T-wave changes in a patient with apical hypertrophic cardiomyopathy: a case with unique 'natural' history

Abstract:A 75-year-old asymptomatic male with a 10-year history of type 2 diabetes and recently developed hypertension was referred to our hospital following an abnormal electrocardiogram. Laboratory tests showed normal troponin levels. Electrocardiography revealed ST-segment depression in leads V4–V6, accompanied by an upright or bidirectional T-wave ( Panel A ; Supplementary data online , Figure ). Contrast echocardiography showed an 'ace-of-spades' configuration of the left ventricular cavity ( Panel B ; Supplementary data online , Video ), with apical segment thickness ranging from 12 to 13.5 mm. Cardiac magnetic resonance imaging confirmed these findings ( Panel C ), leading to a diagnosis of relative apical hypertrophic cardiomyopathy (ApHCM). 1 The coronary angiography revealed 90% stenosis in the left anterior descending artery (LAD), which was successfully stented ( Panels D and E ). Immediately after the intervention, the T-wave inverted in V4–V6 leads ( Panel F ). Over the subsequent 4 years of follow-up, there was a progression in the amplitude of the negative T-wave ( Panels G and H ) and an increase in apical wall thickness to 15.8 mm on cardiac computed tomography ( Panel I ).
