Abstract:Pneumocephalus is an uncommon, often forgotten complication of meningitis caused by gas-producing organisms, especially in advanced aged patients. Attention should be paid to physical signs, and early computed tomography (CT) scan could be helpful in the diagnosis. Accurate antibiotics according to blood culture would be the key point in the treatment procedure. A 95-year-old woman visited the Second Hospital of Jilin University and presented with fever and chills; the highest temperature was 39.5°C and she was alert with stable vital signs at the beginning. Post medical history included tuberculosis caused left pulmonary fibrosis. Hematological investigation revealed mild leukocytosis with a total white blood cell (WBC) count of 13.1×109/L (reference range 3.5-9.5×109/L), and neutrophils count and percentage were 12.50×109/L (reference range 1.8-6.3×109/L) and 95.3% (reference range 40%-75%), respectively. Procalcitonin (PCT) was 0.75 ng/ml (normal 0-0.5 ng/ml), and she had an elevated erythrocyte sedimentation rate (ESR) of 42 mm/h (normal 0-20 mm/h). Chest CT showed left atelectasis with pleural effusion. Blood cultures were negative twice. The patient received ertapenem intravenously, but despite aggressive fluid and antibiotic therapy, she failed to respond to treatment, even followed with diarrhea. Five days after admission, while still febrile, she developed spasm and acute confusion, anisocoric pupils without light reflex, and left upward eyeball deviation was observed. The brain CT scan revealed cerebral air embolism that caused extensive cerebral infarction accompanied by a hypodense lesion located in the bilateral cerebral cortex, mainly on the right side; mass effect and midline shifting were observed, and no mastoiditis and ethmoid sinusitis (Figure 1). The bone window image of brain CT scan revealed no evidence of fracture or bony lesion over the skull base or ethmoid sinus. The patient remained in a deep coma and expired later, with no chance for a lumbar puncture. An autopsy was not performed.Figure 1.: The brain CT scan for this 95-year-old woman. C: No signs of Mastoiditis and ethmoid sinusitis.Pneumocephalus mostly arises as a complication of craniofacial trauma, neurosurgical procedures, and less frequently, of neoplasm, otitis media, post-radiation necrosis, or sinusitis and rarely, spontaneous reasons.1 Various etiologies leading to pneumocephalus are classified and summarized in Table 1.Table 1: Etiologies and classification of pneumocephalusIn this case, a history of a trivial head injury and bone defect was not found in our patient. Mastoiditis and ethmoid sinusitis were also not shown in the CT scan. Because the patient had fever and severe infection, the most likely reason was the possibility of bacterial meningitis with gas-forming organisms. Spontaneous pneumocephalus caused by meningitis is extremely rare, and is usually associated with Clostridium perfringens meningitis,2 and Acinetobacter baumannii,3 but such infection usually happens in immune-compromised patients, such as those taking immunosuppressants or with acquired immune deficiency syndrome (AIDS), or diabetic patients with unsatisfied glycemic control. Seldom have there been case reports of pneumocephalus associated with pneumococcal meningitis in immunosuppressive patients. Recalling from medical history showed that the patient had been under severe mood shock for about 2 months before the fever; the bad mood caused serious depression and under nutrition. This was the possible cause of the immune suppression and the following serious infection. The breakdown of the blood-brain barrier caused central infections, and gas-producing organisms produced cerebral air embolism, which caused extensive cerebral infarction, resulting in death due to hernia. If the patient had been treated earlier with appropriate antibiotics, the result may be different. However, our patient did not get diagnosed by CT scan until 2 to 3 days later after the compressive neurologic symptoms. In the cases published in the literature, headache, hemiplegia, otoliquorrhea, visual alterations, aphasia, and tinnitus have been described.4 Early suspicion, correct diagnosis, appropriate antibiotic therapy with adequate cerebrospinal fluid (CSF) analysis and radiological studies are essential for pneumocephalus associated with meningitis. Careful physical examination is never antiquated no matter which sub-specialty the physician is in for modern medicine.
