Abstract:Aims/Purpose: Systemic sclerosis, also known as scleroderma, is a rare autoimmune disease that involves the hardening of the skin and also hinders the blood supply by occluding the small arteries and capillaries that carry blood to the tissues. Our aim was to compare the retinal thickness of different segmentations at the macula. Methods: In this pilot study 22 eyes from subjects with systemic sclerosis were compared with 22 eyes from healthy subjects (controls). All participants underwent a complete ophthalmological study and optical coherence tomography (OCT) measurements (Spectralis, Heidelberg Engineering, Germany) including complete retina, inner and outer layers, intermediate retinal layers (inner nuclear layer + outer plexiform layer + outer nuclear layer) and ganglion cell complex thickness (macular retinal nerve fibre layer + ganglion cell layer + inner plexiform layer) at the macula using Early Treatment Diabetic Retinopathy Study (ETDRS) grid so foveal, parafoveal and perifoveal subfields were measured. The values of the thickness of each segmentation in relation to the thickness of the complete retina (considered layer segmentation thickness divided by complete retina thickness) were also compared. Results: Best‐corrected visual acuity was lower in systemic sclerosis group but age, gender, spherical equivalent, intraocular pressure and axial length did not show any differences between both groups. Diffuse thinning in parafovea of complete retina, inner retina, outer retina, intermediate retina and ganglion cell complex was found in systemic sclerosis group. Fovea is also thinner in complete retina and intermediate retina in systemic sclerosis group. When considering the comparisons of the results in relation to complete retina thickness no differences were found between groups. Conclusions: Systemic sclerosis induces the thinning specially in the parafovea of all the considered macular segmentations alike.

Abstract:Objectives To evaluate scleral thickness measurements of pterygium patients using anterior segment optical coherence tomography (AS-OCT) and to compare them with healthy individuals. Material and methods Scleral thickness was measured from 2, 4, 6 mm posterior to the scleral spur with AS-OCT (Swept Source OCT Triton, Topcon, Japan) in 4 quadrants (superior, inferior, nasal and temporal). Results Eyes with pterygium were determined as Group 1, and contralateral eyes without pterygium were determined as Group 2. Healthy controls were determined as Group 3. In the measurements made from 4 mm posterior, no significant difference was found between Group 1 and Group 2 in any quadrants (p > 0.05). In all measurements made from 4 mm posterior to the scleral spur, scleral thickness was found to be significantly higher in Group 1 compared to Group 3 (p 0.05). When Group 2 and Group 3 were compared, scleral thickness measurements made from 4 mm posterior to the scleral spur was significantly thicker in all quadrants in Group 2 (p > 0.05). Conclusion Scleral thickness was found to be higher in pterygium patients compared to healthy controls, especially when measured from 4 mm posterior to the scleral spur. It has been predicted that high scleral thickness may be associated with high fibroblast activity in subconjunctival structures, and this may predispose to pterygium.

Abstract:Background: Systemic Lupus Erythematosus is a multi-systemic disease that has a high morbidity rate. Choroids usually have a distinct structural makeup in different systemic disorders which makes it easier to be used as a potential tool for the study of disease activity. Methods: This study was an observational cross-sectional prospective study with a total of 51 Systemic Lupus Erythematosus patients and 51 normal controls were included. The choroidal thickness values were determined using the Spectralis Spectral Domain Optical Coherence Tomography instrument (Heidelberg Engineering). Results: The results showed that the mean subfoveal, nasal, and temporal choroidal thickness in Systemic Lupus Erythematosus patients with ocular manifestations had thinner choroidal thickness compared to normal controls with p<0.001, p=0.008, and p<0.001, respectively. On the other hand, Systemic Lupus Erythematosus patients without ocular manifestations had relatively thicker subfoveal choroidal thickness compared to normal controls (p<0.001) but nasal and temporal choroidal thickness were not statistically significant (p=0.264 and p=0.347 respectively). Conclusions: The findings suggested that choroidal thickness measurement may serve as an indicator of disease activity and prognosis in Systemic Lupus Erythematosus patients, as well as a potential tool to predict the occurrence of ocular manifestations. Thinning of the choroid may be associated with factors such as decreased blood flow leading to atrophy or chronic inflammation, while thickening of the choroid may indicate active stage of the disease and the possibility of severe ocular manifestations in the future.

Abstract:Background The condition of the sclera is associated with many ocular diseases.The measurement of human scleral thickness in vivo is helpful for us to understand the features of the sclera and related diseases.Objective The present study was to measure the anterior sclera thickness(AST) in patients with senile cataract and to analyze the relationship among AST and other associated ocular parameters.Methods This study was approved by the Ethic Commission of First Hospital of Peking University.Written informed consent was obtained from each individual prior to examination.One hundred and five senile cataract patients were recruited in this study.Central corneal thickness (CCT),corneal curvature (CCV) and axial length were measured using ultrasonic pachymeter,keratometer,and A-scan unit,respectively.The AST was measured at 2 mm posterior to the scleral spur in the temporal meridian using ultrasound biomicroscope (UBM).The differences of CCT,CCV,ocular axial length and AST between bilateral eyes and the different sexes were compared by the Paired test and independent sample t test.The correlations among various parameters were assessed by the Pearson linear correlation analysis.The differences of CCT and AST among different axial length groups were evaluated by one-way ANOVA.Results No significant differences were found in the CCT,CCV,axial length and AST between bilateral eyes (t =0.584,P =0.561 ; t =1.161,P =0.248 ; t =0.140,P =0.889 ; t =0.342,P =0.773).Temporal AST at 2 mm posterior to the sclera spur was (0.589 ±0.051)mm in the right eyes.An insignificant decline in CCT was found in the male group compared with the female group (right eyes:t =0.469,P =0.641 ; left eyes:t =0.465,P =0.643).However,compared with the female group,the increase of axial length,reduction of the mean CCV value and enhancement of the mean AST were observed(right eyes:all P＜0.01 ;left eyes:all P＜0.01).CCV showed a negative correlation with ocular axial length (r =-0.50,P＜0.01),but no significant correlation was found among age,CCT,ocular axial length and AST(P＞0.05).No remarkable differences were found in CCT and AST among the various axial length groups (CCT:F =0.998,P=0.372;AST:F=1.919,P=0.383).Conclusions In senile cataract patients,correlation is not found between AST and CCT;the increase of axial length is not associated with the thinning of the eyeball wall to a certain extent.Differences exist in some ocular parameters between different sexes.

Abstract:Purpose To investigate the anterior scleral thickness (AST) in patients with Marfan syndrome (MFS). Methods A prospective, cross‐sectional study was conducted at the Department of Ophthalmology, Ghent University Hospital, Ghent, including patients with a genetically confirmed clinical diagnosis of MFS and age‐, gender‐ and axial length‐matched controls. Subjects with known corneal, conjunctival or scleral pathology and a history of ocular surgery, including pars plana vitrectomy, recent contact lens use or high‐grade astigmatism were excluded. Subjects underwent non‐cycloplegic autorefraction, Scheimpflug‐based corneal tomography, axial length measurement and spectral‐domain optical coherence tomography (OCT). Abstract:Background: Systemic sclerosis is a complex autoimmune disease characterized by vasculopathy, fibrosis, and immune dysregulation. Ocular manifestations in these patients are increasingly recognized, suggesting potential correlations between systemic vascular abnormalities and ocular microvascular changes. Advancements in molecular immunology and imaging technology using ocular coherence tomography (OCT) have unveiled intricate pathways underlying possible disease pathogenesis. Understanding the interplay between retinal vascular abnormalities and molecular immunology parameters could provide insights into disease mechanisms and potential biomarkers. Purpose: The aim of this study was to investigate vascular abnormalities, detected with optical coherence tomography angiography (OCT-A), in systemic sclerosis patients and to find correlations between the severity of the disease detected with molecular immunology findings and OCT-A parameters. Methods: A group of 32 systemic sclerosis patients were compared with 9 healthy controls. Ganglion cell complex thickness (GCC), retina thickness of the fovea and parafovea, nerve fiber layer thickness (RNFL) and cup/disc area ratio were investigated using OCT. Vessel density (VD) of the superficial (SCP) and deep capillary plexus (DCP) of the whole macular area and ETDRS grid, size of the foveal avascular zone (FAZ) and vessel density of the radial peripapillary capillary plexus (RPCP) were evaluated using OCT-A. Modified Rodnan skin score (mRSS), capillaroscopy and disease duration were used to stage disease severity. Results: There was a statistically significant reduction in retina thickness of the fovea and parafovea, VD of the whole DCP, VD of the SCP and DCP in ETDRS grid in the patient group compared to controls (p < 0.001). The patients presented a significant enlargement of the FAZ (p 0.005). No significant correlation between OCT and OCT-A parameters and disease severity scores was found. Conclusions: OCT-A could represent a non-invasive tool to detect retinal microvascular damage in systemic sclerosis.

Abstract:Purpose: To evaluate the relationship between choroidal thickness (CT) and anterior scleral thickness (AST) in patients with subclinical keratoconus (SKC) and established keratoconus (KC). Methods: This single-center prospective case-control study included 97 eyes of 97 patients: 44 KC eyes, 14 SKC eyes, and 39 age- and axial length (AL)-matched healthy eyes. Using swept-source optical coherence tomography, the AST was manually measured in four directions and the CT was obtained automatically from the Early Treatment Diabetic Retinopathy Study (ETDRS) grid. Principal component analysis (PCA) was used to linearly reduce the dimensionality of nine CT inputs to one significant component, CT1. A multivariate model was created to evaluate the association between CT1, AST, and several ocular parameters in SKC and KC patients. Partial correlation was then performed to adjust the confounding factors and to examine the effect of AST on CT1. Results: The PCA showed that CT1 accounts for 86.54% of the total variance in the nine original CTs of the ETDRS grid. The CT1 model was associated with age, AL, and AST in the superior meridian in SKC eyes, whereas in KC eyes, it was correlated with gender, age, AL, and AST in the inferior meridian (p < 0.001). The partial correlation between CT1 and AST in the superior zone was found to be significant, positive, and strong in SKC eyes (r = 0.79, p = 0.019), whereas a significant, positive, and moderate correlation between CT1 and AST at the inferior zone (r = 0.41, p = 0.017) was observed in KC eyes. Conclusions: Choroidal tissue was significantly correlated with the anterior sclera across the vertical meridian. This relationship was observed over the superior sclera in SKC eyes, whereas in established KC, it was over the inferior sclera. These results reveal new insights regarding the interactions between the anterior and posterior structures of the KC eyes and confirm the enigma of the pathophysiology of KC.

Abstract:Abstract To investigate the correlations between finger microvascular morphology and function in patients with systemic sclerosis (SSc) and the status of ocular microcirculation, as detected by nailfold videocapillaroscopy (NVC), laser speckle contrast analysis (LASCA), and optical coherence tomography angiography (OCTA). The enrollment included 32 SSc patients, classified according to the 2013 ACR/EULAR criteria, and 27 sex- and age-matched healthy controls. The participants underwent comprehensive rheumatological and ophthalmological examinations, as well as NVC, LASCA, and OCTA analysis on the same day at a single center from March to October 2022. SSc patients receiving intravenous prostanoids cycles were assessed at least 1 month after infusion. Statistical analysis was conducted using Stata® 15.1. Significant direct correlations were observed between the mean capillary number (at NVC) and the mean perfusion of fingers (at LASCA) with the retinal and choroidal perfusion (at OCTA) (all p < 0.05). In addition, a significantly reduced retinal and choroidal perfusion was detected in SSc patients vs controls (all p < 0.05). Interestingly, diffuse cutaneous SSc (dcSSc) patients exhibited a lower choroidal perfusion ( p = 0.03) but an increased choroidal thickness (CT) than limited cutaneous SSc patients ( p < 0.001). CT was increased also in patients with positive Scl70 antibodies and with a history of digital ulcers directly correlating with disease duration ( r = 0.67, p = 0.001). Finally, the combination of LASCA and OCTA parameters showed a significant discrimination capacity between SSc patients and controls, with an area under the curve of 0.80 [95% CI (0.74, 0.87)]. Peripheral microvascular damage is correlated with impaired ocular microcirculation in SSc. The increased choroidal thickness observed in dcSSc may be related to local sub-endothelial extracellular matrix deposition. The combined analysis of choroidal and fingertip perfusion offers preliminary insights that may complement traditional diagnostic methods for SSc.

Abstract:A bstract Purpose : To evaluate the association between scleral thickness and a newly developed multimodal imaging-based classification of central serous chorioretinopathy (CSC). Design Retrospective, cross-sectional study. Methods : This study included 217 eyes of 217 patients classified as simple or complex CSC based on the established protocols. Clinical and anatomical factors were compared between the two types. The scleral thickness was measured at four locations using anterior-segment optical coherence tomography. Results : Of the 217 eyes, 167 were classified as simple CSC and 50 as complex CSC. The complex CSC group showed older age (P = .011), higher male ratio (P = .001), more bilateral involvement (P < .001), greater subfoveal choroidal thickness (P = .025), and higher frequency of loculation of fluid (P < .001) and ciliochoroidal effusion (P < .001) than the simple CSC group. The complex CSC group had significantly greater scleral thicknesses in the superior, temporal, inferior, and nasal directions (all P < .001) than the simple CSC group. Multivariable analysis revealed that older age (odds ratio [OR] 1.054, 95% confidence interval [CI] 1.013–1.097, P < .001), male sex (OR 10.445, 95% CI 1.151–94.778, P < .001), bilateral involvement (OR 7.641, 95% CI 3.316–17.607, P < .001), and the mean value of scleral thicknesses in four directions (OR 1.022, 95% CI 1.012–1.032, P < .001) were significantly associated with the complex CSC. Conclusions : Older age, male sex, bilateral involvement, and thick sclera were associated with the complex CSC. Scleral thickness seemed to determine the clinical manifestations of CSC.

Abstract:Purpose: To study the ophthalmologic manifestations of systemic sclerosis (SSc) and its correlation with autoantibody profile. Methods: A cross-sectional study on 200 eyes of 100 consecutive adult patients diagnosed with SSc was performed at a tertiary care center in Northern India. The examination of ocular adnexa, anterior segment, and posterior segment with slit-lamp biomicroscopy, tear film break-up time (TBUT), Schirmer’s II test, and choroidal thickness measurement by swept-source ocular coherence tomography was done. Autoantibody profile was available for 85 patients, and its statistical association with the ocular examination findings was analyzed. Results: In total, 100 patients (93 females and 7 males) were included. The mean age was 45.11 ± 11.68 years, and the mean disease duration was 6.93 ± 3.68 years. Meibomian gland disease was more commonly found in patients with the diffuse subtype of SSc ( P = 0.037). Choroidal thickness was increased in 34% and decreased in 7% (reference range = 307 ± 79 mm) patients. Reduced TBUT, meibomian gland dysfunction, and eyelid stiffness had a statistically significant association with the presence of anti-Scl-70 antibody ( P = 0.003, <0.0001, and 0.004, respectively). These patients had ocular fatigue, foreign body sensation, and burning sensation. No significant association was noted with the presence of SS-A/Ro and SS-B/La antibodies. Conclusion: This study highlights the need for an active comprehensive ophthalmic evaluation. Approximately 75% of the patients in our cohort had ocular involvement to varying extent. An isolated presence of anti-Scl70 antibody was also found to have a positive association with dry eye disease.

Abstract:Background: To assess the anterior scleral thickness (AST), Schlemm's canal diameter (SCD), trabecular meshwork diameter (TMD) and conjunctiva tenon capsule thickness (CTT) in high myopic (HM) subjects and HM subjects with glaucoma (HMG) compared to control eyes. Methods: One hundred and twenty eyes were included, and AST at 0, 1, 2 and 3 mm from the scleral spur, SCD, TMD and CTT were measured. Results: Mean age was 64.2 ± 11.0 years, and the temporal SCD and temporal TMD were significantly longer in the HMG subjects compared to the controls (380.0 ± 62 μm vs. 316.7 ± 72 μm, p = 0.001) and (637.6 ± 113 μm vs. 512.1 ± 97 μm, p = 0.000), respectively. There were no significant differences between the HM and HMG subjects in SCD and TMD (all p > 0.025). Compared to the HM subjects, the temporal AST0 (432.5 ± 79 μm vs. 532.8 ± 99 μm, p = 0.000), temporal AST1 (383.9 ± 64 μm vs. 460.5 ± 80 μm, p = 0.000), temporal AST2 (404.0 ± 68 μm vs. 464.0 ± 88 μm, p = 0.006) and temporal AST3 (403.0 ± 80 μm vs. 458.1 ± 91 μm, p = 0.014) were significantly thinner in the HMG group. No differences were found between the CTT in the three groups (all p > 0.025). Conclusions: Our data indicate a thinner AST in HMG subjects and no differences in SCD and TMD between HM and HMG subjects.

Abstract:Background: The comparison of retinal perfusion in the eyes of patients with systemic sclerosis (SSc) and in healthy controls using optical coherence tomography angiography (OCTA). The correlation between nailfold capillaroscopy results and OCTA findings among SSc. Methods: The study enrolled 31 patients with systemic sclerosis and 41 healthy controls. OCTA was performed in both groups to assess the retinal vasculature in the superficial (SCP) and deep (DCP) capillary plexuses and the foveal avascular zone (FAZ) area. Nailfold capillaroscopy (NC) was performed in SSc patients and compared to the FAZ area and the superficial and the deep vessel density. Results: In the SSc group, the parafoveal vessel density in DCP was significantly higher in relation to the mean value (p < 0.0001) and in each quadrant of the macula (p < 0.0001) compared to healthy subjects (p < 0.0001). The patients with early scleroderma patterns in capillaroscopy had a larger superficial and deep FAZ (p = 0.0104, p = 0.0076, respectively) than those with active and late patterns. There was a statistically significant difference in the FAZ when comparing early to active (p < 0.0001) and early to late scleroderma patterns (p < 0.0001). A statistically significant difference was found in the type of interstitial lung disease and the deep FAZ area (p = 0.0484). SSc patients with nonspecific interstitial pneumonia (NSIP) had a larger FAZ than those with usual interstitial pneumonia (UIP) (p = 0.0484). Moreover, NSIP cases had a higher parafoveal mean superficial vessel density than those with UIP (p = 0.0471). Conclusions: Our investigation showed that the peripheral microvascular system correlates with ocular microcirculatory impairment. The results indicate the important role of OCTA in the diagnosis, monitoring, and prognosis of microvascular changes in SSc.

Abstract:Purpose To test the prognostic role of anterior scleral substantia propria (ASSP) thickness in predicting the 3‐month response after half‐dose photodynamic therapy (PDT) in central serous chorioretinopathy (CSCR) and to assess its clinical relevance of ASSP in different CSCR phenotypes. Methods A prospective, exploratory, multi‐centre cohort study conducted at IRCCS San Martino Hospital (Genoa, Italy) and Jules‐Gonin Eye Hospital (Lausanne, Switzerland). Demographic and clinical data, and optical coherence tomography (OCT) were collected at baseline and 3 months after PDT. Based on OCT images, we categorized CSCR phenotypes and collected clinically relevant imaging metrics. ASSP thickness was obtained from four different measurements using anterior segment (AS) OCT. Multivariable regression models were performed to evaluate the distribution of ASSP thicknesses among different CSCR phenotypes and to test the prognostic role of ASSP thickness in discriminating between PDT responders (complete subretinal fluid reabsorption) and partial responders. Results The study cohort comprised 109 Caucasian patients (82 males, 75.2%) with a total of 142 eyes: 84 eyes simple (59.1%) versus 58 eyes complex (40.9%) CSCR. Abstract:In this study, we aim to investigate retinal thickness (RT) and superficial vascular density (SVD) differences between patients with systemic sclerosis (SSc) and healthy controls (HCs) by optical coherence tomography angiography (OCTA). Sixteen patients with a definitive SSc diagnosis without clinical signs of retinopathy and 16 normal control subjects were recruited. All individuals underwent OCTA scanning to assess macular RT and SVD. We divided each image into nine subregions as the early treatment diabetic retinopathy study (ETDRS). Visual acuity (VA) was considerably different between patients with SSc (32 eyes) and control subjects (32 eyes) ( p < 0.001 ). Compared to the control group, individuals with SSc had decreased inner RT in inner superior, outer superior, outer temporal, inner temporal, center, and inner nasal regions ( p < 0.05 ). Outer RT was decreased in the outer and inner temporal regions, and full RT was decreased in the regions of outer superior, inner superior, inner temporal, and outer temporal, in comparison to the control group ( p < 0.05 ). Patients with SSc had significant reduction of SVD in the inner and outer of both superior and temporal, besides outer nasal regions than controls. ( p < 0.05 ). Moreover, SVD was significantly associated with the outer temporal region of patients suffering from SSc ( p < 0.05 ). Diagnostic Sensitivity of RT and SVD of Inner Superior Regions in SSc, as indicated by areas under curves of the Receiver Operating Characteristic (ROC), were 0.874 (95% CI: 0.786–0.962) and 0.827 (95% CI: 0.704–0.950), respectively. In conclusion, VA may be affected by RT variations inside the macula in patients with SSc. Measuring RT with OCTA could be a useful predictor of early diagnosis.

Abstract:Purpose: The aim of the study was to investigate the corneal and anterior segment characteristics in pseudoexfoliation syndrome (PXS), depending on the severity of the disease. Materials and methods: This cross-sectional study was conducted at a tertiary care hospital, in the ophthalmology department, in Turkey. Pseudoexfoliation syndrome group was classified into three groups. Pseudoexfoliative material on the part of the pupillary border was classified as group 1, on the entire pupillary border was classified as group 2, and pseudoexfoliative material on the entire pupillary border and iris surface was classified as group 3. The control group was composed of age and gender-matched individuals who had undergone a completely normal ophthalmologic examination. Scheimpflug tomography was performed to record tomographical data. Results: The PXS groups 1, 2, and 3, along with the control group, consist of 29, 32, 33, and 42 participants, respectively. There was no significant difference in age-gender distribution between patients and the control group (P>0.05). Group 3 showed significant differences in central corneal thickness compared with the control group (P=0.001). In addition, anterior chamber angle was found to be significantly lower in group 3 than the control group (P<0.001). In corneal densitometry (CD) values, significant increases compared with the control group were observed in groups 2 and 3 across all anterior annuli (P<0.001). In group 1, significant increases observed in the anterior 6-10 mm and 10-12 mm annuli (P<0.001). In the middle 6-10 mm annuluses, only group 3 showed a significant increase compared with the control group (P<0.001). In the posterior 0-2, 2-6, and 6-10 mm annuli, significant increases compared with the control were observed only in groups 2 and 3 (P=0.001, P=0.001, P<0.001, respectively). Conclusions: Corneal densitometry values, even without an increase in intraocular pressure, have a tendency to increase in PXS depending on the severity of the disease. While minimal changes were observed in the middle layer, it was particularly noted that the effects of the disease were more prominent in the peripheral cornea.

Abstract:Purpose. To map the corneal epithelial thickness in vivo with Fourier-domain optical coherence tomography in long-term soft contact lens (SCL) wearers.Methods. This is a cross-sectional observational study. Forty eyes from 40 normal subjects who had never worn SCL and 40 eyes from 40 SCL wearers who had worn lenses for more than 2 years were enrolled. Corneal epithelium over the entire cornea was topographically imaged using a novel optical coherence tomography system. An epithelial thickness map was automatically generated. Epithelial thicknesses of the central 2-mm, paracentral 2- to 5-mm(P1), and midperipheral 5- to 6-mm (P2) zones were obtained. In addition, the epithelial map variability in P1 and P2 zones, including maximum - minimum (MAX - MIN), map SD, and coefficient of variation (CV), was measured and analyzed.Results. The average epithelial thickness of the central, P1, and P2 zones was 54.4 +/- 1.1 mu m, 53.2 +/- 2.2 mu m, and 52.3 +/- 2.0 mu m, respectively, in normal eyes and 49.2 +/- 1.9 mu m, 48.8 +/- 2.2 mu m, and 48.7 +/- 2.8 mu m, respectively, in eyes wearing SCL. Compared with normal control subjects, eyes with long-term SCL had significantly thinner epithelial thickness in all three zones (p < 0.05). However, there was no difference in MAX - MIN, SD, and CV of P1 and P2 zones between two groups. In both groups, there was significant difference in the epithelial thickness among different sectors in the paracentral and midperipheral zones.Conclusions. There is a decrease in epithelial thickness in subjects who wear SCL long term. Clinicians should take note of the nonuniformity of the paracentral and midperipheral corneal epithelium thicknesses. This method may be useful for detecting early changes in corneal epithelial thickness caused by long-term SCL wear.

Abstract:Clinical relevance: Expanding practitioner knowledge regarding potential changes in ocular structure of keratoconic eyes will improve the eye care practice and patient management. Background: This study aimed to compare the difference in choroidal thickness between keratoconus patients and two control groups of myopic-astigmatism and emmetropic subjects. Methods: A case-control study was undertaken which included 50 patients with keratoconus, 50 with myopic-astigmatism, 30 with emmetropia aged between 18 and 39 years. Choroidal thickness was measured at three different locations, including the subfoveal, nasal, and temporal (750 micrometres) to the fovea, using a spectral-domain optical coherence tomography with an enhanced depth imaging technique. Results: The mean spherical equivalents were 0.03 ± 0.05, -3.00 ± 0.2, and -3.00 ± 0.3 dioptre in emmetropic, myopic-astigmatism and keratoconus subjects, respectively. The choroid was significantly thicker in keratoconus patients than in myopic-astigmatism and emmetropic subjects in the subfoveal (396 ± 14, 314 ± 12, and 320 ± 18 μm, respectively, p < 0.001), temporal (405 ± 14, 317 ± 12, and 328 ± 19 μm, respectively, p < 0.001) and nasal (376 ± 14, 285 ± 12, and 311 ± 18 μm, respectively; p < 0.001). Conclusion: Choroidal thickness is increased in keratoconus. The exact mechanism for choroidal thickening in individuals with keratoconus is unknown, but inflammatory responses could be the reason.
