clinical effect of high quality nursing service in neonatal jaundice treated by blue light irradiation. Methods Historical control trial was taken in this study. 231 cases of neonatal jaundice after implement of high quality nursing service were set as the research group, while 219 cases of neonatal jaundice before the implement of high quality nursing service were set as the control group, and the incidences of neonatal adverse events were compared between two groups. 100 cases of neonatal families of each group were selected to evaluate the quality of care. Results The incidences of neonatal spilled milk, skin damage, goggles off, testicular damage of research group were significantly lower than those of control group(P 0.05). The scores of primary care, nurse attitude, specialist care and health condition evaluated by neonatal families of research group were significantly higher than those of control group(P 0.05).Conclusions High quality nursing service has significant effect in improving the quality of care, reducing the incidence of adverse nursing events and improving family's satisfaction.

Abstract:Objective:To explore the clinical efficacy of blue-ray with Yinzhihuang granule in the treatment of severe neonatal hyperbilirubinemia.Method:A total of 120 newborn infants with hyperbilirubinemia were divided into treatment group according to the level of total bilirubin(TBL≤12.9 mg/dL,n=60) and control group(12.9 mg/dl≤TBL<20 mg/L,n=60).All infants were given blue-ray illumination with Yinzhihuang granule.The level of bilirubin was tested every 24 hours until it reached the safe level(TBL<12.9 mg/dL).Two groups were tested by MRI respectively when they were admitted to hospital and one month after therapy.BAEP levels were also tested one month after therapy.Result:This treatment in two groups can quickly reduce the level of total ilirubin,and each time point had statistics significance(P<0.05),but the time of removing jaundice had no significance(P>0.05).And it can heal the injury of neural system,but there was no significance(P>0.05).Conclusions:The efficacy of blue-ray with Yinzhihuang granule for severe neonatal hyperbilirubinemia has noninferiority.
Yinzhihuang oral liquid combined with phototherapy for neonatal jaundice: a systematic review and meta-analysis of randomized clinical trials

Abstract:Background Neonatal jaundice affects at least 481,000 newborns every year. Phototherapy is recommended but it’s effects are limited and adverse reactions can occur. In China, phototherapy combined with Yinzhihuang oral liquid is also used for this condition. This systematic review evaluated the effectiveness and safety of combination therapy with Yinzhihuang oral liquid and phototherapy compared to phototherapy alone for treating neonatal jaundice. Method A comprehensive literature search was performed in four Chinese databases, two English language databases and two trial registries from inception to June 2017. Two authors independently screened the citations and retrieved full publications for randomized trials on Yinzhihuang oral liquid combined with phototherapy for neonatal jaundice. The methodological quality of the trials was assessed according to the Cochrane Collaboration’s tool for assessing risk of bias. Data were analyzed using RevMan 5.3. Result Totally 17 trials (involving 2561 neonates) were included in this review. Fourteen of them had a high risk of bias. Significant differences were detected between combination therapy and phototherapy alone for serum bilirubin level (MD − 50.25 μmol/L, 95% CI -64.01 to − 36.50, I 2 = 98%; 7 trials, post-hoc decision choosing random effects model), failure of jaundice resolution (RR 0.21, 95% CI 0.14 to 0.32, I 2 = 0%; 11 trials, fixed effects model), and time to jaundice resolution (MD − 2.17 days, 95%CI -2.96 to − 1.38, I 2 = 98%; 6 trials, random effects model). Adverse events were reported in eight trials but none were serious. Trial sequential analysis for serum bilirubin level suggested that the cumulative Z-curve (which represents 1478 participants) reached the required information size (DARIS = 1301 participants). Conclusion Based on trials with low methodological quality, Yinzhihuang oral liquid combined with phototherapy seemed to be safe and superior to phototherapy alone for reducing serum bilirubin in neonatal jaundice. These potential benefits need to be confirmed in future trials using rigorous methodology. Trial registration Systematic review registration: [PROSPERO registration: CRD42016037691 ].
Analysis the Clinical Efficacy of Blu-Ray Combined with Yinzhihuang Keli for Newborn Pathological Jaundice

Abstract:Objective To analysis the clinical efficacy of blue-ray combined with Yinzhihuang Keli for newborn pathological jaundice.Method120 newborns with neonatal jaundice were randomly divided into control group(traditional treatment group,took enzyme inducers phenobarbital and continuous blue-ray radiation for 8 h everyday) 60 cases and treatment group(Yinzhihuang Keli combined with traditional treatment group) 60cases,7 days for a course of treatment,then the efficacy of two groups were compared.Results After 7 days treatment,we found compared with control group,the speed of decline of serum bilirubin of treatment group was much faster,the degree of decline was much bigger and the average time of jaundice subside was much shorter,all P0.05.The total efficiency of treatment group was 98.33%which was significantly higher than the control group which was 91.67%,P0.05.Conclusion Compared with conventional treatment methods,blue-ray combined with Yinzhihuang Keli for neonatal jaundice would get a better efficacy,faster speed of jaundice subside and smaller side effects.

Clinical observation of traditional Chinese medicine bath on neonatal jaundice

Abstract:: Objective: To explore the clinical effect of traditional Chinese medicine bath on neonatal jaundice. Methods: 74 cases of neonatal jaundice treated in our hospital from February 2021 to October 2021 were randomly divided into control group and experimental group. They were treated with blue light irradiation and blue light irradiation + traditional Chinese medicine bath. The effective rate of clinical treatment, the improvement time of disease symptoms, the change of jaundice index, the level of serum bilirubin and adverse reactions were compared. Results: the effective rate of 97.30% in the experimental group was significantly higher than 81.08% in the control group (P < 0.05). The regression time of skin yellow stain and sclera yellow stain in the experimental group was shorter than that in the control group, the time of fetal stool turning yellow was shorter than that in the control group, and the hospital stay was shorter than that in the control group (P < 0.05). Before treatment, there was no significant difference in jaundice index between the two groups (P > 0.05). After treatment, the jaundice index of the two groups was significantly higher than that before treatment, and the experimental group was higher than that of the control group (P < 0.05). Before treatment, there was no significant difference in the levels of total bilirubin, direct bilirubin and indirect bilirubin between the two groups (P > 0.05). After treatment, the total bilirubin level, direct bilirubin level and indirect bilirubin level of the two groups were significantly lower than those before treatment, and the level of the experimental group was lower than that of the control group (P < 0.05). The incidence of adverse reactions in the treatment group was lower than that in the control group (P > 0.05). Conclusion: the therapeutic scheme of traditional Chinese medicine bath for neonatal jaundice has definite clinical effect and should be popularized in clinic.

Abstract:Introduction: Recent evidence suggests that blue-light phototherapy impacts gut microbiota composition in jaundiced newborns, leading to disturbances closely related to the therapy's side effects. As a result, gut microbiota may serve as a potential intervention target to mitigate these side effects. In this study, we aim to examine the effects of AB-GG (Lactobacillus rhamnosus LGG), Bb-12 (Bifidobacterium animalis Bb-12) and M-16V (Bifidobacterium breve M-16V) and their combination on the intestinal microbiota, metabolomics and phototherapy-related side effects in neonates with jaundice. Methods and analysis: A total of 100 jaundiced newborns aged two weeks or younger will be included in this randomized, single-blind (the parents knew, but the neonatologists did not know), single-center controlled trial to receive either 10 9 colony-forming units of AB-GG, Bb-12, M-16V, a combination of the three probiotics with blue-light phototherapy, or blue-light phototherapy alone. The experimental group will be treated with oral probiotics once daily for 30 days, while the control group will receive only blue-light phototherapy. The follow-up duration will last 30 days. The primary outcomes include changes in gut microbiota, metabolomics, and the incidence of phototherapy side effects, assessed after each phototherapy session, as well as on days 10, 20, and 30. Ethics and dissemination: The study protocol has been approved by the Ethics Committee of our institution. The findings of this trial will be submitted to a peer-reviewed pediatric journal. Abstract:Context: Yinzhihuang oral liquid, a well-known Chinese herbal formula, is a clinical drug for the treatment of neonatal jaundice, and a number of clinical trials have been published addressing this issue, but there is no comprehensive analysis that evaluates its efficacy for the treatment of newborn with hyperbilirubinaemia. Objective: A meta-analysis was conducted to evaluate the efficacy of Yinzhihuang oral liquid on neonatal jaundice. Methods: Search was performed throughout PubMed, Cochrane Library, EMBASE, Ovid, Wanfang, VIP Medicine Information System (VMIS) and China National Knowledge Infrastructure (CNKI) databases up to December 2015. The search terms were (Yinzhihuang oral liquid or Yinzhihuang oral solution), (neonatal jaundice or neonatal hyperbilirubinaemia), and (efficacy). Review Manager 5.2 software was used for analyzing the data. Data were pooled by using the random-effects models and expressed as relative ratio (RR), standardized mean difference (SMD) or mean difference (MD) with a 95% confidence interval (CI). The Cochrane tool was applied to assess the risk of bias of the trials. Results: Yinzhihuang oral liquid in conjunction with other therapy increased effective rate of neonatal jaundice therapy (RR =1.14, 95%CI: 1.08–1.20). Yinzhihuang oral liquid significantly eliminated overproduced bilirubin which was measured by TSB or TCB at the third day and fifth day during the treatment {[third day, SMD = −1.63, 95%CI: −2.20 to (−1.06)], [fifth day, SMD = −5.00, 95%CI: −7.88 to (−2.12)]}; Yinzhihuang oral liquid significantly shortened jaundice subsiding time [MD = −3.20, 95%CI: −6.01to (−0.39)]. Conclusion: Yinzhihuang oral liquid can be considered as an effective treatment option for neonatal jaundice.
Side Effects of Phototherapy for Neonatal Hyperbilirubinemia

Abstract:Blue light has been widely used for the treatment of neonatal hyperbilirubinemia since the 1950s. Neonatal phototherapy can decrease plasma unconjugated bilirubin level, thus preventing bilirubin encephalopathy, and greatly reduces the exchange transfusion rate. Generally, it is accepted that the side effects of neonatal phototherapy are not serious and seem to be well controlled, however recent research has provided new evidence. The short-term side effects of phototherapy include interference with maternal-infant interaction, imbalance of thermal environment and water loss, electrolyte disturbance, bronze baby syndrome and circadian rhythm disorder. In addition, phototherapy may be associated with some long-term side effects such as melanocytic nevi and skin cancer, allergic diseases, patent ductus arteriosus and retinal damage. Therefore, it is necessary to develop evidence-based guidelines, new light devices and alternative agents, as well as individualized treatments, to minimize the side effects of phototherapy.

Methods:Using INCU portable analyzer and BiliBlangket Light MeterII blu-ray irradiation apparatus to respectively test the temperature control parameters and the intensity of blue light phototherapy of the jaundice treatment cases, a lot of research data can be acquired. Results:On the base of researching the quality control parameters detection of neonatal jaundice treatment cases (30 sets in total), we can control the quality of treatment cases smoothly. Through these tests, we can fix the medical equipments which cannot pass the detections. Conclusion:The research on the quality control parameters detection of neonatal jaundice treatment cases makes the preventive maintenance and quality control quick and effective to ensure the treatment effect and safety for the neonatal.

Abstract:Objective. Phototherapy devices have been found to be an effective method for treating neonatal hyperbilirubinemia. We reviewed the current literature to determine whether home-based phototherapy is more effective than hospital-based phototherapy for the treatment of neonatal hyperbilirubinemia. Method. PubMed, Scopus, Embase, Cochrane library, CBM, CNKI, and Wanfang Data were searched to collect the comparative study of home-based phototherapy versus hospital-based phototherapy for the treatment of neonatal hyperbilirubinemia. All studies were found to be of low risk based on Cochrane Collaborative Risk of Bias Tool. Data were statistically extracted and evaluated by RevMan 5.3 software. Result. A total of 259 neonates were included in the meta-analysis. Compared with hospital-based phototherapy, home-based phototherapy appeared more effective for the treatment of neonatal hyperbilirubinemia in reducing the rate of total serum bilirubin (standard mean difference = 0.32, 95% confidence interval = −0.22 to 0.86, P = .04); however, there was no significant difference in duration of phototherapy (standard mean difference = 0.59, 95% confidence interval = 0.28 to 0.90, P = .06) in the 2 groups. Conclusion. Home-based phototherapy was more effective than hospital-based phototherapy in treatment for neonatal hyperbilirubinemia; home-based phototherapy is an effective, feasible, safe, and alternative to hospital-based phototherapy for neonatal hyperbilirubinemia.

Abstract:Objective To explore how to decrease toxic effect of bilirubin on newborn's brain and make the patients avoid the nuclear jaundice.Method 46 cases of hyperbilirubinemia were reviewed.Result 42 cases were cured and 4 were transferred to the advanced hospital.The follow-up interview showed good curative effect.Conclusion The blue light treatment should be applied to cure hyperbilirubinemia because the method is easy,safe and with little side effect.

Abstract:OBJECTIVETo summarize the principles and application of phototherapy consistent with the current 2022 American Academy of Pediatrics "Clinical Practice Guideline Revision for the Management of Hyperbilirubinemia in the Newborn Infant 35 or More Weeks of Gestation."METHODSRelevant literature was reviewed regarding phototherapy devices in the United States, specifically those that incorporate blue to blue-green light-emitting diode, fluorescent, halogen, or fiberoptic light sources, and their currently marketed indications.RESULTSThe efficacy of phototherapy devices varies widely because of nonstandardized use of light sources and configurations and irradiance meters. In summary, the most effective and safest devices have the following characteristics: (1) incorporation of narrow band blue-to-green light-emitting diode lamps (∼460–490 nm wavelength range; 478 nm optimal) that would best overlap the bilirubin absorption spectrum; (2) emission of irradiance of at least 30 μW/cm 2 /nm (in term infants); and (3) illumination of the exposed maximal body surface area of an infant (35% to 80%). Furthermore, accurate irradiance measurements should be performed using the appropriate irradiance meter calibrated for the wavelength range delivered by the phototherapy device.CONCLUSIONSWith proper administration of effective phototherapy to an infant without concurrent hemolysis, total serum or plasma bilirubin concentrations will decrease within the first 4 to 6 hours of initiation safely and effectively.

Abstract:Neonatal jaundice could occur in most neonates and phototherapy is one of the most important therapeutic methods. The optical system of neonatal jaundice phototherapy apparatus based on LED was reported in this paper. Modeling and simulation of fly-eye lens which could be used to generate uniform illumination has been conducted and parameters have been optimized. The simulation results show that in the illuminated region the luminous uniformity is better than 85%. Based on the simulation results, an optimizing design of the optical system based on LED was achieved, which could be widely used for home phototherapy in treating uncomplicated neonatal jaundice for its feasibility, safety and effectiveness.
