Abstract:To evaluate the clinical application value of the corneal cryopreservation in keratonosus.32 patients with keratonosus treated by corneal transplantation were clinically observed.In 32 keratonosus cases, 59.37% (19 cases) of the grafts remained transparant; in 24 corneal ulcer preforation cases, 45.83% (11 cases) of the grafts were transparent, all eyeballs(100%) were remained.Corneal cryopreservation can be a practical, effective method in the treatment of keratonosus in district hospitals.

Combined Penetrating Keratoplasty with Keratolimbal Allograft Transplantation in the Treatment of Severe Corneal Burns

Abstract:OBJECTIVE:To study the clinical results of combined penetrating keratoplasty with keratolimbal allograft transplantation in the treatment of severe corneal burns.METHODS:The present study consisted of 35 eyes (34 patients) with full thickness corneal opacities and extensive corneal conjunctivalization after severe corneal burns. The duration between the injury and the time of presentation ranged from four months to 23 years. Group A: Combined penetrating keratoplasty (PK) with annular keratolimbal allograft transplantation (KLAT) was performed in 19 eyes (18 patients). Group B: Total PK with a scleral rim was performed in 16 eyes (16 patients). Post-operative visual acuity, intraocular pressure, graft rejection and complications were monitored. The average follow up time was 24 (range from 15 to 28) months.RESULTS:(1) Vision: The number of eyes that obtained best corrected visual acuity > 0.05 was 18 eyes (95%) in group A and 14 eyes (84%) in group B at 6 months postoperatively (P = 0.582); 16 eyes (88%) in group A and 6 eyes (38%) in group B at 12 months (P = 0.006); 12 eyes (63%) in group A and 3 (7%) in group B at 24 months (P = 0.016). (2) Graft rejection: Nine allografts (47%) in group A and 8 grafts (50%) in group B developed limbal stem cell rejection (P = 1.000); 6 allografts (32%) in group A and 11 allografts (69%) in group B developed endothelium rejection (P = 0.044). The difference of endothelium rejection rate between group A and B was not significant at different times postoperatively. (3)COMPLICATIONS: Corneal epithelium defect occurred in 2 eyes (11%) in group A and 7 (44%) eyes in group B (P = 0.049); hyphema occurred in 2 eyes (11%) in group A and 8 eyes (50%) in group B (P = 0.022); hypotony occurred in 1 eye (5%) in group A and 6 eyes (38%) in group B (P = 0.032).CONCLUSIONS:PK combined with KLAT is an effective treatment for severe corneal burns at a late stage irrespective of the etiology. This procedure can restore visual function in severely damaged eyes. Patients treated with PK combined with KLAT show less complication and better prognosis as compared with patients treated with total PK with a scleral rim.
Short‐term Results of Acellular Porcine Corneal Stroma Keratoplasty for Herpes Simplex Keratitis

Abstract:Background Corneal transplantation is a common surgical intervention for restoring vision loss due to corneal damages. However, for cultural reasons, there is a huge shortage of donor corneas in China. Acellular porcine corneal stromas (APCSs) can be used as corneal substitutes in lamellar keratoplasty for corneal ulcers. This study was conducted to analyze the results of APCS use for herpes simplex keratitis (HSK). Methods The study involved HSK patients who underwent keratoplasty with APCSs from February 2016 to October 2017 in the second affiliated hospital of Zhejiang University. Patient data were collected at 7 days, 1 month, 3 months, 6 months, and at the last follow-up (7-25 months) postoperative. The corneal transparency, neovascularization, visual acuity, and graft stability were observed. Results Thirteen patients with HSK including five patients with corneal perforation were included in this study, nine patients underwent deep anterior lamellar keratoplasty (DALK) and five perforation patients underwent double lamellar keratoplasty. There were nine men and four women with an average age of 62.5 +/- 5.6 years old (ranging from 52 to 70 years old). The mean postoperative follow-up duration was 15.1 +/- 5.8 months (ranging from 7 to 25 months). At the last visit, visual acuity improved in nine patients (69.2%) compared with preoperative (P = 0.008).The grafts of seven individuals (53.8%) were completely transparent or slightly opaque; their corneal transparency score had improved significantly compared with before the surgery (P = 0.010). Various degrees of neovascularization were present in 11 of the 13 patients (84.6%), most neovascularization gradually stabilized. Graft dissolution occurred in three eyes (23.1%) during the observation period, two underwent regrafting, the other one became stable after treatment. Three patients underwent second allograft transplantation, two of which encountered APCS graft dissolution and one of the patients requested a human donor allograft transplantation due to transparency issues despite the absence of adverse issues. Conclusion Acellular porcine corneal stroma seems to be effective in the treatment of HSK and can be used in HSK with corneal perforation by using double lamellar keratoplasty in an emergency.
Therapeutic Penetrating Keratoplasty in Severe Fungal Keratitis Using Cryopreserved Donor Corneas

Abstract:AIMS:To investigate whether cryopreserved donor cornea could be used for therapeutic penetrating keratoplasty (PKP) to eradicate the infection, obviate complications, and preserve anatomical integrity in severe fungal keratitis.METHODS:In this retrospective, consecutive case series, 45 eyes of 45 patients with severe fungal keratitis, which exhibited anterior chamber collapse, corneal perforation, and/or large suppurative corneal infiltrate, received therapeutic PKP after removal of the infected corneal tissue, irrigation of the anterior chamber by 0.2% fluconazole solution, iris dissection of fibrinoid membrane, and iridectomy and therapeutic PKP using corneas cryopreserved at -20 degrees C.RESULTS:Among 45 eyes, 39 eyes (86.7%) were successfully eradicated the fungal infection without recurrence and maintained their anatomical integrity without any complication. Four of 45 eyes (8.9%) showed postoperative rise of intraocular pressure, of which three were controlled with subsequent antiglaucoma surgeries, whereas one eye needed additional antiglaucoma medications. Two of 45 eyes (4.4%) were enucleated because of uncontrollable fungal infection and secondary retinal detachment, respectively. 23 eyes received subsequent optical PKP and, among them, 21 maintained clear corneal grafts and two suffered from graft failure due to allograft rejections.CONCLUSION:Cryopreserved donor corneas are effective substitutes in therapeutic PKP to control severe fungal corneal infection and preserve the global integrity, and may offer additional advantages over conventional PKP in reducing allograft rejection, eradicating fungal infection during the postoperative period, and improving the success of optical PKP for visual rehabilitation.
Optical Penetrating Keratoplasty in Eyes with Severe Keratomycosis after Therapeutic Keratoplasty

Abstract:OBJECTIVE:To observe and evaluate secondary optical penetrating keratoplasty (PKP) in the eyes with severe keratomycosis after therapeutic keratoplasty using cryopreserved donor tissue.METHOD:There were 35 eyes in 35 cases for whom therapeutic PKP using cryopreserved donor tissue had been carried out to treat severe fungal keratitis. At least 6 months after the PKP, when the fungal infection had been eradicated and ocular inflammation had resolved, a secondary optical penetrating keratoplasty was further performed for visual rehabilitation. If the crystal lens was found to be opaque, it was also simultaneously removed through extracapsular cataract removal with an IOL implantation. Surgical or postoperative complications, visual outcome, graft rejection episode, and graft survival were analyzed.RESULTS:Among 35 eyes, 18 eyes received optical PKP only, and 17 received combined optical PKP with ECCE and IOL implantation. During the follow-up period of 6.8 - 36.8 (15.7 +/- 7.6) months after optical PKP, 3 eyes experienced graft failure due to immunologic rejection and 32 retained clear grafts. Postoperative visual acuity was 0.4 or better in 24 eyes, and 0.1 or better in 32 eyes, and less than 0.1 in 3 eyes.CONCLUSIONS:Optical PKP performed in the eyes after therapeutic PKP due to severe fungal keratitis can recover satisfactory visual acuity, with low frequency of graft rejection, long-term graft survival and few complications.

Abstract:Context. Penetrating keratoplasty (PK) is a recognized treatment for corneal damage but possesses inherent risks, mainly due to its open-sky nature, which potentially lead to severe sight-threatening complications. Protective penetrating keratoplasty (PPK) emerges as a novel procedure aimed at mitigating these risks. Aim. To assess the surgical safety, postoperative outcomes, and the impact of PPK on corneal endothelial cell density through a retrospective analysis of 22 cases. Settings and Design. A retrospective cross-sectional observational study was executed at Hermanos Ameijeiras Hospital from February 2018 to December 2021, involving 22 patients (22 eyes) who were unresponsive to other medical treatments and had a corrected distance visual acuity of ≤0.1. Methods and Materials. Patients underwent PPK, with surgical procedures and postoperative care documented. Statistical analysis was performed on qualitative and quantitative variables to evaluate the surgical outcomes and the corneal endothelial cell density changes postoperation. Results. All surgeries demonstrated a significant improvement in postoperative visual acuity ( p ≤ 0.001 ) and recorded a 9.2% decrease in the corneal endothelial cell density at 12 months. Noteworthy complications included one case of intraoperatively discovered haptic dislocation and one postoperative bacterial keratitis. Conclusions. PPK could potentially mitigate perioperative complications, ensure graft clarity, and reduce corneal endothelial cell loss, presenting itself as a viable alternative to traditional PK. Although the results are encouraging, larger-scale studies are essential to validate the benefits and applicability of PPK in broader clinical settings.

Individualized Penetrating Keratoplasty Using Edge-Trimmed Glycerol-Preserved Donor Corneas for Perforated Corneal Ulcers

Abstract:To report a surgical technique and the surgical outcomes of individualized penetrating keratoplasty (PK) using edge-trimmed glycerol-preserved donor corneas for perforated corneal ulcers. Fourteen perforated eyes from 14 patients who underwent individualized PK using edge-trimmed glycerol-preserved donor corneas, were included in the retrospective study. The perforations were mainly 1–2 mm in size except for one that was 2.5 × 4 mm. Three patients were treated with PK; one patient was treated with PK and a conjunctival flap; ten patients who had large ulcer areas were treated with PK combined with lamellar keratoplasty (LK). Donor corneas were preserved in sterile pure glycerol at − 80 °C. Corneal grafts were specially edge-trimmed to match the perforation, and then sutured onto the recipient bed avoiding the visual axis. All 14 patients recovered anatomical integrity without reinfections of the treated eyes. All patients had improved graft transparency and uncorrected visual acuity after surgery. Among them, four patients suffered from short-term postoperative complications and recovered quickly; four patients suffered from long-term postoperative complications, of them, one was performed further treatment. After individualized PK using glycerol-preserved donor corneas, all perforated corneal ulcers were stably controlled by the end of the follow-up period. This modified surgical technique can be a potential treatment choice for patients with perforated corneal ulcers.
The Clinical Study on Penetrating Keratoplasty in Children

Abstract:Purpose To investigate the efficacy of penetrating keratoplosty in children.Methods A retrospective study was performed on 29 eyes of successive pediatric cases with eorneal blindness that underwent penetrating keratoplasty. Results The visual acuity was more than 0.05 in 12(52.17%)of 23 eyes.Grafts remaines transparent in 19(65.52%) of 29 eyes.The grafts of all eyes with keratoconus and congenital opacities were transparent.Clear grafts were achieved in 5 of 6 eyes with scars from trauma.Allograft rejection was occurred in 4 of 6 eyes with congenital corneal opacity,in 2 of 3 eyes with infective corneal ulcer,in 1 of 2 eyes band shaped corneal dystrophy and in 2 eyes of Alkali chemical injury. Complications of anterior/posterior iris synechia,cataract and glaucoma happened in 8 of 29 eyes(27.6%).Conclusion The successful rates of pediatric keratoplasty were different due to different causes.Early surgical intervention,prevention of surgical complications and intensive amblyopia therapy may be important factors of penetrating keratoplasty in children.
Effects of Deep Lamellar Keratoplasty on Severe Necrotizing Stromal Keratitis

Abstract:Objective: This study is to investigate the application of deep lamellar keratoplasty (DLKP) in the treatment of necrotizing stromal keratitis (NSK).Methods: Corneal confocal microscopy and histopathological examination were performed before surgery.DLKP and penetrating keratoplasty (PKP) were performed on 14 and 20 NSK subjects, respectively, followed by proper drug administration.In the follow-up period, the pathological features were evaluated and compared.Results: Significant corneal neovascularization and inflammatory cell infiltration were observed in NSK patients before surgery.After proper drug treatments, PKP and DLKP were performed.In the followup period, the visual acuity was elevated in the PKP and DLKP groups at 1 y after surgery.Moreover, corneal opacity was observed in 2 corneal grafts in the PKP group, while all the corneal grafts were transparent in the DLKP group.Furthermore, compared with the PKP group, corneal endothelial cell numbers at 6 and 12 months after surgery were significantly higher in the DLKP group.On the other hand, there were 2 eyes reporting virus recurrence in the PKP group, while no virus recurrence was reported in the DLKP group.Moreover, 4 eyes were accompanied with neovascularization at the corneal graft edge in the PKP group, while there was only 1 eye associated with neovascularization in the DLKP group.Furthermore, 5 cases in the PKP group reported graft rejection, while no rejection case was reported for DLKP.Conclusion: DLKP provides superior outcomes to PKP in treating NSK, which might be an ideal alternative option for the disease treatment.

Abstract:Objective To observe the effects of penetrating keratoplasty for treatment of severe fungal corneal ulcer.Methods Penetrating keratoplasty was performed on 68 cases with severe fungal corneal ulcer using donor corneas preserved in pure glycerine.The post operative follow up ranged form 6 months to 5 years.Results In the total 68 cases,the infection was controlled in 67 cases after penetrating keratoplasty,the grafts was survived.In 4 cases the fungal ulcer recurred and then cured after medication and repeated surgery. The main complications were graft rejection,secondary glaucoma and bullous keratopathy.Conclusion To the severe fungal corneal ulcer unresponsive to the maximum medication,penetrating keratoplasty can effectively resolve infection,restore the integrity of the globe and provide support for the optical keratoplasty later.
Combined umbilical cord patching with amniotic membrane graft for corneal surface reconstruction

Abstract:Background: Umbilical cord patch (UCP) grafts have been successfully used for glaucoma shunt tube coverage and conjunctival surface reconstruction. In recent years, the technique has emerged as a novel alternative for the reconstruction of corneal perforation and descemetocele. This study aimed to evaluate the effectiveness of combined UCP grafting and human amniotic membrane (HAM) transplantation for the management of corneal perforation or descemetocele. Methods: This prospective, non-comparative, interventional case series included nine eyes of nine patients with corneal descemetoceles and 28 eyes of 28 patients with corneal perforations, all in a clinically quiescent state. UCP grafting and HAM transplantation were combined to treat all patients. We re-examined the patients daily throughout the first week, weekly for 1 month, and then monthly for the first 6 months using slit-lamp examination and anterior segment optical coherence tomography. Results: We included 37 eyes with descemetocele or corneal perforation in a clinically quiescent state. The mean (standard deviation) ages of patients with corneal descemetocele and corneal perforation were 56.3 (18.8) years and 54.3 (18.1) years, respectively. The male-to-female ratios in patients with corneal descemetocele and corneal perforation were 56% to 44% and 61% to 39%, respectively. Postoperative corneal thickness increased significantly in eyes with descemetocele compared to preoperative values (P < 0.001). Postoperative best-corrected distance visual acuity improved significantly compared to preoperative values in eyes with descemetocele or corneal perforation (both P < 0.001), with relief of accompanying ocular symptoms. We did not observe any recurrence or complications such as rejection, infection, suture-related problems, or severe inflammation and all had a formed anterior chamber up to the final follow-up visit. Conclusions: Combined UCP grafting and HAM transplantation could be a promising alternative treatment for corneal perforation or descemetocele in clinically quiescent eyes, providing satisfactory reconstruction and functional outcomes. Further studies with robust designs, larger sample sizes, and longer follow-up are needed to verify the efficacy and safety of this modified surgical technique in enhancing vision and restoring anterior segment anatomical integrity in compromised corneas.
Therapeutic Deep Anterior Lamellar Keratoplasty for Eccentric Corneal Perforations

Abstract:Objective To demonstrate the efficacy of deep anterior lamellar keratoplasty for the treatment of eccentric corneal perforations.Methods It was a retrospective case series study.Deep anterior lamellar keratoplasty (DLKP) was performed on 11 patients with infected or noninfected corneal perforations from December 2008 to January 2011.The corneal perforations were located between the pupillary edge and limbus with diameters ranging from 1.5 mm to 2.0 mm.Postoperative visual acuity,intraocular pressure, endothelial cell density, transparency rate, corneal neovascularization, graft rejection and ulceration recurrence were recorded.Results Patients were followed postoperatively for 12-24 months (mean 17.5±3.7 months).The majority (10 eyes) had excellent engomphosis between the graft and recipient bed.Graft opacity was observed in 1 patient with rheumatoid arthritis and drug allergy due to the formation of a double anterior chamber at the ulceration site. All other patients had clear corneas, apart from the area of the original perforation. The integrity of the globe was restored in all 11 patients with an improvement in visual acuity (0.04 in one,and 0.1 or better in ten) and normal intraocular pressure.The average endothelial cell density after DLKP was 2286±293 cells/mm2. Immune graft rejections, ulceration recurrence and corneal neovascularization were not observed in any patient during the follow-up period. Conclusion DLKP is a safe and effective therapeutic measure in the management of patients with small eccentric corneal perforations.
Retrospective Clinical Outcomes of Keratoplasty Using Human Donor Corneas Preserved in Eusol-C Hypothermic Storage Medium

Abstract:Objective: To evaluate the clinical outcomes of cornea transplantation (penetrating keratoplasty, Descemet membrane endothelial keratoplasty, Descemet stripping automated endothelial keratoplasty, and deep anterior lamellar keratoplasty) using donor corneas stored in Eusol-C hypothermic storage medium compared to corneas stored in organ-culture. Methods: The clinical outcomes of 92 patients who underwent corneal transplantation with human donor corneas stored in Eusol-C medium at 2–8 °C were retrospectively evaluated. The control group consisted of 169 patients who received corneas organ-cultured at 31 °C. Donor age, sex, death-to-preservation time, and storage time were recorded. Endothelial cell (EC) density (ECD), EC mortality, and EC morphology scores were evaluated during storage in both groups. Complication rates, visual outcomes, and corneal transparency were monitored for up to six months. Results: The mean storage in Eusol-C time was 7.7 ± 2.5 days, while organ-culture time was 14.2 ± 4.0 days. In the Eusol-C group, ECD was 2398 ± 354 cells/mm2, with an average EC morphology score of 3.4 ± 0.7/4. Approximately 28% of the corneas in the Eusol-C group had no EC mortality. In the organ-culture group, ECD was 2256 ± 328 cells/mm2, with an average EC morphology score of 3.5 ± 0.5/4, and 42% were devoid of EC mortality. No complications, such as re-bubbling, were observed in both groups during surgery. Transparent corneas were achieved in 81.3% of the Eusol-C group the day after surgery. Mean corrected distance visual acuity (CDVA) at 3 and 6 months was 4.5 ± 4.0/10 and 5.4 ± 3.7/10 for the Eusol-C group and 5.0 ± 2.9/10 and 5.7 ± 2.8/10 for the organ-culture group, with no statistical differences observed between the groups. No graft failure was observed up to three months. Graft rejection occurred in the Eusol-C group and in the organ-culture group in, respectively, one and two cases at the six-month follow-up. Conclusions: Comparable surgical outcomes were achieved with donor corneas stored in both hypothermic Eusol-C and organ-culture media.

Abstract:Purpose: To evaluate the outcome of excimer laser-assisted deep anterior lamellar keratoplasty (Exc-DALK) and penetrating keratoplasty (Exc-PKP) for keratoconus (KC). Methods: We included 123 eyes and compared the outcome of 30 eyes undergoing big-bubble Exc-DALK after stromal debulking (G1) against a matched group of 83 eyes undergoing Exc-PKP (G2) in our department. G2 was created after performing a pseudorandom stratified bootstrapping of our PKP database starting with 2802 surgeries with removal of any bias. Main outcomes included best corrected visual acuity (BCVA), spherical equivalent (SE), tomographic changes, and endothelial cell density (ECD). Data were compared preoperatively, postoperatively with 2 double-running cross-stitch sutures according to Hoffmann [2 and 10 months postoperatively (T1 and T2)], after first suture removal [15 months postoperatively (T3)], and after final suture removal [26 months postoperatively (T4)]. Results: Preoperatively, all parameters were comparable without significant differences. At T1, G1 showed significantly better BCVA (P = 0.01), lower SE (P = 0.04), lower anterior and posterior topographic astigmatism (ATA and PTA, P < 0.01), lower maximal keratometry (P = 0.02), and lower central corneal thickness (P < 0.01). At T2, G1 showed significantly better BCVA (P = 0.04), lower PTA (P < 0.01), and higher ECD (P < 0.01). At T3, G1 showed significantly lower ATA (P = 0.01) and PTA (P < 0.01) and higher ECD (P = 0.02). At T4, G1 showed significantly lower PTA and higher ECD (P < 0.01) but no significant difference in BCVA (P = 0.07). Conclusions: Exc-DALK provided faster visual rehabilitation and represented an endothelial cell-sparing method compared with Exc-PKP. After final suture removal, Exc-DALK showed noninferior outcomes compared with Exc-PKP if the graft was placed on naked Descemet membrane.

Abstract:In the past, a penetrating keratoplasty (PK) was the mainstay of treatment for corneal endothelial disease. It involves the replacement of full-thickness corneal host tissue by a full-thickness donor. The procedure is relatively simple compared to posterior lamellar procedures, but good visual outcomes are often limited. This is secondary to high or irregular astigmatism. Many months are required to achieve visual stability due to suture-dependent corneal curvature and astigmatism. It can be related to ocular surface and suture-related problems. Globe instability is a major concern with PKs, leading to devastating expulsive hemorrhages intraoperatively or postoperatively secondary to ocular trauma and wound dehiscence [1]. In the 1960s, Barraquer proposed a method to selectively replace the diseased endothelium with an anterior approach via a corneal flap. This was not adopted secondary to vascular in growth at the host-donor interface and technique-induced astigmatism [2].

Abstract:Here, we evaluate the efficacy of cryopreserved human embryonic stem cell (hESC)-derived corneal endothelial cells (CECs) to form a functional monolayer of corneal endothelium (CE) in rabbits and monkeys. We injected cryopreserved hESC-derived CECs into the anterior chamber of rabbits and monkeys either immediately after mechanical scraping of the central CE or a few days later when corneal edema developed. All preclinical models developed deturgesced and clear corneas following the injection of cryopreserved hESC-derived CECs and remained comparable to the corneas of the untreated eye. Confocal scanning microscopy confirmed an intact structure of hexagonal/polygonal cells and immunohistochemical analysis illustrated a monolayer expressing barrier and pump function proteins in the regenerated CE. The necropsy examination confirmed no remarkable change in multiple tissues assessed for teratoma formation. In conclusion, our data demonstrate the efficacy of cryopreserved hESC-derived CECs to form a functional CE on the denuded Descemet's membrane.
Clinical Outcomes in Patients With Traumatic Dehiscence of Penetrating Keratoplasty Grafts

Abstract:Purpose: The purpose of this study was to report clinical characteristics and outcomes of surgical repair for patients with traumatic dehiscence of penetrating keratoplasty (PKP) grafts. Methods: Retrospective, consecutive chart review of patients evaluated at Bascom Palmer Eye Institute between 2015 and 2020 with traumatic dehiscence of penetrating keratoplasty grafts. Results: The study cohort consisted of 65 eyes of 65 patients. The mean age at presentation was 72 years (SD 18), with a male predominance (65%). The most common indications for PKP included keratoconus (42%), corneal scar (31%), and Fuchs corneal dystrophy (8%). Dehiscence occurred as a result of blunt trauma in 94% of cases, and the mean wound length was 6.4 clock hours (SD 2.4), with a predominance of inferior dehiscence. The mean presenting visual acuity (VA) was 2.45 logMAR (SD 0.41), and the mean final VA was 2.17 logMAR (SD 0.99). Graft failure occurred in 64% of patients, and 22% underwent repeat PKP. When stratified by indication for corneal transplantation (keratoconus vs. other), there was no significant difference in graft age at the time of rupture, final VA, rate of graft failure, or rate of repeat PKP. Conclusions: Traumatic dehiscence of corneal grafts remains a rare but serious subtype of ocular trauma with generally poor visual prognoses. Presenting VA along with severity of trauma and posterior segment involvement tend to be the worst prognostic factors in final visual outcome.

Fibrin Glue-Assisted For The Treatment Of Corneal Perforations Using Glycerin-Cryopreserved Corneal Tissue

Abstract:AIM: To evaluate the outcomes and safety of lamellar keratoplasty (LK) assisted by fibrin glue in corneal perforations.METHODS: Six eyes of 6 patients affected by different corneal pathologies (2 posttraumatic corneal scar and 3 bacterial keratitis) underwent LK procedures by using fibrin glue. The mean corneal perforation diameter was 1.35 +/- 0.64mm (range, 0.7 -2.5mm), and the greatest diameter of the ulcerative stromal defect was 2.47 +/- 0.77mm in average (range, 1.5 -3.5mm). The donor corneal lamella diameters were 0.20 -mm larger and thicker than the recipient to restore a physiologic corneal thickness and shape: mean donor diameter was 8.34 +/- 0.28mm (range, 8.2 -8.7mm) and mean thickness was 352 +/- 40.27mm (range, 220-400mm). Mean follow-up was 7.33 +/- 1.97 months (range, 6-11 months). Postoperatively, the graft status, graft clarity, anterior chamber response, the visual prognosis, intraocular pressures, and postoperative complications were recorded.RESULTS: All the corneal perforations were successfully healed after the procedure. The best -corrected visual acuity (BCVA) ranged from 20/1 000 to 20/50 in their initial presentation, and from 20/100 to 20/20 in their last visit, showed increase in all the patients. No major complications such as graft dislocation and graft failure were noted. Neovascularization developed in the superficial stroma of donor graft in 1 case. High intraocular pressure developed on day 2 after surgery, while was remained in normal range after application of anti-glaucomatous eyedrops for 1 week in 1 case.CONCLUSION: Fibrin glue-assisted sutureless LK is valuable for maintaining the ocular integrity in the treatment of corneal perforations.
