Abstract:Coronary artery disease (CAD) exists on a spectrum of disease represented by a combination of risk factors and pathogenic processes. An in silico score for CAD built using machine learning and clinical data in electronic health records captures disease progression, severity and underdiagnosis on this spectrum and could enhance genetic discovery efforts for CAD. Here we tested associations of rare and ultrarare coding variants with the in silico score for CAD in the UK Biobank, All of Us Research Program and Bio Me Biobank. We identified associations in 17 genes; of these, 14 show at least moderate levels of prior genetic, biological and/or clinical support for CAD. We also observed an excess of ultrarare coding variants in 321 aggregated CAD genes, suggesting more ultrarare variant associations await discovery. These results expand our understanding of the genetic etiology of CAD and illustrate how digital markers can enhance genetic association investigations for complex diseases.

Abstract:OBJECTIVES The goal of this study was to investigate the association of stenosis and plaque features with myocardial ischemia and their prognostic implications. BACKGROUND Various anatomic, functional, and morphological attributes of coronary artery disease (CAD) have been independently explored to define ischemia and prognosis. METHODS A total of 1,013 vessels with fractional flow reserve (FFR) measurement and available coronary computed tomography angiography were analyzed. Stenosis and plaque features of the target lesion and vessel were evaluated by an independent core laboratory. Relevant features associated with low FFR (<= 0.80) were identified by using machine learning, and their predictability of 5-year risk of vessel-oriented composite outcome, including cardiac death, target vessel myocardial infarction, or target vessel revascutarization, were evaluated. RESULTS The mean percent diameter stenosis and invasive FFR were 48.5 +/- 17.4% and 0.81 +/- 0.14, respectively. Machine learning interrogation identified 6 clusters for low FFR, and the most relevant feature from each duster was minimum lumen area, percent atheroma volume, fibrofatty and necrotic core volume, plaque volume, proximal left anterior descending coronary artery lesion, and remodeling index (in order of importance). These 6 features showed predictability for low FFR (area under the receiver-operating characteristic curve: 0.797). The risk of 5-year vessel-oriented composite outcome increased with every increment of the number of 6 relevant features, and it had incremental prognostic value over percent diameter stenosis and FFR (area under the receiver-operating characteristic curve: 0.706 vs. 0.611; p = 0.031). CONCLUSIONS Six functionally relevant features, induding minimum lumen area, percent atheroma volume, fibrofatty and necrotic core volume, plaque volume, proximal left anterior descending coronary artery lesion, and remodeling index, help define the presence of myocardial ischemia and provide better prognostication in patients with CAD. Abstract:Coronary heart disease (CHD) is a leading cause of death in the US. CHD associated morbidity and mortality can be reduced if those at risk can be identified well in advance, to put in place prevention interventions prior to an acute coronary event. Limitations of current approaches call for additional efforts to improve risk predictions. We recently developed a Precision Medicine tool capable of predicting 5-year risk, that can also guide personalized prevention interventions and monitor changes in risk over time. It was developed using an application of AI, machine learning, in the Framingham Heart Study to mine integrated genetic-epigenetic biosignatures that capture the complex interplay between our genome and the environment in conferring risk for CHD. The training and test sets consisted of 1180 (19/695 females and 23/485 males developed symptomatic CHD within 5-years) and 524 (8/293 females and 12/231 males developed symptomatic CHD within 5-years) individuals, respectively, of Northern European ancestry. Non-linear data mining and modeling techniques were employed on the training set to identify prediction signatures from genome-wide SNP and DNA methylation (DNAm) data. We identified 14 DNAm and 10 SNPs from the training set (training bootstrap average sensitivity and specificity of 0.61 and 0.71, respectively) that predicted the 5-year risk in the test set with a sensitivity and specificity of 0.60 and 0.69, respectively. Performance was compared to that of the Framingham Risk Score (FRS) and the ASCVD Risk Estimator. The sensitivity and specificity in the test set were 0.05 and 0.99 for FRS, and 0.38 and 0.85 for ASCVD. Even with a small number of incident cases, our tool performed with superior sensitivity, which is vital because, while a false positive would result in additional testing, a false negative can be detrimental. Unlike genetics, because DNAm is dynamic, it can be mapped to actionable risk factors to guide personalized interventions and over time, re-assess risk and continuously monitor heart health. We demonstrate translational feasibility of this tool via next-gen digital PCR assays. Additional efforts are ongoing to optimize, validate and extend our tool to further improve performance in larger, more ethnically diverse cohorts.

Abstract:Due to the lack of precise markers indicative of its occurrence and progression, coronary artery disease (CAD), the most common type of heart diseases, is currently associated with high mortality in the United States. To systemically identify novel protein biomarkers associated with CAD progression for early diagnosis and possible therapeutic intervention, we employed an iTRAQ-based quantitative proteomic approach to analyze the proteome changes in the plasma collected from a pair of wild-type versus apolipoprotein E knockout (APOE(-/-)) mice which were fed with a high fat diet. In a multiplex manner, iTRAQ serves as the quantitative 'in-spectra' marker for 'cross-sample' comparisons to determine the differentially expressed/secreted proteins caused by APOE knock-out. To obtain the most comprehensive proteomic data sets from this CAD-associated mouse model, we applied both MALDI and ESI-based mass spectrometric (MS) platforms coupled with two different schemes of multidimensional liquid chromatography (2-D LC) separation. We then comparatively analyzed a series of the plasma samples collected at 6 and 12wk of age after the mice were fed with fat diets, where the 6- or 12-wk time point represents the early or intermediate phase of the fat-induced CAD, respectively. We then categorized those proteins showing abundance changes in accordance with APOE depletion. Several proteins such as the gamma and beta chains of fibrinogen, apolipoprotein B, apolipoprotein C-I, and thrombospondin-4 were among the previously known CAD markers identified by other methods. Our results suggested that these unbiased proteomic methods are both feasible and a practical means of discovering potential biomarkers associated with CAD progression.
[The use of tactivin for treating patients with IHD].

Abstract:Immunomodulating drug tactivin was used for treatment of 29 IHD patients with low T-suppressor activity and high cell sensitization to apoprotein B-containing lipoproteins and with lipoprotein-antibody immune complex detected in the blood. Tactivin was injected subcutaneously in the dose 10 micrograms (5 injections for 12 days). Tactivin treatment normalized T-suppressor activity, lowered concentration of plasma lipoprotein-antibody immune complex and improved clinical course of IHD. The beneficial effect of tactivin was observed in 70% of patients during 2 months. 30% of patients needed a second course of treatment of recover normal immunological and clinical indices. The treatment with tactivin of IHD patients must be combined with application of routine antianginal drugs.
Application of machine learning algorithms to predict coronary artery calcification with a sibship-based design

Abstract:As part of the Genetic Epidemiology Network of Arteriopathy study, hypertensive non-Hispanic White sibships were screened using 471 single nucleotide polymorphisms (SNPs) to identify genes influencing coronary artery calcification (CAC) measured by computed tomography. Individuals with detectable CAC and CAC quantity > or =70th age- and sex-specific percentile were classified as having a high CAC burden and compared to individuals with CAC quantity <70th percentile. Two sibs from each sibship were randomly chosen and divided into two data sets, each with 360 unrelated individuals. Within each data set, we applied two machine learning algorithms, Random Forests and RuleFit, to identify the best predictors of having high CAC burden among 17 risk factors and 471 SNPs. Using five-fold cross-validation, both methods had approximately 70% sensitivity and approximately 60% specificity. Prediction accuracies were significantly different from random predictions (P-value<0.001) based on 1,000 permutation tests. Predictability of using 287 tagSNPs was as good as using all 471 SNPs. For Random Forests, among the top 50 predictors, the same eight tagSNPs and 15 risk factors were found in both data sets while eight tagSNPs and 12 risk factors were found in both data sets for RuleFit. Replicable effects of two tagSNPs (in genes GPR35 and NOS3) and 12 risk factors (age, body mass index, sex, serum glucose, high-density lipoprotein cholesterol, systolic blood pressure, cholesterol, homocysteine, triglycerides, fibrinogen, Lp(a) and low-density lipoprotein particle size) were identified by both methods. This study illustrates how machine learning methods can be used in sibships to identify important, replicable predictors of subclinical coronary atherosclerosis.

Abstract:Although genome wide association studies (GWAS) in large populations have identified hundreds of variants associated with common diseases such as coronary artery disease (CAD), most disease-associated variants lie within non-coding regions of the genome, rendering it difficult to determine the downstream causal gene and cell type. Here, we performed paired single nucleus gene expression and chromatin accessibility profiling from 44 human coronary arteries. To link disease variants to molecular traits, we developed a meta-map of 88 samples and discovered 11,182 single-cell chromatin accessibility quantitative trait loci (caQTLs). Heritability enrichment analysis and disease variant mapping demonstrated that smooth muscle cells (SMCs) harbor the greatest genetic risk for CAD. To capture the continuum of SMC cell states in disease, we used dynamic single cell caQTL modeling for the first time in tissue to uncover QTLs whose effects are modified by cell state and expand our insight into genetic regulation of heterogenous cell populations. Notably, we identified a variant in the CAD GWAS locus which becomes a caQTL as SMCs de-differentiate by changing a transcription factor binding site for EGR1/2. To unbiasedly prioritize functional candidate genes, we built a genome-wide single cell variant to enhancer to gene (scV2E2G) map for human CAD to link disease variants to causal genes in cell types. Using this approach, we found several hundred genes predicted to be linked to disease variants in different cell types. Next, we performed genome-wide Hi-C in 16 human coronary arteries to build tissue specific maps of chromatin conformation and link disease variants to integrated chromatin hubs and distal target genes. Using this approach, we show that rs4887091 within the CAD GWAS locus modulates function of a super chromatin interactome through a change in a CTCF binding site. Finally, we used CRISPR interference to validate a distal gene, , liked to a CAD GWAS locus. Collectively we provide a disease-agnostic framework to translate human genetic findings to identify pathologic cell states and genes driving disease, producing a comprehensive scV2E2G map with genetic and tissue level convergence for future mechanistic and therapeutic studies.
Mitophagy related diagnostic biomarkers for coronary in-stent restenosis identified using machine learning and bioinformatics

Abstract:Percutaneous coronary intervention (PCI) combined with stent implantation is currently one of the most effective treatments for coronary artery disease (CAD). However, in-stent restenosis (ISR) significantly compromises its long-term efficacy. Mitophagy plays a crucial role in vascular homeostasis, yet its role in ISR remains unclear. This study aims to identify mitophagy-related biomarkers for ISR and explore their underlying molecular mechanisms. Through differential gene expression analysis between ISR and Control samples in the combined dataset, 169 differentially expressed genes (DEGs) were identified. Twenty-three differentially expressed mitophagy-related genes (DEMRGs) were identified by intersecting with mitophagy-related genes (MRGs) from the GeneCards, and functional enrichment analysis indicated their significant involvement in mitophagy-related biological processes. Using Weighted Gene Co-expression Network Analysis (WGCNA) and three machine learning algorithms (Logistic-LASSO, RF, and SVM-RFE), LRRK2, and ANKRD13A were identified as mitophagy-related biomarkers for ISR. The nomogram based on these two genes also exhibited promising diagnostic performance for ISR. Gene Set Enrichment Analysis (GSEA) as well as immune infiltration analyses showed that these two genes were closely associated with immune and inflammatory responses in ISR. Furthermore, potential small molecule compounds with therapeutic implications for ISR were predicted using the connectivity Map (cMAP) database. This study systematically investigated mitophagy-related biomarkers for ISR and their potential biological functions, providing new insights into early diagnosis and precision treatment strategies for ISR.

Abstract:Purpose The purpose of this study was to evaluate the diagnostic performance of automated deep learning in the detection of coronary artery disease (CAD) on photon-counting coronary CT angiography (PC-CCTA). Materials and methods Consecutive patients with suspected CAD who underwent PC-CCTA between January 2022 and December 2023 were included in this retrospective, single-center study. Non-ultra-high resolution (UHR) PC-CCTA images were analyzed by artificial intelligence using two deep learning models (CorEx, Spimed-AI), and compared to human expert reader assessment using UHR PC-CCTA images. Diagnostic performance for global CAD assessment (at least one significant stenosis ≥ 50 %) was estimated at patient and vessel levels. Results A total of 140 patients (96 men, 44 women) with a median age of 60 years (first quartile, 51; third quartile, 68) were evaluated. Significant CAD on UHR PC-CCTA was present in 36/140 patients (25.7 %). The sensitivity, specificity, accuracy, positive predictive value), and negative predictive value of deep learning-based CAD were 97.2 %, 81.7 %, 85.7 %, 64.8 %, and 98.9 %, respectively, at the patient level and 96.6 %, 86.7 %, 88.1 %, 53.8 %, and 99.4 %, respectively, at the vessel level. The area under the receiver operating characteristic curve was 0.90 (95 % CI: 0.83–0.94) at the patient level and 0.92 (95 % CI: 0.89–0.94) at the vessel level. Conclusion Automated deep learning shows remarkable performance for the diagnosis of significant CAD on non-UHR PC-CCTA images. AI pre-reading may be of supportive value to the human reader in daily clinical practice to target and validate coronary artery stenosis using UHR PC-CCTA.

Abstract:Genome-wide association studies (GWASs) have advanced our understanding of coronary artery disease (CAD) genetics and enabled the development of polygenic risk scores (PRSs) for estimating genetic risk based on common variant burden. However, GWASs have limitations in analyzing rare variants due to insufficient statistical power, thereby constraining PRS performance. Here, we conducted whole genome sequencing of 1,752 Japanese CAD patients and 3,019 controls, applying a machine learning-based rare variant analytic framework. This approach identified 59 CAD-related genes, including known causal genes like LDLR and those not previously captured by GWASs. A rare variant-based risk score (RVS) derived from the framework significantly predicted CAD cases and cardiovascular mortality in an independent cohort. Notably, combining the RVS with traditional PRS improved CAD prediction compared to PRS alone (area under the curve, 0.66 vs 0.61; p=0.007). Our analyses reinforce the value of incorporating rare variant information, highlighting the potential for more comprehensive genetic assessment.

Abstract:Coronary artery calcification (CAC), a measure of subclinical atherosclerosis, predicts future symptomatic coronary artery disease (CAD). Identifying genetic risk factors for CAC may point to new therapeutic avenues for prevention. Currently, there are only four known risk loci for CAC identified from genome-wide association studies (GWAS) in the general population. Here we conducted the largest multi-ancestry GWAS meta-analysis of CAC to date, which comprised 26,909 individuals of European ancestry and 8,867 individuals of African ancestry. We identified 11 independent risk loci, of which eight were new for CAC and five had not been reported for CAD. These new CAC loci are related to bone mineralization, phosphate catabolism and hormone metabolic pathways. Several new loci harbor candidate causal genes supported by multiple lines of functional evidence and are regulators of smooth muscle cell-mediated calcification ex vivo and in vitro. Together, these findings help refine the genetic architecture of CAC and extend our understanding of the biological and potential druggable pathways underlying CAC.

Abstract:Introduction: Atherosclerosis is a chronic inflammatory process driven by plaque formation in the major elastic arteries leading to reduced blood flow, coronary artery disease (CAD), myocardial infarction and stroke. Several single-cell RNA-seq (scRNA-seq) studies have highlighted lineage-specific transcriptomic signatures in CAD-relevant tissues; however, there remains variability on the reported cell phenotypes in humans. We evaluated the overall hypothesis that distinct smooth muscle cell (SMC) phenotypes characterized from scRNA-seq data contribute to human atherosclerotic lesion formation. Methods: Here we meta-analyzed scRNA-seq human data from four individual studies to establish a comprehensive map of cell diversity in human atherosclerosis. We applied standardized QC, processing, and integration benchmarking to harmonize datasets and generate an atlas of 118,578 high-quality cells. Results: Using this atlas we characterize vascular and immune cell diversity during atherosclerosis by identifying 100 consensus marker genes across 42 annotated cell types/subtypes. We provide insights into SMC phenotypic modulation through pseudotime, transcription factor activity inference and cell-cell communication analyses. By integrating genome-wide association study (GWAS) data we identify etiologic cell types for subclinical and clinical CAD traits, which uncovered a critical role for modulated SMC phenotypes in CAD and coronary artery calcification. Finally, we identified candidate markers of synthetic and osteochondrogenic human SMCs (e.g., LTBP1 and CRTAC1 ) that may serve as proxies of atherosclerosis progression and provide validation through bulk transcriptomics, proteomics, and immunofluorescence. Conclusions: Together, this represents an important step towards creating a unified cellular map of atherosclerosis to inform cell-specific mechanistic and translational studies of cardiovascular diseases.

Abstract:Acute myocardial infarction dominates coronary artery disease mortality. Identifying bio-signatures for plaque destabilization and rupture is important for preventing the transition from coronary stability to instability and the occurrence of thrombosis events. This computational systems biology study enrolled 2,235 samples from 22 independent bulks cohorts and 14 samples from two single-cell cohorts. A machine-learning integrative program containing nine learners was developed to generate a warning classifier linked to atherosclerotic plaque vulnerability signature (APVS). The classifier displays the reliable performance and robustness for distinguishing ST-elevation myocardial infarction from chronic coronary syndrome at presentation, and revealed higher accuracy to 33 pathogenic biomarkers. We also developed an APVS-based quantification system (APVSLevel) for comprehensively quantifying atherosclerotic plaque vulnerability, empowering early-warning capabilities, and accurate assessment of atherosclerosis severity. It unraveled the multidimensional dysregulated mechanisms at high resolution. This study provides a potential tool for macro-level differential diagnosis and evaluation of subtle genetic pathological changes in atherosclerosis.

Abstract:Personalized interventions are deemed vital given the intricate characteristics, advancement, inherent genetic composition, and diversity of cardiovascular diseases (CVDs). The appropriate utilization of artificial intelligence (AI) and machine learning (ML) methodologies can yield novel understandings of CVDs, enabling improved personalized treatments through predictive analysis and deep phenotyping. In this study, we proposed and employed a novel approach combining traditional statistics and a nexus of cutting-edge AI/ML techniques to identify significant biomarkers for our predictive engine by analyzing the complete transcriptome of CVD patients. After robust gene expression data pre-processing, we utilized three statistical tests (Pearson correlation, Chi-square test, and ANOVA) to assess the differences in transcriptomic expression and clinical characteristics between healthy individuals and CVD patients. Next, the recursive feature elimination classifier assigned rankings to transcriptomic features based on their relation to the case–control variable. The top ten percent of commonly observed significant biomarkers were evaluated using four unique ML classifiers (Random Forest, Support Vector Machine, Xtreme Gradient Boosting Decision Trees, and k-Nearest Neighbors). After optimizing hyperparameters, the ensembled models, which were implemented using a soft voting classifier, accurately differentiated between patients and healthy individuals. We have uncovered 18 transcriptomic biomarkers that are highly significant in the CVD population that were used to predict disease with up to 96% accuracy. Additionally, we cross-validated our results with clinical records collected from patients in our cohort. The identified biomarkers served as potential indicators for early detection of CVDs. With its successful implementation, our newly developed predictive engine provides a valuable framework for identifying patients with CVDs based on their biomarker profiles.

Abstract:Background: Coronary artery disease (CAD) is a progressive disease of the blood vessels supplying the heart, which leads to coronary artery stenosis or obstruction and is life-threatening. Early diagnosis of CAD is essential for timely intervention. Imaging tests are widely used in diagnosing CAD, and artificial intelligence (AI) technology is used to shed light on the development of new imaging diagnostic markers. Objective: We aim to investigate and summarize how AI algorithms are used in the development of diagnostic models of CAD with imaging markers. Methods: This scoping review followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guideline. Eligible articles were searched in PubMed and Embase. Based on the predefined included criteria, articles on coronary heart disease were selected for this scoping review. Data extraction was independently conducted by two reviewers, and a narrative synthesis approach was used in the analysis. Results: A total of 46 articles were included in the scoping review. The most common types of imaging methods complemented by AI included single-photon emission computed tomography (15/46, 32.6%) and coronary computed tomography angiography (15/46, 32.6%). Deep learning (DL) (41/46, 89.2%) algorithms were used more often than machine learning algorithms (5/46, 10.8%). The models yielded good model performance in terms of accuracy, sensitivity, specificity, and AUC. However, most of the primary studies used a relatively small sample ( n < 500) in model development, and only few studies (4/46, 8.7%) carried out external validation of the AI model. Conclusion: As non-invasive diagnostic methods, imaging markers integrated with AI have exhibited considerable potential in the diagnosis of CAD. External validation of model performance and evaluation of clinical use aid in the confirmation of the added value of markers in practice. Prospective deep learning-based quantitative assessment of coronary plaque by computed tomography angiography compared with intravascular ultrasound: the REVEALPLAQUE study

Abstract:Aims: Coronary computed tomography angiography provides non-invasive assessment of coronary stenosis severity and flow impairment. Automated artificial intelligence (AI) analysis may assist in precise quantification and characterization of coronary atherosclerosis, enabling patient-specific risk determination and management strategies. This multicentre international study compared an automated deep learning-based method for segmenting coronary atherosclerosis in coronary computed tomography angiography (CCTA) against the reference standard of intravascular ultrasound (IVUS). Methods and results: The study included clinically stable patients with known coronary artery disease from 15 centres in the USA and Japan. An AI-enabled plaque analysis was utilized to quantify and characterize total plaque (TPV), vessel, lumen, calcified plaque (CP), non-calcified plaque (NCP), and low-attenuation plaque (LAP) volumes derived from CCTA and compared with IVUS measurements in a blinded, core laboratory-adjudicated fashion. In 237 patients, 432 lesions were assessed; mean lesion length was 24.5 mm, and mean IVUS-TPV was 186.0 mm3. AI-enabled plaque analysis on CCTA showed strong correlation and high accuracy when compared with IVUS; correlation coefficient, slope, and Y intercept for TPV were 0.91, 0.99, and 1.87, respectively; for CP volume 0.91, 1.05, and 5.32, respectively; and for NCP volume 0.87, 0.98, and 15.24, respectively. Bland-Altman analysis demonstrated strong agreement with little bias for these measurements. Conclusion: AI-enabled CCTA quantification and characterization of atherosclerosis demonstrated strong agreement with IVUS reference standard measurements. This tool may prove effective for accurate evaluation of coronary atherosclerotic burden and cardiovascular risk assessment.

Abstract:Background: Clonal hematopoiesis of indeterminate potential (CHIP) has been shown to be an independent risk factor for heart disease and heart failure. DNA sequencing is the gold standard to diagnose CHIP. It is unclear if deep learning techniques using routine clinical imaging can aid in identifying those who are likely to have CHIP and thus likely at increased risk for heart disease and heart failure. Hypothesis: We hypothesized that deep learning models trained on routine cardiac MRI (CMR) would be able to help predict who has CHIP Methods: We enrolled patients into an IRB approved study to evaluate for CHIP and associated imaging findings. Whole exome sequencing and our somatic variant pipeline was used to identify those who had CHIP. Various machine learning techniques were employed for training and testing, including SVM, random forest and convolutional neural networks (CNN) to predict who had CHIP from CMR imaging. Results: We enrolled 98 unique patients with CMR and found that 40 (41%) had CHIP. Majority were female 65 (66%) and there was no significant difference in median age between CHIP vs no CHIP (69, IQR 62-78 vs 65, IQR 54-72). There were no significant differences between CHIP vs no CHIP patients in co-morbidities such as CAD, DM, HTN, HF. Compared to SVM and random forest, the best performing model with high sensitivity, specificity and accuracy was derived from CNN (AUC 0.64 vs 0.71 vs 0.85 respectively). We found that multi-view MRI inputs were more accurate than single view MRI inputs for the CNN model. Using an unsupervised approach with CNN and multi-view input, we were able to predict CHIP with 82% accuracy. Conclusions: CNN is a promising deep learning technique that can accurately classify CHIP and thus those at higher cardiovascular disease risk. Ongoing analyses are underway to validate these findings using the UK Biobank and TOPMED datasets and predict who will develop cardiomyopathy with CHIP.

Abstract:Coronary artery calcification (CAC) is a marker of atherosclerosis and is thought to be associated with worse clinical outcomes. However, evidence from large-scale high-resolution imaging data is lacking. We proposed a novel deep learning method that can automatically identify and quantify CAC in massive intravascular OCT data trained using efficiently generated sparse labels. 1,106,291 OCT images from 1,048 patients were collected and utilized to train and evaluate the method. The Dice similarity coefficient for CAC segmentation and the accuracy for CAC classification are 0.693 and 0.932, respectively, close to human-level performance. Applying the method to 1259 ST-segment elevated myocardial infarction patients imaged with OCT, we found that patients with a greater extent and more severe calcification in the culprit vessels were significantly more likely to have major adverse cardiovascular and cerebrovascular events (MACCE) (p < 0.05), while the CAC in non-culprit vessels did not differ significantly between MACCE and non-MACCE groups.

Abstract:This study focuses on identifying anatomical markers with predictive capacity for long-term myocardial infarction (MI) in focal coronary artery disease (CAD). Eighty future culprit lesions (FCL) and 108 non-culprit lesions (NCL) from 80 patients underwent 3D quantitative coronary angiography. The minimum lumen area (MLA), minimum lumen ratio (MLR), and vessel fractional flow reserve (vFFR) were evaluated. MLR was defined as the ratio between MLA and the cross-sectional area at the proximal lesion edge, with lower values indicating more abrupt luminal narrowing. Significant differences were observed between FCL and NCL in MLR (0.41 vs. 0.53, p < 0.001). MLR correlated inversely with translesional vFFR ( r = − 0.26, p = 0.0004) and was the strongest predictor of MI at 5 years (AUC = 0.75). Lesions with MLR < 0.40 had a fourfold increased MI incidence at 5 years. MLR is a robust predictor of future adverse coronary events.

Abstract:Abstract Aims Vessel specific coronary artery calcification (CAC) is additive to global CAC for prognostic assessment. We assessed accuracy and prognostic implications of vessel-specific automated deep learning (DL) CAC analysis on electrocardiogram gated and attenuation correction computed tomography (CT) in a large multicenter registry. Methods and results Vessel-specific CAC was assessed in the left main/left anterior descending (LM/LAD), left circumflex (LCX) and right coronary artery (RCA) using a DL model trained on 3000 gated CT and tested on 2094 gated CT and 5969 non-gated attenuation correction CT. Vessel-specific agreement was assessed with linear weighted Cohen’s Kappa for CAC zero, 1-100, 101-400 and >400 Agatston units (AU). Risk of major adverse cardiovascular events (MACE) was assessed during 2.4±1.4 years follow-up, with hazard ratios (HR) and 95% confidence intervals (CI). There was strong to excellent agreement between DL and expert ground truth for CAC in LM/LAD, LCX and RCA on gated CT [0.90 (95% CI 0.89 to 0.92); 0.70 (0.68 to 0.73); 0.79 (0.77 to 0.81)] and attenuation correction CT [(0.78 (0.77 to 0.80); 0.60 (0.58 to 0.62); 0.70 (0.68 to 0.71)]. MACE occurred in 242 (12%) undergoing gated CT and 841(14%) of undergoing attenuation correction CT. LM/LAD CAC >400 AU was associated with the highest risk of MACE on gated (HR 12.0, 95% CI 7.96, 18.0, p<0.001) and attenuation correction CT (HR 4.21, 95% CI 3.48, 5.08, p<0.001). Conclusion Vessel-specific CAC assessment with DL can be performed accurately and rapidly on gated CT and attenuation correction CT and provides important prognostic information.
