Abstract:Everyone understands the tragedy of cancer; however, the severity of its effects are only known to those who have first-hand experience. Thus, not everyone understands how it feels to receive a diagnosis yourself or to support a loved one, or how colleagues and peers react at work and school. It begs the question: why are people who are dealing with a cancer diagnosis often still keeping it a secret? I have spoken with many patients who were diagnosed with cancer during their childhood or as a teenager or young adult; in this scenario, the parents are responsible for bringing their children to a check-up in the hospital or their health practitioner, which also comes with a heavy emotional burden. A patient who recently finished her chemotherapy told me that she did not want to talk with high school friends about her diagnosis and that she would select carefully when and to whom she would open up about her cancer. People would react differently, ranging from overly friendly to avoiding the topic at all, meaning that it quickly became clear who her true friends were and who she could trust. Now that she has finished treatment, she still does not necessarily bring up the topic with new friends on her college campus. However, speaking with someone who is aware of how difficult life can be after cancer can enable her to understand her circumstances and support her during her studies, as college life can be hard on anyone, even those in perfect health.

Abstract:The Stanford Cancer Survivorship Program is a key initiative of Stanford Cancer Institute. The program's mission is to improve the experience and outcomes of patients and family caregivers throughout all phases of the cancer trajectory by advancing survivorship research, clinical care, and education. The four pillars of the program include clinical care delivery with a focus on primary care–survivorship collaboration and expanding specialty services, education and training of healthcare professionals, transdisciplinary patient-oriented research, and community engagement. Cross-cutting areas of expertise include the following: (a) adolescents and young adults (AYAs), (b) mental health and patient self-management, (c) integration of primary care, and (d) postgraduate medical education. The clinical care model includes embedded survivorship clinics within disease groups in outpatient clinics, novel clinics designed to address unmet needs such as sexual health for women, and primary care-based faculty-led survivorship clinics for patients undergoing active cancer care requiring co-management, those who have completed active therapy and those at high risk for cancer due to genetic risk. Educational initiatives developed to date include an online course and medical textbook for primary care clinicians, a lecture series, monthly research team meetings, and rotations for medical trainees. Patient-facing activities include webinars and a podcast series designed to promote awareness, thus expanding the provision of expert-vetted information. Ongoing research focuses on oncofertility and family building after cancer, improving communication for AYAs, changing mindsets to improve quality of life through targeted digital interventions, increasing capacity to care for cancer survivors, and strengthening collaboration with community partners.

Abstract:I looked after my mum at home, from the point of her clinical diagnosis by her GP to her death (alone in hospital) last year. Kaviraj conveys well how the role of carer is equal in intensity to a full time job.1For me it was like being a junior doctor round-the-clock. My mum lived with dementia, and towards the end I had to be ready for any medical emergency as she was very frail. Every day as a carer presents a "new normal"—a phrase I dislike immensely, but which summarises the moving goalposts well. I do not regret for a second the time I spent caring for my mum—the feelings of intense love and solidarity, and the opportunity to protect a vulnerable loved one. The Experience of Women Receiving Mastectomy Care in an Acute Surgical Ward: A Qualitative Study

Abstract:Aim: The research aim was to understand the inpatient experience of women who received postoperative care for mastectomy surgery in an acute surgical ward. Design: A Qualitative Descriptive Research design was used. Methods: The lead researcher conducted individual semi-structured interviews with 10 women who received postoperative care following mastectomy surgery in an acute hospital surgical ward. The transcribed interview data was analysed using a thematic analysis. Results: A mastectomy is a momentous event in any woman's life. In the initial post-operative phase, the physical intrusion of surgery, aesthetics of the ward, and mixed-gender rooms were all found to influence the women's journey towards holistic recovery. Therapeutic care from all in the healthcare team and a ward setting conducive to recovery were identified as favourable influences towards physical and emotional healing. Conclusion: Recognising that a mastectomy 'is not just physical' may help the breast care team optimise the first 24 h of postoperative care in preparation for the challenges the women may face on discharge. Implications for the profession and/or patient care: In an acute inpatient setting, a women's physical needs may be prioritised over the psychological and emotional ramifications of a mastectomy. Post-operative inpatient care for breast cancer surgery requires not only professional competency with clinical and technical tasks, but therapeutic nursing and communication skills to support women's psychological needs. Patient or public contribution: The findings were drawn from 10 women, narrating their inpatient experience for breast cancer surgery. All women had undergone a unilateral or bilateral mastectomy following breast cancer.

Abstract:Background: Breast cancer is the most common cancer for women in both the developed and the developing world, comprising nearly one quarter of all female cancers. The most advanced forms of treatment, including mastectomy, may produce a five-year survival rate of 75% or more for certain types of cancer. For women experiencing breast cancer, breast reconstruction is often a positive option that can enhance quality of life and plays a significant role in a woman's recovery from breast cancer. Aim: The aim of this article is to present some different cases of women who have undergone autologous breast reconstruction free flaps and discuss advanced and intensive nursing care practice. Implications for practice: Nurses need to not only be proficient in the postoperative physical care of patients who undergo mastectomy but also demonstrate advanced skills related to the educational needs of patients and their families and to ensure care is delivered in a manner that is patient centred and individualised. Nurses require advanced skills to meet the social and psychological care needs of the patient and their family during this major life event.

Abstract:The effect of pediatric cancer and its treatment are overwhelming—these effects are multifaceted and felt by the entire family unit throughout the diagnosis and treatment process. Children experience a plethora of effects as a result of the treatment process; however, it is imperative to remember that a pediatric cancer diagnosis affects parents physically, emotionally, and psychologically as well. While much of the pediatric cancer treatment occurs at the hospital or in clinics, parents are often faced with additional caregiving responsibilities at home, and in many cases, it is mothers who provide care to their children, while also attempting to care for the siblings of their ill children. This secondary data analysis examines the caregiving responsibilities of mothers from Southern Ontario, Canada, during the time from diagnosis to after their children’s pediatric cancer treatment. Three subthemes emerged from the overall theme of caregiving: (1) “We tried to do as much as we could outside of the clinic,” (2) “I had to be there for everything,” and (3) “Most of the time we relied on other people.” Each will be discussed in turn. The findings from this work provides insight to health care professionals on how to create or improve the current supports and resources provided to caregivers of children with cancer.

Abstract:Terminal illness is an irreversible illness that, without life-sustaining procedures, usually results in death or permanent disability from which recovery is unlikely. When involved, family caregivers are believed to improve health outcomes, such as reduced hospitalization, and establishing a patient's initial access to professional treatment services. However, caring for a patient with a terminal illness is viewed as one of the most difficult aspects of providing care. This study aimed to identify the challenges, and coping strategies developed by family caregivers to cope with the care of the terminally ill person.

Abstract:OBJECTIVES: The aim of this study was to explore the experience of being a nurse family member of a relative hospitalised for a critical illness. This paper will describe how nurse family members viewed the challenges of the illness experience and the strategies used to manage the challenges and cope with their loved one's critical illness.METHODS: A qualitative approach using open-ended, focused exploratory interviews was used. Theoretical sampling was used to obtain a total of 22 participants.RESULTS: The knowledge base of the nurse filtered the experience for the nurse family member and their nurse role identity infused each component of the experience. Nurse family members identified their primary role as maintaining guard to protect the patient and family. To accomplish this, six challenges were identified: masking heightened emotional turmoil; assuming the in-charge role; assessing and monitoring; seeking information and meaning; advocating; and, "letting go to assume family and self roles". Strategies to facilitate meeting these challenges are described.CONCLUSIONS: In order to provide family-centered care, the critical care nurse must recognise the unique needs of the "nurse family member." By empathising with the emotional experience, allowing the "in-charge" nurse family member to be part of the team, facilitating ongoing observation and monitoring by the nurse family member, seeking out and clarifying information for the nurse family member and partnering to advocate for the patient, the critical care nurse builds a relationship of trust that allows the nurse family member to assume their family role.

The experience of caring for a spouse undergoing hematopoietic stem cell transplantation: opening Pandora's box

Abstract:Background: Hematopoietic stem cell transplantation (HSCT) is a major treatment option for patients with hematological malignancies. Spouses are frequently asked to provide physical, psychological, and emotional support in both the acute care and outpatient settings, yet few studies have explored the nature and implications of their caring work beyond the acute care phase. Objective: The aims of this exploratory study were to (1) highlight the effect of caregiving on the psychosocial health and well-being of spouses of HSCT recipients; (2) highlight the level of caregiver burden, depression, and/or secondary trauma experienced by spousal caregivers of HSCT recipients; and (3) identify when spouses are most vulnerable to caregiver burden, depression, and/or secondary trauma. METHOD/METHODOLOGICAL APPROACH: A mixed-method exploratory study was undertaken. Participants were followed over 1 year from immediately before transplantation to 1 year after transplantation. Descriptive statistics and thematic analysis highlighted the effect of spousal caregiving on psychosocial health and well-being. Results: Eleven spouses participated in the study. Caregivers experienced caregiver burden, psychological distress (depression), and risk for secondary traumatic stress at 3 points in time over 1 year from before transplantation to 1 year after transplantation. Conclusions: Findings suggest that spouses may be at risk for adverse psychological effects as a result of their role in providing care for a partner undergoing HSCT. Implications for practice: Nurses need to integrate regular psychosocial assessments of caregivers to recognize the early signs of distress and intervene to support and promote psychosocial health and well-being.
Experiences, difficulties and coping methods of female nurses caring for breast cancer surgery patients: A qualitative study

Abstract:Aim The present study aims to qualitatively explore the experiences, problems, and coping methods of nurses caring for mastectomy patients. Methods The study employs an exploratory–descriptive design with the participation of 14 nurses serving in a surgical oncology clinic. We gathered the data through semi-structured, face-to-face interviews and analyzed the transcriptions of audio recordings using the thematic analysis method. We then extracted relevant codes and subthemes to achieve the main themes. The main themes are presented in three relevant contexts. Results Participating nurses' experiences are addressed through the themes of 'emotional difficulties,' 'physical difficulties,' and 'positive awareness.' In addition, their coping methods are considered within the themes of 'coping methods' and 'motivation.' The final context is identified as needs, where the themes of 'organizational needs' and 'psychosocial needs' are explored. Our findings highlighted that although participating nurses usually confront physical and emotional challenges, they exert much effort to develop both positive awareness and engage in behavioral changes. Interestingly, we observed that participants remain ineffective in dealing with and have difficulties employing relevant solutions for professional issues. Conclusion Overall, even though participating nurses need to deal with the burden of physical and emotional challenges when caring for patients, their coping strategies with these difficulties often remain insufficient. Thus, our humble recommendation may lie in satisfying the organizational and psychosocial needs of surgical nurses to promote their coping skills when fulfilling their demanding tasks with mastectomy patients.

Abstract:Children and adolescents with cancer are increasingly treated and cared for at home; hospital stays are reduced to a minimum. Taking care of a sick child at home has an impact on the entire family: the sick child, the siblings, and the parents. This qualitative study examines the experiences of parents taking their child home for the first time after the diagnosis. Parents of 10 children newly diagnosed with cancer were interviewed twice around the time of the first discharge; data were analyzed using content analysis methodology. Findings illustrated parents’ preparation of and experiences around their child’s first discharge, the huge amount of new and changed tasks parents have to fulfill at home when caring for their child with cancer, and consequences for the parents. By providing individualized information and instruction, by having parents anticipate potential problems and solutions, and by describing available community support and integrating district nurses as well as other parents with the same experiences more frequently, health care professionals in the hospital can optimize discharge planning for these parents.

Abstract:Background Being a family member to someone who suffers from a serious illness can change one's perspectives about life. A sudden and severe illness can result in a demanding journey that involves acute admission to hospital, intensive care, post‐care, and finally, returning home. Aim To describe the experience of staying beside a next of kin throughout the healthcare journey, from the onset of illness at home, to the intensive care unit, post‐care, and returning home. Methods The study employed a qualitative design, with data collected via semi‐structured interviews and analysed using reflexive thematic analysis. A purposive sampling was used to recruit participants (n = 14), who had experiences of staying beside a next of kin throughout the healthcare journey from the onset of illness to discharge from the hospital. Findings Family members' experiences were captured under the overarching theme Journey through an emotional turmoil and the themes Entering a new world, Continuing the journey towards something unknown, and Striving for the new normal at home. They described going through a non‐linear process, characterised by transitions or changes in several aspects. The transitions included shifts in the roles that the family members played, in the various environments they found themselves in, and in the progression of the patient's illness or injury. Conclusion The study suggested that family members with a next of kin who experienced life‐threatening situations undergo challenging transitions. The informal caregiver role placed on family members of intensive care unit‐survivors significantly impacts their lives and the healthcare systems should prioritise providing high‐quality support to family members throughout the entire healthcare journey. Elevating the importance of nursing care within the healthcare system can contribute to delivering holistic care and facilitating transitions. Further research should focus on understanding the support that family members perceive as necessary to facilitate their transition and enhance their well‐being.

Abstract:Purpose The purpose of this study was to explore women's experiences with healthcare providers previous to a flat closure after breast cancer. Methods Data was collected using individual semi-structured interviews by telephone or teleconferencing systems with 18 women who shared their experience with health care before and during flat closure operation after breast cancer or a breast gene mutation. The interviews were transcribed verbatim and analysed using thematic analysis. Results The analysis generated three specific themes: (1) Striving to be recognized as a person , (2) Barriers to shared decision making and (3) A need to be empowered , indicating that women's views are not always illuminated and enquired about by health care providers. The care agenda was also more often regulated by norms and standard care than the individual women's intended goals. Furthermore, when the agenda for surgery had been previously set, the women were not presented with viable options to choose from for the upcoming surgery. For these women to influence the reality they face, they have to seek support outside of healthcare. This indicates the important role of patient networks and relatives in empowering the individual women to stand up for their choice and body. Conclusion In order to offer feasible surgery for women diagnosed with breast cancer or a breast mutation gene, the women's wishes regarding flat closure and what matters to her in her life must be identified. Also emphasized is the need for a more systematic approach in involving the affected women in shared decision-making.

Abstract:This particular issue of our journal featuring author contributions related to cancer speaks to me loudly. Last year, on my birthday no less, I got a recall letter from my annual mammogram. We can all agree it was not a great way to mark the occasion. After more imaging a biopsy was required. Luckily the biopsy was benign but still suspicious so based on several factors I am now in a high risk category for breast cancer and require screenings every six months. Last month a second mass was found. I feel understood by Botto et al. as they noted that stress is highest in the follow up stage and ever present during a reoccurrence. While my first mass was not detectable from a self-exam, my second mass was underscoring the importance of the work of Gül and Büyükbayram in doing all possible to increase awareness and improving detection. Karisani et al. further share the important work of screening for cervical cancers. While I have not had to make any surgery decisions yet, I am glad to read the work of Koksalmis and colleagues as their decision-making instrument requires physicians to take into account patient factors. While I cannot say that I would one day opt for conservation over mastectomy, I know that a decision so personal must be made with an individual's characteristics in mind.

Abstract:Introduction There is a pressing need to understand and explore the complex experiences and psychosocial support needs of people LWBC‐CM and their informal caregivers, to inform survivorship and supportive care interventions. Methods In‐depth qualitative interviews were conducted with people LWBC‐CM and their informal caregivers in Scotland, invited via primary care. One‐to‐one, face‐to‐face interviews were conducted with informed consent exploring experiences of symptoms, psychosocial support needs and interactions with health services. Interviews were transcribed and analysed using a thematic approach. Results Forty‐one people LWBC‐CM and twenty‐three informal caregivers were interviewed. Four themes were identified: the Physical and Psychological Impact of Cancer and Comorbidity, Dominant Storie—Prioritising Conditions and Making Sense of Illness, Navigating Health Services and Treatments and Caring for People with Complex Health Conditions. Type and severity of conditions mediated people's experiences and daily living. Complex fatigue—fatigue arising from a number of health conditions—dominated symptomology. Participants navigated multiple appointments and complex medication regimes. Patients identified the need for acknowledgement of other chronic conditions and for streamlined care provision. Mutual caring and social isolation were also identified as part of the caring relationship. Conclusions There is a mandate to address the psychosocial support needs of people LWBC‐CM, and their informal carers, given the burden of treatment for cancer survivors with moderate to severe complex conditions as they navigate health services. Patient or Public Contribution A patient representative has been involved in all stages of the study from development of the application through study design, commenting on documentation, analysis of transcripts and writing the manuscript. They are included as an author on the manuscript.
