Abstract:Hematology-related diseases, such as anemia, malaria, sickle cell disease (SCD), and blood cancers, have differing rates of survival between high-income and low- and middle-income countries (LMICs). Nurses in LMICs have an unmet need for specialty training and education to address hematology and hemato-oncology disorders. A gap in the literature exists about hematology nurse education and clinical service demands in LMICs. This community case study documents a collaborative hematology and basic hemato-oncology education program to sustainably strengthen nurse capacity at a national referral hospital and university in Tanzania. The goal of the intervention was to provide culturally competent nurse training in pediatric and adult hematology. A certified pediatric nurse practitioner with hematology and oncology experience provided culturally competent training and staff development to nurses over two weeks to meet this goal. Prior to development of a training schedule, nurses confidentially identified five of their top learning needs. Main hematology and basic oncology educational needs identified by nurses were the management of anemia, safe handling of cytotoxic agents, and treatment of SCD. The format of the education varied from bedside teaching to formal presentations to one-on-one individual discussions. Overall, nurses expressed satisfaction with the education and verbalized appreciation for teaching and training activities tailored to meet their needs. Specialized training in hematology and hemato-oncology has the potential to increase nurses' confidence, respect, and participation in interprofessional team decision-making. Lessons learned from the impact of collaborative nurse education and partnership in Tanzania can be generalized to other LMICs. This community case study highlights the importance of specialty nurse education, interprofessional development, and global partnerships needed to improve patient outcomes.

Abstract:Education of the pediatric oncology workforce is an important pillar of the World Health Organization CureAll technical package. This is not only limited to healthcare workers, but all stakeholders in the childhood cancer management process. It includes governmental structures, academic institutions, parents and communities. This review evaluated the current educational and advocacy training resources available to the childhood cancer community, the contribution of SIOP Africa in the continental educational needs and evaluated future needs to improve the management of pediatric malignancies in reaching the Global Initiative for Childhood Cancer goals. Childhood cancer, unlike adult cancers, has not been prioritized in African cancer control plans nor the teaching and advocacy surrounding pediatric oncology. The availability of formal training programs for pediatric oncologists, pediatric surgeons and radiotherapy specialists are limited to particular countries. In pharmacy and nutritional services, the exposure to pediatric oncology is limited while training in advocacy doesn't exist. Many nonacademic stakeholders are creating the opportunities in Africa to gain experience and train in these various fields, but formal training programs should still be advocated for. The African continent has various resources to increase the capacity of childhood cancer care stakeholders to increase their knowledge. African pediatric oncology teams rely on a multitude of international sources for training while developing their own. There is a greater need for formal, standardized cancer training especially for pediatric surgeons, radio-oncologists and nurses. Greater inclusion of pathologists, pediatric oncology pharmacists and dieticians into multidisciplinary care and childhood cancer training should be facilitated and resourced. Successful advocacy programs and tool kits exist in parts of Africa, but the training in advocacy is still underdeveloped.

Transfusion medicine education delivery in Rwanda: Adapting Transfusion Camp to a resource-limited setting

Abstract:Background: Due to few teaching faculty, resource-limited settings may lack the education curricula providers need for safe practice. As safe surgery becomes an increasing priority worldwide, it is essential to improve access to critical education content including in transfusion medicine. Transfusion Camp is a longitudinal curriculum, shown to increase knowledge in postgraduate trainees. The objective was to develop a sustainable bilateral partnership between Rwanda and Canada, and to integrate Transfusion Camp into the existing curriculum of the School of Medicine and Pharmacy at University of Rwanda. Methods: A Transfusion Camp pilot course was initiated through collaboration of experts in Rwanda and Canada. Planning occurred over 6 months via online and in-person meetings. Canadian teaching faculty adapted course content via iterative discussion with Rwandan faculty. Final content was delivered through online pre-recorded lectures by Canadian Faculty, and in-person small-group seminars by Rwandan Faculty. Project feasibility was assessed through structured evaluation and informal debriefing. Results: Twenty-seven postgraduate trainees were present for the pilot course, of whom 21 (78%) submitted evaluation forms. While the structure and content of the adapted Transfusion Camp curriculum were well-received, the majority of respondents indicated a preference for in-person rather than pre-recorded lectures. Debriefing determined that future courses should focus on continuing education initiatives aimed at physicians entering or already in independent practice. Conclusion: A partnership between universities and blood operators in high-resource and resource-limited countries results in a transfusion medicine curriculum that is locally applicable, multidisciplinary, and supportive of learning benefitting the learners and educators alike.

Abstract:Introduction: Over two million stillbirths and neonatal deaths occur in sub-Saharan Africa (sSA) annually. Despite multilateral efforts, reducing perinatal mortality has been slow. Although targeted pathologic investigation can often determine the cause of perinatal death, in resource-limited settings, stillbirths, early neonatal deaths, and placentas are rarely examined pathologically. However, the placenta is a key source of diagnostic information and is the main determinant of fetal growth and development in utero, influencing child health outcomes. Methods: In 2016, our collaborative intercontinental group began investigating infectious perinatal death and adverse child health outcomes in Uganda. We developed and initiated a 4-day combined didactic/practical curriculum to train health workers in placental collection, gross placental examination, and tissue sampling for histology. We also trained a local technician to perform immunohistochemistry staining. Results: Overall, we trained 12 health workers who performed gross placental assessment for > 1,000 placentas, obtaining > 5,000 formalin-fixed tissue samples for research diagnostic use. Median placental weights ranged from 425 to 456 g, and 33.3% of placentas were < 10th percentile in weight, corrected for gestational age. Acute chorioamnionitis (32.3%) and maternal vascular malperfusion (25.4%) were common diagnoses. Discussion: Through a targeted training program, we built capacity at a university-affiliated hospital in sSA to independently perform placental collection, gross pathologic examination, and placental tissue processing for histology and special stains. Our training model can be applied to other collaborative research endeavors in diverse resource-limited settings to improve research and clinical capacity and competency for diagnostics and management of stillbirth, neonatal death, and child health outcomes.
Enhancing HIV Treatment Access and Outcomes Amongst HIV Infected Children and Adolescents in Resource Limited Settings

Abstract:Introduction Increasing access to HIV-related care and treatment for children aged 0-18 years in resource-limited settings is an urgent global priority. In 2011-2012 the percentage increase in children accessing antiretroviral therapy was approximately half that of adults (11 vs. 21 %). We propose a model for increasing access to, and retention in, paediatric HIV care and treatment in resource-limited settings. Methods Following a rapid appraisal of recent literature seven main challenges in paediatric HIV-related care and treatment were identified: (1) lack of regular, integrated, ongoing HIV-related diagnosis; (2) weak facility-based systems for tracking and retention in care; (3) interrupted availability of dried blood spot cards (expiration/stock outs); (4) poor quality control of rapid HIV testing; (5) supply-related gaps at health facility-laboratory interface; (6) poor uptake of HIV testing, possibly relating to a fatalistic belief about HIV infection; (7) community-associated reasons e.g. non-disclosure and weak systems for social support, resulting in poor retention in care. Results To increase sustained access to paediatric HIV-related care and treatment, regular updating of Policies, review of inter-sectoral Plans (at facility and community levels) and evaluation of Programme implementation and impact (at national, subnational, facility and community levels) are non-negotiable critical elements. Additionally we recommend the intensified implementation of seven main interventions: (1) update or refresher messaging for health care staff and simple messaging for key staff at early childhood development centres and schools; (2) contact tracing, disclosure and retention monitoring; (3) paying particular attention to infant dried blood spot (DBS) stock control; (4) regular quality assurance of rapid HIV testing procedures; (5) workshops/meetings/dialogues between health facilities and laboratories to resolve transport-related gaps and to facilitate return of results to facilities; (6) community leader and health worker advocacy at creches, schools, religious centres to increase uptake of HIV testing and dispel fatalistic beliefs about HIV; (7) use of mobile communication technology (m-health) and peer/community supporters to maintain contact with patients. Discussion and Conclusion We propose that this package of facility, community and family-orientated interventions are needed to change the trajectory of the paediatric HIV epidemic and its associated patterns of morbidity and mortality, thus achieving the double dividend of improving HIV-free survival.

Advancing Leukemia Care and Research in East Africa:Current Challenges and Future Directions

Abstract:leukemia, encompassing a range of hematological malignancies, presents a significant health challenge in East Africa with distinct epidemiological patterns influenced by age, gender, and geography. Acute Lymphoblastic Leukemia (ALL) predominates in children, while Acute Myeloid Leukemia (AML) is more common in adults, especially the elderly. Chronic Lymphocytic Leukemia (CLL) and Chronic Myeloid Leukemia (CML) exhibit varied prevalence, with CLL being more common in older adults and CML showing lower incidence overall. Despite advancements, leukemia care in East Africa is impeded by limited diagnostic and treatment resources, socio-economic barriers, and inadequate healthcare infrastructure, resulting in lower survival rates compared to high-income countries. Challenges include delayed diagnoses, limited access to advanced treatments, and poor awareness of the disease. Emerging technologies such as targeted therapies and immunotherapy, alongside improved diagnostic tools and public awareness campaigns, offer hope for transforming leukemia care. This review explores the current state of leukemia care and research in East Africa, examining the genetic and environmental risk factors, healthcare infrastructure, survival rates, and socioeconomic impacts. It also highlights the role of public awareness and education, the challenges in pediatric leukemia care, and the importance of research and policy development. Future directions include strengthening healthcare systems, enhancing regional and international collaborations, and integrating advanced technologies to improve outcomes for leukemia patients in the region. Addressing these challenges with a comprehensive approach could significantly enhance leukemia care and research, ultimately improving survival rates and quality of life for patients in East Africa. Evaluation of two acute cardiac ischemia decision-support tools in a rural family practice.

Abstract:This study investigated the applicability in a primary care setting of two decision-support tools for evaluating the necessity of admitting patients with suspected acute cardiac ischemic disease to a cardiac intensive care unit. The heart disease predictive instrument (HDPI) of Pozen et al and the electrocardiogram scoring method of Brush et al were applied to records from all patients with cardiac-related diagnoses admitted to a family practice service. A retrospective review of medical records of all patients admitted with suspected acute ischemic heart disease to a rural community hospital family practice service was conducted. Of 147 patients identified, 108 were admitted primarily for suspected ischemia. Twenty-four myocardial infarctions occurred among these 108 patients (22.2 percent). Patients with HDPI probabilities of less than 50 percent were very unlikely to sustain infarction and in no case required intensive intervention for any problem not apparent at admission. Only 15 instances of the four types of complications that the instrument of Brush et al is designed to predict occurred; this incidence was too low to allow statistical testing. In summary, the heart disease predictive instrument reliably identified patients unlikely to require intensive care services in this population. Because of the low incidence of complications in this population, the instrument described by Brush et al was not found to be clinically useful.

Abstract:Transfusion demand in sub‐Saharan Africa is increasing. Establishment of large national blood services seems to be challenging. Hospital blood services maintain the basic supply. However, they lack effective communication networks. Cell phones/internet provides the infrastructure for establishing such networks and could help in establishing haemovigilance systems. Voluntary non‐remunerated donors are the minority of donors (<20%). It seems more appropriate to develop strategies to increase the safety of replacement donor blood instead of insisting on copying Western‐world models. Red cell serology is usually restricted to ABO/Rh‐D and at best Coombs crossmatch. Introduction of antibody differentiation and additional typing for C,c,E,e and K would prevent 60–70% of delayed haemolytic transfusion reactions. One of the most frequent indications for blood transfusion is major haemorrhage. Why is component therapy still recommended for resource‐limited regions, while whole blood is reintroduced in Europe/US for exactly the same indications? Plasma thawed due to electricity failures is usually discarded anyway. Leuco‐depleted products would be beneficial (multitransfused sickle‐cell patients/neonates/AIDS). An ideal blood product could be whole blood, in‐line leuco reduced by gravity filtration, treated by user‐friendly pathogen‐reduction not requiring irradiation/illumination. The transfusion medicine community should form an alliance to develop an affordable, pathogen‐reduced blood product, to train regional auditors and to bring in external expertise to aid knowledge transfer. However, standards to be reached should be defined by the respective national experts who understand the culture and needs of their country. In respectful collaboration, we could learn from each other to finally improve transfusion in resource‐limited regions.
Inhibitory effect of onion extract on cassava leaf (Manihot esculenta Crantz) polyphenol oxidase.

Abstract:The inhibitory effect of onion extract on cassava leaf polyphenol oxidase was investigated. The polyphenol oxidase from cassava leaves was strongly inhibited by various anti-browning agents such as L-ascorbic acid and L-cysteine. The percentage of inhibition increased with the increased of anti-browning agents concentrations. The addition of heated onion extract exhibited a stronger inhibitory effect on cassava leaf polyphenol oxidase than the fresh onion extract. The highest percentage of inhibition was exhibited with heated onion extract in the presence of glucose and glycine, which was 87.18%. The onion extract inhibited the cassava leaf polyphenol oxidase non-competitively.
African-led health research and capacity building- is it working?

Abstract:Abstract Background Africa bears a disproportionately high burden of globally significant disease but has lagged in knowledge production to address its health challenges. In this contribution, we discuss the challenges and approaches to health research capacity strengthening in sub-Saharan Africa and propose that the recent shift to an African-led approach is the most optimal. Methods and findings We introduce several capacity building approaches and recent achievements, explore why African-led research on the continent is a potentially paradigm-shifting and innovative approach, and discuss the advantages and challenges thereof. We reflect on the approaches used by the African Academy of Sciences (AAS)-funded Sub-Saharan African Network for TB/HIV Research Excellence (SANTHE) consortium as an example of an effective African-led science and capacity building programme. We recommend the following as crucial components of future efforts: 1. Directly empowering African-based researchers, 2. Offering quality training and career development opportunities to large numbers of junior African scientists and support staff, and 3. Effective information exchange and collaboration. Furthermore, we argue that long-term investment from international donors and increasing funding commitments from African governments and philanthropies will be needed to realise a critical mass of local capacity and to create and sustain world-class research hubs that will be conducive to address Africa’s intractable health challenges. Conclusions Our experiences so far suggest that African-led research has the potential to overcome the vicious cycle of brain-drain and may ultimately lead to improvement of health and science-led economic transformation of Africa into a prosperous continent.

Abstract:The effects of some exogenous peripheral hormones (thyroxine, corticosterone, epinephrine, norepinephrine and insulin) on thyroid activity were investigated in juvenile female soft-shelled turtles, Lissemys punctata punctata. Each hormone was injected in three different doses (25 microg, 50 microg or 100 microg each per 100 g body weight, once daily at 9 AM) for 10 consecutive days. Thyroid activity was evaluated by gravimetry, histology (epithelial height) and thyroperoxidase assay. The findings revealed that thyroxine in low dose (25 microg) stimulated thyroid activity by increasing the relative thyroid weight, epithelial height and thyroperoxidase activity, but inhibited gland activity at a high dose (100 microg) by decreasing the values of all these parameters. The medium dose (50 microg) had no significant effect. All other hormones, in all doses, significantly decreased thyroid activity by decreasing the values of all the parameters. Thyroid responses to exogenous hormones are generally dose-dependent in turtles. The mechanisms of actions of the hormones administered are suggested.
Comparison of inhibitory effects of warfarin on gamma-carboxylation between bone and liver in rats.

Abstract:The purposes of this study were to clarify that warfarin (WF, vitamin K antagonist) levels that inhibit gamma-carboxylation are different in liver and bone (experiment 1), and to investigate whether the plasma osteocalcin (OC) level reflects bone OC levels (experiments 2 and 3). Four-week-old male rats were treated with 0.2, 0.4, 0.6, 0.8, 1.0, or 1.2 mg/l of WF solution as drinking water for 4 weeks. Blood coagulation activity, an index of gamma-carboxylation of prothrombin in the liver, was significantly decreased in rats receiving 0.8 mg/l or larger doses of WF. A significant decrease of plasma gamma-carboxylated OC (GlaOC), an index of gamma-carboxylation of OC in bone, was shown in rats receiving 0.2 mg/l or larger doses. Significantly lower OC levels in the femoral diaphysis and metaphysis were shown in the 0.2 mg/l and 0.4 mg/l groups. However, femoral bone mineral density (BMD) values in the WF-treated groups were almost the same as those in the intact group. In experiment 2, we evaluated changes in bone OC levels 4 weeks after discontinuing an 8-week WF treatment. Four-week-old male rats received 0.8 mg/l WF as drinking water for 8 or 12 weeks. Recovery of the OC level after discontinuing the WF treatment was shown in the femoral metaphysis, but not in the diaphysis. In experiment 3, 0.3 mg/kg WF was administrated to 25-week-old male rats three times a week for 8, 12, or 16 weeks. In aged rats, decreased bone OC was shown in the femoral metaphysis, but not in the diaphysis. From these findings, it is suggested that the effects of WF on gamma-carboxylation are likely to appear in bone at lower doses than in the liver, that the bone OC level does not always correspond directly to plasma GlaOC, and that the bone OC level is not directly linked with BMD.
Does breast cancer in Asian geriatric patients have the same biological characteristics as in their Western counterparts? A comparison between Shanghai and Vienna.

Abstract:1550 Background: By 2030 it is estimated that 20% of the population will be 75 years of age or older. The incidence of breast cancer in Asia is especially rising, while the proportion of the population that is 60 and older is also growing. Today, approximately one-third of breast cancer occurs in women over the age of 65. The aim of this study was to investigate the biological differences of breast cancer older than 69 years in Asia and Europe. The results, including the stage at diagnosis and surgical therapy of Chinese breast cancer, were compared with their western counterparts. METHODS A total of 630 women, 70 years of age and older, with operable breast cancer were investigated in Shanghai and Vienna. Histopathological findings of 198 Austrian women seen during 2005 and 2010 were reviewed. The results were compared with 432 Chinese (time matched) obtained from the Breast Cancer Database in Shanghai. RESULTS The mean patients' age in the Viennese and Shanghainese study groups were 75.3 and 77.9 years, respectively. In Austria, the breast-conserving surgery rate was 73.7% (n=146) compared to 10.4% (n=45) in China. Sentinel lymph node biopsy was performed in 40.4% (n=80) in Vienna vs. 8.3% (n=36) in Shanghai. More large tumors (tumor size > 2 cm) were detected in China (41.3%, n=169) than in Austria (34.7%, n=66) (p=0.033). No differences were found in axillary lymph node involvements (p=0.052). However, more grade-3 breast cancers were detected (p<0.001) in Austrians. Furthermore, there was a greater incidence of estrogen receptor-negative breast cancer in Chinese women (27%, n=113) compared to Austrian women (16.2%, n=32) (p<0.001). CONCLUSIONS Chinese women present with a lower rate of grade-3 breast malignancies. As expected, a higher breast conserving surgery rate was noted in Vienna. Interestingly less Chinese women, although older than 69 years, were presented with estrogen receptor-positive breast cancer. We conclude that breast cancer characteristics are different in diverse ethnic groups. Ethnic-specific screening/therapy protocols may benefit this special Asian population, especially Asian women in western countries.
Current Diagnosis of Bleeding Disorders in Lower Income Countries

Abstract:There have been considerable advances in diagnosing and treating bleeding disorders. But the scenario remains dismal in resource‐constrained settings in low and lower‐middle‐income countries (LMICs). Seventy‐five percent of the patients with inherited bleeding disorders do not get diagnosed in LMICs. In resource‐constrained settings, infectious disease and malignancies take the major focus. Bleeding disorders do not get prioritised in LMICs, and this leads to underdiagnoses and suboptimal treatment. There are various challenges like financial status, inadequacy of health care infrastructure, lack of patient registry and lack of awareness across medical staff, general population and government stakeholders. The lack of skilled laboratory personnel and laboratory infrastructure for optimal bleeding disorder diagnosis adds on to the problem. World Federation of Hemophilia (WFH) has been at the forefront in developing strategies to overcome some of these inadequacies; however, more active participation of the stakeholders including patients, medical professionals and policy makers is the need of the hour. This review highlights the different challenges in LMICs in diagnosing bleeding disorders, the gap between high‐income countries and LMICs and the possible strategies in closing the gap.

Abstract:good references for learning about the program, but given their second goal, expanding this appendix would make future editions of the book more effective. An obvious comparison for this book is the text by Venables and Ripley (2002), which is an invaluable resource to the S, R, and S-PLUS user community. Although Martinez and Martinez have not achieved the standard set by that book, they have made a very good start, and have positioned themselves to become the Venables and Ripley of the MATLAB user community. Any MATLAB user who is serious about statistics (as they all should be, in my opinion) will want a copy of this book and the software that it describes.
Treating Rare Diseases in Africa: The Drugs Exist but the Need Is Unmet

Abstract:Rare diseases (RD) pose serious challenges in terms of both diagnosis and treatment. Legislation was passed in the US (1983) and in EU (2000) aimed to reverse the previous neglect of RD, by providing incentives for development of “orphan drugs” (OD) for their management. Here we analyse the current situation in Africa with respect to (1) sickle cell disease (SCD), that qualifies as rare in the US and in EU, but is not at all rare in African countries (frequencies up to 1–2%); (2) paroxysmal nocturnal haemoglobinuria (PNH), that is ultra-rare in Africa as everywhere else (estimated <10 per million). SCD can be cured by bone marrow transplantation and recently by gene therapy, but very few African patients have access to these expensive procedures; on the other hand, the disease-ameliorating agent hydroxyurea is not expensive, but still the majority of patients in Africa are not receiving it. For PNH, currently most patients In high income countries are treated with a highly effective OD that costs about $400,000 per year per patient: this is not available in Africa. Thus, the impact of OD legislation has been practically nil in this continent. As members of the medical profession and of the human family, we must aim to remove barriers that are essentially financial: especially since countries with rich economies share a history of having exploited African countries. We call on the Global Fund to supply hydroxyurea for all SCD patients; and we call on companies who produce ODs to donate, for every patient who receives an expensive OD in a high income country, enough of the same drug, at a symbolic price, to treat one patient in Africa.

Abstract:Each year, 5.3 million children under 5 years of age die in low-resource settings, often due to delayed recognition of disease severity, inadequate treatment, or a lack of supplies. We describe the use of a comprehensive digital facility-readiness survey tool, recently developed by the Pediatric Sepsis Data CoLaboratory, which aims to identify target areas for quality improvement related to pediatric emergency and critical care.

Abstract:BACKGROUND Several years ago, the razor blade was introduced as an alternative device for performing the shave removal and/or biopsy of skin lesions. Though inexpensive and readily available, the razor blade is somewhat cumbersome to use and poses a risk for accidental injury to the practitioner. OBJECTIVE We describe a modification of the razor blade shave biopsy procedure utilizing an innovative adaptor design. METHODS A comparison of the adaptor‐designed shave biopsy instrument to the standard razor blade device is made. RESULTS The modifications implemented in the adaptor design serve to enhance the comfort of performing biopsies, minimize accidental injury, help save time, and provide for a more professional appearing way in which to obtain skin specimens. CONCLUSION The new adaptor design helps to facilitate the safe and effective use of the razor blade in dermatologic practice.
