Abstract:OBJECTIVE:To investigate the clinical effects and safety of the technique of axillary approach brachial plexus blocking by ultrasound-guided four points via one-puncture.METHODS:Eighty patients scheduled for elective operation were randomly divided into 2 equal groups to undergo axillary approach brachial plexus blocking by ultrasound-guided four points via one-puncture technique (Group U) or nerve stimulator-guided brachial plexus blocking (Group N). The main branches of brachial plexus (radial, median, ulnar, and musculocutaneous nerves) were localized by ultrasound-guided or nerve stimulator-guided techniques. In Group U 8 ml of mixed anesthetic solution containing isovolumetric 0.75% ropivacaine and 2% lidocaine was injected into the 4 main branches of brachial plexus, with a total volume of 32 ml. The ultrasonic manifestations of the brachial plexus and its surrounding tissues were observed. The values of diameter and depth of the 4 nerves and the distance of the musculocutaneous nerve to the midpoint of axillary artery were measured. The manipulation time, onset time, maintaining time, efficacy of blocking, and incidence of complication were recorded.RESULTS:The manipulation time of Group U was 5.2 +/- 2.1 min, significantly shorter than that of Group N (14.6 +/- 3.2 min, P = 0.000), The onset times of the median, radial, and ulnar nerves of Group U were 3.3 +/- 1.9 min, 3.0 +/- 1.7 min, and 3.4 +/- 1.9 min respectively, all significantly shorter than those of Group N (4.6 +/- 2.0 min, 7.3 +/- 7.4 min, and 6.4 +/- 6.1 min respectively, P < 0.01 or P < 0.05). The anesthetic success rate of Group U was 100%, significantly higher than that of Group N (77.5%, P = 0.005). The rate of accidental puncture to blood vessel of Group U was 0, significantly lower than that of Group N (40%, P = 0.000).CONCLUSION:With significantly higher anesthetic success rate, shorter manipulation time and onset time, and lower complication rate, the technique of axillary approach brachial plexus blocking by ultrasound-guided four points via one-puncture is a safe and reliable blocking method in comparison with the nerve stimulator-guided method.

Abstract:Objective:Observing the anaesthesia quality of supraclavicular and infraclavicular brachial plexus block by ultrasound-guided,we evaluated the value of the brachial plexus block by mean of high-resolution sonography in order to explore the new approach of peripheral nerve block.Methods:One hundred and twenty patients scheduled for elective surgery of forearm and hand or Arteriovenous fistulization of forearm were randomly divided into two groups,i.e.,supraclavicular(SCB) group and infraclavicular(ICB) group,with 60 subjects in each group.The performance time was recorded.The onset time and the block execution time of axillary nerve,median cutaneous nerve of arm,lateral antebrachial cutaneous nerve,median antebrachial cutaneous nerve,ulnar nerve,median nerve,and radial nerve were measured respectively,and then the block quality was evaluated.Results:High-resolution ultrasound could successfully guided brachial plexus block and clearly displayed the local anesthetic distributing around never.The performance times were(4.5±3.2) min in SCB group and(3.9±1.6) min in ICB group(P0.05).The onset times of median antebrachial cutaneous,ulnar nerves and radial nervers in SCB group were(8.3±3.1) min,(7.1±2.8) min and(3.8±4.1) min,and these were(4.1±2.0) min,(3.6±1.7) min,and(8.5±1.8) min in the ICB.The onset times of the antebrachial cutaneous and ulnar nerves in the ICB group were significantly longer than these in SCB group(P0.05),and the radial nerve was shorter(P0.05).The complete success rates of block in SCB and ICB group were respectively 86% and 80%,respectively(P0.05).The incidence of complication for Horner syndrome in the SCB group was 16%,but there was no complication in ICB group.Conclusion:High-resolution sonography can clearly display the brachial plexus.Supraclavicular and infraclavicular brachial plexus block by ultrasound-guided are feasible technique in regional anaesthesia.Ultrasound-guided infracla vicular brachial plexus block is much safer and is deserved to apply in clinic practice.

Abstract:Pain is one of the most feared conditions a cancer patient may face. Bone is a common site of metastasis in many malignancies, including breast, prostate, kidney, and lung cancer. Conventional therapy for tumor-related bone pain involves the use of opioids, non-steroidal anti-inflammatory drugs, and bisphosphonates. Palliative radiation therapy may be incorporated for refractory bone pain. We describe an innovative case of cancer-related clavicle pain successfully managed with a superficial cervical plexus block in an opioid-intolerant patient. Considering the lack of guidelines for pain interventions in this setting, such peripheral nerve blocks may be a useful adjunctive tool in refractory cancer pain management.

Patients and Methods: A total of 62 female patients undergoing trans-areolar endoscopic thyroidectomy were randomly classified to the TTPB combined with ICPB group with ropivacaine (block group) or superficial cervical plexus block group (control group). The primary outcome measures were resting visual analogue scale (VAS) in the chest area at 6 h after surgery. The secondary outcome measures included chest resting and movement VAS score, neck resting and movement VAS score within 24 h after surgery, intraoperative remifentanil consumption, postoperative analgesia rate and analgesic requirements and patient satisfaction score for pain management at discharge. Results: Compared with the control group, the block group at rest showed consistently lower VAS scores in the chest area at 6 and 12 h after operation; the block group at rest showed lower VAS scores in the neck at 6, 12 and 24 h after operation. Regarding movement, the VAS scores of the chest and neck area at 2, 6, 12 and 24 h after the operation were lower in the block group than in the control group. The consumption of remifentanil, rate of postoperative analgesic requirements, and consumption of postoperative rescue analgesia in the block group were lower than those in the control group. Satisfaction with pain treatment at discharge was higher in the block group than in the control group. Conclusion: Ultrasound-guided TTPB combined with ICPB provides good analgesic effect in the early postoperative period after trans-areola endoscopic thyroidectomy. Keywords: regional anesthesia, nerve block, analgesia, trans-areolar endoscopic thyroidectomy Endoscopic thyroidectomy via the areola approach is one of the important modalities of minimally invasive thyroidectomy; it has the advantages of concealed incision and simultaneous management of bilateral thyroid lesions. 1,2 Breast endoscopic thyroidectomy requires the establishment of artificial channels above the two nipples and under the chest wall to separate the chest wall from the pectoralis major muscle to establish operating tunnels in the neck; thus, it is considered a minimally invasive surgery with great invasiveness. 3 A total of 35–65% of patients complain about moderate to severe pain after transaxillary endoscopic thyroidectomy. 4 Endoscopic thyroidectomy involves considerable subcutaneous mobilisation in the chest and parasternal areas and requires carbon dioxide inflation of the operative space, 5 therefore, patients expectedly experience pain after trans-areolar endoscopic thyroidectomy. Opioid use remains the mainstay of postoperative analgesia, but studies have shown that perioperative opioid use is associated with social abuse, 6 which indicates the importance of reducing perioperative opioid use. At present, multimodal analgesia based on nerve block is being widely investigated and has produced encouraging clinical results. 7 The invasive sites of breast endoscopic thyroidectomy are mainly the anterior chest wall and neck, which are innervated by the intercostal and cervical plexus nerves, respectively. For the analgesia used in this type of surgery, limited studies have been conducted on analgesia in the neck region, whereas no relevant reports have been regarding analgesia in the chest region. Flurbiprofen axetil injection combined with bilateral cervical plexus nerve block in thyroid carcinoma surgery as multimodal preemptive analgesia

Abstract:Background and purpose: Preemptive analgesia is one of the strategies to treat postoperative pain by preventing the establishment of central sensitization. This study was designed to explore whether the method of flurbiprofen axetil injection combined with bilateral cervical plexus nerve block in thyroid carcinoma surgery as multimodal preemptive analgesia can serve as a better analgesia. Methods: Sixty patients with thyroid carcinoma were randomly divided into three groups. Patients in Group A were treated with flurbiprofen axetil injection combined with bilateral cervical plexus nerve block as multimodal preemptive analgesia. Patients in Group B were anesthetized with bilateral cervical plexus nerve block. General anesthesia alone was used in patients of Group C. The onset time of nerve block, operation time, extubation time and dosage of fentanyl were recorded. The VAS (visual analogue scale) was used to evaluate the pain level, the side effects of drugs were also analyzed. Results: The onset time of nerve block in Group A, Group B were (7.47±1.04) min and (8.75±1.36) min, repectively (P<0.05). The dosage of fentanyl n Group A, B and C were (0.36±0.04) mg, (0.40±0.06) mg and (0.45±0.07) mg, respectively (Group A vs Group B P<0.05; Group A vs Group C, P<0.01).VAS scores of patients in Group C were higher than both Group A and B at 4,8 h after operation. Moreover, patients in Group B got higher VAS scores than that of Group A at 8 h. The side effects of both Group A and B were much less serious than that of Group C. Conclusion: Flurbiprofen axetil injection combined with bilateral cervical plexus nerve block as multimodal preemptive analgesia during thyroid carcinoma surgery can supply better analgesia and opioid-sparing effects, with less side effects.

Abstract:We report the first case of using an anterior scalene plane block at the superior trunk level achieving phrenic nerve blockade to treat intolerable referred shoulder pain after liver Radiofrequency Ablation (RFA) of a diaphragm-abutting liver tumor despite prevention with a full-dose non-steroidal anti-inflammatory drug. The anterior scalene plane block rapidly alleviated pain without significant complications.

Abstract:Background Thermage is a monopolar radiofrequency (RF). It has become an indispensable part of facial and body youthful methods. Although the current device is constantly improving in epidermal cooling techniques and even automatically measures the local impedance value, applying surface anesthesia can take some of the pain away caused by thermage, and the patient's severe pain in the jaw and neck areas is still difficult to resolve. Methods The author describes how he uses the combination of the buccal nerve block (BNB) and the superficial cervical plexus block (SCPB) to improve the comfort of the patient's face and neck treatment of patients. It can improve the quality, elasticity, and texture of the skin. Results According to the author's knowledge of oral and maxillofacial surgery, the combination of BNB and SCPB is applied to the neck and facial analgesia. Combining the BNB and SCPB effectively provides facial and neck anesthesia. The BNB in this technique provides pain relief to the facial skin in the mandibular area and SCPB offers pain relief to the skin of the neck. RF facial rejuvenation treatment often involves the junction of the face and neck to improve the contour of the mandibular margin. Therefore, Whether the RF treatment is aimed at the face or neck, or the treatment is performed simultaneously, we advocate the block both of the buccal nerve and the superficial cervical plexus nerves to achieve perfect analgesia. Still, the dosage of anesthetic medication for the nerve block can be adjusted according to the treatment area. Conclusion We applied these two nerve blocks and their combination to improve skin laxity with RF therapy for the first time.

Modification of superficial cervical plexus block anesthesia in clinic

Abstract:Objective To investigate the clinical feasibility and safety of modified bilateral superficial cervical plexus nerve block anesthesia in clinic.Methods Eighty American Society of Anesthesiologists(ASA) Ⅰ or Ⅱ patients with thyroid mass were randomly allocated to experimental group and control group(n=40 each).Experimental group was treated by modified bilateral superficial cervical plexus block,where the local anesthetic drug was directly injected in middle point of rear edge of sternocleidomastoid muscle by subcutaneous injection.Control group received cervical plexus block anesthesia in C4 transverse process(one-point method).The two groups were all blocked with 0.25% ropivacaine.The efficacy,muscle strength of neck and complications of cervical plexus block were assessed by anesthetist and surgeon.The blood pressure(BP),heart rate(HR),and pulse oxygen saturation(SpO2) were monitored before block anesthesia(T0),10 min after block anesthesia(T1),incision of skin(T2),operation(T3),and after operation(T4).Results Compared with T0 point,the systolic blood pressure(SBP) and diastolic blood pressure(DBP) were significantly increased at T2,T3 points in the control group(all P<0.05);HR was significantly increased at T1,T2,and T3 points in the control group(all P<0.05);SBP,DBP were significantly increased at T3 point and HR was significantly increased at T2,T3 points in the experimental group(all P<0.05).Compared with the control group,SBP and DBP were significantly lower in the experimental group at T2,T3 points.The anesthetic efficacy was 95%(38/40) in the experimental group,which was significantly higher than that in the control group(80%,P<0.05).There was no significant difference in SpO2 between the two groups(P>0.05).The head and neck of all patients could move freely and no patients showed serious complications after nerve block.Conclusion Bilateral superficial cervical plexus nerve block anesthesia under the skin of neck is a satisfactory;it has the advantages of being accurate,easy-to-perform,safe,effective,and cheap,with less complications;it is worth popularizing in clinic.

Abstract:OBJECTIVE: To evaluate the efficacy and safety of percutaneous ultrasound-guided thermal ablation in the treatment of cervical metastatic lymph nodes (LNs) from papillary thyroid carcinoma (PTC). MATERIALS AND METHODS: A total of 77 patients with 79 LNs ablated by microwave ablation (MWA) or radiofrequency ablation (RFA) between September 2018 and October 2022 were enrolled in this study. After treatment, patients were followed up with measurement of diameters of LNs and serum thyroglobulin (s-Tg) at 1, 3, 6, and 12 months and annually thereafter. The paired t-test was used to compare the changes of s-Tg level, diameters of LNs before and after ablation. RESULTS: There were no serious complications related to ablation while one case of incomplete ablation in MWA was found during follow-up. The mean longest and shortest diameter of the ablated LNs reduced from 11.6 ± 4.3 mm to 5.0 ± 4.1 mm (p < 0.001), and from 6.1 ± 1.9 mm to 3.0 ± 2.5 mm (p < 0.001) at the last follow-up visit. Besides, the final volume reduction rate (VRR) was 61.8 ± 56.4% (range, –67.0 –100%). The complete disappearance rate was 46.8%, but there were 4 (5.1%) LNs becoming bigger than before. The average s-Tg level was 9.2 ± 26.6 ng/mL, a data significantly decreased to 3.7 ± 7.0 ng/mL at the last follow-up, but no statistical difference was shown. CONCLUSION: Thermal ablation is an effective and safe modality for the treatment of metastatic LNs from PTC.

Abstract:PurposeThe aim of this study was to assess the effectiveness and safety of ultrasound guided percutaneous radiofrequency ablation (RFA) of cervical metastatic lymph nodes (LNs) from papillary thyroid carcinoma.Methods54 metastatic LNs confirmed by percutaneous biopsy in 33 patients with previous total thyroidectomy and radioiodine therapy were enrolled in this retrospective study. US and contrast-enhanced ultrasound (CEUS) examinations were performed before ablation. Follow-up consisted of conventional US, CEUS, thyroglobulin (Tg) level at 1, 3, 6, and 12 months and every 6 months thereafter. In 3 months after ablation, US-guided core needle biopsy (CNB) was performed in the center, at the edge of the ablation area to exclude recurrence.ResultsTechnical success was obtained in all 54 lymph nodes (100%) without immediate or later major complications occurred. With a mean follow-up of 21 ± 4 months (range 12–24 months), there were no evidence of recurrence at ablated sites. After RFA, 33 metastatic LNs completely disappeared (33/54, 61.1%) and 21 metastatic lymph nodes remained as small scarlike lesions (21/54, 38.9%) at the last follow-up visit. The mean volume reduction ratio (VRR) was 32.7 ± 8.6% (range 21.2–59.3%), 46.8 ± 9.7% (range 33.6–68.1%), 62.5 ± 12.1% (range 42.5–95.4%), 77.1 ± 10.6% (range 54.3–100.0%), 89.2 ± 8.3% (range 68.7–100.0%) and 94.9 ± 5.3% (range 78.2–100.0%) at 1, 3, 6, 12, 18 and 24 months after RFA respectively. Significant differences in the VRR were found between every two follow-up visits (P < 0.001). At the last follow-up visit, the mean serum Tg level decreased from 10.2 ± 5.1 ng/ml (range 0.8–16.2 ng/ml) to 1.1 ± 0.8 ng/ml (range 0.2–3.1 ng/ml) (P < 0.001).ConclusionsUltrasound guided percutaneous RFA for cervical metastatic LNs from papillary thyroid carcinoma is a feasible, effective and safe therapy. This procedure shows a nonsurgical therapeutic option that can eradicate the lesions with a very low complication rate.

Abstract:Study objective: To determine the safety and effectiveness of ultrasound-guided paracervical nerve blocks for the painless treatment of patients with cervical cancer post-implantation. Design: Single-center randomized controlled trial. Setting: Fourth Hospital of Hebei Medical University (July 2023 to October 2023). Trial number: ChiCTR2300071580 [https://clin.larvol.com/trial-detail/ChiCTR2300071580]. Patients: Eighty patients with cervical cancer underwent post-implantation treatment. Interventions: Patients receiving and not receiving paracervical nerve blocks (Groups T and C, respectively) were randomly allocated. Measurements: Primary measures included visual analog scale (VAS) scores and patient body movement scores at various stages, including vaginal speculum placement (T1), applicator/needle insertion (T2), treatment administration following connection of the treatment tube (T3), needle withdrawal and hemostasis (T4), and willingness to receive further treatment. The secondary observation indices in this study included the operation time, incidence of hypoxemia, occurrence of nausea and vomiting, adverse events related to the circulatory system, patient satisfaction score, operator satisfaction score, and operation duration required by patients with an Alderte score of ≥ 9. Main results: Forty patients each were randomly allocated into Groups T and C. The VAS scores did not differ significantly between the two groups at T1. However, at T3 and T4, the VAS scores of Group T was significantly lower than that of Group C. No significant difference was observed in the body movement scores between the two groups at T1 and T3. However, the body movement score of Group T was significantly lower than that of Group C (P < 0.001) at T2 and T4. Group T showed higher postoperative satisfaction and willingness to receive further treatment compared to that of Group C. Conclusions: Ultrasound-guided paracervical nerve block effectively reduced the pain scores in patients with cervical cancer undergoing post-implantation treatment and enhanced their inclination to undergo further treatment.

Abstract:Objective:To evaluate the safety and efficacy of neurolytic celiac plexus bloek(NCBP)in controlling pain due to upper abdominal malignancies.Methods:11 patients with advanced malignancies accepted NCBP.All procedures were performed via pre-crus of diaphragm approach under CT guidance.Results:The technical success was 100% in 11 cases.1 week after the procedure,CR(complete relief),PR(partial relief),and MR(moderate relief)were achieved in 7,3 and 1 patients respec tively,producing the effective rate of 90.9%.2 months after the procedure.CR,PR,and MR were achieved in 5,3 and 3 pa tients respectively,producing the effective rate of 72.7%.Postural hypotension occurred in 3 patients,and diarrhea occurred in 3 patients.Conclusion:NCBP is a safe and effective treatment in controlling pain due to upper abdominal malignancies.

Abstract:Percutaneous image guided thermal ablation has become a cornerstone of therapy for patients with oligometastatic disease and primary liver malignancies. Evolving from percutaneous ethanol injection (PEI), thermal ablation utilizing radiofrequency ablation (RFA) and microwave ablation (MWA) have become the standard approach in the treatment of isolated lesions that fit within the size criteria for curative intent therapy (typically 3-4cm). With the evolution of more intense thermal ablation, such as MWA, the dramatic increase in both the size of ablation zone and intensity of heat generation have extended the limits of this technique. As a result of these innovations, intra-procedural and post-procedural pain have also significantly increased, requiring either higher levels of intravenous sedation or, in some institutions, general anesthesia. In addition to the increase in therapeutic intensity, the use of intravenous sedation during aggressive ablation procedures carries the risk of over-sedation when the noxious insult (i.e. the ablation) is removed, adding further difficulty to post-procedural recovery and management. Furthermore, high subdiaphragmatic lesions become challenging in this setting due to issues relating to sedation and compliance with breath hold/breathing instructions. Although general anesthesia may mitigate these complications, the added resources associated with providing general anesthesia during ablation is not cost effective and may result in substantial delays in treatment. The reduction of Aerosol Generating Medical Procedures (AGMP), such as intubation due to the COVID-19 Pandemic, must also be taken into consideration. Due to the potential increased risk of infection transmission, alternatives to general anesthesia should be considered when safe and possible. Upper abdominal regional nerve block techniques have been used to manage pain related to trauma, surgery, and cancer; however, blocks of this nature are not well described in the interventional radiology literature. The McGill University group has developed experience in using such blocks as splanchnic, celiac and hepatic hilar nerve blocks to provide peri-procedural pain control [1]. Since incorporating these techniques (along with hydrodissection with tumescent anesthesia), we have also observed in our high volume ablation center a dramatic decrease in the amount of sedatives administered during the procedure, a decrease in patient discomfort during localization and ablation, as well as decreased pain post-procedure. Faster time to discharge and overall reduction in room procedural time serve as added benefits. The purpose of this publication is to outline and illustrate the practical application and use of nerve block/regional anesthesia techniques with respect to percutaneous hepatic thermal ablation.
Clinical application of ultrasound in thoracic medial branch block and thoracic cooled-radiofrequency ablation: Case report and literature review

Abstract:Ultrasound, Ahead of Print. Introduction:Radiofrequency ablation is a procedure used to alleviate pain by destroying nerves with by radiofrequency-generated heat. Traditionally, radiofrequency ablation is preceded by diagnostic medial branch block injections, both guided by fluoroscopy. Fluoroscopic visualization of the superolateral aspect of the thoracic transverse process, where thoracic medial branch nerves occur, can be challenging due to anatomical complexities, especially in obese patients. We present a novel technique in which ultrasound was utilized in conjunction with fluoroscopy to perform medial branch block and radiofrequency ablation of the thoracic medial branch nerves.Case Report:First, two diagnostic thoracic medial branch nerve blocks were performed under ultrasound guidance. For the subsequent radiofrequency ablation, spinal needles were first advanced under ultrasound guidance to the target thoracic medial branch nerves. The position of those spinal needles was then used to guide the placement of cooled radiofrequency ablation probes using fluoroscopy. The patient reported 100% pain relief following the procedures.Discussion:We found that the addition of ultrasound allowed us to overcome the challenge of visualizing the superolateral aspect of thoracic transverse process under fluoroscopy alone. Direct ultrasound visualization allowed us to accurately and safely perform a thoracic medial branch block and radiofrequency ablation in a patient with poor fluoroscopic anatomy, as demonstrated by the patient's complete pain relief after both medial branch block and radiofrequency ablation. We also theorize that our novel technique allows the provider to directly visualize the pleura, which could reduce the risk of severe pneumothorax associated with thoracic medial branch block and cooled radiofrequency ablation.
Efficacy and Safety of Ultrasound-guided Percutaneous Radiofrequency Ablation in the Treatment of Benign Thyroid Nodules

Abstract:Objective To explore the efficacy and safety of ultrasound-guided percutaneous radiofrequency ablation in the treatment of benign thyroid nodules, and to provide evidence for related treatment. Methods A total of 258 patients with benign thyroid nodules treated in our hospital between April 2014 and April 2017 were selected as subjects, who underwent local anes-thesia and ultrasound-guided percutaneous radiofrequency ablation. The subjective symptoms ( pain, dysphagia and foreign body sensation of the neck) were evaluated with the visual analogue scale (0-10 cm) before surgery and in three different months after surgery. The World Health Organization classification system was used for scoring thyroid aesthetics of the neck. In the mean-time, the changes of nodule volumes before and after operation were compared. Results The pain score, dysphagia score, foreign body sensation score of the neck were decreased after surgery and the difference was of statistical significance (P<0. 05). The volume of thyroid nodules was significantly reduced at 1, 2 and 4 months after operation (P=0. 007). There were no significant complications in any patient after 4 months postoperatively. Conclusion Ultrasound-guided percutaneous radiofrequency ablation of benign thyroid nodules can effectivley reduce the nodule volume and relieve pain, dysphagia, foreign body sensation of the neck and other symptoms. It is also quite safe.
Outcomes of cooled radiofrequency ablation of cervical nerves for the treatment of chronic pain

Abstract:Background: Chronic pain, defined as pain persisting for more than 3-6 months, has a mean one-year prevalence in the United States of 25.8% and is one of the most frequent reasons adults seek medical care. Treatment options include physical therapy, analgesics, anticonvulsants, exercise, and muscle relaxants. Even with conventional treatment modalities, in a subset of patients, pain may persist. Cooled radiofrequency ablation (c-RFA), a minimally invasive therapy, employs thermal energy generated by electrical currents to disrupt the transmission of pain stimuli along nociceptive pathways. This leads to an attenuation of pain impulses, primarily through nerve tissue necrosis. The potential of c-RFA to alleviate chronic pain for patients who struggle to find relief elsewhere accentuates the importance of rigorously investigating its outcomes. This study investigates whether patients receiving c-RFA for relief of chronic neck pain caused by cervical facet joint arthropathy experience a reduction in pain scores, the length of this reduction in pain scores, and the magnitude of this reduction in pain. Methods: This study was a retrospective analysis of data extracted from UW-Health Electronic Medical Health records (EMR), encompassing cervical c-RFA procedures performed from 2015 through 2022. Patient data were obtained, including diagnosis, pre-operative pain score, post-operative pain score, duration of relief, patient age, sex, and BMI. A two-tailed paired t-test was used to statistically analyze the pre-operative and post-operative pain scores, in which a p-value ≤0.05 was considered significant. Results: A total of 450 cervical c-RFA procedures were reviewed, 152 were excluded due to absent pre- or post-op pain scores. 298 were included in the analysis, comprising 203 unique patients: 129 females and 74 males with an average age of 56.03 ± 12.76 and a BMI of 28.76 ± 6.05. Improvement of pain symptoms was reported in 85.23% (n = 263), 6% (n = 19) reported complete pain remission, 8.72% (n = 26) reported no change, and 3.02% (n = 9) reported worsening symptoms. Patients reported an average pre-operative pain score of 6.15 (M = 6.15, SD = 1.55) and an average post-operative pain score of 3.64 (M = 3.64, SD = 2.09) this achieved significance (p < 0.0001). Of the 85.23% (n = 263) charts that noted improvement, there is an average of 48.04% ± 26.53 reduction from their pre-operative pain scores. The average duration of relief lasted 6.67 ± 4.84 months. Conclusion: This study supports the potential efficacy of c-RFA as a minimally invasive treatment for chronic neck pain secondary to cervical facet joint arthropathy refractory to conventional treatment measures, demonstrating significant relief for a substantial length of time. Due to chronic pain's detrimental effect on one's quality of life, finding effective treatment options is essential, especially for those refractory to conventional treatments.
Ultrasound-Guided Extraforaminal Thoracic Nerve Root Block Through the Midpoint of the Inferior Articular Process and the Parietal Pleura: A Clinical Application of Thoracic Paravertebral Nerve Block.

Abstract:Purpose: Thoracic nerve root (TNR) block is performed primarily under computed tomography or X-ray fluoroscopy but is associated with radiation exposure. Ultrasound requires no radiation and distinguishes vessels, nerves, pleura, and other tissues. Few reports of ultrasound-guided TNR (US-TNR) block have been described, and the puncture end point has not been clearly defined. Herein, we evaluated the feasibility of US-TNR block using the midpoint of the inferior articular process (IAP) and parietal pleura (PP) as the puncture end point. Patients and Methods: A prospective series of 10 patients with Herpes Zoster-associated pain underwent US-TNR-guided block performed using an in-plane technique with the midpoint of thoracic IAP and PP as the puncture end points of ultrasonography. The US-TNR block procedure was performed with ultrasound as the primary imaging tool followed by fluoroscopic confirmation. Results: In all patients, the needle tips were visible at the lateral margin of the pedicle in the anteroposterior view and at the extraforaminal zone in the lateral view. The TNR and dorsal root ganglion (DRG) were delineated in all 10 patients. Furthermore, 2 mL of radiopaque agent could delineate the epidural space in 8 patients and the thoracic paravertebral (TPV) space in the other 2 patients. All patients developed numbness along the corresponding dermatome 30 min after injection of local anesthetics. The numeric rating scale (NRS) score at baseline, and at two-and four-week follow-ups were 6.50 +/- 1.35, 3.50 +/- 0.85 (vs NRS at baseline, P < 0.01), and 4.00 +/- 0.82 (vs NRS at baseline, P < 0.01), respectively. Conclusion: This study demonstrated the feasibility of US-TNR block using the in-plane technique with the midpoint of thoracic IAP and PP as the puncture end point to effectively block the TNR and DRG. This technique is an accurate clinical application of TPV nerve block and provides a potential therapeutic option for the treatment of neuropathic pain.

Abstract:Background Postherpetic neuralgia (PHN) seriously affects a patient's quality of life, and it is urgent to find a method that can effectively alleviate the PHN of the upper extremity. Objective To observe the Efficacy of pulsed radiofrequency (PRF) to cervical nerve root for PHN in upper extremity under CT guidance. Study Design Retrospective comparative study. Setting Shengjing Hospital of China Medical University. Methods Fifty patients with PHN in upper extremity were enrolled in Pain Management. Patients were randomized into two groups: cervical nerve root block (A group, n = 25) and cervical nerve root PRF (B group, n = 25). At each observation time, the general characteristics, visual analog scale (VAS), quality of life scores assessment (SF-36), the total efficacy rate, dosage of antiepileptic and narcotic analgesics, and the incidence of complications were followed up. Results Compared with the preoperative, the postoperative VAS decreased, the physical component summary (PCS) and the mental component summary (MCS) increased in both groups (P < 0.05). The differences between group B and group A were statistically significant after 1 month, which could be maintained for 1 year (P < 0.05). The total efficacy rate of group A and group B was 52.0% and 80.0% at 1 Year, respectively. The total efficacy rate of group B was higher than that of group A (P < 0.05). The dosage of antiepileptic and narcotic analgesics in group B decreased significantly, and the decline was significant compared with group A (P < 0.05). The incidence of complications between the two groups were similar (P > 0.05). Conclusion CT-guided PRF to cervical nerve root for the treatment of PHN in the upper extremity is safe and effective. PRF can replicate the location of pain, precise positioning, reduce trauma, and increased pain relief rate.

Abstract:The sensory innervation of the clavicle is complex, and the best regional block technology for clavicular surgery has yet to be determined. The purpose of this study was to compare the application of ultrasound-guided superficial cervical plexus block combined with clavipectoral fascial plane block verses interscalene brachial plexus block in clavicular surgery. Fifty patients undergoing internal fixation surgery for elective clavicle fractures were randomly divided into two groups (n = 25 for each group): group I and group II. Superficial cervical plexus block combined with clavipectoral fascial plane block was used in group I, and superficial cervical plexus block combined with interscalene brachial plexus block was used in group II. The primary outcome measure was the duration of analgesia. The duration of analgesia in group I was significantly longer than that in group II (P < 0.05). The modified Bromage scale function score in group II was lower than that in group I (P < 0.01). There was no significant difference in the skin acupuncture pain score 30 min after block and visual analog scale (VAS) scores at 6 and 12 h after surgery. However, the 24 h VAS score in group I was lower than that in group II (P < 0.05). The incidence of diaphragmatic paralysis was significantly increased in group II (P < 0.01). Ultrasound-guided superficial cervical plexus block combined with clavipectoral fascial plane block can be used for clavicular surgery. It has a long postoperative analgesia time, can retain the motor function of the involved upper limb and does not cause hemidiaphragmatic paresis.Clinical trial number and registry URL: Clinical Trials.gov; Trial registration number: ChiCTR2000039383; Date of registration: 25 October 2020.
Intraoperative Radiofrequency Ablation for Contralateral Benign Nodules in Unilateral Thyroid Cancer Patients to Relieve Anxiety

Abstract:Introduction: For unilateral papillary thyroid carcinoma (PTC) with contralateral benign nodules, optimal extent of surgery remains controversial. This retrospective cohort study was performed to evaluate the life quality of patients who underwent lobectomy alone and lobectomy with radiofrequency ablation (RFA). Methods: From October 2014 to October 2018, unilateral PTC patients with contralateral benign nodules reported by fine-needle aspiration cytology who encountered anxiety about contralateral nodule progression underwent lobectomy for PTC and intraoperative RFA for contralateral nodules. The patients who underwent thyroid lobectomy were matched for sex, age at time of surgery, number, size, and location of primary tumors and contralateral nodules to the patients who underwent lobectomy with intraoperative RFA. Three questionnaires were used to evaluate life quality in the two groups. The complications and rate of patients who were not required to receive thyroid-stimulating hormone suppression therapy were recorded. Results: One hundred forty-eight patients with 194 contralateral nodules underwent RFA in the lobectomy plus RFA group, and age- and sex-matched patients underwent thyroid lobectomy alone. The mean volume reduction ratio was 67.7% at 12 mo and 95.2% at 24 mo. After a median follow-up of 4.2 y, nine patients (6.1%) in the lobectomy plus RFA group and 17 (11.5%) in the thyroid lobectomy-alone group underwent completion thyroidectomy (P = 0.100). Patients who underwent lobectomy plus RFA had a better quality of life in terms of anxiety, physiological health, social and family aspects, and psychological and sensory features that were measured cross-sectionally at 6 mo using three instruments. Conclusions: Intraoperative RFA is effective in terms of volume reduction of contralateral nodules and improved quality of life for unilateral PTC patients with anxiety about disease progression.
