Abstract:Objective: We report two rare cases of lung metastasis from maxillary ameloblastoma, in order to review its risk and analyse the types of metastases that can present with this disease.Methods: A 40-year-old male with multiple recurrences and a 46-year-old female, who had undergone successful surgical treatment of a maxillary ameloblastoma, presented with metastatic lesions. The primary tumour and metastases were benign in both patients. We reviewed and analysed 20 cases of the same condition reported in recent years.Results: Our initial treatment for the primary maxillary lesion was performed more than 10 years before the pulmonary lesions presented. Due to the aggressive nature of this tumour, metastases in the lungs and cervical lymph nodes (male patient) were confirmed.Conclusion: These cases presented a diagnostic challenge due to the multiple and varied sites of recurrence, which indicate the natural behaviour of this tumour. Different routes of metastasis can occur, including implanting, haematogenous, and lymphatic spread. CT-guided percutaneous transthoracic lung biopsy is an important method to confirm metastatic ameloblastoma. (C) 2013 European Association for Cranio-Maxillo-Facial Surgery. A Retrospective Study of 158 Cases on the Risk Factors for Recurrence in Ameloblastoma

Abstract:Background: Ameloblastoma is an odontogenic tumor occurring in jaws, with local aggressiveness and postoperative recurrence. This study was aim to investigate the clinical and radiographic risk factors for recurrence in ameloblastoma. Methods: Patients diagnosed with ameloblastoma between March 2009 and March 2019 were retrospectively analyzed. Clinical and Radiological data and follow-up records were collected. Survival analyses were performed by Kaplan-Meier and log-rank tests, as well as Cox proportional hazards model. Results: One hundred and fifty-eight patients (104 males and 54 females were enrolled. The overall recurrence rate for ameloblastoma was 13.29%, and 10.76% recurred within 5 years. Most of the tumors were located in mandible (86.71%), while the rest 21 cases were in maxilla (13.29%). More than half cases (55.06%) showed multilocular radiolucency, 61 cases (38.61%) showed unilocular radiolucency. Significant differences were found with amelobastoma recurrence rate related to treatment modality, impacted tooth and root resorption (P =0.002, 0.022 and 0.007 respectively). Conclusions: Treatment modality, impacted tooth and root resorption all showed statistically significant associations with the recurrence rate in ameloblastoma. However, due to the limitation of this study, further studies are needed to reveal the true mechanism of ameloblastoma recurrence.

Abstract:BACKGROUND Ameloblastoma is a locally aggressive, benign, odontogenic tumor. Reports suggest that the chances of recurrence of this tumor are high if treated with a conservative approach. Concordantly, surgical removal of the lesion along with the affected adjacent tissues and bone structure is recommended to reduce the chances of recurrence. Post-surgical prosthetic rehabilitation is advised to improve speech, mastication, and aesthetic appearance. This case report highlights the treatment and reconstruction challenges that maxillofacial surgeons and their teams face in managing cases of large ameloblastoma. CASE REPORT A 41-year-old Sudani man was referred for the management of a large ameloblastoma associated with the left border of the mandible. Management consisted of surgical removal of the affected mandible along with prosthetically preserving the mandible with grafts and screws. Histopathological, computed tomography, and incisional biopsy evaluation confirmed the presence of ameloblastoma. Postoperatively, no complications were reported. Six months postoperatively, no sign of recurrence was seen. The patient was referred to a surgeon for placement of an endosseous implant. CONCLUSIONS When dealing with large ameloblastoma, an interdisciplinary dental team is essential for improving the treatment results. This case highlights the importance of precise and timely primary care diagnosis and a collaborative approach to treatment. By embracing advancements in digital technologies, surgeons can enhance functional and aesthetic results, improving long-term quality of life.
Carbon Ion Radiotherapy for Recurrent Ameloblastoma: A Case Report

Abstract:Ameloblastoma is a kind of benign, odontogenic tumor of epithelial origin, and surgery is mainstay treatment method; however, recurrence is common, and usually the treatment for recurrence is still surgery. We report on a patient of recurrent ameloblastoma treated with carbon ion radiation therapy and achieved a good efficacy. A 25-year-old female with relapse of an ameloblastoma was referred to the Wuwei Heavy Ion Center for carbon ion therapy. She had been initially diagnosed with ameloblastoma 8 years ago and underwent operation of right mandible ameloblastoma. After she transferred to our center, she accepted a dose of 60 GyE carbon ion radiation therapy, and the efficacy is good. Carbon ion radiation therapy can be an effective treatment option for ameloblastoma.

Abstract:Ameloblastoma is a highly recurrent odontogenic neoplasm with variable global distribution. However, impact of race and ethnicity on ameloblastoma recurrence are still unclear. The primary aim of this study was to assess duration of time between primary and recurrent ameloblastomas in a predominantly Black multi-institutional patient cohort and secondarily to determine whether recurrent ameloblastomas are more readily discovered when clinically-symptomatic rather than by radiographic surveillance. A retrospective cross-sectional design was used to evaluate demographic, clinical, and pathological information on recurrent ameloblastomas patients. Outcome variable was time to recurrence, determined as period between the diagnosis of primary and recurrent ameloblastomas. We assessed associations between outcome variable and race, time lapse between primary and recurrent ameloblastomas and clinical symptoms of recurrent ameloblastomas at time of diagnosis. Among 115 recurrent ameloblastomas identified, 90.5% occurred in adults, 91.3% in Blacks, and similarly, 91.3% were conventional ameloblastomas. About 41% affected the posterior mandible. 93.9% were clinically symptomatic at time of presentation while 6.1% non-symptomatic lesions were discovered by routine diagnostic radiology. Median time to presentation of recurrent tumor was significantly longer in females (90 months, p = 0.016) and clinically symptomatic group of ameloblastoma patients (75 months, p = 0.023). Ameloblastoma recurrence was distinctively high in Black patients, occurred faster in males than females and was located mostly in the posterior mandible. Concomitant with delayed access to healthcare of Black individuals, routine post-surgical follow-up is essential because time lag between primary and recurrence tumors was longer in clinically symptomatic ameloblastomas at the time of diagnosis.

Abstract:Recurrent ameloblastomas rarely present in the anterior skull base but are, common following inadequate excision.Three patients are presented with ameloblastoma in the anterior skull base including frontotemporal fossa and pterygomaxillary fossa recurring following multiple enucleations, segmental mandibulectomy, or partial maxillectomy. The recurrences were found 3-4 years following the primary operations. All 3 patients were treated by radical dissection.Microscopically one case was a mixed follicular and plexiform, two cases were of a follicular type. All patients healed without serious complications.Ameloblastomas in these regions have a greater recurrence potential even when treated by radical dissection.

Abstract:BACKGROUND:To investigate the clinicopathological features of six cases of soft tissue recurrent ameloblastoma and explore the role of increased aggressive biological behavior in the recurrences and treatment of this type of ameloblastomas.MATERIAL AND METHODS:In this study, we retrospectively reviewed recurrent ameloblastomas during a 15-year period; six cases were diagnosed as soft tissue recurrent ameloblastoma. The clinical, radiographic, cytological and immunohistochemical records of these six cases were investigated and analyzed.RESULTS:All the six soft tissue recurrent ameloblastomas occurred after radical bone resection, and were located in the adjacent soft tissues around the osteotomy regions. In Case 4, the patient developed pulmonary metastasis, extensive skull-base infiltration and cytological malignancy after multiple recurrences and malignant transformation was diagnosed. In the other five cases, although there were no cytological signs are sufficient to justify an ameloblastoma as malignant, some malignant features were observed. In Case 1, the tumor showed moderate atypical hyperplasia and the Ki-67 staining percentage was 40% positive, which are strongly suggestive of potential malignance. In Case 5, the patient developed a second soft tissue recurrence in the parapharyngeal region and later died of tumor-related complications. All the remaining three patients showed cytology atypia of varying degrees and high expression of PCNA or Ki-67, which confirmed active cell proliferation.CONCLUSIONS:Increased aggressiveness is an important factor of soft tissue recurrence. An intraoperative rapid pathological examination and more radical treatment are suggested for these cases.
Recurrence factors in pediatric ameloblastoma: Clinical features and a new classification system.

Abstract:Background Ameloblastomas of jaw in the pediatric population are a rare clinical entity and have not been well addressed in the literatures. The present retrospective study analyzed the risk factors associated with recurrence of pediatric ameloblastomas. Methods Cases of primary pediatric ameloblastomas seen in a tertiary hospital between 2005 and 2015 were analyzed to identify the clinical factors associated with recurrence. Results There were a total of 104 cases of primary pediatric ameloblastomas. The overall mean maximum tumor diameter was 4.11 +/- 1.339 cm. The receiver operating characteristic curve and the Youden Index showed an optimal cutoff point of 4.95 cm to accurately predict recurrence. Bone cortex/soft tissue invasion were associated with tumor recurrence (P < .001). Conclusions The maximum tumor diameter, root resorption, and bone cortex/soft tissue invasion were risk factors for recurrence of pediatric ameloblastomas. The new classification system may serve as a predictor of recurrence in pediatric ameloblastomas.

Abstract:Objective: To study the clinic features,radiology,pathology,and diagnosis of recurrent ameloblastoma accompanied metastasis to the lung.Methods: A case of recurrent ameloblastoma accompanied metastasis to the lung was retrospected,the comprehensive diagnosis,treatment and prognosis were analyzed.Results: The patient suffered from recurrent ameloblastoma for several times,extensive maxillofacial bone and soft tissues were involved.The radiology showed involvement of fundus cranii and metastasis to the lung.The pathology showed rapid growth of ameloblasts.The immunohistology results demonstrateded PCNA(+)and nm23(-).Conclusion: The pathology of recurrent amelobalstoma shows rapid growth of ameloblasts and infiltration of peripheral tissues,extensive maxillofacial regions are involved and even metastasis could be found,the high expression of PCNA and low expression of nm23 may relate to recurrence and metastasis.
Recurrence and Cancerization of Ameloblastoma: Multivariate Analysis of 87 Recurrent Craniofacial Ameloblastoma to Assess Risk Factors Associated with Early Recurrence and Secondary Ameloblastic Carcinoma.

Abstract:OBJECTIVE:The recurrence and progression of ameloblastoma are unpredictable. Therefore, we examined the influence of clinical factors on recurrence time and analyzed the clinical factors associated with early recurrence and cancerization. We then developed a staging system to predict early recurrence and cancerization.METHODS:All of the primary craniofacial ameloblastoma patients treated in Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine were recorded. There were 87 recurrent cases used to create a staging system and tested in a Cox regression analysis for risk factors associated with early recurrence or cancerization following surgery.RESULTS:There were 890 craniofacial ameloblastoma patients, and 72 cases had recurrence. There were also 15 cases with cancerous recurrence. The overall recurrence rate was 9.78%, and the cancer rate was 1.69%. The primary cases were classified into the following 3 stages based on clinicopathological features: stage I, the maximum tumor diameter ≤6 cm; stage II, the maximum diameter of tumor >6 cm or tumor invasion to the maxilla sinus/orbital floor/soft tissue; and stage III, tumor invasion of the skull base or metastasis into regional lymph nodes. When the method of surgery was controlled by partial correlation, the staging had significance with recurrence time (P=0.004). The Cox analysis showed the tumor stage was correlated with recurrence time (P=0.027) and cancerization time (P=0.002). However, the surgical method did not influence the recurrence time when adjusted for cofounding variables.CONCLUSIONS:Tumor larger than 6 cm and invasion to soft tissues or adjacent anatomical structures are associated with early recurrence. This staging system can be used to predict the risk factors of early recurrence and cancerization in ameloblastoma patients.

Abstract:Ameloblastoma is the most common odontogenic tumor known for its slow and invasive growth, sometimes reaching substantial sizes. The patient, ERS, a 60-year-old individual, had previously undergone surgery for ameloblastoma approximately 10 years ago. At that time, the lesion had occupied a significant portion of the mandible, and a partial resection was performed, along with the installation of a reconstruction plate. Around six months ago, the patient experienced a recurrence of the lesion, which measured approximately 15 cm in its largest diameter.

Desmoplastic Ameloblastoma: A case report of rare pulmonary metastasis

Abstract:Pulmonary metastasis of ameloblastoma is a rare associated with the histopathologically plexiform types of ameloblastoma. In this report, we present an exceptionally rare case of pulmonary metastatic ameloblastoma without local recurrence, emerging 12 years post-initial resection. A female patient, initially diagnosed with mandibular desmoplastic ameloblastoma, revealed masses in both lung fields of the lung on chest radiography, while chest computed tomography revealed more than 10 nodules in both lungs. Histopathological analysis of a lung biopsy specimen revealed a nest-like structure of tumor cells surrounded by highly columnar cells, exhibiting a palisade-like arrangement, and squamous metaplasia. Subsequently, the diagnosis of pulmonary metastatic ameloblastoma was confirmed through immunohistochemical examination. Despite clinical trial treatment with molecular targeted therapy for three years, the lung disease remained clinically stable. This case highlights the need for oral surgeons to recognize that pulmonary metastasis of desmoplastic ameloblastoma can manifest as a late recurrence, extending beyond a decade after initial curative surgery.
Analysis of risk factors and survival for recurrence of ameloblastoma

Abstract:Objective:Analyze the risk factors of ameloblastoma (AM) postoperative recurrence to provide reference for the treatment of AM.Methods:The basic information of AM patients who had a clear pathological diagnosis in the Department of Oral and Maxillofacial Surgery of the First Affiliated Hospital of Zhengzhou University from June 2012 to October 2020 were collected. The demographic characteristics, clinical characteristics, and risk factors related to recurrence of the patients were analyzed. The independent sample t test was used for the age difference between different groups. The difference between the recurrence group and the non-recurrence group was analyzed by the χ2 test, and the ROC analysis was used to obtain the best cut-off value of age. Logistic regression was used to analyze the risk factors for postoperative recurrence. Kaplan-Meier survival analysis was used to analyze the postoperative recurrence-free survival period. Cox regression analysis of risk factors affecting postoperative recurrence-free survival time. Results:In the study of 330 AM patients, there were 207 male patients (62.73%) and 123 female patients (37.27%), with an age of (33.44±15.41) years. Most of the tumors are located in the mandible (91.21%). A total of 125 patients have recurred, and the overall recurrence rate is 37.88%. The main pathological type was classic ameloblastoma (242/330, 73.33%). Logistic regression analysis indicated that conservative surgery (including curettage and excision) ( P<0.01) and the pathological type of classic ameloblastoma ( P<0.01) were independent risk factors for AM recurrence. Survival analysis results suggest that there is a significant statistical difference in recurrence-free survival time between non-classical AM and classic AM ( P<0.01); there are also significant statistics in recurrence-free survival time between radical surgery and conservative surgery Academic difference ( P<0.01). COX regression analysis showed that the operation method was an independent factor affecting postoperative recurrence-free survival time [hazard ratio ( HR)=0.287, 95% confidence interval ( CI): 0.177-1.467]; classic AM was also an independent factor affecting postoperative recurrence-free survival time ( HR=2.792, 95% CI: 1.759-1.432). The average recurrence period of 125 relapsed patients was 2.64 years. The postoperative recurrence period of non-classical AM was longer than that of classic AM ( P<0.05). The recurrence period of mandibular AM was longer than that of upper mandibular AM ( P<0.05). Among the relapsed patients, 9 patients (9/125, 7.2%) had malignant transformation, and 2 patients had metastasis. Conclusion:Surgical treatment of ameloblastoma has a higher recurrence rate. Conservative surgery and classic AM are independent risk factors for postoperative recurrence. Compared with radical surgery and non-classical AM, the recurrence-free survival time is shorter.
Recurrent benign cementoblastoma: A case report and literature review

Abstract:A 16-year-old male presented with pain in the right posterior mandible on chewing that had lasted for several months. The radiographic features of the lesion included a radiolucent-radiopaque mixed-density mass with a radiolucent rim attached to the root of the mandibular right first molar. The preliminary radiographic diagnosis was benign cementoblastoma, which was confirmed by histopathological examination following surgical excision. The lesion recurred 3 years after treatment; radiographically, it consisted of 3 round foci with mixed radiopacity, each with a radiolucent rim near the root of the mandibular right second premolar and the edentulous postoperative region. The lesion was diagnosed as recurrent benign cementoblastoma and a second surgery was scheduled. This report presented an unusual case of recurrent benign cementoblastoma following surgical excision and extraction of the involved tooth, along with a literature review on reported cases of recurrent benign cementoblastoma with a focus on its clinical features and the best treatment options.
The Effect of Conservative vs. Radical Treatment of Ameloblastoma on Recurrence Rate and Quality of Life: An Umbrella Review

Abstract:Ameloblastoma is a rare, benign, but locally aggressive odontogenic tumor that originates from the epithelial cells involved in tooth development. The surgical approach to treating an ameloblastoma depends on the type, size, location, and extent of the tumor, as well as the patient's age and overall health. This umbrella review's aim is to summarize the findings from systematic reviews (SRs) and meta-analyses on the effect of radical or conservative treatment of ameloblastoma on the recurrence rate and quality of life, to evaluate the methodological quality of the included SRs and discuss the clinical management. Three electronic databases (PubMed, Scopus, The Cochrane Library) were checked. The primary outcome was the recurrence rate after surgical treatment, while the secondary outcomes were the post-operative complications, quality of life, esthetic, and functional impairment. The methodological quality of the included SRs was assessed using the updated version of "A Measurement Tool to Assess Systematic Review" (AMSTAR-2). Eighteen SRs were included. The quality of the included reviews ranged from critically low (three studies) to high (eight studies). Four studies were included in meta-analysis, and they revealed that the recurrence rate is about three-times more likely in the conservative treatment group compared to the radical treatment group, and this result is statistically significant. Despite the high recurrence rate, the latter was more appropriate in the case of smaller lesions and younger patients, due to better post-operative quality of life and reduced functional and esthetic impairments. Based on the results of this overview, conservative treatment may be recommended as the first-line approach for intraosseous ameloblastoma not involving soft tissue. However, given the expectation of a higher recurrence rate, it is advisable to reduce the interval between follow-up visits. However, further prospective studies are needed to establish the best treatment choice and follow-up period.

Abstract:Ameloblastic carcinoma is a rare malignant neoplasm with characteristic histopathological features that are directed towards an aggressive surgical approach than benign odontogenic lesions. It affects people of all ages, mostly in the posterior mandible, without a preference for race or gender. De novo cancer is one of its primary types, while the second type is defined as a malignant change from an antecedent case of benign ameloblastoma. The rapid progression of molecular biology led to the revelation that ameloblastoma contains a BRAF-V600E genetic mutation over 60%. Besides conventional ameloblastic carcinomas, rare histologic variants have also been described in the literature, including clear and spindle cells. These variants pose diagnostic challenges as to whether it is a dedifferentiation or a distinct entity. The dearth of data lends credence to the notion that these histologic variations are related to high-grade neoplasms and more aggressive outcomes. As a result, the current report intends to analyze a series of patients diagnosed with conventional ameloblastic carcinoma of the head and neck region with spindle and clear cell types along with a brief assessment of the literature.

Abstract:Cementoblastoma is an odontogenic tumor of benign nature. It is of mesenchymal origin with a relatively low incidence of reappearance. Hereby we present a case report of a 14-year-old male patient with recurrent swelling on the right mandibular posterior region. Despite surgical removal of the tumor mass along with the right first molar, recurrence of the lesion was noticed. Hence, our report gives an insight on the treatment and management of the lesion by en-bloc resection on the right side of the mandible.

Abstract:Ameloblastoma is the most common odontogenic tumor of epithelial origin. These tumors are benign in nature but have a high rate of recurrence and possible malignant development when treated inadequately. However, recurrent ameloblastomas in bone grafts are rare. This article presents a rare case of a multiple recurrent ameloblastoma, with the last recurrence in a nonvascularized free iliac bone graft of the mandible, reconstructed with vascularized deep circumflex iliac artery free flap for the surgical defect. Furthermore, an updated review of the literature on recurrent ameloblastomas affecting the bone graft is provided.
