The problem that this paper attempts to solve is to develop a nomogram to intuitively predict the risk of new - onset atrial fibrillation (NOPAF) after pneumonectomy and provide suggestions for protective treatment. Specifically, the research objective is to screen out independent predictors by analyzing patient data and construct a model that can effectively predict the risk of NOPAF after pneumonectomy, thereby helping clinicians to assess the probability of NOPAF occurrence in individual patients and take appropriate preventive measures. ### Background New - onset postoperative atrial fibrillation (NOPAF) refers to atrial fibrillation that lasts for more than 30 seconds or paroxysmal atrial fibrillation that requires drug or electrical cardioversion treatment in patients without a history of atrial fibrillation before pneumonectomy. NOPAF is a common complication after thoracic surgery, which has a significant impact on the short - term and long - term prognosis of patients and may lead to adverse events such as hypotension, heart failure, bleeding, drug toxicity, stroke and myocardial infarction, increasing the length of hospital stay and medical resource consumption. ### Research methods The study included patients who underwent pneumonectomy in a hospital from January 2018 to December 2020. Forward step - by - step multivariable logistic regression analysis was used to screen independent predictors and construct a nomogram model. The performance of the model was evaluated by calibration, discrimination and clinical utility, and verified by bootstrap resampling. ### Main findings - **Study population**: A total of 3,583 patients were included, of which 54 (1.507%) developed NOPAF. - **Independent predictors**: Age, admission heart rate, resection range, left - right side and maximum percentage of minute ventilation (%MVV) are independent predictors of NOPAF. - **Model performance**: - **Discrimination**: The concordance index (C - index) is 0.811, and the area under the receiver operating characteristic curve (ROC) (AUC) is 0.811 (95% CI 0.758 - 0.864). - **Calibration**: The calibration curve shows good agreement between the model - predicted values and the actual observed values. - **Clinical utility**: Decision curve analysis (DCA) indicates that using this model to predict NOPAF and implement appropriate interventions may be more beneficial than the initial treatment strategy. ### Conclusion The study has developed a new nomogram that can predict the risk of NOPAF after pneumonectomy, which helps clinicians to assess the probability of NOPAF occurrence in individual patients and take preventive measures.
