Abstract:Objective: To develop a breast cancer risk predictive model based on questionnaire survey data using artificial neural network (ANN) approach, and thus to provide an effective tool for initial screening of breast cancer. Methods: During the period of 2008-2012, an organized breast cancer screening project was conducted among 15 148 healthy women at age of 35-74 years in Minhang District of Shanghai, China. The information on demographic characteristics, reproductive factors, history of any breast diseases, and family history of breast cancer was collected by in-person interview using a structured questionnaire. Sixty-six breast cancer cases were identified through pathological examination. Logistic backward regression was used to select significant risk factors. An ANN model was developed and tested by Feed-forward Networks and limited memory Broyden-Fletcher-Goldfarb-Shanno (L-BFGS) method. Results: The variables including age, age at menarche, family history of breast cancer, breast lumps, nodules or thickening, years since first delivery, and days of fatty meat intake per week were included in ANN model. In the training set, the model achieved an accuracy of 66.5% [95% confidence interval (CI): 65.6-67.4], a sensitivity of 63.8% (95% CI: 50.1-77.6), a specificity of 66.5% (95% CI: 65.6-67.4), and the area under receiver operating characteristic curve (AUC) of 0.706 (95% CI: 0.635-0.777). In the test setting, the model had an accuracy of 64.9% (95% CI: 63.5-66.3), a sensitivity of 79.0% (95% CI: 60.6-97.3), a specificity of 64.8% (95% CI: 63.4-66.2) and an AUC of 0.762 (95% CI: 0.655-0.869). Conclusion: The ANN model based on questionnaire survey data has predictive value of breast cancer risk in Chinese women in Shanghai, and has potential to be used in risk self-assessment and preliminary screening in population. Identification of Risk Factors for Mortality Associated with COVID-19

Abstract:Objectives Coronavirus Disease 2019 (COVID-19) has become a pandemic outbreak. Risk stratification at hospital admission is of vital importance for medical decision making and resource allocation. There is no sophisticated tool for this purpose. This study aimed to develop neural network models with predictors selected by genetic algorithms (GA). Methods This study was conducted in Wuhan Third Hospital from January 2020 to March 2020. Predictors were collected on day 1 of hospital admission. The primary outcome was the vital status at hospital discharge. Predictors were selected by using GA, and neural network models were built with the cross-validation method. The final neural network models were compared with conventional logistic regression models. Results A total of 246 patients with COVID-19 were included for analysis. The mortality rate was 17.1% (42/246). Non-survivors were significantly older (median (IQR): 69 (57, 77) vs. 55 (41, 63) years; p < 0.001), had higher high-sensitive troponin I (0.03 (0, 0.06) vs. 0 (0, 0.01) ng/L; p < 0.001), C-reactive protein (85.75 (57.39, 164.65) vs. 23.49 (10.1, 53.59) mg/L; p < 0.001), D-dimer (0.99 (0.44, 2.96) vs. 0.52 (0.26, 0.96) mg/L; p < 0.001), and α-hydroxybutyrate dehydrogenase (306.5 (268.75, 377.25) vs. 194.5 (160.75, 247.5); p < 0.001) and a lower level of lymphocyte count (0.74 (0.41, 0.96) vs. 0.98 (0.77, 1.26) × 109/L; p < 0.001) than survivors. The GA identified a 9-variable (NNet1) and a 32-variable model (NNet2). The NNet1 model was parsimonious with a cost on accuracy; the NNet2 model had the maximum accuracy. NNet1 (AUC: 0.806; 95% CI [0.693–0.919]) and NNet2 (AUC: 0.922; 95% CI [0.859–0.985]) outperformed the linear regression models. Conclusions Our study included a cohort of COVID-19 patients. Several risk factors were identified considering both clinical and statistical significance. We further developed two neural network models, with the variables selected by using GA. The model performs much better than the conventional generalized linear models.
Predicting Depression, Anxiety, and Their Comorbidity among Patients with Breast Cancer in China Using Machine Learning: A Multisite Cross‐Sectional Study

Abstract:Depression and anxiety are highly prevalent among patients with breast cancer. We tested the capacity of personal resources (psychological resilience, social support, and process of recovery) for predicting depression, anxiety, and comorbid depression and anxiety (CDA) among such patients using machine learning (ML). We conducted a cross‐sectional survey in Liaoning Province, China, including questions about demographics, COVID‐19′s impact, and personal resources (707 valid responses). In the training set, we used Lasso logistic regression to establish personal resource models. Subsequently, we used six ML methods and a tenfold cross‐validation strategy to establish models combining personal resources, demographics, and COVID‐19 impacts. Findings indicate that in total, 21.9%, 35.1%, and 14.7% of participants showed depression, anxiety, and CDA, respectively. Loneliness, vitality, mental health, bodily pain, and self‐control predicted depression, anxiety, and CDA. Furthermore, general health predicted depression, and physical function predicted anxiety. Demographic and COVID‐19 models were far less predictive than personal resource models (0.505–0.629 vs. 0.826–0.869). Among combined models, the support vector machine model achieved the best prediction (AUC: 0.832–0.873), which was slightly better than the personal resource models. Personal resources features with ML and personal resources can help predict depression, anxiety, and CDA in patients with breast cancer. Accordingly, interventions should target loneliness, bodily pain, vitality, mental health, and self‐control.

Artificial neural networks for simultaneously predicting the risk of multiple co-occurring symptoms among patients with cancer

Abstract:Patients with cancer often exhibit multiple co-occurring symptoms which can impact the type of treatment received, recovery, and long-term health. We aim to simultaneously predict the risk of three symptoms: severe pain, moderate-severe depression, and poor well-being in order to flag patients who may benefit from pre-emptive early symptom management. This was a retrospective population-based cohort study of adults diagnosed with cancer between 2008 and 2015. We developed and tested an Artificial Neural Network (ANN) model to predict the risk of multiple co-occurring symptoms within 6 months after diagnosis. The ANN model derived from a training cohort was assessed on an independent test cohort for model performance based on sensitivity, specificity, accuracy, AUC, and calibration. The mutually exclusive training and test cohorts consisted of 35,606 and 10,498 patients, respectively. The area under the curve for the risk of experiencing severe pain, moderate-severe depression, and poor well-being were 71%, 73%, and 70%, respectively. Patient characteristics at highest risk of simultaneously experiencing these three symptoms included: those with lung cancer, late stage cancer, existing chronic conditions such as osteoarthritis, mood disorder, hypertension, diabetes, and coronary disease. Patients with over a 40% risk of severe pain also had over a 70% risk of depression, and over a 55% risk of poor well-being. Our ANN model was able to simultaneously predict the risk of pain, depression, and lack of well-being. Accurate prediction of future symptom burden can serve as an early indicator tool so that providers can implement timely interventions for symptom management, ultimately improving cancer care and quality of life.

Abstract:Objective To develop and evaluate a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy. Methods 647 patients who received chemotherapy and met the inclusion and exclusion criteria were selected at 4 tertiary hospitals in Shanghai,Shandong,Jiangsu,and Zhejiang from October 2019 to May 2021. Cases were randomly assigned to a modeling group and a validation group based on 5-fold cross-validation method at a ratio of 8:2. The random forest algorithm was used to develop the risk predictive model in the modeling group. The receiver operating characteristic curve,Hosmer-Lemeshow goodness-of-fit test,calibration curve,and decision curve were used to comprehensively evaluate the prediction performance of the model in the validation group. Risk factors were identified based on the order of the importance of each influencing factors. Results The incidence of high symptom cluster distress was 33.27% in the modeling group and 29.23% in the validation group. The area under the receiver operating characteristic curve of the prediction model was 0.91;the sensitivity was 65.8%;the specificity was 93.5%;Hosmer-Lemeshow goodness-of-fit test was insignificant（P =0.1365）;the decision curve was above the reference line. Body image,self-efficacy,and financial burden were the most important predictors.Conclusion The risk predictive model based on random forest algorithm has good predictive performance,which is of great significance to help identify breast cancer subgroups at high risk of symptom cluster distress,and will potentially promote symptom management.

Abstract:ObjectiveThis study aims to investigate the psychological distress and its influencing factors in patients with biliary tract malignant tumors, alongside the development of a predictive model.MethodsA total of 219 patients diagnosed with biliary tract malignant tumors who were admitted to the Department of Liver Surgery at Fudan University Shanghai Cancer Center from July 2021 to May 2023, were selected using a convenience sampling method. Research tools involve psychological distress management screening tools, a demographic questionnaire, self-rating anxiety and depression scales, and the Chinese version of the Memorial Symptom Assessment Scale. Bootstrap method was utilized for repeated sampling to identify relevant factors influencing psychological distress in biliary tract cancer patients. The R software was employed to create a nomogram model, and the model's accuracy and predictive performance were assessed using the receiver operating characteristic curve (ROC) and the Hosmer-Lemeshow test.ResultsThe average score of psychological distress among the 219 patients was (3.91 ± 2.44), with a psychological distress detection rate of 54.8%. Regression model results indicated that factors such as the presence of distant metastasis, comorbidity with other major diseases, poor sleep quality, anxiety, and severity of anxiety and depression were the primary influencers of psychological distress.ConclusionThe detection rate of psychological distress in patients with biliary tract malignant tumors is notably high. The predictive model constructed in this study exhibits good predictive efficacy and clinical value, providing valuable reference for healthcare professionals in developing targeted intervention strategies.

Abstract:As people pay ever-increasing attention to the problems caused by psychological stress, research on its influencing factors becomes crucial. This study analyzed the Health Information National Trends Survey (HINTS, Cycle 3 and Cycle 4) data (N = 5484) and assessed the outcomes using descriptive statistics, Chi-squared tests, and t-tests. Four machine learning algorithms were applied for modeling: logistic regression (linear), random forests (RF) (ensemble), the artificial neural network (ANN) (nonlinear), and gradient boosting (GB) (ensemble). The samples were randomly assigned to a 50% training set and a 50% validation set. Twenty-six preselected variables from the databases were used in the study as predictors, and the four models identified twenty predictors of psychological distress. The essence of this paper is a binary classification problem of judging whether an individual has psychological distress based on many different factors. Therefore, accuracy, precision, recall, F1-score, and AUC were used to evaluate the model performance. The logistic regression model selected predictors by forward selection, backward selection, and stepwise regression; variable importance values were used to identify predictors in the other three machine learning methods. Of the four machine learning models, the ANN exhibited the best predictive effect (AUC = 73.90%). A range of predictors of psychological distress was identified by combining the four machine learning models, which would help improve the performance of the existing mental health screening tools.

Abstract:Lung infection seriously affects the effect of chemotherapy in patients with lung cancer and increases pain. The study is aimed at establishing the prediction model of infection in patients with lung cancer during chemotherapy by an artificial neural network (ANN). Based on the data of historical cases in our hospital, the variables were screened, and the prediction model was established. A logistic regression (LR) model was used to screen the data. The indexes with statistical significance were selected, and the LR model and back propagation neural network model were established. A total of 80 cases of advanced lung cancer patients with palliative chemotherapy were predicted, and the prediction performance of different model was evaluated by the receiver operating characteristic curve (ROC). It was found that age ≧ 60 years, length of stay ≧ 14 d, surgery history, combined chemotherapy, myelosuppression, diabetes, and hormone application were risk factors of infection in lung cancer patients during chemotherapy. The area under the ROC curve of the LR model for prediction lung infection was 0.729 ± 0.084 , which was less than that of the ANN model ( 0.897 ± 0.045 ). The results concluded that the neural network model is better than the LR model in predicting lung infection of lung cancer patients during chemotherapy.

Abstract:Recent studies proved that psychological distress is an accelerator of Alzheimer disease (AD). However, the factors that affect the psychological distress of AD patients are still unknown. The aim of this study was to predict the incidence and identify the risk factors of psychological distress in AD patients. Artificial neural networks and Machine learning models were used to predict the incidence of psychological distress in AD patients. Linear regression and decision tree models were used to identify the factors of psychological distress in AD patients. Among all models for predicting the incidence of psychological distress in AD patients, the artificial neural networks with 8 hidden neurons achieved the highest predictive accuracy of 81.92%. In the five machine learning models, the ADTree algorithm made the highest Predictive Accuracy of 77.94%. As for risk factor analysis, the Linear Regression and Decision Tree models reported similar sets of variables that affect the psychological distress of AD patients. Three variables were reported by Linear Regression to be in negative correlation with psychological distress: the use of professional care service, caregiver consuming cigarette, and caregiver consuming alcohol. The incidence of psychological distress in AD patients can be predicted by artificial neural networks with an accuracy of 81.92%. There are four main risk factors for psychological distress of AD patients: “Caregiver experiencing psychological distress”, “Caregiver suffering from chronic disease or cancer”, “Care recipient’s health status being serious or getting worse”, and “Lack of professional care service”. These findings are otentially helpful for the prediction, prevention and intervention of psychological distress in AD patients.
Identifying the Predictors of Severe Psychological Distress by Auto-Machine Learning Methods

Abstract:Social stress in daily life and the COVID-19 pandemic have greatly impacted the mental health of the population. Early detection of a predisposition to severe psychological distress is essential for timely interventions. This paper analyzed 4036 samples participating in the 2019-2020 National Health Information Trends Survey (HINTS) and identified 57 candidate predictors of severe psychological distress based on univariate chi-square and t-test analyses. Five machine learning methods, namely logistic regression (LR), automatic generalized linear models (Auto-GLM), automatic random forests (Auto-Random Forests), automatic deep neural networks (Auto-Deep learning) and automatic gradient boosting machines (Auto-GBM), were employed to model synthetic minority oversampling technique-based (SMOTE) resampled data and identify predictors of severe psychological distress. Predictors were evaluated by odds ratios in logistic models and variable importance in the other models. Forty-seven variables were identified as significant predictors of severe psychological distress, including 13 sociodemographic variables and 34 variables related to individual lifestyle and behavioral habits. Among them, new potentially relevant variables related to an individual's level of concern and trust in cancer information, exposure to health care providers, and cancer screening and awareness are included. The performance of each model was evaluated using five-fold cross-validation. The optimal model performance-wise was Auto-GBM with an accuracy of 89.75%, a precision of 89.68%, a recall of 89.31%, an F1-score of 89.48% and an AUC of 95.57%. Significant predictors of severe psychological distress were identified in this study and the value of machine learning methods in predicting severe psychological distress is demonstrated, thereby enhancing pre-prediction and clinical decision-making of severe psychological distress problems.
Construction of a nomogram risk prediction model for depressive symptoms in elderly breast cancer patients

Abstract:The primary objective of this study was to identify the factors associated with the development of depressive symptoms in elderly breast cancer (BC) patients and to construct a nomogram model for predicting these symptoms. We recruited 409 patients undergoing BC treatment in the breast departments of two tertiary-level hospitals in Jiangsu Province from November 2023 to April 2024 as our study cohort. Participants were categorized into depressed and non-depressed groups based on their clinical outcomes. Univariate and multivariate logistic regression analyses were employed to identify independent risk factors for depression among BC patients. Multivariate analysis revealed that monthly income, pain score, family support score, and physical activity score significantly influenced the onset of depression in older BC patients ( P < 0.05).The risk prediction model, constructed using these identified factors, demonstrated excellent discriminatory power, as evidenced by an area under the ROC curve (AUC) of 0.824. The maximum Youden index was 0.627, with a sensitivity of 90.60%, specificity of 72.10%, and a diagnostic threshold value of 1.501. The results of the Hosmer-Lemeshow goodness-of-fit test (χ2 = 3.181, P = 0.923) indicated that the model fit the data well. The calibration curve for the model closely followed the ideal curve, suggesting a strong fit and high predictive accuracy. Our nomogram model exhibited superior predictive performance, enabling healthcare professionals to identify high-risk patients early and implement preventative measures to mitigate the development of depressive symptoms. This study is a cross-sectional study that lacks longitudinal data and has a small sample size. Future research could involve larger samples, multicenter studies, and prospective designs to build better clinical predictive models.

A novel model to predict mental distress among medical graduate students in China

Abstract:Abstract Background Poor mental health was reported among medical graduate students in some studies. Identification of risk factors for predicting the mental health is capable of reducing psychological distress among medical graduate students. Therefore, the aim of the study was to identify potential risk factors relating to mental health and further create a novel prediction model to calculate the risk of mental distress among medical graduate students. Methods This study collected and analyzed 1079 medical graduate students via an online questionnaire. Included participants were randomly classified into a training group and a validation group. A model was developed in the training group and validation of the model was performed in the validation group. The predictive performance of the model was assessed using the discrimination and calibration. Results One thousand and fifteen participants were enrolled and then randomly divided into the training group ( n = 508) and the validation group ( n = 507). The prevalence of severe mental distress was 14.96% in the training group, and 16.77% in the validation group. The model was developed using the six variables, including the year of study, type of student, daily research time, monthly income, scientific learning style, and feeling of time stress. The area under the receiver operating characteristic curve (AUROC) and calibration slope for the model were 0.70 and 0.90 (95% CI: 0.65 ~ 1.15) in the training group, respectively, and 0.66 and 0.80 (95% CI, 0.51 ~ 1.09) in the validation group, respectively. Conclusions The study identified six risk factors for predicting anxiety and depression and successfully created a prediction model. The model may be a useful tool that can identify the mental status among medical graduate students. Trial registration No. ChiCTR2000039574 , prospectively registered on 1 November 2020.

Abstract:PurposeTo examine a predictive theoretical model of psychological distress based on the following variables reflected on family caregivers of patients with cancer: the unmet supportive care needs, subjective caregiving burden, social support, and the positive aspects of caregiving.MethodsA cross-sectional descriptive study was conducted on a sample of 484 dyads of patients and their family caregivers. The caregivers completed structured questionnaires designed to measure psychological distress, unmet supportive care needs, subjective caregiving burden, positive aspects of caregiving, and social support. Patients’ demographic variables and medical data were collected from a medical record review. We used a structural equation modeling to test the predictive theoretical model.ResultsPath analysis results partially supported the proposed model with satisfactory fit indices. Specifically, family caregivers with an increasing number of unmet needs or a heavier caregiving burden were more likely to have more severe psychological distress. Bootstrapping results supported that the caregiving burden and social support were significant mediators. Greater unmet supportive care needs predicted higher psychological distress through increasing caregiving burden. Stronger social support predicted lower psychological distress through decreasing caregiving burden. Positive aspects of caregiving predicted lower caregiving burden through the increasing perceived social support, which in turn eliminated psychological distress.ConclusionsUnmet supportive care needs could cause psychological distress through increasing caregiving burden. The positive aspects of caregiving reduced caregiving burden through increasing social support, which subsequently alleviated psychological distress. Interventions that aim to satisfy supportive care needs, to reduce caregiving burden, and to strengthen social support ties may boost the mental health of family caregivers.

Prevalence and Determinants of Psychological Distress in Adolescent and Young Adult Patients with Cancer: A Multicenter Survey

Abstract:OBJECTIVE: This study aimed to investigate the current prevalence and determinants of psychological distress and the association of distress with general information, character strengths, medical coping, and social support in Chinese adolescents and young adults (AYAs) aged 15-39 years with cancer.METHODS: A multicenter survey was carried out. Eight hundred and nine patients completed the Chinese version of the National Comprehensive Cancer Network distress thermometer (DT), the three-dimensional inventory of character strengths, the medical coping modes questionnaire, and the Social Support Rating Scale.RESULTS: The current prevalence of psychological distress in Chinese AYAs with cancer was 83.4%. Binary logistic regression analysis showed that gender, age, educational level, marriage, monthly income, exercise intensity, cancer classifications, treatments, self-control, confrontation, avoidance, and subjective support were all associated with distress. AYA cancer patients who were female, younger, or divorced or had a lower monthly income, education level, or exercise intensity were more likely to have higher level of distress. Compared to AYAs with other cancer classifications, patients with digestive system malignancies, breast cancers, and head and neck malignancies had a higher prevalence of distress. Patients receiving chemotherapy or radiotherapy were more likely to be distressed. The higher the self-control, confrontation, avoidance, and subjective support scores were, the lower the prevalence of distress was among AYA cancer patients.CONCLUSIONS: The prevalence of psychological distress in AYA cancer patients was relatively high. Potential interventions targeting exercise intensity, character strengths, medical coping, and social support may decrease the prevalence of psychological distress in this patient population.

Abstract:Background This study aims to develop an artificial neural network (ANN) prediction model incorporating random forest (RF) screening ability for predicting the risk of depression in adolescents and identifies key risk factors to provide a new approach for primary care screening of depression among adolescents. Methods The data were from a large cross‐sectional study conducted in China from July to September 2021, enrolling 8635 adolescents aged 10–17 with their parents. We used the Patient health questionnaire (PHQ‐9) to rate adolescent depression symptoms, using scales and single‐item questions to collect demographic information and other variables. Initial model variables screening used the RF importance assessment, followed by building prediction model using the screened variables through the ANN. Results The rate of depression symptoms in adolescents was 24.6%, and the depression risk prediction model was built based on 70% of the training set and 30% of the test set. Ten variables were included in the final prediction model with a model accuracy of 85.03%, AUC of 0.892, specificity of 89.79%, and sensitivity of 70.81%. The top 10 significant factors of depression risk were adolescent rumination, adolescent self‐esteem, adolescent mobile phone addiction, peer victimization, care in parenting styles, overprotection in parenting styles, academic pressure, conflict in parent–child relationship, parental rumination, and relationship between parents. Conclusions The ANN model based on the RF effectively identifies depression risk in adolescents and provides a methodological reference for large‐scale primary screening. Cross‐sectional studies and single‐item scales limit further improvements in model accuracy.

Abstract:Aims and objectives To investigate and analyse the prevalence of depression among patients with lung cancer, identify risk factors of depression, and develop a visual, non‐invasive, and straightforward clinical prediction model that can be used to predict the risk probability of depression in patients with lung cancer quantitatively. Background Depression is one of the common concomitant symptoms of patients with lung cancer, which can increase the risk of suicide. However, the current assessment tools cannot combine multiple risk factors to predict the risk probability of depression in patients. Design A cross‐sectional study. Methods The clinical data from 297 patients with lung cancer in China were collected and analysed in this cross‐sectional study. The clinical prediction model was constructed according to the results of the Chi‐square test and the logistic regression analysis, evaluated by discrimination, calibration, and decision curve analysis, and visualised by a nomogram. This study was reported using the TRIPOD checklist. Results 130 patients with lung cancer had depressive symptoms with a prevalence of 43.77%. A visual prediction model was constructed based on age, disease duration, exercise, stigma, and resilience. This model showed good discrimination at an AUC of 0.842. Calibration curve analysis indicated a good agreement between experimental and predicted values, and the decision curve analysis showed a high clinical utility. Conclusions The visual prediction model developed in this study has excellent performance, which can accurately predict the occurrence of depression in patients with lung cancer at an early stage and assist the medical staff in taking targeted preventative measures. Relevance to clinical practice The visual, non‐invasive, and simple nomogram can help clinical medical staff to calculate the risk probability of depression among patients with lung cancer, formulate personalised preventive care measures for high‐risk groups as soon as possible, and improve the quality of life of patients.
