Abstract:Aims: To describe how people diagnosed with chronic kidney disease and their family members describe uncertainty related to impending death. Background: There has been little research about the experiences of people with chronic kidney disease and their family members as they near the end-of-life. We need to understand these experiences to give holistic person-centred care. Design: A narrative enquiry was undertaken using a social constructionist perspective. Methods: Data were collected in two in-depth interviews, approximately 3-4 months apart, followed by a telephone follow-up 2-3 months later. Results: Eleven people with advanced chronic kidney disease and nine family members/significant others participated. For both participants in the dyad, a number of key themes/storylines were identified. These included recognition that: @life has a boundary, living on the edge, I'm not afraid to die but… and remembering loss and death experiences@. Conclusions: This enquiry illuminated four key storylines of about death and dying with advanced chronic kidney disease for people with the illness and their family members. Discussion about past experiences with illness, loss, death, and dying can give an entry for discussion about end-of-life. The person with illness and family members often held very similar perceptions and desires, but healthcare professionals' contributions to understanding of the experience were not mentioned. Implications for clinicians include attending to peoples' awareness of death as a distinct opportunity, listening attentively and posing questions that genuinely invite concerns and wishes about end-of-life and living well.

Abstract:This study examines the reasons of people with dementia request euthanasia and how these reasons change over time with a special focus on reasons related to family. In addition, it examines how family relates to their loved one's euthanasia wish. Seven people with dementia and a euthanasia wish in the Netherlands were interviewed, and three years later, two of them and five family members were interviewed again. Four themes were identified using thematic analyses: (1) Protecting the relationship from the impact of dementia; (2) Private domain as the primary setting to discuss euthanasia; (3) Implicit expectation to respect and agree with the euthanasia wish; (4) Experienced responsibilities of family regarding the euthanasia wish. Professionals should be aware of the position of family and the interrelatedness of the person's concern for family happiness and families' moral commitment to agree and support the wish.

Abstract:The diary of the writer Annie Ernaux, Je ne suis pas sortie de ma nuit (I did not get out of my night) relates the author experience of accompanying her mother, suffering from Alzheimer's disease. This journal is analysed to understand what allows the writer to come closer to her mother, despite the disease progression. How does manage Annie Ernaux to exchange with her mother till the end of the dementia process? The aim of this paper is to understand, for Annie Ernaux first, then for other family carers, how to avoid reducing the patient to be a "burden" to be cared for. On the contrary, how to find a meaning in maintaining a relationship with demented subjects. Two linked carer's challenges are put forward, allowing to keep alive a quality relationship with the demented subject: 1) to accept, not only to give but to receive from him his experience of humanity in dependence, old age, and the proximity of death; this experience, when received, makes it possible to extend the carer's vision of humanity, and of his or her own humanity; 2) to fully recognise the demented as a human being, which supposes to leave the cartesian concept of man, where Reason is the cardinal faculty; another anthropology is needed, where the body and our incarnation are central, a body which is the mean for opening onto the world, communicating, and understanding each other. When facing this challenges, the carer will be able to exchange with the demented subject, an exchange which is certainly difficult, but which makes life more interesting, and better for him and the patient.

Abstract:An adolescent's last moment of life is an emotionally and medically complex time. Children may grapple with understanding the things happening to them and with grief of a future lost; caregivers struggle to simultaneously balance deep sorrow, hope, and love; and healthcare providers fight to maintain sound medical and ethical decision making. Increased discussion regarding adolescent end-of-life care is needed so that clinicians may better understand how to engage in ethically based medical management during these events. This holds particularly true in situations where potentially conflicting ideas exist between clinicians and family members. We describe the case of an acutely and terminally ill adolescent who remained cognitively intact but with rapidly advancing multiple organ failure and whose parents requested that he remain uninformed of his critical illness and prognosis.

Abstract:This article explores the role of end-of-life conversations between palliative home-care patients and their relatives in shaping the bereavement process. Through interviews with 17 bereaved relatives, four distinct modes of contingency were identified: denial, acknowledgment, accepting, and receiving. While some individuals remained in a single mode, others transitioned through all four, suggesting a deeply personal process independent of external influences. The findings underscore the importance of healthcare providers facilitating these conversations, as they can help relatives process emotions and support their grieving. By addressing these modes, caregivers can aid relatives in becoming compassionate advocates for patients nearing death. The study highlights the need for further research into strategies for supporting relatives in navigating these modes, potentially enhancing bereavement care practices.

Abstract:Little is known about end-of-life care for individuals with Alzheimer's disease and other dementias. Four case studies are presented, using data collected by qualitative interviews conducted with family caregivers who were closely involved with end-of-life care for relatives with dementia. The case studies are formatted in two pairs, with one reflecting two deaths occurring at home and the other pair representing two deaths in the nursing home. The cases reveal a range of end-of-life experiences, suggesting that there is not just one "good" path. The extent of care needed, the responsiveness of the individual, the health of the caregiver(s), and the residence and support situations, can all intersect in a variety of ways that make no one scenario the answer for all. Although most people say they would prefer to die at home, in some situations the nursing home can be a satisfactory choice, particularly if hospice is involved. These narrative case studies give the reader insight into the variety of the end-of-life experiences and suggest the environment should be considered as part of the care provision.

Abstract:Background: Little attention has been paid to the ways in which nurses personally experience, understand and assign meaning to providing palliative care. Aim: A qualitative study of four nurses working with patients in the terminal phase in a hospital in Mexico was conducted to understand their lived professional experiences and relationships with death. Methods: Four interviews were analysed using the Greimasian actantial-semiotic model. Actants were categorised by narrative role and their actions were analysed. The grammatical features of the narration were also examined. Findings: Nurses sought a good death for the patient, which they typically achieved, and spiritual peace for themselves, which they often did not. Nurses placed a high value on personal, social and professional recognition for their work. The philosophical themes affecting nursing as a vocation that emerged included life and death, truth and honesty and the role of God and the family. These professional values were often contradictory, and these dilemmas should be addressed in professional training and support.

Abstract:Aim: To explore and develop understanding of nursing home staff's emotional experiences of being in a close relationship with a resident in long-term care who later died. Design: Ethnographic fieldwork. Methods: As part of fieldwork, narrative interviews were conducted with nursing home staff ( n = 6) in two nursing homes in Norway and analyzed using interpretative phenomenological analysis. Findings: Through data analysis, we identified three superordinate themes: (1) wanting to be something good for the resident and their families, (2) striving to make sense of the resident's death, and (3) struggling to balance being personal and professional. Implications for holistic nursing and conclusion: Nursing home staff experience tensions between ideals of distanced professionalism and the emotional experience of proximity, evidenced by personal commitment and mutual recognition in relationships with “special residents” in long-term care. To support holistic practice, awareness is needed of the emotional impact of relationships on health professionals. Suppressing feelings puts staff at risk of moral distress, compassion fatigue, and burnout, as well as higher turnover and absenteeism. Managers should facilitate discussions on professionals’ ideals of relationship-based practice, including processing of, and reflection on, emotional experiences in long-term care. Rituals to mark a resident's death can provide further emotional containment.

Abstract:Was this a silver-lining playbook-my mother's death? We had planned for this moment, talked together about her wishes, filled out the paperwork: We were that kind of family-the one that talked and debated about life and death. And my mother, in her true tenacious fashion, rehearsed with us her dying wishes. The papers were filed with her primary care physician, scanned into the Electronic Medical Record (EMR), and a copy placed in their freezer-a clever way to find it in a crisis. The playbook was in place. The rest of the story is on replay in my mind. I remember asking to speak to the physician in a demanding tone and wanting to know why my mom was intubated . . . "Didn't any one look at the paper work?" The Emergency Department (ED) physician explained, "Your mom's blood pressure is dropping. She doesn't have a complicated medical condition so we want to give her pressors so that we can keep her alive." I replied that neither my mom nor the family wanted any intervention. This experience pulled us into the power of the protocols that make it possible to keep hearts beating and lungs breathing. In my mother's case, her uncomplicated medical history in the EMR triggered those standing lifesaving orders. But no one assessed her personhood. No one asked about her level of functioning, her pain, her memory loss, her desires. No one took a minute to ask about this mother of five children who sang her way through life. The tension between overt talk and covert emotions in illness narratives: transition from clinician to researcher

Abstract:In this article I discuss my interview experiences as joint constructions with Finnish parents whose children had been diagnosed with cancer. In doing so both the interviewer and interviewees became engaged as anxious "defended subjects" in an intersubjectivity of acknowledging but unable to talk of "how it really is" to have a child diagnosed with cancer. This appeared as tensions between overt talk and covert emotions. In the overt talk the parents underlined the manageable aspects of the illness. The speechless and painful part of the illness experience appeared as covert emotions that the interviewer was able to recognise and contain. Only when both the overt talk and the covert emotions were taken into account did it open up a more comprehensive way to understand the depth of the illness suffering. This is illustrated with one interview as an example.

Abstract:This paper joins a discussion about the representational dissonance and commemorative ethics of two self-referential works that engage with Susan Sontag’s 2004 death from Myelodysplastic Syndrome: Annie Leibovitz’s A Photographer’s Life1990–2005 (2006) and David Rieff’s Swimming in a Sea of Death: A Son’s Memoir (2008). Instead of approaching these two texts as testimonial accounts measured by standards of reliability and grace, this paper considers how the temporal dissonance produced by an incurable cancer diagnosis thwarts questions of personhood and ethical intention in Leibovitz’s photography and Rieff ’s prose. By contextualizing these works as the caregivers’ experience of Sontag’s illness, this paper reads them as attempts at gauging two distinct temporal perspectives that confound identification—those of living through and of remembering terminal time.
